ABSTRACT
We present a case of disseminated cutaneous histoplasmosis in a male patient, rural worker, HIV positive for 20 years, with a history of irregular use of antiretroviral therapy, T cell counts below 50 cells/mm3 and with good response to treatment with Itraconazole. We highlight importance of skin lesions in clarifying early diagnosis, since this co-infection often leads patients to death.

The term "Wolf's isotopic response" describes the occurrence of a new skin disorder at the site of another unrelated and already healed skin disease. In most cases, herpes zoster is the inicial disease. Different disorders may develop on the same site, most commonly granulomatous and lichenoid reactions, infiltration of hematologic diseases, skin tumors and infections. There are few related cases of lichen planus presenting as isotopic response. We report a case of a 74 year-old woman, with multiple itchy, rose-colored and shiny papules that developed at site of previously healed herpes zoster, on the right arm and shoulder. The pathogenesis of this phenomenon is still unknown and further studies are needed.

Contexto: Úlcera de Marjolin é a malignização de cicatrizes crônicas em geral, sendo mais frequente em cicatrizes antigas de queimaduras.É considerada aguda, quando o câncer se instala até um ano depois da ocorrência da cicatriz; e crônica, após esse prazo. O carcinoma espinocelular é o mais frequente, podendo ser encontrados carcinoma basocelular, melanoma e sarcoma. Descrição do caso: São relatadosdois casos desta enfermidade com boa evolução. Em ambos foram realizadas biópsias das lesões, com comprovação histopatológica, querevelou carcinoma espinocelular. Discussão: Os achados deste estudo são concordantes com aqueles descritos anteriormente por vários autores. A cirurgia ampla mostrou-se curativa até enquanto foi possível ser feito acompanhamento dos doentes, sem necessidade de terapia adjuvante. Assim, demonstra-se a maior ocorrência de carcinomas do tipo espinocelular sobre cicatrizes cutâneas de queimadura e alerta-separa a necessidade do diagnóstico e intervenção precoces. Destaca-se também a importância do acompanhamento rígido destes pacientes,devido a possíveis recidivas e metástases, na tentativa de reduzir morbidade e mortalidade. Conclusão: Os profissionais da saúde devem estar atentos à ocorrência da úlcera de Marjolin, visando diagnóstico precoce com consequente terapêutica adequada e melhor prognóstico.Salienta-se a orientação adequada aos pacientes portadores de cicatrizes crônicas, especialmente aquelas provenientes de queimaduras, informando-os da obrigatoriedade de avaliação médica especializada, caso ocorram qualquer tipo de alteração nessas regiões.

Contexto: Líquen escleroso é uma doença crônica que se apresenta clinicamente na forma genital e, mais raramente, extragenital. Acometepreferencialmente as mulheres, e, em menor frequência, os homens. Sua etiologia permanece desconhecida; postula-se a associação com doenças autoimunes, predisposição genética, além de envolvimento de fatores hormonais e a importância do estresse oxidativo na gênese da doença. O tratamento de eleição é a corticoterapia tópica de alta potência com excelentes resultados, sendo utilizados ainda imunomoduladores, retinoides, progesterona e estrógeno, além de laser de CO2 e fototerapia. Relato de caso: Descrevemos um caso raro e excêntrico de líquen escleroso hemorrágico de apresentação incomum. Conclusão: Acentuamos a importância do diagnóstico clínico e histológico de forma exuberante desta dermatose, resolvida com sucesso.

Homem, 25 anos, com manchas acastanhadas, assintomáticas, nas regiões cervicais, axilares, inguinais e poplíteas há nove meses. Anatomopatológico com degeneração hidrópica da basal, incontinência pigmentar e infiltrado inflamatório linfocitário moderado na derme. O líquen plano pigmentoso invertido é um subtipo raro de líquen plano, caracterizado por máculas hipercrômicas, assintomáticas ou discretamente pruriginosas, de milímetros a centímetros de diâmetro, com limites nítidos, nas áreas intertriginosas, sendo mais comum nas flexuras axilares e inguinais de indivíduos de pele clara. Apresenta histologia liquenóide peculiar. Relata-se um caso com clínica, histologia e evolução típicas.

Cutaneous metastasis is a phenomenon that results from a tumor spreading via lymphatic or vascular embolization, direct implant during surgery or skin involvement by contiguity. The primary malignant tumor that most commonly metastasizes to the skin in women is breast cancer, which can be manifested through papulonodular lesions, erysipeloid or sclerodermiform infiltration, en cuirasse. We report the case of a female patient, 78 years old, with papular, scaly and confluent lesions in the right breast for one year, progressing to edema and skin infiltration, reduction of breast volume and plaque en cuirasse, and similar lesions in the contralateral breast and abdomen for four months. The pathological diagnosis was invasive ductal breast carcinoma with Paget-like foci, epidermal skin metastases and lymphatic embolization.

Childhood Granulomatous Periorificial Dermatitis is an acneiform facial rash that affects the periorificial area in children. The clinical aspectare asymptomatic 1-3 mm papules of, monomorphic, erythematous or hypopigmented in periorificial areas - mouth, nose and eyes. It's a benign and self-limited disease that heals spontaneously without scarring and specific therapy. Differential diagnoses include perioral dermatitis, granulomatous-rosacea, sarcoidosis, and lupus miliaris disseminatus faciei. We present the case of a 4-year-old boy, presenting papules in periorificials areas. Due to its low incidence and low number of publications we report the present case.

A metástase cutânea é conseqüente à disseminação do tumor por embolização linfática, vascular, implantação direta durante cirurgias ou envolvimento da pele por contiguidade. Em mulheres, o tumor maligno primário que mais comumente metastatiza para a pele é o de mama, que tanto pode se expressar por lesões tumorais papulonodulares, infiltração erisipelóide ou esclerodermiforme, em couraça. Relatamos o caso de paciente do sexo feminino, 78 anos, apresentando lesões nodulares, descamativas e confluentes em mama direita, evoluindo com edema e infiltração cutânea, com redução do volume mamário e placa endurecida ilimitada. Invasão da mama contralateral e abdome ocorreram 4 meses após o início dos sinais. O diagnóstico histopatológico foi de adenocarcinoma ductal invasivo de mama com focos pagetóides epidérmicos e embolização linfática.

A Dermatite Periorificial Granulomatosa da Infância é erupção facial acneiforme que afeta área periorificial do segmento cefálico de pré-puberes. Consiste em pápulas assintomáticas de 1 a 3 mm, monomorfas, eritematosas ou hipopigmentadas em áreas periorificiais - boca, nariz e olhos. A doença é benigna e auto-limitada, curando sem deixar cicatriz e por regra sem terapia específica. Diagnósticos diferenciais incluem a dermatite perioral, rosácea granulomatosa, sarcoidose e lúpus miliar da face. Relata-se paciente de 4 anos, masculino, com erupção papulosa há 2 anos em áreas periorificais. Devido à sua baixa incidência e o reduzido número de publicações relata-se o presente caso.

Glomus tumor is a painful tumor derived from the glomus terminal which regulates blood circulation and body temperature Despite its rarity, particular attention needs to be paid to symptoms associated with this tumor: paroxysmal pain, sensitivity to local pressure and cold, and its location - typically in the distal phalanx. Manifestation of extradigital occurrence is possible. Single lesions are most common in adult females, while multiple lesions are prevalent in children and adolescents, generally those with a positive family history. The diagnosis is clinical with the aid of ultrasonography and magnetic resonance imaging, confirmed by histopathological examination.
