ABSTRACT

The purpose of this study is to investigate the presence of IL-4, IL-8, IL-13 and IFN-γ in equine colostrum and in foals' serum. Samples were obtained from 14 mares and their healthy foals. Soon after parturition, 10ml of colostrum was collected, filtered, centrifuged and frozen until assayed. Blood samples were obtained from each foal at birth (TO) and again after 24h (T24), after which they were frozen until assayed. Serum IgG was measured at 24h of age with an immunoturbidimetric quantitative method. Cytokine concentration was determined using commercially available ELISA tests. Statistical analyses revealed a significant difference in serum concentration of IL-4 at T0 and at T24 (p<0.05) and a significant correlation between the serum IL-4 at T24 and colostral IL-4. These results suggest the absorption of IL-4 from colostrum. The presence of IL-8 in the pre-suckle foal's serum may be due to an endogenous production. With the exception of two samples, there was no IL-13 detected in the foals' serum at birth and remained undetectable in 8/14 samples after 24h. This cytokine was also undetectable in four colostrum samples, where its concentration showed a wide range and a high standard deviation. IFN-γ was present in both the colostrum and in the foals serum at birth.

ABSTRACT

In 2013, the circulation of West Nile virus (WNV) was detected in the Lombardy region and the following year a surveillance programme was activated with the aim of early identification of the viral distribution in mosquitoes and wild birds. A total of 50 959 Culex spp. mosquitoes grouped in six hundred and forty-seven pools as well as 1400 birds were screened by RT-PCR for the presence of West Nile virus leading to the identification of the viral genome in 32 mosquito pools and 13 wild birds. The surveillance was able to detect the WNV circulation on an average of 42 days (CI 95% 29.98-53.86; Student's t-distribution) before the occurrence of human West Nile disease (WND) cases in the same area. These results demonstrate the presence of WNV in the Lombardy region and confirm entomological and wild birds surveillance as an effective measure for the early identification of WNV circulation in infected areas, thus providing a useful and cost-effective tool for the public health authorities in the application of measures to prevent human infection.

ABSTRACT

Being a rare entity, GISTs represents the most common subset of mesenchymal tumours that arise from the digestive tract. Their immunohistochemical and histopathologic features distinguish them from other gastrointestinal mesenchymal neoplasms. These tumours have been the matter of considerable debate in the literature regarding their histogenesis, criteria for diagnosis, prognostic features and treatment. GISTs express Kit protein that not only is a marker for diagnosis but has also permitted to identify a specific medical treatment. The exceptional interest aroused in the literature leads us to make a review about this subject reporting five cases treated in the last 2 years.

ABSTRACT

OBJECTIVE: Data concerning optimal treatment of elderly patients with ovarian cancer are scanty. The management of ovarian cancer in the aged patient is many-sided: the diagnosis can be difficult and delayed, and aggressive surgery is often not attempted because of concomitant morbidity. We tested a combination of carboplatin and mitoxantrone potentially associated with low toxicity in elderly patients with ovarian cancer. METHODS: Eighty-two patients older than 70 years (median age, 75; range, 70-88) with epithelial ovarian cancer were referred to our multicenter group and enrolled into this pilot study. Carboplatin (JM8) was given at the dose of 230 mg/m2 and mitoxantrone at the dose of 9 mg/m2 every 28 days. RESULTS: Dose-limiting toxicity was represented by 4 cases of thrombocytopenia and 1 case of gastrointestinal toxicity. These 5 episodes occurred in 328 assessable cycles, representing a low toxicity profile (3%). Of the 68 assessable patients, 36 (53%) did not respond to chemotherapy (no change + progressive disease), complete response was observed in 15 (22%), and partial remission was observed in 16 (23.5%), accounting for an overall response rate of 45%. CONCLUSION: The carboplatin-mitoxantrone combination, at the dosage tested in this study, appears to be well tolerated by elderly patients with advanced ovarian cancer and is associated with an acceptable response rate. Optimally debulked patients also showed improved survival when compared with patients with more extensive tumor.

ABSTRACT

PURPOSE: The usefulness of extensive and repetitive surgery for patients with ovarian cancer still remains unproven (at least for some conditions). We planned an accurate prospective test of the hypothesis that patients with advanced-stage disease, after they had reached a clinical complete remission (CR), may benefit from surgical second look (SSL). PATIENTS AND METHODS: One hundred two patients in CR (as assessed by clinical findings, markers, and visualization by computed tomographic [CT] scan and laparoscopy), after initial debulking and first-line chemotherapy, were randomized to two arms, which were well balanced for predictive criteria such as age, stage at presentation, histology, grading, date of randomization, and residua after first surgery. Forty-eight patients were randomly assigned to receive follow-up evaluation only, while 54 were assigned to receive second surgery (eight of them refused). Of 46 surgical patients, 35 had negative and 11 positive surgical findings (24% clinically false-negative). RESULTS: Despite the microscopic residua found at open surgery, and the fact that the patients were then treated with second-line chemotherapy, SSL did not increase the probability of survival in this setting. In an analysis of the results according to the intention-to-treat criteria, after a 60-month follow-up period, the overall survival rates in the two groups of patients (SSL v no SSL) were 65% and 78%, respectively (P = .14). Multivariate analysis according to predictive criteria confirmed there was no significant difference between the two groups (P = .39). CONCLUSION: Our study shows the following: (1) our second-line treatment is scarcely effective; (2) SSL accurately defines complete responders to first-line chemotherapy; (3) SSL per se does not prolong survival; and (4) if confirmed, a less invasive procedure could replace SSL as a valuable method in new first-line regimens in ovarian cancer patients with clinical CR confirmed by laparoscopy.

ABSTRACT

Here comes presented three cases of biliary cysts treated with laparoscopic technique. In the first case the discovery of a single cyst of small size on the hepatic margin have consented the total removal. In the second case was about a voluminous unique cyst, while in the third has been found an hepatic polycystosis. In both of them has been applied Lin's procedure with videolaparoscopic technique.

ABSTRACT

The authors report their experience regarding primary gastric lymphoma. The series presented consists of 5 cases: a correct preoperative diagnosis of primary gastric lymphoma was made in one case, whereas pseudolymphoma was hypothesised in a second patient. The remaining three cases were all maltomas. After examining the histological characteristics and classification of primary gastric lymphomas, the authors underline the importance of an early preoperative diagnosis of gastric lymphoma. The repeated use of endoscopic techniques with biopsy compared to radiological examination increases the possibility of diagnosing lymphoma from 20% to 85%. The success rate is further increased if ultrasonic endoscopy is used instead of endoscopy alone, thus making it possible to achieve a 97% diagnostic success rate. The authors then emphasise the fundamental role played by surgery in the treatment of gastric lymphoma. Depending on the depth of neoplastic infiltration, the dimensions of the neoplasia and its multifocality, in localised forms of the antrum (IE, IIE with a low degree of malignancy) surgery consists of resection with partial gastrectomy, provided the resection margins are undamaged, or total gastrectomy in those forms involving the fundus and body of the stomach.

The authors starting from the description of a clinical case of intestinal endometriosis, who underwent emergency surgery for acute intestinal obstruction, take into consideration the incidence, the pathogenesis and the pathological characteristics of intestinal endometriosis. They attempt to define it while accepting that the indications for resection should be extremely limited from both gynecological and intestinal points view.

ABSTRACT

Eighteen patients affected with epithelial ovarian cancer have been treated by intraperitoneal alpha 2a-Interferon. The usual single dosage was 54 x 10(6) U at weekly intervals for four times (eight times in responders): 10 histologically confirmed complete remissions and 3 partial remissions have been obtained; the toxicity was represented by fever in the majority of the cases, by abdominal discomfort in some cases, and by a mild transitory neurological complication in 1 case. Seventeen of 18 patients were pretreated, and 11 of 18 had only cytologic and/or microscopic disease. alpha 2a-Interferon has proven to be very effective and moderately toxic in microscopic peritoneal disease of ovarian epithelial cancer previously treated with extensive surgery and intravenous cytotoxic chemotherapy.

ABSTRACT

The Authors report five examples of arteriovenous fistulas examined in the last two years. The different aetiology concerning these lesions, the most suitable diagnosis and the therapeutic tactics are examined and discussed. When they present results and considerations, they show optimism and discrete realism about results and prognosis as a consequence of technological progress and more and more common multidisciplinary approach towards this pathology.
