ABSTRACT

Human behavior recognition technology is widely adopted in intelligent surveillance, human-machine interaction, video retrieval, and ambient intelligence applications. To achieve efficient and accurate human behavior recognition, a unique approach based on the hierarchical patches descriptor (HPD) and approximate locality-constrained linear coding (ALLC) algorithm is proposed. The HPD is a detailed local feature description, and ALLC is a fast coding method, which makes it more computationally efficient than some competitive feature-coding methods. Firstly, energy image species were calculated to describe human behavior in a global manner. Secondly, an HPD was constructed to describe human behaviors in detail through the spatial pyramid matching method. Finally, ALLC was employed to encode the patches of each level, and a feature coding with good structural characteristics and local sparsity smoothness was obtained for recognition. The recognition experimental results on both Weizmann and DHA datasets demonstrated that the accuracy of five energy image species combined with HPD and ALLC was relatively high, scoring 100% in motion history image (MHI), 98.77% in motion energy image (MEI), 93.28% in average motion energy image (AMEI), 94.68% in enhanced motion energy image (EMEI), and 95.62% in motion entropy image (MEnI).

ABSTRACT

Although the existing deep supervised solutions have achieved some great successes in medical image segmentation, they have the following shortcomings; (i) semantic difference problem: since they are obtained by very different convolution or deconvolution processes, the intermediate masks and predictions in deep supervised baselines usually contain semantics with different depth, which thus hinders the models' learning capabilities; (ii) low learning efficiency problem: additional supervision signals will inevitably make the training of the models more time-consuming. Therefore, in this work, we first propose two deep supervised learning strategies, U-Net-Deep and U-Net-Auto, to overcome the semantic difference problem. Then, to resolve the low learning efficiency problem, upon the above two strategies, we further propose a new deep supervised segmentation model, called µ-Net, to achieve not only effective but also efficient deep supervised medical image segmentation by introducing a tied-weight decoder to generate pseudo-labels with more diverse information and also speed up the convergence in training. Finally, three different types of µ-Net-based deep supervision strategies are explored and a Similarity Principle of Deep Supervision is further derived to guide future research in deep supervised learning. Experimental studies on four public benchmark datasets show that µ-Net greatly outperforms all the state-of-the-art baselines, including the state-of-the-art deeply supervised segmentation models, in terms of both effectiveness and efficiency. Ablation studies sufficiently prove the soundness of the proposed Similarity Principle of Deep Supervision, the necessity and effectiveness of the tied-weight decoder, and using both the segmentation and reconstruction pseudo-labels for deep supervised learning.

ABSTRACT

Primary head and neck hematolymphoid neoplasms (PHNHLN) are defined as a series of hematolymphoid system-derived neoplasms which primarily emanate in head and neck region. Due to the rarity and absence of symptomatic specificity, PHNHLN is easily neglected. The objective of this study is to investigate demographics, pathological subtype distribution, anatomical location, survival outcomes and prognostic factors of PHNHLN among older patients aged ≥ 60. The individual patient information in our study was derived from Surveillance, Epidemiology and End Results database. Descriptive epidemiological methods were used to analyze the distribution of histologic subtypes and primary anatomical sites. Kaplan-Meier survival curves and log-rank test were conducted to evaluate the effect of variables on the prognosis. Cox hazard regression was conducted to identify the independent prognostic factors. The male-to-female ratio in most pathological subtypes was close to 1:1. The most common pathological subtype was diffuse large B-cell lymphoma. The most commonly involved sites outside the lymph nodes were salivary glands, especially parotid gland, followed by tonsil, thyroid gland and tongue. The prognosis of mature T- and NK-cell non-Hodgkin lymphoma (NHL) was bleaker than Hodgkin lymphoma, mature B-cell NHL and plasma cell neoplasm. Age at diagnosis, presence of second primary malignancy (SPM), pathological subtype, Ann-Arbor stage, chemotherapy and radiation were independent prognostic factors of overall survival. Our study comprehensively reported the subtype distribution, anatomical sites and survival outcomes of PHNHLN among older patients, improving understanding of this rare group of cancer entities.

ABSTRACT

There is growing concern about the impact of agricultural practices on water quality. The loss of nutrients such as nitrogen and phosphorous through agricultural runoff poses a potential risk of water quality degradation. However, it is unclear how dissolved organic matter (DOM) composition is associated with pollution levels in water bodies. To address this, we conducted a cross-year investigation to reveal the nature of DOM and its relationship to water quality in agricultural effluents (AEs) and livestock effluents (LEs). We discovered that DOM fluorescence components of AEs were mainly from autochthonous and terrestrial sources, while in LEs it was primarily from autochthonous sources. LEs showed a higher ß:α and biological index (BIX) than AEs, indicating that LEs had higher biological activity. Compared to the LEs, DOM in AEs exhibited a higher humification index (HIX), illustrating that DOM was more humic and aromatic. Overall, our results suggest that the BIX and fluorescence index (FI) were best suited for the characterization of water bodies impacted by LEs and AEs. Excitation-emission matrix spectroscopy and parallel factor (EEMs-PARAFAC) analysis showed that DOM in AEs was mainly a humic-like material (~64%) and in LEs was mainly protein-like (~68%). Tryptophan-like compounds (C1) were made more abundant in AEs because of the breakdown of aquatic vegetation. The microbial activity enhanced protein-like substances (C1 and C2) in LEs. Our study revealed a positive correlation between five-day biochemical oxygen demand (BOD5) concentrations and tyrosine-like substance components, suggesting that fluorescence peak B may be a good predictor of water quality affected by anthropogenic activities. For both LEs and AEs, our results suggest that peak D may be a reliable water quality surrogate for total phosphorus (TP).

ABSTRACT

Ecological risks can vary dramatically depending on abiotic factors, such as soil properties and the background values of elements. This study developed a framework for an integrated risk assessment system to derive soil quality criteria (SQC) for heavy metals (HMs) applicable to different soil types and to assess ecological risks at a multi-regional scale. Through the construction of normalization and species sensitivity distribution models, 248 SQC values for Cd, Pb, Zn, As, Cu, Cr, Sb, and Ni in 31 Chinese provinces were derived. These SQC considered the soil types and background values of the elements and effectively reduced the uncertainty caused by spatial heterogeneity. Using the derived SQC values, the qualitative and quantitative ecological risks of HMs in the terrestrial environment of China were comprehensively assessed using a three-level ecological risk assessment (ERA) approach. Compared to traditional ERA methods, the new methodology reached a more quantitative conclusion. The mean overall probabilities of ecological risk in China were 2.42 % (Cd), 2.82 % (Pb), 12.17 % (Zn), 14.89 % (As), 10.42 % (Cu), 32.20 %(Cr), and 8.88 % (Ni). The new framework could be useful for the ERA of various soil types.

ABSTRACT

Semi-supervised learning has a great potential in medical image segmentation tasks with a few labeled data, but most of them only consider single-modal data. The excellent characteristics of multi-modal data can improve the performance of semi-supervised segmentation for each image modality. However, a shortcoming for most existing multi-modal solutions is that as the corresponding processing models of the multi-modal data are highly coupled, multi-modal data are required not only in the training but also in the inference stages, which thus limits its usage in clinical practice. Consequently, we propose a semi-supervised contrastive mutual learning (Semi-CML) segmentation framework, where a novel area-similarity contrastive (ASC) loss leverages the cross-modal information and prediction consistency between different modalities to conduct contrastive mutual learning. Although Semi-CML can improve the segmentation performance of both modalities simultaneously, there is a performance gap between two modalities, i.e., there exists a modality whose segmentation performance is usually better than that of the other. Therefore, we further develop a soft pseudo-label re-learning (PReL) scheme to remedy this gap. We conducted experiments on two public multi-modal datasets. The results show that Semi-CML with PReL greatly outperforms the state-of-the-art semi-supervised segmentation methods and achieves a similar (and sometimes even better) performance as fully supervised segmentation methods with 100% labeled data, while reducing the cost of data annotation by 90%. We also conducted ablation studies to evaluate the effectiveness of the ASC loss and the PReL module.

ABSTRACT

OBJECTIVE: To investigate the effiicacy of laparoscopic assisted microsurgical vasovasostomy in the treatment of vas deferens obstruction caused by inguinal herniorrhaphy. METHODS: Clinical data of patients undergoing surgical treatment for deferential obstruction after inguinal hernia repair in the andrology department of the First Affiliated Hospital of Zhengzhou University from 2018 to 2022 were retrospectively analyzed, and they were divided into two groups according to different surgical methods: double mirror combined group and microscope group. The basic clinical data, intraoperative conditions, postoperative effects and complications of the two groups were compared. RESULTS: There were 14 cases in the double mirror group and 34 cases in the microscope group. There was no significant difference in age and history of groin operation between the two groups (P>0.05). The average length of hospital stay in the two-lens group was less than that in the microscope group (5.07±0.26 days vs 7.09±1.86 days, P< 0.01), and the average operation time in the two-lens group was more than that in the microscope group (211.93±58.55min vs 162.26±40.70min, P<0.01). The postoperative recurrence rate (85.7% vs 73.5%, P > 0.05) was similar between the two groups. There was no significant difference in early postoperative complications (0% vs 2.9%, P > 0.05). Only 1 patient in the microscope group experienced fat liquefaction and recovered after intensive dressing change. CONCLUSION: Laparoscope-assisted microscopy provides natural fertility opportunities for patients with vas deferens obstruction after inguinal hernia repair, reduces the difficulty of surgery and the length of hospital stay, and is a safe and effective surgical method comparable to traditional surgical methods.

ABSTRACT

Objective: To evaluate the safety and efficacy of transvesical incision in the treatment of ejaculatory duct obstruction. Methods: The clinical data of 26 male infertile patients with ejaculatory duct obstruction were retrospectively analysed at the First Affiliated Hospital of Zhengzhou University from June 2020 to August 2021. All patients were treated with seminal vesicle neck incision for ejaculatory duct obstruction. The general clinical characteristics, intraoperative conditions and postoperative effects on the patients were recorded, and the therapeutic effect was evaluated. Results: The ejaculatory duct was found through fenestration, and the seminal vesicle gland was smoothly entered in 25 patients (96.2%). Among them, 22 cases underwent bilateral endoscopy and three underwent unilateral endoscopy. Sperm appeared in 23 cases (88.5%) 3 months after surgery. The sperm concentration and motility postoperatively at 6 months were higher than that at 3 months postoperatively. No postoperative complications, such as epididymitis or retrograde ejaculation, occurred. Conclusion: Searching for the ejaculatory duct via the neck of the prostatic utricle, assisted by a low-energy holmium laser, is a new method for the treatment of ejaculatory duct obstruction. Microscopic vision is clear using this approach and the postoperative complications are few, which has high value for clinical application.

ABSTRACT

The study explored the clinical efficacy of a type 5 phosphodiesterase inhibitor (PDE5i) combined with Ziyin Baihuo granules in the treatment of patients suffering from erectile dysfunction (ED) with yin deficiency and fire-hyperactivity syndrome. A total of 163 patients with erectile dysfunction were divided into observation and control groups. The observation group took tadalafil (Cialis) and Ziyin Baihuo granules orally, and the control group took only tadalafil orally, for 12 weeks. An additional 40 healthy people were selected as a normal group for comparison of the sex hormone levels before and after treatment of the participants in the erectile dysfunction group. After treatment, the symptoms of dry throat and tongue, tidal fever and night sweats, liking cold and avoiding heat, and waist pain showed significant improvement in the observation group (p < .05). Compared with before treatment, the clinical indexes of erectile function in the control group and the observation group were improved after treatment (p < .05). After treatment, Ziyin Baihuo granules combined with tadalafil restored the abnormal indexes of blood (p < .05) in the observation group. Our research shows that PDE5i combined with Ziyin Baihuo granules could effectively improve erectile function.

ABSTRACT

Chromosome 1q21 aberration is one of the most common cytogenetic abnormalities in multiple myeloma, and is considered an important prognostic factor. The present study analyzed the clinical relevance and prognostic impact of 1q21 gain in 194 patients with newly diagnosed multiple myeloma treated with bortezomib-based regimens. 1q21 gain was detected in 45.9% (89/194) of patients, and those with 1q21 gain had a worse prognosis. Strikingly, our results showed that excluding the effects of other coinciding genetic anomalies, patients carrying at least four copies of 1q21 had worse survival outcome. Moreover, del(13q) strongly correlates with 1q21 gain, and the coexistence of del(13q) and 1q21 gain plays an important role in reducing PFS and OS times. Therefore, 1q21 gain should be considered a high-risk feature in multiple myeloma patients treated with a bortezomib-based regimen.

ABSTRACT

This study aimed to compare the success rates of two approaches for seminal vesiculoscopy: through the interior of the prostatic utricle and through the neck of the prostatic utricle. The patients were divided into two groups based on the seminal vesiculoscopy used. Group A was an interior of the prostatic utricle group (152 cases), and group B was a neck of the prostatic utricle group (146 cases). The general clinical data, intraoperative conditions and surgical results of the two groups were compared. Compared with group A, group B had a higher surgical success rate (94.5% vs. 62.5%, p < .001), a shorter operation time (33 min vs. 45 min, p < .001), less blood loss (0.5 ml vs. 2 ml, p < .001), a higher pain relief rate (86.6% vs. 52.3%, p < .001), a higher remission rate of haemospermia (82.2% vs. 58.5%, p = .011), a lower recurrence rate of pain (10.4% vs. 35.4%, p < .001), a lower recurrence rate of haemospermia (15.6% vs. 37.7%, p = .014), a higher symptom remission rate of the lower urinary tract (90.9% vs. 50.0%, p = .030), a higher remission rate of scrotal moisture (84.6% vs. 45.5%, p = .042) and a higher remission rate of frequent spermatorrhea (80.0% vs. 55.6%, p = .033). Seminal vesiculoscopy undertaken through the neck of the prostatic utricle has the characteristics of high success rate, short operation time and good surgical effect and is worthy of promotion and application.

The soil environmental pollution situation has been severe in recent years, but studies on evaluating with bioavailability testing and prediction models are lacking, which makes it difficult to accurately assess the ecological risks of contaminated soil. As an important indicator of bioavailability, the bioaccessibility of cadmium (Cd), arsenic (As), copper (Cu), zinc (Zn), and lead (Pb) in the soil was analyzed in this study. The bioaccessibility content and their corresponding soil property data were screened and systematically analyzed to explore the relationship between bioaccessibility content and soil properties. Furthermore, some testing methods for bioaccessibility were summarized to analyze the relationship between bioaccessibility content, test methods, and bioavailability content. Additionally, the bioaccessibility content prediction models were established. The results showed that there was a strong correlation between the bioaccessibility content and the total content of heavy metals (P<0.01) and a significant (P<0.05) correlation with the soil pH. Different test methods had obvious effects on bioavailability. The proportion of bioaccessibility content determined via various test methods was as follows:in vitro gastrointestinal tract simulation>chemical reagent extraction. The proportions of bioaccessibility content of Cd and Pb in natural soil were relatively high, with mean values of 42.12% and 37.33%, respectively, indicating that Cd and Pb had higher risks of being absorbed by soil organisms. Moreover, 30 bioaccessibility prediction models for five heavy metals were constructed, which involved the soil properties and test methods. The results of this study can provide scientific information and bioaccessibility prediction models that can help in accurately assessing the ecological risks of contaminated soil.

Background: Multiple myeloma (MM) is a highly heterogeneous disease with enormously variable outcomes. It remains to be a major challenge to conduct a more precise estimation of the survival of MM patients. The existing stratifications attached less importance to the prognostic significance of comorbidities. In the present study, we aimed to develop and validate a novel and simple prognostic stratification integrating tumor burden and comorbidities measured by HCT-CI. Method: We retrospectively enrolled 385 consecutive newly diagnosed multiple myeloma (NDMM) patients in Xijing Hospital from January 2013 to December 2020. The cohort between January 2016 and December 2020 was selected as development cohort (N = 233), and the cohort between January 2013 and December 2015 was determined as validation cohort (N = 152). By using LASSO analysis and univariate and multivariable Cox regression analyses, we developed the MM-BHAP model in the way of nomogram composed of ß2-MG, HCT-CI, ALB, and PBPC. We internally and externally validated the MM-BHAP model and compared it with ISS stage and R-ISS stage. Results: The MM-BHAP model was superior to the ISS stage and partially better than the R-ISS stage according to time-dependent AUC, time-dependent C-index, DCA, IDI, and continuous NRI analyses. In predicting OS, only the MM-BHAP stratification clearly divided patients into three groups while both the ISS stage and R-ISS stage had poor classifications in patients with stage I and stage II. Moreover, the MM-BHAP stratification and the R-ISS stage performed well in predicting PFS, but not for the ISS stage. Besides, the MM-BHAP model was also applied to the patients with age ≤65 or age >65 and with or without HRCA and could enhance R-ISS or ISS classifications. Conclusions: Our study offered a novel simple MM-BHAP stratification containing tumor burden and comorbidities to predict outcomes in the real-world unselected NDMM population.

ABSTRACT

A fixed-time trajectory following problem for quadrotors via output feedback is concerned. Based on the inner-outer separation design philosophy, the under-actuated quadrotor is formulated as a hierarchical structure composed by position and attitude dynamics. With an emphasis on removing the demand on unmeasured velocity and eliminating the negative effect of disturbances, fixed-time extended state observers utilizing two kinds of polynomial feedback terms are proposed to simultaneously identify unavailable velocity states and unknown uncertainties with a fixed-time estimation capability. With these observation results, a velocity free fixed-time control protocol is synthesized to enable a satisfied trajectory regulation with a uniform convergence time independent of initial positions, such that a prescribed fixed-time stability and enhanced robustness can be obtained with chattering-free inputs. By virtue of bi-limit homogeneity properties, all error variables of the resultant quadrotor system are demonstrated to be fixed-time convergent. Eventually, the benefits of developed algorithm are illustrated via simulations.

OBJECTIVE: To analyze the blood biochemical characteristics of the ED patients with different types of kidney deficiency or non-kidney deficiency. METHODS: We reviewed the clinical data on 156 ED patients treated in our Department of Andrology from May to July 2018 and, according to the traditional Chinese medicine (TCM) syndromes, divided them into four groups: kidney-yang deficiency (n = 48), kidney-yin deficiency (n = 34), kidney-yin+yang deficiency (n = 36) and non-kidney deficiency control (n = 38). We obtained and compared their blood biochemical indexes, including the levels of testosterone (T), estradiol (E2), cortisol (CORT), thyroid stimulating hormone (TSH), free thyroxine (FT4), free triiodothyronine (FT3), nitric oxide (NO), total nitric oxide synthase (tNOS), and inducible nitric oxide synthase (iNOS). RESULTS: There were no statistically significant differences in the mean age, course of disease, IIEF-5 score and erection hardness score (EHS) among the four groups of patients. Pairwise comparison showed that, compared with the non-kidney deficiency controls, the patients in the kidney-yin deficiency group exhibited a dramatically higher level of CORT (ï¼»87.97 ± 45.59ï¼½ vs ï¼»121.78 ± 41.87ï¼½ µg/L, P = 0.002) and those in the kidney-yang deficiency group a remarkably lower level of FT3 (ï¼»5.44 ± 0.38ï¼½ vs ï¼»5.11 ± 0.54ï¼½ pmol/L, P = 0.008). The iNOS level was significantly higher in the kidney-yin deficiency group (14.42 ± 2.49 U/ml) than in either the control (12.71 ± 2.58 U/ml) (P = 0.039) or the kidney-yang deficiency group (13.05 ± 2.17 U/ml) (P =0.049). CONCLUSIONS: ED patients with different types of kidney deficiency syndromes have different blood biochemical indexes, which may help clarify the biological basis of the TCM syndromes of kidney deficiency in ED patients.

OBJECTIVE: To investigate the factors influencing the recovery from childhood inguinal herniorrhaphy (IH)-induced vas deferens obstruction following microscopic vasovasostomy. METHODS: We retrospectively analyzed the clinical data on 41 cases of microscopic vasovasostomy for obstructive azoospermia in our hospital from July 2015 to September 2018. All the patients had a history of inguinal hernia treated by IH in the childhood. We performed scrotal ultrasonography, semen analysis and seminal plasma biochemistry to confirm vas deferens obstruction preoperatively. If sperm was observed for ≥2 times in semen examination after vasovasostomy, we considered the vas deferens successfully unobstructed. RESULTS: Microscopic vasovasostomy was successfully completed in 39 of the cases, of which2 were lost to follow-up, with a follow-up rate of 94.8% (37/39). The patients, at the mean age of (25.54 ± 2.85) years and with body mass index (BMI) of (24.92 ± 2.79) kg/m2 and post-IH time of (18.97 ± 2.58) years, were followed up for (13.05 ± 3.74) months. Successful recovery from vas deferens obstruction was observed in 78.4% (29/37) of the patients after IH, 80.0% (16/20) in the < 26-year-olds, 76.5% (13/17) in the ≥26-year-olds (P = 0.795), 75.0% (12/16) in those with BMI < 24.92 kg/m2 , 81.0% (17/21) in those with BMI ≥24.92 kg/m2 (P = 0.807), 78.6% (11/14) in those with post-IH time of < 19 years, 18.3% (18/23) in those with post-IH time of ≥19 years (P = 0.982), 60.0% (12/20) in those with sperm and 82.4% (14/17) in those without sperm found intraoperatively (P = 0.428), 42.9% (3/7) in those treated by unilateral and 82.4% (26/30) in those by bilateral vasovasostomy (P = 0.027). Multivariate logistic regression analysis showed a close correlation between the operation side and postoperative recovery from vas deferens obstruction (P = 0.022). CONCLUSIONS: For male patients undergoing microscopic vasovasostomy for childhood IH-induced vas deferens obstruction, the operation side is an independent factor influencing postoperative recovery, while age, BMI, post-IH time, and intraoperative presence or absence of sperm are not significantly correlated with it.

ABSTRACT

BACKGROUND: Chloroplasts are essential for photosynthesis and play key roles in plant development. High temperature affects structure of chloroplasts and metabolism in plants. The seryl-tRNA synthetase plays an important role in translation of proteins. Although seryl-tRNA synthetase has been widely studied in microbes and animals, few studies have reported about its role in chloroplast development under high temperature in rice. RESULTS: In this study, we isolated a novel temperature-sensitive chlorophyll-deficient 11 (tscd11) mutant by ethyl methane sulfonate (EMS) mutagenesis of japonica variety Wuyujing7. The tscd11 mutant developed albino leaves at the 3-leaf stage under high temperature (35 °C), but had normal green leaves under low temperature (25 °C). Consistent with the albino phenotype, impaired chloroplasts, decreased chlorophyll content and increased ROS accumulation were found in the tscd11 mutant at 35 °C. Fine mapping and DNA sequencing of tscd11 revealed a missense mutation (G to A) in the eighth exon of LOC_Os11g39670 resulted in amino acid change (Glu374 to Lys374). The TSCD11 gene encodes a seryl-tRNA synthetase localized to chloroplast. Complementation test confirmed that the point mutation in TSCD11 is responsible for the phenotype of tscd11. TSCD11 is highly expressed in leaves. Compared with the wild type (WT), mutation in TSCD11 led to significant alteration in expression levels of genes associated with chlorophyll biosynthesis, photosynthesis and chloroplast development under high temperature. CONCLUSIONS: TSCD11, encoding a seryl-tRNA synthetase localized to chloroplast, is vital to early chloroplast development at high temperature in rice, which help to further study on the molecular mechanism of chloroplast development under high temperature.

ABSTRACT

Indoleamine 2,3-dioxygenase 1 (IDO1) is emerging as a promising therapeutic target for the treatment of malignant tumors characterized by dysregulated tryptophan metabolism. However, the antitumor efficacy of existing small-molecule IDO1 inhibitors is still unsatisfactory, and the underlying mechanism remains largely undefined. To identify novel IDO1 inhibitors, an in-house natural product library of 2000 natural products was screened for inhibitory activity against recombinant human IDO1. High-throughput fluorescence-based screening identified 79 compounds with inhibitory activity > 30% at 20 µM. Nine natural products were further confirmed to inhibit IDO1 activity by > 30% using Ehrlich's reagent reaction. Compounds 2, 7, and 8 were demonstrated to inhibit IDO1 activity in a cellular context. Compounds 2 and 7 were more potent against IDO1 than TDO2 in the enzymatic assay. The kinetic studies showed that compound 2 exhibited noncompetitive inhibition, whereas compounds 7 and 8 were graphically well matched with uncompetitive inhibition. Compounds 7 and 8 were found to bind to the ferric-IDO1 enzyme. Docking stimulations showed that the naphthalene ring of compound 8 formed "T-shaped" π-π interactions with Phe-163 and that the 6-methyl-naphthalene group formed additional hydrophobic interactions with IDO1. Compound 8 was identified as a derivative of tanshinone, and preliminary SAR analysis indicated that tanshinone derivatives may be promising hits for the development of IDO1 inhibitors. This study provides new clues for the discovery of IDO1/TDO2 inhibitors with novel scaffolds.

ABSTRACT

OBJECTIVE: To investigate the efficacy and safety of regular oral use of sildenafil in the treatment of ED. METHODS: We randomly divided 334 ED patients into three groups to be treated orally with sildenafil tablets at 50 mg qd (sildenafil regular), sildenafil tablets at 100 mg 30 minutes before intercourse (sildenafil on-demand), and tadalafil tablets at 10 mg qd (tadalafil regular), all for 3 months. Then we recorded the IIEF-5 score and penile erection hardness score (EHS) and adverse reactions and compared them among the three groups of patients. RESULTS: There were no statistically significant differences among the three groups of patients in age, body mass index, education, ED duration, or baseline IIEF-5 and EHS (P > 0.05). After 3-month medication, both IIEF-5 score and EHS were significantly improved in the three groups of patients as compared with the baseline (P < 0.05), with no statistically significant difference in the IIEF-5 score among the sildenafil regular, sildenafil on-demand and tadalafil regular groups (15.15 ± 2.05 vs 15.55 ± 2.36 vs 15.54 ± 2.27, P > 0.05), but the EHS markedly higher in the sildenafil on-demand than in the sildenafil regular group (3.48 ± 1.80 vs 3.12 ± 1.52, P < 0.05). The effectiveness rates in the sildenafil regular, sildenafil on-demand and tadalafil regular groups were 76.2%, 62.4% and 80.8%, respectively, significantly lower in the sildenafil on-demand than in the other two groups (P < 0.05). Adverse reactions were mild and showed no statistically significant difference in the incidence rate among the three groups (P > 0.05). CONCLUSIONS: Regular use of sildenafil has a therapeutic effect similar to that of tadalafil but better than that of sildenafil on-demand, without more adverse effects.

ABSTRACT

The effect of blood magnesium disorder on chronic kidney disease (CKD) has been gradually confirmed in recent years. At present, magnesium homeostasis depends on dietary intake, gastrointestinal absorption and kidney metabolism. In recent years, it has been proposed that the magnesium concentration of dialysate and the influence of drugs (especially proton pump inhibitors) on magnesium homeostasis cannot be ignored in the treatment of CKD patients. Although the mechanism of the effect of magnesium on CKD has not been fully elucidated, recent studies suggest that magnesium deficiency may aggravate hypertension and vascular calcification in CKD patients, affecting mineral metabolism, leading to increased mortality. The application of different magnesium-based binders, such as magnesium citrate, calcium acetate/magnesium carbonate, and magnesium oxide, can help to alleviate the effect of low magnesium levels on CKD, which needs further research.
