ABSTRACT

PURPOSE OF REVIEW: Respiratory allergies are non-communicable diseases caused by the hypersensitivity of the immune system to environmental aeroallergens. The culprits are aero-transported proteins eliciting respiratory symptoms in sensitized/allergic individuals. This review intends to provide a holistic overview on the categorization of aeroallergens into protein families (Part 1) and to exploit the impact of physicochemical properties on inhalant protein allergenicity (Part 2). This first part will focus particularly on aeroallergen organization into families and how this classification fits their physicochemical properties. RECENT FINDINGS: Aeroallergen classification into protein families facilitates the identification of common physicochemical properties, thus aiding a better comprehension of known allergens, while predicting the behavior of novel ones. The available online databases gathering important features of aeroallergens are currently scarce. Information on distinct aeroallergen classification is still lacking, as data is dispersed and often outdated, hampering an efficient evaluation of new aeroallergens.

ABSTRACT

PURPOSE OF REVIEW: A holistic perspective on how physicochemical properties modulate the allergenicity of proteins has recently been performed for food allergens, launching the challenge of a similar analysis for aeroallergens. After a first review on aeroallergen classification into protein families (Part 1), this second part (Part 2) will exploit the impact of physicochemical properties (abundance/biological function, protein structure/presence of post-translational modifications, ligand/cofactor/lipid-binding) on inhalant protein allergenicity. RECENT FINDINGS: The abundance linked to biological function is correlated with increased allergenic risk for most protein families, while the loss of structural integrity with consequent destruction of conformational epitopes is well linked with decreased allergenicity. Ligand-binding effect totally depends on the ligand type being highly variable among aeroallergens. Knowledge about the physicochemical properties of aeroallergens is still scarce, which highlights the need for research using integrated approaches (in silico and experimental) to generate and analyze new data on known/new aeroallergens.

ABSTRACT

Background Central retinal artery occlusion (CRAO) results in sudden, painless vision loss. As an analogous condition to acute ischemic stroke, CRAO is an ophthalmological emergency, but a standardized treatment is lacking. Hyperbaric oxygen therapy (HBOT) has been widely used in spite of the inconsistent results reported. Purpose To report the visual acuity (VA) outcomes in all patients submitted to HBOT with non-arteritic CRAO in a tertiary center. Methods This retrospective study included all adult patients with CRAO and symptoms lasting for less than 24 hours who were prescribed HBOT in the Hyperbaric Medicine Unit of a Portuguese hospital from March 2009 to February 2023. Patient demographic information, medical history, ophthalmologic evaluation, hospital of referral, time until HBOT, supplementary treatments, number of HBOT sessions, adverse effects, and patient subjective VA gain were collected. All patients were subjected to 90-minute HBOT sessions with 100% oxygen at 2.4 ATA. The primary outcome was VA change (dif-logMAR) before and after treatment. A clinically significant visual improvement was defined as a dif-logMAR≥0.3. Data were analyzed using IBM SPSS Statistics for Windows, Version 29 (Released 2021; IBM Corp., Armonk, New York, United States) (p<0.05 is considered significant). Results A total of 114 patients were included in this study; 68% (n=77) were male, with a mean age of 69 years, and were subjected to a median number of seven HBOT sessions. No serious adverse effects from HBOT were reported. The mean time delay from symptoms to treatment was 12 hours, and best-corrected visual acuity (BCVA) at baseline was counting fingers or worse in 84% (n=96) of the patients. A dif-logMAR≥0.3 occurred in 46% (n=52) of the patients, and 58% (n=66) reported subjective VA improvement after the treatment. A significant improvement between BCVA before HBOT (2.12±0.74) and after HBOT (1.67±0.74) was observed. The VA outcome was found to be related to the total number of sessions, age, obesity, supplementary treatments, and cherry-red spot (CRS) at presentation. There were no significant effects of the time delay from symptoms to treatment in the explanation of the VA outcome. Conclusions HBOT appears to be safe and has a beneficial effect on VA outcomes in patients with non-arteritic CRAO, particularly depending on the number of sessions. Patient factors such as age, obesity, and the presence of CRSs also appear to influence the VA outcome.

BACKGROUND: Previne Brasil is a new way of financing Primary Health Care (PHC) in Brazil that places users of public health services at the center of the system's analysis and attention through the use of seven performance indicators. This study aims to analyze the performance of Primary Health Care in Brazil under the new financing model (Previne Brasil) in the period from the first four months of 2022 to the first four months of 2023. METHODS: This is a territorial ecological study using secondary data from the Primary Care Health Information System (SISAB). Seven performance indicators were included and analyzed according to Brazil, its regions, states and capitals. A comparative descriptive analysis was carried out between the spatial units. RESULTS: Of the seven indicators analyzed, only one (Proportion of pregnant women tested for syphilis and HIV) reached the proposed target considering the average of the four-month periods evaluated (target: 60%; Observed: 62.5%). The Northeast was the region with the best performance, surpassing the target in three of the seven indicators (prenatal consultations, tests for syphilis and HIV in pregnant women and dental care for pregnant women). The states and capitals showed progressive improvement in the performance indicators over the four quarters analyzed, although unevenly between the indicators. The indicators related to hypertension and diabetes were the worst. The Final Synthetic Indicator (FSI) showed an increase in the median over the four quarters (4.78; 5.65; 6.02 6.29), which represents an increase of 1.51 in the indicator between the first four months of 2022 and the first four months of 2023. CONCLUSION: Although there have been improvements in the performance of Primary Health Care in Brazil, the achievement of the agreed targets for the indicators seems a long way off. The socio-spatial inequalities in the indicators reflect the country's health complexity and reinforce the need for policies that take into account the local-regional context.

Lupine is a legume commonly used in human diet as a functional food due to its high nutritional content and important technological properties. However, its consumption can lead to the manifestation of adverse immunological reactions, posing significant health issues in sensitized/allergic patients. This work aims to investigate the effect of food processing combined with simulated gastrointestinal (GI) digestion on the immunoreactivity of lupine γ-conglutin. Model foods of wheat pasta containing 35% of lupine flour (Lupinus albus, L. luteus, and L. angustifolius) were prepared and submitted to a boiling process. The proteins were extracted and their profiles characterized by SDS-PAGE. Simulated GI digestion was performed on thermally treated pasta using the INFOGEST harmonized digestion protocol 2.0. The IgG binding capacity of γ-conglutin was assessed by immunoblotting in non-reducing conditions and indirect ELISA with specific antibodies. Results demonstrate that the boiling treatment affected the immunoreactivity of the three lupine species differently. Simulated GI digestion led to extensive destruction of the protein structure, more significant in the intestinal phase, reducing but not abolishing the IgG affinity to γ-conglutin and its potential presentation to immunocompetent cells. This information can offer valuable insights to the food industry for developing food formulations with reduced allergenic properties.

ABSTRACT

Combater as desigualdades e iniquidades em saúde exige uma abordagem abrangente e colaborativa. Para tal, é necessário investir em sistemas universais de saúde, acessíveis e de qualidade, com foco na população vulnerabilizada. Políticas públicas efetivas devem ser implementadas para abordar as disparidades socioeconômicas, de gênero e étnicas, que afetam a saúde das pessoas. Nesse sentido, as Experiências Latino-americanas podem ser bem diferentes considerando a organização dos sistemas de saúde, mas que precisam ser conhecidas pensando nos desafios de desenvolvimento econômico e social no "Sul Global". Esta obra está organizada em três partes. A primeira é composta por três capítulos iniciais que trazem perspectivas de análise para o tema da desigualdade e das iniquidades em saúde na América Latina e o Caribe, a categoria "questão social" enquanto expressão das desigualdades e política social de saúde brasileira e o tema sobre as vulnerabilidades, a determinação social e a APS. Os capítulos visam contribuir para o debate mais conceitual, como pano de fundo para o livro. A segunda parte é composta por sete capítulos que abordam experiências do território nacional, e a terceira é formada por cinco capítulos que abordam experiências internacionais nos países de Chile, Colômbia e Perú. Esperamos que esta obra se constitua como um recurso educativo para pesquisadores, profissionais de saúde, gestores e formuladores de políticas, promovendo reflexões coletivas, críticas e profundas sobre o que é e o que deveria ser a APS no contexto sociopolítico latino-americano e caribenho de hoje e como poderia contribuir para reduzir disparidades e iniquidades em saúde.

ABSTRACT

In this study, we analyzed 989 Brazilian individuals identified as flexitarians to explore the relationship between gender, sexual orientation, and meat consumption reduction behavior, while evaluating the impact of motivations driving individuals towards this dietary model. To achieve this goal, we tested the following hypotheses: women consume less meat than men; LGBT women consume less meat than heterosexual women; LGBT men consume less meat than heterosexual men; and women are more motivated by the environmental impact of meat. Our findings revealed that although gender significantly influences motivations, with women showing a greater influence from animal ethics, flexitarians of both genders exhibit similar meat consumption patterns. Neither sexual orientation nor gender influence meat reduction patterns among flexitarians. However, the race/ethnicity of white individuals is a predominant factor among those who reduce meat consumption the most. Consequently, we conclude that (i) the social theories applied to explain gender differences in meat exclusion behavior may not be the most suitable for explaining meat reduction behavior, and (ii) intersectional approaches are needed to investigate flexitarianism.

ABSTRACT

O presente relatório apresenta a análise qualitativa dos dados do projeto multicêntrico intitulado "Prevenção e controle da covid-19: estudo multicêntrico sobre a percepção e práticas no cotidiano das orientações médico-científicas pela população dos territórios de abrangência da Atenção Primária à Saúde". Este projeto foi desenvolvido pela Rede de Pesquisa e Formação do Programa de Pós-Graduação em Saúde da Família ­ PROFSAÚDE sobre covid-19. A equipe do projeto de pesquisa foi composta por uma coordenação nacional e por coordenações institucionais, representadas pelas figuras dos(as) coordenadores(as) e docentes do PROFSAÚDE nas Instituições de Ensino e Pesquisa, além de mestrandos e mestrandas do programa. Os dados da pesquisa foram coletados nas Unidades Básicas de Saúde pelos discentes do PROFSAÚDE, sob supervisão dos coordenadores institucionais, através de entrevistas individuais, semiestruturadas por um roteiro. Os participantes do estudo foram selecionados em todo território brasileiro sob responsabilidade dos pesquisadores responsáveis pelo estudo multicêntrico. A análise de dados de natureza qualitativa seguiu a técnica de análise de conteúdo do tipo temática, segundo referencial de Bardin (2016), utilizando o software MaxQDA ® 1 de forma indutiva (categorias que emergem dos dados) sob responsabilidade de especialistas. Os resultados da análise estão reunidos através de relatório com segmentos codificados, mapa de hierarquia de códigos e nuvem de palavras. Ademais, esses dados foram organizados em resultados nacionais, regionais e temáticos. Em nome do grupo de pesquisa Territórios, Modelagens e Práticas em Saúde da Família, cadastrado no Conselho Nacional de Desenvolvimento Científico e Tecnológico (CNPq), agradecemos a participação dos(as) alunos(as) do curso, dos docentes e coordenadores(as) institucionais da rede PROFSAÚDE, gestores municipais e trabalhadores e trabalhadoras da saúde que apoiaram a pesquisa nas Unidades Básicas de Saúde. Por fim, agradecemos aos usuários que aceitaram participar do estudo, ressaltando a importância da colaboração de todos para o desenvolvimento da pesquisa.

ABSTRACT

O Mestrado Profissional em Saúde da Família (PROFSAÚDE) é um programa de pós-graduação stricto sensu, organizado em uma rede nacional formada por 45 instituições públicas de ensino superior, lideradas pela Associação Brasileira de Saúde Coletiva (ABRASCO) e pela Fundação Oswaldo Cruz (FIOCRUZ). O programa tem por finalidade formar profissionais de saúde para exercerem atividades de docência, preceptoria, gestão, investigação e ensino no Sistema Único de Saúde (SUS), fortalecendo as atividades de produção de conhecimento e ensino na Atenção Primária, nas diversas regiões do país. Ao longo de seus sete anos de criação, foram ofertadas quatro turmas, que formaram mais de 500 mestres em Saúde da Família em todo o Brasil. A modalidade de mestrado profissional oportuniza que os alunos utilizem os conhecimentos científicos para desenvolver produtos finais que respondam as necessidades do SUS, trazendo respostas para os problemas identificados no campo de prática, qualificando processos e produzindo conhecimento. O produto final pode trazer inovação e o desenvolvimento de produtos técnicos e tecnológicos para o SUS. O programa organizou no último ano uma série de debates e criou um Grupo de Trabalho (GT) para discutir o trabalho de conclusão do mestrado (TCM) e a produção técnica tecnológica. O foco das discussões foi a importância de pensar o produto final enquanto reflexões e discussões da prática profissional dos mestrandos e resolução de problemas de seus cenários de atuação, considerando o impacto dessa produção para transformação dos processos de trabalho e dos territórios. O PROFSAÚDE definiu que a produção técnica e tecnológica deve contribuir para o desenvolvimento e a disseminação de novos conhecimentos, processos ou tecnologias aplicados ao campo da atenção, gestão e educação, visando o fortalecimento da Atenção Primária. O programa iniciou, portanto, um processo de aperfeiçoamento teóricometodológico do itinerário de construção dos produtos finais. Ao reconhecer os desafios dos profissionais de fazer com que seus produtos sejam implantados nos diversos territórios do SUS, cada mestre pode percorrer um trajeto após a conclusão do curso, para incorporar seus trabalhos ao campo de prática. Esta obra apresenta uma coleção de 80 produtos desenvolvidos por egressos das três primeiras turmas do PROFSAÚDE. Essa produção técnica e tecnológica visa responder às demandas cotidianas do trabalho na Atenção Primária, constituindo-se como uma das principais interfaces entre a produção do conhecimento ­ fruto das reflexões desenvolvidas durante o mestrado ­ e os serviços e comunidades em que os discentes estão inseridos profissionalmente. Os produtos foram catalogados e estão dispostos segundo a região, o eixo e a Instituição de Ensino Superior, e apresentam um QR Code para acesso na íntegra. No eixo de atenção, foram desenvolvidos produtos com o objetivo de aprimorar e assegurar a qualidade do cuidado em saúde no âmbito da Atenção Primária à Saúde, bem como otimizar os processos de trabalho, incluindo fluxos de atendimento e serviços, desenvolvimento de instrumentos de sistematização, avaliação e acompanhamento do cuidado, matriz de ações, entre outros. No eixo de gestão, foram elaborados produtos voltados à melhoria dos processos administrativos e organizacionais para otimizar a eficiência e a qualidade dos serviços, tais como relatórios técnicos, planos de intervenção, modelo de avaliabilidade de plano de contingência, entre outros. No eixo de educação, foram desenvolvidos produtos voltados à educação em saúde e relacionados à formação de profissionais de saúde no âmbito da graduação e da pós-graduação, incluindo matriz e currículos inovadores, formação para preceptores, oficinas pedagógicas, projetos de residência e materiais didáticos. Este Portfólio representa o compromisso do PROFSAÚDE com a sociedade e com o fortalecimento da área. Trata-se da divulgação do que vem sendo desenvolvido dentro do programa e do impacto gerado pelo processo formativo do PROFSAÚDE em todo o Brasil. Acreditamos que esse movimento traz contribuições significativas, pois os produtos catalogados, e aqui reunidos, representam a pluralidade do país e contemplam diversas temáticas, na medida em que foram desenvolvidos em diferentes eixos, diferentes regiões e distintas realidades.

Objectives: This study analyzed the influence of 23 comorbidities on COVID-associated acute distress respiratory syndrome (CARDS) mortality in people with a history of diabetes mellitus. Methods: An observational, analytical, cross sectional study was utilized to investigate data from 6723 health services in Brazil, comprising 5433 people with diabetes. Adjusted logistic regression models for demographic factors such as age, sex, and race were used to analyze the association between CARDS mortality and comorbidities. Results: Persons with two (p < 0.001), three (p < 0.001), four (p < 0.001), and five (p < 0.001) simultaneous comorbidities had a higher chance of dying. We identified that diabetes patients who had concomitant metabolic diseases (p = 0.019), neurological disorders (p < 0.001), or were smokers (p < 0.001) had a higher predicted mortality risk based on CADRS. Conclusion: The number of comorbidities plays a determining role in CARDS mortality in people with diabetes, especially those who suffer from smoking and neurological diseases simultaneously.

Dietary protein quality plays a key role in maintaining intestinal mucosal integrity, but also modulates the growth of luminal microorganisms. This work assessed the effect of dietary protein sources on the performance, gut morphology, and microbiome in Nile tilapia. Four isonitrogenous and isolipidic diets comprising equivalent amounts of the protein supply derived from either PLANT, ANIMAL, INSECT, or BACTERIAL (bacterial biomass) sources were fed to triplicate groups of fish (IBW: 12 g) for 46 days. Fish fed the ANIMAL and BACTERIAL diets showed significantly higher weight gains than those fed the PLANT and INSECT diets (p < 0.05). Relative abundance at the phylum level showed that Bacteroidetes, Fusobacteria, and Proteobacteria were the more abundant phyla in tilapia's intestine, while Cetobacterium was the most representative genus in all treatments. Interesting patterns were observed in the correlation between amino acid intake and genus and species abundance. Metabolism prediction analysis showed that BACTERIAL amine and polyamine degradation pathways are modulated depending on diets. In conclusion, different protein sources modulate the relationship between bacteria functional pathways and amino acid intake.

Coronavirus disease 2019 (COVID-19) is often linked to a broad range of opportunistic bacterial and fungal infections. The second wave of the COVID-19 pandemic has witnessed an unprecedented surge in mucormycosis cases, predominantly in India, while the disease remained relatively rare in Europe. The authors describe the case of a 62-year-old female patient admitted to the hospital for consolidation therapy with chemotherapy as a part of the treatment protocol for acute myeloid leukemia. During hospitalization, she was diagnosed with nosocomial COVID-19, which later progressed to respiratory deterioration. COVID-19 with bacterial superinfection was presumed, leading to the initiation of empirical antibiotic therapy. A bronchoscopy was performed several days later due to a lack of improvement, revealing an infection by the Rhizopus microsporus complex. Despite antifungal treatment, the patient experienced an unfavorable clinical course and ultimately died. Given the high index of suspicion required to diagnose pulmonary mucormycosis, which can lead to delays in appropriate treatment and increase the burden of disease, the authors are aiming to enhance its awareness.

ABSTRACT

Retinal pigment epithelium (RPE) tears occur when the RPE acutely breaks and retracts, leaving the underlying Bruch's membrane and choroid exposed. They usually happen in areas of previous pigment epithelial detachments and are generally associated with age-related macular degeneration (AMD). The purpose of this report is to describe a case of a spontaneous massive central RPE tear in a patient with untreated AMD. A 67-year-old female patient presented with complaints of sudden decreased vision in her right eye. Her best-corrected visual acuity was 2/20, and fundoscopy revealed a massive central retinal hemorrhage with intraretinal, subretinal, and sub-RPE blood. The patient started anti-vascular endothelial growth factor (VEGF) treatment, and after the blood was reabsorbed, a very large central tear of the RPE involving the central macula was evident, with a layer of detached retina folded on itself. She received continuous anti-VEGF therapy, and the final measurement of her visual acuity was 2/200, despite the complete reabsorption of the hemorrhage. RPE tears may occur spontaneously as part of the natural history of AMD or be triggered by the initiation of anti-VEGF treatment in the presence of large pigment epithelium detachments. There are currently no strategies to prevent their spontaneous development, and they constitute a dramatic complication of AMD. The prognosis is dependent on the size and location of the lesion, and the visual loss is irreversible.

ABSTRACT

AIM: This study aims to characterize sociodemographic and clinical characteristics, use of lipid-lowering therapies (LLTs), and low-density lipoprotein cholesterol (LDL-C) control in a population with increased cardiovascular (CV) risk. METHODS: A cross-sectional observational study that uses electronic health records of patients from one hospital and across 14 primary care health centers in the North of Portugal, spanning from 2000 to 2020 (index date). Patients presented at least (i) 1 year of clinical data before inclusion, (ii) one primary care appointment 3 years before the index date, and (iii) sufficient data for CV risk classification. Patients were divided into three cohorts: high CV risk; atherosclerotic cardiovascular disease (ASCVD) risk equivalents without established ASCVD; evidence of ASCVD. CV risk and LDL-C control were defined by the 2019 and 2016 European Society of Cardiology (ESC)/European Atherosclerosis Society (EAS) dyslipidemia guidelines. RESULTS: A total of 51 609 patients were included, with 23 457 patients classified as high CV risk, 19 864 with ASCVD equivalents, and 8288 with evidence of ASCVD. LDL-C control with 2016 ESC/EAS guidelines was 32%, 10%, and 18% for each group, respectively. Considering the ESC/EAS 2019 guidelines control level was even lower: 7%, 3%, and 7% for the same cohorts, respectively. Patients without any LLT prescribed ranged from 37% in the high CV risk group to 15% in patients with evidence of ASCVD. CONCLUSION: We found that LDL-C control was very low in patients at higher risk of CV events. An alarming gap between guidelines on dyslipidemia management and clinical implementation persists, even in those at very high risk or with established ASCVD.

ABSTRACT

In this study, we conducted the first scientific investigation focusing on Brazilian flexitarians, aiming to characterize their socio-economic and demographic profiles, motivations for adopting flexitarianism, the frequency of animal-based meat consumption, and the primary meat substitutes they consume. To accomplish this, we distributed an online questionnaire with the assistance of university students and researchers from various regions of the country. Data were collected from 1029 individuals in Brazil who self-identified as flexitarians. Our findings reveal that the flexitarian dietary model is primarily adopted by women, constituting 76% of the sample (n = 786). Their motivations include concerns about the environmental impact of meat consumption (n = 361, 35%), personal health (n = 344, 33%), and animal welfare (n = 219, 21%). Flexitarians exhibit varying consumption patterns, which can be categorized into three groups: light flexitarians (consuming meat 36 times a week), medium flexitarians (consuming meat 7 times a week), and heavy flexitarians (consuming meat 4 times a week). The flexitarian dietary pattern is characterized by reduced beef consumption (less than 2 times per week) and higher consumption of chicken (3 times per week). It is complemented by plant-based protein sources and eggs as the primary meat substitutes. The recognition of legumes as the principal meat substitutes opens avenues for an expanded discussion on sustainable food systems and alternative meat products in Brazil. This provides opportunities to enhance the availability and accessibility of these foods and to develop nutritional interventions that prioritize plant-based proteins.

Edible insects have been proposed as an environmentally and economically sustainable source of protein, and are considered as an alternative food, especially to meat. The migratory locust, Locusta migratoria, is an edible species authorised by the European Union as a novel food. In addition to their nutritional value, edible insects are also sources of bioactive compounds. This study used an in silico approach to simulate the gastrointestinal digestion of selected L. migratoria proteins and posteriorly identify peptides capable of selectively inhibiting the N-subunit of the somatic angiotensin-I converting enzyme (sACE). The application of the molecular docking protocol enabled the identification of three peptides, namely TCDSL, IDCSR and EAEEGQF, which were predicted to act as potential selective inhibitors of the sACE N-domain and, therefore, possess bioactivity against cardiac and pulmonary fibrosis.

to analyze the correlation between time on target and glycated hemoglobin in people living with diabetes mellitus and carrying out continuous blood glucose monitoring or self-monitoring of capillary blood glucose.systematic review of etiology and risk based on JBI guidelines and reported according to Preferred Reporting Items for Systematic Reviews and Meta- Analyses, covering six databases and grey literature. The sample included 16 studies and methodological quality was assessed using JBI tools. Protocol registered in the Open Science Framework, available at https://doi.org/10.17605/OSF.IO/NKMZB.time on target (70-180 mg/dl) showed a negative correlation with glycated hemoglobin, while time above target (>180 mg/dl) showed a positive correlation. Correlation coefficients ranged between -0.310 and -0.869 for time on target, and between 0.66 and 0.934 for time above target. A study was carried out on a population that performed self-monitoring.there is a statistically significant correlation between time on target and time above target with glycated hemoglobin. The higher the proportion in the adequate glycemic range, the closer to or less than 7% the glycated hemoglobin will be. More studies are needed to evaluate this metric with data from self-monitoring of blood glucose.
