ABSTRACT

Corneal epithelial abnormality is a common manifestation of diabetic keratopathy and leads to delayed epithelial wound healing. The Wnt/ß-catenin signaling pathway participates in the development, differentiation and stratification of corneal epithelial cells. The present study compared the expression of Wnt/ß-catenin signaling pathway related factors, including Wnt7a, ß-catenin, cyclin D1 and phosphorylated (p-) glycogen synthase kinase 3 ß (Gsk3b) between normal and diabetic mouse corneas, by reverse transcription-quantitative PCR, western blotting and immunofluorescence staining. It was found that the expression of the Wnt/ß-catenin signaling pathway related factors was downregulated in diabetic corneas. Upon corneal epithelium scraping, the wound healing rate was significantly increased in diabetic mice after topical treatment with lithium chloride. After further investigation, significantly upregulated levels of Wnt7a, ß-catenin, cyclin D1 and p-Gsk3b were found in the diabetic group 24 h after treatment, accompanied by ß-catenin nuclear translocation observed by immunofluorescence staining. These results suggest that active Wnt/ß-catenin pathway can promote diabetic corneal epithelial wound healing.

ABSTRACT

The detection of disease-related biomarkers, including microRNA (miRNA), is of crucial importance in reducing the morbidity and mortality of cancer. Thus, there is a great desire to develop an efficient and simple sensing method to fulfill the detection of miRNAs. In this study, a novel amplification assay strategy is demonstrated for the highly sensitive detection of miRNA-21 by combining a structure-switchable molecular beacon with nicking-enhanced rolling circle amplification (SMB-NRCA). A circular padlock probe (CPP) contains a target recognition sequence, two binding sites for nicking endonuclease and three hybridization sites for SMBs. miRNA-21 can hybridize with the CPP and act as polymerization primer that initiates the rolling circle amplification (RCA) reaction and two different nicking-mediated RCA processes, releasing a large amount of SMBs and leading to a significantly amplified fluorescence signal originating from the restoration of pre-quenched fluorescence via their structural switching. Via the signal amplification based on the combination of RCA, nicking and SDA, this assay system can quantitatively detect miRNA-21 in a linear change of three orders of magnitude with a detection limit of 1 pM. The assay specificity is very high so that there is no interference from coexisting miRNAs. Moreover, the sensing system possesses ideal anti-interference ability in complicated milieux such as human serum. The novel sensing strategy shows tremendous prospects for application in tumor diagnosis and clinical therapy guidance.

ABSTRACT

Dry eye disease (DED) features the inflammatory response of the ocular surface. Pro-inflammatory T helper 17 (Th17) cells are important for the pathogenesis of DED. In the present study a mouse DED model was used to discover two Th17 subsets in draining lymph nodes and conjunctivae based on the expression of IL-17 receptor E (IL-17RE) and CCR10: IL-17RElowCCR10- Th17 and IL-17REhighCCR10+ Th17. IL-17REhighCCR10+ Th17 expressed more retinoic acid-related orphan receptor gamma t but fewer T-box-expressed-in-T-cells than IL-17RElowCCR10- Th17. In addition, the former expressed higher IL-17A, IL-21, and IL-22 but fewer IFN-γ than the latter. Further analysis showed that IL-17REhighCCR10+ Th17 did not express IFN-γ in vivo, whereas IL-17RElowCCR10- Th17 contained IFN-γ-expressing Th17/Th1 cells. Moreover, IL-17REhighCCR10+ Th17 possessed more phosphorylated p38 mitogen-activated protein kinase (MAPK) and Jnk than IL-17RElowCCR10- Th17, suggesting higher activation of MAPK signaling in IL-17REhighCCR10+ Th17. In vitro treatment with IL-17C effectively maintained IL-17A expression in Th17 cells through p38 MAPK rather than Jnk MAPK. Furthermore, the adoptive transfer of the two Th17 subpopulations indicated their equivalent pathogenicity in DED. Interestingly, IL-17REhighCCR10+ Th17 cells were able to phenotypically polarize to IL-17RElowCCR10- Th17 cells in vivo. In conclusion, the current study revealed novel Th17 subsets with differential phenotypes, functions, and signaling status in DED, thus deepening the understanding of Th17 pathogenicity, and exhibited Th17 heterogeneity in DED.

ABSTRACT

The first-line treatment for colorectal cancer (CRC) is 5-fluorouracil (5-FU). However, the efficacy of this treatment is sometimes limited owing to chemoresistance as well as treatment-associated intestinal mucositis and other adverse events. Growing evidence suggests that certain phytochemicals have therapeutic and cancer-preventing properties. Further, the synergistic interactions between many such plant-derived products and chemotherapeutic drugs have been linked to improved therapeutic efficacy. Polysaccharides extracted from Albuca bracteata (Thunb.) J.C.Manning and Goldblatt (ABP) have been reported to exhibit anti-oxidant, anti-inflammatory, and anti-tumor properties. In this study, murine CRC cells (CT26) and a murine model of CRC were used to examine the anti-tumor properties of ABP and explore the mechanism underlying the synergistic interactions between ABP and 5-FU. Our results revealed that ABP could inhibit tumor cell proliferation, invasion, and migratory activity in vitro and inhibited tumor progression in vivo by suppressing ß-catenin signaling. Additionally, treatment with a combination of ABP and 5-FU resulted in better outcomes than treatment with either agent alone. Moreover, this combination therapy resulted in the specific enrichment of Ruminococcus, Anaerostipes, and Oscillospira in the intestinal microbiota and increased fecal short-chain fatty acid (SCFA) levels (acetic acid, propionic acid, and butyric acid). The improvement in the intestinal microbiota and the increase in beneficial SCFAs contributed to enhanced therapeutic outcomes and reduced the adverse effects of 5-FU. Together, these data suggest that ABP exhibits anti-neoplastic activity and can effectively enhance the efficacy of 5-FU in CRC treatment. Therefore, further research on the application of ABP in the development of novel anti-tumor drugs and adjuvant compounds is warranted and could improve the outcomes of CRC patients.

ABSTRACT

The trans-activation response DNA-binding protein of 43 kDa (TDP-43) is a nuclear protein that has been shown to be involved in the growth and metastasis of breast cancer, neuroblastoma, and melanoma. However, the effect of TDP-43 on hepatocellular carcinoma (HCC) metastasis remains unclear. Here, we demonstrated that TDP-43 was highly upregulated in both clinical samples and cell lines of HCC. Moreover, knockdown and overexpression of TDP-43 efficiently affected the proliferation and metastasis of HCC cells as well as the expression of some proteins associated with epithelial-mesenchymal transition (EMT) and Wnt/ß-catenin signaling pathway. Furthermore, activation of the Wnt/ß-catenin pathway by LiCl restored the effect of TDP-43 knockdown on EMT and HCC cells, whereas inhibition of the Wnt/ß-catenin pathway by XAV939 negated the effect of TDP-43 overexpression. Importantly, we found that TDP-43 protein could interact with GSK3ß mRNA and regulate the level of GSK3ß protein translation. Taken together, our findings suggest that TDP-43 may activate the Wnt/ß-catenin pathway by targeting the inhibition of GSK3ß protein translation, thus inducing the proliferation and metastasis of HCC cells, which supports its potential value as a therapeutic target for the treatment of metastatic HCC.

ABSTRACT

Wide-angle X-ray diffraction on large-scale laser facility is a well-established experimental method, which is used to study the shock response of single crystal materials by recording X-rays diffracted from numerous lattice planes. We present a three-dimensional graphical method for extracting physical understanding from the raw diffraction data in shocked experiments. This method advances beyond the previous iterative process by turning abstract diffraction theories in shock physics into mathematic issues, providing three-dimensional visualization and quick extraction of data characteristics. The capability and versatility of the method are exhibited by identifying lattice planes for single crystal samples with different orientations and quantitatively measuring the lattice compression and rotation under dynamic loading.

BACKGROUND CONTEXT: Surgical treatment for unstable atlas fractures has evolved in recent decades from C1-C2 or C0-C2 fusion to motion-preservation techniques of open reduction and internal fixation (ORIF). However, regardless of a transoral or a posterior approach, the reduction is still not satisfactory. PURPOSE: The article describes and evaluates a new technique for treating unstable atlas fractures by using a monoaxial screw-rod system. STUDY DESIGN: This is a retrospective study. PATIENT SAMPLE: The sample includes adult patients with unstable C1 fractures treated with a posterior monoaxial screw-rod system. OUTCOME MEASURES: The outcome measures included a visual analog pain scale, radiographic reduction (lateral mass displacement [LMD]), maintenance of reduction, C1-C2 instability (anterior atlantodens interval), and complications. MATERIALS AND METHODS: From August 2013 to May 2016, nine consecutive patients with unstable atlas fractures were retrospectively reviewed. All patients were treated with posterior ORIF by using a monoaxial screw-rod system. The medical records and the preoperative and postoperative radiographs were reviewed. Preoperative and postoperative computed tomography scans were used to specify the fracture types and to assess the reduction. RESULTS: All nine patients with a mean age of 50.3 years successfully underwent surgery with this technique, and a follow-up of 17.4±9.3 months was performed. Transverse atlantal ligament (TAL) injury was found in eight of the nine patients: one of type I and seven of type II. The preoperative LMD averaged 7.0±2.2 mm and was restored completely after surgery; all the fractures achieved bony healing without loss of reduction or implant failure. None of the patients had complications of neurologic deficit, vertebral artery injury, or wound infection associated with the surgical procedure. Two patients complained of greater occipital nerve neuralgia after the operation, which gradually disappeared in 1 month. All patients had a well-preserved range of motion of the upper cervical spine at the final follow-up. CONCLUSIONS: Posterior osteosynthesis with a monoaxial screw-rod system is capable of an almost anatomical reduction for the unstable atlas fractures. The TAL incompetence may not be a contraindication to ORIF for C1 fractures, but the long-term effect of C1-C2 instability remains to be further investigated.

ABSTRACT

The x-ray backlighting systems, including a 1.865 keV (Si Heα line) spherically bent crystal imaging system and an ∼8.3 keV (Cu Heα line) point-projection imaging system, newly fielded on the Primary Test Stand facility are introduced and its preliminary experimental results in radiography of the aluminium (Al) liners with seeded sinusoidal perturbations are presented. The x-ray backlighter source is created using a 1 TW, 1 kJ Nd: glass high power laser, kilo-joule laser system, recently constructed at China Academy of Engineering Physics. The ablation melt and instability of the imploding Al liner outer edge under the driving current of ∼7.5 MA are successfully observed using these two backlighting systems, respectively.

OBJECTIVE: To observe the potency ratio of hyperbaric to isobaric solutions of ropivacaine in subarachnoid block for knee arthroscopy. METHODS: Fifty patients receiving knee arthroscopy under combined spinal-epidural anesthesia were randomly divided into isobaric ropivacaine group and hyperbaric ropivacaine group (0.5% ropivacaine, prepared with equal volume of 10% glucose and 1% isobaric ropivacaine). Successful criteria of spinal anesthesia were (1) a bilateral loss of pinprick sensation at or above the level of T12; (2) adequate motor block during knee arthroscopy (modified Bromage's score ≥2); and (3) no requirement of additional epidural administration at least within 60 min after intrathecal injection. Drug consumption was determined with up-and-down method, and then ED50 was calculated. RESULTS: The ED50 of isobaric ropivacaine was 9.71 mg (95% CI 8.11-11.32), and the ED50 of hyperbaric ropivacaine was 6.55 mg (95% CI 6.07-7.04), and the relative potency ratio was 0.67 (95% CI 0.56-0.80) for hyperbaric/isobaric ropivacaine. CONCLUSIONS: The ED50 of hyperbaric ropivacaine is less than that of isobaric ropivacaine in subarachnoid block anesthesia for knee arthroscopy.

ABSTRACT

PURPOSE: The axis body fractures are relatively uncommon and have a variety of presentations. Surgical management to them has been only reported as case reports or included as a minor part of clinical management. The objective of this study is to summarize the indications for surgery and report the clinical outcome of surgical treatment based on different fracture patterns. METHODS: A retrospective analysis of 28 consecutive patients presenting with the axis body fractures was undertaken. The indications for surgical treatment were defined as: (1) fractures associated with instability of adjacent joints; (2) irreducible displaced superior articular facet fracture; (3) fractures resulting in spinal cord compression. The fractures were classified as sagittal, coronal, transverse and lateral mass fracture. One of the following surgical procedures was applied according to the fracture pattern: posterior C1-C2 pedicle screws fixation and fusion (I); posterior C1-C3 screws fixation and fusion (II); posterior osteosynthesis with C2 transpedicular half-thread lag screws (III). RESULTS: 13 patients were successfully managed operatively. Two transverse and two unilateral lateral mass fractures were treated with surgical procedure I, five sagittal fractures with II, four coronal fractures with III. Complications of malposition of screws and neurologic deficit did not occur during operation. Satisfactory reduction and bony union were demonstrated on postoperative radiographics. CONCLUSIONS: Conservative treatment is still advocated as primary management for most axis body fractures. But for patients with obvious adjacent joints instability or irreducible displaced superior articular facet fracture, surgical intervention based on the different fracture pattern is necessary.

ABSTRACT

A new logarithmically bent Laue imaging crystal analyzer (LBLICA) was proposed to obtain the monochromatic image of plasmas and exhibited a great potential for application in the Inertial Confinement Fusion experiment over a large field of view (FOV) and with a high spatial resolution. The imaging geometry of the LBLICA has been discussed. According to the Bragg condition and the equation of the logarithmic spiral, the key image parameters of the crystal analyzer, including the system magnification, the spatial resolution, and the FOV, have been analyzed theoretically. An experiment has been performed with a Cu target X-ray tube as a backlighter to backlight a mesh grid consisting of 50-µm Cu wires, and the monochromatic image of the grid has been obtained with a spatial resolution of approximately 30 µm.

ABSTRACT

A fiber interferometry for transverse velocity measurement has been developed. This diagnostic is similar to photonic Doppler velocimetry in the way in which laser propagates and couples. The interferometer mainly consists of a fiber coupler, an emitting probe, and two receiving probes. A pair of scattered laser beams mix in the coupler and generates fringes with frequency proportional to transverse velocity. Measurement of transverse velocity is independent of longitudinal velocity. The feasibility of the technique has been verified by rotating wheel experiment and shock loading experiment.

ABSTRACT

OBJECTIVE: To employ trans-esophageal echocardiography (TEE) to observe the tourniquet's influence on cardiac function in total knee arthroplasty (TKA). METHODS: Twenty ASA I-II patients undergoing TKA under general anesthesia from September 2011 to February 2012 at Department of Orthopedics, Beijing Jishuitan Hospital were selected. Tourniquet was loosened at 30 minutes after the start of TKA. And TEE was employed to observe the occurrence rate and degree of intra-cardiac embolus and the changes of cardiac function at 5, 10 and 15 min post-loosening respectively. RESULTS: Different degrees of "meteor-shaped" microemboli images appeared in right atriums of all patients. The "meteor shape" image was the most obvious after loosening for 5 min. It started to decrease after 10 min, persisted generally and declined further after 15 min. The left ventricular areas at end diastole (LVAd) after loosening tourniquet for 5, 10 and 15 min were (7.62 ± 0.54), (7.86 ± 0.46) and (8.55 ± 0.56) cm(2) respectively. And there were remarkable changes compared with the area (9.80 ± 0.48) cm(2) pre-loosening (P < 0.01). The fractional area change (FAC) showed obvious decrease since loosening for 5 min (P < 0.01). CONCLUSION: Massive meteor-shaped microembolis appear on TEE with weakened cardiac function.

ABSTRACT

A dual-channel, focusing x-ray spectrograph with uniform dispersion (i.e., the linear dispersion of this spectrograph is a constant) is described for measuring the x-ray spectra emission from the hot, dense Al Z pinch plasmas. The spectrograph uses double uniform-dispersed crystals (e.g., a Quartz 1010 crystal and a Mica 002 crystal) as dispersion elements and a double-film box as detector to achieve the simultaneous recording of the time integrated spectrum covering a wide spectral range of ~5-9 Å. Since this spectrograph disperse the x-rays on the detector plane with uniform spacing for every wavelength, it needs not the calibration of the wavelength with spatial coordinate, thereby own the advantages of easiness and veracity for spectra identification. The design of this spectrograph and the example of experiment on the "Yang" accelerator are presented.

ABSTRACT

An in-chamber, mini x-ray imaging instrument employs a pinhole and a logarithmic spiral crystal has been developed for obtaining K-shell line images of the imploding aluminum wire array on the "Yang" accelerator. The logarithmic spiral crystal acts as a monochromator and a non-dispersive mirror that reflects the pinhole image to a x-ray film detector with a very narrow photon energy bandwidth (<1 eV, mainly determined by the width of rocking curve of the crystal). Two imaging configurations with the use of Quartz (10 ̅10) crystal and Mica (002) crystal are designed, respectively, to image the Al Ly(α2) line (1727.7 eV) emission and Al He(α) intercombination line (1588.3 eV) emission. The primary experimental data corresponding to these two configurations are presented and discussed.

ABSTRACT

OBJECTIVE: To explore the feasibility of C1-2 pedicle screw fixation and fusion technique in treating atlantoaxial instability. METHODS: From January 2006 to January 2009,18 patients with atlantoaxial instability were treated with C1-2 pedicle screws and plates fixation under general anesthesia. There were 11 males and 7 females, the age for 17-62 years with the mean of 37.7 years. The course of disease was from 3 days to 30 months with an average of 10.6 months. The patients had different degrees neck pain and disturbance of sensation or (and) dyskinesia, had atlantoaxial instability from images before operation. The JOA scoring before operation were from 8 to 15 with an average of 11.4. Bone fusion of patients was observed with X-rays, CT and MR image at the same time after operation and the JOA scoring was compared with preoperative. RESULTS: A total of 72 screws were successfully placed in 18 cases, among them, 15 cases were followed up from 6 to 24 months with an average of 11.5 months. Fifteen cases obtained bone fusion with time for 3-6 months without the complications of internal fixation failure or redislocation of atlas. The spinal compression had differently improved, postoperative JOA scoring was from 12 to 17 scores with an average of 14.5 scores. CONCLUSION: Pedicle screw fixation and fusion in atlas has advantages of firm fixation and high fusion rate, it is a better choices for atlantoaxial instability. It is not suitable for variations of sulcus of vertebral artery of atlas and posterior arch of atlas.

ABSTRACT

OBJECTIVE: To evaluate the effect of anterolateral approach in treating thoracolumbar disc protrusion. METHODS: From May 2004 to May 2008, 11 patients with thoracolumbar disc protrusion (T10-L3) underwent discectomy, autologous iliac bone graft, and internal fixation via anterolateral approach. There were 9 males and 2 females aged 26-57 years old (average 42.8 years old). The course of disease was 1 week-11 years (average 2.7 years). Nine patients showed the lower limbs weakness, 8 had sensory disturbance, 6 presented with lumbodorsal pain, 5 had the lower limb pain, 4 presented with sphincter muscle disturbance, 3 suffered from foot drop, and 5 had a history of lower back injury. X-ray, CT, and MRI test showed that 9 cases had the single-segmental protrusion, 2 cases had double-segmental protrusion, 2 cases were accompanied with ossification of the thoracic vertebra yellow ligament, 2 cases were combined with ossification of the vertebra posterior longitudinal ligament, and 1 was complicated with Scheuermann disease. Preoperatively, the intervertebral height was (7.2 +/- 1.3) mm and JOA score was 6.4 +/- 2.8. RESULTS: The time of operation was 2.5-5.5 hours (average 3.5 hours); the blood loss during operation was 500-1,200 mL (average 750 mL). During operation, intraoperative pleural tear occurred in 1 patient, peritoneal tear in 1 patient, and dural laceration in 1 patient. Repairing was performed intraoperative and preventive suction drainage was used for 3-5 days. No postoperative hydropneumothorax and cerebrospinal fluid leakage occurred. All incisions healed by first intention. No postoperative complications of nerve system occurred. All the patients were followed up for 1-4 years (average 2.4 years). X-ray films showed that all the patients achieved bony fusion without the occurrence breakage and loosening of titanium plate and screw 6-9 months after operation. Three cases displayed subtle scoliosis (10-15 degrees) without influence on spinal equilibration. The intervertebral height increased to (12.3 +/- 1.5) mm 2 weeks after operation, indicating there was a significant difference compared with preoperative value (P < 0.05). The intervertebral height 1 year after operation when the bony fusion was reached decreased to (7.5 +/- 1.2) mm, indicating there was no significant difference compared to the preoperative value (P > 0.05). The JOA score increased to 7.6 +/- 3.2 at 1 year after operation, indicating there was a significant difference compared with preoperative value (P < 0.05). The symptoms and signs of all the patients were improved to various degrees after operation. According to improvement rate evaluation system proposed by Hirabayashi et al., 4 cases were graded as excellent, 6 as good, 1 as fair, and the excellent and good rate was 90.9%. CONCLUSION: Treating thoracolumbar disc protrusion via anterolateral approach is safe and effective.

ABSTRACT

We describe the use of a Wigner distribution function approach for exploring the problem of extending the depth of field in a hybrid imaging system. The Wigner distribution function, in connection with the phase-space curve that formulates a joint phase-space description of an optical field, is employed as a tool to display and characterize the evolving behavior of the amplitude point spread function as a wave propagating along the optical axis. It provides a comprehensive exhibition of the characteristics for the hybrid imaging system in extending the depth of field from both wave optics and geometrical optics. We use it to analyze several well-known optical designs in extending the depth of field from a new viewpoint. The relationships between this approach and the earlier ambiguity function approach are also briefly investigated.

ABSTRACT

OBJECTIVE: To observe the effect of electroacupuncture (EA) on cytokines in the cardiac surgical patient and to evaluate the application of combined acupuncture anesthesia to cardiac surgery. METHODS: Thirty patients with atrial septal defect were divided into 3 groups, general anesthesia group (A), acupuncture anesthesia group (B) and combined general anesthesia and EA group (C). Peripheral blood samples were collected before anesthesia, before cardiopulmonary bypass (CPB), 30 min after CPB and 24 h after operation to determine the levels of interferon-gamma (IFN-gamma), interleukin-2 (IL-2), IL-6 and IL-10. RESULTS: The levels of IFN-gamma and IL-2 decreased in the 3 groups after CPB and further decreased 24 h after operation, and in the group C were higher than those in the group B. The levels of IL-6 and IL-10 significantly increased 24 h after operation in the 3 groups with no significant difference among the 3 groups. CONCLUSION: The general anesthesia combined with EA can not completely improve the decrease of IFN-gamma and IL-2 induced by CPB, indicating that the good response of the general anesthesia combined with EA to stress can partially improve the immunosupression induced by CPB. Acupuncture does not have significant effect on inflammatory cytokine reaction induced by cardiac surgery.

ABSTRACT

The ambiguity function was employed as a merit function to design an optical system with a high depth of focus. The ambiguity function with the desired enlarged-depth-of-focus characteristics was obtained by using a properly designed joint filter to modify the ambiguity function of the original pupil in the phase-space domain. From the viewpoint of the filter theory, we roughly propose that the constraints of the spatial filters that are used to enlarge the focal depth must be satisfied. These constraints coincide with those that appeared in the previous literature on this topic. Following our design procedure, several sets of apodizers were synthesized, and their performances in the defocused imagery were compared with each other and with other previous designs.
