ABSTRACT

In order to find the most advantageous bioactive compounds from mulberry latex for drug development in the near future, this study was conducted to characterize and evaluate antioxidant and antimicrobial properties from four different mulberry lattices (BR-2, S-1, AR-14, and S-146). The characterization of the lattices was performed by scanning electron microscopy with energy-dispersive X-ray spectroscopy, gas chromatography coupled to mass spectroscopy, and Fourier transform infrared spectroscopy. Further, screenings of the antioxidant and antimicrobial potential of selected lattices were performed in vitro using 2,2-diphenyl-1-picrylhydrazyl assay and agar well diffusion methods, respectively. Interestingly, the outcome of the current study revealed that tested mulberry lattices contain a considerable amount of bioactive phytoconstituents, particularly antimicrobial and antioxidant compounds, as revealed by chromatographic analysis. BR-2 latex was found to have significant antioxidant activity (75%) followed by S-146 (64.6%) and AR-14 (52.9%). The maximum antimicrobial activity was found in BR-2 latex compared to other tested latex varieties. The results of this investigation showed that mulberry latex from the BR-2 type may successfully control both bacterial and fungal infections, with the added benefit of having enhanced antioxidant capabilities.

ABSTRACT

Cutaneous adverse drug reactions are known side effects of first-line antitubercular therapy, which ranges from mild pruritus to life-threatening toxic epidermal necrolysis. Severe cutaneous adverse drug reactions can lead to antitubercular therapy discontinuation and further complicates tuberculosis treatment. Here we present the case of a 49-year-old obese male who developed a generalized maculopapular rash within 24 hours of initiation of therapy followed by bullae over palms in 3 days. Antitubercular therapy was immediately discontinued, and he was managed with antihistamines, intravenous fluid, and electrolyte supplementation. He was discharged on antihistamines, a short course of systemic steroids, moxifloxacin, and bedaquiline (second-line antitubercular therapy (ATT)). Proper guidelines about rechallenge therapy will enormously aid in managing cutaneous adverse drug reactions, and efficient treatment of tuberculosis in these patients, and ceasing its progression to multisystemic complications. This article aims to discuss the presentation and management of cutaneous adverse drug reactions in the setting of Nepal.

ABSTRACT

Intracranial hematoma is a common variety of brain insults in trauma. However, posterior fossa hematoma in the retroclival location is quite unusual. There are limited numbers of case reports regarding traumatic retroclival hematoma. Some are managed with surgery in this condition. We present a traumatic retroclival hematoma in a 34-year-old gentleman who sustained brain trauma in a motor vehicle accident. His condition was further complicated by hyponatremia and delayed traumatic intracerebral hematoma in a distant location. The only symptom he had later was severe headache which could be attributed to delayed traumatic intracerebral hematoma and hyponatremia. He was managed conservatively and discharged on the 12th day from the hospital.

ABSTRACT

A functional network of blood vessels is essential for organ growth and homeostasis, yet how the vasculature matures and maintains homeostasis remains elusive in live mice. By longitudinally tracking the same neonatal endothelial cells (ECs) over days to weeks, we found that capillary plexus expansion is driven by vessel regression to optimize network perfusion. Neonatal ECs rearrange positions to evenly distribute throughout the developing plexus and become positionally stable in adulthood. Upon local ablation, adult ECs survive through a plasmalemmal self-repair response, while neonatal ECs are predisposed to die. Furthermore, adult ECs reactivate migration to assist vessel repair. Global ablation reveals coordinated maintenance of the adult vascular architecture that allows for eventual network recovery. Lastly, neonatal remodeling and adult maintenance of the skin vascular plexus are orchestrated by temporally restricted, neonatal VEGFR2 signaling. Our work sheds light on fundamental mechanisms that underlie both vascular maturation and adult homeostasis in vivo.

ABSTRACT

A biocatalytic membrane offers an ideal alternative to the conventional treatment process for the removal of toxic pentachlorophenol (PCP). The limelight of the study is to utilize superparamagnetic iron oxide nanoparticles (SPIONs) incorporated (poly (methyl vinyl ether-alt-maleic acid) (PMVEAMA) and poly (ether - ether) sulfone (PEES)) membrane for immobilization of laccase and its application towards the removal of PCP. In regard to immobilization of Tramates versicolor laccase onto membranes, 5 mM glutaraldehyde with 10 h cross-linking time was employed, yielding 76.92% and 77.96% activity recovery for PEES/PMVEAMA/La and PEES/PMVEAMA/SPIONs/Lac, respectively. In the context of kinetics and stability studies, the immobilized laccase on PEES/PMVEAMA/Lac membrane outperforms the free and PEES/PMVEAMA laccases. At pH 7.0, the free enzyme loses half of its activity, while the immobilized laccases maintained more than 87% of their initial activity even after 480 min. With regard to PCP removal, the removal efficiency of immobilized laccase on the membrane was more than free enzyme. With 100 ppm of PCP, immobilized laccase on PEES/PMVEAMA/SPIONs membrane at pH 4.0 and 50 °C had a removal efficacy of 61.65% in 24 h. Furthermore, to perk up the removal of PCP, the laccase-aided system with mediators was investigated. Amongst, veratryl alcohol displayed 71.04% of PCP removal using immobilized laccase. The reusability of the laccase heightened after immobilization on PEES/PMVEAMA/SPIONs portraying 62.44% of the residual activity with 39.4% of PCP removal even after five cycles. The current investigation reveals the efficacy of the mediator-aided PEES/PMVEAMA/lac membrane system towards removing PCP from the aqueous solution, which can also be proposed for a membrane bioreactor.

ABSTRACT

Synthesis and characterization of highly active cross-linked laccase aggregates (CLLAs) were performed and evaluated for removal of pentachlorophenol and phenanthrene from lignocellulosic biorefinery wastewater. Laccase from Tramates versicolor MTCC 138 was insolubilized as CLLAs via precipitation with 70% ammonium sulphate and simultaneous cross-linking with 5 mM glutaraldehyde to obtain activity recovery of 89.1%. Compared to the free laccase, the pH and thermal stability of the prepared CLLAs were significantly higher. At a high temperature of 60 °C, free laccase had a half-life of 0.25 h, while CLLAs had a half-life of 6.2 h. In biorefinery wastewater (pH 7.0), the free and CLLAs were stored for 3 day at a temperature of 30 °C. Free laccase completely lost their initial activity after 60 h; however, the CLLAs retained 39% activity till 72 h. Due to its excellent stability, free laccase and CLLAs were assessed for removing pentachlorophenol and phenanthrene in wastewater. CLLAs could remove 51-58% of pentachlorophenol (PCP) and phenanthrene (PHE) in 24 h. Biosurfactants, including surfactin, sophorolipid, and rhamnolipid, were assessed for their aptitude to improve the removal of organic contaminants in wastewater. Biorefinery wastewater incubated with all surfactants enhanced PCP and PHE removal compared to the no-surfactant controls. Further, 1 µM rhamnolipid significantly amplified pentachlorophenol and phenanthrene removal to 81-93% for free laccase and CLLAs, respectively.

ABSTRACT

Hydatid cyst is an uncommon parasitic disease caused by larval stages of Echinococcus granulosus. The liver is the most frequently affected organ followed by the lungs and the spleen. Intracranial hydatid cysts are uncommon and occur mostly in supratentorial region. It can present with nonspecific symptoms and can be difficult to diagnose, thus regardless of unusual clinical presentation and unusual location of cystic lesion in brain, it is crucial to keep hydatid cyst as one of the differentials. We describe a case of a 28-year-old male who presented with headache, vomiting and cerebellar signs. MRI showed multiple cystic lesions in posterior fossa with asymmetrically dilated posterior horn of left lateral ventricle. Biopsy from one of the cystic lesions from posterior fossa was performed which confirmed the diagnosis of hydatid cyst. Patient was started on Albendazole and subsequently planned for surgery.

ABSTRACT

Background The coronavirus disease 2019 (COVID-19) pandemic strained the already weak health system of Nepal, especially during the surge of the delta variant. A telephonic consultation service was rapidly established to provide free consultations to assist those in home isolation due to severe acute respiratory syndrome coronavirus 2 infection. In this study, we describe the process of establishing the hotline and share preliminary findings. During the peak of the delta wave in Nepal, the hotline was started by a local non-profit organization. Methodology We established the hotline with help of a private telecommunication company. The hotline was advertised on social media, radio, and newspapers. Healthcare workers were recruited and trained and the service was provided for free. Patient data were recorded and de-identified for analysis, monitoring, and evaluation. Results The majority of the callers were from Kathmandu valley, which includes three districts, Kathmandu, Lalitpur, and Bhaktapur. Overall, 44% of the callers inquired about the clinical manifestations of COVID-19. On average, there were 75 calls each day between May 2021 and February 2022. The average call duration was three minutes and 42 seconds. Trained healthcare workers answered the calls for 15.5 hours a day. Conclusions Our work established the feasibility of a rapid hotline service in response to the pandemic causing high strain on the health system. Lessons learned from this experience can be useful for future disasters in Nepal and other places with similar health system strains.

ABSTRACT

The novelty of this study deals with the biocatalytic treatment of trace organic contaminants (TrOCs) from municipal wastewater by insolubilized laccase. Laccase from Trametes versicolor was aggregated by three-phase partitioning technique followed by cross-linking with glutaraldehyde to produce insolubilized laccase as cross-linked enzyme aggregates (CLEAs). The optimal conditions for CLEAs preparation include ammonium sulphate concentration of 83% (w/v), crude to t-butanol ratio of 1.00: 1.05 (v/v), pH 5.3, and glutaraldehyde concentration of 20 mM obtained via statistical design. The efficiency of insolubilization of the CLEAs laccase based on the kcat/km ratio was approximately 4.8-fold greater than that of free laccase. The developed CLEAs showed greater resistance to product inhibition mediated by ABTS than the free enzyme and exhibited excellent catalytic activity even after the tenth successive cycle. Further, free laccase and the synthesized CLEAs laccase were utilized to treat five analgesics, two NSAIDS, three antibiotics, two antilipemics, and three pesticides in the municipal wastewater. Under the batch process with operating conditions of pH 7.0 and 20 °C, 1000 U/L of CLEAs, laccase removed 11 TrOCs in the range of about 20-99%. However, the inactivated CLEAs only adsorbed 2-25% of TrOCs. It was observed that acetaminophen, mefenamic acid, trimethoprim, and metolachlor depicted almost complete removal with CLEAs laccase. The performance of CLEAs laccase in a perfusion basket reactor was tested for the removal of TrOCs from municipal wastewater.

ABSTRACT

This study explains the use of a ultrafiltration membrane made of polyvinyl pyrrolidone (PVP) and poly(ether ether sulfone) (PEES)/Nano-titania (n-TiO2) for the separation of organic compounds. The results of the tests for porosity, water content, surface chemistry, membrane morphology, and contact angle demonstrated that the developed membranes have more hydrophilicity than PEES membranes due to the redundant hydrophilic nature of PVP and n-TiO2. The membrane pure water flux, which contains 5 wt% PVP and 1.5 wt% n-TiO2, was 312.76 Lm-2h-1, about three-fold higher than that of pristine membrane (95.71 Lm-2h-1). Employing bovine serum albumin as a model foulant, the fouling resistance of the PEES/PVP/n-TiO2 membrane was examined. According to the analysis of flux recovery ratio and irreversible resistance, modified membranes were less likely to foul, and the PEES/n-TiO2 membrane with 5% PVP addition was recommended as optimal. The fabricated membranes effectively removed more than 95% of various organic compounds such as humic acid, safranin O, egg albumin, pepsin, and trypsin from aqueous solution. Permeability of safranin O and humic acid of PEES/PVP/n-TiO2 membranes was about 118 Lm-2h-1 and 138 Lm-2h-1, respectively.
