ABSTRACT

The abstract should be a single paragraph which summarises the content of the article. This study utilizes density functional theory (DFT) calculations to explore the energetic and magnetic stability of neutral and charged potassium (K) clusters doped with yttrium (Y). We aim to elucidate the influence of Y doping on the energetics and structures of these clusters, comparing them to undoped K clusters. Ground states of 64 polyhedral structures, all of them encapsulating at least one Y atom, were obtained. Y doping generally increased the spin magnetic moment, with charged clusters exhibiting the highest values. Notably, the K12Y+ cluster, possessing an icosahedral geometry and a magnetic moment of 6µB, is classified as a magnetic superatom. Additionally, we identified 14 superatoms with high magnetic robustness. The density of states (DOS) and spin density calculations additionally highlighted the substantial role played by yttrium in the electronic density of the electrons responsible for inducing magnetism in these clusters.

ABSTRACT

DFT calculations were performed to study the effect on energetic and magnetic stability when clusters with up to 24 lithium atoms were doped with one and two atoms of yttrium. In this, the effect of the charge was considered. As a result, some stable structures were identified as possible magnetic superatoms, among them, the YLi12+ cluster with an icosahedron geometry with a spin magnetic moment of 4 bohr magnetons. The participation of yttrium in the electron density of the unpaired electrons providing magnetism in clusters was corroborated at the level of a density of states (DOS) calculation and a spin density calculation. In particular, in the Y2Li12+ superatom, it was found that the encapsulated yttrium atom participates with 35.02% and the second yttrium atom with 15.04%. These percentages, with a contribution from p orbitals, but to a greater extent by d orbitals. The complementation to these percentages is due to the participation of the s and p orbitals of the lithium atoms. In general, doping with a second yttrium atom allowed to obtain a greater amount of high magnetic moments, and considering charged clusters allowed to obtain also high magnetic moments.

INTRODUCCIÓN. La falla hepática ya sea aguda o crónica reagudizada representa un reto para el clínico ya que sus complicaciones conllevan una gran mortalidad, esto se ve aún más complicado ya que las opciones terapéuticas son limitadas, incluso muchas veces no se puede acceder a un programa de trasplante hepático oportuno que mejore la sobrevida de estos pacientes, es así que se ha desarrollado un sistema de "diálisis" hepática conocido como sistema de recirculación de adsorbentes moleculares el cual hace un efecto de detoxificación para eliminar sustancias que generan una noxa en el cuerpo humano. OBJETIVO. Entender la utilidad del sistema recirculante molecular adsorbente en la falla hepática, conocer sus indicaciones y complicaciones. METODOLOGÍA. Se realizó una revisión de la literatura con un enfoque descriptivo, retrospectivo cualitativo no experimental, de documentos que tratan sobre la utilización del sistema MARS para tratar la falla hepática, con evidencia desde el año 2004 hasta el 2021. La revisión bibliográfica se llevó a cabo en bases de datos como Pubmed, Embase, BVS, Google Scholar y Elsevier. RESULTADOS. Se identificaron 30 artículos que cumplieron criterios de inclusión de un grupo original de 343 artículos revisados. Se ha determinado que la evidencia sobre este sistema está compuesta sobre todo por reportes de caso y son pocos los ensayos controlados aleatorizados que empleen su uso, sin embargo, se ha podido determinar que este sistema es un puente al trasplante renal mientras se estabiliza al paciente en la Unidad de Cuidados Intensivos, disminuye los marcadores de falla hepática. CONCLUSIÓN. En Latinoamérica su uso es casi nulo de ahí la necesidad de entender el mecanismo de este novedoso sistema.


INTRODUCTION. Hepatic failure, whether acute or chronic, represents a challenge for the clinician since its complications entail a great mortality, this is even more complicated since the therapeutic options are limited, even many times it is not possible to access a timely liver transplant program to improve the survival of these patients, Thus, a hepatic "dialysis" system known as molecular adsorbent recirculation system has been developed, which has a detoxification effect to eliminate substances that generate a noxa in the human body. OBJECTIVE. To understand the usefulness of the molecular adsorbent recirculating system in liver failure, to know its indications and complications. METHODOLOGY. A literature review was performed with a descriptive, retrospective qualitative non-experimental qualitative approach, of papers dealing with the use of the MARS system to treat liver failure, with evidence from 2004 to 2021. The literature review was conducted in databases such as Pubmed, Embase, BVS, Google Scholar and Elsevier. RESULTS. Thirty articles were identified that met inclusion criteria from an original group of 343 articles reviewed. It has been determined that the evidence on this system is mainly composed of case reports and there are few randomized controlled trials that employ its use, however, it has been determined that this system is a bridge to renal transplantation while the patient is stabilized in the Intensive Care Unit, decreasing the markers of liver failure. CONCLUSIONS. In Latin America its use is almost null, hence the need to understand the mechanism of this novel system.

ABSTRACT

We review here the origin, outbreak characteristics and main epidemiological features of the novel Coronavirus (2019nCoV) responsible of a new coronavirus disease (COVID-19). Rapid global health authorities' responses are now in course and international scientific collaboration is urgently need. Previous outbreaks experiences with similar viral agents have increased the capacity to containment and control of these recurrent health menaces.


Revisamos aquí el origen, características del brote y la epidemiología del nuevo Coronavirus (2019nCoV) responsable de una nueva enfermedad por coronavirus (COVID-19). Una rápida respuesta de las autoridades de salud mundiales está en marcha y se ha hecho un llamado urgente para colaboración científica internacional. Las lecciones aprendidas de brotes previos con agentes virales similares han aumentado las capacidades para contener y controlar estas amenazas recurrentes a la salud global.

ABSTRACT

Labiaplasty has rapidly increased in popularity over the past 5 years.Traditional labiaplasty is associated with potential complications, such as dehiscence, hematoma, flap necrosis, narrowed introitus, pain, and asymmetry. Minimally invasive techniques such as radiofrequency (RF) have emerged as viable alternatives to traditional labiaplasty through a temperature-controlled bipolar mechanism to heat tissues to target temperatures of 40°C-45°C. This controlled energy delivery leads to an inflammatory cascade initiating neocollagenesis, angiogenesis, and elastogenesis over the coming 3-4 months. A single surgeon series of labia minora and majora treatment by RF (InMode, Lake Forrest, Calif.) was reviewed between April 2018-October 2018. Demographic data were collected as well grade of hypertrophy (pre/posttreatment), number of vaginal deliveries, and reason for treatment. Procedural parameters were recorded, including internal/external temperatures, total energy used, and time of treatment. All adverse events were recorded. Objective and subjective data points were obtained in the form of patient surveys and photographic evaluation by lay persons as well as plastic surgeons objective to the treatment. Ten consecutive patients were treated with bipolar RF (InMode, Lake Forrest, Calif.) between April 2018-October 2018. Mean age was 44 (29-54). Average number of pregnancies was 2 (STD 1.1). Three patients were treated for aesthetic concerns, 3 for functional complaints, and 4 desired improvement in both. Overall graded improvement in labia size/contour was +50% (STD ±15.3). Patient satisfaction scale data demonstrated 9.5/10 (±1.7). All patients (10/10) stated that they would undergo treatment again. In all cases, the surgeon observed tightening of the clitoral hood, introitus, forchett, as well as improved distribution of dark pigmentation of the labia minora. There were no significant complications and no need for additional procedures. Average recovery time was 14 days (STD 2.2). Treatment of labia hyperplasia and laxity with bipolar RF may potentially fill a treatment gap of women seeking aesthetic and functional improvements without surgical labiaplasty. A powered prospective randomized double-blinded study is needed to further elucidate the role of this technology.

ABSTRACT

The working equations for the calculation of the electron paramagnetic resonance (EPR) g-tensor within the framework of the auxiliary density functional theory (ADFT) are presented. The scheme known as gauge including atomic orbitals (GIAOs) is employed to treat the gauge origin problem. This ADFT-GIAO formulation possesses an inherent high computational performance, allowing for the calculation of the EPR g-tensor of molecules containing some hundreds of atoms in reasonable computational time employing moderate computational resources. The effect of the use of a gauge independent auxiliary density on the quality of the g-tensor calculation for the evaluation of the exchange-correlation contribution is analyzed in this work. The best agreement with the experiment is obtained with the BLYP functional (Becke 1988 exchange and Lee-Yang-Parr correlation) in combination with a double-ζ basis set, in particular aug-cc-pVDZ. Furthermore, models of endohedral fullerenes N@Cn, with n = {60, 70, 100, 180, 240}, were used for benchmarking its computational performance.

BACKGROUND: Postpartum pelvic floor disorders are estimated to impact 24% of women in the United States. This study describes the use of a radiofrequency device (Votiva, InMode) for postpartum pelvic floor restoration using an electrostimulator to objectively measure treatment effect. METHODS: A retrospective evaluation was conducted between April 2017 and May 2018 of consecutive patients undergoing vaginal radiofrequency treatment. Inclusion criteria were patients at least 6 weeks postvaginal delivery with symptoms of pelvic floor dysfunction. Resting pelvic floor muscle tone and maximal pelvic floor contraction were measured. RESULTS: Fifty women were included in the study with an average age of 32 (29-40) years old, average of 2.6 pregnancies, and 1.8 vaginal deliveries. Two patients were lost to follow-up and excluded. Three complete radiofrequency treatments were performed in 31/50 patients, whereas 19 patients received 1-2 treatments. There were no adverse events from the radiofrequency treatment. No changes were found in resting pelvic muscle tone after Votiva treatment [Wilks' lambda = 0.98, F (1, 45) = 0.86, P = 0.36]. The quantity of treatments seemed to impact mean values of maximal pelvic floor contraction [F (1, 45) = 105.14, P < 0.001]. On the patient questionnaire, patients felt subjective improvement correlated to number of treatments. CONCLUSIONS: Radiofrequency is safe for the treatment of pelvic floor dysfunction. This study showed no changes in resting pelvic muscle tone but an improvement in maximal pelvic floor contraction. A prospective randomized study is being conducted to further evaluate the efficacy of this technology.

ABSTRACT

Background: Patients not eligible for stem cell transplantation (SCT) have been treated with melphalan (M) plus prednisone (P); however, the standard of care has shifted to MP plus thalidomide(T) due to a greater survival benefit. Bortezomib (B) and lenalidomide have also emerged as effective agents. Methods: Randomized clinical trials (RCTs) that compared MP to any otherregimen were identified from the databases of Cochrane Library, PubMed, LILACS, EMBASE and Scirus. Results: Twenty-two trials were included from 2159 potential eligible references. MP vs.M plus dexamethasone (MD): (3 RCTs) MD was superior in partial response (PR) rate and non-hematological toxicity. MP vs. T-based regimens: (4 RCTs) significant differences favoring T-basedregimens in complete response (CR) rate, partial response (PR) rate, and progression-free survival (PFS). MP vs. B based regimens: (1 RCT) significant differences in overall survival (OS) , time to progression (TTP), CR and PR rate favored B-based regimens according to the European Group for Blood and Marrow Transplantation (EBMT) criteria. MP vs. chemotherapy regimens withoutM: (3 RCTs) A significantly higher number of patients treated with BP achieved a CR. TTP was alsosignificantly longer in BP-treated patients (p < 0.02). MP vs. other polychemotherapy regimens:(13 RCTs) No significant differences in PR, OS, hematological or other type of toxicity were observed between MP and the other chemotherapy regimens. Conclusions: Symptomatic multiplemyeloma patients ineligible for SCT should receive as first-line treatment a combination of MP plus B or T; these regimens are associated with improved outcome but greater toxicity comparedto MP alone. More homogeneous clinical trials using a cytogenetic risk approach are required

ABSTRACT

Introducción: los procesos degenerativos, inflamatorios y traumáticos alteran la estabilidad articular interfalángica, afectando significativamente la función de la mano por dolor, deformidad e inestabilidad. La artrodesis es un tratamiento que provee disminución del dolor y estabilidad del dedo. La artrodesis de articulaciones interfalángicas con tornillos de Herbert ha demostrado ser más estable que otros métodos de fijación, con una tasa menor de complicaciones y una rehabilitación rápida. El objetivo es mostrar las ventajas que ofrece esta técnica, y describir la evolución clínica en pacientes con artrodesis interfalángica, midiendo tiempo de consolidación y complicaciones. Métodos y materiales: estudio retrospectivo observacional tipo serie de casos en doce pacientes con doce artrodesis de la IFD y seis artrodesis de la IFP, intervenidos por el autor principal usando el tornillo de Herbert. Se evaluó el tiempo de consolidación, el tiempo de seguimiento y la presentación de complicaciones. Resultados: el promedio de consolidación fue de 12 semanas (rango 10-24 semanas) con un seguimiento de 8,7 meses (5-12 meses). Hubo un caso de retardo de consolidación y otro caso con lesión de lecho ungueal por protrusión de la cabeza del tornillo de Herbert. Discusión: los resultados fueron buenos funcionalmente, con integración rápida a sus actividades. Todos consolidaron, con una baja tasa de complicaciones. La técnica quirúrgica es sencilla. Podemos recomendar esta técnica en los problemas de artrosis o artritis que comprometan seriamente la funcionalidad de las articulaciones interfalángicas distales y, en casos seleccionados, la interfalángica proximal.

ABSTRACT

Se propone utilizar la prueba ortostatica como un metodo de rutina para evaluar la capacidad de trabajo. En un primer intento de aprender a interpretar los resultados de la prueba, esta se efectuo en 83 sujetos jovenes. De 41 hombres, 24 se clasificaron como deportistas y 17 se clasificaron como sedentarios; de 42 mujeres, hubo 21 en cada clase. El analisis de varianza mostro efectos en el nivel de actividad habitual y del sexo sobre la frecuencia cardiaca medida en los sujetos acostados y de pie. Las frecuencias cardiacas de las mujeres fueron en ambos casos mayores que las de los hombres, y las frecuencias cardiacas de los deportistas fueron menores que las de los sedentarios. Como no se encontraron diferencias significativas entre las frecuencias en decubito y de pie, se procedio a comparar los datos de los sedentarios masculinos con los de un grupo de futbolistas profesionales. Los resultados muestran que en los atletas al ponerse de pie, el aumento de la frecuencia cardiaca es significativamente menor que en los sedentarios, diluyendo asi algunas dudas sobre la validez del metodo para establecer la capacidad fisica en el medio colombiano
