ABSTRACT

INTRODUCTION: Traumatic Arteriovenous Fistulas (AVFs) are a rare complication of vascular injuries and pose a problem of topographic diagnosis and therapeutic management. Delayed treatment may cause cardiac and trophic complications. PRESENTATION OF CASES: We describe 4 cases of AVFs complicating stab wounds of the lower limb, associated in 2 cases with pseudoaneurysms. DISCUSSION: In two cases we performed a surgical repair, whereas in the other 2 patients we choose an endovascular treatment with wall graft stent placement. CONCLUSION: All traumatic AVFs must be treated to avoid vascular, local and general complications. Using both techniques, we had great results with no complications.

ABSTRACT

Extracorporeal shock wave lithotripsy (ESWL) can lead to rare but severe arterial complications. The causal relationship of ESWL with arterial pseudoaneurysm formation in a patient with Behçet disease is discussed. A 35-year-old man presented with acute recrudescence of right flank pain caused by ureteral lithiasis immediately after having undergone a last session of ESWL. Abdominal examination revealed a painful pulsatile mass in the right iliac region. Contrast-enhanced computed tomography identified a pseudoaneurysm of the right external iliac artery in a patient with a medical history of Behçet disease. The pseudoaneurysm was treated endovascularly, including the use of a covered stent with intensive immunosuppressive therapy. We obtained successful exclusion of the pseudoaneurysm. This case highlights that arterial pseudoaneurysm may occur after ESWL in patients with Behçet disease. Strict follow-up after ESWL sessions is necessary.

ABSTRACT

INTRODUCTION: Aneurysms of the hepatic artery are relatively rare. Diagnosis often occurs only after rupture, and mortality is substantial. We report two new cases. CASES: The first patient was a 70-year-old woman whose asymptomatic aneurysm of the common hepatic artery was identified during ultrasound for vomiting and abdominal pain. Open aneurysm repair and a hepatosplenic bypass led to a favorable outcome. The second patient was a 54-year-old man; his right hepatic artery aneurysm was diagnosed after it ruptured in the gall bladder. DISCUSSION: We review the literature about hepatic artery aneurysms and discuss the clinical features and methods for diagnosis and treatment of these uncommon lesions.

ABSTRACT

INTRODUCTION: Gallbladder volvulus remains a relatively rare disorder. OBSERVATION: We report two cases, both in men older than 80 years and both diagnosed during surgery. Both patients presented signs of peritonitis (one generalized and the other localized) and severe sepsis. DISCUSSION: We review the literature about gallbladder volvulus to analyze its incidence, etiology, and clinical, radiologic and therapeutic characteristics.

ABSTRACT

Extracranial carotid aneurysm due to Behçet disease is extremely rare. To our knowledge, this complication has been previously reported in only 12 cases. We report two new cases of extracranial carotid aneurysm in Behçet disease and discuss the clinical features, therapeutic modalities, and postoperative complications of these uncommon lesions.
