ABSTRACT

Stroke is a leading cause of adult disability worldwide, and better drugs are needed to promote functional recovery after stroke. Growing evidence suggests the critical role of network excitability during the repair phase for stroke recovery. Here, we show that ß-hydroxybutyrate (ß-HB), an essential ketone body (KB) component, is positively correlated with improved outcomes in patients with stroke and promotes functional recovery in rodents with stroke during the repair phase. These beneficial effects of ß-HB depend on HDAC2/HDAC3-GABA transporter 1 (GAT-1) signaling-mediated enhancement of excitability and phasic GABA inhibition in the peri-infarct cortex and structural and functional plasticity in the ipsilateral cortex, the contralateral cortex, and the corticospinal tract. Together with available clinical approaches to elevate KB levels, our results offer a clinically translatable means to promote stroke recovery. Furthermore, GAT-1 can serve as a pharmacological target for developing drugs to promote functional recovery after stroke.

ABSTRACT

Objective: This study aimed to assess the impact of urogenital ureaplasma urealyticum (Uu), Mycoplasma hominis (Mh), and Chlamydia trachomatis (Ct) infections on semen quality in men.Methods: In this study, 1022 males were enrolled at the Department of Reproductive Medicine, Rizhao People's Hospital, Shandong Province from October 2014 to January 2023. The participants included 393 in the infertility group, 139 in the recurrent miscarriage group, and 490 in the control group. Based on age, 852 cases were < 36 years old, and 170 cases were ≥ 36 years old. All patients underwent routine semen analysis and tests for Uu, Mh, and Ct, with results statistically analyzed for their impact on semen quality and compared among different age groups. Results: Among the 1022 patients, 344 (33.6%) were Uu-positive, 49 (4.7%) were Mh-positive, and 31 (3.0%) were Ct positive. The sperm concentration, total sperm count, forward sperm motility rate (PR), sperm motility rate (PR+NP) and normal sperm morphology rate of Uu Mh and/or Ct-positive patients were significantly lower than those of the negative group, and the overall difference between the two groups was statistically significant (P<0.05). The positive rate of Uu infection was 41.7% in the infertility group, 30.2% in the recurrent miscarriage group and 28.2% in the control group, and the overall positive rate of the three groups was statistically significant(P<0.05). The positive rate of Mh infection was 6.9% in the infertility group, 8.6% in the recurrent miscarriage group and 2.0% in the control group, and the overall positive rate of the three groups was statistically significant (P<0.001). The positive rate of Ct infection was 6.1% in the infertility group, 2.9% in the recurrent miscarriage group and 0.6% in the control group, and the overall positive rate of the three groups was statistically significant (P<0.001). The positivity rate of Uu infection was 35.8% at the age of <36 years and 22.9% at the age ≥ 36 years, and there was a statistically significant difference in the positivity rate between the two groups (P<0.05). Conclusion: The prevalence of Uu infection in the male urogenital tract is significantly higher than that of Mh and Ct, which is the main pathogen of urogenital tract infection in men. Uu, Mh and Ct infections have adverse effects on sperm concentration, total sperm count, sperm forward motility rate (PR), sperm motility rate (PR+NP) and normal sperm morphology rate, which will lead to a decrease in semen quality and affect male fertility. Genital tract infections are closely related to age, and the prevalence of Uu infection is higher in the younger age group.

ABSTRACT

OBJECTIVE: To observe the effects of matrix acupuncture on the neck disability index (NDI) score, clinical efficacy, and the calcification size of the nuchal ligament in patients of cervical spondylotic radiculopathy with nuchal ligament calcification. METHODS: A total of 120 cases were randomly divided into a matrix acupuncture group and a routine acupuncture group, with 60 cases in each group. In the matrix acupuncture group, Ashi-point, bilateral Tianzhu (BL10), bilateral Fengchi (GB20), bilateral Dazhu (BL11), bilateral Jianzhongshu (SI15), and Jianjing (GB21), Tianzong (SI11), Quchi (LI11), Shousanli (LI10), Waiguan (TE5), and Hegu (LI4) at the affected side were selected. In the routine acupuncture group, C3-C7 Jiaji points at the neck and Jianjing (GB21), Tianzong (SI11), Quchi (LI11), Shousanli (LI10), Waiguan (TE5), Hegu (LI4), and Ashi-point at the affected side were selected. The patients in the two groups were treated 30 min once, six days a week, for a total of four weeks. The NDI scores, clinical efficacies, and calcification sizes of nuchal ligament were compared between the two groups every wee-kend. RESULTS: After four weeks of treatment, the NDI scores and calcification volumes of nuchal ligament decreased in both groups (P<0.05). Compared with the routine acupuncture group, the matrix acupuncture group showed decreased NDI scores and reduced calcification volumes of nuchal ligament at the 1st, 2nd, 3rd, and 4th weeks of treatment (P<0.05). The cure and marked effective rate in the matrix acupuncture group at the 2nd, 3rd, and 4th weeks of treatment were superior to those of the routine acupuncture group (P<0.05). CONCLUSION: The matrix acupuncture group and the routine acupuncture group are effective in reducing the NDI score and calcification size of nuchal ligament in patients of cervical spondylotic radiculopathy with nuchal ligament calcification. However, matrix acupuncture has obvious advantages over routine acupuncture.

ABSTRACT

OBJECTIVE: To observe the effect of electroacupuncture (EA) on the expression of soluble guanylate cyclase (sGC), cyclic guanosine phosphate (cGMP) and protein kinase G (PKG) of cerebral vascular smooth muscle in cerebral infarction (CI) rats, so as to study its dynamic regulation mechanism. METHODS: Male Wistar rats were randomly divided into normal control (n=10), sham operation (n=40), model (n=40), and EA (n=40) groups, and the latter three groups were further di-vided into 3, 6, 12 and 24 h subgroups (n=10 in each subgroup). The CI model was established by occlusion of the middle cerebral artery (MCAO). EA(15 Hz, 2 mA)was applied to "Shuigou" (GV26) for 20 min. The cGMP, sGC and PKG activity and expression levels in the vascular smooth muscle of cerebral artery were detected using ELISA and Western blot, respectively. RESULTS: After modeling, the immunoactivity and activities of sGC at 3 h, PKG at 3 and 6 h and cGMP from 3 h to 24 h were ob-viously decreased in the model group relevant to the normal control and sham-operation groups (P<0.05, P<0.01). After the intervention, the expression levels and activities of sGC at 3 h, PKG at 3 and 6 h and cGMP at 3 and 6 h were apparently up-regulated in the EA group relevant to the model group (P<0.05). CONCLUSION: EA intervention can significantly inhibit the down-regulation of sGC, PKG and cGMP expression of cerebral artery smooth muscle in MCAO model rats, which plays an important role in inhibiting cerebral artery smooth muscle spasm after ischemia, maintaining normal vascular function and state, and thus increasing blood perfusion around cerebral infarction area. However, acupuncture effect has a certain time-effectiveness.

ABSTRACT

OBJECTIVE: To observe the difference of circulating tumor cells (CTC) in peripheral blood of patients with different degrees of cervical lesions, and to evaluate the effectiveness of CTC detection in screening early invasive cervical cancer. METHODS: From December 2015 to October 2017, 63 cases of cervicitis, low-grade and high-grade intraepithelial lesions (LSIL, HSIL) and early invasive cervical cancer were confirmed by histopathological and clinical stages in Zhongshan Boai Hospital and Zhongshan Hospital Affiliated to Zhongshan University. The immunomagnetic bead negative enrichment technique combined with immunofluorescence was used. In situ hybridization (imFISH) was used to detect CTC in peripheral blood of patients. The positive rate and quantity of CTC in four groups were analyzed. The diagnostic efficacy of CTC in early invasive cervical cancer was evaluated based on the results of histopathological diagnosis and clinical staging. RESULTS: ①The positive rates of CTC in cervicitis group, LSIL group, HSIL group and early invasive cervical cancer group were 0, 0, 19.05% and 84.13% respectively. The overall difference was statistically significant ( χ 2=504.00, P<0.05). The positive rate of CTC in early invasive cervical cancer group was higher than that in other groups ( P<0.008 3). The positive rates of CTC in HSIL group were significantly different from those in LSIL group and cervicitis group ( P<0.008 3). ②The average number (median) of CTC positive in cervicitis group, LSIL group, HSIL group and early invasive cervical cancer group was 0, 0, 1/4 mL, 3/4 mL, respectively. The average number of positive CTC in early invasive cervical cancer group was higher than that in other groups, and the difference was significant compared with other groups ( P<0.008 3). The average number of CTC positive in HSIL group was significantly different from that in LSIL and cervicitis group ( P<0.008 3). ③The sensitivity, specificity, coincidence rate, positive predictive value and negative predictive value of CTC positive results in the diagnosis of early invasive cervical cancer were 84.13%, 93.65%, 91.27%, 81.54% and 94.65%, respectively. CONCLUSION: CTC exists in patients with HSIL and early invasive cervical cancer. With the aggravation of cervical lesions, the positive rate and number of CTC test results increase. CTC detection in early invasive cervical cancer screening has a certain practical value in clinic.

The structures of pestalrones A-B were revised via reinterpretation of the NMR data and a brief chemical transformation from the co-occurring polyketides, in our investigation on the secondary metabolites of Pestalotiopsis zonata, which also afforded a new α-pyrone derivative, pestazonatic acid, and four known analogs.

ABSTRACT

A new hybrid of dehydroergosterol and nitrogenous alternariol derivative, pestauvicomorpholine A (1), and three alternariol analogues (2-4) including a new aminated one, pestauvicolactone A (2), were isolated from the fermentation product of the fungus Pestalotiopsis uvicola on rice media. Compounds 1 and 2 represent the first example of aza-alternariol and aza-alternariol-steroid derived from transamination followed by intermolecular hetero-Diels-Alder reaction.

ABSTRACT

Wet chemical etching is a key process in fabricating silicon (Si) nanostructures. Currently, wet etching of Si is proposed to occur through the reaction of surface Si atoms with etchant molecules, forming etch intermediates that dissolve directly into the bulk etchant solution. Here, using in situ transmission electron microscopy (TEM), we follow the nanoscale wet etch dynamics of amorphous Si (a-Si) nanopillars in real-time and show that intermediates generated during alkaline wet etching first aggregate as nanoclusters on the Si surface and then detach from the surface before dissolving in the etchant solution. Molecular dynamics simulations reveal that the molecules of etch intermediates remain weakly bound to the hydroxylated Si surface during the etching and aggregate into nanoclusters via surface diffusion instead of directly diffusing into the etchant solution. We confirmed this model experimentally by suppressing the formation of nanoclusters of etch intermediates on the Si surfaces by shielding the hydroxylated Si sites with large ions. These results suggest that the interaction of etch intermediates with etching surfaces controls the solubility of reaction intermediates and is an important parameter in fabricating densely packed clean 3D nanostructures for future generation microelectronics.

ABSTRACT

Two new d-threitol orsellinates (1-2) were isolated from the EtOAc extract of a sea mud-derived fungus, Ascotricha sp. ZJ-M-5, cultured in Czapek Dox broth. These two compounds featured in the symmetrical substitution of orsellinic acid or acetic acid, which was established on the basis of 1D and 2D NMR experiments. The characteristic optical rotations enabled the assignment of the absolute configuration.

ABSTRACT

OBJECTIVE: To compare the difference in the efficacy on spasmodic torticollis between matrix needling technique combined with puncturing "Wuxin points" and the local injection of botox type A. METHODS: Forty-two cases were randomized into an acupuncture group and a botox group, 21 cases in each one. In the acupuncture group, the matrix needling technique was used and acupuncture was adopted at Fengchi (GB 20), "Wuxin points" [Shuigou (GV 26), bilateral Laogong (PC 8) and bilateral Yongquan (KI 1)] and local Ashi points. In the botox group, botox type A was injected at several spots of local region. The treatment course was 1 month in the two groups. The efficacy in each weekend and the results of follow-up visit in half a year after treatment were compared between the two groups. RESULTS: In the 1st weekend, the markedly effective rate was 42.8% (9/21) and the total effective rate was 90.5% (19/21) in the botox group, which were better than those of 4.8% (1/21) and 28.6% (6/21) respectively in the acupuncture group (both P < 0.01). In the 2nd weekend, the markedly effective rate was 71.4% (15/21) and the total effective rate was 95.2% (20/21) in the botox group, which were better than those of 19.1% (4/21) and 61.9% (13/21) respectively in the acupuncture group (both P < 0.01). In the 3rd weekend, the markedly effective rate was 66.7% (14/21) and the total effective rate was 95.2% (20/21) in the botox group, which were better than those of 38.1% (8/21) and 71.4% (15/21) respectively in the acupuncture group (both P < 0.05). The markedly effective rate and the total effective rate were not different significantly between the two groups in the treatment of 1 month and the follow-up visit in half a year (all P > 0.05). There was no adverse reaction in the acupuncture group. There were 14 cases of adverse reactions in the botox group, in which 9 cases of neck muscular weakness and 5 cases of neck muscular pain. CONCLUSION: The treatment of matrix needling technique and combined with acupuncture at "Wuxin points" achieves the significant efficacy on spasmodic torticollis. Although the effect is not obtained as quickly as the local injection of botox, the efficacy of matrix needling technique is similar to that of botox in the treatment of 1 month and the follow-up visit in half a year. Moreover, this therapy has no adverse reaction.

ABSTRACT

OBJECTIVE: To explore the classification basis of functional dyspepsia in disease menu of acupuncture, and to provide regular analyzing process for classification of other diseases. METHODS: Taking the control group as the classification basis, and the 5-level literature classification of evidence based medicine as the estimation basis, literatures which complied with the inclusion criteria were examined according to the sequence from high level to low. Evidences of low levels were given up if support can be acquired from documents of high level. RESULTS: Acupuncture is approved to be effective for intervention on functional dyspepsia. Compared to the international accepted medicines such as Cisapride and Motilium, regular acupuncture has better therapeutic effects and less side effects in improving the gastro-power and relieving discomfort sensations. CONCLUSION: Functional dyspepsia pertains to grade I of disease menu of acupuncture.

ABSTRACT

OBJECTIVE: To compare therapeutic effects of herb-partitioned spread moxibustion and western medicine on chronic nonspecific ulcerative colitis. METHODS: Sixty cases were randomly divided into a spread moxibustion group (n = 28) and a western medicine group (n = 32). The spread moxibustion group was treated with herb-partitioned spread moxibustion at lower limb around stomach meridian, abdomen region around Guanyuan (CV 4) and lower Jiaji (EX B 2) points; and the western medicine group was treated with oral administration of Sulfasalazine. Their therapeutic effects were observed after treatment. RESULTS: The cured-markedly effective rate was 71.4% (20/ 28) in the spread moxibustion group, and 25.0% (8/32) in the western medicine group, the former was better than the latter (P < 0.05). CONCLUSIONS: The therapeutic effect of herb-partitioned spread moxibustion for treatment of chronic nonspecific ulcerative colitis is better than that of the oral administration of Sulfasalazine with less adverse reaction, and is worth popularizing in clinic.

ABSTRACT

OBJECTIVE: To study the chemical constituents of Schisandra pubescens. METHODS: The chemical constituents were isolated by silica gel, Prep-HPLC and Prep-TLC methods. And their structures were elucidated by physical-chemical and spectroscopic methods. RESULTS: Seven compounds were obtained and identified as (+)-gomisin K2 (I), gomisin S (II), Pregomisin (III), Schisantherin A (IV), Schicantherin B (V), Angeloylgomisin Q (VI), Rubrildilactione (VII). CONCLUSION: All compounds are isolated from this plant for the first time.
