ABSTRACT

Introduction: The rapid transmission and severe symptoms associated with acute chikungunya virus (CHIKV) infection in children make it a highly concerning health issue. Objective: This study aimed to describe the characteristics of acute chikungunya virus infection in children from the Department of Caaguazú, Paraguay. Material and Methods: A retrospective observational study was conducted in the Department of Caaguazú, Paraguay, in 2023, with all patients who came to the Regional Hospital within 5 days of developing characteristic symptoms of acute CHIKV infection and tested positive for the virus by RT-PCR. Patients with Dengue or Zika infections were excluded. We collected data on clinical characteristics using a standardized case record form and created an electronic dataset for analysis. Results: A total of 461 children were included in the study. 51.6% were women. Cases were divided into groups based on pediatric age: infants (0-23 months, n=88, 19.1%), preschoolers (2-5 years, n=115, 24.9%), schoolchildren (6-11 years, n=163, 35.4%), and adolescents (12-17 years and 11 months, n=95, 20.6%). Schoolchildren and adolescents experienced a higher prevalence of myalgia (64.6%) and arthralgia (63.7%). Vomiting (89%), headache (89.4%), and retro-orbital pain (95%) were more common in preschoolers, schoolchildren, and adolescents, while rash (39.5%) and petechiae (18.5%) were more prevalent in infants. Four children died during the study period. Conclusion: The 2023 CHIKV virus epidemic in Paraguay had different clinical presentations depending on the age of the affected children, with fatal outcomes occurring in a small percentage of cases.


Introducción: La rápida transmisión y los síntomas graves asociados con la infección aguda por el virus de la chikungunya (CHIKV) en niños lo convierten en un problema de salud altamente preocupante. Objetivo: Este estudio tuvo como objetivo describir las características de la infección aguda por el virus de la chikungunya en niños del Departamento de Caaguazú, Paraguay. Materiales y métodos: Un estudio retrospectivo fue realizado en el Departamento de Caaguazú, Paraguay, en 2023, con todos los pacientes que acudieron al Hospital Regional dentro de los 5 días posteriores al desarrollo de síntomas característicos de la infección aguda por CHIKV y que dieron positivo para el virus mediante RT-PCR. Pacientes con infecciones por Dengue o Zika fueron excluidos. Se recogieron datos sobre las características clínicas mediante un formulario normalizado de registro de casos y se creó un conjunto de datos electrónicos para su análisis. Resultados: Se incluyeron en el estudio un total de 461 niños. 51,6% eran mujeres. Los casos se dividieron en grupos según la edad pediátrica: lactantes (0-23 meses, n=88, 19,1 %), preescolares (2-5 años, n=115, 24,9%), escolares (6-11 años, n=163, 35.4%), and adolescentes (12-17 años and 11 meses, n=95, 20.6 %). La mialgia (64,6%) y la artralgia (63.7%) fueron más prevalentes en escolares y adolescentes. El vómito (89%), la cefalea (89,4%) y el dolor retroorbitario (95%) fueron más comunes en preescolares, escolares y adolescentes, mientras que la erupción cutánea (39,5%) y las petequias (18,5%) fueron más prevalentes en lactantes. Cuatro niños fallecieron durante el período de estudio. Conclusión: La epidemia de virus de la CHIKV de 2023 en Paraguay tuvo diferentes presentaciones clínicas dependiendo de la edad de los niños afectados, con resultados fatales ocurriendo en un pequeño porcentaje de casos.

Introducción: El virus chikungunya (CHIKV) causa una enfermedad con manifestaciones agudas bien documentadas, pero existen pocos datos sobre la persistencia de síntomas y secuelas a largo plazo. Objetivos: Evaluar la persistencia de signos, síntomas y factores asociados en pacientes con infección por CHIKV en un seguimiento de 6 meses posinfección. Materiales y métodos: Estudio de cohorte ambispectivo que incluyó casos confirmados de CHIKV notificados en abril de 2023 en Coronel Oviedo, Paraguay. Se realizaron entrevistas al mes, a los tres meses y a los 6 meses posinfección. Se analizaron características demográficas, comorbilidades, manifestaciones reumáticas y sintomáticas. Resultados: Ingresaron 333 participantes, completando seguimiento a 6 meses 170 pacientes. A los 6 meses persistieron: dolor de espalda (61.2%), artritis (57.1%), cefalea (54.1%), fatiga (51.2%), mialgia (49.4%), debilidad (47.6%), depresión (45.9%) y artralgia (45.9%). La persistencia de artritis (OR 7.54; p=0.008) y mialgia (OR 3.24; p=0.031) a 6 meses fue mayor en el grupo de 36-45 años. Conclusiones: Alta persistencia de síntomas musculoesqueléticos y fatiga hasta 6 meses posinfección, con tendencia decreciente, pero exacerbación de depresión. Edad entre 36-45 años asociada a mayor persistencia. Se requieren protocolos de seguimiento integral, investigar mecanismos fisiopatológicos y fortalecer prevención, dado el impacto individual y socioeconómico de la enfermedad.


Introduction: Chikungunya virus (CHIKV) causes a disease with well-documented acute manifestations, but there is limited data on the persistence of symptoms and long-term sequelae. Objectives: To evaluate the persistence of signs, symptoms, and associated factors in patients with CHIKV infection during a 6-month post-infection follow-up. Material and Methods: An ambispective cohort study that included confirmed cases of CHIKV reported in April 2023 in Coronel Oviedo, Paraguay. Interviews were conducted at 1, 3 and 6 months post-infection. Demographic characteristics, comorbidities, rheumatic, and symptomatic manifestations were analyzed. Results: 333 participants were enrolled, with 170 patients completing the 6-month follow-up. At 6 months, the following symptoms persisted: back pain (61.2%), arthritis (57.1%), headache (54.1%), fatigue (51.2%), myalgia (49.4%), weakness (47.6%), depression (45.9%), and arthralgia (45.9%). The persistence of arthritis (OR 7.54, p=0.008) and myalgia (OR 3.24, p=0.031) at 6 months was higher in the 36-45 age group. Conclusions: High persistence of musculoskeletal symptoms and fatigue up to 6 months post-infection, with a decreasing trend but exacerbation of depression. Age between 36-45 years was associated with greater persistence. Comprehensive follow-up protocols, investigation of pathophysiological mechanisms, and strengthening prevention are required, given the individual and socioeconomic impact of the disease.

BACKGROUND: Systemic lupus erythematosus (SLE) often mimics symptoms of other diseases, and the interval between symptom onset and diagnosis may be long in some of these patients. Aims: To describe the characteristics associated with the time to SLE diagnosis and its impact on damage accrual and mortality in patients with SLE from a Latin American inception cohort. METHODS: Patients were from a multi-ethnic, multi-national Latin-American SLE inception cohort. All participating centers had specialized lupus clinics. Socio-demographic, clinical/laboratory, disease activity, damage, and mortality between those with a longer and a shorter time to diagnosis were compared using descriptive statistical tests. Multivariable Cox regression models with damage accrual and mortality as the end points were performed, adjusting for age at SLE diagnosis, gender, ethnicity, level of education, and highest dose of prednisone for damage accrual, plus highest dose of prednisone, baseline SLEDAI, and baseline SDI for mortality. RESULTS: Of the 1437 included in these analyses, the median time to diagnosis was 6.0 months (Q1-Q3 2.4-16.2); in 721 (50.2%) the time to diagnosis was longer than 6 months. Patients whose diagnosis took longer than 6 months were more frequently female, older at diagnosis, of Mestizo ethnicity, not having medical insurance, and having "non-classic" SLE symptoms. Longer time to diagnosis had no impact on either damage accrual (HR 1.09, 95% CI 0.93-1.28, p = 0.300) or mortality (HR 1.37, 95% CI 0.88-2.12, p = 0.200). CONCLUSIONS: In this inception cohort, a maximum time of 24 months with a median of 6 months to SLE diagnosis had no apparent negative impact on disease outcomes (damage accrual and mortality).

Introducción: el tratamiento nutricional está afectado por la conducta de los sujetos para generar adherencia. Objetivo: determinar factores que influyen en la no adherencia al tratamiento nutricional en pacientes hipertensos que acuden al Hospital Regional de Coronel Oviedo, 2021. Metodología: estudio descriptivo transversal. La población estuvo comprendida por pacientes registrados en el Programa de hipertensión arterial/Departamento cardiovascular del Hospital Regional de Coronel Oviedo. Se estudiaron los datos sociodemográficos, los factores de tratamiento nutricional y el grado de adherencia al tratamiento nutricional. Resultados: participaron del estudio 206 sujetos, el 53,4 % poseía 55 años o menos y el 81,1 % fue del sexo femenino. Se pudo hallar que solo el 3,8 % de los pacientes se adhieren al tratamiento. La no adherencia al factor conocimiento estuvo relacionado con el bajo nivel educativo (p=0,032), al factor equipo de salud con provenir del área rural y tener un bajo nivel educativo (p=0,006, p=0,002), al factor paciente con provenir del área rural (p=0.002), ser de bajo nivel educativo (p=0,008) y poseer obesidad grado II y III (p=0,036). La no adherencia global estuvo relacionada a estar casado (p=0.001) y realizar trabajos domésticos (p=0,009). Conclusiones: la adherencia al tratamiento es baja en la población de estudio.


Introduction: nutritional treatment is affected by the behavior of the subjects to generate adherence. This study was carried out to determine factors that influence non-adherence to nutritional treatment in hypertensive patients who attend the Coronel Oviedo Regional Hospital, 2021. Methodology: this was a cross-sectional descriptive observational study. The population was comprised of patients registered in the hypertension department of the Coronel Oviedo Regional Hospital. Sociodemographic data, nutritional treatment factors and the degree of adherence to nutritional treatment were studied. Results: 206 subjects participated in the study, 53.4 % were 55 years old or younger and 81.1 % were female. We found that only 3.8 % of patients adhere to treatment. Non-adherence due to the knowledge factor was related to low educational level (p=0.032), to the health team factor with coming from a rural area and having a low educational level (p=0.006, p=0.002), to the patient factor with coming from a rural area (p=0.002), to have a low educational level (p=0.008) and to have obesity grade II and III (p=0.036). Global non-adherence was related to being married (p=0.001) and doing housework (p=0.009). Conclusions: adherence to treatment is low in the study population.

Introducción. La infección por el virus del Chikunguña (CHIKV) se presenta típicamente como una enfermedad febril aguda que es autolimitada y se caracteriza predominantemente por poliartralgia y mialgias graves. Objetivo. el propósito de este estudio fue describir las características clínicas de la infección aguda por el virus del Chikunguña en una población adulta. Materiales y Método. Este fue un estudio observacional, descriptivo y transversal. La población de estudio incluyó adultos de una región específica de Paraguay durante la epidemia de CHIKV en 2022-2023. Los criterios de inclusión abarcaron a todos los pacientes que se presentaron el Hospital Regional dentro de los cinco días posteriores al desarrollo de síntomas típicos de CHIKV, confirmados por RT-PCR, y tras excluir dengue y Zika. Resultados. Se incluyeron un total de 803 adultos, con una edad media de 45,13 ± 18.63 años y una proporción de género masculino/femenino de 1:1.79. En las semanas epidemiológicas 8, 10 y 11 se documentaron 78, 120 y 132 casos, respectivamente. Las manifestaciones clínicas más comunes fueron fiebre en el 97% de los casos (n=583), artralgia y artritis en el 86,03% (n=653), mialgias en el 77,08% (n=585) y cefalea en el 73,91% (n=561). Hubo 6 muertes (0,75% del total de casos). Conclusión. La presentación clínica de CHIKV durante la epidemia 2023 en Paraguay es consistente con descripciones previas en otras regiones. La enfermedad generó un número significativo de consultas médicas y llevó a la saturación del sistema de salud pública, siendo fatal en un pequeño porcentaje de casos. Introduction. Chikungunya virus (CHIKV) infection typically presents as an acute febrile illness that is self-limiting and predominantly characterized by severe polyarthralgia and myalgia. Objective.The purpose of this study was to describe the clinical characteristics of acute chikungunya virus infection in an adult population. Materials and Methods.This was a descriptive, cross-sectional, observational study. The study population included adults from a specific region of Paraguay during the CHIKV epidemic in 2022-2023. Inclusion criteria encompassed all patients who presented to the hospital within five days after developing typical symptoms of acute CHIKV infection, confirmed by RT-PCR, and after excluding the presence of dengue and Zika. Results. A total of 803 adults were included, with an average age of 45.13 ± 18.63 years and a male/female ratio of 1:1.79. At epidemiological weeks 8, 10, 11, 78, 120, and 132 cases were documented, respectively. The most common clinical manifestations were fever in 97% of the cases (n=583), arthralgia and arthritis in 86.03% (n=653), myalgias in 77.08% (n=585), and headache in 73.91% (n=561). There were 6 deaths (0.75% of the total cases). Conclusion. The clinical presentation of CHIKV during the 2023 epidemic in Paraguay is consistent with previous descriptions in other regions. The disease generated a significant number of medical consultations and led to saturation of the public health system, which was fatal in a small percentage of cases. Fruit by-products are a source of biocompounds with antioxidant properties and potential role in the obesity treatment. This study aimed to assess the effect of pomegranate (Punica granatum) peel (PP) supplementation on the total antioxidant capacity (TAC) in diet-induced obese rats. Thus, an in vitro gastrointestinal digestion was performed to evaluate the total phenolic content (TPC) and the antioxidant capacity of PP. Moreover, 15 male Wistar rats were randomized into three groups: control diet (CTL; 3.35 kcal/g), cafeteria (CAF) diet (3.72 kcal/g), and CAF diet supplemented with PP (CAF + PP; 200 mg/kg body weight; 3.72 kcal/g). Serum TAC was analyzed by ferric reducing antioxidant power and 2,2-Diphenil-1-picrylhydrazil assay. TPC in PP accounted for 8.82 ± 0.14 mg GAE/g in undigested samples. However, an in vitro digestion process was decreased by 94% the bioaccessibility of PP phenolic compounds in the intestinal phase, while PP supplementation increased serum TAC in diet-induced obese rats. Therefore, although PP phenolic compounds diminished after an in vitro digestion process, antioxidant effect was found in obese rats supplemented with PP.

Introducción: la donación de órganos sigue siendo un problema incluso en aquellos sistemas más exitosos en la detección de donantes. Objetivo: identificar la actitud hacia la Ley Anita en estudiantes de la Facultad de ciencias de la salud Universidad Nacional del Caaguazú, año 2021. Metodología: estudio observacional, tipo descriptivo y de corte transversal. Se llevó a cabo en la Facultad de Ciencias de la Salud, de la Universidad Nacional de Caaguazú, en el periodo comprendido durante los meses de junio a agosto del año 2021. El instrumento fue un cuestionario de preguntas cerradas con opciones múltiples. Resultado: en cuanto a la actitud respecto a la donación de órganos el 81.9 % donaría sus órganos y el 95.1 % donaría sus órganos si es un familiar el que necesita del mismo, el 69.8 % afirmó donarían los órganos de sus familiares. La opinión de la familia es favorable para el 43.4 % es favorable mientras que el 52.2 % de la población manifestó que su religión condice con la donación de órganos. El 92.3 % está a favor de la ley Anita. Conclusión: la actitud favorable hacia la donación de órganos estuvo relacionada con la opinión favorable de la familia y de la religión, además de ser positiva para los que están de acuerdo con la donación de sangre. La opinión de la familia y la religión influye también en la actitud favorable hacia la donación de órganos de familiares, además de asociarse hacia la actitud favorable hacia la Ley Anita.


Introduction: organ donation continues to be a problem even in the most successful systems in donor detection. Objective: to identify the attitude towards the Anita Law in students at the College of Health Sciences, Universidad Nacional de Caaguazú, during 2021. Methodology: this was an observational, descriptive and cross-sectional study. It was carried out at the College of Health Sciences, Universidad Nacional de Caaguazú, from June to August, 2021. The instrument was a questionnaire of closed questions with multiple-choice answers. Result: regarding attitudes towards organ donation, 81.9 % would donate their organs and 95.1 % would donate their organs if a family member required so, 69.8 % said they would donate the organs of their relatives. Family opinions were favorable for 43.4 % while 52.2 % of the population stated that their religious denomination agrees with organ donation. 92.3 % support the Anita law. Conclusion: the favorable attitude towards organ donation was related to the favorable opinions of the family and religious denomination, in addition to being positive for those who agree with blooddonation. The opinion of the family and religious denomination also influences the favorable attitude towards family organ donation, in addition to being associated with a favorable attitude towards the Anita Law.

OBJECTIVES: To evaluate whether simplification of antiretroviral treatment to dual therapy (DT) negatively impacts immune recovery (IR), immune activation and inflammation (IA/I), and HIV reservoir. METHODS: An open-label, single-centre, randomized controlled trial conducted in adult virologically suppressed HIV-infected patients on triple therapy (TT) with elvitegravir-cobicistat, emtricitabine and tenofovir alafenamide or dolutegravir (DTG), abacavir, and lamivudine (3TC). Participants were randomized to continue TT or switch to DTG, or darunavir/cobicistat (DRVc) plus 3TC. IR was assessed by CD4+/CD8+ ratio at 48 and 96 weeks. Changes in immune activation, proliferation, exhaustion, senescence, and apoptosis in CD4+ and CD8+ T cells, plasma sCD14, hsCRP, D-dimers, ß2-microglobulin, IL-6, TNF-α and IP-10 levels, cell-associated HIV-DNA (CA-DNA), and unspliced HIV-RNA (usRNA) were also analysed. RESULTS: One hundred and fifty-one participants were enrolled. Fourteen patients did not complete the follow up. In the ITT and PP analysis, the IR was similar between the treatment arms. In the ITT analysis, the median increase in CD4+/CD8+ ratio was 0.10, 0.04, and 0.07 at week 48, and 0.09, 0.05, and 0.08 at week 96 for TT, DTG/3TC, and DRVc/3TC, respectively. After adjusting for confounding factors, the slopes of changes in CD4+/CD8+ ratio over time were independent of treatment (F = 1.699; p = 0.436) and related only to baseline values (F = 756.871; p = 0.000). There were no differences in IA/I, CA-DNA, or usRNA between treatment arms. DISCUSSION: Both IR and IA/I, CA-DNA, and usRNA were similar in the three treatment groups, regardless of maintaining TT or simplifying to DTG/3TC or DRVc/3TC in virologically suppressed HIV-infected patients.

ABSTRACT

The obesity pandemic has been strongly associated with the Western diet, characterized by the consumption of ultra-processed foods. The Western lifestyle causes gut dysbiosis leading to impaired fatty acid metabolism. Therefore, this study aimed to evaluate shifts in gut microbiota and correlate these with serum fatty acid profiles in male Wistar rats fed a cafeteria diet. Ten male rats were fed with standard diet (CTL, n = 5) and cafeteria diet (CAF, n = 5) for fifteen weeks. Body weight and food intake were recorded once and three times per week, respectively. At the end of the study, fresh fecal samples were collected, tissues were removed, and serum samples were obtained for further analyses. Gut microbiota was analyzed by sequencing the V3-V4 region of 16S rRNA gene. Serum fatty acid profiles were fractioned and quantified via gas chromatography. The CAF diet induced an obese phenotype accompanied by impaired serum fatty acids, finding significantly higher proportions of total saturated fatty acids (SFAs) and C20:3 n-6, and lower C18:1 n-7 and C18:3 n-3 in the phospholipid (PL) fraction. Furthermore, circulating C10:0, total n-3 and n-7 decreased and total monounsaturated fatty acids (MUFAs), including oleic acid C18:1 n-9, increased in the cholesterol ester (CE) fraction. The obesity metabotype may be mediated by gut dysbiosis caused by a cafeteria diet rich in C16:0, C18:0, C18:1 n-9 and C18:2 n-6 fatty acids resulting in a 34:1 omega-6/omega-3 ratio. Therefore, circulating C10:0 was associated with several genera bacteria such as Prevotella (positive) and Anaerotruncus (negative). Two classes of Firmicutes, Bacilli and Erysipelotrichi, were positively correlated with PL- C20:3 n-6 and CE- 18:1 n-9, respectively. TM7 and Bacteroidetes were inversely correlated with PL-SFAs and CE- 18:2 n-6, respectively.

ABSTRACT

Mosaic loss of the Y chromosome (LOY) is the most frequent chromosomal aberration in aging men and is strongly correlated with mortality and disease. To date, studies of LOY have only been performed in humans, and so it is unclear whether LOY is a natural consequence of our relatively long lifespan or due to exposure to human-specific external stressors. Here, we explored whether LOY could be detected in rats. We applied a locus-specific PCR and target sequencing approach that we used as a proxy to estimate LOY in 339 samples covering eleven tissues from young and old individuals. We detected LOY in four tissues of older rats. To confirm the results from the PCR screening, we re-sequenced 60 full genomes from old rats, which revealed that the Y chromosome is the sole chromosome with low copy numbers. Finally, our results suggest that LOY is associated with other structural aberrations on the Y chromosome and possibly linked to the mosaic loss of the X chromosome. This is the first report, to our knowledge, demonstrating that the patterns of LOY observed in aging men are also present in a rodent, and conclude that LOY may be a natural process in placental mammals.

ABSTRACT

The coronavirus disease 2019 (COVID-19) pandemic has affected most countries in the world. Studying the evolution and transmission patterns in different countries is crucial to enabling implementation of effective strategies for disease control and prevention. In this work, we present the full genome sequence for 17 SARS-CoV-2 isolates corresponding to the earliest sampled cases in Mexico. Global and local phylogenomics, coupled with mutational analysis, consistently revealed that these viral sequences are distributed within 2 known lineages, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) lineage A/G, containing mostly sequences from North America, and lineage B/S, containing mainly sequences from Europe. Based on the exposure history of the cases and on the phylogenomic analysis, we characterized 14 independent introduction events. Additionally, three cases with no travel history were identified. We found evidence that two of these cases represented local transmission cases occurring in Mexico during mid-March 2020, denoting the earliest events described for the country. Within this local transmission cluster, we also identified an H49Y amino acid change in the Spike protein. This mutation represents a homoplasy occurring independently through time and space and may function as a molecular marker to follow any further spread of these viral variants throughout the country. Our results provide a general picture of the SARS-CoV-2 variants introduced at the beginning of the outbreak in Mexico, setting the foundation for future surveillance efforts.IMPORTANCE Understanding the introduction, spread, and establishment of SARS-CoV-2 within distinct human populations as well as the evolution of the pandemics is crucial to implement effective control strategies. In this work, we report that the initial virus strains introduced in Mexico came from Europe and the United States and that the virus was circulating locally in the country as early as mid-March. We also found evidence for early local transmission of strains with a H49Y mutation in the Spike protein, which could be further used as a molecular marker to follow viral spread within the country and the region.

ABSTRACT

In this work, a nondestructive and noninvasive technique, based on laser technology and the use of the Kubelka-Munk model to calculate the dynamic behavior of the cement paste from the diffuse reflection properties of both cement components and hydration products, is proposed. Also, the Powers-Brunauer model is used to explain this behavior during the first 9 h of the hydration process. This method allows us to obtain the initial and final cement setting times from the diffuse reflection measurements.

ABSTRACT

Las afecciones del embarazo en general, y la preeclampsia en particular, son problemas que habitualmente se tratan en las unidades de recuperación postanestesia y de hospitalización. La preeclampsia es la forma más frecuente de hipertensión arterial asociada al embarazo (50%), que afecta del 7% al 10% de las mujeres gestantes. Se define como un síndrome multisistémico del embarazo y puerperio, en el que se presenta una reducción de la perfusión sistémica generada por vasoespasmo y activación de los sistemas de coagulación. Se presenta un caso clínico del postoperatorio inmediato de una paciente intervenida de cesárea urgente tras ser diagnosticada de preeclampsia, elaborando un plan de cuidados enfermeros basado en los patrones funcionales de M. Gordon y guiado por la taxonomía NANDA-NOC-NIC, donde se identifican 6 diagnósticos de Enfermería, que serán la base de la realización de este proceso enfermero: riesgo de infección, exceso de volumen de líquidos, riesgo de sangrado, conocimientos deficientes sobre su proceso patológico, dolor agudo y ansiedad. La aplicación de este plan de cuidados supone una mejora en la atención al paciente y en la organización del trabajo

The application of this care plan leads to an improvement in the patient care and in the work organization

PURPOSE: Despite the high prevalence of respiratory diseases in the world and the extensive information available on the mucosal immune system, research on the development of the lung immune system in humans is limited by technical and ethical considerations; therefore, we studied the postnatal development of T lymphocytes in lung lobes in a porcine model. METHODS: Using less than 36-hour-old (NB), 1-week-weaned (5-week-old -AW-), 3-month-old (3M), and 4-year-old (4YR) healthy, nonvaccinated, specific pathogen free (SPF) Vietnamese miniature pigs, we studied the CD3+, CD4+, CD8+, TCR1 (gamma-delta T cells), and CD25+ (IL-2R-alpha) cell subpopulations in lung lobes parenchyma, bronchoalveolar lavage (BAL), peripheral blood mononuclear cells (PBMC), and cervical lymph nodes (LN) by flow cytometry. RESULTS: No differences among lung lobes were detected in any of the cell subpopulations tested. A low proportion of T cell subsets was detected in NB and 4YR groups in lung and BAL. Besides, the AW and 3M groups showed important changes in T cell subpopulations. CONCLUSIONS: These results suggest that in healthy animals the lung lobes behave as a homogeneous immune organ. T cells were detected in very low percentages at birth and in adult life, which may explain the high susceptibility to respiratory infections both early and later in life. Postweaning antigenic challenges and endocrine and sexual maturity at 3M had important effects on the development of the mucosal immune system. It was also evident that changes at mucosal sites were poorly correlated with PBMC and LN.

ABSTRACT

OBJECTIVE: Determine whether remote ischaemic postconditioning (RIP) protects against percutaneous coronary intervention-related myocardial infarction (PCI-MI). DESIGN: Single-centre, randomised, blinded to the researchers, clinical trial. ClinicalTrials.gov (NCT 01113008). SETTING: Tertiary hospital centre. PATIENTS: 232 patients underwent elective PCI for stable or unstable angina. INTERVENTIONS: Patients were randomised to RIP (induction of three 5-min cycles of ischaemia in the arm after the PCI) versus placebo. MAIN OUTCOME MEASURES: The primary outcome measure was the peak 24-h troponin I level. PCI-MI was defined by an elevation of troponin values >3 or >5 of the 99th percentile according to the classical or the new definition. The secondary outcome measure was hospital admission, PCI for stable angina or acute coronary syndrome and mortality after 1 year of follow-up. The use of RIP in diabetic patients was specifically studied. RESULTS: The mean age was 64.6 years, and 42% were diabetic. The peak troponin in the RIP patients was 0.476 vs 0.478 ng/mL (p=0.99). PCI-MI occurred in 36% of the RIP patients versus 30.8% in the placebo group (p=0.378). Diabetic RIP patients had more PCI-MI (new definition): OR 2.7; 95% CI 1.10 to 6.92; p=0.027. The secondary outcome measure was seen in 11.7% of the RIP patients versus 10.8% in the placebo group (p=0.907). CONCLUSIONS: RIP did not reduce the damage associated with elective PCI or cardiovascular events during the follow-up. The diabetic population who underwent RIP had more PCI-MI.

ABSTRACT

For the first time to our knowledge, carbon implantation into YAG crystals doped with Nd has been used to produce optical waveguides. A considerable index decrease in the nuclear region (i.e., the region where the energetic ions stop) of ~ 2.5% was obtained with a low dose implant, while giving an index enhancement in the guiding region of ~ 0.35%. After an annealing step necessary to recover the transparency of the crystals, the layer of reduced refractive index produced by implantation is preserved. Spectroscopic studies carried out in a waveguiding configuration show that emission bands coming from the 4F3/2 level present a slight broadening, while its associated lifetime is similar to that reported in bulk crystals (240 ms).
