ABSTRACT

Abstract Biofilm formation by Bacillus cereus strains is now recognized as a systematic contaminaron mechanism in foods; the aim of this study was to evaluate the production of submerged and interface biofilms in strains of B. cereus group in different materials, the effect of dex-trose, motility, the presence of genes related to biofilms and the enterotoxigenic profile of the strains. We determine biofilm production by safranin assay, motility on semi-solid medium, toxin gene profiling and genes related to biofilm production by PCR in B. cereus group iso-lated from food. In this study, we observe strains used a higher production of biofilms in PVC; in the BHI broth, no submerged biofilms were found compared to phenol red broth and phenol red broth supplemented with dextrose; no strains with the ces gene were found, the enterotoxin profile was the most common the profile that includes genes for the three enterotoxins. We observed a different distribution of tasA and sipW with the origin of isolation of the strain, being more frequent in the strains isolated from eggshell. The production and type of biofilms are differential according to the type of material and culture medium used.


Resumen La formación de biopelículas por cepas de Bacillus cereus es reconocida como un mecanismo de contaminación sistemática en alimentos; el objetivo del estudio fue evaluar la producción de biopelículas sumergidas y de superficie en cepas del grupo de Bacillus cereus en diferentes materiales, el efecto de la dextrosa, la motilidad, la presencia de genes relacionados a biopelículas y el perfil enterotoxigénico de las cepas. Determinamos la producción de biopelículas por el ensayo de safranina, motilidad en medio sólido, perfil enterotoxigénico y genes relacionados a producción de biopelículas por PCR en aislados del grupo de Bacillus cereus de alimentos. En este estudio, observamos en las cepas utilizadas una alta producción de biopelículas en PVC; en caldo BHI, no se encontraron biopelículas sumergidas en comparación con el caldo rojo de fenol y caldo rojo de fenol suplementando con dextrosa; no se encontraron cepas con el gen ces, el perfil de enterotoxinas más común fue el perfil que incluía los genes de las tres enterotoxinas, también observamos una distribución diferente de tasA y sipW con relación al origen de la cepa, siendo más frecuente estos genes en las cepas aisladas de huevos. La producción y el tipo de biopelículas es diferente de acuerdo con el tipo de material y el medio de cultivo utilizado.

The risk of choledocholithiasis should be assessed in every patient undergoing cholecystectomy to define the next step. The American Society for Gastrointestinal Endoscopy proposed a stratified predictor scale of choledocholithiasis. Therefore, we aimed to describe our experience managing patients with an intermediate risk of choledocholithiasis according to the American Society for Gastrointestinal Endoscopy guidelines and the actual presence of bile duct stones in magnetic resonance cholangiopancreatography. Methods: A retrospective observational study with a prospective database was conducted. The analysis included sociodemographic data, laboratory values, and imaging. Bivariate, multivariate, and receiver operating characteristic analysis were performed. Results: Three hundred twenty-seven patients had an intermediate risk for choledocholithiasis. Half the patients were at least 65 years old. 24.77% were diagnosed with choledocholithiasis. Bile duct dilation was documented in only 3.06% of cases. Diagnosis of choledocholithiasis is associated with an age odds ratio (OR): 1.87 (P 0.02), alkaline phosphatase OR: 2.44 (P 0.02), and bile duct dilation greater than 6 mm OR: 14.65 (P 0.00). Conclusions: High variability in the accuracy of imaging techniques results in a large number of patients classified as intermediate risk without choledocholithiasis in cholangioresonance. Therefore, enhancing the criteria to define intermediate risk for patients in order to optimize resources is of paramount importance.

ABSTRACT

Complex abdominal wall defects are important conditions with high morbidity, leading to impairment of patients' physical condition and quality of life. In the last decade, the abdominal wall reconstruction paradigm has changed due to the formation of experienced and excellence groups, improving clinical outcomes after surgery. Therefore, our study shows the perspective and outcomes of an abdominal wall reconstruction group (AWRG) in Colombia, focused on the transverse abdominis release (TAR) procedure. A retrospective review of a prospectively collected database was conducted. All the patients older than 18 years old that underwent TAR procedures between January 2014-December 2020 were included. Analysis and description of postoperative outcomes (recurrence, surgical site infection (SSI), seroma, hematoma, and re-intervention) were performed. 47 patients underwent TAR procedure. 62% of patients were male. Mean age was 55 ± 13.4 years. Mean BMI was 27.8 ± 4.5 kg/m2. Abdominal wall defects were classified with EHS ventral Hernia classification having a W3 hernia in 72% of all defects (Mean gap size of 11.49 cm ± 4.03 cm). Mean CeDAR preoperative risk score was 20.5% ± 14.5%. Preoperative use of BOTOX Therapy (OR 1.0 P 0.00 95% CI 0.3-1.1) or pneumoperitoneum (OR 0.7 P 0.04 95% CI 0.3-0.89) are slightly associated with postoperative hematoma. In terms of hernia relapse, we have 12% of cases; all of them over a year after the surgery. TAR procedure for complex abdominal wall defects under specific clinical conditions including emergency scenarios is viable. Specialized and experienced groups show better postoperative outcomes; further studies are needed to confirm our results.

ABSTRACT

The rapid spread of COVID-19 on all continents and the mortality induced by SARS-CoV-2 virus, the cause of the pandemic coronavirus disease 2019 (COVID-19) has motivated an unprecedented effort for vaccine development. Inactivated viruses as well as vaccines focused on the partial or total sequence of the Spike protein using different novel platforms such us RNA, DNA, proteins, and non-replicating viral vectors have been developed. The high global need for vaccines, now and in the future, and the emergence of new variants of concern still requires development of accessible vaccines that can be adapted according to the most prevalent variants in the respective regions. Here, we describe the immunogenic properties of a group of theoretically predicted RBD peptides to be used as the first step towards the development of an effective, safe and low-cost epitope-focused vaccine. One of the tested peptides named P5, proved to be safe and immunogenic. Subcutaneous administration of the peptide, formulated with alumina, induced high levels of specific IgG antibodies in mice and hamsters, as well as an increase of IFN-γ expression by CD8+ T cells in C57 and BALB/c mice upon in vitro stimulation with P5. Neutralizing titers of anti-P5 antibodies, however, were disappointingly low, a deficiency that we will attempt to resolve by the inclusion of additional immunogenic epitopes to P5. The safety and immunogenicity data reported in this study support the use of this peptide as a starting point for the design of an epitope restricted vaccine.

ABSTRACT

INTRODUCTION: Paraganglioma and pheochromocytoma are uncommon conditions that affect around 1.5-9 patients per million. The most frequent symptoms are headache, hypertension and diaphoresis; however, palpitations or tachycardia could be present. Malignancy is not frequent, and when is suspected, positron emission tomography (PET) should be performed. Surgery it's the gold standard treatment, with acceptable rates of morbidity and mortality. PRESENTATION OF THE CASE: A 33-year-old woman presented to private practice with long-standing symptoms consisting of asthenia, adynamia, and sensation of palpable masses in the neck. Due to her medical history and imaging findings, urine metanephrines were obtained, showing high values of adrenaline 6.69 (µg/24 h), noradrenaline 130.09 (µg/24 h), dopamine 262.59 (µg/24 h). PET was performed to identify hyperfunctioning masses in other locations, finding bilateral carotid hypermetabolic masses and a nodular lesion anterior to the aortoiliac bifurcation, probably malignant. Laparoscopic retroperitoneal tumor resection was performed by a laparoscopic and metabolic surgeon, with intraoperative findings of a vascularized mass (30 × 25 mm) closely related to the left aortoiliac bifurcation and peritoneal fluid. DISCUSSION: Paragangliomas are rare tumors that frequently produce catecholamines with varied symptoms. Diagnosis requires patient history, laboratory studies including 24-hour urine-metanephrines and plasma metanephrine levels. Imaging such as CT, MRI and PET scan are necessary. Perioperative management needs to be performed and surgery is the basis of the treatment in patients with localized disease. Metastatic disease has a 50% mortality at 5 years and requires a different approach. CONCLUSION: Paraganglioma is a rare and complex entity that requires a multidisciplinary approach.

BACKGROUND: In the past decade, there has been increasing guideline development for short-term medical missions (STMMs) traveling from high-income to low- and middle-income countries for the purpose of supporting health care services. The ethics of STMMs is criticized in the literature and there is frequently a lack of host country collaboration. This typically results in guidelines which are developed through the lens of the sending (high-income) countries' staff and organizations. The aim of this paper is to evaluate an existing best practice guideline document from the perspective of host country participants with knowledge of STMMs from Honduras, Malawi, and the Philippines. METHODS: The guideline used for the evaluation consisted of nine best practice elements that were discerned based on literature and the experience of those working within the field. Semi-structured interviews were conducted in a cross-sectional study with participants (n = 118) from the host countries. Thematic analysis was conducted by two researchers and the results were assessed by working group members to confirm interpretations of the data. RESULTS: Overall, participants expressed a strong interest in having more structured guidance surrounding STMM practices. There was a positive response to and general acceptance of the proposed STMM guidelines, although participants found the 24-page document onerous to use; a companion checklist was developed. The key themes that emerged from the interviews included collaboration and coordination, care for hard-to-reach communities, capacity building, critical products and essential medical supplies, and opportunity and feasibility. CONCLUSIONS: Host input suggests that the guidelines provide structured regulation and coordination of the medical mission process and have the potential to improve the way STMMs are carried out. The guidelines have also proven to be a useful tool for the actual implementation of STMMs and can be a tool to strengthen links and trust between mission teams and local health staff. However, local contexts vary considerably, and guidelines must be adapted for local use. It is recommended that STMM teams work in conjunction with host partners to ensure they meet local needs, increase capacity development of local health workers, and provide continuity of care for patients into the local system.


ANTECEDENTES: En la última década, ha habido un incremento en el desarrollo de guías para las misiones médicas de corto plazo (STMM) que viajan desde países de ingresos altos a países en vías de desarrollo con el fin de apoyar los servicios de atención médica. La ética de las (STMM) es criticada en la literatura y hay una falta frecuente de colaboración entre los países anfitriones. Esto normalmente da como resultado directrices que se desarrollan a través de la lente del personal y las organizaciones de los países que envían (países de altos ingresos). El objetivo de este documento es evaluar un documento guía de mejores prácticas existente desde la perspectiva de los participantes del país anfitrión con conocimiento de las misiones médicas de corto plazo (STMM) de Honduras, Malawi y Filipinas. MéTODOS: la directriz utilizada para la evaluación consistió en nueve elementos de mejores prácticas que se discernieron en base a la literatura y la experiencia de quienes trabajan en el campo. Se realizaron entrevistas semiestructuradas en un estudio transversal con participantes (n = 118) de los países anfitriones. El análisis temático fue realizado por dos investigadores y los resultados fueron evaluados por miembros del grupo de trabajo para confirmar las interpretaciones de los datos. RESULTADOS: En general, los participantes expresaron un gran interés en tener una guía más estructurada en torno a las prácticas de las misiones médicas de corto plazo (STMM). Hubo una respuesta positiva y una aceptación general de las pautas de las misiones médicas de corto plazo (STMM) propuestos, aunque los participantes encontraron oneroso el uso del documento de 24 páginas y se desarrolló una lista de verificación complementaria. Los temas clave que surgieron de las entrevistas incluyeron colaboración y coordinación, atención a comunidades de difícil acceso, desarrollo de capacidades, productos críticos y suministros médicos esenciales, y oportunidad y viabilidad. CONCLUSIONES: Los comentarios del anfitrión sugieren que las directrices proporcionan una regulación y coordinación estructuradas del proceso de la misión médica y tienen el potencial de mejorar la forma en que se llevan a cabo las misiones médicas de corto plazo (STMM). Las pautas también han demostrado ser una herramienta útil para la implementación real de de las misiones médicas de corto plazo (STMM) y pueden servir para fortalecer los vínculos y la confianza entre los equipos de misión y los sistemas de salud locales. BACKGROUND: High-risk surgical procedures represent a fundamental part of general surgery practice due to its significant rates of morbidity and mortality. Different predictive tools have been created in order to quantify perioperative morbidity and mortality risk. POSSUM (Physiological and Operative Severity Score for the enumeration of Mortality and morbidity) is one of the most widely validated predictive scores considering physiological and operative variables to precisely define morbimortality risk. Nevertheless, seeking greater accuracy in predictions P-POSSUM was proposed. We aimed to compare POSSUM and P-POSSUM for patients undergoing abdominal surgery. METHODS: A retrospective observational study with a prospective database was conducted. Patients over 18 years old who complied with inclusion criteria between 2015 and 2016 were included. Variables included in the POSSUM and P-POSSUM Scores were analyzed. Descriptive statistics of all study parameters were provided. The analysis included socio-demographic data, laboratory values ​​, and imaging. Bivariate analysis was performed. RESULTS: 350 Patients were included in the analysis, 55.1% were female. The mean age was 55.9 ± 20.4 years old. POSSUM revealed a moderated index score in 61.7% of the patients, mean score of 12.85 points ± 5.61. 89.1% of patients had no neoplastic diagnosis associated. Overall morbidity and mortality rate was 14.2% and 7.1%. P-POSSUM could predict more precisely mortality (p < 0.00). CONCLUSIONS: The POSSUM score is likely to overestimate the risk of morbidity and mortality in patients with high/moderate risk, while the P-POSSUM score seems to be a more accurate predictor of mortality risk. Further studies are needed to confirm our results.

ABSTRACT

INTRODUCTION: Appendicular agenesis is a rare condition that accounts for 1 of every 100,000 exploratory laparotomies, usually in clinical suspicion of appendiceal inflammatory processes with higher incidence in adults. CLINICAL FINDINGS: We present a case of a 19-year-old female with appendicular agenesis who underwent exploratory laparoscopy in the context of appendiceal inflammatory process. CONCLUSION: Vermiform appendix agenesis is a challenging diagnosis made in most cases as an intraoperatively finding. Systematic revision of abdominal cavity should be performed after ruling out other causes of acute abdomen. No implications have been described in patients with appendix abnormalities.

ABSTRACT

Ex-situ conservation in hatcheries is a successful strategy for the recovery of sea turtle populations. However, it alters the ontogenesis of the brain and gonads, as well as body size and locomotor performance at nest emergence. Relocation to hatcheries may alter immune system development, since this depends highly on the nest environment. We hypothesized that ex-situ brooding would negatively associate with immune traits of Lepidochelys olivacea. Splenic cytoarchitecture and leukocyte quantification were used as proxies for the immune configuration. Body size, gonadal sex and sand temperature during incubation were determined. Additionally, the success of nest hatching and emergence was quantified. Linear mixed models of splenic cytoarchitecture, leucocyte proportions and body size, using sex and nest type as explanatory variables, evaluated the effects of ex-situ brooding. Generalized linear mixed models using quasibinomial distributions (log link) analyzed effects on hatching and emergence success. Hatchlings from ex-situ nests were heavier, larger and showed a greater spleen-somatic index. They showed more and better defined splenic periarteriolar lymphoid sheaths, as well as a higher proportion of heterophils but less monocytes. Moreover, ex-situ brooding increased hatching and emergence success. Sand temperatures in hatcheries favored male sex determination, while the opposite occurred for in-situ incubation. Interestingly, the immune configuration and body size were independent of sex but associated with ex-situ conservation. Greater body size promotes early hatchling survival, while better spleen development is related to a greater antibody production and a better immune response to pathogens. Altogether, the results suggest that ex-situ incubation is associated with a better immune configuration and higher survival success.

ABSTRACT

In addition to the full-length beta-amyloid peptides (Aß 1-40/42), several Aß variants, truncated at their N- or C-termini and bearing different post-translational modifications, have been detected in the brain of Alzheimer´s disease (AD) patients. AßN3(pE), an Aß peptide bearing an amino-terminal pyroglutamate at position 3, is a significant constituent of intracellular, extracellular and vascular Aß deposits in brain tissue from individuals with AD and Down syndrome. Pioneering immunotherapy studies have primarily focused on the full-length Aß peptide, disregarding the presence of N-truncated/modified species. However, in recent years, increasing attention has been directed towards AßN3(pE), in both pre-clinical studies and clinical trials. In the present study, we generated and characterized an anti-AßN3(pE) mouse monoclonal antibody (3B8) that recognizes amyloid aggregates in brain tissue from AD patients and in 3xTg-AD transgenic mice. To identify the epitope recognized by 3B8, a library of random heptapeptides fused to the minor coat protein of M13 phage was screened. Results from screening, along with those from ELISA assays against distinct Aß fragments, suggest recognition of two conformational epitopes present in AßN3(pE) and Aß 3-42, regardless of the glutamate-pyroglutamate modification. The novel 3B8 antibody may be useful in future therapeutic and diagnostic applications for AD.
