ABSTRACT

During the COVID-19 pandemic, longstanding issues involving nursing work, which has always involved significant risks of illness and infection, were intensified. It is necessary to acknowledge the risks and nurses' perceptions about the risks qualitatively during the period of the health crisis. The aim of this study was to examine the health repercussions perceived by nursing workers in emergency services during the outbreak of the COVID-19 pandemic in Brazil. This was a qualitative, descriptive, cross-sectional study. The settings of the study were emergency services with a national scope; the participants were nursing workers. Data were collected via face-to-face virtual calling interviews and analyzed via a content analysis technique, which was supported by IRAMUTEQ software. The formation of textual classes pointed in three thematic directions, from which three categories emerged: nursing workers' exposure, due to a lack of protective equipment, to the risk of being contaminated with, falling ill from, and transmitting the COVID-19 virus; changes in work environments, processes, and relations in response to the pandemic; and physical, mental, and psychosocial alterations perceived by emergency service nursing workers. The exposure to the virus, risk of contamination, and changes in the work environment and relations all resulted in health repercussions, which were perceived as physical, mental, and psychosocial alterations that were described as dietary disturbances, physical fatigue, burnout, increased smoking, anxiety, sleep alterations, fear, exhaustion, stress, social isolation, loneliness, distancing from relatives, and social stigma.

ABSTRACT

OBJECTIVE: To describe actions taken by the organizational management of an emergency service due to the COVID-19 pandemic, determined according to the prevalence of cases of infection by the coronavirus, severe acute respiratory syndrome, and flu-like illnesses. METHODS: Experience report based on a retrospective analysis of the attention for respiratory syndromes in the first semester of 2019 and 2020, in addition to an analysis of documents from the institutional protocols of a federal emergency service.Development: An increase in the number of attendances was observed, representing 7.25% and 19.4% of cases in 2019 and 2020, respectively. This was due to the creation of the Crisis Office, including a multidisciplinary team created to elaborate the plan of action, changes in the physical structure and in the work processes, and training sessions. FINAL CONSIDERATIONS: It became clear that the planning, coordination of actions based on the decisions of the Crisis Office, and the dissemination of reliable information, taking into consideration a focal point, were essential for the organization, management of the emergency service, and protection to the workers.

RESUMEN Objetivo: Describir acciones de gestión organizacional de un servicio de urgencia resultantes de la pandemia de COVID-19, definidas basadas en la prevalencia de casos de infección por coronavirus, síndrome respiratoria aguda grave y síndromes gripales. Método: Relato de experiencia basado en análisis retrospectivo de atenciones de síndromes respiratorias en el primer semestre de 2019 y 2020, junto análisis documental de protocolos institucionales de servicio de urgencia federal. Desarrollado: Observado aumento de atenciones, representando 7,25% y 19,4% de los casos de 2019 y 2020, respectivamente, debido la formación del Gabinete de Crisis, con equipe multidisciplinar responsable por la construcción del plan de acción con cambios en la estructura física, procesos de trabajo y entrenamientos. Consideraciones finales: Evidenciado que planeamiento, coordinación de acciones pautadas en decisiones del Gabinete de Crisis y divulgación de informaciones confiables mediante un punto focal fueron esenciales para organización, gestión del servicio de urgencia y protección a trabajadores.


RESUMO Objetivo: Descrever ações de gestão organizacional de um serviço de emergência decorrentes da pandemia de COVID-19, definidas com base na prevalência de casos de infecção por coronavírus, síndrome respiratória aguda grave e síndromes gripais. Métodos: Relato de experiência baseado na análise retrospectiva dos atendimentos de síndromes respiratórias no primeiro semestre de 2019 e de 2020, além de análise documental dos protocolos institucionais de um serviço de emergência federal. Desenvolvimento: Observou-se aumento dos atendimentos, representando 7,25% e 19,4% dos casos de 2019 e 2020, respectivamente, devido à formação do Gabinete de Crise, com equipe multidisciplinar responsável pela construção do plano de ação com mudanças na estrutura física, processos de trabalho e treinamentos. Considerações finais: Evidenciou-se que planejamento, coordenação das ações pautadas nas decisões do Gabinete de Crise e divulgação de informações confiáveis mediante um ponto focal foram essenciais para organização, gestão do serviço de emergência e proteção aos trabalhadores.

Objetivo: relatar a experiência na atualização de trabalhadores de enfermagem sobre suporte básico de vida em um hospital público. Método: trata-se de um estudo descritivo, tipo relato de experiência, com 123 profissionais de Enfermagem das unidades de internação clínica e cirúrgica e centro cirúrgico, sobre a atualização de trabalhadores de Enfermagem em suporte básico de vida. Aplicaram-se instrumentos de aprendizagem pré-teste e pós-teste. Resultados: observou-se que, dos 86 participantes que responderam aos instrumentos pré-teste e pós-teste, 36% acertaram todas as questões no pré-teste e pós-teste; 54,7% erraram questões na seção pré-teste, mas acertaram no pós-teste e 9,3% erraram as respostas no pré-teste e pós-teste. Verificou-se que as opções incorretas que obtiveram destaque estavam relacionadas à sequência correta para a realização da ressuscitação cardiopulmonar e à relação entre as compressões torácicas e a ventilação. Conclusão: faz-se necessária a realização de capacitações periódicas, uma vez que se identificou uma importante lacuna no conhecimento sobre o suporte básico de vida e o risco de implicações negativas à assistência aos pacientes graves.(AU)


Objective: to report the experience of updating nursing workers on basic life support in a public hospital. Method: this is a descriptive study, an experience report type, with 123 nursing professionals from the clinical and surgical inpatient units and operating room, about the updating of nursing workers in basic life support. Pre-test and post-test learning instruments were applied. Results: it was observed that, from the 86 participants who answered the pretest and posttest instruments, 36% answered all questions in the pretest and posttest; 54.7% missed questions in the pretest section, but got it right in the posttest and 9.3% missed the answers in the pretest and posttest. The incorrect options that were highlighted were related to the correct sequence to perform cardiopulmonary resuscitation and the relationship between chest compressions and ventilation. Conclusion: it is necessary to perform periodic training, as it has identified an important gap in the knowledge about basic life support and the risk of negative implications for the care of critically ill patients.(AU)


Objetivo: informar sobre la experiencia de actualizar a los trabajadores de enfermería en soporte vital básico en un hospital público. Método: este es un estudio descriptivo, un tipo de informe de experiencia, con 123 profesionales de Enfermería de las unidades de internación clínica y quirúrgica y quirófano, sobre la actualización de los trabajadores de Enfermería en soporte vital básico. Se aplicaron instrumentos de aprendizaje previo y posterior al examen. Resultados: se observó que, de los 86 participantes que respondieron a los instrumentos de la prueba previa y posterior, el 36% respondió todas las preguntas en la prueba previa y posterior; el 54.7% erraron preguntas en la sección de la prueba previa, pero acertó en la prueba posterior y el 9.3% erraron las respuestas en la prueba previa y posterior. Las opciones incorrectas que se resaltaron estaban relacionadas con la secuencia correcta para realizar la reanimación cardiopulmonar y la relación entre las compresiones torácicas y la ventilación. Conclusión: se necesita capacitación periódica, ya que se identificó una brecha importante en el conocimiento sobre el soporte vital básico y el riesgo de implicaciones negativas para la atención de pacientes críticos.(AU)
