ABSTRACT

The objective of this study was to determine the incidence of respiratory viruses associated with severe pneumonia among children less than 2 years of age in the rural district of Matiari in Sindh, Pakistan. This study was a community-based prospective cohort active surveillance of infants enrolled at birth and followed for 2 years. Cases were identified using the World Health Organization's Integrated Management of Childhood Illnesses' definition of severe pneumonia. Nasopharyngeal swabs were obtained for assessment by multiplex RT-PCR for eight viruses and their subtypes, including RSV, influenza virus, human metapneumovirus, enterovirus/rhinovirus, coronavirus, parainfluenza virus, adenovirus, and human bocavirus. Blood cultures were collected from febrile participants. A total of 817 newborns were enrolled and followed with fortnightly surveillance for 2 years, accounting for a total of 1,501 child-years of follow-up. Of the nasopharyngeal swabs collected, 77.8% (179/230) were positive for one or more of the above mentioned respiratory viruses. The incidence of laboratory confirmed viral-associated pneumonia was 11.9 per 100 child-years of follow-up. Enterovirus/rhinovirus was detected in 51.7% patients, followed by parainfluenza virus type III (8.3%), and RSV (5.7%). Of the uncontaminated blood cultures, 1.4% (5/356) were positive. Respiratory viruses are frequently detected during acute respiratory infection episodes in children under 2 years old in a rural community in Pakistan. However, causal association is yet to be established and the concomitant role of bacteria as a co-infection or super-infection needs further investigation. J. Med. ABSTRACT

The purpose of this study was to identify the 100 top-cited articles in the radiology of trauma, analyze the resulting database to understand factors resulting in highly cited works, and establish trends in trauma imaging. An initial database was created via a Web of Science (WOS) search of all scientific journals using the search terms "trauma" and either "radiology" or a diagnostic modality. Articles were ranked by citation count and screened by two attending radiologists plus a tiebreaker for appropriateness. The following information was collected from each article: WOS all database citations, year, journal, authors, department affiliation, study type and design, sample size, imaging modality, subspecialty, organ, and topic. Citations for the top 100 articles ranged from 82-252, and citations per year ranged from 2.6-37.2. A plurality of articles were published in the 1990s (n = 45) and 1980s (n = 31). Articles were published across 24 journals, most commonly Radiology (n = 31) and Journal of Trauma-Injury, Infection, and Critical Care (n = 28). Articles had an average of five authors and 35 % of first authors were affiliated with a department other than radiology. Forty-six articles had sample sizes of 100 or fewer. Computed tomography (CT) was the most common modality (n = 67), followed by magnetic resonance (MR; n = 22), and X-ray (XR; n = 11). Neuroradiology (n = 48) and abdominal radiology (n = 36) were the most common subspecialties. The 100 top-cited articles in the radiology of trauma are diverse. Subspecialty bibliometric analyses identify the most influential articles of a particular field, providing more implications to clinical radiologists, trainees, researchers, editors, and reviewers than radiology-wide lists.

ABSTRACT

OBJECTIVES: To determine the burden and molecular epidemiology of rotavirus gastroenteritis in children hospitalized with severe acute watery diarrhea in Pakistan prior to introduction of rotavirus vaccine. METHODS: A cross-sectional study was carried out over a period of two years from 2006 - 2008 at five sentinel hospitals in the cities of Karachi, Lahore, Rawalpindi, and Peshawar. Stool samples collected from children under five years of age hospitalized with severe acute watery diarrhea were tested for rotavirus antigen via enzyme immunoassay (EIA) (IDEA REF K6020 Oxoid Ltd (Ely), Cambridge, United Kingdom). A subset of EIA positive stool samples were further processed for genotyping. RESULTS: 6679 children were enrolled and stool specimens of 2039 (30.5%) were positive for rotavirus. Rotavirus positivity ranged from 16.3% to 39.4% in the 5 hospitals with highest positivity in Lahore. 1241 (61%) of all rotavirus cases were in infants under one year of age. Among the strains examined for G-serotypes, the occurrence of G1, G2, G9 and G4 strains was found to be 28%, 24%, 14% and 13%, respectively. Among P-types, the most commonly occurring strains were P6 (31.5%) followed by P8 (20%) and P4 (12%). Prevalent rotavirus genotype in hospitalized children of severe diarrhea were G1P[8] 11.6% (69/593), followed by G2P[4] 10.4% (62/593), and G4P[6] 10.1% (60/593). CONCLUSIONS: Approximately one third of children hospitalized with severe gastroenteritis in urban centers in Pakistan have rotavirus. Introduction of rotavirus vaccine in Pakistan's national immunization program could prevent many severe episodes and diarrheal deaths.
