ABSTRACT

BACKGROUND: Oblique course of some left accessory pathways is rare An incomplete electrophysiological study may confuse us between an oblique accessory pathway or the presence of two accessory pathways. The proximity of all atrial and ventricular electrograms, at each pole of the catheter, within the coronary sinus may be a novel finding. CASE PRESENTATION: A 68-year-old woman patient presented arrhythmias with hypotension requiring electrical cardioversion. Her electrocardiogram (ECG) was interpreted as atrial fibrillation by accessory pathway. We performed with the protocol of ablation stablished in our laboratory: two punctures on the right femoral vein with placement of introducers (8F and 7F) by Seldigner technique and one puncture on the left femoral vein (7F). The study was performed with BIOTRONIK technology (Multicath study catheter), a non-deflectable 7F quadripolar catheter with 2 mm tip electrode to record the His electrogram, a non-deflectable decapolar catheter with 5 pairs of coronary sinus (CS) electrodes. Accessory pathway mapping was performed in right and left cavities and within the CS. All electrograms into CS showed short AV from proximal to distal CS. Finally, ablation of two accessory pathway recordings was achieved at two distant epicardial points within the CS. CONCLUSIONS: Ablation at two distant sites, one on the ventricular side and the other on the mitral annulus, suggests the presence of an oblique accessory pathway and at the same time the differential diagnosis of the presence of two accessory pathways. In our point of view according the above, we consider this is a very rare case of oblique AP with epicardial trajectory. The sequence of electrograms (in this case) along the CS has not been seen before in the literature reviewed. It is important, regardless of the urgency, to follow diagnostic and therapeutic protocols in invasive electrophysiology.

RESUMEN La implementación de las Guías de Práctica Clínica, con intervenciones eficaces basadas en la evidencia, ha mejorado los resultados del manejo de pacientes con infarto agudo de miocardio con elevación del segmento ST en países de altos ingresos, sin embargo, no ocurre lo mismo en países con medianos y bajos ingresos, donde existen grandes brechas en su implementación, debido a múltiples factores. La presente Guía de Práctica Clínica se refiere a los pacientes que presentan síntomas isquémicos o sus equivalentes y una elevación persistente del segmento ST en el electrocardiograma. Esta guía no aborda los elementos relacionados directamente con los procedimientos de intervencionismo coronario percutáneo ni la revascularización miocárdica quirúrgica.


ABSTRACT The implementation of Clinical Practice Guidelines, with effective evidence-based interventions, has improved the results of managing patients with ST-segment elevation acute myocardial infarction in high-income countries, however, the same is not true in countries with medium and low incomes, where there are large gaps in its implementation, due to multiple factors. This Clinical Practice Guideline refers to patients presenting with ischemic symptoms or their equivalents and persistent ST-segment elevation on the electrocardiogram. This guideline does not address elements directly related to percutaneous coronary intervention procedures or surgical myocardial revascularization.

ABSTRACT

RESUMEN En Cuba, el primer implante de marcapasos se realizó el 6 de julio de 1964, mientras que en Cienfuegos se inicia esta actividad en 1979, por un grupo de profesionales liderados por el Dr. Francisco Riverón Mena, quien se mantuvo durante más de diez años al frente de la estimulación cardíaca, en el Hospital Dr. Gustavo Aldereguía Lima. El presente trabajo se propone describir el método para el implante de marcapasos permanente aplicado en dicha institución, introducido por el Dr. Riverón, como alternativa al modo convencional. Esta técnica requiere muy poca utilización de radiaciones ionizantes; resulta eficaz y más segura, tanto para el paciente como para el personal de salud, pues se emplea radioscopia solo en el último momento del procedimiento, lo cual la diferencia del método convencional. Aunque en la actualidad no se practica, durante la etapa que se aplicó en el hospital, brindó seguridad a pacientes y personal de salud, por lo que debería retomarse su uso.


ABSTRACT In Cuba, the first pacemaker implant was carried out on July 6, 1964, while in Cienfuegos this activity began in 1979, by a group of professionals led by Dr. Francisco Riverón Mena, who stayed for more than ten years in charge of cardiac stimulation, at the Dr. Gustavo Aldereguía Lima Hospital. The present work aims to describe the method for permanent pacemaker implantation applied in said institution, introduced by Dr. Riverón, as an alternative to the conventional mode. This technique requires very little use of ionizing radiation; It is effective and safer, both for the patient and for the health personnel, since fluoroscopy is used only at the last moment of the procedure, which differentiates it from the conventional method. Although it is not currently practiced, during the stage that it was applied in the hospital, it provided security to patients and health personnel, so its use should be resumed.
