The author suggests a multifactor program for decoding the variant of the course of the postoperative period in patients with duodenal ulcer. It was tested in 199 patients. Three groups of patients with a favourable prognosis, an uncertain prognosis, and with a high risk of recurrent ulcer were distinguished. An individual therapeutic program with consideration for the risk group allows the incidence of postoperative recurrence of ulcer to be reduced by more than three times.

The incidence of reflux-esophagitis was studied in 574 dispensary patients and 217 patients operated upon for ulcer disease. It was found that reflux-esophagitis is observed in 19.8% of patients with ulcer disease and in 34% of such patients subject to operation. The main cause of it is incompetence of the cardiac sphincter without or in combination with a hernia of the esophageal opening. It was noted that vagotomy leads to a disturbance of the ligamentous system of the cardio-esophageal zone and in all cases leads to the development of reflux-esophagitis. Fundoesophagorraphy is thought to be the best method of preoperative treatment of reflux-esophagitis.

The authors have studied results of 228 reinfusions of UV irradiated blood in 75 patients with ulcer disease. It was found that the procedure had a favourable effect on the patient's state, immunostimulating action resulted in a smooth postoperative period and a real reduction of the amount of postoperative complications.

The authors studied the valvular function of the cardia in 714 patients who were kept under regular medical observation and in 138 patients who had been operated on for duodenal ulcer. Changes were found in the cardia of 9.4% of patients who underwent regular medical examination and in 17% of these subjected to operation. The valvular function of the cardia was impaired after vagotomy in all cases. Fundoesophagocrurorhaphy is the best method for correction of cardiac dysfunction.

Results of investigations of sialic acids in blood serum of 115 patients and in gastric mucosa of 85 patients have been analyzed. It was established that sialic acids reflect the activity of the inflammatory process. Examination of the sialic acid level in the process of preoperative management and postoperative treatment allows to make timely prognosis of the risk of inflammatory complications, anastomositis in particular, and begin preventive measures and therapy of the inflammatory focus.

Results of the examination of 142 patients with ulcer disease were studied. The incidence of DGR was found to be 48% in gastric ulcer and 44% in duodenal ulcer. The unfavorable influence on the gastric mucosa was greater under conditions of the acidic intragastric medium. Special attention to causes of DGR in the surgical treatment of the ulcer disease and the use of areflux anastomoses can improve results of the surgical treatment of such patients.

The effect of 6 pharmacological drugs on the motor function of the stomach in patients with ulcer disease was studied. It was established that Cerucal was the best modulator of gastric motor activity. There were three types of reaction of electrogastrogram to the administration of Cerucal. It was noted that in the first type there was a real danger of the development of postvagotomic kinetic disorders. In the second and third types such a danger was as a rule absent. This method of prognosis of motor disorders after vagotomy was successfully used in 81 patients with ulcer disease of the duodenum treated with the help of vagotomy.

Under analysis were results of the surgical treatment of 120 patients with different gastroenterological diseases. It was shown that laser scalpel can be successfully used not only at key steps of the operation, but also for the dissection of other tissues but the skin and fatty tissue. Using laser scalpels gives better functional results of the abdominal surgery.

The examination of 146 patients with the ulcer disease of the stomach (26 cases) and duodenum (120 cases) has established that the ulcer was colonized by different microbes and their associations in 81,5% of cases. The colon bacillus was found almost in half the cases. The application of corresponding antibiotics in the pre- and post-operative treatment gave better immediate results.
