ABSTRACT

The objectives were to assess the self-efficacy and consistent condom use by people living with HIV (PLHIV). A cross-sectional, comparative study was carried out in outpatient clinics in Ceará State, Brazil, with a sample of 190 PLHIV, 95 serodiscordant and 95 seroconcordant. Interviews were conducted using the Socio-Demographic, Clinical, Epidemiological and Vulnerability Form and the Condom Use Self-Efficacy Scale. Descriptive analysis, associations between variables, odds ratio and 95% confidence interval were determined. P < 0.05 was considered statistically significant. Of the sample, 43.1% consistently used condoms (50.5% serodiscordant and 35.7% seroconcordant). Serodiscordant PLHIV without guidance on HIV prevention (P = 0.027) and without access to testing (P = 0.002) had lower self-efficacy and 11.5 times more chances for inconsistent condom use (P = 0.006), while those satisfied with follow-up in health were less likely to use condoms inconsistently (P = 0.011). We conclude that there is low consistent use of condoms among PLHIV, which increases the risk of HIV transmission and the acquisition of other sexually transmitted infections. Consistent condom use was greater among serodiscordant individuals, although there was no difference in self-efficacy in condom use between the groups.

ABSTRACT

PURPOSE: To verify the effectiveness of low-frequency transcutaneous electrical nerve stimulation (TENS) simultaneously applied to the performance of vocal exercises in women with behavioral dysphonia. METHODS: This is a randomized, blinded clinical trial (Register Number: RBR-5k95vs). Twelve adult women with vocal nodules, randomly divided into three groups (G1: 4 participants - 12 sessions - application of placebo TENS simultaneously to the execution of vocal exercises; G2: 5 participants - 12 sessions - application of low-frequency TENS (frequency at 10 Hz, 200 µs duration phase, motor threshold, with electrodes placed on the thyroid cartilage lamina, bilaterally); and G3: 3 participants - 12 sessions - application of low-frequency TENS (same condition as G2) simultaneously to the execution of vocal exercises), participated in this study. The therapies were performed for 30 minutes in each session, twice a week. The participants were evaluated regarding vocal quality through acoustic voice analysis (fundamental frequency, Cesptral Peak Prominence-Smoothed (CPPs), alpha ratio, L1-L0, Acoustic Breathiness Index (ABI), and Acoustic Vocal Quality Index (AVQI)), vocal economy through electroglottography, and vocal self-assessment using the Voice-Related Quality of Life (V-RQOL) protocol. Assessments were performed before and immediately after voice therapy. Data were analyzed using the two-way repeated-measures ANOVA (variance analysis) test to compare assessment times and intervention groups. RESULTS: It was observed that G2 presented a reduction in the ABI acoustic parameter after the intervention and an increase in the values of the CPPs and L1-L0 parameter and in the scores of the physical and total V-RQOL domains. There were no differences for the other outcomes in relation to time and group. CONCLUSION: Preliminary results indicate that low-frequency TENS applied alone can reduce ABI parameter values and improve voice-related quality of life in dysphonic women.

ABSTRACT

OBJECTIVE: To map the volitional and non-volitional devices used by speech and language pathologists (SLPs) in voice training and therapy and characterize their use in research on voice interventions. METHODS: This scoping review is the first part of a larger study. The electronic search was carried out by mapping the references in PubMed/Medline, LILACS/BVS, Scopus, Web of Science, EMBASE, and the Cochrane Library, and the manual search was carried out in the grey literature. Two blind independent reviewers selected and extracted data; divergences were solved by consensus. The data extracted in this part of the study were the authorship and year of publication, country, study design, sample characteristics, intervention modality, ingredient, target, mechanism of action, dosage, and outcome measures. They were addressed with descriptive analysis. RESULTS: Publications that use devices as ingredients are mostly from the last two decades, mainly carried out in the United States of America and Brazil, in adults of both sexes with behavioral dysphonia. Forty-two types of devices were used, many of them with similar approaches but different nomenclatures. Most devices were used voluntarily, focusing on vocal function, and aiming to increase source and filter interaction. Most studies used silicone tubes. The most reported technical specification to apply the ingredient was surface electrodes on the neck. Device dosage was time-controlled, and the most used outcomes were self-assessment and acoustic analysis. CONCLUSION: Devices are currently used as ingredients in vocal interventions, with a greater focus on increasing the source and filter interaction, associated with silicone tubes (the most used devices in these studies), which have been dosed with performance time. Outcomes were measured with self-assessment instruments.

ABSTRACT

OBJECTIVE: To analyze the determining factors for using devices in vocal interventions and characterize their use by Brazilian speech-language pathologists (SLPs). METHODS: This cross-sectional observational study had a sample of 148 SLPs with clinical practice in voice. They answered an online questionnaire via Google Forms about sociodemographic data, training, work in the area, and the use of devices in vocal interventions. Data were analyzed descriptively and inferentially. RESULTS: Tubes, straws, and masks were the most commonly used devices. SLPs specializing in voice are more inclined to use thermotherapy and kinesio tapings while being less inclined to use therapeutic ultrasounds and nebulizers. Voice specialists are less likely to employ electrostimulation. The choice to use photobiomodulation and auditory monitoring devices is influenced by the years of clinical experience, whereas the speech-language therapy training duration affects the use of electrostimulation. The age of the professional also plays a role in the utilization of vibratory stimulation. Vibratory stimulation, auditory monitoring devices, thermotherapy, and nebulization are more frequently utilized among individuals who rely on their voices for occupational purposes, whereas electrostimulation is less common. The use of photobiomodulation is infrequent in children; vibratory stimulation is more common in adolescents, and thermotherapy is relatively common among older individuals. Most of these devices are typically prescribed in execution time during vocal intervention. CONCLUSION: The specialization, the time since graduation and in the occupation, and the target population of the service are the determining factors for the use of devices. They are used in vocal therapy and training, targeting vocal function.

Introdução: O sono é uma função vital e essencial para a sobrevivência humana e tem por objetivo favorecer o reparo corporal e mental. Objetivo: Investigar os fatores associados à autopercepção sobre a qualidade do sono, bem como possíveis alterações das estruturas orofaciais e funções estomatognáticas em indivíduos com queixa de ronco. Material e Método: Trata-se de estudo observacional, analítico e transversal, realizado com 30 indivíduos adultos com queixa de ronco durante o sono, com idade média de 45 anos e 5 meses. Para a avaliação de motricidade oromiofuncional foi utilizado o protocolo AMIOFE. Foram aplicados questionários para investigação do ronco e qualidade de sono dos participantes: Questionário de Berlin, Escala de Sonolência de Epworth e Questionário de qualidade de sono de Pittsburgh. Resultados: Os resultados do questionário de Pittsburgh indicaram disfunção na qualidade do sono em 66,67% dos participantes. A sonolência diurna foi evidenciada em 33,33%, por meio da Escala de Epworth. 60% dos participantes apresentaram alto risco para AOS, por meio dos escores obtidos no instrumento de Berlin. Indivíduos que não praticam exercício físico e com posição habitual de língua no assoalho bucal apresentaram maiores escores no questionário de Berlin. Houve correlação positiva estatisticamente significante e moderada entre as variáveis Índice de massa corporal (IMC) e os escores do Questionário de Berlin. Conclusão: Indivíduos com queixa de ronco apresentam dificuldades relacionadas à qualidade do sono. A autopercepção negativa de qualidade do sono teve relação com dados de motricidade orofacial, hábitos alimentares, composição corporal e hábitos de estilo de vida. (AU)


Introduction: Sleep is a vital and essential function for human survival and aims to promote bodily and mental repair. Objective: To investigate factors associated with self-perception of sleep quality, as well as possible alterations in orofacial structures and stomatognathic functions in individuals with snoring complaints. Material and Method: Observational, analytical and cross-sectional study, conducted with 30 adult individuals complaining of snoring during sleep, with a mean age of 45 years and 5 months. For the evaluation of orofacial myofunctional motricity, the (AMIOFE) was used. Questionnaires were applied to investigate the snoring and sleep quality of the participants: Berlin Questionnaire, Epworth Sleepiness Scale and Pittsburg Sleep Quality Index Questionnaire. Results: The Pittsburgh questionnaire results indicated sleep quality dysfunction in 66.67% of participants. Daytime sleepiness was evidenced in 33.33%, using the Epworth Scale. 60% of the participants presented high risk for obstructive sleep apnea syndrome (OSA), through the scores obtained in the Berlin questionnaire. Individuals who do not practice physical exercise and with usual tongue position on the oral floor presented higher scores in the Berlin questionnaire. There was a statistically significant and moderate positive correlation between the variables Body Mass Index (BMI) and the Berlin questionnaire scores. Conclusion: Individuals complaining of snoring self-reported difficulties related to sleep quality. The sleep quality difficulties self-reported were related to data on orofacial motricity, eating habits, body composition and lifestyle habits. (AU)

Objetivo: Investigar los factores asociados con la autopercepción de la calidad del sueño, así como posibles alteraciones en las estructuras orofaciales y funciones estomatognáticas en individuos con quejas de ronquidos. Metodo: Se trata de estudio observacional, analítico, transversal, realizado con 30 individuos adultos con queja de ronquidos durante el sueño y edad media de 45 años y 5 meses. Para la evaluación de la motricidad oromiofuncional se utilizó el protocolo (AMIOFE/OMES). Se aplicaron cuestionarios para investigar el ronquido y la calidad de sueño de los participantes: Cuestionario de Berlín; Escala de Somnolencia de Epworth; Cuestionario de Calidad del Sueño de Pittsburgh. Resultados: Los resultados del cuestionario de Pittsburgh indicaron disfunción en la calidad del sueño en 66,67%. La Escala de Epworth evidenció somnolencia diurna en 33,33%. El 60% presentaron alto riesgo de AOS, conforme las puntuaciones obtenidas en el instrumento de Berlín. Los que no practican ejercicio físico y que tienen la posición habitual de la lengua en el piso de la boca obtuvieron puntuaciones más altas en el cuestionario de Berlín. Hubo correlación positiva estadísticamente significativa y moderada entre las variables del IMC y las puntuaciones del Cuestionario de Berlín. Conclusión: Individuos con quejas de ronquidos refieren dificultades relacionadas con la calidad del sueño. Las dificultades estuvieron relacionadas con datos de motricidad orofacial, hábitos alimentarios, composición corporal y los hábitos de estilo de vida. (AU)

OBJECTIVES: To identify the presence of vocal fatigue and dysphonia risk in evangelical pastors and to verify associations of vocal fatigue with dysphonia risk and other factors. STUDY DESIGN: Cross-sectional observational study. METHODS: Ninety-two evangelical pastors, professionally active, answered an on-line questionnaire divided in three steps. The first step consisted of data for the characterization of the sample. In the second stage, participants answered the vocal fatigue index questionnaire (VFI). Finally, they answered to the Brazilian Dysphonia Screening tool. A descriptive analysis of the data was performed along with Spearman's Correlation test between VFI factors and the three possible decision rules for the Brazilian Dysphonia Screening tool. A linear regression model was designed to verify predictive variables associated with the outcome "vocal fatigue". RESULTS: Ninety male individuals were included. The average sample was off the threshold scores of tiredness and voice impairment, physical discomfort, improvement of voice symptoms with rest, and the total score of VFI. The dysphonia risk was low and was weakly negatively correlated with the improvement of voice symptoms with rest. The linear regression indicated that the presence of a current vocal complaint increases about four points in the VFI. CONCLUSION: Evangelical pastors present symptomatology of vocal fatigue and low risk for dysphonia. However, the greater the improvement of voice symptoms with rest, the lower the risk for dysphonia. Furthermore, the presence of a current vocal complaint is associated with increased symptoms of vocal fatigue.

ABSTRACT

Current therapeutic ways adopted for the treatment of leishmaniasis are toxic and expensive including parasite resistance is a growing problem. Given this scenario, it is urgent to explore treatment alternatives for leishmaniasis. The aim of this study was to evaluate the effect of 3-phenyl-lawsone (3-PL) naphthoquinone on Leishmania (Viannia) braziliensis infection, both in vitro and in vivo, using two local routes of administration: subcutaneous (higher dose) and tattoo (lower dose). In vitro 3-PL showed low toxicity for macrophages (CC50 >3200 µM/48h) and activity against intracellular amastigotes (IC50 = 193 ± 19 µM/48h) and promastigotes (IC50 = 116 ± 26 µM/72h), in which induced increased ROS generation. Additionally, 3-PL up-regulated the production of cytokines such as tumor necrosis factor alpha (TNF-α), monocyte chemotactic protein 1 (MCP-1), interleukin-6 (IL-6) and IL-10 in infected macrophages. However, the anti-amastigote action was independent of nitric oxide production. Treatment of hamsters infected with L. (V.) braziliensis from one week after infection with 3-PL by subcutaneous (25 µg/Kg) or tattooing (2.5 µg/Kg) route, during 3 weeks (3 times/week) or 2 weeks (2 times/week) significantly decreased the parasite load (p<0.001) in the lesion. The reduction of parasite load by 3-PL treatment was comparable to reference drug meglumine antimoniate administered by the same routes (subcutaneous 1mg/Kg and tattoo 0.1mg/Kg). In addition, treatment started from five weeks after infection with 3-PL per tattoo also decreased the parasite load. These results show the anti-leishmanial effect of 3-PL against L. (V.) braziliensis and its efficacy by subcutaneous (higher dose) and tattoo (lower dose) routes. In addition, this study shows that drug delivery by tattooing the lesion allows the use of lower doses than the conventional subcutaneous route, which may support the development of a new therapeutic strategy that can be adopted for leishmaniasis.

OBJECTIVE: To investigate the self-perception of vocal fatigue symptoms and musculoskeletal pain in home office workers before and during the coronavirus disease (COVID-19) pandemic. MATERIALS AND METHODS: A total of 424 individuals participated in this cross-sectional, observational, and descriptive study; they were stratified into the experimental group (EG), consisting of 235 individuals working from home office during the COVID-19 pandemic; and the control group (CG), with 189 individuals who continued to work in person during this period. All participants answered the vocal fatigue index and the musculoskeletal pain investigation questionnaires. The data were analyzed in a descriptive and inferential manner. RESULTS: Participants in the EG reported more vocal fatigue symptoms and musculoskeletal pain than those in the CG before the pandemic. However, during the pandemic, the EG presented a higher frequency of pain in the posterior of the neck, shoulder, upper back, and temporal and masseter muscles, while the CG presented a higher frequency of pain in the larynx. With regard to vocal fatigue, during the pandemic, EG had an increase in scores to factors such as tiredness and voice impairment, avoidance of voice use, and total scores. No such differences were noted in the CG. CONCLUSION: Workers who migrated to home offices during the COVID-19 pandemic are at risk of developing vocal disorders.

Resumo Objetivo Analisar as produções científicas acerca da eficácia de intervenções utilizando a entrevista motivacional para adesão à terapia antirretroviral por pessoas com o vírus da imunodeficiência humana. Métodos Revisão sistemática realizada em quatro bases de dados, o MEDLINE, CINAHL, IBECS, LILACS, e em uma biblioteca eletrônica, a SciELO, sem restrição de idioma, data e tamanho amostral. O levantamento de artigos foi realizado em setembro de 2021, utilizando-se os descritores Motivational Interviewing, HIV, Acquired Immunodeficiency Syndrome e Antiretroviral Therapy, Highly Active. Foram incluídos artigos do tipo ensaio clínico randomizado, com amostra de pessoas com vírus da imunodeficiência humana maiores de 18 anos, e excluídas pesquisas envolvendo crianças, adolescentes e gestantes. Dez artigos foram selecionados e analisados quanto ao rigor e características de cada estudo. Resultados A entrevista motivacional foi aplicada presencialmente e associada à chamada telefônica, visita domiciliar, fitas de áudio, encartes informativos, aconselhamento, teorias cognitivas-comportamentais e cognitivas-sociais. Os estudos incluídos nesta revisão evidenciaram que as intervenções utilizando a entrevista motivacional aumentaram a adesão aos antirretrovirais por pessoas com vírus da imunodeficiência humana. Conclusão O estudo contribuiu para identificar os dados existentes sobre a eficácia de intervenções com a entrevista motivacional, com foco na adesão à terapia antirretroviral por pessoas com vírus da imunodeficiência humana, tornando visíveis os pontos que precisam ser aprofundados e mostrando a importância desta estratégia, que pode ser utilizada pelos enfermeiros e demais profissionais de saúde, visando o bem-estar dos pacientes. Se incluyeron artículos tipo ensayo clínico aleatorizado, con muestreo de personas con virus de la inmunodeficiencia humana mayores de 18 años; y se excluyeron estudios que incluían niños, adolescentes y mujeres embarazadas. Se seleccionaron diez artículos y se analizó el rigor y características de cada estudio. Resultados La entrevista motivacional se realizó presencialmente y estuvo relacionada con llamadas telefónicas, visitas domiciliares, cintas de audio, suplementos informativos, asesoramiento, teorías cognitivas conductuales y cognitivas sociales. Los estudios incluidos en esta revisión evidenciaron que las intervenciones que utilizan la entrevista motivacional aumentaron la adhesión a los antirretrovirales de personas con el virus de la inmunodeficiencia humana. Conclusión El estudio ayudó a identificar los datos existentes sobre la eficacia de intervenciones con entrevistas motivacionales, con énfasis en la adhesión al tratamiento antirretroviral de personas con el virus de la inmunodeficiencia humana, se visibilizaron los puntos en los que es necesario profundizar y se mostró la importancia de esta estrategia, que puede ser utilizada por enfermeros y demás profesionales de la salud, para el bienestar de los pacientes.


Abstract Objective To analyze scientific productions about the effectiveness of interventions using motivational interviewing for adherence to antiretroviral therapy by people with the human immunodeficiency virus. Methods This is a systematic review carried out in four databases, MEDLINE, CINAHL, IBECS, LILACS, and in an electronic library, SciELO, without language, date and sample size restrictions. The survey of articles was carried out in September 2021, using the descriptors Motivational Interviewing, HIV, Acquired Immunodeficiency Syndrome and Antiretroviral Therapy, Highly Active. Randomized clinical trial articles were included, with a sample of people with human immunodeficiency virus over 18 years old, and research involving children, adolescents and pregnant women was excluded. Ten articles were selected and analyzed regarding the rigor and characteristics of each study. Results Motivational interviewing was applied in person and associated with a telephone call, home visit, audio tapes, informational inserts, counseling, cognitive-behavioral and cognitive-social theories. The studies included in this review showed that interventions using motivational interviewing increased adherence to antiretrovirals by people with human immunodeficiency virus. Conclusion The study contributed to identify existing data on the effectiveness of interventions with motivational interviewing, focusing on adherence to antiretroviral therapy by people with human immunodeficiency virus, making visible the points that need to be deepened and showing the importance of this strategy, which can be used by nurses and other health professionals, aiming at patients' well-being.International Prospective Register Systematic Reviews: CRD42019123724

Resumo Objetivo Avaliar a efetividade de uma cartilha educativa para promoção do estilo de vida saudável e adesão aos antirretrovirais em pessoas com HIV. Métodos Ensaio clínico randomizado em ambulatório de Fortaleza, Ceará, Brasil, com 70 pacientes no grupo intervenção e 74 no controle. Critérios de inclusão: pessoas com HIV maiores de 18 anos, de ambos os sexos, em terapia antirretroviral por mais de seis meses, ter telefone fixo ou celular, e 35 ou mais pontos na Escala de Letramento em Saúde. A coleta de dados ocorreu em quatro momentos, utilizando-se a Escala de Perfil do Estilo de Vida Individual e o Questionário para Avaliação da Adesão ao Tratamento Antirretroviral. O grupo controle recebeu a consulta médica de rotina, e o grupo intervenção, além da consulta de rotina, realizou a leitura da cartilha em consultório, e depois levou-a para o domicílio. Na estatística, utilizou-se análise de variância não paramétrica de medidas repetidas. Resultados A maioria dos participantes tinha estilo de vida insatisfatório na avaliação basal. A cartilha melhorou o estilo de vida dos pacientes após dois (P<0,001), quatro (P<0,001) e seis meses (P<0,001) da intervenção, quando comparados os diferentes tempos ao valor basal e ao controle. Houve melhora dos escores de adesão aos antirretrovirais no grupo intervenção quando comparado à linha de base (P<0,001) e ao controle (P<0,001). Conclusão A cartilha foi efetiva para promover estilo de vida saudável e melhorar a adesão aos antirretrovirais em pessoas com HIV, portanto, pode complementar as consultas ambulatoriais. Resumen Objetivo Evaluar la efectividad de una cartilla educativa para la promoción del estilo de vida saludable y adhesión a los antirretrovirales para personas con VIH. Métodos Ensayo clínico aleatorizado en consultorios externos de Fortaleza, estado de Ceará, Brasil, con 70 pacientes en el grupo experimental y 74 en el de control. Criterios de inclusión: personas con VIH mayores de 18 años, de ambos sexos, en tratamiento antirretroviral por más de seis meses, con teléfono fijo o celular y con 35 puntos o más en la Escala de Alfabetización en Salud. La recolección de datos se realizó en cuatro momentos, mediante la Escala de Perfil del Estilo de Vida y el Cuestionario para la Evaluación de la Adhesión al Tratamiento Antirretroviral. El grupo de control recibió una consulta médica de rutina, y el grupo experimental, además de la consulta de rutina, leyó la cartilla en el consultorio y después se la llevó a su casa. En la estadística, se usó el análisis de varianza no paramétrico de medidas repetidas. Resultados La mayoría de los participantes tenía un estilo de vida insatisfactorio en la evaluación de base. La cartilla mejoró el estilo de vida de los pacientes después de dos (P<0,001), cuatro (P<0,001) y seis meses (P<0,001) de la intervención, si se lo compara con los diferentes tiempos al valor de base y al control. Se observó una mejora de la puntuación de adhesión a los antirretrovirales en el grupo experimental si se lo compara con la línea de base (P<0,001) y con el control (P<0,001). Conclusión La cartilla fue efectiva para promover un estilo de vida saludable y mejorar la adhesión a los antirretrovirales en personas con VIH; por lo tanto, puede complementar las consultas de los consultorios externos.


Abstract Objective To assess the effectiveness of an educational booklet to promote healthy lifestyle and antiretroviral compliance in people with HIV. Methods This is a randomized clinical trial at an outpatient clinic in Fortaleza, Ceará, Brazil, with 70 patients in the intervention group and 74 in the control group. People with HIV over 18 years of age, of both sexes, on antiretroviral therapy for more than six months, with a landline or cell phone, and 35 or more points on the Health Literacy Scale were included. Data collection took place in four moments, using the Individual Lifestyle Profile Scale and the Assessment of Antiretroviral Therapy Compliance Questionnaire (Cuestionario para la Evaluación de la Adhesión al Tratamiento Antirretroviral). The control group received routine medical consultation, and the intervention group, in addition to routine consultation, read the booklet in the office, and then took it home. Non-parametric analysis of variance of repeated measures was used for statistics. Results Most participants had an unsatisfactory lifestyle at baseline. The booklet improved patients' lifestyle after two (P<0.001), four (P<0.001) and six months (P<0.001) of intervention, when comparing the different times to baseline and control. There was an improvement in antiretroviral compliance scores in the intervention group when compared to baseline (P<0.001) and control (P<0.001). Conclusion The booklet was effective in promoting a healthy lifestyle and improving antiretroviral compliance in people with HIV, therefore, it can complement outpatient consultations. ABSTRACT

Objective: to compare the immediate effects of low-frequency TENS employment on vocal quality in women with behavioral dysphonia before and after vocal exercises.Methodology: 30 women (mean = 31.3 years old), diagnosed with behavioral dysphonia received low-frequency TENS before (TENS + VE Group) and after vocal exercises (VE + TENS Group) with a 1-week washout. They had their sustained vowel/a/and running speech recorded before and after each procedure for auditory-perceptual analysis and acoustic measures. The low-frequency TENS parameters applied were symmetrical biphasic quadratic pulse, 200 µs phase, 10 Hz frequency, intensity on the motor threshold, and the electrodes were positioned on the submandibular and superior fibers of the trapezius muscle region. The vocal exercises: tongue trill, humming, finger kazoo, and water resistance therapy were performed totalizing 20 min.Results: intragroup analysis of sustained vowel/a/showed reduction in both groups of strain parameter and increased the breathiness; only VE + TENS Group increased the instability parameter, decreased fundamental frequency, and increased in SPI values; the running speech analysis showed an increase in the overall degree, roughness, and breathiness parameters. However, in VE + TENS Group, there was a statistically significant decrease in the intensity of the strain and an increase in breathiness. The acoustic measures showed that VE + TENS Group had a higher variation than TENS + VE Group regarding NHR.Conclusion: vocal exercises followed by low-frequency TENS have more immediate positive effects on voice quality than the low-frequency TENS followed by vocal exercises. This is a preliminary immediate effects study, and these effects could be verified through long-term assessments.

ABSTRACT

PURPOSE: To investigate the effectiveness of vocal therapy with the use of low-frequency transcutaneous electrical nerve stimulation (TENS) followed by voice exercises on vocal fold lesion size, vocal quality and quality of life in dysphonic women. METHODS: 27 women with vocal nodules participated, randomized into to: experimental group (EG)-13 women who received vocal therapy with 12 sessions of 20 min of TENS application (pulse:200µs, frequency:10Hz, motor threshold intensity, electrodes positioned in the trapezius muscle [descending fibers and submandibular region, bilaterally]). Each TENS session was followed by 30 min of vocal exercises; and the Control Group (CG)- 14 women who received 12 sessions with 20 min of application of placebo TENS (same conditions EG, but without receiving the stimulus electric), followed by 30 min of vocal exercise. Before, immediately after and one month after vocal therapy, participants underwent vocal recording for acoustic analysis, vocal self-assessment, laryngological examination and answered voice-related quality of life (V-RQOL) protocol. RESULTS: There was reduction in the size of vocal fold lesions only in the EG, immediately after treatment and one month after treatment. Acoustic analysis showed decreases in SPI values immediately after and one month after treatment in both groups. There was improvement in voice self-perception in both groups after treatment and one month after, but no significant difference in V-RQOL values. CONCLUSION: TENS followed by vocal exercises produced results similar to vocal therapy without TENS regarding voice quality, self-perception and quality of life in voice. However, vocal therapy with low-frequency TENS followed by vocal exercise was effective in reducing vocal fold lesion size in dysphonic women.

ABSTRACT

Introdução: Doenças exantemáticas (DE) são patologias infecciosas agudas, que têm como característica principal a manifestação dermatológica. Entre elas, sarampo e rubéola são de notificação compulsória. A permanência da circulação destes vírus no mundo tem causado surtos em diversas regiões, como em SC, embora sejam doenças evitáveis por meio da imunização. Métodos: Trata-se de um estudo de delineamento ecológico, constituído de casos notificados por DE em SC no período de 2009 a 2019. Os dados foram coletados no Sistema Nacional de Agravos e Notificações (SINAN/DATASUS). Resultados: Foram notificados 190 casos de DE, 41,57% na faixa etária de 20 a 29 anos, 55,91% masculinos, 88,70% brancos e 2,15% gestantes. Houve confirmação laboratorial em 85,16%, e 72,72% foram causados pelo vírus sarampo selvagem. Exantema predominou em novembro em 46,77%, e febre em outubro, em 41,93%. Tosse esteve presente em 89,24%, coriza em 67,20% e conjuntivite em 50,53%. A macrorregião com maior média de cobertura vacinal foi a Grande Oeste, com 86,85%, e a menor, a Grande Florianópolis, com 72,07%. A maior incidência de sarampo ocorreu em 2019 no Nordeste e Planalto Norte com 11,34, e a de rubéola 0,13 no Grande Oeste no mesmo ano. Foi realizado bloqueio vacinal em 95,34% dos casos, e 64,28% foram autóctones. Conclusão: Observou-se que as DE acometem indistintamente ambos os sexos, etnias e idades, sendo a vacinação a forma mais eficaz de prevenir surtos. A reemergência ocorrida no estado foi consequência das baixas taxas de cobertura vacinal, apontando para a necessidade de melhorias nas ações de imunização.


Introduction: Exanthematous diseases (ED) are acute infectious pathologies that present mainly dermatological manifestations. Among them, measles and rubella demand compulsory notification. The continued circulation of these viruses has caused outbreaks in several regions, such as Santa Catarina, although they are preventable diseases through immunization. Methods: This is a study of ecological design consisting of cases notified of ED in SC from 2009 to 2019. Data collection from the National System of Diseases and Notifications (SINAN/DATASUS). Results: 190 cases of ED were notified, 41.57% in the age group 20 to 29 years, 55.91% male, 88.70% white, and 2.15% pregnant women. Laboratory confirmation occurred in 85.16%, and the wild measles virus was the cause in 72.72%. Exanthema predominated in November with 46.77% and fever in October with 41.93%. Cough was present in 89.24%, coryza in 67.20%, and conjunctivitis in 50.53%. The macro-region with the highest average vaccination coverage was the great west, 86.85%, and the lowest was greater Florianópolis, 72.07%. The highest incidence of measles occurred in 2019. Northeast and North Plateau scored 11.34, and rubella at 0.13 in the Great West in the same year. Vaccine block occurred in 95.34% of cases, and 64.28% were autochthonous. Conclusions: The study observed that EDs affect both sexes, ethnicities, and ages indistinctly, and vaccination is the most effective way to prevent outbreaks. The re-emergence that occurred in the state was a consequence of low rates of vaccination coverage, pointing to the need for improvements in immunization activities

ABSTRACT

OBJECTIVES: to assess the lifestyle and adherence to antiretrovirals in people living with HIV (PLHIV) in the COVID-19 pandemic. METHODS: a cross-sectional study, through telephone interview to 150 patients, using a sociodemographic, epidemiological and clinical form, and questionnaires to assess lifestyle profile and adherence to antiretrovirals. Statistics analysis used Fisher's exact test, odds ratio and 95% confidence interval. RESULTS: most patients had a satisfactory lifestyle (121; 80.7%) and adequate adherence to antiretrovirals (133; 88.7%). All were in social isolation, without follow-up appointments, with access to the health service only to receive antiretrovirals, and 16 (10.7%) had COVID-19 infection. Evangelicals (p=0.002), Spiritists (p=0.045), patients using atazanavir (p=0.0001) and ritonavir (p=0.002) had a more unsatisfactory lifestyle. Adherence to antiretrovirals was more inadequate in female patients (p=0.009), with two (p=0.004) and three or more children (p=0.006), retired (p=0.029), with serodiscordant partner (p=0.046) and diagnosis time of 5 to 10 years (p=0.027). CONCLUSIONS: the most PLHIV had a satisfactory lifestyle and adequate adherence to antiretrovirals, but some groups needed intervention to improve medication adherence and lifestyle.

ABSTRACT

OBJECTIVE: To determine the prevalence of testing and COVID-19 among nurses during the pandemic in the State of Ceará. METHOD: A cross-sectional study with 379 nurses, through a network sampling technique, using a sociodemographic, labor, and clinical questionnaire. The study performed a descriptive statistics, univariate and multivariate logistic regression analysis. RESULTS: The prevalence of testing and COVID-19 were, respectively, 63.3% and 25.0%. The most common symptoms were anosmia, ageusia, and myalgia. There was inadequate use of personal protective equipment due to material shortage. The odds ratio for COVID-19 was higher in those with children, people with diabetes, from the capital, with more than two jobs, in hospital and emergency room, and from the frontline. In the multivariate logistic regression, nurses with children (p=0.011), diabetics (p=0.018) and frontline (p<0.001) had more chances for COVID-19. CONCLUSION: Expanded testing, ongoing in-service education, and adequate personal protective equipment are needed to improve nurses' work.

ABSTRACT

OBJECTIVE: To evaluate the effectiveness of an educational booklet on the knowledge, attitude and practice of a healthy lifestyle in people with HIV. METHODS: Randomized clinical trial with 75 intervention group patients using the booklet, and 75 control group patients undergoing usual service care. Data collection occurred in four moments, with a Survey pertaining Knowledge, Attitude and Practice. Nonparametric analysis of variance of repeated measures was used. RESULTS: 70 participants in the intervention group and 74 in the control group completed the study. In terms of knowledge, there was no difference between groups, the booklet impact came late, increasing after reassessments. In attitudes, there was a difference between groups in the two- and four-month reassessments compared to the baseline. In practices, there was a difference in the intervention group compared to the control group at two, four, and six months compared to baseline. CONCLUSION: The booklet was effective in improving healthy lifestyle knowledge, attitudes, and practices in people with HIV.

ABSTRACT: The aim of this study was to determine the prevalence of sleep disorders and associated factors in people living with HIV (PLWH). This was a cross-sectional study with a sample of 385 participants followed-up on an outpatient basis in Fortaleza, Ceará, Brazil. Interviews were conducted using the Sociodemographic, Epidemiological and Clinical Form for PLWH and the Pittsburgh Sleep Quality Index. Descriptive statistics and univariate and multivariate logistic regression analysis were performed, and the odds ratio (OR) and 95% confidence interval (95% CI) were calculated, considering p < .05 as statistically significant. The prevalence of sleep disorders was 43.38%. Having children ( p = .0054; OR = 1.91; 95% CI = 1.21-3.01), less than 8 years of education ( p = .0013; OR = 2.11; 95% CI = 1.34-3.34), and not engaging in regular physical exercise ( p = .0001; OR = 2.61; 95% CI = 1.61-4.23) were factors associated with the occurrence of sleep disorders. It was concluded that almost half of the sample had sleep disorders, especially those with children, low level education, and sedentary habits. These data point to the need for increased guidance on sleep hygiene, in addition to the practice of regular physical exercise for this population.

OBJECTIVES: The present study aimed to characterize and to verify the relationship between shyness and self-assessment of vocal symptoms, public speaking, and aspects of daily communication. METHODS: This is an observational, analytical, and cross-sectional study. Two hundred and sixty-four male and female participants, with a mean age of 21 years and without any vocal disorder. To measure the outcomes, the participants' scores were measured using the following self-assessment instruments: Revised Cheek and Buss Shyness Scale, Voice Symptom Scale, Self-Statements during Public Speaking Scale, and the Daily Communication Self-evaluation Questionnaire. A descriptive and inferential analysis of the data was performed. RESULTS: Shy persons had higher vocal symptomatology scores in the limitation, emotional, and total domain. Furthermore, they scored higher on the negative aspects of public speaking and lower on its positive aspects. In addition, they showed weaker loudness and higher pitch in daily communication, compared with nonshy persons. Statistically significant correlations were found between positive shyness scores and all domains of vocal symptoms, negative aspects of public speaking, and pitch in daily communication. However, negative correlation was found between the positive aspects of public speaking and loudness in daily communication. CONCLUSION: Shy persons have a higher self-evaluation with regard to the negative and less positive aspects of public speaking and most vocal symptoms, as well as weaker loudness and higher pitch in daily communication when compared to nonshy persons. The greater the perception of shyness, the greater the self-assessment with regard to daily communication and vocal symptoms and the negative aspects of public speaking.

ABSTRACT

PURPOSE: To verify the influence of work characteristics and the occupational voice use on the self-perception of vocal fatigue symptoms in individuals working in the home office during the COVID-19 pandemic. METHODS: This was a cross-sectional, observational, and descriptive study. The sample consisted of 206 individuals (123 women and 83 men), with an average age of 34 years, working exclusively in a home office mode due to the pandemic. Through an online form, all participants responded to the Vocal Fatigue Index (VFI) protocol and the questionnaire on the characterization of work and the occupational voice use during the pandemic, developed by the authors of this study. Multiple linear regression using the backward elimination technique was performed. RESULTS: The variables the interlocutor does "not listen to me in home office work," "noise in the home office work environment," "the daily workload in home office," "vocal quality worsened in home office work," "increased vocal loudness in home office work" and "lack of training about voice use in home office work" are predictors of the dependent variable vocal fatigue symptoms. CONCLUSION: The characteristics of work and occupational voice use influence the self-perception of vocal fatigue symptoms in individuals working in the home office mode during the COVID-19 pandemic.

ABSTRACT

The aim of this study was to determine the effect of lipodystrophy on self-esteem and adherence to antiretroviral therapy (ART) in people living with HIV (PLHIV). A cross-sectional and comparative study was carried out in an infection clinic, with 125 patients with lipodystrophy and 125 without lipodystrophy. Sociodemographic, clinical and epidemiological data were collected, using the Rosenberg Self-Esteem Scale and Assessment of Adherence to Antiretroviral Treatment Questionnaire (CEAT-VIH). Descriptive statistics and univariate and multivariate logistic regression analysis were used. Of the total sample, 57.2% had unsatisfactory self-esteem and 57.6% adequate adherence to ART. Self-esteem was lower in PLHIV with lipodystrophy (66.4%). PLHIV with monthly income less than or equal to two minimum wages (P < 0.001) and those with lipodystrophy had more unsatisfactory self-esteem (P < 0.001). Catholics had better self-esteem (P = 0.012), when compared to those without religion. Patients with monthly income less than or equal to two minimum wages (P = 0.021) and people with unsatisfactory self-esteem had more inadequate adherence to ART (P = 0.001). Catholics had better adherence to antiretrovirals (P = 0.007). In conclusion, lipodystrophy and low income negatively affect the self-esteem of PLHIV. Low income and unsatisfactory self-esteem make adherence to ART difficult. Religion is a protective factor for satisfactory self-esteem and adherence to antiretrovirals.
