ABSTRACT

The Canalis Sinuosus is a structure of the maxilla that allows the passage of the anterosuperior alveolar nerve and has a neurovascular activity. To visualize this structure, Conical Beam Computed Tomography (CBCT) is best recommended. This article aimed to report a case of facial pain after the insertion of a dental implant due to compression of the Canalis Sinuosus. Thus, the implant was removed, followed by the insertion of a bone graft. After that, the facial pain stopped. In conclusion, the identification of anatomical structures in preoperative examinations is essential in surgical dental procedures.


O Canalis Sinuosus é uma estrutura da maxila, que permite a passagem do nervo alveolar anterosuperior e tem uma atividade neurovascular. Para visualizar essa estrutura, a Tomografia Computadorizada de Feixe Cônico (TCFC) é melhor recomendada. Este artigo teve como objetivo relatar um caso de dor facial após a inserção de um implante dentário, devido à compressão do Canalis Sinuosus. Assim, o implante foi removido, seguido pela inserção de um enxerto ósseo. Depois disso, a dor facial foi interrompida. Em conclusão, a identificação de estruturas anatômicas em exames pré-operatórios é essencial em procedimentos odontológicos cirúrgicos.

ABSTRACT

INTRODUCTION: This study assessed the influence of voxel size and filter application in detecting second mesiobuccal (MB2) canals in cone-beam computed tomographic (CBCT) images. METHODS: Using the OP300 CBCT system (Instrumentarium, Tuusula, Finland) and 3 voxel size protocols (80 µm, 125 µm, and 200 µm), we scanned 40 first molars: 20 with an MB2 canal and 20 without. All molars received silver palladium pins on the palatal root, whereas the non-MB2 molars were also filled with gutta-percha. Five oral radiologists assessed the presence of an MB2 canal under 3 filter application conditions: without filter, with sharpen 1 × filter, and with sharpen 2 × filter. Intra- and interobserver reproducibility was evaluated using the weighted kappa index. We compared the area under the receiver operating characteristic curves with SPSS Statistics v.20.0 (IBM Corp, Armonk, NY) using 2-way analysis of variance and the Tukey post hoc test with 5% significance level. RESULTS: Our analysis found median intra- and interobserver agreement values of 0.70 and 0.56, respectively. The 80-µm voxel with sharpen 1 × filter image group had the highest sensitivity, accuracy, and negative predictive values. As for specificity and positive predictive, the 80-µm voxel group without filter application presented the highest values. The areas under the receiver operating characteristic curve were higher in the 80-µm groups than in the 125-µm and 200-µm voxel size groups (P < .05). We found no differences among the filters used (P = .22) or for the filter-voxel size interactions (P = .88). CONCLUSIONS: A smaller voxel size increased the accuracy in detecting MB2 canals, whereas the enhancement filters did not.

OBJECTIVE: To compare the diagnostic efficacy of different devices for visualizing digital images in detecting simulated internal root resorption (IRR). METHODS: 26 uniradicular human teeth were sectioned along its long axis and randomly divided into two groups. Then, they were submerged in hydrochloric acid (HCl) P.A 37% at different times to make two types of defects: Type 1 (2 hours), and type 2 (3 hours). Digital periapical radiographs were acquired in three moments: before section, after section and after immersion in acid. The images were evaluated on three different devices: 27-inch iMac Desktop, 8-inch Android Tablet, and 5.5-inch iPhone 8 Plus using a 5-point scale. RESULTS: The sensitivity and accuracy values were higher with the use of the Android Tablet, for both types of defects. For the values of the area under ROC curve, higher results were observed with the Android Tablet and lower results with the iMac, showing a significant difference (p < 0.05) when comparing the use of the iMac with the Android Tablet and iPhone 8, for type 1 defects. When comparing the different devices, in the evaluation of images of type 2 defects, the values showed no statistical difference, regardless of the device evaluated (p > 0.05). CONCLUSIONS: The size of the lesion interferes with the detection capacity in the image. However, the use of portable devices does not impair the diagnosis of IRR, regardless of size and the dental practioners may use different devices/display, under optimal viewing conditions, to detect IRR lesions, without jeopardizing the diagnostic ability.

ABSTRACT

INTRODUCTION: The aim of this study was to evaluate the influence of different intracanal materials and the metallic artifact reduction (MAR) tool of cone-beam computed tomographic (CBCT) imaging on the detection of the second mesiobuccal (MB2) canal in maxillary first molars. METHODS: Forty maxillary first molars were selected; half of them had the MB2 canal in the mesiobuccal root confirmed by micro-computed tomographic imaging. All teeth were instrumented, except for the MB2 canal, and filled with gutta-percha, which was passively inserted up to the working length. Each tooth was individually inserted into a dry human skull, and CBCT scans were performed using the OP300 device (Instrumentarium Dental, Tuusula, Finland) with and without the use of the MAR tool. For each scanned tooth, the following intracanal materials were used in the palatal root: gutta-percha, silver palladium, nickel chromium, and cobalt chromium. Five examiners assessed the images for the detection of the MB2 canal using a 5-point scale. The weighted kappa test was applied to evaluate intra- and interexaminer reproducibility. The diagnostic values (sensitivity, specificity, and area under the receiver operating characteristic curve) were calculated in the different groups and compared using 2-way analysis of variance and the Tukey post hoc test. RESULTS: The intra- and interobserver agreement ranges for the different conditions were studied. The diagnostic values were not significantly different (P > .05) regardless of the intracanal material and the use of the MAR tool. CONCLUSIONS: The presence of different intracanal materials and MAR activation does not influence CBCT-based detection of MB2 canals.

ABSTRACT

OBJECTIVES: To assess C-shaped root canal configuration by identifying its prevalence and each configuration type proportion, according to tooth (mandibular first or second molar) and demographic characteristics in a Brazilian population, using cone-beam computed tomography (CBCT). Moreover, it was verified if there is a relationship between root canal configuration observed in two-dimensional reconstructions of CBCT and presence of C-shape. METHODS: Mandibular jaw CBCT scans (184 males and 220 females, aged 15 to 80 years), which presented 1464 mandibular molars (710 first molars and 754 second molars), were assessed. Teeth were evaluated for the presence and type of C-shaped root canals by observing the roots at five levels in CBCT axial reconstructions. Root canal configuration was assessed in panoramic reconstructions. Data were statistically analyzed at a significance level of 5%. RESULTS: Of the 1464 mandibular molars, 125 (8.5%) were classified as C-shaped. This variation was more prevalent in females (n = 107, 85.6%) and in second molars (n = 108, 86.4%). C1 (uninterrupted C-shaped canal) was the most prevalent type of C-shaped configuration (41.76%), while C5 (no canal lumen) was the least prevalent type (0.96%). Single root with single canal in panoramic reconstructions was the most predominant configuration for C-shaped teeth (n = 54, 43.2%). Fused roots presented 17.2 higher odds of being associated with C-shaped root canals than non-fused roots. CONCLUSIONS: C-shaped root canals were more prevalent in mandibular second molars and in females. Additionally, clinicians should bear in mind the greater possibility of C-shaped configuration in mandibular molars with fused roots. CLINICAL RELEVANCE: Mandibular molars with C-shaped canals present a clinical challenge. A higher C-shaped proportion was noted in radiographic fused root types, which had 17.2 higher odds of presenting such anatomy when compared to radiographic non-fused roots. Root radiographic features may help in diagnosis of complex C-shaped morphologies.

ABSTRACT

Objetivo: Avaliar a internação hospitalar por câncer de boca e faringe no Brasil no período de 2008 a 2017. Métodos: Estudo das internações por estas neoplasias registradas no Sistema de Informação Hospitalar disponíveis no DATASUS/MS entre 2008 e 2017. Resultados: Foram registradas 263.556 mil internações por câncer de boca e faringe. O Sudeste apresentou a maior frequência de casos (44,3%). A maior taxa de internação ocorreu no Sul (19,6/100 mil habitantes). As maiores taxas de letalidade encontradas foram no Norte (12,7%) e Sudeste (12,3%). Houve um predomínio de internações em regime privado (53,5%) e em caráter de urgência no Brasil (53,5%). A média de permanência hospitalar foi de 6,4 dias. A taxa de letalidade em caráter de urgência foi maior do que em caráter eletivo. A taxa de internação apresentou tendência decrescente e a taxa de letalidade hospitalar, tendência de incremento de 0,2% a cada ano. A análise de índice de desenvolvimento humano médio e taxa de internação mostrou correlação moderada positiva entre essas variáveis e as regiões do país. Houve maior custo médio em reais por internação hospitalar em caráter eletivo e a região Centro-Oeste obteve maior média de custo financeiro (por guia autorizada) nesta modalidade. Conclusão: Observa-se um elevado número de internações por estes tipos de câncer no Brasil, além de um acentuado custo e alta média de permanência hospitalar, sendo variável entre as regiões do país.


Aim: To analyze hospital admission for oral and pharyngeal cancer in Brazil and regions from 2008 to 2017. Methods: This study analyzed the hospitalizations for this neoplasm registered in the Hospital Information System available in DATASUS/MS between 2008 and 2017. Results: A total of 263,556 hospitalizations were registered. The Southeast presented the highest frequency of cases (44.3%). The highest hospitalization rate occurred in the South (19.6/100 thousand). The highest rates of lethality were found in the North (12.7%) and Southeast (12.3%). There was a predominance of hospitalizations in the private care (53.5%) and emergency care (53.5%) sectors in Brazil. The mean hospital stay was 6.4 days. The case fatality rate was higher than the elective rate. The hospitalization rate declined, and the hospital lethality rate showed a tendency toward an increase of 0.2% per year. The correlation analysis of the average human development index and the hospitalization rates were moderately positive between the variables and the regions of the country. There was a higher average cost in reais for hospital admission on an elective basis, and the Mid-West region obtained a higher average financial cost (per authorized guide) in this modality. Conclusion: It was possible to verify a high number of hospitalizations for these types of cancer in Brazil, in addition to a high cost and average hospital stay, which tend to vary throughout the country.

INTRODUCTION: This retrospective cross-sectional study investigated the applicability of the periapical and endodontic status scale (PESS) to determine the association of endodontically treated teeth with maxillary sinus (MS) abnormalities through cone-beam computed tomographic imaging. METHODS: A total of 631 endodontically treated teeth were analyzed. MS abnormalities were classified as mucosal thickening, sinus polyp, antral pseudocyst, nonspecific opacification, periostitis, and antral calcification. The PESS was used to evaluate the quality of endodontic treatment as well as periapical tissue conditions. The data were analyzed by chi-square tests (P < .05). RESULTS: MS abnormalities were detected in 70.52% of the sample, with a greater prevalence of mucosal thickening (38.19%), whereas periapical lesions were observed in 55.94% of the cases. Treated root canals with unsatisfactory filling, homogeneity, and coronal sealing had an odds Ratio (OR) of 2.21, 2.88, and 2.99, respectively (P < .001). Periapical lesions larger than 5 mm (OR = 314.95), in more than 1 root (OR = 3.72), involving the furcation region (OR = 5.21), in contact with important structures (OR = 7.37), and with cortical bone destruction (OR = 4.09) were significantly related to the presence of MS abnormalities (P < .001). An OR of 99,668 was observed in periostitis lesions greater than 5 mm (P < .001). CONCLUSIONS: The PESS proved to be an important tool for the analysis of endodontic and periapical conditions and was applicable to determine potential associations with MS abnormalities.

ABSTRACT

PURPOSE: To compare the diagnostic ability of undergraduate dental students to detect maxillary sinus abnormalities in panoramic radiographs (PR) and cone-beam computed tomography (CBCT). MATERIALS AND METHODS: This was a retrospective study based on the evaluation of PR and CBCT images. A pilot study was conducted to determine the number of students eligible to participate in the study. The images were evaluated by 2 students, and 280 maxillary sinuses were assessed using the following categories: normal, mucosal thickening, sinus polyp, antral pseudocyst, nonspecific opacification, periostitis, antrolith, and antrolith associated with mucosal thickening. The reference standard was established by the consensus of 2 oral radiologists based on the CBCT images. The kappa test, receiver operating characteristic curves, and 1-way analysis of variance with the Tukey-Kramer post-hoc test were employed. RESULTS: Intraobserver and interobserver reliability showed agreement ranging from substantial (0.809) to almost perfect (0.922). The agreement between the students' evaluations and the reference standard was reasonable (0.258) for PR and substantial (0.692) for CBCT. Comparisons of values of sensitivity, specificity, and accuracy showed that CBCT was significantly better (P<0.05). CONCLUSION: CBCT was better than PR for the detection of maxillary sinus abnormalities by dental students. However, CBCT should only be requested after a careful analysis of PR by students and more experienced professionals.
