ABSTRACT

The recent description and re-classification of the mesentery as an organ prompted renewed interest in its role in physiological and pathological processes. With an improved understanding of its anatomy, accurately and reliably assessing the mesentery with non-invasive radiological investigation becomes more feasible. Multi-detector computed tomography is the main radiological modality employed to assess the mesentery due to its speed, widespread availability, and diagnostic accuracy. Pathologies affecting the mesentery can be classified as primary or secondary mesenteropathies. Primary mesenteropathies originate in the mesentery and subsequently progress to involve other organ systems (e.g., mesenteric ischemia or mesenteric volvulus). Secondary mesenteropathies describe disease processes that originate elsewhere and progress to involve the mesentery with varying degrees of severity (e.g., lymphoma). The implementation of standardized radiological imaging protocols, nomenclature, and reporting format with regard to the mesentery will be essential in improving the assessment of mesenteric anatomy and various mesenteropathies. In this article, we describe and illustrate the current state of art in respect of the radiological assessment of the mesentery.

ABSTRACT

PURPOSE: Transjugular intrahepatic portosystemic shunt (TIPS) procedure is an established procedure carried out by interventional radiologists to achieve portal decompression and to manage the complications of portal hypertension. The aim of this study was to evaluate the quality and readability of information available online for TIPS procedure. METHODS: Websites were identified using the search terms "TIPS procedure", "TIPSS procedure", "transjugular intrahepatic portosystemic shunt procedure", with the first 25 pages from the three most popular search engines (Google, Bing and Yahoo) being selected for evaluation with a total of 225. Each Website was grouped by authorship into one of five categories: (1) Physician, (2) Academic, (3) For-profit, (4) Non-profit (including government and public health), or (5) Other (discussion/social media). Readability of each Website was assessed using the Flesch-Reading Ease score, Flesch-Kincaid grade level, Gunning-Fog Index, Coleman-Liau and SMOG index. Quality was calculated using the DISCERN instrument, the Journal of the American Medical Association (JAMA) benchmark criteria and the presence of Health on the Net (HON) code certification. RESULTS: After disregarding duplicate and non-accessible Websites a total of 81 were included. The mean DISCERN score assessing the quality of information provided by Websites was "good" (59.3 ± 10.2) with adherence to the JAMA Benchmark being 54.3%. Websites with HON-code certification were statistically significantly higher in terms of DISCERN (p = 0.034) and JAMA scores (p = 0.003) compared to HON-code negative sites. The readability scores of Websites ranged from 10 to 12th grade across calculators. Thirty-two out of the 81 Websites were targeted towards patients (39.5%), 46 towards medical professionals (56.8%) and 3 were aimed at neither (3.7%). The medical professional aimed Websites were statistically significantly more difficulty to read across all readability formulas (all p < 0.001). CONCLUSION: While quality of online information available to patients is "good", the average readability for information on the internet for TIPS is set far above the recommended 7th-grade level. Academic Websites were of the highest quality, yet most challenging for the general public to read. These findings call for the production of high-quality and comprehensible content around TIPS procedure, where physicians can reliably direct their patients for information.

ABSTRACT

A 20 year old female, 14 days post partum, presented to the Emergency Department in severe respiratory distress. Imaging of her chest revealed a left tension hydropneumothorax with significant mediastinal displacement. A chest drain was inserted and over 4L of cloudy-brown malodourous fluid was drained. Gardnerella Vaginalis was isolated on culture of the fluid. The pyohydropneumothorax and associated sepsis, was refractory to management with a chest drain and antimicrobial therapy. She required a video-assisted thoracoscopic surgery and decortication of her unexpanded lung. She ultimately made a full recovery. Gardnerella vaginalis is considered a dysbiosis of the genitourinary tract, rather than an overtly virulent pathogen. Although extremely rare, there are occasional reports of Gardnerella vaginalis causing infection, even at sites distant from the genitourinary tract. To our knowledge, this is the first documented case of Gardnerella vaginalis causing respiratory sepsis and a pyohydropneumothorax in a healthy, immunocompetent female during the post-partum period. Although it is a unique case, nevertheless, it highlights the need for physicians to be congnisant of G. vaginalis as a potential pathogen when treating post-partum sepsis and indeed, even as a potential pathogen when treating pulmonary infections in obstetric patients. This will lend to prompt initiation of appropriate antimicrobial treatment.

ABSTRACT

Seasonal Weight Loss (SWL) is an important constraint, limiting animal production in the Tropics and the Mediterranean. As a result, the study of physiological and biochemical mechanisms by which domestic animal breeds respond to SWL is important to those interested in animal breeding and the improvement thereof. To that end, the study of the proteome has been instrumental in gathering important information on physiological mechanisms, including those underlying SWL. In spite of that, little information is available concerning physiological mechanisms of SWL in production animals. The objective of this study was to determine differential protein expression in the muscle of three different breeds of sheep, the Australian Merino, the Dorper and the Damara, each showing different levels of tolerance to weight loss (low, medium and high, respectively). Per breed, two experimental groups were established, one labeled "Growth" and the other labeled "Restricted." After forty-two days of dietary treatment, all animals were euthanized. Muscle samples were then taken. Total protein was extracted from the muscle, then quantified and two-dimensional gel electrophoresis were conducted using 24 cm pH 3-10 immobiline dry strips and colloidal coomassie staining. Gels were analyzed using Samespots® software and spots of interest were in-gel digested with trypsin. The isolated proteins were identified using MALDI-TOF/TOF. Results indicated relevant differences between breeds; several proteins are suggested as putative biomarkers of tolerance to weight loss: Desmin, Troponin T, Phosphoglucomutase and the Histidine Triad nucleotide-binding protein 1. This information is of relevance to and of possible use in selection programs aiming towards ruminant animal production in regions prone to droughts and weight loss.

ABSTRACT

OBJECTIVE: The human mesentery is now regarded as contiguous from the duodenojejunal (DJ) to anorectal level. This interpretation prompts re-appraisal of computed tomography (CT) images of the mesentery. METHODS: A digital model and reference atlas of the mesentery were generated using the full-colour data set of the Visible Human Project (VHP). Seventy one normal abdominal CT images were examined to identify mesenteric regions. CT appearances were correlated with cadaveric and histological appearances at corresponding levels. RESULTS: Ascending, descending and sigmoid mesocolons were identifiable in 75%, 86% and 88% of the CTs, respectively. Flexural contiguity was evident in 66%, 68%, 71% and 80% for the ileocaecal, hepatic, splenic and rectosigmoid flexures, respectively. A posterior mesocolic boundary corresponding to the anterior renal fascia was evident in 40% and 54% of cases on the right and left, respectively. The anterior pararenal space (in front of the boundary) corresponded to the mesocolon. CONCLUSIONS: Using the VHP, a mesenteric digital model and reference atlas were developed. This enabled re-appraisal of CT images of the mesentery, in which contiguous flexural and non-flexural mesenteric regions were repeatedly identifiable. The anterior pararenal space corresponded to the mesocolon. KEY POINTS: The Visible Human Project (VHP) allows direct identification of mesenteric structures. Correlating CT and VHP allows identification of flexural and non-flexural mesenteric components. Radiologic appearance of intraperitoneal structures is assessed, starting from a mesenteric platform.

BACKGROUND: Muscle fatty acid profile reflects the body condition of animals and has a noticeable effect on meat quality. Herein, longissimus dorsi muscle of three different sheep breeds, Damara (a fat-tailed breed), Dorper and Australian Merino sheep, was analysed for fatty acid composition. The three breeds were subjected to two distinctive feeding levels (ad libitum and restricted feeding) over 42 days. RESULTS: The Damara sheep revealed several differences compared to the other two breeds, namely a higher concentration of polyunsaturated fatty acids, which can be related to being a fat-tailed breed. Even in restricted feeding conditions, this breed revealed the highest levels compared to Merino and Dorper sheep respectively, of linoleic acid (+31% and +28%), linolenic acid (+97% and +51%), eicosapentaenoic acid (EPA) (+65% and +37%), docosapentanenoic acid (DPA) (+31% Merino) and dodosahexanenoic acid (DHA) (+63% and +77%). EPA, DPA and DHA are three omega-3 fatty acids, with described beneficial characteristics. CONCLUSION: With this work we show other qualities (higher levels of the omega-3 fatty acids, EPA, DPA and DHA) of Damara meat that might present this breed as an interesting alternative for animal production in semi-arid climates.

ABSTRACT

PURPOSE: Remote ischemic conditioning has been shown to protect against kidney injury in animal and human studies of ischemia-reperfusion. Recent evidence suggests that conditioning may also provide protection against kidney injury caused by contrast medium. The purpose of this study was to determine if conditioning protected against increases in serum creatinine (SCr) after contrast-enhanced computed tomography (CECT). METHODS: A randomised controlled trial (NCT 01741896) was performed with institutional review board approval and informed patient consent. Adult in-patients undergoing abdomino-pelvic CECT were allocated to conditioned or control groups. Conditioning consisted of four cycles of five minutes of cuff-induced arm ischemia with three minutes of reperfusion applied ~40 minutes before CECT. The primary outcome was SCr change after CECT. RESULTS: Baseline characteristics were similar in both groups. For all patients, conditioning reduced the risk ratio (RR) of increased SCr; RR 0.65 (95% confidence intervals 0.41 to 1.04). The protective effect was greater and the evidence for protection stronger when analysis was restricted to patients with pre-scan reduced renal function (eGFR.

ABSTRACT

The prediction of the preswirl cooling air delivery and disk metal temperature are important for the cooling system performance and the rotor disk thermal stresses and life assessment. In this paper, standalone 3D steady and unsteady computation fluid dynamics (CFD), and coupled FE-CFD calculations are presented for prediction of these temperatures. CFD results are compared with previous measurements from a direct transfer preswirl test rig. The predicted cooling air temperatures agree well with the measurement, but the nozzle discharge coefficients are under predicted. Results from the coupled FE-CFD analyses are compared directly with thermocouple temperature measurements and with heat transfer coefficients on the rotor disk previously obtained from a rotor disk heat conduction solution. Considering the modeling limitations, the coupled approach predicted the solid metal temperatures well. Heat transfer coefficients on the rotor disk from CFD show some effect of the temperature variations on the heat transfer coefficients. Reasonable agreement is obtained with values deduced from the previous heat conduction solution.

OBJECTIVE: The purpose of this article is to evaluate the utility of a tool in quantifying the peak antegrade velocity when assessing patients with cardiac valvular pathology. MATERIALS AND METHODS: Directionally independent peak velocity evaluation (MaxVelocity, Siemens Healthcare) phase-contrast cardiac MRI was performed for 44 patients referred to our institution with a diagnosis or concern for aortic valvular disease or undergoing imaging for thoracic aortic aneurysm. In addition, standard through-plane phase-contrast MR angiography at the level of the aortic valve was performed. The MaxVelocity technique provides a simple tool to extract the magnitude of the peak velocity, independently of its direction, from phase-contrast imaging with velocity encoding. Recent echocardiography (within 1 month) and assessment of peak forward velocity at the level of the aortic valve were required for inclusion in the study. RESULTS: The MaxVelocity technique shows significantly lower error in estimating peak antegrade velocity at the level of the aortic valve than does standard unidirectional through-plane phase-contrast MRI, using transthoracic echocardiography as the reference noninvasive imaging method. CONCLUSION: Relative to standard through-plane imaging, MaxVelocity more closely approximates echocardiography for noninvasive assessment of peak antegrade velocity. Improved accuracy is critical for surgical decision making in patients with aortic valvular disease. Therefore, MaxVelocity provides an easy approach to quantify peak velocity as part of a routine clinical MRI protocol.

ABSTRACT

OBJECTIVE: The joint guidelines of the American College of Cardiology and American Heart Association support the use of contrast-enhanced MR angiography (CEMRA) to diagnose the location and degree of stenosis in patients with known or suspected peripheral arterial disease (PAD). The high prevalence of chronic renal impairment in diabetic patients with PAD and the need for high doses of gadolinium-based contrast agents place them at risk for nephrogenic systemic fibrosis. The purpose of our study was to evaluate the accuracy of the rapid technique of quiescent-interval single-shot (QISS) unenhanced MR angiography (MRA) compared with CEMRA for the diagnosis in diabetic patients referred with symptomatic chronic PAD. SUBJECTS AND METHODS: This prospective two-center study evaluated 25 consecutive diabetic patients with documented or suspected symptomatic PAD. Both centers used identical imaging protocols. Images were independently analyzed by two radiologists. A subgroup analysis was performed of patients who were also assessed with digital subtraction angiography (DSA) as part of the standard-of-care protocol before revascularization. RESULTS: For this study, 775 segments were analyzed. On a per-segment basis, the mean values of the diagnostic accuracy of unenhanced MRA compared with reference CEMRA for two reviewers, reviewers 1 and 2, were as follows: sensitivity, 87.4% and 92.1%; specificity, 96.8% and 96.0%; positive predictive value, 90.8% and 94.0%; and negative predictive value, 95.5% and 94.6%. Substantial agreement was found when overall DSA results were compared with QISS unenhanced MRA (κ = 0.68) and CEMRA (κ = 0.63) in the subgroup of patients who also underwent DSA. There was almost perfect agreement between the two readers for stenosis scores, with Cohen's kappa values being greater than 0.80 for both MRA techniques. CONCLUSION: The results of our study indicate that QISS unenhanced MRA is an accurate noncontrast alternative to CEMRA for showing clinically significant arterial disease in patients with diabetes with symptomatic PAD.

ABSTRACT

PURPOSE: : To develop a flow-sensitive dephasing (FSD) preparative scheme to facilitate multidirectional flow-signal suppression in 3-dimensional balanced steady-state free precession imaging and to validate the feasibility of the refined sequence for noncontrast magnetic resonance angiography (NC-MRA) of the hand. MATERIALS AND METHODS: : A new FSD preparative scheme was developed that combines 2 conventional FSD modules. Studies using a flow phantom (gadolinium-doped water 15 cm/s) and the hands of 11 healthy volunteers (6 males and 5 females) were performed to compare the proposed FSD scheme with its conventional counterpart with respect to the signal suppression of multidirectional flow. In 9 of the 11 healthy subjects and 2 patients with suspected vasculitis and documented Raynaud phenomenon, respectively, 3-dimensional balanced steady-state free precession imaging coupled with the new FSD scheme was compared with spatial-resolution-matched (0.94 × 0.94 × 0.94 mm) contrast-enhanced magnetic resonance angiography (0.15 mmol/kg gadopentetate dimeglumine) in terms of overall image quality, venous contamination, motion degradation, and arterial conspicuity. RESULTS: : The proposed FSD scheme was able to suppress 2-dimensional flow signal in the flow phantom and hands and yielded significantly higher arterial conspicuity scores than the conventional scheme did on NC-MRA at the regions of common digitals and proper digitals. Compared with contrast-enhanced magnetic resonance angiography, the refined NC-MRA technique yielded comparable overall image quality and motion degradation, significantly less venous contamination, and significantly higher arterial conspicuity score at digital arteries. CONCLUSION: : The FSD-based NC-MRA technique is improved in the depiction of multidirectional flow by applying a 2-module FSD preparation, which enhances its potential to serve as an alternative magnetic resonance angiography technique for the assessment of hand vascular abnormalities.

ABSTRACT

PURPOSE: To assess the diagnostic performance of quiescent-interval single-shot (QISS) magnetic resonance (MR) angiography, a nonenhanced two-dimensional electrocardiographically gated single-shot balanced steady-state free precession examination for the evaluation of symptomatic chronic lower limb ischemia. MATERIALS AND METHODS: For this prospective institutional review board-approved, HIPAA-compliant study, the institutional review board waived the requirement for informed patient consent. The QISS nonenhanced MR angiography technique was evaluated in a two-center trial involving 53 patients referred for lower extremity MR angiography for suspected or known chronic peripheral arterial disease (PAD), with contrast material-enhanced MR angiography serving as the noninvasive reference standard. The accuracy of stenosis assessments performed with the nonenhanced MR angiography sequence was evaluated relative to the reference standard. Per-segment, per-region, and per-limb sensitivities and specificities were calculated, and assessments were considered correct only if they were in exact agreement with the reference standard-derived assessments. Generalized estimating equation (GEE) modeling with use of an unstructured binomial logit analysis was used to account for clustering of multiple measurements per case. The sensitivity and specificity of QISS MR angiography for the determination of nonsignificant (<50%) versus significant (50%-100%) stenosis were compared with the sensitivity and specificity of the reference standard. RESULTS: The diagnostic performance of nonenhanced MR angiography was found to be nearly equivalent to the diagnostic performances of contrast-enhanced MR angiography and digital subtraction angiography. Non-GEE segment-based analysis revealed that for the two reviewers, nonenhanced MR angiography had sensitivities of 89.7% (436 of 486 segments) and 87.0% (423 of 486 segments) and specificities of 96.5% (994 of 1030 segments) and 94.6% (973 of 1028 segments). CONCLUSION: QISS nonenhanced MR angiography offers an alternative to currently used imaging tests for symptomatic chronic lower limb ischemia, for which the administration of iodinated or gadolinium-based contrast agents is contraindicated.

• The influence of soil P level and plant development on carboxylate concentrations in the rhizosphere of lateral roots of chickpea (Cicer arietinum) cultivars Heera and Tyson were studied to investigate the relationship between carboxylate exudation and P acquisition by chickpea. • Two chickpea cultivars were grown in soil supplied with 0-100 µg P g-1 . Plants were harvested four times during plant development. In two other experiments, carboxylate concentrations were measured along root sections, using plants grown in soil or in hydroponics. • Carboxylate concentrations in the rhizosphere of lateral roots steadily increased with plant development for cv. Heera, but not for Tyson. Carboxylate concentrations increased with increasing distance from the root apex in soil. This increase might be due to accumulation, because an experiment with plants in hydroponics showed that older segments released at least as much carboxylates as younger segments. • Carboxylate concentrations in the rhizosphere of chickpea appear to be developmentally controlled. Unlike some other plant species, there is no simple correlation with plant or soil P status.
