ABSTRACT

Atypical mycobacterial infections are increasingly important in immunosuppressed patients as well as in healthy hosts. The atypical mycobacterium that most commonly affects the skin is Mycobacterium marinum. The infection should be suspected upon the presence of ulcers, nodules or chronic plaques and a history of contact with fresh or salt water. Optimal therapy is yet to be established. We report a case of Mycobacterium marinum infection in a patient receiving immunosuppressive therapy that responded favourably to treatment with doxicycline. We review the different antibiotic regimens prescribed in the past years for the treatment of Mycobacterium marinum infection.

ABSTRACT

Las infecciones por micobacterias atípicas están adquiriendo cada vez mayor importancia en los pacientes inmunodeprimidos, así como en huéspedes sanos. El Mycobacterium marinum es la micobacteria atípica que afecta la piel con mayor frecuencia. Debe sospecharse ante la existencia de úlceras, nódulos o placas crónicas y el antecedente de un contacto con medios acuáticos. El tratamiento óptimo no está aún bien establecido. Presentamos un nuevo caso de infección por Mycobacterium marinum en un paciente que seguía tratamiento con fármacos inmunosupresores, que respondió favorablemente al tratamiento con doxiciclina y revisamos los distintos regímenes antibióticos utilizados para el tratamiento de la infección por Mycobacterium marinum en los últimos años

ABSTRACT

Lupus erythematosus profundus is a rare manifestation of lupus. We conducted a study to observe the epidemiologic and clinical aspects of lupus erythematosus profundus, its development into other types of lupus or systemic disease, and its response to treatment. Six patients were followed for variable periods. We conclude that lupus erythematosus profundus is a benign form of lupus, distinguishable by systemic manifestations, clinical evolution and resolution, and heterogeneity in clinical manifestations. A skin biopsy is necessary to make an accurate diagnosis.

ABSTRACT

Melanoma is a tumor that has a high tendency to metastasize to the skin. Zosteriform metastases are a rare form of metastasizing melanoma. We describe a 79-year-old man with melanoma presenting with a dermatomal distribution and no visceral metastases.

ABSTRACT

Erythema gyratum repens (EGR) is an uncommon but distinctive dermatosis characterized by marble-like swirls of erythema and a thin covering scale over the trunk, axillae and groins which has been associated with malignancy. Bronchial carcinoma has been the most frequent neoplasm associated. A case of EGR in a 50-year-old man with carcinoma of the lung is reported. The onset of dermatosis preceded the discovery of the neoplasm by 9 months. Oral corticosteroids induced the disappearance of the skin lesions. No recurrence was observed after discontinuation of the treatment. The patient died 1 year after the onset of dermatosis.

ABSTRACT

A severe form of crusted scabies is described in a patient with acquired immunodeficiency syndrome. He was treated with 1 percent lindane lotion and 5 percent sulfur ointment with 2 percent salicylic acid, which resulted in the clearing of the skin lesions.

ABSTRACT

The concurrence of Reiter's syndrome and human immunodeficiency virus infection has recently been noted. In this report the evolution of Reiter's syndrome (arthritis, urethritis, and conjunctivitis) in a patient with a history of intravenous drug abuse and positive results of serologic tests for human immunodeficiency virus is described and the literature is reviewed.

ABSTRACT

Secondary lymphangioma (lymphangiectasis) has been reported as a consequence of lymphatic damage. No specific histological criteria can be used to differentiate primary lymphangioma circumscriptum from lymphangiectasis. We describe a woman who developed lymphangiectases on both non-lymphoedematous upper thighs after hysterectomy and radiation therapy for carcinoma of the cervix. This case differs from the secondary lymphangiomas reported previously in that the vascular channels appeared in newly formed adventitial dermis without histological changes in the epidermis.

ABSTRACT

Factitious disorders in the buccal cavity are infrequent. A 9-year-old boy with a 5-year history of an artefact ulcer of the tongue is described. Psychiatric evaluation revealed an altered personality structure with a background of psychotic traits. Complete remission was observed with psychiatric management.

ABSTRACT

An Elastic Nevus is presented in the form of a solitary plate located in the cheek of a 24-year-olf woman, which had developed over the period of one year. Its histology very much resembles that of a solar elastosis. Its identity is discussed with a very similar case described by Larrotcha and Contreras and with those published by Dégos et al. as "Elastome en nappe du nez".
