ABSTRACT

RATIONAL & OBJECTIVE: Although the relationship between obesity, anxiety and depression is known, the clinical benefits of nutrition therapy in individuals with obesity have been scarcely studied. This study assessed the effectiveness of a traditional Brazilian diet (DieTBra) and extra virgin olive oil (EVOO) on the symptoms of anxiety and depression in severe obese participants. METHOD: This was a parallel randomized clinical trial in adults with severe obesity (Body Mass Index - BMI ≥ 35 kg/m2), DietBra Trial. Participants were randomized into three intervention groups in a ratio of 1: 1: 1 and followed up for 12 weeks. The three intervention groups were: 1) DieTBra, 2) 52 mL/day EVOO, and 3) DieTBra +52 mL/day EVOO. The symptoms of anxiety and depression were assessed by the Hospital Anxiety and Depression Scale. The outcome variables were: anxiety, depression and simultaneous anxiety and depression in the same individual. RESULTS: 149 severe obese individuals were randomized. The mean BMI was 46.3 ± 6.5 kg/m2 with a mean age of 38.9 ± 8.7 years. After randomization, individuals on medication for depression and anxiety were excluded, totaling 129 at baseline, with 44:43:42 per group. At the end of 12 weeks 113 individuals completed the study. There was a significant reduction of symptoms for the three groups: EVOO, DieTBra and DieTBra + EVOO, respectively: anxiety 38.23% (p = 0.019), 45.83% (p=<0.014), 33.33% (p < 0.013) depression 55.17% (p=<0.001), 50.00% (p=<0.007), 34.61% (p=<0.035), and anxiety/depression 57.69% (p=<0.001), 66.67% (p=<0.014), 27.78% (p=<0.013). CONCLUSIONS: Both DieTBra and olive oil interventions were effective in reducing symptoms of anxiety and depression in severe obese adults. These interventions can be integrated with clinical protocols for treatment of symptoms of anxiety and depression in severe obese individuals. TRIAL REGISTRATION: Clinicaltrials.gov NCT02463435.

ABSTRACT

BACKGROUND: Understanding the association between sarcopenia-related variables and several risk factors may help to implement interventions aimed at preventing its occurrence by reducing or controlling the identified risk factors. Although changes in body composition occur in both sexes, in women, muscle loss is accentuated due to decreased estrogen levels following menopause. This study aims to determine the factors associated with sarcopenia-related parameters in middle-aged women identified with class II/III obesity (body mass index [BMI] ≥ 35 kg/m2). METHODS: The study included 104 women with severe obesity (40.23 ± 8.49 years) with an average body fat percentage of 52.45 ± 4.14%. Sarcopenia was assessed using total appendicular skeletal muscle mass (ASMM), appendicular skeletal muscle mass index (ASMMI), and appendicular skeletal muscle mass adjusted by BMI (ASMM/BMI) as evaluated using dual energy X-ray absorptiometry (DXA). Hand grip strength (HGS) and HGS adjusted by BMI (HGS/BMI) were evaluated using dynamometry. Functional performance was assessed using the walking speed test (WS). The explanatory variables were age, lifestyle, comorbidities, food consumption, and metabolic parameters. A multivariate linear regression was performed. RESULTS: Factors associated with sarcopenia-related variables in 104 severely obese women with a mean BMI of 43.85 kg/m2 were as follows: ASMMI negatively correlated with serum levels of tetraiodothyronine (T4) and tobacco use; ASMM/BMI negatively correlated with age, serum T4 levels, and diabetes; ASMM negatively correlated with T4 serum levels and diabetes; HGS negatively correlated with age and hypercholesterolemia, and positively correlated with low-density lipoprotein cholesterol (LDL-c); HGS/BMI negatively correlated with age and hypercholesterolemia and positively correlated with LDL-c; and WS negatively correlated with hypothyroidism and diabetes. CONCLUSION: In severely obese women, muscle mass and function were inversely associated with age, smoking status, endocrine parameters, hypercholesterolemia, and comorbidities such as diabetes. Thus, the results of this investigation are relevant in supporting the development of clinical interventions to aid in the prevention of sarcopenia in adult women with severe obesity.

Rapid population aging and abdominal obesity (AO) are worldwide public health problems among the elderly and have been insufficiently investigated. This article sets out to identify the prevalence of AO among the elderly, its association with different morbidities, sociodemographic variables, and lifestyle according to sex. It is a cross-sectional study with a representative sample of elderly people resident in Goiânia, Brazil. Home visits were conducted to measure weight, height, waist circumference (WC), blood pressure, and to fill out a standardized form. The outcome variable, was calculated at WC> 102 cm for men and> 88 cm for women. Multiple Poisson regression was performed using a hierarchical model, adopting a significance level of 5%. The prevalence of AO was 55.1%, with 65.5% in women and 34.8% in men, with a significant difference (p <0.001). In men, AO was associated with age range between 70 and 74 years, the presence of respiratory diseases and high blood pressure. In women, AO continued to be associated with the presence of diabetes mellitus. The results contribute to the understanding of AO and developing preventive approaches in public health.

Nos homens a OA foi associada a faixa de idade entre 70 e 74 anos, presença de doenças do aparelho respiratório e hipertensão arterial. Nas mulheres, a OA manteve-se associada com a presença de diabetes mellitus. Os resultados contribuem para a compreensão da OA e o desenvolvimento de abordagens preventivas em saúde coletiva.

Abstract Objective: To evaluate the association between triglycerides and HDL-cholesterol (TG/HDL-c) ratio and cardiovascular risk factors among the elderly. Method: A cross-sectional epidemiological study with a random sample of elderly persons (n=349) of both genders, who received care under the Family Health Strategy in the municipality of Viçosa, in the state of Minas Gerais, was performed. Cardiovascular risk was calculated by the relationship between the TG and the HDL-c levels, with values greater than 3.5 considered a risk. Social and economic variables, lifestyle, noncommunicable chronic diseases, serum glucose levels, waist circumference (WC) and body mass index were evaluated. Multiple linear regression was used to evaluate the association between the TG/HDL-c ratio and other variables. Variables associated with the dependent variable with a level of significance lower than 0.20 in univariate regression analysis were included in the final model (stepwise-forward), applying a significance level of p<0.05. Results: The highest TG/HDL-c ratio values were associated with the presence of hypertension, having been or currently be a smoker, having elevated serum glucose and an increased waist circumference. Conclusion: The findings reflect the importance of studies on cardiovascular risk in the elderly, as health professionals should be familiar with the parameters that classify at risk individuals. The TG/HDL-c ratio is a reliable classification method that is easy to apply and correlates closely with adverse health effects. AU


Resumo Objetivo: Avaliar a associação entre razão Triglicerídeos e HDL-colesterol (TG/HDL-c) e fatores de risco cardiovascular em idosos. Método: Estudo epidemiológico transversal, com amostra probabilística de idosos (n=349), de ambos os sexos, atendidos na Estratégia Saúde da Família do município de Viçosa, MG. O risco cardiovascular foi calculado pela relação entre os níveis de TG sobre os níveis de HDL-c, sendo considerado em risco um valor maior que 3,5. Foram investigadas variáveis sociais e econômicas de estilo de vida, doenças crônicas não transmissíveis, níveis séricos de glicose, circunferência da cintura (CC) e índice de massa corporal. Para avaliar a associação entre a razão TG/HDL-c e as demais variáveis, foi utilizada regressão linear múltipla. As variáveis que se associaram à variável dependente com nível de significância menor que 0,20, na análise de regressão univariada, foram incluídas no modelo final (Stepwise-forward), adotando-se nível de significância p<0,05. Resultados: Os maiores valores da razão TG/HDL-c associaram-se com a presença da hipertensão arterial, ter sido ou ser fumante, apresentar elevados níveis séricos de glicose e possuir maior CC. Conclusão: Os achados refletem a importância de estudos sobre risco cardiovascular em idosos, devendo os profissionais da área da saúde ter familiaridade com os parâmetros que classificam os indivíduos com risco cardiovascular, sendo a razão TG/HDL-c um método confiável, com fácil aplicabilidade e boa correlação com efeitos adversos à saúde. Abstract Background: Obesity affects a large part of elderly individuals worldwide and is considered a risk predictor for the development of chronic diseases such as cardiac diseases, the leading causes of death in the elderly population. Objective: To investigate the prevalence of obesity and associated factors, with emphasis on the occurrence of other diseases and on food consumption in elderly individuals treated at the Brazilian Unified Health System (Sistema Único de Saúde, SUS). Methods: Cross-sectional sampling study performed in the city of Goiânia (Brazil) including elderly individuals (≥ 60 years) receiving primary care. During home visits, we performed anthropometric measurements and applied a structured, standardized, and pre-tested questionnaire assessing socioeconomic, demographic and lifestyle conditions, occurrence of diseases, and food consumption. We performed multiple Poisson regression analysis using a hierarchical model and adopting a significance level of 5%. Results: We evaluated 418 elderly patients with a mean age of 70.7 ± 7 years. Their body mass indices had a mean value of 27.0 kg/m2 and were higher in women than in men (27.4 kg/m2 versus 26.1 kg/m2, respectively, p = 0.017). Obesity had a prevalence of 49.0%, a risk 1.87 times higher between the ages of 60-69 years and 70-79 years, and a rate 1.4 times higher among individuals with more than four morbidities. On multivariate analysis, the factors associated with obesity were age 60-69 and 70-79 years, inadequate consumption of whole-wheat grains and adequate consumption of fruit, musculoskeletal diseases, diabetes mellitus, and acute myocardial infarction. Conclusions: Obesity had a high prevalence in the evaluated elderly population and was associated with food consumption, musculoskeletal disease, diabetes mellitus, and acute myocardial infarction.


Resumo Fundamento: A obesidade atinge uma grande parcela de idosos em todo o mundo e é considerada preditora de risco para o desenvolvimento de doenças crônicas, como as doenças cardíacas, as principais causas de óbito na população idosa. Objetivo: Investigar a prevalência de obesidade e fatores associados, com ênfase na presença de outras doenças e no consumo alimentar, em idosos usuários do Sistema Único de Saúde (SUS). Métodos: Estudo transversal, por amostragem, realizado no município de Goiânia, Brasil. Foram incluídos idosos (≥ 60 anos) atendidos na rede de atenção básica. Durante visitas domiciliares, foram realizadas medidas antropométricas e aplicação de questionário estruturado, padronizado e pré-testado sobre condições socioeconômicas, demográficas, estilo de vida, presença de doenças e consumo alimentar. Realizou-se regressão de Poisson múltipla a partir de um modelo hierárquico, adotando-se um nível de significância de 5%. Resultados: Foram avaliados 418 idosos com idade média de 70,7 ± 7 anos. O índice de massa corporal apresentou um valor médio de 27,0 kg/m2 e esteve mais elevado nas mulheres do que nos homens (27,4 kg/m2 x 26,1 kg/m2, respectivamente, p = 0,017). A obesidade teve prevalência de 49,0%, risco 1,87 vezes maior entre as idades de 60-69 anos e 70-79 anos, e taxa 1,4 vezes maior nos indivíduos com mais de quatro morbidades. Em análise multivariada, os fatores associados à obesidade foram idade de 60-69 e 70-79 anos, consumo inadequado de cereais integrais e adequado de frutas, doenças osteomusculares, diabetes mellitus e infarto agudo do miocárdio. Conclusões: A obesidade teve elevada prevalência na população idosa estudada e esteve associada com consumo alimentar, doença osteomuscular, diabetes mellitus e infarto agudo do miocárdio.

Background: the aging population has been accompanied by epidemiological changes of the Brazilian population, with the highlight being the continued growth of the prevalence of non-communicable chronic diseases especially cardiovascular or artery-coronary, resulting from changes in the lipid profile of the elderly. Objective: this study had the aim to describe the behavioral, anthropometric, lifestyle and body composition factors and their association with changes in the lipid profile of elderly people. Methodology: the sample included 402 participants attended at the Family Health Strategy, Viçosa (MG), to which a questionnaire with socio-demographic, behavioral and lifestyle information was applied. Blood sample was collected to obtain the lipid fractions, and the weight, height, waist circumference and body fat percentage were measured. Multiple linear regression was performed to identify independently associated factors with changes in each of the selected lipid fractions. Results: the factors independently associated with increased levels of total cholesterol were the presence of sedentary behavior, high body fat percentage, greater waist height and greater waist circumference. The consumption of alcoholic beverages and a higher waist-hip ratio remained independently associated with decreased high-density lipoprotein levels. The increased waist circumference was independently associated with low values of the low-density lipoprotein levels. The value of increased triglyceride was independently associated with higher waist-hip ratio, higher body mass index and smoking. Conclusions: modifiable risk factors associated with a changed lipid profile should be prioritized among the actions to be considered in structuring health programs for the elderly (AU)


Introducción: el envejecimiento de la población ha ido acompañado de cambios epidemiológicos de la población brasileña, destacando el crecimiento continuo de la prevalencia de enfermedades crónicas no transmisibles, especialmente cardiovasculares o de la arteria coronaria, como resultado de los cambios en el perfil lipídico de las personas mayores. Objetivo: describir las variables antropométricas, estilo de vida y composición corporal como factores de comportamiento y su asociación con los cambios en el perfil lipídico de las personas de edad avanzada. Metodología: la muestra incluyó a 402 participantes que asistieron a la Estrategia Salud de la Familia, Viçosa (MG), a los que se aplicó un cuestionario con información socio-demográfica, de comportamiento y de estilo de vida. Se recogió una muestra de sangre para obtener las fracciones de lípidos, y se midió el porcentaje de peso, talla, circunferencia de la cintura y grasa corporal. La regresión lineal múltiple se realizó para identificar factores independientemente asociados con los cambios en cada una de las fracciones de lípidos seleccionados. Resultado: los factores asociados de forma independiente con un aumento de los niveles de colesterol total fueron la presencia de conducta sedentaria, un porcentaje de grasa corporal alto, mayor altura de la cintura y una mayor circunferencia de la cintura. El consumo de bebidas alcohólicas y una proporción cintura-cadera más alta se mantuvo asociado de forma independiente con la disminución de los niveles de lipoproteínas de alta densidad. El aumento de la circunferencia de la cintura se asoció de forma independiente con valores bajos de los niveles de lipoproteínas de baja densidad. El valor del aumento de triglicéridos se asocia de forma independiente con una mayor relación cintura-cadera, un mayor índice de masa corporal y tabaquismo. Conclusión: los factores de riesgo modificables asociados con un perfil lipídico cambiado deben priorizarse entre las acciones a considerar en la estructuración de los programas de salud para los ancianos (AU)

Background: various anthropometric indicators can be used as predictors of cardiovascular risk in the elderly. Objective: Evaluate the predictive capacity of anthropometric measurements in identifying cardiovascular risk in elderly patients at the Family Health Strategy of Viçosa-MG. Methodology: this was a cross-sectional epidemiological study with 349 elderly persons. Cardiovascular risk was calculated using the ratio of triglyceride levels with HDL-cholesterol (TG/HDL-c) levels. The anthropometric variables measured were waist circumference, body mass index, waist-to-height ratio, and conicity index. A biochemical assessment of triglycerides and HDL-cholesterol was performed. The anthropometric measurements were also related to cardiovascular risk using Receiver Operating Characteristic (ROC) curves. Results: the observed results suggest that all these anthropometric indexes can be used to predict cardiovascular risk in males. However, in females, only BMI showed predictive capacity. The cutoff points identified appeared very close to the cutoffs recommended and recognized in other studies, with the exception of waist circumference measured at the midpoint between the last rib and the iliac crest, which showed a considerable difference. Introducción: diversos indicadores antropométricos se pueden utilizar como predictores de riesgo cardiovascular en los ancianos. Objetivo: evaluar la capacidad predictiva de las medidas antropométricas en la identificación de riesgo cardiovascular en pacientes ancianos en la Estrategia Salud de la Familia de Viçosa-MG. Metodología: se realizó un estudio epidemiológico transversal con 349 ancianos. El riesgo cardiovascular se calculó mediante la relación de los niveles de triglicéridos con niveles de HDL-colesterol (TG / HDL-c). Las variables antropométricas medidas fueron: circunferencia de la cintura, índice de masa corporal, relación cintura-altura e índice de conicidad. Se realizó una evaluación bioquímica de triglicéridos y HDL-colesterol. Las mediciones antropométricas también estaban relacionados con el riesgo cardiovascular utilizando la característica (ROC) Curvas Receiver Operating. Resultados: los resultados observados indican que todos estos índices antropométricos se pueden usar para predecir el riesgo cardiovascular en los hombres. Sin embargo, en las mujeres, solo el IMC mostró capacidad predictiva. Los puntos de corte identificados aparecieron muy cerca de los puntos de corte recomendados y reconocidos en otros estudios, con la excepción de la circunferencia de la cintura, que se mide en el punto medio entre la última costilla y la cresta ilíaca, que mostró una diferencia considerable. Conclusión: todos los índices antropométricos se pueden usar para predecir el riesgo cardiovascular en hombres y mujeres. La circunferencia de la cintura en el punto medio entre la última costilla y la cresta ilíaca era la mejor medida antropométrica para predecir el riesgo cardiovascular en los varones y la circunferencia de cintura más pequeña y cintura-altura fueron las mejores medidas antropométricas en las mujeres (AU)

Objective: The aim of this study is to investigate the abdominal perimeter determinants in adults who live in the city of Lages, SC. Design: A population-based cross-sectional study in adults from 20 to 59 years-old of the urban area (n=2.022). The dependent variable is the abdominal perimeter, the independent variables are: age, skin color self-reported, marital status, number of children, per capita income, education, physical activity, smoking, nutrition, self-reported diabetes mellitus, high blood pressure, body weight index. The differences between the mean perimeters were tested using ANOVA test and multiple linear regression for confounding adjustment. Results: The response rate was 98.2%, 52.3% were women. The mean abdominal perimeter for men was 93.66 cm (SD 13.8) and for women 92.80 cm (SD 14.5). Therewas a positive association of abdominal circumference with age (p<0.001) and negative regarding education. The abdominal perimeter means were higher for those insufficiently active (p<0.001), for former smokers (p<0.001), for those who consumed meat without fat removal (p = 0.001), for those who consumed fruit less than 5 times a week (p<0.001) and for those who were overweight (p<0.001). Remained positively associated with changes in abdominal obesity, insufficient physical activity, smoking, former smoker and consumption of meat without fat removal. All proximal variables remained positively associated with abdominal perimeter. Conclusions: The results have confirmed that diet, lifestyle and sociodemographic conditions determine a different distribution in abdominal fat, it is needed actions to promote a healthy lifestyle (AU)

Diseño: Estudio transversal de base poblacional en adultos 20-59 años de edad, de la zona urbana (n= 2022). La variable dependiente fue el perímetro abdominal y las variables independientes fueron: edad, color de la piel auto dicho, estado civil, número de hijos, renta per cápita, nivel educacional, actividad física, tabaquismo, nutrición, diabetes mellitus auto dicho, presión arterial alta e índice de masa corporal. Las diferencias entre los promedios de perímetros se probaron a través de la ANOVA y de la regresión lineal múltiple, ajustada para los factores de confusión. Resultados: La tasa de respuesta fue de un 98,2%, de los cuales un 52,3% eran mujeres. El perímetro abdominal promedio para los hombres fue 93,66 cm (SD= 13,8 cm) y para las mujeres 92,80 cm (SD= 14,5). Hubo asociación positiva entre la circunferencia abdominal y la edad (p<0,001) y negativa entre la circunferencia abdominal y el nivel educacional. El promedio de perímetro abdominal fue más grande en personas insuficientemente activas (p<0,001), en los ex fumadores (p<0,001), en los que consumen carne sin la eliminación de grasa (p=0,001), en aquellos que consumían frutos menos de 5 veces a la semana (p<0,001) y en los que tenían sobrepeso (p<0,001). Se mantuvo na asociación positiva con los cambios en la obesidad abdominal, la insuficiente actividad física, el tabaquismo, ex fumador y el consumo de carne sin la eliminación de grasa. Todas las variables proximales se mantuvieron asociadas positivamente con el perímetro abdominal. Conclusiones: Los resultados confirman que la dieta, el estilo de vida y las condiciones sociodemográficas determinan una distribución diferente de la grasa abdominal, siendo necesarias acciones para promover un estilo de vida saludable (AU)

ABSTRACT

Este estudo teve como objetivo descrever o perfil de utilização dos serviços de saúde pelos idosos cadastrados no Programa Municipal de Terceira Idade (PMTI), da cidade de Viçosa ? MG. Trata-se de estudo observacional, transversal, com amostragem não probabilística. A coleta de dados se deu por meio de entrevistas semiestruturadas. Paraverificação das variáveis utilizou-se teste de normalidade com nível de significância de p igual a 0,05. Os resultados demonstraram que 60,0% dos idosos realizavam atividade física mais de duas vezes na semana, 76,19% eram hipertensos, 15,24% diabéticos e 50,48% relataram possuir dislipidemia. O perfil de utilização dos serviços de saúde talvezpossa ser atribuído à situação socioeconômica do idoso e/ou da sua família, sendo estabelecidas relações estatísticas positivas de que quanto mais alta a renda, maior a utilização dos serviços privados. Tal fato deve ser considerado na oferta de condições para prevenção de agravos e promoção de envelhecimento saudável.


This study had as its aim to describe the profile of health service use by seniors registered on a Third-Age Municipal Program (TAMP), in Viçosa, Minas Gerais, Brazil. It is an observational and cross-sectional study, with non-probabilistic sampling. Data collection was performed by semi-structured interviews. In order to verify the variables, the normality test was used with a significance level of p=0.05. Results demonstrated that 60.0% of seniors did a physical activity more than twice a week, 76.19% were hypertensive, 15.24% were diabetic, and 50.48% had dyslipidemia. The profile of health services use may be attributed to the socioeconomic situation of the senior and/or their families. Positive statistic relations state that when the income is higher, the use of private services increases. Such fact should be taken into account when offering conditions to prevent losses and health aging promotion.
