ABSTRACT

The COVID-19 pandemic has led to the admission of a high number of patients to the ICU, generally due to severe respiratory failure. Since the appearance of the first cases of SARS-CoV-2 infection, at the end of 2019, in China, a huge number of treatment recommendations for this entity have been published, not always supported by sufficient scientific evidence or with methodological rigor necessary. Thanks to the efforts of different groups of researchers, we currently have the results of clinical trials, and other types of studies, of higher quality. We consider it necessary to create a document that includes recommendations that collect this evidence regarding the diagnosis and treatment of COVID-19, but also aspects that other guidelines have not considered and that we consider essential in the management of critical patients with COVID-19. For this, a drafting committee has been created, made up of members of the SEMICYUC Working Groups more directly related to different specific aspects of the management of these patients.

La alta incidencia de insuficiencia respiratoria aguda en el contexto de la pandemia por COVID-19 ha conllevado el uso de ventilación mecánica hasta en un 15%. Dado que la traqueotomía es un procedimiento quirúrgico frecuente, este documento de consenso, elaborado por 3 Sociedades Científicas, la SEMICYUC, la SEDAR y la SEORL-CCC, tiene como objetivo ofrecer una revisión de las indicaciones y contraindicaciones de traqueotomía, ya sea por punción o abierta, esclarecer las posibles ventajas y exponer las condiciones ideales en que deben realizarse, y los pasos que considerar en su ejecución. Se abordan situaciones regladas y urgentes, así como los cuidados postoperatorios

En enero de 2020 China identificó un nuevo virus de la familia de los Coronaviridae como causante de varios casos de neumonía de origen desconocido. Inicialmente confinado a la ciudad de Wuhan, se extendió posteriormente fuera de las fronteras chinas. En España, el primer caso se declaró el 31 de enero de 2020. El 11 de marzo, la Organización Mundial de la Salud declaró el brote de coronavirus como pandemia. El 16 de marzo había 139 países afectados. Ante esta situación, las Sociedades Científicas SEMICYUC y SEEIUC han decidido la elaboración de este plan de contingencia para dar respuesta a las necesidades que conllevará esta nueva enfermedad. Se pretende estimar la magnitud del problema e identificar las necesidades asistenciales, de recursos humanos y materiales, de manera que los servicios de medicina intensiva del país tengan una herramienta que les permita una planificación óptima y realista con que responder a la pandemia

ABSTRACT

Presentamos el caso de una paciente con hemorragia subaracnoidea espontánea, secundaria a rotura de un aneurisma de la arteria cerebral media derecha, que presentó alteraciones electrocardiográficas sugestivas de isquemia, acompañadas además de anomalías segmentarias de la contractilidad ventricular izquierda, que se observaron en el estudio ecocardiográfico. La monitorización eléctrica, hemodinámica y ecocardiográfica puso de manifiesto que ambas alteraciones fueron transitorias, y se recuperaron espontáneamente y sin ocasionar en ningún momento arritmias, elevación de enzimas cardíacas por encima del rango normal ni inestabilidad hemodinámica.La paciente fue tratada de su proceso cerebral y su pronóstico vino determinado por dicha lesión (AU)

ABSTRACT

Se presenta el caso de un paciente con abscesos cerebrales múltiples, con un antecedente de meningitis purulenta y el hallazgo de un aneurisma de tabique interauricular silente con shunt derecha - izquierda. Si bien en la edad pediátrica se encuentra una gran asociación entre la presencia de malformaciones cardíacas con shunt y los abscesos cerebrales, la mayoría de la bibliografía revisada de comunicaciones interauriculares y shunts silentes derecha-izquierda en la población adulta hacen referencia a la producción de fenómenos embólicos cerebrales de tipo isquémico, pero muy pocos la señalan como posible vía de escape al filtro pulmonar de forma que una bacteriemia pueda producir embolismos sépticos a nivel cerebral.Ante la presencia de abscesos cerebrales múltiples no debemos olvidar en el proceso diagnóstico el realizar una búsqueda dirigida, ya que la presencia de malformaciones cardíacas con shunt derecha - izquierda puede ser la vía de acceso de la infección al parénquima cerebral. ABSTRACT

El tratamiento fibrinolítico ha mejorado significativamente el pronóstico de los pacientes con infarto agudo de miocardio. Pero esta estrategia terapéutica tiene limitaciones importantes como son: la escasa utilización, no conseguir óptima reperfusión en todos los pacientes y las complicaciones sobre todo hemorrágicas que en ocasiones pueden ser graves.Se están investigando fármacos que eviten o disminuyan estos problemas, que actúen más rápidamente, sean más fáciles de administrar, lo que facilitaría el uso prehospitalario, y que tengan más capacidad lítica y menores complicaciones hemorrágicas.
