ABSTRACT

RESUMEN Fundamento: la mortalidad excesiva es considerada como un indicador especialmente útil para evaluar el impacto sanitario de la COVID-19; a nivel internacional, su empleo ha ido en aumento durante todo el 2021, pero en la literatura científica nacional su presencia es, hasta donde conocemos, nula. Objetivo: definir el indicador mortalidad excesiva y describir su empleo en el cálculo de la mortalidad excesiva para Cuba en el 2020, comparándola con la de otros países. Métodos: estudio ilustrativo del empleo e interpretación de un indicador de impacto de la COVID-19. Resultados: la mortalidad excesiva en Cuba en 2020 se compara, favorablemente, con la de otros países, en particular del área Latinoamericana. Conclusiones: la incidencia de la enfermedad, junto con las intervenciones no farmacéuticas implementadas por los gobiernos, entre otros factores, influyen en la mortalidad excesiva. En el caso de Cuba, es necesario calcularla para 2021, dado que en ese año se observaron las mayores tasas de incidencia, y de esa manera contribuir a la evaluación de la resiliencia global de nuestro Sistema de Salud en el enfrentamiento a la pandemia


ABSTRACT Background: Excessive Mortality is considered an especially useful indicator to assess the health impact of COVID-19 at the international level; its employment has been increasing throughout 2021, but in the national scientific literature its presence is, as far as we know, null. Objectives: to define and discuss the characteristics and computation requirements. To illustrate the use of the indicator through calculation of Excessive Mortality for Cuba, and compare results with other countries. Methods: Illustrative study of the use and interpretation of an indicator of COVID-19 impact. Results: Excessive Mortality in Cuba compares favorably with that of other countries, in particular in the Latin American region. Conclusions: The incidence of the disease, together with the Non-Pharmaceutical Interventions implemented by Governments, among other factors, influence Excessive Mortality. In the case of Cuba, it is necessary to calculate the Excessive Mortality for 2021, given that in that year the highest incidence rates were observed, and thus contribute to the evaluation of the global resilience of our Health System in the face of the pandemic.

ABSTRACT

INTRODUCTION: COVID-19 sequelae, or the short-, medium-, and long-term manifestations of the disease are under continuous study. There are currently few reports on the evolution of hematological variables following a demonstrated absence of SARS-CoV-2 after infection. OBJECTIVE: Identify hematological alterations in Cuban adults recovered from SARS-CoV-2 infection, and their relation with disease severity. METHODS: We selected 348 persons recovered from COVID-19 residing in Havana, Cuba with an RT-PCR study negative for SARS-CoV-2 performed two weeks after hospital discharge; a structured survey was administered to obtain clinical-epidemiological data. Three groups were established according to COVID-19 clinical criteria: asymptomatic, mild/moderately symptomatic, and severely symptomatic, which, in turn, were divided according to hospital discharge date and blood sample collection date. We performed hemograms with differential leukocyte counts and compared results among groups. We then measured the associations between hematological variables, personal medical history, and relevant lifestyle habits (smoking). RESULTS: All hematological variables were within normal reference limits, although men from the group of severely ill patients had increased total leukocytes, neutrophils and lymphocytes, and decreased hemoglobin and eosinophils, which was also evident in those with a recovery time of 31-90 days. CONCLUSIONS: The relation between hematological variables and degree of clinical severity offers evidence as to persistence of systemic alterations (possibly inflammatory) associated with viral infection. Their identification and characterization can facilitate personalized patient followup and rehabilitation.

ABSTRACT

In El Salvador, a form of chronic kidney disease (CKD) of nontraditional causes (CKDnt) affecting farmers is being reported. Its behavior has been epidemic and is responsible for tens of thousands of deaths. This article summarizes the results obtained from a series of studies conducted to identify the epidemiology and clinical behavior of this disease, proposing a case definition and an etiopathogenic hypothesis. Methods included a survey of CKD in agricultural communities studying 2,388 people ≥ 18 years and 1,755 < 18, a descriptive clinical study followed by histopathological assessment conducted in 46 possible cases of CKDnt ≥ 18 years, and a national survey to study the prevalence of CKD and associated risk factors in 4,817 participants ≥ 20 years followed by a nested case-control study. In the agricultural communities, the prevalence of CKD in adults was 18% (men: 23.9%, women: 13.9%), 26.8% in agricultural workers (non-agricultural 13.8%), CKDnt accounted for 51.9% of cases. CKD in the population < 18 years was 3.9% (mean estimated glomerular filtration rate > 160 mL/1.73m2). The national CKD prevalence was 12.6% (urban: 11.3%; rural: 14.4%; males: 17.8%, females 8.5%), and CKDnt was only 3.8%; with associations between CKD and exposure to agrochemicals. The clinical study revealed the presence of markers of kidney damage (A3 albuminuria: 80.4%; ß2-microglobulin: 78.2%), urine electrolyte anomalies (100% hypermagnesuria, 45.7% hypernatriuria, 43.5% osmotic polyuria), abnormal osteotendinous reflexes (45.7%), sensorineural hearing loss (56.5%), and damage of the tibial arteries by Doppler imaging (66.7%). Biopsies revealed a chronic tubulointerstitial nephropathy. The etiopathogenesis of CKDnt is possibly multifactorial, including environmental contamination by agrochemicals, heat stress, and dehydration.

ABSTRACT

In El Salvador, chronic kidney disease had reached epidemic propor-tions towards the end of this century's first decade. In 2011-2012, the Ministry of Health reported it was the leading cause of hospital deaths in men, the fifth in women, and the third overall in adult hospital fa-talities. Farming was the most common occupation among men in dialysis (50.7%). By 2017, chronic kidney disease admissions had overwhelmed hospital capacity.In 2009, El Salvador's Ministry of Health, Cuba's Ministry of Public Health and PAHO launched a cooperative effort to comprehensively tackle the epidemic. The joint investigations revealed a total prevalence of chronic kidney disease in the adult population of farming communities higher than that reported internationally (18% vs.11%-14.8%), higher in men than in women (23.9% vs 13.9%) and higher in men who were farmers/farmworkers than in men who were not (31.3% vs. 14.8%). The disease was also detected in children. An association was found between chronic kidney disease and exposure to agrochemicals (OR 1.4-2.5). In 51.9% of all chronic kidney disease cases, traditional causes (diabetes, hyperten-sion, glomerulopathies, obstructive nephropathies and cystic diseases) were ruled out and the existence of a particular form of chronic kidney disease of nontraditional etiology was confirmed (whose initial cases were reported as early as 2002). In the patients studied, functional altera-tions and histopathologic diagnosis confirmed a chronic tubulointerstitial nephritis; most presented with neurosensory hearing loss, altered tendon refiexes and tibial artery damage. The main results of this cooperation were the epidemiologic, physio-pathologic, clinical and histopathologic characterization of chronic kid-ney disease of nontraditional etiology. This characterization facilitated case definition for the epidemic and led to the hypothesis of systemic toxicity from agrochemicals (e.g., paraquat, glyphosate), which par-ticularly affect the kidneys and to which farmers/farmworkers (who may also become dehydrated in the fields) are most exposed. The research thus also laid the foundations for design of comprehensive intersectoral government actions to reduce cases and put an end to the epidemic. Objetivos. La atención prenatal es uno de los pilares de la salud pública y permite el acceso a intervenciones tales como la prevención de la transmisión materno-infantil del VIH y de la sífilis congénita. Este artículo tiene como objetivo describir los factores sociales asociados con la utilización de los servicios de atención prenatal en Ecuador. Métodos. Entre 2011 y 2012, se realizó un análisis de la información procedente de las historias clínicas y de la entrevista a las participantes, que integraron una muestra probabilística a nivel nacional de 5 998 mujeres atendidas por parto o aborto en 15 servicios sanitarios en Ecuador con el objetivo de estimar la prevalencia de VIH, sífilis, enfermedad de Chagas y la cobertura de atención prenatal. Resultados. El estudio mostró que 94,1% de las mujeres había acudido a algún control prenatal, pero la asistencia al menos a cuatro controles fue 73,1%. Se encontró que el menor nivel educativo, el mayor número de embarazos, la ocupación en el sector agrícola o ganadero y la pertenencia a los grupos étnicos indígena, afroecuatoriano u otros minoritarios fueron factores asociados con la falta de uso (ningún control prenatal) o al uso inadecuado de la atención prenatal (menos de cuatro controles o primer control después de las 20 semanas de gestación) en Ecuador. Conclusiones. Estos resultados apuntan a la persistencia de desigualdades marcadas en el acceso y en la utilización de servicios de atención prenatal atribuibles a factores socioeconómicos y a la necesidad de fortalecer las estrategias para su abordaje para alcanzar la meta de la cobertura universal de atención prenatal.

ABSTRACT

RESUMEN Tras varios decenios de críticas a las técnicas inferenciales basadas en las pruebas de significación estadística orientadas al rechazo de la llamada "hipótesis nula" y, a pesar del notable consenso alcanzado entre los estadísticos profesionales, este recurso se mantiene vigente tanto en las publicaciones biomédicas, entre ellas las de Salud Pública, como en cursos introductorios de estadística. Entre las muchas deficiencias señaladas por los más prominentes especialistas se destacan tres por ser las más obvias y fáciles de comprender: que no contribuyen a cumplimentar la encomienda de la ciencia, que se conocen de antemano las respuestas a las preguntas que se encaran por su conducto y que los resultados que producen dependen de un elemento ajeno a la realidad estudiada: el tamaño muestral. El artículo discute en detalle tales limitaciones, ilustra su perniciosa presencia en la investigación actual y valora las razones para la subsistencia de la sinrazón en esta materia.


ABSTRACT After decades of criticism against inferential techniques based on statistical significance tests, which mainly reject the so-called "null hypothesis", and in spite of the remarkable consensus among professional statisticians, this resource remains prevalent in both biomedical publications (including public health journals) and introductory statistics courses. Among the many problems identified by the most prominent specialists, three of them are the most obvious and easy to understand: that these tests do not contribute to the actual enterprise of science, that the answers to the questions that are addressed are known in advance and that their results depend critically on an element that is external to the domain that is being studied: sample size. This paper discusses in detail these limitations, illustrates their pernicious presence in current research and evaluates the reasons for the survival of the senselessness in this matter.


RESUMO Trás vários decênios de críticas as técnicas inferenciais baseadas nas provas de significação estatística orientadas ao rejeito da chamada "hipótese nula" e, embora do notável consenso alcançado entre os estatísticos profissionais, este recurso se mantem vigente tanto nas publicações biomédicas, entre elas as de Saúde Pública, como nos cursos introdutórios de estatística. Entre as muitas deficiências assinaladas pelos mais proeminentes especialistas se destacam três por ser as mais obvias e fácies de compreender: que não contribuem a complementar a encomenda da ciência, que se conhecem de antemão as respostas ás perguntas que se encaram pelo seu conduto y que os resultados que produzem depende dum elemento alheio á realidade estudada: o tamanho amostral. O artigo discute em detalhe tais limitações, ilustra a sua perniciosa presença na investigação atual e valora as razões para a subsistência da sem-razão em esta matéria.

ABSTRACT

A nationwide survey was conducted to obtain an estimate of Chagas disease prevalence among pregnant women in Ecuador. As part of a national probability sample, 5,420 women seeking care for delivery or miscarriage at 15 healthcare facilities were recruited into the study. A small minority of participants reported knowing about Chagas disease or recognized the vector. A national seroprevalence of 0.1% (95% confidence interval [95% CI] = 0.0-0.2%) was found; cases were concentrated in the coastal region (seroprevalence = 0.2%; 95% CI = 0.0-0.4%). No cases of transmission to neonates were identified in the sample. Seropositive participants were referred to the National Chagas Program for evaluation and treatment. Additional studies are necessary to determine if areas of higher prevalence exist in well-known endemic provinces and guide the development of a national strategy for elimination of mother-to-child transmission of Chagas disease in Ecuador.

ABSTRACT

OBJECTIVES: This study aimed to obtain nationally representative estimates of HIV and syphilis prevalence and coverage of preventive antenatal services in pregnant women in Ecuador, in order to develop a national strategy for the elimination of mother-to-child transmission of HIV and syphilis. METHODS: A national probability sample of 5988 women presenting for delivery or miscarriage services was selected from 15 healthcare facilities during 2011-2012, using a two-stage cluster sample technique. Biological specimens were collected and an interview and review of medical records were performed. Agreement between these last two sources was measured. Estimates were adjusted for the sampling design. RESULTS: Estimated national HIV prevalence (0.60%) was higher than confirmed syphilis infection prevalence (0.25%). In the coastal region, HIV prevalence (1.13%) exceeded the threshold that defines a generalised epidemic and syphilis prevalence reached 0.37%. An estimated 5.9% of women did not use antenatal care services while 73.0% completed at least four consultations. HIV testing coverage (89.9%) was higher than for syphilis (71.6%). Agreement between medical records and interviews was mostly moderate (0.40-0.75). Important variables were frequently not recorded, such as timing of syphilis testing, which was not recorded in 49.6%. CONCLUSIONS: The concentration of HIV and syphilis infections in the coastal region of Ecuador highlights the need for intensified prevention and a response tailored to local epidemic conditions. Major challenges for the elimination initiative include achieving universal, early access to antenatal care, improving coverage of HIV and syphilis testing, and improving the quality of medical records to support progress monitoring.

ABSTRACT

Con vistas a identificar las experiencias más relevantes de la población en materia de atención continuada a los pacientes, así como para conocer el criterio de los profesionales de la APS en relación con el papel que desempeñan en el proceso de referencia de pacientes y el apoyo que reciben (para su gestión) de las otras unidades con las cuales interrelacionan, se realizó un estudio descriptivo de índole retrospectiva en el área de salud Plaza de la Revolución en la Ciudad de La Habana. A través de una encuesta se exploraron los criterios de Médicos de Familia y la población que recibe el servicio. Nuestro universo estuvo constituido por los 32 médicos que laboran en el policlínico y que atienden a la población residente en el área. El universo de destinatarios de los servicios lo constituyeron todas las familias que pertenecen a esta área de salud (5 895). Se realizó un muestreo por conglomerados bietápico con selección de consultorios con probabilidad proporcional al número de familias y selección sistemática de familias dentro del consultorio (196 en total). Se identificó que, si bien es aceptable el acceso de la población a los servicios del policlínico, la regionalización de los servicios entre los niveles primario y secundario presenta dificultades apreciables. Además se evidencia que a pesar de que la inmensa mayoría de los pacientes accedió a la atención secundaria por mecanismos colaterales, la accesibilidad de la población al sistema (en última instancia) estuvo garantizada(AU)


In order to identify the most significant experiences of the population concerning the continuing attention to the patients and to know the criteria of the primary health care professionals in relation to the role they played in the process of patient referral and to the support they received (for their management) from other units with which they are interrelated, a descriptive and retrospective study was conducted in the health area of Plaza de la Revolución, in Havana City. A survey was done to know the criteria of the family physicians and of the population receiving the medical services. Our universe was composed of 32 physicians who worked at the polyclinic and attended the population living in the area and of all the families (5 895) from this health area that recieved the medical services. A two-stage cluster sampling was performed by selecting the family physician's offices with a probability proportional to the number of families and by systematic selection of families within the family physician's office (l96 in total). It was observed that though the access of the population to the services of the polyclinic is acceptable, the health care delivery by regions at the primary and secondary levels have considerable difficulties. It was also proved that in spite of the fact that most of the patients had access to secondary health care by collateral mechanisms, the accessibility of the population to the system was guaranteed(AU)

ABSTRACT

Con vistas a identificar las experiencias más relevantes de la población en materia de atención continuada a los pacientes, así como para conocer el criterio de los profesionales de la APS en relación con el papel que desempeñan en el proceso de referencia de pacientes y el apoyo que reciben (para su gestión) de las otras unidades con las cuales interrelacionan, se realizó un estudio descriptivo de índole retrospectiva en el área de salud " Plaza de la Revolución" en la Ciudad de La Habana. A través de una encuesta se exploraron los criterios de Médicos de Familia y la población que recibe el servicio. Nuestro universo estuvo constituido por los 32 médicos que laboran en el policlínico y que atienden a la población residente en el área. El universo de destinatarios de los servicios lo constituyeron todas las familias que pertenecen a esta área de salud (5 895). Se realizó un muestreo por conglomerados bietápico con selección de consultorios con probabilidad proporcional al número de familias y selección sistemática de familias dentro del consultorio (196 en total). Se identificó que, si bien es aceptable el acceso de la población a los servicios del policlínico, la regionalización de los servicios entre los niveles primario y secundario presenta dificultades apreciables. Además se evidencia que a pesar de que la inmensa mayoría de los pacientes accedió a la atención secundaria por mecanismos colaterales, la accesibilidad de la población al sistema (en última instancia) estuvo garantizada
