ABSTRACT

INTRODUCTION: Traumatic spinal root injury caused by shoulder dislocation may involve the brachial plexus or, in some cases, a single nerve. The degree of severity of the injury depends on many patient-specific factors as well as the mechanism of injury. It is essential to suspect this type of lesion by means of a thorough physical examination in order to have better patient outcomes. CASE PRESENTATION: We presented the subtle magnetic resonance imaging (MRI) findings in a 35-yearold male with left shoulder trauma and dislocation after falling off a bicycle. He complained of decreased muscle strength and sensitivity in the C8 dermatome. Atrophy of the hypothenar region and flexion deformity of the 4th and 5th digits were noted. Magnetic resonance imaging findings were consistent with a partial preganglionic C8 motor root lesion. We found T2 increased signal intensity and thinning of the intradural segment of the C8 motor nerve root and low signal in the sequence of a multi- echo gradient recalled echo (GRE). CONCLUSION: MRI is a noninvasive tool that allows a detailed anatomical characterization of the nerves. In brachial plexus injuries, the use of the GRE sequence is useful to identify the lesions, even if they are subtle; however, some lesions may go unnoticed. It is important to note that these patients require an interdisciplinary group to reach a correct diagnosis, which is vital to establish the appropriate treatment and follow-up.

ABSTRACT

Introducción: la pandemia por el SARS-CoV-2 o COVID-19ha causado inmensas afectaciones a la población mundialy ha puesto al personal de la salud en primera plana porser los responsables de su manejo. Los médicos se hanvisto afectados en muchos aspectos (laboral, económico,salud física y mental), siendo importante identificar estasalteraciones. El objetivo de este trabajo fue evaluar lasimplicaciones que la pandemia ha tenido en los endoscopistas de nuestro país, comparándolo con la región y conun país europeo.Métodos: durante el mes de junio de 2021 se aplicó unaencuesta electrónica bajo el método de muestreo no probabilístico en la que participaron endoscopistas de la Asociación Colombiana de Endoscopia Digestiva (ACED); deEspaña y de Suramérica, evaluando el impacto de la pandemia en la práctica profesional, económica y académicay en la salud física y mental de los encuestados.Resultados: el 69,9 % de los encuestados informó disminución en el número de procedimientos. El 77,2 % de losparticipantes de Colombia informaron una caída en susingresos. El 43,8 % manifestó deterioro en la relación médico-paciente. El 56,5 % consideró que la pandemia ha afectado de forma negativa el entrenamiento de los residentes.La infección por COVID-19 se presentó en el 24,7 % de losendoscopistas y el 19,2 % perdió algún familiar. El 70,9 %de los encuestados informó cambios emocionales relacionados especialmente con tristeza y depresión. (AU)

Abstract Introduction: The practice of digestive endoscopy is a physically demanding activity, with musculoskeletal disorders present in 39 % to 89 % of endoscopists, associated with "excessive use" maneuvers. Due to a lack of knowledge of this problem in endoscopists in Colombia, the main objective is to determine the prevalence, types, and risk factors of musculoskeletal disorders in specialists and graduate students. The secondary objective is to identify the occupational impact, treatments used, and importance of prevention and education in ergonomics. Materials and methods: Analytical cross-sectional observational study. Electronic survey methodology, open from June 1 to 30, 2021. Purposive sampling of 450 endoscopists from four scientific associations and eleven graduate programs, including 50 questions in six groups according to the objectives. We validated 203 responses, with 131 confirmations of musculoskeletal disorders, the group on which the analysis was performed. Results: Global prevalence of musculoskeletal disorders of 64.5 % and prevalence in graduate students of 58.6 %. There was more significant involvement of the upper limbs (right shoulder, left thumb, right elbow), followed by lower back, neck, knees, and hips. Graduate students reported pain in the right hand/fingers (right thumb) and the lower back. There was no significant difference due to work factors, but there was a tendency for more reports when increasing the volume of procedures and years of professional practice. The labor impact showed 78 % absenteeism. The most used treatments were medication, physiotherapy, and rest; 93.8 % had not received ergonomic education. However, there is a positive perception (74.1 % to 90.9 %) of receiving formal training. Discussion: The prevalence reflected the health and safety problem for the endoscopist. Demographic risk factors plus those of the endoscopic practice give rise to an individualized risk framework that enables endoscopists to understand learning and training as a way to prevent musculoskeletal disorders in themselves and their work team.


Resumen Introducción: la práctica de la endoscopia digestiva es una actividad físicamente exigente, con trastornos musculoesqueléticos presentes en el 39 % a 89 %, asociados con maniobras de "uso excesivo". Debido al desconocimiento de este problema en endoscopistas de Colombia, el objetivo principal fue determinar la prevalencia, tipos y factores de riesgo del trastorno musculoesquelético en especialistas y estudiantes de posgrado. El objetivo secundario fue identificar el impacto laboral, los tratamientos utilizados y la importancia atribuida a la prevención y educación en ergonomía. Metodología: estudio observacional de corte transversal analítico. Metodología de encuesta electrónica, abierta del 1 al 30 de junio de 2021. Muestreo intencionado a 450 endoscopistas de 4 asociaciones científicas y 11 programas de posgrado, en el que se incluyeron 50 preguntas en 6 grupos según los objetivos. Se validaron 203 respuestas, con 131 confirmaciones de trastornos musculoesqueléticos, grupo al cual se le realizó el análisis. Resultados: prevalencia global de trastornos musculoesqueléticos de 64,5 % y prevalencia en estudiantes de posgrado de 58,6 %. Hubo mayor afectación de miembros superiores (hombro derecho, pulgar izquierdo, codo derecho); luego, en la espalda baja y cuello; y en las rodillas y caderas. En estudiantes de posgrado se registró dolor en la mano-dedos derechos (pulgar derecho) y en la espalda baja. No hubo diferencia significativa por aspectos laborales, pero hubo tendencia a un mayor reporte al aumentar el volumen de procedimientos y años de práctica profesional. El impacto laboral mostró 78 % de ausentismo laboral. Los tratamientos más empleados fueron medicamentos, fisioterapia y reposo. El 93,8 % no había recibido capacitación ergonómica y hay percepción positiva (74,1 % a 90,9 %) de recibir una didáctica formal. Discusión: la prevalencia evidenció el problema de salud y seguridad para el endoscopista. Los factores de riesgo demográficos más los propios de la práctica endoscópica generan un marco de riesgo individualizado que le permite al endoscopista entender el aprendizaje y la capacitación para prevenir trastornos musculoesqueléticos en sí mismo y su equipo de trabajo.

ABSTRACT

INTRODUCTION: the COVID-19 pandemic has seriously affected the world population, and has put health personnel in the spotlight as they are tasked with its management. Given their important role in the current health crisis, physicians have been heavily affected in terms of their professional practice, financial conditions and physical and mental health. The objective of this study was to determine the consequences of the COVID-19 pandemic for Colombian endoscopists and compare the effects on endoscopists from South America and from Spain. METHODS: in June 2021, an electronic survey was sent to 292 endoscopists from Colombia, South America and Spain to determine the impact of the pandemic on their professional practice, economic situation, physical and mental health, and training of the residents under their charge. Participants were recruited via convenience sampling. RESULTS: a reduction in the number of endoscopic procedures was reported by 69.9 % of the respondents, and 72.3 % of endoscopists in Colombia stated that their income had decreased. Regarding the doctor-patient relationship, 43.8 % of the respondents believed it had worsened. In addition, 70.9 % reported having experienced changes in their emotional state, especially those related to feeling sad or depressed, and 56.5 % reported that the pandemic had negatively affected the training of residents. Finally, 24.7 % of endoscopists tested positive for COVID-19, and 19.2 % reported the death of a family member due to COVID-19. CONCLUSION: the COVID-19 pandemic has had a negative impact on the professional practice, financial situation and physical and mental health of Ibero-American endoscopists. These problems must be identified in a timely manner to develop and implement strategies aimed to prevent their occurrence.

ABSTRACT

El presente estudio tuvo como objetivo identificar el desarrollo de la producción científica según año y revistas; determinar la estructura temática e identificar la red de coautoría de la literatura sobre micotoxinas en el análisis de los alimentos. Se realizó un estudio descriptivo con indicadores bibliométricos y mapas de visualización, en el que se usó la base de datos Scopus de los documentos incluidos desde el año 2011 al 2020. Se consideraron 453 artículos que cumplieron con los criterios de inclusión, 382 artículos de investigación y 71 revisiones. El promedio de citas por artículo varió anualmente entre 6 (2019) y 40,5 (2012). La revista más productiva fue Mycotoxin Research, de la editorial Springer Nature. En el mapa de coocurrencias se obtuvieron 5 clústeres de descriptores. En el mapa de coautoría se hallaron 21 clústeres, 14 de ellos correspondieron a autores que investigaron en coautoría. El desarrollo científico tuvo una producción anual no uniforme, con crecimiento lento. La estructura temática según la red de coocurrencia indica que el término más frecuente es micotoxinas, correspondiente al enfoque micotoxinas que influyen en la salud pública del primer clúster. La red de coautoría indica que el 98,2 por ciento de los artículos fueron elaborados en coautoría y el autor más productivo fue Krska Rudolf, de la Queen's University Belfast, con 24 artículos(AU)


The purpose of the study was to identify the development of scientific production by year and journal, determine the thematic structure and identify the co-authorship network of the literature about mycotoxins in food analysis. A descriptive study was conducted of documents published in the database Scopus from 2011 to 2020, using bibliometric indicators and visualization maps. A total 453 articles were included which met the inclusion criteria. Of these, 382 were research articles and 71 were reviews. The average annual number of citations per article ranged from 6 in 2019 to 40.5 in 2012. The most productive journal was Mycotoxin Research from Springer Nature publishers. The co-occurrence map displayed five descriptor clusters, whereas the co-authorship map displayed 21 clusters, 14 of which corresponded to authors researching in co-authorship. Scientific development had a non-uniform, slowly growing annual output. The thematic structure according to the co-occurrence network shows that the most frequent term is mycotoxins, corresponding to mycotoxins influencing public health from the first cluster. The co-authorship network shows that 98.2percent of the articles were co-authored, and the most productive author was Krska Rudolf, from Queen's University, Belfast, with 24 articles(AU)

BACKGROUND: Balance disorders are common in patients with neurological or vestibular diseases. Telerehabilitation program is a treatment to be as safe as conventional treatment. One of the most used methods to perform telerehabilitation is the incorporation of Virtual Reality. In general, rehabilitation programs train predictive postural control, so the patient does not always acquire the necessary autonomy to react to situations of instability. On the other hand, the objective and systematic supervision and measurement of these programs is limited, making it necessary to create clinical protocols with precise and measurable rehabilitation objectives. This study present the training selection methodology and clinical protocol for patients with balance disorders inserted in a Telerehabilitation Program based on Virtual Reality. METHODS: Descriptive study where physiotherapists were trained to use RehaMetrics®. To evaluate their level of agreement in the selection of the exercise clusters developed, the Interobserver Reliability was measured through the kappa statistic. Subsequently, the exercises were applied to a group of patients recruited with sedentary trunk control (Berg Balance Scale = 3 points in item 3), mild or normal cognitive level (Montreal Cognitive Assessment> 21 points), and prescribed for tele-rehabilitation by a doctor. RESULTS: The agreement among the expert physiotherapists irrespective of the cluster exceeds 80%, which indicates a very good strength of agreement, while the novices reached a level of agreement of 45%, which suggests a moderate strength of agreement. All clinical outcomes showed statistically significant differences between the median times, as did the Maximum Width Left Side (MWLS) (cm). The average number of minutes of training was 485.81 (SD 246.49 min), and the number of sessions performed during the 4 weeks of intervention was 17 (SD 7.15 sessions). CONCLUSIONS: This analysis what had excellent interobserver reliability with trained physiotherapists. Regarding the second phase of the study, the results show a statistically significant difference between the initial and final evaluation of the clinical tests, which could result in better performance in aspects such as: balance, gait functionality, meter walked and cognition. Telerehabilitation Program based on Virtual Reality is an excellent alternative to provide continuity of treatment to patients with balance disorders.

ABSTRACT

The injection of hyaluronic acid (HA), a naturally occurring biopolymer, is a common cosmetic procedure. Despite their efficacy and growing adaptation by the medical community, HA fillers occasionally give rise to adverse events. Adverse events from HA fillers range from temporary, such as edema and erythema, to more long-term effects, including granulomas or, in rare cases, sequelae from vascular occlusion. Here, we present a case of a 61-year-old Caucasian woman with prolonged infraorbital hollow edema after injection of HA filler for nasojugal groove correction. We review the anatomy of the nasojugal area and differing injection techniques as a possible explanation for the development of prolonged edema. Similarly, the rheological properties of the specific hyaluronic acid used during this procedure might be crucial in the development of this complication.

STUDY OBJECTIVE: To determine the surgical time, suture time, presence of postoperative dyspareunia, and complications that occur after closing the vaginal cuff with a barbed suture compared with conventional suture. DESIGN: A randomized, controlled clinical trial (Canadian Task Force classification I). SETTING: Private gynecologic clinic in Medellin, Colombia. PATIENTS: One hundred fifty women who underwent total laparoscopic hysterectomy for benign pathology. INTERVENTIONS: The patients underwent total laparoscopic hysterectomy with intracorporeal closure of the vaginal cuff and were randomized to 2 groups, 1 using a barbed suture (V-Loc 90; Medtronic/Covidien, New Haven, CT) and 1 using polyglactin 910 (coated Vicryl suture; Ethicon/Johnson & Johnson, New Brunswick, NJ). MEASUREMENTS AND MAIN RESULTS: The total operative time, closing time of the vaginal vault, presence of complications in the cuff, and incidence of postoperative dyspareunia were recorded. The patients were evaluated at a postoperative office visit 2 weeks after the procedure and by telephone interview at 24 weeks. Seventy-five patients were included in the barbed suture group and 75 patients in the polyglactin 910 group. The average time to complete the suture of the vaginal cuff was 12.01 minutes (± 5.37 standard deviation) for the barbed suture group versus 13.49 minutes (± 6.48) in the polyglactin 910 group (95% confidence interval, -.44 to 3.4; p = .130). Blood loss was 31.56 ± 22.93 mL in the barbed suture group versus 30.82 ± 21.75 mL in the polyglactin 910 group (95% confidence interval, -7.95 to 6.47; p = .840). The frequency of postoperative events such as hematoma, cellulitis, cuff dehiscence, fever, emergency consultation, and hospitalization was not statistically significant between groups. No statistically significant difference was found regarding deep dyspareunia at 24 postoperative weeks. CONCLUSION: No differences were found in surgical time or frequency of adverse events when comparing patients after vaginal cuff closure with barbed suture versus polyglactin 910.

Introducción. La esclerodermia es una enfermedad rara, de la cual existe información limitada en América Latina. Estudios preliminares en genética muestran que la ascendencia de República Dominicana tiene fuerte influencia africana, lo cual podría modular la expresión de la enfermedad. El propósito de este estudio es describir las características clínicas y demográficas de esclerodermia en una serie de pacientes dominicanos. Materiales y métodos. Se seleccionaron los pacientes que cumplieron con los criterios del EULAR/ACR para esclerosis sistémica de la base de datos del servicio de reumatología de un centro terciario. Se definieron los subtipos de esclerodermia de acuerdo a la clasificación EULAR. La información clínica y demográfica al momento del diagnóstico fue obtenida de forma retrospectiva de los expedientes médicos. Resultados. La prevalencia fue estimada de 9,3 por millón de habitantes. Veinte y seis pacientes entraron al estudio. La edad media al momento del primer síntoma fue 32,6 ± 15 años; el 68% de los pacientes tenía 40 años de edad o menos cuando aparecieron los síntomas. El 73,1% de los pacientes fue de sexo femenino, con una relación mujer:hombre 2,7:1. Los sistemas orgánicos más afectados fueron el pulmonar y el gastrointestinal; la afectación renal fue rara. Los anticuerpos anti-Scl-70 se encontraron positivos en el 64% de los casos y, en 2 casos, en coexistencia con anticentrómero. Conclusiones. La prevalencia de esclerosis sistémica es menor en la población dominicana que la reportada internacionalmente. La edad de inicio de la enfermedad parece ser menor en la población dominicana que la reportada en la literatura. Un patrón distinto de autoanticuerpos es observado en esta población (AU)

Preliminary genetic studies suggest a strong African ascendance in the Dominican Republic, which could modulate the expression of the disease. The objective of this study is to describe the clinical and demographic characteristics of scleroderma in a series of 26 Dominican patients. ABSTRACT

OBJECTIVE: To determine if transversus abdominis plane anesthetic blockage (TAP block) diminishes early postoperative pain scores and facilitates ambulatory management following total laparoscopic hysterectomy. DESIGN: Randomized triple blind trial. SETTING: Gynecological endoscopy unit at a referral center for laparoscopic surgery. POPULATION: A total of 197 patients. METHODS: Comparison of a treatment group receiving TAP block with bupivacaine 0.25% and placebo group with comparably placed bilateral injection of sterile saline solution. MAIN OUTCOME MEASURES: Pain scores at discharge 24, 48 and 72 h after surgery, opioid requirement after procedure. RESULTS: Patients who had TAP block had a significant reduction in their pain score at discharge compared with the placebo group (p = 0.017). There were no significant differences in the pain scores between groups at 24 h (95% CI 1.36-0.133, p = 0.237), 48 h (95% CI 0.689-0.465, p = 0.702) and 72 h (95% CI -0.631 to 0.223, p = 0.347). No differences were found between the groups regarding opioid requirements following the procedure (χ(2)  = 3.62, p = 0.46). CONCLUSION: Although TAP block after a total laparoscopic hysterectomy reduced the pain score at discharge compared with placebo, its role in this setting is debatable due to the possible lack of clinical significance of the small difference found.

ABSTRACT

Sciatic hernia constitutes the scarcest group of hernias of pelvic floor; however, they should be considered in the origin of chronic pelvic pain. The proper diagnosis of sciatic hernias has allowed to surgeons treating successfully patients with acute or chronic pelvic pain, with intestinal or ureteral obstruction and with urinary or gluteus sepsis. It has to be considered as differential diagnosis before the finding of a congenital or acquired gluteus mass. Laparoscopist gynecologist should know the existence of this defect, to be familiar with its aspect in laparoscopic view and to know the laparoscopic treatment of this disease.

ABSTRACT

Objetivo: Determinar la eficacia diagnóstica de la angiotomografía multidetectores en el diagnóstico del trauma arterial de las extremidades en pacientes con sospecha de lesión arterial sin indicación de cirugía inmediata. Métodos Durante 44 meses, entre agosto del 2004 y abril del 2008, se realizó angiotomografía multidetectores de 64 canales a 99 extremidades con sospecha de lesión vascular traumática. Los estudios fueron interpretados por el radiólogo de turno y sus hallazgos se compararon con los de cirugía o los del seguimiento clínico. Se evaluó la variabilidad interobservador comparando la lectura de la angiotomografía realizada por el radiólogo de turno con la lectura retrospectiva de un radiólogo experto en trauma. Resultados La angiotomografía multidetectores como método diagnóstico del trauma vascular de las extremidades interpretada por el radiólogo general demostró una sensibilidad del 98% (IC 95%: 93-100), una especificidad del 88% (IC 95%: 77-99), un valor predictivo positivo del 91% (IC 95%: 82-99), un valor predictivo negativo del 97% (IC 95%: 90-100), una razón de verosimilitud positiva de 8,24 (3,6-18,7) y una razón de verosimilitud negativa de 0,02 (0-0,15). La variabilidad interobservador comparando la interpretación de la angiotomografía del radiólogo de turno con la del radiólogo experto en trauma tuvo una kappa de 0,869.ConclusiónLa angiotomografía con multidetectores es un método de imagen con una alta precisión diagnóstica en el trauma arterial de las extremidades permitiendo un adecuado y oportuno enfoque terapéutico. Podría considerarse como nuevo patrón de oro para el diagnóstico del trauma arterial de extremidades (AU)

ABSTRACT

En este artículo se presenta el caso de una mujer de 29 años de edad con un cuadro clínico consistente en síndrome constitucional y linfopenia. En una tomografía se evidenciaron nódulos retroperitoneales, resecados quirúrgicamente, y se confirmó, mediante un estudio histológico, como enfermedad de Castleman tipo vascular hialino. Esta es una entidad de comportamiento generalmente benigno, poco frecuente, con hallazgos imagenológicos tomográficos y por resonancia magnética que sugieren la presencia de la enfermedad. En este artículo se revisan las características imagenológicas de esta enfermedad y sus posibles diagnósticos diferenciales.

ABSTRACT

Objetivo: hacer una revisión de la literatura sobre la utilidad de la laparoscopia en el tratamiento del absceso tubovárico en términos de mejoría, estancia y fertilidad posterior. Materiales y métodos: se presentan tres casos de pacientes con enfermedad pélvica inflamatoria severa que consultaron a una institución privada de referencia ubicada en Medellín (Colombia), a quienes se les practicó una laparoscopia operatoria, además de la administración de un antibiótico parenteral como manejo. Se realizó una búsqueda en la base de datos Medline vía PubMed con las palabras clave: “tubo-ovarian abscess, treatment, laparoscopy, fertility”. Se realizó además una búsqueda de textos clásicos y guías de manejo. Resultados: se encontraron 49 artículos de los cuales 13 eran relevantes al problema en cuestión. De estas publicaciones, 8 eran artículos de revisión, 4 series de casos, y 1 un estudio de cohortes retrospectivas. Conclusión: el manejo laparoscópico conservador del absceso tubovárico en el período inicial de la enfermedad, adicional a los antibióticos, surge como una alternativa que podría mejorar los resultados a corto y largo plazo. Se requieren estudios de alta calidad que respalden esta observación...


Objective: carrying out a literature review about the usefulness of laparoscopy in treating tubo-ovarian abscesses in terms of improvement, hospital stay and subsequent fertility.Materials and methods: three cases of patients suffering from severe pelvic inflammatory disease are presented; they consulted at a private reference institution in Medellín, Colombia, and were then submitted to laparoscopy, accompanied by parenteral antibiotics as management. A search was made in the Medline database via PubMed using the following key words: “tubo-ovarian abscess, treatment, laparoscopy, fertility.” A search was also made in classical texts on the subject and management guidelines. Results: 49 pertinent articles were found, of which 13 were relevant to the matter in hand; 8 were review articles, 4 were case-series and 1 was a retrospective cohort study. Conclusion: conservative laparoscopic management of tubo-ovarian abscess during the disease’s initial period, accompanied by the appropriate antibiotic regime, emerged as an alternative which could improve short- and long-term results. High-quality studies are thus needed to confirm such observation...
