ABSTRACT

OBJECTIVE: To evaluate the utility of low-cost simulation models to teach surgical techniques for placenta accreta spectrum (PAS), included in a multimodal education workshop for PAS. METHODS: This was an observational, survey-based study. Participants were surveyed before and after the use of low-fidelity mannequins to simulate two surgical techniques for PAS (one-step conservative surgery [OSCS] and modified subtotal hysterectomy [MSTH]), within a multimodal educational workshop. The workshops included pre-course preparation, didactics, simulated practice of the techniques using low-cost models, and viewing live surgery. RESULTS: Six OSCS/MSTH training workshops occurred across six countries and a total of 270 participants were surveyed. The responses of 127 certified obstetricians and gynecologists (OB-GYNs) were analyzed. Participants expressed favorable impressions of all components of the simulated session. Perceived anatomical simulator fidelity, scenario realism, educational component effectiveness, and self-assessed performance improvement received ratings of 4-5 (positive end of the Likert scale) from over 90% of respondents. When asked about simulation's role in technique comprehension, comfort level in technique performance, and likelihood of recommending this workshop to others, more than 75% of participants rated these aspects with a score of 4-5 (positively) on the five-point scale. CONCLUSION: Low-cost simulation, within a multimodal education strategy, is a well-accepted intervention for teaching surgical techniques for PAS.

ABSTRACT

Fundamento: La colocación de un catéter periférico es la alternativa a una vía venosa periférica para alcanzar sitios distantes de la punción como las inmediaciones de la desembocadura de la vena cava superior, el cual posee las características de ser notablemente flexible y bastante atraumático. Sin embargo, dicha técnica posee el inconveniente de la posible ruptura o fragmentación del catéter. Presentación de caso: Se presenta un caso de una paciente intervenida quirúrgicamente por un embarazo ectópico no complicado a la que se le coloca en la vena cefálica del brazo derecho un catéter intravenoso ORX en silicona. Finalizada la operación se retira el mismo el cual no se obtiene completamente debido a la ruptura espontánea y rápidamente se determina su extracción quirúrgica con una recuperación satisfactoria. Conclusiones: La ruptura espontánea de catéter venoso periférico resulta rara en los reportes científicos actuales, la conducta ante esta complicación se subdivide en: profiláctica y terapéutica(AU)


Background: The placement of a peripheral catheter is the alternative to a peripheral vein to reach distant sites of the puncture as close to the vena cava, which has the characteristics of being remarkably flexible and rather atraumatic. However, this technique has the disadvantage of breakage or fragmentation of the catheter. Case presentation: We report a case of a patient undergoing surgery for an ectopic pregnancy uncomplicated which is placed in the cephalic vein of an IV right arm silicone ORX. After the operation is removed the same which is not achieved completely due to spontaneous rupture and quickly determines the surgical removal of a successful recovery. Conclusions: Spontaneous rupture of peripheral venous catheter is rare in today's scientific reports; the conduct in this complication is subdivided into prophylactic and therapeutic(AU)
