ABSTRACT

PURPOSE: To assess the influence of the listener experience, measurement scales and the type of speech task on the auditory-perceptual evaluation of the overall severity (OS) of voice deviation and the predominant type of voice (rough, breathy or strain). METHODS: 22 listeners, divided into four groups participated in the study: speech-language pathologist specialized in voice (SLP-V), SLP non specialized in voice (SLP-NV), graduate students with auditory-perceptual analysis training (GS-T), and graduate students without auditory-perceptual analysis training (GS-U). The subjects rated the OS of voice deviation and the predominant type of voice of 44 voices by visual analog scale (VAS) and the numerical scale (score "G" from GRBAS), corresponding to six speech tasks such as sustained vowel /a/ and /ɛ/, sentences, number counting, running speech, and all five previous tasks together. RESULTS: Sentences obtained the best interrater reliability in each group, using both VAS and GRBAS. SLP-NV group demonstrated the best interrater reliability in OS judgment in different speech tasks using VAS or GRBAS. Sustained vowel (/a/ and /ɛ/) and running speech obtained the best interrater reliability among the groups of listeners in judging the predominant vocal quality. GS-T group got the best result of interrater reliability in judging the predominant vocal quality. CONCLUSION: The time of experience in the auditory-perceptual judgment of the voice, the type of training to which they were submitted, and the type of speech task influence the reliability of the auditory-perceptual evaluation of vocal quality.

ABSTRACT

Dynamic vocal analysis (DVA) is an auditory-perceptual and acoustic vocal assessment strategy that provides estimates on the biomechanics and aerodynamics of vocal production by performing frequency and intensity variation tasks and using voice acoustic spectrography. The objective of this experience report is to demonstrate the use of DVA in the assessment of vocal functionality of dysphonic and non-dysphonic individuals, with a special focus on the laryngeal musculature. Phonatory tasks consisted of sustained vowel, "a" or "é", and/or connected speech, in three intensities (habitual, soft, and loud) and three frequencies (habitual, high, and low), as well as ascending and descending glissando. The adjustments of the laryngeal and paralaryngeal muscles can be inferred from the different DVA tasks. The main characteristics of the laryngeal muscles analyzed are control of glottic adduction, stretching, and shortening of the vocal folds; the main characteristics of the paralaryngeal musculature are mainly related to the vertical laryngeal position in the neck. While the sustained vowel evaluates the vocal functionality with a focus on the larynx, connected speech allows the evaluation of the articulatory adjustments employed. An acoustic spectrographic software can be used to visualize the performance of such tasks. The clinical application of the DVA will be exemplified using acoustic spectrography plates from normal and dysphonic voices, taken from a voice bank. Individuals who perform the DVA tasks in a balanced way, with adequate vocal quality and without phonatory effort, demonstrate good vocal functionality. On the other hand, difficulties in performing these tasks with worsening vocal quality and/or increased muscle tension may be indications of altered vocal functionality.


O campo dinâmico vocal (CDV) é uma estratégia de avaliação vocal perceptivo-auditiva e acústica que oferece estimativas sobre a biomecânica e a aerodinâmica da produção vocal por meio da realização de tarefas de variação de frequência e de intensidade e do uso da espectrografia acústica da voz. Esse relato de experiência tem como objetivo demonstrar a aplicação do CDV na avaliação da funcionalidade vocal de indivíduos disfônicos e não-disfônicos, com foco especial na musculatura laríngea. As tarefas fonatórias envolvem emissão de vogal sustentada e/ou de fala encadeada em três intensidades autosselecionadas (habitual autorreferida, fraca e forte) e em três frequências (habitual autorreferida, aguda, grave), além do glissando. As tarefas de variação de intensidade e de frequência possibilitam a avaliação da musculatura laríngea no controle da adução glótica e do alongamento e encurtamento das pregas vocais. A tarefa de variação de frequência permite também a análise da musculatura extrínseca no controle da posição vertical da laringe no pescoço. Enquanto a vogal sustentada avalia a funcionalidade vocal com foco na laringe, a fala encadeada permite a avaliação dos ajustes articulatórios empregados. A aplicação do CDV será demonstrada por meio de pranchas espectrográficas de indivíduos normais e disfônicos. Indivíduos vocalmente saudáveis realizam as tarefas do CDV de forma equilibrada, com qualidade vocal adequada e sem esforço fonatório, denotando boa funcionalidade vocal. Por outro lado, indivíduos com dificuldade na realização das tarefas do CDV, com piora da qualidade vocal e/ou com aumento da tensão muscular, podem apresentar funcionalidade vocal alterada.

OBJECTIVES: Assess respiratory muscle strength, voice and the quality of life of patients with traumatic cervical and thoracic spinal cord injury. METHODS: Preliminary analytical case control study of 28 adult men, aged between 18 and 65 years, divided into two groups: a research group consisting of 14 patients with cervical and thoracic spinal cord injury, and control group (CG) with 14 noninjured individuals. The subjects had their maximum inspiratory and expiratory pressures measured, underwent voice handicap index-10 assessment, had their voices recorded for auditory perception analysis, using the voice deviation and GRBASI scales, and for acoustic analysis, based on extraction of the parameters selected and completed a quality-of-life questionnaire (WHOQOL-bref protocol). RESULTS: Intergroup comparison indicated that the research group exhibited lower respiratory muscle strength in relation to the CG, MIP and MEP P-values < 0.001 and < 0.001, respectively; and in relation to the device's reference value, MIP and MEP P-values < 0.001 and < 0.001, respectively; the patients reported no voice handicaps according to the voice handicap index; overall vocal deviation severity on the voice deviation was higher in the CG, P = 0.040; there was greater occurrence of roughness and instability on the GRBASI scale, P= 0.040; there was greater difficulty in modulating vocal frequency (P= 0.002) and resonance, with a laryngopharyngeal focus; the acoustic parameters were normal; and lower scores were obtained in the physical domain of quality of life (P< 0.001). CONCLUSION: The individuals with traumatic spinal cord and thoracic injury exhibited changes in respiratory measures, vocal deviation and dissatisfaction in the physical domain of the WHOQOL-bref protocol.

ABSTRACT

During the XXVIII Brazilian Congress of SBFa, 24 specialists met and, from a leading position on scientific research as a tool for connecting laboratory and clinic, five fronts of knowledge of the voice specialty were discussed as following: Perceptual-auditory judgment of vocal quality; 2. Acoustic analysis of the vocal signal; 3. Voice self-assessment; 4. Traditional techniques of therapy; 5. Modern techniques of electrostimulation and photobiomodulation (PBMT) in voice. Part "a" of this publication was associated with the consolidation of the analyses of the first three aspects. The trend in the perceptual-auditory judgment of vocal quality was related to the use of standard protocols. The acoustic evaluation of the vocal signal is accessible and can be done descriptively or by extraction of parameters, thus preferring multiparametric measures. Finally, the analysis of the individual himself closes this triad of voice documentation, which will be the basis for the conclusion of the evaluation, reference for monitoring progress, and evaluation of treatment results.


No XXVIII Congresso Brasileiro da SBFa, 24 especialistas reuniram-se e, a partir de um posicionamento condutor sobre pesquisa científica como ferramenta de conexão entre laboratório e clínica, cinco frentes de conhecimento da especialidade de voz foram discutidas: 1. Julgamento perceptivo-auditivo da qualidade vocal; 2. Análise acústica do sinal vocal; 3. Autoavaliação em voz; 4. Técnicas tradicionais de terapia; 5. Técnicas modernas de eletroestimulação e fotobiomodulação em voz. A parte "a" desta publicação é a consolidação das análises dos três primeiros aspectos. A tendência no julgamento perceptivo-auditivo da qualidade vocal é o uso de protocolos padrão. A avaliação acústica do sinal vocal é acessível e pode ser feita de modo descritivo ou por extração de parâmetros, preferindo-se medidas multiparamétricas. Finalmente, a análise do próprio indivíduo fecha essa tríade de documentação fonoaudiológica, que será base para a conclusão da avaliação, referência para monitoramento do progresso e avaliação de resultado de tratamento.

ABSTRACT

This text is the continuation of the XVIII SBFa Congress publication. In part "A" we presented the analyses on clinical vocal evaluation. Part "B" focuses on vocal rehabilitation: 4. Traditional techniques of vocal therapy; 5. Modern techniques of electrostimulation and photobiomodulation applied to vocal rehabilitation. The numerous studies on the various programs, methods, and techniques of traditional rehabilitation techniques, and many with high quality of evidence, allow us to consider such procedures relatively well described, safe, and with known effects, accounting for the treatment of various vocal disorders. The scientific evidence with traditional techniques is recognized worldwide. New fronts of evolution, with electrostimulation or photobiomodulation used to handle voice problems, seem to be promising as coadjutant approaches. There are more studies on electrostimulation in vocal rehabilitation than with photobiomodulation; however, scientific evidence for these two modern techniques is still limited. Knowledge and caution are required for the application of either technique.


O presente texto é a continuação da publicação referente ao XVIII Congresso da SBFa. Na parte "A" apresentamos análises sobre avaliação clínica vocal. O foco da parte "B" são aspectos de reabilitação vocal: 4. Técnicas tradicionais de terapia vocal; 5. Técnicas modernas de eletroestimulação e fotobiomodulação aplicadas à reabilitação. Os inúmeros estudos sobre os diversos programas, métodos ou técnicas tradicionais de reabilitação, muitos de elevada qualidade de evidência, permitem considerar tais procedimentos relativamente bem descritos, seguros e com efeitos conhecidos, dando conta do tratamento de diversos distúrbios vocais. As evidências científicas com as técnicas tradicionais são reconhecidas mundialmente. Novas frentes de evolução, como o uso da eletroestimulação ou fotobiomodulação em voz parecem ser promissoras como abordagens coadjuvantes. Há mais estudos sobre eletroestimulação em voz do que com fotobiomodulação, contudo, evidências científicas para essas duas técnicas modernas são ainda limitadas. Conhecimento e cautela são necessários para a aplicação de quaisquer técnicas.

ABSTRACT

PURPOSE: Verify the effect of anchor repetition in the perceptual auditory judgement of the type of vocal deviation performed by speech-language pathology (SLP) students; analyze the correlation between the amount of different vocal dimensions in the same stimuli and accuracy; investigate the correlation between type of vocal deviation and accuracy. METHODS: 54 SLP students were divided in two groups: Group with repetition (GwR), 28 students; and, Group with no repetition (GnR), 26 students. The analyzed sample counted with 220 dysphonic human voices, vowel /ε/. The GwR heard three anchor stimuli before the judgement and every 20 voices during the assessment. The GnR heard the anchor only before beginning the judgement. The anchor stimuli counted with one rough, one breathy and one strain voice. These classifications were compared with reference judgements from three expert SLPs. The intra and inter-rater reliability, the correlation between the amount of different vocal dimensions in the same stimuli and type of vocal deviation with accuracy were assessed. RESULTS: The accuracy between type of deviation was similar among groups. The GwR presented slightly higher intra and inter-rater reliability. The student's accuracy was inversely proportional to the amount of different vocal dimensions in the stimuli. Breathiness presented the highest accuracy and strain presented the lowest accuracy. CONCLUSION: The repetition of anchor stimuli improved intra and inter-rater reliability. However, it was not effective in the accuracy of the type of vocal deviation. The amount of different vocal dimensions in the stimuli have influence in the students' accuracy.


OBJETIVO: Verificar o efeito da repetição de estímulos âncoras no julgamento perceptivo auditivo do desvio vocal predominante realizado por estudantes de fonoaudiologia; analisar a relação entre o número de dimensões vocais dos estímulos sonoros e a acurácia; e investigar a relação entre o desvio vocal predominante e a acurácia. MÉTODO: Participaram da pesquisa 54 estudantes de fonoaudiologia, divididos em: Grupo Com Repetição - GCR com 28 alunos; e Grupo Sem Repetição - GSR, 26 alunos. A amostra vocal consistiu de 220 vozes humanas disfônicas, vogal /ε/. O GCR escutou três estímulos âncoras, uma voz rugosa, uma soprosa e uma tensa, no início da tarefa e a cada 20 vozes. O GSR escutou os estímulos âncoras somente no início da tarefa de avaliação. Estas classificações foram comparadas com respostas referências produzidas por 3 fonoaudiólogas experientes. Analisamos a acurácia e a confiabilidade intra e interavaliadores, a correlação entre número de dimensões vocais e acurácia e a relação entre desvio vocal predominante e acurácia. RESULTADOS: Os grupos tiveram desempenhos semelhantes na acurácia do desvio vocal predominante. A confiabilidade intra e interavaliadores foi discretamente maior no GCR. A acurácia dos alunos foi inversamente proporcional ao número de dimensões vocais presentes nos estímulos. O desvio vocal de maior acurácia foi a soprosidade, e a de menor, a tensão. CONCLUSÃO: A repetição dos estímulos âncoras melhorou a confiabilidade dos estudantes, mas não se mostrou efetiva na acurácia da classificação do desvio vocal predominante. O número de dimensões vocais nos estímulos sonoros interferiu na acurácia dos alunos.

OBJECTIVE: To determine the dimensions of mucosal defects that can be covered by a bipedicled vocal fold mucosal flap. METHODS: We used 20 adults human larynges (10 of each gender) excised from cadavers, divided into 2 groups of 10 larynges (5 of each gender) each. In one group (the normal flap group), we created the largest possible bipedicled vocal fold mucosal flap and then quantified the dimensions of the largest defect that could be covered by displacing the flap medially. In the other group (the augmented flap group), the flap was augmented laterally with mucosa from the laryngeal ventricle and we determined whether the larger flap would effectively cover larger defects. RESULTS: The mean width of mucosal defect capable of being covered was 1.51 mm when the normal bipedicled flap was employed and was 1.67 mm when the augmented flap was applied. However, the difference was not statistically significant. We found that defect size correlated with vocal fold length, width and flap size in the normal flap group, whereas it correlated only with vocal fold length in the augmented flap group. The bipedicled flap is capable of covering larger defects in males. CONCLUSION: Enlargement of a bipedicled vocal fold mucosal flap with laryngeal ventricular mucosa does not necessarily translate to an increase in the size of defect that can be covered. On average, the flap should be 30% larger than the width of the defect. The statistical model for predicting the defect size based on the vocal fold length, vocal fold width, and flap size has excellent predictive quality when a normal flap is employed.

RESUMO Objetivo Verificar se a estimulação das habilidades auditivas de fonoaudiólogos com e sem dificuldades no processamento auditivo central (PAC) interfere no desempenho da avaliação perceptivo-auditiva (APA) do desvio vocal predominante. Métodos Estudo prospectivo, quase-experimental, realizado com 26 fonoaudiólogos, desenvolvido em cinco etapas: Etapa1: Pré-intervenção - primeira APA, composta por 30 amostras da vogal sustentada /ε/, acrescidas de 20% de repetição (seis amostras repetidas), apresentadas randomicamente, na qual os participantes deveriam identificar o desvio vocal predominante; Etapa 2: Triagem das habilidades do PAC com nove testes da plataforma AudBility; Etapa 3: Treinamento perceptivo-auditivo do desvio vocal predominante e segunda APA, idêntica à anterior; Etapa 4: Intervenção - estimulação das habilidades auditivas (EHA), realizada por 14 fonoaudiólogos, sendo oito que não passaram na triagem da Etapa 2 (Grupo1) e seis que passaram na triagem e, espontaneamente, realizaram a EHA (Grupo G2a). Os outros 12 participantes não realizaram EHA e formaram o Grupo G2b; Etapa 5: Pós-intervenção - realização da terceira APA, idêntica às anteriores, por todos os fonoaudiólogos. A acurácia das respostas e confiabilidade intra-avaliadores foram verificadas ao longo das três APAs. Resultados Os três grupos apresentaram desempenhos semelhantes nas três APAs. G1 e G2a não apresentaram melhora no desempenho da análise do desvio vocal predominante pós-EHA. A confiabilidade intra-avaliador pré-intervenção no G1 foi inferior a do G2a e G2b nas APA1 e APA2, e similar a eles na APA3, pós-intervenção. Conclusão EHA proposto não impactou a acurácia da APA, mas influenciou a confiabilidade intra-avaliador dos fonoaudiólogos com dificuldades nas habilidades auditivas do PAC.


ABSTRACT Purpose To verify if the stimulation of auditory skills of speech therapists with and without difficulty in auditory processing (CAP) interferes in performance of auditory-perceptual assessment (APA) regarding predominant vocal deviation. Methods Prospective, quasi-experimental study, conducted with 26 speech therapists and developed in five stages: Stage1: Pre-intervention - APA, composed of 30 samples of sustained vowel emissions /ε/, plus 20% repetition (six repeated samples) in which participants should identify the predominant vocal deviation. Stage2: Screening of CAP skills through nine tests of AudBility web platform. Stage3: Perceptual-auditory training of predominant vocal deviation and, later, second APA, identical to previous one. Stage4: Intervention - Stimulation of auditory skills (EHA) performed by 14 participants, eight of which failed the screening from Stage2 and six who passed th e screening and spontaneously performed EHA (Group G2a ). The other 12 participants did not perform EHA and formed the G2b Group. Stage5: Post-intervention - Third execution of APA, identical to previous, by all participants. Accuracy of analysis and intra-rater reliability were assessed in all APAs. Results The three groups showed similar performances in the three APAs. G1 and G2a showed no improvement in the performance of the analysis of predominant vocal deviation after EHA. Pre-intervention intra-rater reliability in G1 was lower than in G2a and G2b in APA1 and APA2, and similar to them in APA3, post-intervention. Conclusion Proposed EHA had no impact on APA accuracy, but influenced in intra-rater reliability of speech therapists with difficulty in the hearing skills of CAP.

ABSTRACT

RESUMO O presente texto é a continuação da publicação referente ao XVIII Congresso da SBFa. Na parte "A" apresentamos análises sobre avaliação clínica vocal. O foco da parte "B" são aspectos de reabilitação vocal: 4. Técnicas tradicionais de terapia vocal; 5. Técnicas modernas de eletroestimulação e fotobiomodulação aplicadas à reabilitação. Os inúmeros estudos sobre os diversos programas, métodos ou técnicas tradicionais de reabilitação, muitos de elevada qualidade de evidência, permitem considerar tais procedimentos relativamente bem descritos, seguros e com efeitos conhecidos, dando conta do tratamento de diversos distúrbios vocais. As evidências científicas com as técnicas tradicionais são reconhecidas mundialmente. Novas frentes de evolução, como o uso da eletroestimulação ou fotobiomodulação em voz parecem ser promissoras como abordagens coadjuvantes. Há mais estudos sobre eletroestimulação em voz do que com fotobiomodulação, contudo, evidências científicas para essas duas técnicas modernas são ainda limitadas. Conhecimento e cautela são necessários para a aplicação de quaisquer técnicas.


ABSTRACT This text is the continuation of the XVIII SBFa Congress publication. In part "A" we presented the analyses on clinical vocal evaluation. Part "B" focuses on vocal rehabilitation: 4. Traditional techniques of vocal therapy; 5. Modern techniques of electrostimulation and photobiomodulation applied to vocal rehabilitation. The numerous studies on the various programs, methods, and techniques of traditional rehabilitation techniques, and many with high quality of evidence, allow us to consider such procedures relatively well described, safe, and with known effects, accounting for the treatment of various vocal disorders. The scientific evidence with traditional techniques is recognized worldwide. New fronts of evolution, with electrostimulation or photobiomodulation used to handle voice problems, seem to be promising as coadjutant approaches. There are more studies on electrostimulation in vocal rehabilitation than with photobiomodulation; however, scientific evidence for these two modern techniques is still limited. Knowledge and caution are required for the application of either technique.

RESUMO No XXVIII Congresso Brasileiro da SBFa, 24 especialistas reuniram-se e, a partir de um posicionamento condutor sobre pesquisa científica como ferramenta de conexão entre laboratório e clínica, cinco frentes de conhecimento da especialidade de voz foram discutidas: 1. Julgamento perceptivo-auditivo da qualidade vocal; 2. Análise acústica do sinal vocal; 3. Autoavaliação em voz; 4. Técnicas tradicionais de terapia; 5. Técnicas modernas de eletroestimulação e fotobiomodulação em voz. A parte "a" desta publicação é a consolidação das análises dos três primeiros aspectos. A tendência no julgamento perceptivo-auditivo da qualidade vocal é o uso de protocolos padrão. A avaliação acústica do sinal vocal é acessível e pode ser feita de modo descritivo ou por extração de parâmetros, preferindo-se medidas multiparamétricas. Finalmente, a análise do próprio indivíduo fecha essa tríade de documentação fonoaudiológica, que será base para a conclusão da avaliação, referência para monitoramento do progresso e avaliação de resultado de tratamento.


ABSTRACT During the XXVIII Brazilian Congress of SBFa, 24 specialists met and, from a leading position on scientific research as a tool for connecting laboratory and clinic, five fronts of knowledge of the voice specialty were discussed as following: Perceptual-auditory judgment of vocal quality; 2. Acoustic analysis of the vocal signal; 3. Voice self-assessment; 4. Traditional techniques of therapy; 5. Modern techniques of electrostimulation and photobiomodulation (PBMT) in voice. Part "a" of this publication was associated with the consolidation of the analyses of the first three aspects. The trend in the perceptual-auditory judgment of vocal quality was related to the use of standard protocols. The acoustic evaluation of the vocal signal is accessible and can be done descriptively or by extraction of parameters, thus preferring multiparametric measures. Finally, the analysis of the individual himself closes this triad of voice documentation, which will be the basis for the conclusion of the evaluation, reference for monitoring progress, and evaluation of treatment results.

Resumo Objetivo Verificar o efeito da repetição de estímulos âncoras no julgamento perceptivo auditivo do desvio vocal predominante realizado por estudantes de fonoaudiologia; analisar a relação entre o número de dimensões vocais dos estímulos sonoros e a acurácia; e investigar a relação entre o desvio vocal predominante e a acurácia. Método Participaram da pesquisa 54 estudantes de fonoaudiologia, divididos em: Grupo Com Repetição - GCR com 28 alunos; e Grupo Sem Repetição - GSR, 26 alunos. A amostra vocal consistiu de 220 vozes humanas disfônicas, vogal /ε/. O GCR escutou três estímulos âncoras, uma voz rugosa, uma soprosa e uma tensa, no início da tarefa e a cada 20 vozes. O GSR escutou os estímulos âncoras somente no início da tarefa de avaliação. Estas classificações foram comparadas com respostas referências produzidas por 3 fonoaudiólogas experientes. Analisamos a acurácia e a confiabilidade intra e interavaliadores, a correlação entre número de dimensões vocais e acurácia e a relação entre desvio vocal predominante e acurácia. Resultados Os grupos tiveram desempenhos semelhantes na acurácia do desvio vocal predominante. A confiabilidade intra e interavaliadores foi discretamente maior no GCR. A acurácia dos alunos foi inversamente proporcional ao número de dimensões vocais presentes nos estímulos. O desvio vocal de maior acurácia foi a soprosidade, e a de menor, a tensão. Conclusão A repetição dos estímulos âncoras melhorou a confiabilidade dos estudantes, mas não se mostrou efetiva na acurácia da classificação do desvio vocal predominante. O número de dimensões vocais nos estímulos sonoros interferiu na acurácia dos alunos.


ABSTRACT Purpose Verify the effect of anchor repetition in the perceptual auditory judgement of the type of vocal deviation performed by speech-language pathology (SLP) students; analyze the correlation between the amount of different vocal dimensions in the same stimuli and accuracy; investigate the correlation between type of vocal deviation and accuracy. Methods 54 SLP students were divided in two groups: Group with repetition (GwR), 28 students; and, Group with no repetition (GnR), 26 students. The analyzed sample counted with 220 dysphonic human voices, vowel /ε/. The GwR heard three anchor stimuli before the judgement and every 20 voices during the assessment. The GnR heard the anchor only before beginning the judgement. The anchor stimuli counted with one rough, one breathy and one strain voice. These classifications were compared with reference judgements from three expert SLPs. The intra and inter-rater reliability, the correlation between the amount of different vocal dimensions in the same stimuli and type of vocal deviation with accuracy were assessed. Results The accuracy between type of deviation was similar among groups. The GwR presented slightly higher intra and inter-rater reliability. The student's accuracy was inversely proportional to the amount of different vocal dimensions in the stimuli. Breathiness presented the highest accuracy and strain presented the lowest accuracy. Conclusion The repetition of anchor stimuli improved intra and inter-rater reliability. However, it was not effective in the accuracy of the type of vocal deviation. The amount of different vocal dimensions in the stimuli have influence in the students' accuracy.

RESUMO Objetivo investigar a ocorrência de queixa vocal autorreferida no uso profissional da voz em atores profissionais de teatro; correlacionar presença de queixa vocal com três protocolos de autoavaliação do impacto de uma alteração vocal: Escala de Sintomas Vocais (ESV), Escala de Desconforto do Trato Vocal (EDTV) e Índice de Desvantagem Vocal -10 (IDV-10); verificar quais protocolos são mais robustos à detecção do impacto de possível disfonia nessa população. Métodos Participaram 75 atores profissionais de teatro, ambos os gêneros, 18 a 64 anos. Os atores informaram a presença ou ausência de queixa vocal e responderam a três protocolos de autoavaliação: Escala de Sintomas Vocais (ESV), Escala de Desconforto do Trato Vocal (EDTV) e Índice de Desvantagem Vocal-10 (IDV-10). Resultados constatou-se que 25% dos atores apresentaram queixa vocal. O grupo com queixa (GCQ) apresentou maior número de sintomas vocais e maior frequência e intensidade de desconforto de trato vocal, quando comparado ao grupo sem queixa (GSQ). IDV-10 não identificou desvantagem vocal nos grupos. No GCQ, houve correlação forte entre ESV e IDV-10 e correlação muito forte entre os escores de frequência e intensidade da EDTV. No GSQ, houve correlação moderada entre ESV e EDTV, ESV e IDV-10 e correlação muito forte entre os escores de frequência e intensidade da EDTV. Conclusão neste estudo, 25% dos atores profissionais de teatro apresentaram queixa vocal. A correlação entre a queixa vocal e os protocolos foi positiva e variou de moderada à forte. No GCQ, a ESV e a EDTV foram instrumentos mais robustos na detecção do impacto de uma possível disfonia.


ABSTRACT Purpose To investigate the occurrence of self-reported vocal complaints in the professional use of the voice in professional theater actors; analyze the correlation between the presence of vocal complaint and three self-assessment protocols of the vocal problem impact; The Voice Symptom Scale (VoiSS), the Vocal Tract Discomfort (VTD) scale and the Voice Handicap Index -10 (VHI -10); verify the most robust protocols to identify dysphonia in this population. Methods The participants were 75 professional theater actors, men and women, between 18 and 64 years old. They answered to a questioner informing presence or absence of vocal complaint and to three self-assessment protocols: VoiSS, VTD and VHI-10. Results 25% of the actors presented vocal complaints. The group with vocal complaint (GwVC) had more voice symptoms and higher frequency and intensity of vocal tract discomfort than the group with no vocal complaint (GnVC). No group presented voice handicap considering the VHI-10 and a very high correlation between the VTD frequency and intensity scores. The GnVC presented moderate correlation between the VoiSS and the VTD, moderate correlation between the VoiSS and the VHI-10 and, a very high correlation between the VTD frequency and intensity scores. Conclusion In this study, 25% of professional theater actors presented vocal complaints. The correlation between vocal complaints and protocols was positive and ranged from moderate to strong. The VoiSS and the VTD were more robust identify dysphonia in the GwVC.

INTRODUCTION: The output sound has important changes throughout life due to anatomical and physiological modifications in the larynx and vocal tract. Understanding the young adult to the elderly speech acoustic characteristics may assist in the synthesis of representative voices of men and women of different age groups. OBJECTIVE: To obtain the fundamental frequency (f0), formant frequencies (F1, F2, F3, F4), and bandwidth (B1, B2, B3, B4) values extracted from the sustained vowel /a/ of young, middle-aged, and elderly adults who are Brazilian Portuguese speakers; to present the application of these parameters in vowel synthesis. STUDY DESIGN: Prospective study. METHODS: The acoustic analysis of tokens of the 162 sustained vowel /a/ produced by vocally healthy adults, men, and women, between 18 and 80 years old, was performed. The adults were divided into three groups: young adults (18 to 44 years old); middle-aged adults (45 to 59 years old) and, elderly adults (60 to 80 years old). The f0, F1, F2, F3, F4, B1, B2, B3, B4 were extracted from the audio signals. Their average values were applied to a source-filter mathematical model to perform vowel synthesis in each age group both men and woman. RESULTS: Young women had higher f0 than middle-aged and elderly women. Elderly women had lower F1 than middle-aged women. Young women had higher F2 than elderly women. For the men's output sound, the source-filter acoustic measures were statistically equivalent among the age groups. Average values of the f0, F1, F2, F3, F4, B1, and B2 were higher in women. The sound waves distance in signals, the position of formant frequencies and the dimension of the bandwidths visible in spectra of the synthesized sounds represent the average values extracted from the volunteers' emissions for the sustained vowel /a/ in Brazilian Portuguese. CONCLUSION: Sustained vowel /a/ produced by women presented different values of f0,F1 and F2 between age groups, which was not observed for men. In addition to the f0 and the formant frequencies, the bandwidths were also different between women and men. The synthetic vowels available represent the acoustic changes found for each sex as a function of age.

ABSTRACT

OBJECTIVES: The objective of this study was to present a novel surgical technique involving the use of a "bipedicled vocal fold mucosal flap" to repair a mucosal defect and to evaluate the outcomes of patients in whom it was used. MATERIAL AND METHODS: This was a retrospective study of 6 clinical cases. All patients underwent surgery between November 2000 and July 2018, and all procedures were performed by the same surgeon. For the auditory-perceptual assessment, the Grade-Roughness-Breathiness-Asthenia-Strain hoarseness scale was used. We based the stroboscopic evaluation on the European Laryngological Society protocol, analyzing the parameters glottal closure, mucosal wave, and phase symmetry. RESULTS: Ages at the time of surgery ranged from 10 to 52 years, and all of the patients were male. Preexisting vocal fold lesions included polyps, cysts, a sulcus, and mucosal bridges. Among the stroboscopic parameters, only the mucosal wave differed significantly between the preoperative and postoperative periods (P = 0.046). There were also significant postoperative improvements in the overall grade of dysphonia (P = 0.025) and in the degree of breathiness (P = 0.025). CONCLUSIONS: The use of a bipedicled vocal fold mucosal flap appears to promote significant improvements in the mucosal wave and in voice quality. In the patients evaluated here, the technique was used without preoperative planning. However, it proved to be a safe and appropriate means of repairing mucosal defects in the vocal folds, with the potential to preserve rheological properties and promote healing with less chance of fibrosis.

PURPOSE: To gather information about vocal health and hygiene, voice symptoms, and voice handicap from drama students with and without vocal complaints. METHODS: A total of 57 drama students participated of this study. They were divided into two groups: no vocal complaints group (NCG), with 39 students; and with vocal complaints group (WCG), with 18 students. The participants answered to three self-assessment protocols: 1. Vocal Health and Hygiene Questionnaire (VHHQ), 2. Voice Symptom Scale (VoiSS); and 3. Voice Handicap Index (VHI-10). RESULTS: No difference was observed between groups related to the knowledge of vocal health and hygiene. The drama students from the WCG presented higher scores, more voice symptoms in all subscales of the VoiSS, and more voice handicap, e.g,higher VHI-10 scores than the NCG. The more knowledge on vocal health and hygiene (VHHQ total score) was followed by less voice symptoms, which represents lower scores in the VoiSS total score and, in emotional domain, and it was only perceived voice handicap (VHI-10 total score) in the NCG. CONCLUSION: There was no difference in knowledge of vocal health and hygiene among drama students with and without vocal complaints. However, drama students with vocal complaints presented more voice symptoms and reported greater perception of voice handicap, which was negatively correlated with knowledge of vocal health and hygiene.

OBJECTIVE: This study aims to (1) determine the cutoff values of the overall severity (OS) of vocal deviation on the visual analog scale (VAS) based on the ratings of the numerical scale (NS); and (2) verify the power of discrimination of these cutoff values according to different degrees of vocal deviation. STUDY DESIGN: This is a prospective study. METHODS: The auditory-perceptual evaluation was performed by four speech-language pathologists who used two protocols with different scalar properties: the VAS and the 4-point NS. Vocal samples from142 women and 69 men, plus 10% of repetition, with and without vocal complaints, ranging from 19 to 60 years were included. The analyzed speaking task was the counting from 1 to 10. For both protocols, the judges rated the OS. RESULTS: Based on the correspondence between the two scales, the cutoff values of the OS on the VAS obtained were 35.5, 50.5, and 90.5 points. The 35.5 value corresponds to the cutoff point between normal variability and mild/moderate vocal deviations; the 50.5 value corresponds to the cutoff point between mild/moderate and moderate vocal deviations; the 90.5 value corresponds to the cutoff point between moderate and severe deviations. Areas under the receiver operating characteristic curve for the three cutoff values were 0.918, 0.948, and 0.970, respectively. CONCLUSIONS: The cutoff values of the OS on the VAS were obtained, and the areas under the ROC curve showed that all the three cutoff points had excellent accuracy that represents a higher power of discrimination of different degrees of vocal deviations.

ABSTRACT

OBJECTIVE: To compare vocal tract (VT) adjustments of dysphonic and non-dysphonic women before and after flexible resonance tube in water exercise (FRTWE) at rest and during phonation using magnetic resonance imaging. STUDY DESIGN: Prospective study. METHODS: Twenty women, aged 20-40 years, 10 dysphonic with vocal nodules (VNG) and 10 controls (CG), underwent four sets of sagittal VT MRI: two pre-FRTWE, at rest and during phonation, and two post-FRTWE, during phonation and at rest. The subjects performed 3 minutes of exercise. Nine parameters at rest and 21 during phonation were performed. RESULTS: Pre-FRTWE, eight significant differences were found, three at rest and five during phonation: at rest - laryngeal vestibule area, distance from epiglottis to pharyngeal posterior wall (PPW) and interarytenoid complex length were smaller in the VNG; during phonation - laryngeal vestibule area, angle between PPW and vocal fold (VF), epiglottis to PPW, and anterior commissure of the larynx to laryngeal posterior wall were smaller in the VNG; tongue area was larger in the VNG. Post-FRTWE, only three significant differences were found, two during phonation and one at rest: during phonation - angle between PPW and VF and the membranous portion of the VF length were smaller in the VNG; at rest - distance from epiglottis to PPW was smaller in the VNG. CONCLUSIONS: Results suggest that the habitual VT adjustments of dysphonic and non-dysphonic women are different at rest and during phonation. The FRTWE promoted positive VT changes in the VNG, reducing the intergroup differences.
