ABSTRACT

The distinctive features of a well-known NaI(Tl) scintillation detector, by virtue of its crystal size, are experimentally investigated by observing changes in parameters such as intrinsic efficiency (εi), photo-peak efficiency (εp), resolution, and response function to incident gamma photon energy. This study provides a better understanding for the choice of crystal size of the scintillation detector in Compton scattering experiments. The response function of the NaI(Tl) detector is in the form of an inverse matrix focusing on the retort of the crystal when gamma photons are incident upon it. The response function of the NaI(Tl) detector depends upon the distance between the source and the detector, composition of the material for the crystal itself, photo-fraction, solid angle, incident gamma energy, and geometry of the experimental setup. The factors responsible for broadening of full energy and backscattered peaks are discussed for present investigations. The observed results indicate that the resolution of the detector varies with the incident energy of gamma radiation, and it also depends upon the size of the crystal of the detector. Statistical fluctuations related with the scintillation mechanism are found to be responsible for broadening of instrumental line width (photo-peak). The signal-to-noise ratio and photo-fraction for different crystal sizes of the scintillation detector corrected for efficiency of the detector are also discussed.

ABSTRACT

BACKGROUND: Insulinoma is an islet-cell neoplasm that secretes insulin. It is usually localized to the pancreas and is often the most common cause of endogenous hyperinsulinemic hypoglycaemia in non-diabetic adult patients. Surgical excision with a curative intent is the standard modality of treatment, and it requires precise localization of tumor tissue. Ga-68 DOTA-exendin-4 PET/CT scan is a clinically reasonable and sensitive scan for the identification of insulinoma. The aim of this prospective cohort study was to determine the overall accuracy of Ga-68 DOTA-exendin-4 PET/CT scan in the detection of insulinoma. MATERIALS AND METHODS: Eight patients with fasting hyperinsulinemic hypoglycaemia with neuroglycopenic symptoms were enrolled in this study which was conducted during October 2016 to October 2017. Whole body PET/CT scan was performed on a Philips time of flight PET/CT scanner, 60 minutes after injection of Ga-68 DOTA-exendin-4 (and also Ga-68 DOTANOC). The imaging findings were compared to the histopathological diagnosis in six out of eight patients and to subsequent follow up in the remaining two patients who did not undergo surgery. RESULTS: The sensitivity of Ga-68 DOTA-Exendin-4 PET/CT scan in insulinoma detection was found to be 75%. CONCLUSION: Ga-68 DOTA-Exendin-4 PET/CT scan is highly sensitive for identification and exact localization of insulinoma which can guide better surgical exploration. However, randomised controlled trials are needed to assess the accuracy of Ga-68 DOTA-Exendin PET/CT scan in localization of insulinoma.

ABSTRACT

Retroperitoneum is a relatively uncommon site for pediatric teratomas. Rarely, such tumors can have an intraspinal extension and few cases of retroperitoneal teratomas associated with spinal dysraphism have been reported. Teratomas consist of tissues arising from all three embryonic layers. However, mature renal tissues in the form of glomeruli and tubules are sparingly found in teratomas. A 15-day-old female presented with spina bifida occulta and on evaluation a cystic presacral mass was detected. Intraoperatively the cyst was found densely adherent to the hemivertebrae but not entering the spinal canal. Histopathological examination confirmed a mature cystic teratoma but also demonstrated presence of mature renal elements in the cyst wall. The teratomas lying in proximity to spine and associated with spinal dysraphism are likely to contain mature renal tissues or even nephroblastic elements. It supports the dysembryogenic model of origin of intradural teratomas from native progenitor cells rather than aberrantly migrated germ cells.

ABSTRACT

LR-115 (type II)-based radon-thoron discriminating twin-chamber dosemeters have been used for estimating radon ((222)Rn) and thoron ((220)Rn) concentrations in dwellings of south-western Punjab, India. The present study region has shown pronounced cases of cancer incidents in the public [Thakur, Rao, Rajwanshi, Parwana and Kumar (Epidemiological study of high cancer among rural agricultural community of Punjab in Northern India. Int J Environ Res Public Health 2008; 5(5):399-407) and Kumar et al. (Risk assessment for natural uranium in subsurface water of Punjab state, India. Hum Ecol Risk Assess 2011;17:381-93)]. Radon being a carcinogen has been monitored in some dwellings selected randomly in the study area. Results show that the values of radon ((222)Rn)  varied from 21 to 79 Bq m(-3), with a geometric mean of 45 Bq m(-3) [geometric standard deviation (GSD 1.39)], and those of thoron ((220)Rn)  from minimum detection level to 58 Bq m(-3) with a geometric mean of 19 Bq m(-3) (GSD 1.88). Bare card data are used for computing the progeny concentration by deriving the equilibrium factor (F) using a root finding method [Mayya, Eappen and Nambi (Methodology for mixed field inhalation dosimetry in monazite areas using a twin-cup dosemeter with three track detectors. Radiat Prot Dosim 1998;77(3):177-84)]. Inhalation doses have been calculated and compared using UNSCEAR equilibrium factors and by using the calculated F-values. The results show satisfactory comparison between the values.

ABSTRACT

Radon concentration levels in water and soil gas from 36 locations pertaining to some areas of Malwa region of Punjab have been measured on an in situ basis using a continuous active radon detector (AlphaGuard, Model - PQ 2000 PRO, Genitron instruments, Germany). Exhalation rate measurements have also been carried out at these places, using a closed-circuit technique. The radon concentrations in soil and water varied from 1.9 to 16.4 kBq m(-3) and 5.01 to 11.6 kBq m(-3), respectively. The exhalation rate (E (Rn)) ranged between 7.48 and 35.88 mBq m(-2) s(-1) with an average value of 18.17 mBq m(-2) s(-1). Annual dose rates have been calculated for water radon concentrations. The minimum to maximum values of dose rates were found to be 13.42-31.08 µSv y(-1). The recorded values of radon concentration in water are within the safe limit of 11 Bq l(-1) recommended by the US Environment Protection Agency [National Research Council, Risk Assessment of Radon in Drinking Water (Academy Press, Washington, DC, USA, 1999)]. All measurements were made in similar climatic and environmental conditions to ensure minimal variations in meteorological parameters. An intermediate correlation coefficient (0.5) was observed between radon exhalation rates and soil gas values.

ABSTRACT

The response function, converting the observed pulse-height distribution of a NaI(Tl) detector to a true photon spectrum, is obtained experimentally with the help of an inverse matrix approach. The energy of gamma-ray photons continuously decreases as the number of scatterings increases in a sample having finite dimensions when one deals with the depth of the sample. The present experiments are undertaken to study the effect of target thickness on intensity distribution of gamma photons multiply backscattered from an aluminium target. A NaI(Tl) gamma-ray detector detects the photons backscattered from the aluminium target. The subtraction of analytically estimated singly scattered distribution from the observed intensity distribution (originating from interactions of primary gamma-ray photons with the target) results in multiply backscattered events. We observe that for each incident gamma photon energy, the number of multiply backscattered photons increases with increase in target thickness and then saturates at a particular target thickness called the saturation thickness (depth). Saturation thickness for multiply backscattering of gamma photons is found to decrease with increase in energy of incident gamma-ray photons.

ABSTRACT

We report a case of congenital tuberculosis diagnosed by liver biopsy in a 8-week-old infant presented with acute abdomen. Liver biopsy showed multiple lymphoepitheloid cell granulomas with Langhans' giant cells and central necrosis. The culture of caseous material from the liver showed Mycobacterium tuberculosis . Antituberculous treatment was started. Endometrial biopsy in the asymptomatic mother confirmed the source of infection.
