ABSTRACT

The use of bioactive materials, such as Ximenia americana L., to stimulate the bone repair process has already been studied; however, the synergistic effects of its association with light emitting diode (LED) have not been reported. The present work aims to evaluate the effect of its stem bark extract incorporated into methacrylate gelatin hydrogel (GelMA) on the bone repair process using pure hydrogel and hydrogel associated with LED therapy. For this purpose, the GelMA hydrogel loaded with Ximenia americana L. extract (steam bark) was produced, characterized and applied in animal experiments. The tests were performed using 50 male Wistar rats (divided into 5 groups) submitted to an induced tibia diaphyseal fracture. The therapy effects were verified for a period of 15 and 30 days of treatment using histological analysis and Raman spectroscopy. After 15 days of induced lesion/treatment, the new bone formation was significantly higher in the GXG (GelMA + X. americana L.) group compared to the control group (p < 0.0001). After 30 days, a statistically significant difference was observed when comparing the GXLEDG (GelMA + X. americana L. + LED) and the control group (p < 0.0001), the GXG and the control group (p < 0.001), and when comparing the GG, GXG (p < 0.005) and GXLEDG (p < 0.001) groups. The results shows that the Ximenia americana L. stem extract incorporated into GelMA hydrogel associated with LED therapy is a potentiator for animal bone repair.

Introdução: O câncer bucal corresponde a cerca de 30% de todos os tumores de cabeça e pescoço. Aproximadamente 90% dessas neoplasias malignas são carcinomas espinocelulares (CEC) e cerca de 15 mil casos novos são estimados a cada ano no Brasil. Objetivo: Avaliar os aspectos clínico-epidemiológicos de pacientes com CEC oral. Método: Estudo observacional, com delineamento transversal, quantitativo e retrospectivo, a partir da análise de prontuários de pacientes com diagnóstico histopatológico de CEC bucal atendidos em um centro de referência em Oncologia da Região Nordeste do Brasil. Variáveis clínicas e epidemiológicas foram coletadas e analisadas. A análise estatística foi realizada utilizando-se o teste de qui-quadrado (p≤0,05). Resultados: Entre os 298 prontuários avaliados, predominaram pacientes do sexo masculino (75,2%), com idade média de 60,4 anos, tabagistas e etilistas (62,0%). A queixa mais frequente foi a presença de ferida ou lesão na boca (61,1%), e o local mais comum foi a língua (62,1%). As variáveis clínicas revelaram estadiamento clínico avançado (III ou IV) em 76,4% dos pacientes. Entre os pacientes com idade até 50 anos, houve maior concentração de homens (p=0,015) e maior consumo de álcool do que entre os pacientes acima dos 50 anos (p=0,010). As demais variáveis não exibiram diferença estatística significante entre os grupos. Conclusão: As características clínico-epidemiológicas relacionadas ao CEC bucal devem ser consideradas para o planejamento de políticas públicas, a fim de prevenir novos casos e permitir a realização de diagnóstico precoce


Introduction: Oral cancer corresponds to about 30% of all head and neck tumors. Approximately 90% of these malignancies are squamous cell carcinomas (SCC) and about 15 thousand new cases are estimated each year in Brazil. Objective: Evaluate the clinical and epidemiological aspects of patients with oral SCC. Method: Observational cross-sectional, quantitative and retrospective design study, based on the analysis of medical records of patients with histopathological diagnosis of oral SCC treated at a reference center in oncology in Brazil's Northeast. Clinical and epidemiological variables were collected and analyzed. Statistical analysis was performed using the chi-square test (p≤0.05). Results: Among the 298 medical records evaluated, male patients predominated (75.2%), with an average age of 60.4 years, smokers and alcoholics (62.0%). The most reported complaint was the presence of a wound or injury in the mouth (61.1%) and the most common location was the tongue (62.1%). Clinical variables revealed advanced clinical staging (III or IV) in 76.4% of patients. Among patients aged up to 50 years, there was high concentration of men (p=0.015) and greater use of alcohol than among patients over 50 years (p=0.010). The other variables did not show a statistically significant difference between the groups. Conclusion: The clinical and epidemiological characteristics related to oral SCC should be considered when planning public policies, in order to prevent new cases as well as to allow for early diagnosis


Introducción: El cáncer oral corresponde a aproximadamente 30% de todos los tumores de cabeza y cuello. Aproximadamente 90% de estos tumores malignos son carcinomas de células escamosas (CCE) y se estiman alrededor de 15 mil casos nuevos cada año en Brasil. Objetivo: Evaluar los aspectos clínicos y epidemiológicos de los pacientes con CCE oral. Método: Estudio observacional, con diseño transversal, cuantitativo y retrospectivo, basado en el análisis de historias clínicas de pacientes con diagnóstico histopatológico de CCE oral tratados en un centro de referencia en Oncología del Noreste de Brasil. Se recogieron y analizaron variables clínicas y epidemiológicas. El análisis estadístico se realizó mediante la prueba de chi-cuadrado (p≤0,05). Resultados: Entre las 298 historias clínicas evaluadas, predominaron los pacientes del sexo masculino (75,2%), con una edad promedio de 60,4 años, fumadores y alcohólicos (62,0%). La queja más frecuente fue la presencia de herida o lesión en la boca (61,1%) y la ubicación más común fue la lengua (62,1%). Las variables clínicas revelaron estadificación clínica avanzada (III o IV) en 76,4% de los pacientes. Entre los pacientes de hasta 50 años, hubo una mayor concentración de hombres (p=0,015) y un mayor consumo de alcohol que entre los pacientes mayores de 50 años (p=0,010). Las otras variables no mostraron una diferencia estadísticamente significativa entre los grupos. Conclusión: Las características clínicas y epidemiológicas relacionadas con el CCE oral deben considerarse al planificar las políticas públicas, a fin de prevenir nuevos casos y permitir un diagnóstico precoz

OBJECTIVES: This study aimed to evaluate the effects on bone repair of different concentrations of mineral trioxide aggregate (MTA) added to AH Plus. MATERIALS AND METHODS: Bone tissue reactions were evaluated in 30 rats (Rattus norvegicus) after 7 and 30 days. In the AH + MTA10, AH + MTA20, and AH + MTA30 groups, defects in the tibiae were filled with AH Plus with MTA in proportions of 10%, 20% and 30%, respectively; in the MTA-FILL group, MTA Fillapex was used; and in the control group, no sealer was used. The samples were histologically analyzed to assess bone union and maturation. The Kruskal-Wallis and Mann-Whitney tests were performed for multiple pairwise comparisons (p ≤ 0.05). RESULTS: At the 7-day time point, AH + MTA10 was superior to MTA-FILL with respect to bone union, and AH + MTA20 was superior to MTA-FILL with respect to bone maturity (p < 0.05). At the 30-day time point, both the AH + MTA10 and AH + MTA20 experimental sealers were superior not only to MTA-FILL, but also to AH + MTA30 with respect to both parameters (p < 0.05). The results of the AH + MTA10 and AH + MTA20 groups were superior to those of the control group for both parameters and experimental time points (p < 0.05). CONCLUSIONS: The results suggest the potential benefit of using a combination of these materials in situations requiring bone repair.

ABSTRACT

The oromandibular limb hypogenesis syndromes (OLHS) represent a group of rare conditions characterized by congenital malformations involving the tongue, mandible, and limbs. In this report, we describe a newborn girl with paralysis of abducens and facial nerves, transverse agenesis of the distal segments of the limbs, micrognathia, cleft lip and palate, and ankyloglossia superior. This observation confirms an overlap between Moebius syndrome and ankyloglossia superior syndrome with severe limb defects. The etiology of the OLHS is not clearly understood. The intriguing link between facial and limb anomalies can result from their simultaneous development from the fourth to eighth week of gestation, making both areas susceptible to the same teratogenic stimuli. There is an overlap between OLHS conditions, supporting a clustering, rather than a divided nosology and requiring an appropriate classification of these conditions. Patients with OLHS can be successfully managed using a multidisciplinary approach.

ABSTRACT

Objective: this study aimed to evaluate long-term survival and prognostic factors in patients with oral squamous cell carcinoma (OSCC) in an economically poor region of Brazil. Methods: the data were obtained from analysis of medical and mortality records of 210 patients with OSCC treated at an oncology hospital providing services to the Brazilian Unified National Health System in a State of northeastern Brazil between January 2006 and December 2008. Sociodemographic and clinical information, treatment performed, recurrence and evolution were collected. Survival curves were estimated by the Kaplan-Meier method and the log rank and Cox regression tests were used to compare the curves. Results: the median survival in the study period was 47.4 months (95% CI = 38.2 - 56.7). The overall survival rates at 5 and 10 years were 29% and 19.8%, respectivelly. Individuals over 60 years of age (HR = 1.70; 95% CI = 1.06 - 2.73), presence of regional metastasis (HR = 2.51; 95% CI = 1.55 - 4.08), presence of recurrence (HR = 3.18; 95% CI = 1.88 - 5.39) and no surgical treatment (HR = 2.10; 95% CI = 1.31 - 3.35) had a worse prognosis. Conclusions: advanced age, presence of regional metastasis, tumor recurrence and non-surgical treatment predict poorer survival in patients diagnosed with OSCC.


Objetivo: este estudo objetivou avaliar a sobrevida a longo prazo e fatores prognósticos de pacientes com carcinoma espinocelular bucal em uma região pouco desenvolvida economicamente do Brasil. Métodos: os dados foram obtidos a partir da análise de registros médicos de 210 pacientes com carcinoma espinocelular bucal tratados em um hospital especializado em oncologia que presta serviços ao Sistema Único de Saúde em um estado do Nordeste brasileiro entre janeiro de 2006 e dezembro de 2008. Informações clínicas, sociodemográficas e referentes aos tratamentos realizados, recidivas e evoluções foram coletadas. As curvas de sobrevida foram estimadas pelo método de Kaplan-Meier, sendo utilizado o teste de Log-rank para comparar as curvas. Resultados: a sobrevida média no estudo foi de 47.4 meses (95% IC = 38.2 - 56.7). As taxas de sobrevida global em 5 e 10 anos foram de 29% e 19.8%, respectivamente. Indivíduos com idade superior a 60 anos (RR = 1.70; 95% IC = 1.06 - 2.73), que apresentaram metástases regionais (RR = 2.51; 95% IC = 1.55 - 4.08), recidivas tumorais (RR = 3.18; 95% IC = 1.88 - 5.39) e que foram tratados apenas não cirurgicamente (RR = 2.10; 95% IC = 1.31 - 3.35) tiveram pior prognóstico. Conclusões: idade avançada, presença de metástases regionais, recidivas tumorais e tratamento apenas não cirúrgico foram preditores de pior sobrevida em pacientes diagnosticados com carcinoma espinocelular bucal.

ABSTRACT

Violence against women has increased over the years and is a serious violation of human rights. This study aimed to evaluate public policies, women's rights legislation and health care for victims of sexual violence. This is an exploratory and descriptive study, with interviews to professionals of the Care Center for Women Victims of Violence of Teresina-PI, and collection of medical records data of victims. We analyzed data in the light of legislation and guidelines recommended by the Ministry of Health, according to the established public policies. We noted an improvement of the Brazilian legislation and increasing intervention of government in order to control the violence. The evaluated service calls for the humanization of care, the principles of dignity, non-discrimination, confidentiality and privacy, avoiding exposure and distress of victims. Physical and gynecological examination are conducted, besides laboratory tests such as serological tests and collecting traces aiming at identifying the offender, as well as pharmaceutical care and multi-professional support. We can conclude that the current legislation and the guidelines and procedures recommended by public policies to protect women are effective in the referral service studied.

ABSTRACT

Resumo A violência contra mulheres tem crescido, constituindo-se grave violação dos direitos humanos. Objetivou-se avaliar as políticas públicas, a legislação de proteção à mulher e os atendimentos de saúde às vítimas de violência sexual. Realizou-se estudo exploratório e descritivo, com entrevistas a profissionais do Serviço de Atendimento à Mulher Vítima de Violência de Teresina-PI, além de coleta de dados de prontuários das vítimas. Os dados foram analisados à luz da legislação e das diretrizes preconizadas pelo Ministério da Saúde, consoante com as políticas públicas instituídas. Observou-se evolução da legislação brasileira e crescente intervenção do poder público no intuito de controlar a violência. O serviço avaliado preconiza a humanização do atendimento, os princípios da dignidade, não discriminação, do sigilo e da privacidade, evitando a exposição e o desgaste das vítimas. São realizados exames físico e ginecológico, outros complementares como testes sorológicos e coleta de vestígios em busca da identificação do agressor, além de assistência farmacêutica e acompanhamento multiprofissional. Pode-se concluir que a legislação vigente, bem como as diretrizes e os procedimentos preconizados pelas políticas públicas de proteção à mulher são eficazes no serviço de referência estudado.

ABSTRACT

O carcinoma de colo do útero é o segundo mais frequente em mulheres no Brasil, tendo acometido mais de 18.000 mulheres no ano de 2008. O presente estudo objetivou analisar e correlacionar os resultados obtidos nos laudos citológicos e histopatológicos, conforme a faixa etária das pacientes. Para tanto, metodologicamente foi realizado um estudo observacional transversal retrospectivo por meio de 106 prontuários de pacientes que apresentavam laudos citopatológicos e histopatológicos com atipias cervicais atendidas no período de janeiro de 2007 a dezembro de 2008 no Centro de Saúde da Mulher em Piripiri, Piauí, Brasil. Foram obtidos os seguintes resultados: quanto ao exame citológico os resultados mais frequentes foram os de ASCH e ASC/AG, correspondendo a respectivamente, 39,6% e 30%. Houve maior número de pacientes com laudo de ASC/AG e ASCH no intervalo de 25 a 34 anos, correspondendo a respectivamente, 13,2% e 17,9% destas lesões. Na histopatologia, 44 casos (41,5%) foram considerados LSIL, 53 casos (50%) foram diagnosticados com HSIL e 7 casos (6,6%) foram considerados como neoplasia maligna invasiva. Sendo assim, este estudo confirma a acuidade diagnóstica dos exames citológicos e histopatológicos, tendo em vista o grande número de lesões observadas, especialmente em pacientes jovens, destaca-se a importância de orientar práticas sexuais seguras, melhor controle na frequência de rastreamento e seguimento clínico destas pacientes.


Cervical carcinoma is the second most common carcinomas among women in Brazil, having involved over 18,000 women in 2008. Analyze and correlate the most frequent cervical lesions observed in cytological and histological examinations. A cross -sectional observational study was retrospectively performed with 106 patients from a Woman Health Center in Piripiri, Piaui, Brazil, from January 2007 to December 2008, who presented atypical cytopathologic and histopathologic cervical findings. The cytologic results were more frequent in ASC and ASCH/AG, representing 39.6% and 30%, respectively. There was a greater number of patients with report of ASC/AG and ASCH in the range of 25 to 34, representing respectively 13.2% and 17.9% of these lesions. Histopathologically, 44 cases (41.5%) were considered LSIL, 53 cases (50%) were diagnosed as HSIL and 7 cases (6.6%) were considered invasive malignancy. This study confirms the diagnostic accuracy of Pap smears, colposcopy and histopathology exams. Given the large number of lesions, especially in young patients, it highlights the importance of guidelines for safe sex practices, better control in the screening frequency and follow-up for these patients.

AIM: To assess the knowledge and attitude of undergraduate dental students about oral cancer. METHODS: A cross-sectional, quantitative study was conducted based on a questionnaire containing 15 questions about prevention, clinical aspects, and attitudes towards oral cancer. One hundred-thirty-three undergraduate dental students between the second and fifth years were enrolled. The statistical significance was measured by Pearson's chi-square test. RESULTS: There was a predominance of females (58.65%) and most students were between 20 and 30 years of age (75.19%). The risk factors for oral cancer mainly described by the students were smoking (92.48%) and drinking (84.21%). Squamous cell carcinoma was described as the most common type of oral cancer by 48.12% of the students. As much as 53.38% of the participants considered the tongue as the most affected anatomic region, 57.89% reported ulcers as the most frequent clinical aspect, and 44.36% attributed a firm and painless aspect to the regional metastatic lymph nodes. Most students reported regularly conducting a thorough examination of the oral cavity (81.95%). Two of the 15 variables showed differences between the students, according to the school time: previous head and neck carcinoma (Pearson's chi-square, p = 0.03) and guidance on the discontinuation of harmful habits (Pearson's chi-square, p = 0.02). CONCLUSIONS: Students have a good knowledge of the etiology of oral cancer and are apparently alert in their examinations. The clinical aspects of the oral carcinoma, however, are not so clear. The difference regarding knowledge and attitudes towards oral cancer was minimal when different undergraduate years were considered. It is necessary to implement the clinical suspicion of oral cancer throughout the undergraduate course to enable awareness and early diagnosis.

AIM: To verify the orofacial and dental complex in individuals affected by leprosy concerning lesions, prevalence of dental caries and biofilm storage. METHODS: This study was performed on 56 patients with leprosy: 28 under treatment and 28 treated for the disease. A clinical form was prepared to include demographic and leprosy data. The prevalence of caries was analyzed using the decayed, missing, filled teeth (DMFT) index and oral hygiene was evaluated using the Plaque Control Record. The statistical significance was measured by chi-square and Student's t-test. RESULTS: The mean age was 56.4 years, and there was predominance of males. The treatment group had more women (Pearson ÷2=12.47, p=0.0004) and younger patients (Pearson ÷2=9.688, p=0.0079) than the healed group. As much as 30.4% of patients had orofacial complaints related to the disease. The most often observed variations of normality as racial melanin pigmentation (67.8%) and lingual varices (32.1%), and lesions such as atrophy of the anterior nasal spine (25%), inflammatory papillary hyperplasia (17.8%), and collapse of the nasal bridge (14.3%). There were no specific oral lesions among leprosy patients. The mean DMFT was 20.8 and the Plaque Control Record was 70.2%. The treated group had higher DMFT than the treatment group (Student's t p=0.003). CONCLUSIONS: The oral health status of individuals with leprosy is poor, with high levels of plaque and DMFT. No oral lesions associated with the disease were seen.

Introdução: O câncer colorretal costuma ser pouco frequente em indivíduos abaixo dos 50 anos, persistindo ainda controvérsias quanto ao seu comportamento e prognóstico. Objetivo: Avaliar as características epidemiológicas, clínicas, histopatológicas e terapêuticas de pacientes com câncer colorretal, comparando os grupos de faixa etária até 50 anos e idade superior a 50 anos. Método: Trata-se de um estudo descritivo, em que foram analisados prontuários de 505 pacientes com câncer colorretal, atendidos de janeiro de 1998 a dezembro de 2008. Parâmetros clínicos e histopatológicos foram coletados. Resultados: Os indivíduos com idade até 50 anos corresponderam a 26,53 por cento da amostra e não houve predileção por gênero. Alteração no hábito intestinal como queixa inicial e anemia ao exame físico foram significativamente mais comuns no grupo mais jovem. Os pacientes até 50 anos exibiram também estadiamento linfonodal mais avançado, maior ocorrência de carcinoma espinocelular ou neuroendócrino e pior diferenciação histopatológica, bem como maior recorrência local, apesar de maior índice de tratamento cirúrgico e quimioterápico. Conclusão: O câncer colorretal em pacientes com idade até 50 anos mostrou-se mais agressivo e indiferenciado. As queixas, mesmo dos pacientes jovens, devem ser prontamente investigadas, a fim de possibilitar o diagnóstico precoce e maior chance de cura ao paciente com câncer colorretal.

ABSTRACT

A cintilografia óssea é um exame de imagem que tem por objetivo a análise do metabolismo ósseo. É indicada em diversas situações, no entanto, é usada com maior freqüência no estadiamento e seguimento clínico de pacientes com tumores malignos com potencial de desenvolver metástases ósseas. Eventualmente, áreas cintilográficas positivas podem ser observadas na mandíbula e maxila desses pacientes levantando a suspeita de tumor metastático. Por outro lado, estes locais comumente apresentam afecções dentárias, levando à dúvida quanto ao diagnóstico. Neste trabalho, relatamos o caso de uma paciente de 55 anos em follow-up de carcinoma de mama que procurou tratamento odontológico devido região de hipercaptação na mandíbula.


Bone scintilography is an image exam that analyzes bone metabolism. It is performed in patients with different diseases; however it is more frequently used for bone metastasis survey in staging and follow-up of oncologic patients. Eventually, focal increased uptake in the mandible and maxilla may be observed, leading to a suspicion of a metastatic tumor. On the other hand, jaw bones usually present dental diseases, which make the condition a diagnostic challenge. In this study, we report a case of a 55 year-old-female under breast cancer follow-up, who was referred for dental evaluation due to increased uptake in the mandible.

ABSTRACT

Rhinoliths are calcified masses formed within the nasal cavity because of the solidification of mucus, debris, or foreign objects by gradual accretion of mineral salts. They are mostly asymptomatic and incidentally diagnosed, although some patients may complain of nasal discharge and obstruction. Ulceration of the nasal mucosa, facial swelling, and bone destruction are less frequently observed but bring about difficulties for differential diagnoses with several pathologic entities. This article presents a case of a 54-year-old woman with a rhinolith causing palatal perforation, focusing on the diagnostic process and histopathologic characteristics.

ABSTRACT

Plasma cell neoplasia is a lymphoid neoplastic proliferation of B cells. This denomination encloses multiple myeloma (MM), solitary bone plasmacytoma and extramedullary plasmacytoma. MM consists of a clonal proliferation of plasma cells based in the bone marrow, with various degrees of differentiation. Neoplastic cells usually produce great amounts of monoclonal light or heavy chains of immunoglobulin that can be detected in serum or urine. The disease is more frequently in men and the average age at diagnosis is about 60 years. The diagnosis is established by blood and urine exams and medullary biopsy. Patients may present renal failure, bone pain, fatigue, recurrent infections and nervous system dysfunction. Oral manifestations may be the first sign of MM, highlighting the importance of the dentist in the early diagnosis of the disease. Treatment involves mainly irradiation and chemotherapy and the prognosis is generally poor. This paper reports a case of a 65 years old black female who had a complaint of a painful mass in the maxilla that prompted a MM diagnosis.

ABSTRACT

Diversos programas preventivos em Odontologia têm resultado em maior educação em saúde bucal, diminuição da doença cárie e menor edentulismo. Conseqüentemente, podem ser notadas alterações nas necessidades de tratamento odontológico, como a diminuição dos procedimentos mais invasivos e o aumento dos procedimentos preventivos. Sendo assim, o objetivo deste estudo foi avaliar as necessidades dos pacientes que procuram atendimento odontológico na Faculdade de Odontologia de Piracicaba (FOP-Unicamp), visando melhorias no planejamento da grade curricular e no serviço oferecido à comunidade. Foram avaliados prontuários de mil pacientes infantis e mil pacientes adultos. Observou-se entre os pacientes adultos um grande número de necessidades, sendo mais freqüetes nas áreas de periodontia, dentística e prótese. Já entre os pacientes infantis, pudemos observar que a maioria (87,6%) apresentava necessidades preventivas e procedimentos mais simples, como restaurações diretas Classe I. Por outro lado, apenas 12,4% dos pacientes infantis tinham necessidades mais invasivas, como tratamento endodôntico ou restaurações indiretas. De acordo com os resultados obtidos, podemos concluir que, embora os pacientes adultos tenham um acúmulo de necessidades complexas a serem efetuadas, uma alteração no enfoque da Odontologia, de curativo para preventivo, já pode ser observada entre os pacientes infantis.

ABSTRACT

Apesar da radiografia panorâmica permitir avaliação abrangente de toda a região maxilo-mandibular, mostrando-se como importante ferramenta diagnóstica, seu uso rotineiro ainda é discutido. O Objetivo deste estudo foi avaliar a importância deste exame complementar em pacientes infantis. Foram analisadas mil radiografias panorâmicas de pacientes com idade de 3 a 12 anos, que procuraram atendimento odontológico na FO de Piracicaba - Universidade Estadual de Campinas. Os achados radiográficos mais comuns foram agenesias dentárias, anomalias dentárias de forma ou tamanho de dentes supranumerários. Em menor frequência foram observadas algumas imagens sugestivas de cisto dentígeno e odontoma. Pudemos concluir que embora as alterações radiográficas não sejam comuns nesta faixa etária, o uso da radiografia panorâmica contribui para o melhor planejamento odontológico.
