ABSTRACT

BACKGROUND: Several conservative and surgical treatment methods have been demonstrated to be useful in the management of lymphedema. METHODS: In a patient with an enormous lymphedema of the leg, we first used complex decongestive physiotherapy as proposed by Földi et al. in 1989, and then followed it with surgery. RESULTS: Satisfactory results were obtained in a short period of time and were maintained during the 2-year follow-up. CONCLUSIONS: We recommend a combined medical, surgical, and physiotherapeutic approach in patients with marked lymphedema.

ABSTRACT

A male patient of 52 years old is studied with an scabiei by Sarcoptes scabiei v. hominis. After a month the patient developed a clinic and histopathologic generalized granuloma annulare. With the specific local treatment of his scabiei, the disease was cured in 7 days. After this treatment, the generalized granuloma annulare disappeared within 2 months. Due to the sequence of both diseases and the way in which they disappeared after local therapy, we suggest that the antigens of the scabies are the ones responsible for the granulomatous lesions. We concluded this since once the antigens have disappeared, the immunologic mechanisms depending on the interactions of lymphocyte T-macrophage which form the granuloma are interrupted. Since the patient is homosexual, we studied the HIV antibodies which were negatives in several opportunities. Nevertheless, an immune disbalance which could be the coadjuvant factor was founded. With this case we show that specific exogens may cause a generalized granuloma annulare.

ABSTRACT

We studied a white skin type 3 male patient aged 68 years old with a paraneoplastic acrokeratosis of Bazex, suffering from an oesophagus neoplasm. Cutaneous manifestations preceded in months the cancer. Dermatosis affected fingers of hands and feet, soles, nose and helix. Black melanic hyperpigmentation was the first cutaneous sign and were followed by an hyperkeratosis, becoming the most clinical characteristic of the syndrome. The sequence of these signs induces us to suggest that hormonal factors elaborated by the tumoral cells are the ones responsible for the pigmentary changes since they appeared before the hyperkeratosis.

ABSTRACT

Se realizó un estudio retrospectivo para conocer la asociación entre liquen plano, hepatitis crónica y cirrosis. Se analizaron 11280 historias del Hospital de Clíncias (Montevideo). La incidencia de liquen plano fue de 108/11280 casos (0,95%. de hepatitis crónica 183/11280 casos (1,62%) y cirrosis 260/11280 casos (2,30%). Un único paciente tenía liquen plano y hepatitis crónica (chi cuadrado 0,328) y 2 pacientes liquen plano y cirrosis (chi cuadrado 0,099). Ambas asociaciones no son de significación estadística. Los resultados sugieren que la presentación clínica entre estas afecciones es casual

ABSTRACT

Estudios clínicos y la función in vitro del neutrófilo fueron estudiados en 10 pacientes con acné vulgar y lesiones inflamatorias severas tratados con isotretinoína (134-cis-retinoide). Luego de 12 semanas de tratamiento se observó la mejoria clínica de las lesiones. Al inicio se comprobó en todos los pacientes una depresión de las funciones quimiotácticas, prueba NAT y actividad lisosómica, que fueron más evidentes cuando se inclcuyó suero autólogo en estas pruebas. Luego de 8 semanas de tratamiento la función quimiotáctica y la prueba NAT aumentaron significativamente. En cambio, la actividad lisosómica aumentó significativamente luego de 12 semanas de tratamiento. La mejoría de la función in vitro se evidencío también en las pruebas con suero autológo. Los autores sugieren que la isotretinoína posee múltiples acciones locales y sistémicas, en particular la disminución del Priopionibacterium acnes con la subsequente disminución de las respuestas inmunes específicas e inespecíficas

Desde el punto de vista histopatológico, se trata de un tumor vascular linfático sin malignidad citológica, pero arquitecturalmente de carácter disecante e infiltrante, biológicamente maligno, originado en restos embrionários. Actuarían eventualmente como factores inductores de su desarrollo el traumatismo y estímulos de carácter hormonal

ABSTRACT

Se describe la primer observación de un granuloma cutáneo por Mycobacterium vaccae en el Uruguay, considerada como una micobacteria no patógena incluida en el grupo IV de Runyon. Es un paciente de 58 años de edad con dos lesiones de rodilla de 5 meses de evolución en el momento de suprimer consulta. Las mismas son secundarias a un mínimo traumatismo sobre tierra contaminada con excretas de vacuno. El diagnóstico fue realizado por el hallazgo de la bacteria en material contraído de la lesión, su aislamiento en cultivo, y la reproducción experimental de lesiones en la almohadilla plantar del hamster dorado. Se destaca su evolución regresiva favorable con la termoterapia local

ABSTRACT

Using immunocytochemical methods the inflammatory-cell infiltrate from 7 halo nevi were studied. Monoclonal antibodies, Cris-1, OKT3, OKT4, OKT8, A47, Edu-1, OKT6 and anti-immunoglobulins were employed. The results shows that the predominant cells in the inflammatory-cell infiltrate were T cytotoxic/suppressor activated lymphocytes. An increased number of epidermal Langerhans cells and IgM deposits in epidermal-dermal junction was also observed. Thus, the results are able to postulate that self-involution of halo nevi was mediated by cytotoxic cellular immune reactions.

ABSTRACT

The evoked potentials visual and auditory are studied in 43 patients with latent syphilis without previous mucocutaneous lesions. Twenty nine of them had not received treatment, and 14 due to their positive serology were treated with benzathine penicillin. At the visual potential evoked there were no abnormalities but in 20 cases evoked auditive potential were abnormal. This findings suppose the existence of subclinical neurological indicates the existence of lesions in almost 50% of the cases of this period of syphilis. All these patients had positive circulate immune complexes and normal cerebro-spinal fluid. The neurological alteration consists in a lesion of the cerebral trunk. According to these results the auditory potential evoked test is very important. The authors doubt on the efficacity of the benzathine penicillin in this period of syphilis.

ABSTRACT

With the acquisition of specialized laboratory techniques, as the determination of serum immune complexes and the auditory evoked potentials, which bring new knowledge, the study of syphilis must be reviewed, in particular the classification of the evolution periods. The authors consider latent syphilis and neuro-syphilis. Based on these facts the new ways of treatment as well as the evolution control must be revised. These test should always be performed in order to determine exactly the syphilis period of the patient.

ABSTRACT

The authors describe the clinical and histological characteristics of palmar keratose associated to visceral cancer. Their clinical forms are small and numerous keratotic rings, large papulae, 4 to 8 mm., scarce two or three lesions and multiple punctiform depressions. The histology shows a compact orthokeratotic hiperkeratosis accompanied in a variable way to an epidermic depression. In a strict sense it is not a paraneoplase, it is a clinic visceral cancer marker. It is suggested to name it oncogenic lenticular acrokeratose.

ABSTRACT

A case of connatal flabby skin or generalized dermatochalasia without genetical familial background is presented. The cutaneous clinic is striking to which pulmonary emphysema with overcharge of the right heart is added. By optical and electronic microscopy the characters of elastic fibers are studied which show diminished with orceing with a normal microfibrilar component lacking the amorphous element or elastine with the electronic microscope. The formation of the dense normal element also appears compromized .

ABSTRACT

The authors study three cytotoxicity system in psoriasis: direct cytotoxicity (DCC), antibodies mediated cytotoxicity (ADCC) and activated lymphocytes mediated cytotoxicity (PCC). The results of DCC and PCC are in the normal values, while ADCC are a somewhat increased, showing a hyperactivity of this system, which could explain some physiopathological phenomena of this disease. We consider this finding as secondary to an alteration of the immune system by dysbalance of B-T lymphocytes.
