Resumo O objetivo deste artigo é analisar a associação entre o tipo de aleitamento no primeiro semestre de vida e o consumo de vitamina A e ferro do 6º ao 12º mês. Estudo de coorte com 226 crianças. Avaliou-se o tipo de aleitamento do 1º ao 6º mês de vida e o consumo alimentar do 6º ao 12º mês. O consumo de nutrientes entre os grupos foi comparado pelo teste Kruskal-Wallis. A análise da associação entre o consumo de nutrientes e o tipo de aleitamento se deu pela Regressão de Poisson. O consumo abaixo da recomendação de vitamina A e ferro foi de 33,6% e 67,7%, respectivamente. Crianças amamentadas de maneira exclusiva e predominante no 1º e 2º mês tiveram maior ingestão de vitamina A do 6º ao 12º mês de vida. Já as que estavam em aleitamento materno no 6º mês tiveram maior ingestão de vitamina A e menor de ferro na análise bivariada. O grupo que recebeu aleitamento materno misto e artificial no 1º mês e os não amamentados no 6º mês tiveram consumo de vitamina A abaixo do recomendado do 6º ao 12º mês de vida. Os resultados reforçam a importância do aleitamento materno exclusivo nos seis primeiros meses de vida, contribuindo para o maior consumo de vitamina A. O consumo de ferro foi menor entre as crianças amamentadas, porém, a biodisponibilidade desse nutriente no leite materno é maior.

BACKGROUND: Obesity is a multifactorial disease and a serious public health problem. Some of the associated factors are modifiable and, among them, the diet is highlighted. OBJECTIVE: To evaluate the association of dietary patterns of schoolchildren with obesity and body adiposity. METHODS: A cross-sectional study was carried out with 378 children aged 8 and 9 years, enrolled in urban schools in the city of Viçosa, Minas Gerais, Brazil. A semi-structured questionnaire was applied to the children and their caregivers on sociodemographic characteristics and life habits. Three 24-hour food recalls were used to identify dietary patterns; the Principal Component Analysis was employed. Weight and height were measured for the calculation of the body mass index (BMI) of the children and their mothers, waist circumference and neck circumference. Body composition was also evaluated through dual-energy X-ray absorptiometry (DXA). For all performed tests, the level of significance was set at 5%. RESULTS: Five dietary patterns (DP) were identified: "unhealthy", "snacks", "traditional", "industrialized" and "healthy". There was an association between excess weight (prevalence ratio [PR]: 1.38, 95% confidence interval [95%CI]: 1.02 to 1.87) and body fat (PR: 1.32, 95%CI : 1.07 to 1.64) with industrialized DP. There was an association between excess body fat (PR: 1.31, 95%CI: 1.01 to 1.74) and lower adherence to traditional DP. The other patterns were not associated with obesity and body adiposity. CONCLUSION: Children with excess weight and body adiposity showed greater adherence to the industrialized DP and lower adherence to the traditional DP. We suggest that early assessments of dietary habits should be undertaken for monitoring and modifying these habits when necessary.

Resumo Fundamentos: A obesidade é uma doença multifatorial e um grave problema de saúde pública. Alguns dos fatores associados são modificáveis; dentre eles destaca-se a alimentação. Objetivo: Avaliar a associação dos padrões alimentares de escolares com a obesidade e adiposidade corporal. Métodos: Estudo transversal, com 378 crianças de 8 e 9 anos, matriculadas em escolas urbanas no município de Viçosa, Minas Gerais (MG), Brasil. Foi aplicado um questionário semiestruturado com as crianças e seus responsáveis sobre características sociodemográficas e hábitos de vida. Utilizaram-se três recordatórios 24 horas para identificar os padrões alimentares; a Análise de Componentes Principais foi empregada. Foram aferidos peso e estatura para o cálculo do índice de massa corporal (IMC) das crianças e de suas mães, perímetro da cintura e perímetro do pescoço A composição corporal também foi avaliada, por meio de absorciometria por dupla emissão de raios X (DXA, do inglês dual energy X-ray absorptiometry). Para todos os testes realizados, o nível de significância adotado foi de 5%. Resultados: Foram identificados cinco padrões alimentares (PA): "não saudável", "lanche", "tradicional", "industrializado" e "saudável". Houve associação entre o excesso de peso (razão de prevalência [RP]: 1,38; intervalo de confiança de 95% [IC95%]: 1,02 a 1,87) e gordura corporal (RP: 1,32; IC95%: 1,07 a 1,64) com o PA industrializado. Foi encontrada associação entre o excesso de gordura corporal (RP: 1,31; IC95%: 1,01 a 1,74) e a menor adesão ao PA tradicional. Os demais padrões não estiveram associados a obesidade e adiposidade corporal. Conclusão: As crianças com excesso de peso e de adiposidade corporal apresentaram maior adesão ao PA industrializado e menor adesão ao PA tradicional. Sugerimos que avaliações precoces dos hábitos alimentares devam ser realizadas para monitoramento e modificação destes, quando necessário.

ABSTRACT

This article aimed to assess the prevalence of dyslipidemia and associated factors in children aged from 4 to 7 years old. It is a cross-sectional study conducted with 402 children aged from 4 to 7 years old, accompanied by a Lactation Support Program in the first six months of life. We measured total cholesterol, triglycerides and high-density lipoprotein (HDL) and low-density lipoprotein (LDL). We selected the variables that could be associated with the lipid profile, such as the family history of dyslipidemia, nutritional status and food consumption of children. We performed Poisson regression analysis with robust variance. The level of significance adopted was p<0.05. We observed increased LDL values in 46.8% (188), total cholesterol in 37.6% (151), triglycerides in 10.4% (42) and below-desirable HDL in 33.8% (136) of the children. There was a statistically significant association between the family history of dyslipidemia with total cholesterol, LDL and triglycerides (p<0.05 and p < 0.001, respectively); Early weaning with LDL (p < 0.05); Sedentarism with LDL and triglycerides (p < 0.05 and p < 0.001, respectively); as well as HDL with candy consumption (p < 0.05). There was a significant prevalence of changes in the lipid profile of the children. Nutrition education activities and programs aimed at this group are needed.


O objetivo deste artigo é avaliar a prevalência de dislipidemia e os fatores associados em crianças de 4 a 7 anos de idade. Estudo transversal, realizado com 402 crianças de 4 a 7 anos de idade, acompanhadas por um Programa de Apoio a Lactação nos primeiros seis meses de vida. Foram dosados colesterol total, triglicerídeos e lipoproteínas de alta (HDL) e baixa densidades (LDL). Selecionou-se variáveis que poderiam estar associadas ao perfil lipídico, como o histórico familiar de dislipidemia, estado nutricional e o consumo alimentar das crianças. Realizou-se análise de Regressão de Poisson com variância robusta. O nível de significância adotado foi de p < 0,05. Observou-se valores aumentados da LDL em 46,8% (188), colesterol total em 37,6% (151), triglicerídeos em 10,4% (42) e HDL abaixo do desejável em 33,8% (136) das crianças. Houve associação estatisticamente significativa entre o histórico familiar de dislipidemia com colesterol total, LDL e triglicerídeos (p < 0,05 e p < 0,001, respectivamente); Desmame precoce com LDL (p < 0,05); Sedentarismo com LDL e triglicerídeos (p < 0,05 e p < 0,001, respectivamente); bem como HDL com o consumo de bala (p < 0,05). Houve importante prevalência de alterações no perfil lipídico das crianças. São necessárias atividades de educação nutricional e programas voltados para esse grupo.

ABSTRACT

OBJECTIVE: To identify the dietary patterns of children aged 4-7 years and verify their association with sociodemographic characteristics, lifestyle habits and exclusive breast-feeding (EBF). DESIGN: A cross-sectional study nested within a cohort, performed with Brazilian children aged 4-7 years. The children were re-evaluated at age 4 to 7 years and food patterns were identified a posteriori through principal component analysis. The predictive variables were related to socio-economic characteristics, lifestyle habits and duration of EBF. SETTING: Viçosa, Minas Gerais, Brazil.ParticipantsRepresentative sample of 403 children followed up by the Lactation Support Program from the Extension Program of the Universidade Federal de Viçosa during the first 6 months of life. RESULTS: Five dietary patterns were identified: 'Traditional', 'Unhealthy', 'Milk and chocolate', 'Snack' and 'Healthy'. Children who did not receive EBF until they were at least 4 months old had a higher adherence to the 'Unhealthy' and 'Snack' patterns, and older children also consumed more 'Unhealthy' foods. The highest income was associated with the highest consumption of foods of the patterns 'Unhealthy', 'Milk and chocolate' and 'Healthy'. CONCLUSIONS: In view of the results, we emphasize the importance of providing support and encouragement towards EBF in the first months of life, as it can positively influence lifelong eating habits.

Métodos: Estudo transversal com 257 crianças de 4 a 7 anos de idade do município de Viçosa, Minas Gerais. Foram realizadas avaliações do estado nutricional e do perfil lipídico (colesterol total e frações e triglicerídeos), além do registro de história de dislipidemia dos pais. Para análise estatística, foi utilizado o teste do qui-quadrado de Pearson para identificar associações e teste t de Student para comparação de médias. Foi realizada análise de regressão de Poisson para avaliar a associação independente entre história familiar e a presença de dislipidemia em crianças, sendo adotado o nível de significância estatística de 5%. Resultados: Crianças com pais dislipidêmicos apresentaram maiores concentrações séricas de colesterol total e triglicerídeos. Na análise de regressão após ajuste, a presença de dislipidemia no pai ou na mãe (RP: 2,43; IC95% 1,12-5,27) bem como a presença de dislipidemia no pai e na mãe (RP: 5,62; IC95% 2,27-13,92) estiveram associadas à hipertrigliceridemia nas crianças. Crianças com pais e mães dislipidêmicos apresentaram maior prevalência de lipoproteína de baixa densidade (LDL-c) elevada (RP: 1,52; IC95% 1,18-1,97). Conclusões: A investigação da história familiar de dislipidemia deve fazer parte de protocolos para verificar a presença de hipertrigliceridemia e dislipidemias na infância.

ABSTRACT

OBJECTIVE: To evaluate the ability of anthropometric measurements to identify excess android fat and to propose cut-off points for excess central adiposity in children, according to age and sex. DESIGN: A cross-sectional study with children from a municipality of Minas Gerais, Brazil. Receiver-operating characteristic curve analyses were performed to evaluate waist circumference (WC), waist-to-height ratio (WHtR) and conicity index (C-index) in estimating excess android fat by dual energy X-ray absorptiometry (DXA). SETTING: Viçosa, Minas Gerais, Brazil. SUBJECTS: Children aged 4-9 years (n 788). RESULTS: Overweight prevalence was 29·1 % and android fat percentage was higher among girls. All central fat measurements were able to discriminate excess android fat in the age groups evaluated, especially WC and WHtR, with cut-off points showing good sensitivity and specificity overall. CONCLUSIONS: Because these methods are easy to obtain and inexpensive, it is possible to use WC, WHtR and C-index in population surveys to evaluate central obesity. The proposed cut-off points showed satisfactory values of sensitivity and specificity and can be used in epidemiological studies.

Foram dosados colesterol total, triglicerídeos e lipoproteínas de alta (HDL) e baixa densidades (LDL). Selecionou-se variáveis que poderiam estar associadas ao perfil lipídico, como o histórico familiar de dislipidemia, estado nutricional e o consumo alimentar das crianças. Realizou-se análise de Regressão de Poisson com variância robusta. O nível de significância adotado foi de p < 0,05. Observou-se valores aumentados da LDL em 46,8% (188), colesterol total em 37,6% (151), triglicerídeos em 10,4% (42) e HDL abaixo do desejável em 33,8% (136) das crianças. Houve associação estatisticamente significativa entre o histórico familiar de dislipidemia com colesterol total, LDL e triglicerídeos (p < 0,05 e p < 0,001, respectivamente); Desmame precoce com LDL (p < 0,05); Sedentarismo com LDL e triglicerídeos (p < 0,05 e p < 0,001, respectivamente); bem como HDL com o consumo de bala (p < 0,05). Houve importante prevalência de alterações no perfil lipídico das crianças. São necessárias atividades de educação nutricional e programas voltados para esse grupo.


Abstract This article aimed to assess the prevalence of dyslipidemia and associated factors in children aged from 4 to 7 years old. It is a cross-sectional study conducted with 402 children aged from 4 to 7 years old, accompanied by a Lactation Support Program in the first six months of life. We measured total cholesterol, triglycerides and high-density lipoprotein (HDL) and low-density lipoprotein (LDL). We selected the variables that could be associated with the lipid profile, such as the family history of dyslipidemia, nutritional status and food consumption of children. We performed Poisson regression analysis with robust variance. The level of significance adopted was p<0.05. We observed increased LDL values in 46.8% (188), total cholesterol in 37.6% (151), triglycerides in 10.4% (42) and below-desirable HDL in 33.8% (136) of the children. There was a statistically significant association between the family history of dyslipidemia with total cholesterol, LDL and triglycerides (p<0.05 and p < 0.001, respectively); Early weaning with LDL (p < 0.05); Sedentarism with LDL and triglycerides (p < 0.05 and p < 0.001, respectively); as well as HDL with candy consumption (p < 0.05). There was a significant prevalence of changes in the lipid profile of the children. Nutrition education activities and programs aimed at this group are needed.

ABSTRACT

This study aimed at determining the dietary patterns and investigating their association with cardiometabolic risk markers in a brazilian population at risk. This transversal study was carried out with data of 265 patients (n = 123 M/172 W, age 42 ± 16 years) of the Cardiovascular Health Care Program-PROCARDIO-UFV, Brazil-who had their first appointment between 2012 and 2017. A 24-hour recall was applied. The dietary patterns were determined by Principal Component Analysis. Anthropometric, clinical-metabolic, sociodemographic, and lifestyle data were collected through medical record analysis. Five patterns were identified: "Traditional", "Caloric", "Unhealthy", "Healthy," and "Healthy Snacks". In bivariate analysis, the "Healthy" pattern was negatively associated with WC (waist circunference), BMI (body mass index), WHR (waist-to-hip ratio), SBP (systolic blood pressure), fasting glucose, TG/HDL, LDL/HDL, and TG/HDL values and positively to HDL. The "Traditional" pattern was positively associated with adiposity indicators (WC, BMI, and WHR) and negatively associated with body fat, TyG (triglyceride-glucose index), HDL, and LDL (P < 0.05). However, in adjusted models of Poisson regression, individuals with positive factor score (higher adherence) in the "Traditional" and "Healthy" patterns had less occurrence of abdominal obesity (PR 0.85; 95% CI 0.74-0.99/PR 0.88; 95% CI 0.02-0.76), as well as dyslipidemia (PR 0.06; 95% CI 0.02-0.51/PR 0.03; 95% CI 0.01-0.27), diabetes (PR 0.05; 95% CI 0.01-0.45/PR 0.02; 95% CI 0.01-021), and hypertension (PR 0.06; 95% CI 0.02-0.50/PR 0.02; 95% CI 0.01-0.21). A greater adherence to the "Healthy" pattern was associated with lower values to cardiometabolic risk markers and less occurrence of chronic diseases, while the "Traditional" pattern presented contradictory results.

ABSTRACT Objective To evaluate the association between diet and constipation in children between four and seven years of age. Methods A cross-sectional study with 152 children between 4 and 7 years of age residing in Viçosa, Brazil. Constipation was defined in accordance with the Rome IV diagnostic criteria. Dietary habits were evaluated based on the frequency of food intake using a food diary for three days. Dietary analysis considered foods deemed healthy and unhealthy which could be associated with constipation, as well as the children's consumption of fruits and vegetables. The amounts of fiber and water consumed were also rated as adequate or inadequate. As part of the analyses, Student's t-test and the chi-square test were performed with a significance level of p<0.05. Results The prevalence of constipation was 32.2%. Constipation was more common among children who did not consume fruits (p=0.020) or greens (p=0.002), as well as among children who consumed "instant" chocolate drinks (p=0.033). An association was found between the frequent consumption of fried food (p=0.020), a daily water intake lower than 600mL, and constipation (p=0.028). Conclusion Unhealthy eating habits among children, such as the lack of fruit and greens intake and the consumption of "instant" chocolate drinks and fried foods, were found to be positively correlated with constipation. The low fruit and vegetable consumption observed herein reflects the need for changes in children's eating habits and the implementation of measures to promote the intake of healthy foods.


RESUMO Objetivo O estudo visa avaliar a associação do consumo alimentar de crianças de quatro a sete anos de idade com constipação intestinal. Métodos Foi realizado estudo transversal com 152 crianças de 4 a 7 anos, residentes em Viçosa, Minas Gerais, Brasil. A constipação intestinal foi identificada de acordo com os critérios de Roma IV. Avaliou-se o consumo alimentar por meio da frequência de consumo de alimentos e do registro alimentar de três dias. Na análise do consumo alimentar selecionaram-se os alimentos marcadores de alimentação saudável e não saudável que poderiam estar associados à constipação, além do consumo agrupado de frutas, verduras e legumes. Verificou-se ainda adequação do consumo de fibras e ingestão de água. Nas análises, foram realizados os testes t-Student e qui-quadrado, considerando-se como significância estatística p<0,05. Resultados A prevalência de constipação foi de 32,2%, sendo maior entre as crianças que não consumiam frutas (p=0,020) e verduras (p=0,002) e consumiam achocolatados (p=0,033). Houve associação entre a constipação (p=0,028) e o consumo frequente de frituras (p=0,020), ingestão diária de água inferior a 600mL. Conclusão Hábitos não saudáveis das crianças, como a baixa ingestão de frutas e verduras, e o consumo de achocolatado e frituras, contribuíram para a associação positiva com a constipação. O baixo consumo de frutas, verduras e legumes revela a necessidade de alteração dos hábitos alimentares das crianças, bem como da implantação de ações para incentivo ao consumo de alimentos saudáveis.

Abstract Objectives: to evaluate the association between smoking during pregnancy and nutritional status. Methods: cohort study with a sample of 460 children in the baseline. The children were assessed four times, being measured for weight and length to be converted in indexes length forage (L/A) and body mass index forage (BMI/A) in Z-score. The time until occurrence of growth deficit and overweight was calculated in days and compared to maternal smoking during pregnancy. To assess the association between smoking during pregnancy and the outcomes, a Hazard Ratio by Cox regression was obtained, adjusting by confounding variables selected from Directed Acyclic Graphs (DAG). Results: the time until occurrence of growth deficit and overweight was lower in children whose mothers smoked during pregnancy. Smoking during pregnancy was a risk factor for length deficit (HR = 2.84; CI95% = 1.42 to 5.70) and for overweight (HR = 1.96; CI95% = 1, 09 to 3.53), even after the adjustment. Conclusions: maternal smoking was a changeable factor associated with anthropometric outcomes, which demonstrates the need for actions to combat smoking during pregnancy in order to prevent early nutritional deviations.


Resumo Objetivos: avaliar a associação entre o fumo na gestação e a ocorrência de excesso de peso e déficit de crescimento no primeiro semestre de vida. Métodos: estudo de coorte com amostra de 460 crianças no baseline. As crianças foram avaliadas em quatro momentos, sendo aferidos em todas as avaliações peso e comprimento para serem convertidos nos índices comprimento por idade (C/I) e Índice de massa corporal por idade (IMC/I) em escore-z. O tempo até a ocorrência de déficit de crescimento e excesso de peso foi calculado em dias e comparado ao fumo materno durante a gestação. Para avaliar a associação entre tabagismo na gestação eos desfechos, foi calculado o Hazard Ratio por meio da regressão de Cox, ajustando-se por variáveis de confusão selecionadas a partir de gráficos acíclicos direcionados. Resultados: o tempo até a ocorrência de déficit de crescimento e excesso de peso foi menor em crianças cujas mães fumaram durante a gestação. O tabagismo na gestação foi um fator de risco para o déficit de comprimento (HR=2,84; IC95%=1,42-5,70) e para o excesso de peso (HR=1,96; IC95%=1,09-3,53), mesmo após o ajuste. Conclusão: o tabagismo materno foi um fator modificável associado com desfechos antropométricos independente de fatores de confusão, demonstrando a necessidade de ações de combate ao tabagismo na gestação, afim de prevenir precocemente desvios nutricionais.

INTRODUCTION AND OBJECTIVE: Studies in adults have shown an association between increased adiposity and hypertension, but few studies have analyzed this association in childhood. The aim of this study was to investigate the association between blood pressure (BP) and body adiposity indicators in children, controlling for the variables of birth conditions, sociodemographics, lifestyle and diet. METHODS: In this cross-sectional study of 399 children aged 4 to 7 years, the dependent variable was BP, measured according to the protocol established by the 7th Brazilian Guidelines on Hypertension. The explanatory variables of the study were waist-to-height ratio (WHtR) and body mass index (BMI). Multiple linear regression was used to assess the independent association between adiposity and BP indicators adjusted for the variables of birth conditions, sociodemographics, lifestyle and diet. Statistical significance was set as α=5%. RESULTS: BMI for age and WHtR correlated positively with systolic (SBP) and diastolic blood pressure (DBP). After multivariate analysis, higher values of BMI (model 1) and WHtR (model 2) were associated with increased SBP and DBP. CONCLUSION: Increased body adiposity, as assessed by BMI and WHtR, was associated with increased SBP and DBP among the children studied. Thus, we suggest the use of BMI and WHtR in the nutritional assessment of children to detect changes in BP and other cardiometabolic risk factors in this population.

RESUMO Objetivo: Identificar um indicador de adiposidade abdominal de baixo custo e com maior acurácia para predizer o excesso de peso em crianças de quatro a sete anos idade. Métodos: Estudo transversal com amostra de 257 crianças de 4 a 7 anos. Os indicadores de adiposidade abdominal avaliados foram: perímetro da cintura (PC), relação cintura-estatura (RCE) e percentual de gordura central (avaliado pela técnica dual energy X-ray absorptiometry - DEXA). O excesso de peso foi classificado pelo índice de massa corporal por idade (IMC/I). Nas análises, estimou-se a razão de prevalência (RP) pela regressão de Poisson com variância robusta e utilizou-se a curva (receiver operating characteristics ­- ROC), considerando como significância estatística p<0,05. Resultados: A prevalência de excesso de peso foi de 24,9%, e observou-se maior mediana dos indicadores de adiposidade abdominal no grupo de crianças com excesso de peso. As crianças com valores aumentados de PC (RP=4,1; IC95% 2,86-5,86), RCE (RP=5,76; IC95% 4,14-8,02) e percentual de gordura central (RP=2,48; IC95% 1,65-3,73) apresentaram maior prevalência de excesso de peso. Verificou-se, na análise de curva ROC, que o índice RCE apresentou maior área sob a curva, comparado ao PC e ao percentual de gordura central estimada pelo DEXA, na predição do excesso de peso. Conclusões: Diante dos resultados, sugere-se a utilização da RCE para triagem de crianças com excesso de peso.

ABSTRACT Objective To evaluate factors associated with the iron nutritional status of Brazilian children aged 4 to 7 years in the city of Viçosa, Minas Gerais state, Brazil. Methods This is a cross-sectional study of 357 children aged 4-7 years who had been followed-up up for during their first six months of life by the Breastfeeding Support Program. Socioeconomic and demographic characteristics, feeding practices, nutritional status (height-for-age and body mass index-for-age), and serum ferritin and hemoglobin concentrations were evaluated. Multiple linear regression analysis was carried out to evaluate factors independently associated with iron nutritional status (hemoglobin and ferritin), considering α=0.05 as the significance level. Results The prevalence of anemia and iron deficiency was (34) 9.52% and (11) 11.00%, respectively. The factors independently associated with anemia were younger child age, low maternal education, low height-for-age Z-scores, and children of single and separated mothers or widows. Iron deficiency was associated with child younger age and consumption of chocolates and chocolate flavored milk. Conclusion The results obtained allow us to conclude that anemia among children 4-7 years of age is a public health problem in the city of Viçosa, Minas Gerais. Therefore, there is a need for intervention measures targeting children in this age group. These measures can be implemented through food and nutritional education by encouraging the consumption of iron-rich foods.


RESUMO Objetivo Avaliar fatores associados ao estado nutricional de ferro em crianças brasileiras de 4 a 7 anos de idade, no município de Viçosa, Minas Gerais. Métodos Trata-se de um estudo transversal com 357 crianças que foram acompanhadas nos primeiros seis meses de vida, pelo Programa de Apoio à Lactação. Obteve-se informações referentes as condições socioeconômicas, demográficas, práticas alimentares, estado nutricional (estatura/idade e índice de massa corporal/idade), con-centrações de ferritina e de hemoglobina. Realizou-se análise de regressão linear múltipla para avaliar os fatores independentemente associados ao estado nutricional de ferro (hemoglobina e ferritina) considerando-se o nível de significância de α=0,05. Resultados As prevalências de anemia e deficiência de ferro foram de (34) 9,52% e (11) 11,00% respectivamente. Os fatores independentemente associados a anemia foram menor idade da criança, menor escolaridade da mãe, baixa estatura para idade em escore-Z, e ser filho de mães solteiras, separadas ou viúvas. A deficiência de ferro foi associada a menor idade da criança e consumo de chocolates/achocolatados. Conclusão Os resultados do presente estudo permitem concluir que no município de Viçosa, a anemia apresenta-se como um problema de saúde pública ligeiro em crianças de 4 a 7 anos de idade. Diante desse problema, verifica-se a necessidade de medidas de intervenção voltadas para o grupo infantil. Essas medidas podem ser feitas com a educação alimentar e nutricional, estimulando o consumo dos alimentos fontes de ferro.

Resumo Objetivo Avaliar a prevalência e os fatores associados à anemia ferropriva e hipovitaminose A em menores de um ano. Métodos Estudo transversal com amostra de 93 crianças. Foram consideradas anêmicas crianças com hemoglobina < 11 g/dL. O ponto de corte utilizado para a classificação de valores baixos de vitamina A foi < 0,70 mMol/L, e para caracterizar deficiência < 0,35 mMol/L. As análises estatísticas foram processadas no software Stata 10.0. As variáveis estão apresentadas em frequência simples, a associação entre os fatores analisados e as deficiências de ferro e vitamina A foi realizada por meio do teste do qui-quadrado de Pearson. Para a comparação de médias de hemoglobina e vitamina A, utilizaram-se os testes t de Student para variáveis paramétricas e o de Mann-Whitney para as não paramétricas. Resultados A prevalência, de anemia e de hipovitaminose A, foi de 29,03% e de 19,10%, respectivamente. Encontraram-se ainda, valores baixos de vitamina A em 90,32% das crianças. Baixa idade e baixa escolaridade materna associaram-se com a presença de anemia ferropriva. Valores baixos de vitamina A foram significativos em mães não brancas. As prevalências de inadequação do consumo de ferro e vitamina A foram de 23,66% e 22,58%, respectivamente. Conclusão Evidencia-se elevada prevalência dessas enfermidades, ressaltando-se a importância da adoção de medidas preventivas.


Abstract Objective To assess the iron deficiency anemia and vitamin A deficiency prevalence and associated factors in children under one year. Methods Cross-sectional study with a sample of 93 children. Children with hemoglobin <11 g/dL were considered anemic. The cutoff point used for the classification of low levels of vitamin A was < 0.70 mMol/L and to characterize disability < 0.35 mmol/L. Statistical analyzes were carried out using Stata 10.0 software. Variables were presented in simple frequency and the association among the factors and deficiencies of iron and vitamin A was performed by the Pearson chi- square test. Mean hemoglobin and vitamin A were compared using the Student t test for parametric variables and Mann-Whitney test for nonparametric. Results The prevalence of anemia and vitamin A deficiency was 29.03% and 19.10%, respectively. It was found also low vitamin A values in 90.32% of children. The presence of iron deficiency anemia was associated to low maternal age and education. Low values of vitamin Awere significant in non-white mothers. The prevalence of inadequate iron intake and vitamin A was 23.66% and 22.58%, respectively. Conclusion This study evidences the high prevalence of these diseases, highlighting the importance of adopting preventive measures.

Resumo O estudo teve como objetivo verificar os fatores associados às velocidades de ganho de peso (VGP) e de comprimento (VGC) de lactentes nos primeiros quatro a seis meses de vida. Estudo transversal, com análise de dados secundários, no qual foram obtidas informações em prontuários de atendimento de 371 lactentes acompanhados por um Programa de Apoio à Lactação. Foram calculadas as VGP e VGC em lactentes do nascimento aos quatro a seis meses de vida. Foram investigados os fatores associados às VGP e VGC por meio de regressão linear múltipla. Verificou-se que os lactentes do sexo feminino e os que não estavam em aleitamento materno exclusivo ou predominante apresentaram menor VGP aos quatro a seis meses. Os lactentes cujas mães não viviam com companheiro, os filhos de mães que trabalhavam fora do lar e as que realizaram menos de seis consultas no pré-natal apresentaram menor VGC. Lactentes com maior comprimento ao nascer também apresentaram menor VGC aos quatro a seis meses. Os resultados deste estudo evidenciam a diversidade de fatores que podem estar associados às VGP e VGC nos primeiros meses de vida, como a prática do aleitamento materno, a atenção pré-natal e as variáveis socioeconômicas. A identificação precoce dos fatores relacionados ao crescimento de lactentes pode direcionar ações para promoção do adequado desenvolvimento nessa fase.


The study aimed to verify factors associated with weight (WGR) and length (LGR) gain rates of infants during their first 4 to 6 months of life. A cross-sectional study with secondary data analysis, that collected information on medical charts of 371 infants followed during their first six months of life by the Lactation Support Program. The WGR and length LGR gain of infants from birth to 4-6 months old were calculated. Factors associated with WGR and LGR were examined by multiple linear regression analysis. We found that female infants and those that were not exclusively or predominantly breastfed had lower WGR at 4-6 months. Infants whose mothers were not living with a partner, infants of working mothers, and mothers that had less than six prenatal visits during their pregnancy had a lower LGR. Infants with a greater birth length also had a lower LGR at 4-6 months. The results of this study highlight the diversity of factors the may be associated with the length and weight gain rate during the first months of life, such as breastfeeding, prenatal care and socioeconomic variables. The early identification of factors related to infants growth can direct actions promote a proper development on that stage.

ABSTRACT

Este artigo faz uma revisão dos estudos que avaliaram a influência de diferentes micronutrientes sobre os componentes da síndrome metabólica (SM) em crianças, discutindo as bases bioquímicas e mecanismos envolvidos. O levantamento bibliográfico foi realizado a partir da busca nas bases de dados eletrônicas PubMed, Science Direct e SciELO, utilizando, inicialmente, os descritores "micronutrientes" e "síndrome metabólica" e, em seguida, os termos referentes a cada micronutriente e aos componentes da síndrome metabólica, ambos combinados ao descritor "criança", em português e seus respectivos correlatos em inglês e espanhol. Não se delimitou o período das publicações, tendo em vista que os trabalhos sobre o tema são relativamente novos na população infantil. Foram encontrados estudos com micronutrientes antioxidantes, vitaminas do complexo B, vitamina D, zinco, ferro, sódio, cálcio e magnésio, os quais sugerem uma possível associação destes com a presença de componentes da SM na na infância. Mecanismos bioquímicos e fisiológicos envolvidos fornecem algumas bases plausíveis para sustentar essas evidências epidemiológicas. Observa-se que ainda há uma escassez de trabalhos sobre o tema, sendo necessários mais estudos, de forma a contribuir para o esclarecimento e confirmação de vários aspectos da dieta relacionados com o desenvolvimento da SM.
