ABSTRACT

Airway edema following burns is a typical occurrence. It poses a threat, independent of percent Total Burn Surface Area (TBSA), to the life of the patient. Fiber optic bronchoscopy is the gold standard in its diagnosis and is preferred if the facilities are present. Its availability remains a problem in the majority of burn centers in developing countries like India. Ascoring system based on clinical findings, if formulated in a manner that reflects bronchoscopy results, may help not only with diagnosis but also with airway management in inhalation burns. One hundred patients suffering from facial burns were included in the study. They were observed clinically and bronchoscopically and airway was managed on the basis of clinical, biochemical and bronchoscopic findings. Fifty patients who showed significant bronchoscopic findings on day 1 were followed up. Clinicobronchoscopic correlation revealed a positive correlation of various clinical variables as well as bronchoscopic grading with subsequent need for endotracheal intubation. Edema of tongue/floor of the mouth and palatal edema showed a positive correlation with subsequent need for tracheostomy. This clinicobronchoscopic correlation was then used retrospectively to formulate the Safdarjung Hospital 'INHALATION' score. This score can be used for predicting impending airway compromise when bronchoscopy facilities are not readily available.


La survenue d'un œdème des voies aériennes est un classique après une brûlure, entraînant un risque indépendant de la surface atteinte. L'endoscopie bronchique est l'examen diagnostique de référence et doit être réalisée si le matériel est disponible, ce qui est problématique dans les pays en développement dont l'Inde. Un score reflétant les données endoscopiques peut aider non seulement au diagnostic mais aussi au traitement des lésions d'inhalation. Cette étude a porté sur 100 patients ayant une brûlure faciale, ayant bénéficié d'un examen clinique et endoscopique et traités selon ces données et celles de la biochimie. La moitié des patients avaient des anomalies endoscopiques au premier examen. Les données cliniques (dont un œdème de la langue, du plancher buccal ou du palais) et endoscopiques prédisaient bien la nécessité d'une trachéotomie. Ces corrélations nous ont permis de développer le score INHALATION de l'hôpital Safdarjung, qui peut permettre de prédire une complication ventilatoire quand l'endoscopie n'est pas disponible.

Con el fi n de evaluar científi camente algunos de los usos tradicionales de la buganvilla (Bougainvillea glabra Choisy), se realizó el presente estudio para examinar los efectos antidiarreicos, antiulcerosos y antimicrobianos del extracto acuoso, etanólico y acetónico de sus hojas. Se probó la actividad antidiarreica en un modelo de diarrea inducida con aceite de ricino en ratas y se utilizó loperamida (3 mg/kg) como estándar de referencia. Se determinó la acción antiulcerosa mediante un modelo de úlcera inducida con alcohol y se utilizó omeprazol (10 mg/kg) como estándar. Ambos estudios se realizaron con dos niveles de dosis, 200 mg/kg y 400 mg/kg, respectivamente. La actividad antimicrobiana se estudió mediante un método de difusión en disco con una concentración de 500 ìg/disco de extracto, utilizando ofl oxacina (5 ìg/disco) como estándar. Los organismos utilizados fueron Escherichia coli, Bacillus subtilis, Klebsiella pneumoniae, Staphylococcus aureus y Proteus vulgaris, y se determinó la zona de inhibición. Los extractos de plantas mostraron una signifi cativa acción antidiarreica, antiulcerosa y antimicrobiana en el presente estudio. Los resultados obtenidos corroboran lo sostenido por los profesionales de la medicina locales


In order to scientifi cally appraise some of the folkloric uses of Bougainvillea glabra Choisy, the present study was undertaken to examine the anti-diarrhoeal, antiulcer and antimicrobial activities of acetone, ethanolic and aqueous extract of leaves. Anti-diarrhoeal activity was tested in Castor oil induced diarrhea model using rats and Loperamide (3 mg/kg) was used as reference standard. The antiulcer activity was determined using alcohol induced ulcer model and Omeprazole 10 mg/ kg was used as the standard. Both the studies were carried out at two dose level, 200 mg/kg and 400 mg/kg respectively. Antimicrobial activity was done by disc diffusion method at a concentration of 500 ìg/disc of the extract, using ofl oxacin (5 ìg/disc) as the standard. The organisms used were Escherichia coli, Bacillus subtilis, Klebsiella pneumoniae, Staphylococcus aureus and Proteus vulgaris and the zone of inhibition was determined. The plant extracts showed signifi cant anti-diarrhoeal, antiulcer and antimicrobial activities in the present study. The results obtained support the claim of local medical practitioners

ABSTRACT

Drug trial with CIBA 1906 was conducted in 50 cases of lepromatous leprosy who were intolerant to dapsone therapy. Drug was tolerated well and lepra reactions were infrequent and mild. Clinical improvement was seen in 72% cases whereas no appreciable change was detected in 28% cases. An average reduction of 0.4 in BI was detected in 62% cases, while remaining cases did not show any reduction in BI. No significant side effects were encountered during the study. Antibacterial activity of CIBA 1906 was not found superior to dapsone.
