ABSTRACT
BACKGROUND: Fatigue is one of the most prevalent symptoms among cancer patients. Specifically, in metastatic castration-resistant prostate cancer (mCRPC) patients, fatigue is the most common adverse event associated with current treatments. The purpose of this study is to describe the prevalence of fatigue and its impact on quality of life (QoL) in patients with CRPC in routine clinical practice. METHODS: This was a cross-sectional, multicentre study. Male chemo-naïve adults with high-risk non-metastatic (M0) CRPC and metastatic (M1) CRPC (mCRPC) were eligible. Fatigue was measured using the Brief Fatigue Inventory (BFI) and QoL was assessed using the Functional Assessment of Cancer Therapy questionnaire for patients with prostate cancer (FACT-P) and the FACT-General (FACT-G) questionnaire. Data were analysed using Mann-Whitney or Kruskal-Wallis tests (non-parametric distribution), a T-test or an ANOVA (parametric distribution) and the Fisher or chi-squared tests (categorical variables). RESULTS: A total of 235 eligible patients were included in the study (74 [31.5%] with M0; and 161 [68.5%] with M1). Fatigue was present in 74%, with 38.5% of patients reporting moderate-to-severe fatigue. Mean FACT-G and FACT-P overall scores were 77.6 ± 16.3 and 108.7 ± 21.4, respectively, with no differences between the CRPC M0 and CRPC M1 subgroups. Fatigue intensity was associated with decreased FACT-G/P scores, with no differences between groups. Among 151 mCRPC patients with available treatment data, those treated with abiraterone-prednisone ≥3 months showed a significant reduction in fatigue intensity (p = 0.043) and interference (p = 0.04) compared to those on traditional hormone therapy (HT). Patients on abiraterone-prednisone ≥3 months showed significantly better FACT-G/P scores than patients on HT (p = 0.046 and 0.018, respectively). CONCLUSION: Our data show a high prevalence and intensity of fatigue and its impact on QoL in chemo-naïve CRPC patients. There is an association between greater fatigue and less QoL, irrespective of the presence or absence of metastasis. Chemo-naïve mCRPC patients receiving more than 3 months of abiraterone acetate plus prednisone showed an improvement of fatigue and QoL when compared to those on traditional HT. OBJECTIVE: We present the case of a fibroepithelial polyp from the uretra of a young female, analysing the factors that make this an interesting case. MATERIAL AND METHODS: A case-report of a fibrous polyp from the urethra of a young female, an analysis of its differential diagnosis and a review of the literature. RESULTS AND CONCLUSIONS: The fibrous polyp of the urethra (fibroepithelual polyp, simple polyp, pedunculated polyp) is a rare benign epithelial tumour, more frequently encountered in males and in the posterior urethra. When it appears in females it is usually located in the external meatus and normally asymptomatic, although it can also present with a mictional syndrome, terminal hematuria or urethral hemorrhage. The differential diagnosis includes other benign epithelial tumours of the urethra, such as the caruncula, mainly the granulomatous, very difficult to differentiate, the squamous papilloma and the acuminated chondiloma. A clinical examination and histological study are necessary for a correct diagnosis, and treatment consists of surgical resection.

OBJETIVO: Aportación de un caso de pólipo fibroepitelial de uretra femenina en mujer joven. Análisis de los factores que lo convierten en un caso interesante. MATERIAL Y MÉTODOS: Exposición de un caso clínico de pólipo fibroso de uretra femenina en mujer joven, estudio de los diferentes diagnósticos diferenciales y revisión de la literatura. RESULTADO Y CONCLUSIONES: El pólipo fibroso de uretra (pólipo fibroepitelial, pólipo simple, pólipo pedunculado), es una rara tumoración epitelial benigna, más frecuente en varones y en la uretra posterior. Cuando aparece en la mujer suele asentar en el meato externo y ser asintomático, aunque también puede debutar como síndrome miccional, hematuria terminal o uretrorragia. El diagnóstico diferencial incluye otros tumores epiteliales benignos de la uretra, como la carúncula, fundamentalmente la granulomatosa, de la que puede ser muy difícil de diferenciar el papiloma escamoso y el condiloma acuminado. Para su diagnóstico son necesarias la exploración clínica y el estudio histológico, y el tratamiento consiste en la extirpación (AU)

We report the case of a female young woman with an incidental cystic lesion in the left adrenal gland while during a routine study by her gynecologist. The change of the aspect in the content of the lesion, its limit size and a not definitive study of punction went to the surgical act. This was made with preservation of both, adrenal and renal units. The pathological report was adrenal pseudocyst.

OBJECTIVE: According to current concepts, multilocular renal cysts are the most benign type of embryonary tumors by contrast to wilm's tumor. Our experience with multilocular renal cyst is presented herein. METHODS: 12 patients (4 boys, 6 adult females and two males) were diagnosed and treated for multilocular renal cyst. One of the adult male patients had an adenocarcinoma located on one of the cysts. Ten patients underwent nephrectomy and two patients were treated by conservative surgery. RESULTS: At 9 years' mean follow-up, no tumor recurrence has been observed. CONCLUSIONS: In the adult patient, all segmental, large and multiloculated cystic masses with a homogeneous content that do not compromise the rest of the parenchyma or adjacent organs except by compression, are suggestive of a multilocular cyst. Conservative surgery is the most reasonable therapeutic option if the foregoing is suspected. This condition has a very good long-term prognosis.
