ABSTRACT

OBJECTIVES: This double-blind, randomized, placebo-controlled clinical trial evaluated the adjuvant effects of Bifidobacterium lactis HN019 on the treatment of plaque-induced generalized gingivitis. MATERIALS AND METHODS: Sixty patients were submitted to professional supragingival scaling and prophylaxis. They were randomly assigned to test (probiotic lozenges containing B. lactis HN019, n = 30) or control (placebo lozenges, n = 30) groups. Lozenges were consumed twice a day for 8 weeks. Bleeding on probing (BoP), Gingival Index (GI), Plaque Index (PI), probing depth (PD), and clinical attachment level (CAL) were evaluated at baseline and after 2 and 8 weeks. Gingival crevicular fluid (GCF) was collected at baseline and at 8 weeks for analysis of the inflammatory mediators IL-1ß, IL-1α, IL-8, MCP-1, and MIP-1ß. Data were statistically analyzed (p < 0.05). RESULTS: After 8 weeks, both groups showed reduction in the percentage of PI, with no significant difference between groups (p = 0.7423). The test group presented a lower percentage of BoP and a higher percentage of sites with GI ≤ 1 when compared with the control group at the end of the study (p < 0.0001). At 8 weeks, the test group had a greater number of patients without generalized gingivitis than the control group (20 and 11 patients, respectively; p < 0.05). The test group presented significantly lower levels of IL-1α, IL-1ß, and MCP-1 in GCF than the control group at the end of the study (p < 0.05). CONCLUSION: The adjunct use of B. lactis HN019 promotes additional clinical and immunological benefits in the treatment of generalized gingivitis. CLINICAL RELEVANCE: B. lactis HN019 can be an efficient and side-effect-free adjunct strategy in the treatment of generalized gingivitis.

ABSTRACT

This study aimed to assess the effects of the probiotic (PROB) Bifidobacterium animalis subsp. lactis HN019 in two different delivery vehicles in experimental periodontitis (EP), including the gene expression for IL-10, IFN-γ, and FOXP3. In total, 32 rats were assigned into groups (n=8): C (control), EP, EP-PROB/Water, and EP-PROB/Milk. The probiotic was administered for 4 weeks, from baseline to euthanasia. Periodontitis was induced by ligatures 14 days after baseline. Data were statistically analyzed (p<0.05). Both probiotic groups presented decreased alveolar bone loss and increased interproximal attachment level than group EP. Also, these parameters were significantly improved in the Milk group when compared with the Water group. EP-PROB/Milk showed higher gene expression for IL-10 and lower for FOXP3 in relation to EP-PROB/Water and EP groups. The use of milk was able to potentiate the protective effects of B. lactis HN019 in rats under EP.

ABSTRACT

As análises clínicas laboratoriais possuem papel fundamental na saúde, pois é através de seus resultados que 70% dos diagnósticos são concluídos pelos médicos, e desta forma é necessário que se preze pela emissão de resultados confiáveis. Entretanto, uma das fases do processo de realização dos exames laboratoriais tem se destacado como a fase que concentra o maior percentual de erros no âmbito laboratorial até os dias atuais. Trata-se da fase pré-analítica, que se inicia com a solicitação do exame e finaliza quando se inicia a análise. Os erros pré-analíticos podem ser responsáveis por até 75% dos erros laboratoriais, o que mostra a vulnerabilidade desta fase. Portanto, este trabalho tem como objetivo identificar os principais erros que estão vinculados à fase pré-analítica, assim como os impactos que esses erros podem causar. Os tipos de erros pré-analíticos foram revisados na literatura entre os anos de 2011 a 2020. Os resultados apontaram que os principais erros pré-analíticos estão associados ao momento da coleta de amostras biológicas. Isto mostra a necessidade de gerar melhorias nos setores de coleta, através da realização de treinamentos voltados para as equipes de flebotomia, com o intuito de aperfeiçoar este processo e diminuir as taxas dos erros provocados nesta fase.


The clinical laboratory analyses have a fundamental role in health, because it is through their results that 70% of the diagnoses are concluded by the doctors, therefore, it is necessary to give priority to the issuance of reliable and safe results. However, one of the phases of the process of performing laboratory tests has been highlighted as the phase that concentrates the highest percentage of errors in the laboratory environment until the present day.It is the pre-analytical phase, which begins with the request of the exam and ends when the analysis begins.The pre-analytical errors can be responsible for up to 75% of laboratory errors, which shows the vulnerability of this phase. Therefore, this work aims to identify the main errors that are linked to the pre-analytical phase, as well as the impacts that these errors may cause. The types of pre-analytical errors were reviewed in the literature between 2011 and 2020. The results showed that the main pre-analytical errors are associated with the time of collection of biological samples. This shows the need to generate improvements in the collection sectors, through training for phlebotomy teams, in order to improve this process and decrease the rates of errors caused in this phase.

ABSTRACT

We report the case of a 32-year-old man from Rio de Janeiro, who was infected in the Amazon region of Brazil by Leishmania (Viannia) naiffi. Generally, patients with L. naiffi cutaneous leishmaniasis exhibit a good therapeutic response to either pentavalent antimonials or pentamidine. However, after pentamidine treatment, this patient's infection evolved to therapeutic failure. To understand this clinical outcome, we investigated the presence of the Leishmania RNA virus (LRV) in parasites isolated from the cutaneous lesion; herein, we discuss the possible association between a poor response to pentamidine therapy and the presence of the LRV.

ABSTRACT

In the Brazilian Amazon, American tegumentary leishmaniasis (ATL) is endemic and presents a wide spectrum of clinical manifestations due, in part, to the circulation of at least seven Leishmania species. Few reports of Leishmania (Viannia) naiffi infection suggest that its occurrence is uncommon and the reported cases present a benign clinical course and a good response to treatment. This study aimed to strengthen the clinical and epidemiological importance of L. (V.) naiffi in the Amazon Region (Manaus, state of Amazonas) and to report therapeutic failure in patients infected with this species. Thirty Leishmania spp samples isolated from cutaneous lesions were characterised by multilocus enzyme electrophoresis. As expected, the most common species was Leishmania (V.) guyanensis (20 cases). However, a relevant number of L. (V.) naiffi patients (8 cases) was observed, thus demonstrating that this species is not uncommon in the region. No patient infected with L. (V.) naiffi evolved to spontaneous cure until the start of treatment, which indicated that this species may not have a self-limiting nature. In addition, two of the patients experienced a poor response to antimonial or pentamidine therapy. Thus, either ATL cases due to L. (V.) naiffi cannot be as uncommon as previously thought or this species is currently expanding in this region.

ABSTRACT

Low-level laser therapy (LLLT) benefits bone metabolism, but its use needs to be standardized. We evaluated the effects of LLLT on bone defects in calvaria of ovariectomized rats. Stereology was used to calculate tissue repair volume (V tr ), density of trabecular bone volume (Vv t ), total volume of newly formed trabecular bone (Vtot), and the area occupied by collagen fibers (A C ). Fifty-four Wistar rats were submitted to bilateral ovariectomy, and bone defects were created in calvaria after 150 days. The animals were divided into nine groups (n = 6), and 24 h after defects, the treatment started with a 780-nm low-intensity GaAlAs laser: G1, G2, and G3 received 3 sessions of 0, 20, and 30 J/cm(2) respectively; G4, G5, and G6 received 6 sessions of 0, 20, and 30 J/cm(2), respectively; and G7, G8, and G9 received 12 sessions of 0, 20, and 30 J/cm(2), respectively. A normal distribution was found for all of the data. The test used to verify the normality was the Kolmogorov-Smirnov (KS, p > 0.05). The one-way ANOVA followed by Tukey's post hoc test was used for data processing. A difference of p < 0.05 was considered statistically significant. Groups G2 and G1 showed significance for V tr , Vv t , Vtot, and (A C ). Results were significant for (Vv t ) and (Vtot) between G3 and G1. There were no significant results between G5 and G4 as well as between G8 and G7. Groups G6 and G4 results showed statistical difference for V tr , Vv t , Vtot, and (A C ). Groups G9 and G7 showed significance for V tr , Vv t , Vtot, and (A C ). In conclusion, there was new bone formation in the groups that received 20 and 30 J/cm(2) when compared to control groups, but over time, the dose of 30 J/cm(2) showed better stereological parameters when compared to 20 J/cm(2).

A principal estratégia para controlar a leishmaniose tegumentar Americana (LTA) seria a imunoprofilaxia. Entretanto, até o momento ainda não existe uma vacina, dificultando o controle da doença. [...] O propósito desta tese foi avaliar a evolução pós-tratamento de casos de infecção por Leishmania naiffi (Ln) e L. braziliensis (Lb) sob dois aspectos diferentes: 1) descrever uma série de casos de LTA causados por Ln selecionados dentre trinta pacientes atendidos na Fundação de Medicina Tropical Dr. Heitor Vieira Dourado(FMT-HVD), Manaus, no período de 2011 a 2013. 2) avaliar se a assinatura imunológica, em termos de subclasses de imunoglobulina e perfil de diferenciação celular e memória, é similar entre pacientes de Lb com até 26 anos de cura após tratamento com dois diferentes esquemas terapêuticos com antimônio. Foram estudados 43 pacientes curados de leishmaniose cutânea (LCC), sendo estes divididos em dois grupos: pacientes de LCC acompanhados desde a fase ativa até três anos (a) após cura da infecção por Lb (n= 23, LCC<3a) e pacientes LCC com até 26 anos de cura da infecção porLb (n= 12, LCC 12-26a). Além disso, 30 isolados de Leishmania de pacientes de leishmaniose cutânea (LC) proveniente da FMT-HVD foram avaliados. Células mononucleares do sangue periférico (CMSP) foram analisadas após estímulo in vitro com antígenos de Lb. A fenotipagem das subpopulações de linfócitos T foi realizada através da citometria de fluxo quanto à ativação, imunosenescência e memória imunológica. A produção de IgG e suas subclasses anti-Leishmania foram avaliadas por ELISA. Os isolados de Leishmania foram caracterizados quanto à espécie pela técnica de isoenzimas...
