ABSTRACT

BACKGROUND: Long-term longitudinal studies with multimodal biomarkers are needed to delve into the knowledge of preclinical AD. Subjective cognitive decline has been proposed as a risk factor for the development of cognitive impairment. Thus, including individuals with SCD in observational studies may be a cost-effective strategy to increase the prevalence of preclinical AD in the sample. OBJECTIVES: To describe the rationale, research protocols and baseline characteristics of participants in the Fundació ACE Healthy Brain Initiative (FACEHBI). DESIGN: FACEHBI is a clinical trial (EudraCT: 2014-000798-38) embedded within a long-term observational study of individuals with SCD. SETTING: Participants have been recruited at the memory clinic of Fundació ACE (Barcelona) from two different sources: patients referred by a general practitioner and individuals from an Open House Initiative. PARTICIPANTS: 200 individuals diagnosed with SCD with a strictly normal performance in a comprehensive neuropsychological battery. MEASUREMENTS: Individuals will undergo an extensive neuropsychological protocol, risk factor assessment and a set of multimodal biomarkers including florbetaben PET, structural and functional MRI, diffusion tensor imaging, determination of amyloid species in plasma and neurophthalmologic assessment with optical coherence tomography. RESULTS: Two hundred individuals have been recruited in 15 months. Mean age was 65.9 years; mean MMSE was 29.2 with a mean of 14.8 years of education. CONCLUSIONS: FACEHBI is a long-term study of cognition, biomarkers and lifestyle that has been designed upon an innovative symptom-based approach using SCD as target population. It will shed light on the pathophysiology of preclinical AD and the role of SCD as a risk marker for the development of cognitive impairment.

ABSTRACT

OBJECTIVE: The aim of this study is to value the hearing results obtained in our department following stapedectomy and the reasons of primary failures. MATERIAL AND METHOD: We present a retrospective study of 95 stapedectomies in 82 patients, performed from January 1997 to September 2002 at Hospital Xeral-Cies of Vigo (Pontevedra). RESULTS: Medium presurgical audiometric threshold was 65.71 and postsurgical 26.68. Good results were obtained in 84.21% of cases (air-bone gap closure difference under 20 dB), 10.52% had not change in air threshold and the rest deteriorated. CONCLUSIONS: We compare our results with other authors and we consider that stapedectomy is a safe surgical technique, but it has important complications and they have to be bear in mind to prevent them.

ABSTRACT

Objetivo: El objetivo del presente estudio es valorarlos resultados audiométricos obtenidos por nuestro servicio en la cirugía de la estapedectomía y los motivos que llevan al fallo de la misma. Material y método: Presentamos un estudio retrospectivo deun total de 95 estapedectomías y 82 pacientes, intervenidos desde enero de 1997 hasta septiembre de 2002 en el Hospital Xeral-Cíes de Vigo (Pontevedra). Resultados: El umbral audiométrico prequirúrgico medio fue de 65,71 y el postquirúrgico de 26,68. Obtuvimos una mejoría de la audición en el 84,21% de los casos (umbral auditivo menor de 20 db), en el 10,52% no encontramos mejoría audiométrica y en el resto el umbral audiométrico postquirúrgico empeoró. Conclusiones: Comparando nuestros resultados con los de otros autores podemos decir que la estapedectomía es una técnica quirúrgica segura, pero con complicaciones importantes y que deben tenerse presentes para evitar si es posible (AU)

INTRODUCTION: We present the cases registred from 1998 to 2003 and a revision of the bibliography. MATERIAL AND METHOD: In the section of Rhinology of a second level Hospital 11 patients affected by inverted papilloma type 1 of the OMS classification were diagnosed aged between 40 and 79, 9 men and 2 women. They were explored through endoscopy and biopsy and the exeresis of the lesion was done by endoscopic polipectomy with microdebrider (PEM). RESULTS: All the cases at stage I, II and III of the Krouse classification. Treated by CEN, 6 by mixed endonasal approach and through the canine fossa 4 by nasal approach only and 1 by Caldwell-Luc approach. The follow-up was done between 9 to 64 months and there was a recidive who was operated by open surgery and no one malignant transformation was seen. DISCUSSION: We coment the aspects related to etiology, the symptons and the surgical technics. CONCLUSIONS: We propose to stage the tumor and the realisation of a PEM with topic anesthesia with deep biopsies in all unilateral poly formations to avoid a retarded diagnosis and to be able to apply endoscopic technics.

ABSTRACT

Introducción. Presentamos los casos registrados durante los años 1998 al 2003, haciendo una revisión de la bibliografía. Material y Método. Se diagnosticaron, en la sección de Rinología de un Hospital de 2° nivel, 11 pacientes de Papiloma invertido tipo 1 de la clasificación de OMS, con edades comprendidas entre 40 y 79 años, 9 varones y 2 mujeres. Fueron explorados mediante endoscopia y biopsia, y/o exéresis de la lesión mediante polipectomía endoscópica con microdesbridador (PEM). Resultados. Todos los casos en estadio I, II y III de la clasificación de Krouse. Tratados mediante CEN: 6 con abordaje mixto endonasal y a través de fosa canina, 4 con abordaje sólo endonasal y 1 con abordaje CadlweIl-Luc. Con un seguimiento entre 9 y 64 meses, hubo una recidiva que fue intervenida por cirugía abierta, y ninguna transformación maligna. Discusión. Comentamos los aspectos relacionados con la etiología, los síntomas y las técnicas quirúrgicas. Conclusiones. Proponemos el estadiaje del tumor y la realización de una PEM con anestesia tópica con biopsias profundas en todas las formaciones poliposas unilaterales para evitar el diagnóstico tardío y poder aplicar técnicas endoscópicas

ABSTRACT

In 23 patients suffering from neurofibromatosis (von Recklinghausen's disease), we determined the serum HLA antigens. The phenotypical frequencies obtained were compared with those of 1314 people presumably healthy. Higher frequency along with a relative risk of more than 3 were found for the antigens HLA-A2, HLA-B5, HLA-B15, HLA-CW2, and HLA-CW3.
