ABSTRACT

Soft tissue sarcomas (STS) account for less than 1% of adult cancers with a median overall survival of 12 months in the metastatic setting. Although chemotherapy remains the standard of treatment for advanced disease, molecular targeted agents (MTAs) and immunotherapies are under intensive investigation in STS. The success of MTAs comes mainly from antiangiogenic agents in various STS subtypes, from colony-stimulating factor-1 receptor inhibitor in tenosynovial giant cell tumor and neurotrophic tropomyocin receptor kinase (NTRK) inhibitors while others, such as cyclin-dependent kinase (CDK)-4 inhibitors, remain under evaluation. In advanced STS the activity of single-agent immunotherapy was not paradigm-changing as in other tumor types. A better understanding of tumor microenvironment, the immunogenic properties of MTAs, and finding an optimal treatment combination to improve patients outcomes became a central topic of research and discussion. Furthermore, the development and incorporation of transcriptomic profiling-based classification will allow identification, refined patient selection, and guided-treatment assignment. This article reviewed recent advances in STS treatment in MTAs and immunotherapy, strategies to overcome resistance, and outcomes of combination treatments in different STS subtypes. Promising preliminary results from combination strategies have shed light on STS treatment. The increasing understanding of this heterogeneous group of tumors and its microenvironment biology may help develop and guide treatment strategies with MTA and immunotherapies, alone or in combination, in a tailored way based on predictive and validated biomarkers and tumor molecular profiling in this new coming era.

ABSTRACT

BACKGROUND: An extremely elevated erythrosedimentation rate (ESR), defined as equal or higher than 100 mm/h, has been linked to serious underlying conditions, such as infections, connective tissue and oncologic disease. AIM: To analyze a group of patients in order to determine the underlying diagnosis and the characteristics associated with extremely elevated ESR in our environment. METHODS: Cross-sectional study of adult patients, who presented with at least one ESR equal or higher than 100 mm/h at Hospital Italiano, in Buenos Aires (Buenos Aires, Argentina) between January 2002 and August 2014. RESULTS: During the previously stated period of time, we analyzed the results of 879 patients. All patients were over 18 years of age. The median for the ESR results was 111 mm/h (interquartile range 105-120). The most prevalent etiology of an elevated ESR was infectious (41.64%), followed by malignancies (21.62%) and autoimmune / inflammatory diseases (12.97%). The most frequent individual diagnosis found was pneumonia (11.49%), followed by undetermined causes (5.92%). CONCLUSION: When comparing inpatient versus outpatient populations, the most frequent cause was infectious in the former group, while malignancies were the most frequent diagnosis in the latter.

Resumen Introducción: Una velocidad de eritrosedimentación (VHS) extremadamente elevada, definida como mayor o igual a 100 mm/h, se ha asociado a condiciones graves subyacentes como enfermedades infecciosas, enfermedades del colágeno u oncológicas. Objetivo: Analizar un grupo de pacientes para determinar los diagnósticos de base y las características que se asocian con valores de VHS mayores a 100 mm/h en nuestro medio. Pacientes y Métodos: Estudio tipo observacional de corte transversal, con recolección retrospectiva de datos de pacientes adultos con al menos un valor de VHS mayor o igual a 100 mm/h, registrado en el laboratorio entre enero de 2002 y agosto de 2014 en el Hospital Italiano de Buenos Aires. Resultados: Durante el período evaluado se analizaron 879 pacientes mayores de 18 años. La mediana de los valores de VHS fue 111 mm/h (Rango intercuartil 105-120). La etiología prevalente de VHS elevada fueron las enfermedades infecciosas (41,6%), seguida de malignidad (21,6%) y de autoinmune/inflamatoria (12,9%). El diagnóstico individual más frecuente fue el de neumonía (11,4%), seguido por causa indeterminada (5,9%). Conclusión: En pacientes internados, la causa más frecuente de VHS ≥ 100 mm/h fue las enfermedades infecciosas, mientras que en pacientes ambulatorios la causa más frecuente fue la malignidad.

ABSTRACT

Introducción: la Residencia de Medicina Interna del Hospital Italiano de Buenos Aires posee una larga trayectoria en la formación de médicos clínicos e internistas. Existe bibliografía que sugiere que las percepciones de los residentes no son consistentes con las de aquellos encargados de organizar y evaluar los programas de residencia. Nuestro objetivo es conocer las percepciones de los residentes acerca de cuáles son las instancias formativas que mejor contribuyen a la generación de conocimiento. Materiales: estudio de corte transversal utilizando una encuesta virtual que fue enviada a todos los Residentes de Medicina Interna del Hospital Italiano de Buenos Aires. Se recolectaron datos generales de los residentes y año de residencia. Posteriormente se evaluaron las diferentes instancias involucradas en el aprendizaje: pases de guardia, recorrida de internación, pases de ambulatorio, pases de internación, ateneos y, por último, los cursos superiores universitarios. Resultados: la actividad individual más reconocida por su papel en el aprendizaje fue la discusión de los pacientes durante la recorrida (18% de los residentes). En contrapartida, la participación en cursos superiores universitarios no fue elegida como estrategia útil desde el punto de vista individual. Específicamente respecto de la utilidad de los pases de guardia (realizados por los residentes de primer año y los jefes de residentes), gran parte de los participantes optó por el pase de la tarde como el momento de mayor utilidad para el aprendizaje. Conclusión: los residentes consideran que los pases de guardia de la tarde, los pases de internación y ambulatorios y los ateneos de internación son los momentos más propicios y eficaces para la adquisición de nuevo conocimiento. Dichos datos pueden resultar de utilidad en la planificación de diversas instancias educativas y servir como base para estudios similares en poblaciones disímiles. (AU)


Introduction: the Internal Medicine Program at the Hospital Italiano de Buenos Aires has long term experience in training medical physicians. There is mounting evidence suggesting that residents' perceptions about their education are not consistent with the ones of who are responsible for designing and evaluating residency programs. Our goal is to evaluate residents' perceptions on the instances that contribute the most to their training. Methods: we conducted a cross-sectional study using an online survey offered to all Internal Medicine Residents at Hospital Italiano de Buenos Aires, Argentina. Baseline characteristics of each resident were collected. We subsequently assessed residents' perceptions of different instances of the residency program: residents´ morning and evening rounds, patients´ rounds, case discussions with staff physicians, grand rounds and postgraduate courses. Results: overall, the activity that was perceived as the most relevant for their education was the residents' evening rounds (18% of the residents). Conversely, participation in postgraduate courses was not selected as an important learning resource individually. Most residents highlighted the importance of patient discussion with their chief resident during morning and evening rounds. Conclusion: residents systematically prefer patient discussion on medical rounds rather than lectures and courses. This information might prove useful in the future so as to improve Internal Medicine residency programs. (AU)
