ABSTRACT

OBJECTIVE: Recently, RapidSorb plates (DePuy Synthes) made of 85.15 poly (L-lactide-co-glycolide) have been used for orthognathic surgery; however, reports regarding their effectiveness are limited. We aimed to compare the postoperative stability of RapidSorb plates, RapidSorb combined with titanium (MOJ plates), and MOJ plates in patients who underwent Le Fort I osteotomy at Tokyo Medical and Dental University Hospital. STUDY DESIGN: The use of RapidSorb in the maxilla is a load-sharing application and therefore constitutes an approved indication. Discrepancies in the maxillary positions were measured using postoperative computed tomography data at 1 week and 1 year using the centroid method 3-dimensionally. Treatment with RapidSorb alone showed a more vertical discrepancy in the maxilla treatment with MOJ and RapidSorb+MOJ. The RapidSorb4 group was subdivided into 2 groups (under and over 1.0-mm) based on the change in the maxillary centroid. RESULTS: The bone gap at the lateral border of the piriform aperture was significantly larger in the over-1.0-mm group than in the 1.0-mm group. CONCLUSIONS: The fixation of RapidSorb alone is not appropriate in load-bearing and unstable applications but is not contraindicated for load-sharing indications. Fixation with RapidSorb combined with MOJ was clinically effective, with results similar to titanium plate-only fixation regarding postoperative stability.

ABSTRACT

Unlike bulk counterparts, two-dimensional (2D) superconductors are sensitive to disorder. Here, we investigated superconductivity of Pb atomic layers formed on vicinal substrates to reveal how surface steps with an interval shorter than the coherence length ξ affect it. Electrical transport showed reduced critical temperature and enhanced critical magnetic field. Scanning tunneling microscopy exhibited vortices elongated along the steps, that is, Abrikosov-Josephson vortices squeezed normal to the steps due to the reduced ξ. These results demonstrate that steps work as disorder and vicinal substrates provide a unique platform to manipulate the degree of disorder on 2D superconductors.

ABSTRACT

AIMS: Current approaches to classify chronic heart failure (HF) subpopulations may be limited due to the diversity of pathophysiology and co-morbidities in chronic HF. We aimed to elucidate the clusters of chronic patients with HF by data-driven approaches with machine learning in a hospital-based registry. METHODS AND RESULTS: A total of 4649 patients with a broad spectrum of left ventricular ejection fraction (LVEF) in the CHART-2 (Chronic Heart Failure Analysis and Registry in the Tohoku District-2) study were enrolled to this study. Chronic HF patients were classified using random forest clustering with 56 multiscale clinical parameters. We assessed the influence of the clusters on cardiovascular death, non-cardiovascular death, all-cause death, and free from hospitalization by HF. Latent class analysis using random forest clustering identified 10 clusters with four primary components: cardiac function (LVEF, left atrial and ventricular diameters, diastolic blood pressure, and brain natriuretic peptide), renal function (glomerular filtration rate and blood urea nitrogen), anaemia (red blood cell, haematocrit, haemoglobin, and platelet count), and nutrition (albumin and body mass index). All 11 significant clinical parameters in the four primary components and two disease aetiologies (ischaemic heart disease and valvular heart disease) showed statistically significant differences among the 10 clusters (P < 0.01). Cluster 1 (26.7% of patients), which is characterized by preserved LVEF (<59%, 37% of the total) with lowest brain natriuretic peptide (>111.3 pg/mL, 0.9%) and lowest left atrial diameter (>42 mm, 37.4%), showed the best 5 year survival rate of 98.1% for cardiovascular death, 95.9% for non-cardiovascular death, 92.9% for all-cause death, and 91.7% for free from hospitalization by HF. Cluster 10 (6.0% of the total), which is co-morbid disorders of all four primary components, showed the worst survival rate of 39.1% for cardiovascular death, 68.9% for non-cardiovascular death, 23.9% for all-cause death, and 28.1% for free from hospitalization by HF. CONCLUSIONS: These results suggest the potential applicability of the machine leaning approach, providing useful clinical prognostic information to stratify complex heterogeneity in patients with HF.

Background and Aim: Nonalcoholic fatty liver disease (NAFLD) is closely associated with metabolic syndrome. This study was performed to examine the association between NAFLD and each factor of metabolic syndrome and to identify the factors that are most strongly associated with NAFLD in participants undergoing health checkups. Methods: We studied 6538 participants who underwent a health checkup from 2017 to 2018 in our institution. Participants with alcohol intake exceeding 20 g/day or with other chronic liver diseases were excluded. Fatty liver was detected by ultrasonography. Results: In total, 4310 participants were enrolled, and 28.4% had fatty liver (NAFLD). The prevalence of NAFLD was highest in the diabetes mellitus (DM)-only group than in the dyslipidemia-only or hypertension-only group. The DM-only group was the only group whose prevalence of NAFLD was >50% in the overall study and in males. The prevalence of NAFLD was higher in males than in females in the DM-only, hypertension-only, and dyslipidemia-only groups. The prevalence of NAFLD was >70% in the dyslipidemia and DM combined group. Multivariate analysis showed that gender and HbA1c were the independent factors most strongly associated with NAFLD. The cutoff value for HbA1c by receiver operating characteristic curve analysis was 5.8% (sensitivity, 57.9%; specificity, 72.6%; area under the curve, 0.70). Conclusion: NAFLD was most strongly associated with DM, among the various components of metabolic syndrome. We strongly recommend abdominal ultrasonography to detect NAFLD in patients with an HbA1c of ≥5.8% in general practice and during health checkups.

ABSTRACT

BACKGROUND: A hairline crack on the cranium can occur even under a small external load or impact and are thus often observed in patients who have experienced an accidental fall or collision. Typical finite element analysis is useful to analyze the stress concentration or the propagation of stress waves. However, a stress propagation model does not accurately reproduce the features of hairline cracks on the cranium. The objective in this study was to reproduce cranial hairline cracks. METHODS: A three-dimensional finite element model of the cranial bone was developed from a patient CT images. The model consists of the frontal, parietal, occipital and temporal bones, and the bones are connected with the sutures. Additionally, the model comprised three layers; the external and internal tables and the diploe. The model was analyzed using the extended finite element method (X-FEM), and a forming limit diagram (FLD) was embedded in the model. In this study, the model was symmetrized bilaterally using the model developed from the left side of the skull. The FLD in this study was assumed to be a relationship between the maximum and minimum strains when a fracture occurs. A total of 13 typical loadings were applied to the model: loading points on the top, left, and back of the cranium were considered, and at each loading point, loads were applied with four or five different directions, namely perpendicular to the cranium and inclined in the anterior, posterior, superior, or inferior at an angle of 45∘. RESULTS: Under all loading conditions, many small cracks formed radially at the loading points. Moreover, some large cracks formed under the certain loading conditions. The crack shapes on the top and left side could be associated with the specific loading directions, whereas cracks on the back did not show distinguishing characteristics depending on the loading directions. The present model was reproduced anatomically and morphologically, and the results were similar to those obtained in previous cadaver experiments. CONCLUSIONS: Through X-FEM analysis of the FE model embedded with an FLD, hairline cracks in the cranium were reproduced, and a few crack shapes were identified as potential markers for estimating the loading conditions.
