ABSTRACT

Medical thoracoscopy with chemical pleurodesis is a last resort for managing patients who suffer. from recurrent hepatic hydrothorax. However, despite pleurodesis, the rapid fluid build-up can hinder the successful apposition of the pleural surfaces. To improve the chances of success, we investigated the effectiveness of abdominal paracentesis before chemical pleurodesis via medical thoracoscopy to reduce significant fluid shifts from the peritoneal to the pleural cavity. We present a series of three patients with liver cirrhosis complicated by hepatic hydrothorax who underwent medical thoracoscopy with talc pleurodesis. Before the procedure, we optimised medical treatment, and if needed, we performed large-volume paracentesis to prevent rapid reaccumulation of pleural fluid. All study subjects achieved treatment success, defined as relief of breathlessness and absence of pleural effusion at 12 months. Complications related to the treatment included hepatic encephalopathy and acute kidney injury, which were managed conservatively. To manage symptomatic and recurrent hepatic hydrothorax, medical thoracoscopy with talc pleurodesis, preceded by the evacuation of ascites, can be considered as a treatment option. This procedure should be considered early for those who do not respond to medical management and are not suitable candidates for TIPS or liver transplantation.

ABSTRACT

INTRODUCTION: Innovation education programs can help guide and empower surgeons and surgical trainees through the many steps to analyze and de-risk novel ideas to impact healthcare or care delivery. Providing quality programming that is conducive to a surgeon's busy schedule is an ongoing challenge, but even more so for a surgical trainees' schedule. Through a needs assessment and applying lean principles, the "Surgical Trainee Agile Innovation and empoweRment" (STAIR) framework was created to encourage surgical trainees to participate in innovation programming. OBJECTIVE: Numerous works have explored the design and development of learning experiences to support physician innovators. However, there is a lack of evidence supporting frameworks that contribute to innovation empowerment for surgical trainees as a complement to the demands of clinical practice. The following catalogs the systematic design and development of a novel innovation and discovery agile framework tailored to surgical residents with original innovation ideas. DESIGN AND SETTING: The course designed to implement the "Surgical Trainee Agile Innovation and empoweRment" (STAIR) is funded through the Department of Surgery at the University of Michigan. It is an 8-week team-based innovation course designed for surgical residents to learn and apply fundamental innovation concepts to clinical problems. PARTICIPANTS AND RESULTS: Surgical trainees in all programs at the host institution are all eligible to participate. A total of four innovation teams were selected to participate in inaugural course. CONCLUSIONS: The novel instructional design and development of this course encouraged participation in surgical innovation programming. The course aided the development of innovative ideas and inspired interest in innovation among surgical trainees. The STAIR framework is exemplary for inspiring and facilitating innovation among surgical trainees, enhancing the professional experience.

ABSTRACT

A comprehensive examination of human action recognition (HAR) methodologies situated at the convergence of deep learning and computer vision is the subject of this article. We examine the progression from handcrafted feature-based approaches to end-to-end learning, with a particular focus on the significance of large-scale datasets. By classifying research paradigms, such as temporal modelling and spatial features, our proposed taxonomy illuminates the merits and drawbacks of each. We specifically present HARNet, an architecture for Multi-Model Deep Learning that integrates recurrent and convolutional neural networks while utilizing attention mechanisms to improve accuracy and robustness. The VideoMAE v2 method ( https://github.com/OpenGVLab/VideoMAEv2 ) has been utilized as a case study to illustrate practical implementations and obstacles. For researchers and practitioners interested in gaining a comprehensive understanding of the most recent advancements in HAR as they relate to computer vision and deep learning, this survey is an invaluable resource.

ABSTRACT

As a pioneering battery technology, even though sodium-ion batteries (SIBs) are safe, non-flammable, and capable of exhibiting better temperature endurance performance than lithium-ion batteries (LIBs), because of lower energy density and larger ionic size, they are not amicable for large-scale applications. Generally, the electrochemical storage performance of a secondary battery can be improved by monitoring the composition and morphology of electrode materials. Because more is the intricacy of a nanostructured composite electrode material, more electrochemical storage applications would be expected. Despite the conventional methods suitable for practical production, the synthesis of metal-organic frameworks (MOFs) would offer enormous opportunities for next-generation battery applications by delicately systematizing the structure and composition at the molecular level to store sodium ions with larger sizes compared with lithium ions. Here, the review comprehensively discusses the progress of nanostructured MOFs and their derivatives applied as negative and positive electrode materials for effective sodium storage in SIBs. The commercialization goal has prompted the development of MOFs and their derivatives as electrode materials, before which the synthesis and mechanism for MOF-based SIB electrodes with improved sodium storage performance are systematically discussed. Finally, the existing challenges, possible perspectives, and future opportunities will be anticipated.

ABSTRACT

Bovine alphaherpesvirus-1 (BoAHV-1) is an important viral pathogen that causes significant economic losses to the dairy industry. The present study aimed to determine the prevalence of BoAHV-1 in cases of bovine reproductive disorder. Clinical samples were collected from various villages in Gujarat using specialized FTA® cards and were tested using real-time PCR assay targeting the gB gene of BoAHV-1. Out of 401 animals, 18.20% (95% CI: 14.74-22.28%) tested positive for BoAHV-1 DNA. The percentage positivity of BoAHV-1 was 20.37% in abortion cases and 19.55% in retention of fetal membrane cases, while only one out of nine metritis cases screened in the study was positive for BoAHV-1 DNA. A higher percentage positivity in buffaloes (22.14%) compared to cattle (16.30%) was recorded, but this difference was not statistically significant (p = 0.169). The frequency of BoAHV-1 detection was higher among crossbreeds (16.76%) and exotics (19.61%) than among indigenous cattle (8.82%), although this difference was not statistically significant (p = 0.400). There was also no significant difference in frequency distribution among animals of varying parity, ranging from 15.20 to 33.33% (p = 0.540). This study confirms the widespread circulation of BoAHV-1 and highlights the need for its control and prevention.

OBJECTIVE: The quantitative measurement of handgrip strength is important in assessing and charting the progress of patients with neuromuscular diseases. The aim of this research was to determine the intra-rater and inter-rater reliability and the validity of the Squegg digital dynamometer. SUBJECTS AND METHODS: Twenty-one females and nine male participants with an age range between 18 and 40 years volunteered for the study. Three testers each took three measurements with a Squegg device and a Jamar dynamometer using standardized measurement techniques. Intra- and inter-tester reliability were calculated using the intra-class correlation coefficient (ICC). To investigate the relationship between hand measures and isometric handgrip strength, the Pearson correlation coefficient test was used. To determine the agreement between the two devices, a Bland Altman plot was constructed, and the concurrent validity of Squegg was calculated. RESULTS: The intra-rater reliability coefficients for both Jamar and Squegg were greater than 0.99 for all three testers, indicating excellent intra-rater reliability. The inter-rater reliability of Jamar (ICC=0.93) and Squegg (ICC=0.87) was excellent. With an ICC of 0.844 and an r-value of 0.720, Squegg with Jamar demonstrates good validity and statistical significance (p=0.001). CONCLUSIONS: The isometric handgrip strength and hand measures showed a moderate correlation in the study population. The Squegg isometric handgrip dynamometer has good concurrent validity and great intra- and inter-rater reliability in healthy individuals. The validity of Squegg in patients with neuromuscular diseases that affect hand function has to be investigated further.

ABSTRACT

PURPOSE: The Government of India offers various schemes for various classes of citizens. Most of the application forms of schemes to be filled are in English and it is observed that monolingual individuals find it difficult to access and fill the forms. This paper addresses the challenges faced by monolingual individuals in India, particularly the elderly, people with impairments, and those from marginalized communities. The proposed work is to create an interactive system called "Dhvani" voicebot, specifically designed for the Kannada language. It helps users in identifying suitable government schemes and fills forms in English. MATERIALS AND METHODS: The proposed system is developed using the RASA chatbot framework and NLP techniques to comprehend user utterances. RNN and SVM algorithms are employed to ensure smooth conversation flow and interaction with the users. To enhance scheme suggestion accuracy, a knowledge graph is created, containing relevant data on government schemes. RESULTS: The intent classification model achieves an accuracy of 97%, indicating its ability to accurately understand user intentions. The integration of a knowledge graph improves the accuracy of scheme identification and suggestion to users. The system automates the process of filling out government scheme forms based on user inputs. CONCLUSION: Dhvani voicebot system presents a practical solution to address the challenges faced by monolingual individuals in accessing government schemes in India. The high accuracy of intent classification and the use of a knowledge graph contribute to the system's effectiveness. The study suggests that this system can be extended to other languages.


An automated tool called "Dhvani" will solve the problem of aged, illiterate and physically challenged persons filling forms in post offices and banks. Most of the schemes, pension funds, cash withdrawal, cash deposit is through these organizations. So. the tool makes the process easier for the above mention persons without the help of others.An intent recognition and interactive tool developed in Kannada Language which is widely spoken in Karnataka, India. The digital resources available in Kannada Language is very sparce.Use of technology like interactive tool, Knowledge graph, RNN and SVM are used in the development of the tool.Government scheme recommendation interactively makes the users to choose the scheme faster in an interactive way.The form is filled automatically and can be edited to rectify mistakes.

ABSTRACT

The development of cancer has been a multistep process involving mutation, proliferation, survival, invasion, and metastasis. Of all the characteristics of cancer, metastasis is believed to be the hallmark as it is responsible for the highest number of cancer-related deaths. In connection with this, Matrix metalloproteinases (MMPs), that has a role in metastasis, are one of the novel therapeutic targets. MMPs belong to the family of zinc-dependent endopeptidases and are capable of degrading the components of the extracellular matrix (ECM). The role of MMPs in ECM remodeling includes tissue morphogenesis, uterine cycling, growth, tissue repair, and angiogenesis. During pathological conditions, MMPs play a critical role in the excessive degradation of ECM which includes arthritis, tumour invasion, tumour metastasis, and several other autoimmune disorders. Moreover, they are believed to be involved in many physiological aspects of the cell, such as proliferation, migration, differentiation, angiogenesis, and apoptosis. It is reported that dysregulation of MMP in a variety of cancer subtypes have a dual role in tumour growth and metastasis processes. Further, multiple studies suggest the therapeutic potential of targeting MMP in invading cancer. The expression of MMP-2 correlates with the clinical characteristics of cancer patients, and its expression profile is a new diagnostic and prognostic biomarker for a variety of human diseases. Hence, manipulating the expression or function of MMP-2 may be a potential treatment strategy for different diseases, including cancers. Hence, the present review discusses the therapeutic potential of targeting MMP in various types of cancers and their recent patents.

ABSTRACT

The present study was conducted to assess stress distribution around dental implants based on the all-on-four treatment concept. The finite element analysis (FEA) models comprised cancellous bone covered with cortical bone. Four dental implants were placed in two different designs. In model 1, the four implants were placed parallel to each other, whereas, in model 2, the all-on-four concept was followed. The vertical and lateral loads of various values were applied, and stress was evaluated. In model 2, the least stress was observed in both lateral and vertical loading in the peri-implant region. The all-on-four concept proved to be beneficial in reducing the stress around dental implants, thereby reducing the treatment cost.

ABSTRACT

Aplastic Anemia (AA) is a condition where the body's immune system attacks and destroys the hematopoietic stem cells, leading to pancytopenia and an empty bone marrow. AA can be effectively treated using immunosuppressive therapy or hematopoietic stem-cell transplantation. The damage to stem cells in the bone marrow can be due to several causes, such as autoimmune diseases, medication like cytotoxic drugs and antibiotics, and exposure to toxins or chemicals in the environment. In this case report, we present the diagnosis and treatment of a 61-year-old man who was diagnosed with Acquired Aplastic Anemia, which could have developed following his serial immunizations with the SARS-CoV-2 COVISHIELD viral vector vaccine. The patient received immunosuppressive therapy consisting of cyclosporine, anti-thymocyte globulin, and prednisone, resulting in a significant improvement in his condition.

ABSTRACT

The significance of 3D printing has risen exponentially in biomedical and pharmaceutical applications. Its potential in the field of fabricating drug delivery systems, by virtue of processing biocompatible polymers, has been very lucrative. This work aims to tap the interstitial drug delivery kinetics that are often inaccessible through machine-specific infill patterns in additive manufactured tablets fabricated using PVA biopolymer as an excipient. In this regard, a myo-inositol containing tablet has been printed using Fused Deposition Modeling preceded by Hot Melt Extrusion drug loading route. Two machine-specific infill patterns were taken, namely straight and grid. Later, these two distinct patterns were juxtaposed to obtain novel hybrid infill patterns in the tablets. Then, these tablets and their filament were subjected to various thermal, mechanical, imaging and pharmaceutical characterization tests to assess the feasibility of the research attempt. Finally, dissolution tests were conducted to evaluate their dissolution behavior over a time period. The characterization tests proved the scientific viability of this attempt along with amorphous existence of drug in the polymeric filament. The dissolution results showed favorable drug release by achieving interstitial dissolution timings with surface area/volume (SA/V) ratio being found to be the principal factor.

ABSTRACT

Cryptococcal meningitis is a life-threatening infection commonly seen in patients with advanced HIV infection and solid organ transplant recipients. We report a case of cryptococcal meningitis with immune reconstitution syndrome (IRIS) who presented to us with a headache and complete loss of vision in the left eye. He was managed with antifungals and a short course of steroids, and he regained vision completely. In the hospital, he developed complications including tacrolimus toxicity, fluconazole-induced QT prolongation, and flucytosine-induced thrombocytopenia. Our case demonstrates the importance of a multidisciplinary approach in the management of complex cases like cryptococcal meningitis in solid organ transplant recipients.

ABSTRACT

AIMS AND OBJECTIVES: Assessment of micro tensile bond strength (microTBS) and micro-leakage scores of total-etch adhesive (TAE) and self-etch adhesive (SAE) bonded to carious affected dentin (CAD) sterilized using different cavity disinfectant (curcumin photosensitizer (CP) and malachite green (MG) and Chlorhexidine (CHX) in comparison to no disinfection (ND) control group. MATERIALS AND METHODS: One hundred and twenty human molars having International Caries Detection and Assessment System (ICDAS) scores of 4 and 5 were included. Visual inspection, dental explorer hardness testing, and caries detector solution using 5% basic Fuchsin dye solution was applied to dentin to identify the CAD surface. All the specimens were divided into four groups (n = 30) according to the cavity disinfectants used. Group A: 2% CHX, Group B: CP, Group C: MG, and Group D: ND. Each group was further divided into two subgroups (n = 15) based on the adhesion protocol. Groups A1, B1, C1, and D1 were held using TEA, and groups A2, B2, C2, and D2 were adhered using SEA system. The composite material was then built in 2 mm increments and then cured with light. MicroTBS and failure mode assessment using a universal testing machine (UTM) and stereomicroscope at a 40X magnification was performed on 10 samples from each subgroup. The microleakage assessment was performed using a dye penetration test on five samples from each group. ANOVA and Tukey post-hoc tests were used to compare means and standard deviation (SD) of bond strength and microleakage (p < 0.05). RESULTS The maximum microTBS was displayed by A1= CHX and TEA (13.28± 1.01 MPa). The lowest bond scores were demonstrated by C2= MG and SEA (5.98±0.44 MPa). The highest micro-leakage was exhibited by C1= MG and TEA (58.32 ± 2.11 nm). Whereas, the lowest micro-leakage values were displayed by A2= CHX and SEA (24.34± 1.11 nm). CONCLUSION: Chlorohexidiene displayed better bond strength and the lowest microleakage scores with Total-etch adhesive and Self-etch adhesives when used as a cavity disinfectant. Total-etch adhesives performed better in terms of microTBS scores whereas self-etch adhesives displayed superior seal ability within the same disinfectant group.

AIM: Evaluation of the push-out bond strength (PBS) of zirconia post-bonded to radicular dentin after using different final irrigants (MTAD, Malachite green (MG), Ti-sapphire laser and Salvadora persica (S.persica). MATERIAL AND METHOD: Forty human permanent single-rooted were decoronated above the cement-enamel junction. An experienced endodontist performed all the root canal instrumentation using ProTaper universal rotary files. Canals were irrigated using 5.25% NaOCl solution followed by EDTA as a final sterilant. Obturation with gutta-percha using AH Plus sealer was performed. Post-space preparation was completed using Gates Glidden and specimens were randomly assigned to four groups based on the final disinfectant received (n=10). Group 1: 5.25% NaOCl + MTAD, group 2: 5.25% NaOCl + MG, group 3: 5.25% NaOCl +Ti-sapphire laser, and group 4: 5.25% NaOCl + S. persica. The chemically polymerized resin was used to lute zirconia posts. PBS and failure mode analysis were performed using a universal testing machine and stereomicroscope at 40X magnification. Data were compared between the two groups with 95% CI using one-way analysis of variance (ANOVA) with the Tukey post hoc test. p=0.05. RESULTS: Group4(5.25% NaOCl+ S.persica) specimens demonstrated the maximum (8.94±0.14 MPa) bond strength. Conversely, the apical third of Group 2 (5.25% NaOCl+ MG) (2.87±0.15 MPa) samples revealed the minimum bond strength scores. Intergroup comparison exposed that Group 1 (1.3% NaOCl+ MTAD), Group 3 (5.25% NaOCl+Ti-sapphire laser), and Group 4 (5.25% NaOCl+ S. persica) at all three-thirds unveiled no significant difference in PBS (p<0.05). CONCLUSION: Ti-sapphire laser and Salvedora Persica possess the potential to be used as a final root canal irrigant to improve the push-out bond strength of zirconia post-to-root dentin.

Present study aimed to assess effect of pre-treatment with Mucuna pruriens seed extract and its bioactive molecule(s) on NMDAR and Tau protein gene expression in cerebral ischemic rodent model. Methanol extract of M. pruriens seeds was characterized by HPLC, and ß-sitosterol was isolated by flash chromatography. In vivo studies to observe the effect of pre-treatment (28 days) with methanol extract of M. pruriens seed and ß-sitosterol on the unilateral cerebral ischemic rat model. Cerebral ischemia induced by left common carotid artery occlusion (LCCAO) for 75 min (on day 29) followed by reperfusion for 12 h. Rats (n = 48) divided into four groups. GroupI (control,Untreated + LCCAO)-No pre-treatment + cerebral ischemia; GroupII(ß-sitosterol + Sham)-pre-treatment with ß-sitosterol, 10 mg/kg/day + sham-operated; GroupIII(ß-sitosterol + LCCAO)-pre-treatment with ß-sitosterol, 10 mg/kg/day + cerebral ischemia; GroupIV(methanol extract + LCCAO)-pre-treatment with methanol extract of M. pruriens seeds, 50 mg/kg/day + cerebral ischemia. Neurological deficit score was assessed just before sacrifice. Experimental animals were sacrificed after 12 h reperfusion. Brain histopathology was performed. Gene expression of NMDAR and Tau protein of left cerebral hemisphere (occluded side) was performed by RT-PCR. Results revealed that the neurological deficit score was lower in groups III and IV compared to group I. NMDAR and tau protein mRNA expression in left cerebral hemisphere were upregulated in Group I, downregulated in groups III and IV. Histopathology of left cerebral hemisphere (occluded side) in Group I showed features of ischemic brain damage. Groups III and IV, left cerebral hemisphere showed less ischemic damage compared GroupI. Right cerebral hemisphere showed no areas of ischemia-induced brain changes. Pre-treatment with ß-sitosterol and methanol extract of M. pruriens seeds may reduce ischemic brain injury following unilateral common carotid artery occlusion in rats.

The influence of pH on the human serum albumin (HSA) interaction with ionic liquid (IL)1-butyl 3-methylimidazolium octyl sulfate ([BMIM][OSU]) at its sub-micellar concentration of 5 mM (well below CMC ∼31 mM at 25 °C) in aqueous solution has been monitored employing different methods, viz., circular dichroism (CD), fluorescence, electrokinetic determination of the zeta potential (ZP), nuclear magnetic resonance (NMR), small-angle neutron scattering (SANS), and molecular docking (MD). CD analysis indicated a noticeable reduction of the α-helical content of HSA by IL at pH 3. A significant interaction of the anionic part of IL with HSA was evident from the 1H chemical shifts and saturation transfer difference (STD) NMR. A strong binding between IL and HSA was observed at pH 3 relative to pH 5, revealing the importance of electrostatic and hydrophobic interactions assessed from global binding affinities and molecular correlation times derived from STD NMR and a combined selective/nonselective spin-relaxation analysis, respectively. ZP data supported the electrostatic interaction between HSA and the anionic part of IL. The nature of IL self-diffusion with HSA was assessed from the translational self-diffusion coefficients by pulse field gradient NMR. SANS results revealed the formation of prolate ellipsoidal geometry of the IL-HSA complex. MD identified the preferential binding sites of IL to the tryptophan centers on HSA. The association of IL with HSA was supported by fluorescence measurements, in addition to the structural changes that occurred in the protein by the interaction with IL. The anionic part of IL contributed a major interaction with HSA at the pH levels of study (3, 5, 8, and 11.4); at pH > 8 (effectively 11.4), the protein also interacted weakly with the cationic component of IL.

ABSTRACT

Water Conservation is the need of the hour and attitudes towards conserving the resources is what is sure to become a priority in the coming years. In order to understand what could influence the change in attitude and thereby bring about a change of behavior, we first need to shift our attention from understanding the water crisis to understanding the existing attitude of the society towards a water crisis. In the current work we address the current attitude towards water conservation by providing baseline data about Indians' attitudes and behavior/behavioral intentions in conserving water. We introduce a scale constructed to test the Attitude towards water conservation in India. The scale consists of 20 items divided into 5 sub-scales. We conducted a nationwide survey on 430 Participants and their responses were checked for reliability. The internal consistency values of all 5 scales were ranged between 0.68 and 0.73 •Out of the 15 questions from Attitude towards water conservation from Dolnicar, S., & Hurlimann, A. (2010) 1 question was changed to suit Indian context•5 questions were added to show Perceived Moral Obligation, Behavioral Intensions and Perceived Water Rights.

ABSTRACT

Neurocysticercosis is caused by cysticercus cellulosae, the larval stage of Taenia solium, commonly referred to as the pork tapeworm. These larvae form cysts in several organs, including the brain, spinal cord, and eye. Neurocysticercosis is recognized by the World Health Organization as a public health issue and stands as the foremost preventable cause of epilepsy worldwide. Ocular neurocysticercosis refers to the concurrent involvement of the eyes and brain with cysticercus lesions. Neurological symptoms include focal deficits, intracranial hypertension, or cognitive decline. In the eye, the orbital type commonly presents with periocular swelling, ptosis, diplopia, restriction of ocular motility, or decreased vision. The ocular type shows signs of retinal detachment, a macular hole, and inflammation. A 45-year-old female presented with pain in his right eye with blurred vision for 15 days. On USG and MRI of the eye, a thin-walled lesion was noted. The brain showed a few calcified granulomas in the right parietal lobe on MRI. The left eye was normal. If left untreated, the cysts can lead to a severe inflammatory reaction in the eye, which may eventually lead to blindness. This blindness caused by cysticercus is preventable, and hence, early diagnosis and prompt medical or surgical treatment are essential.

ABSTRACT

In the formulation of efficient drug delivery systems, it is essential to unravel the structural and dynamical aspects of the drug's interaction with biological membranes. This has been done for the anticancer drug-membrane system comprising doxorubicin hydrochloride (DOX), a water-soluble anticancer drug, and the micellar sodium dodecyl sulfate (SDS), the latter serving as a useful mimic for membrane proteins. Using a multimodal NMR approach involving 1H, 2H, and 13C as probe nuclei and through the determination of chemical shifts, spin-relaxation, nuclear Overhauser enhancements (NOE), and translational self-diffusion (SD), the binding characteristics of the DOX with SDS have been determined. The perturbation to 13C chemical shifts of SDS indicate the penetration of DOX into the SDS micelle, which is further revealed by 1H-1H NOESY and SD measurements. 2H spin-relaxation measurements and their analysis using a two-step model show DOX induced SDS micellar volume changes, which determine the correlation times involved in the DOX-SDS mobility.
