ABSTRACT

OBJECTIVE: To provide local policymakers with a guideline of potential actions to prevent the high consumption of Non-Nutritive Sweeteners (NNS) among children and adolescents observed in Chile, given the potential health problems related to NNS intake. DESIGN: The Delphi method was used for the evaluation of twenty-one recommendations to decrease the intake of NNS in paediatric population, with the participation of a panel of relevant actors. SETTING: The proposed recommendations were developed by the research team using the NOURISHING framework; potential actions were based on the increase in the use and intake of NNS by Chilean children, current local food regulations, recommendations of health organisations and foreign policy experiences. PARTICIPANTS: Twenty-five relevant actors related to NNS, nutrition, food technology and paediatrics (out of thirty-nine invitations made to scholars, professional institutions and civil society's organisations) participated in the Delphi study. RESULTS: A consensus was reached on nine recommendations regarding relevance and feasibility to be part of the guideline. Recommendations involved measures mostly related to improving the delivery of information (food content and potential health effects of NNS), supporting the generation of more evidence of NNS health effects and substitutes, and marketing restrictions when targeted to children. CONCLUSIONS: The process produced a nine-action guideline to reduce the excessive NNS consumption among Chilean children and adolescents. Developed through a consensus-driven approach among key stakeholders, this guideline provides policymakers with a framework to adopt a precautionary stance, particularly concerning vulnerable populations, given the currently inconclusive evidence on the long-term health effects of NNS consumption.

ABSTRACT

Abstract Background Autoimmune encephalitis (AIE) is a group of inflammatory diseases characterized by the presence of antibodies against neuronal and glial antigens, leading to subacute psychiatric symptoms, memory complaints, and movement disorders. The patients are predominantly young, and delays in treatment are associated with worse prognosis. Objective With the support of the Brazilian Academy of Neurology (Academia Brasileira de Neurologia, ABN) and the Brazilian Society of Child Neurology (Sociedade Brasileira de Neurologia Infantil, SBNI), a consensus on the diagnosis and treatment of AIE in Brazil was developed using the Delphi method. Methods A total of 25 panelists, including adult and child neurologists, participated in the study. Results The panelists agreed that patients fulfilling criteria for possible AIE should be screened for antineuronal antibodies in the serum and cerebrospinal fluid (CSF) using the tissue-based assay (TBA) and cell-based assay (CBA) techniques. Children should also be screened for anti-myelin oligodendrocyte glucoprotein antibodies (anti-MOG). Treatment should be started within the first 4 weeks of symptoms. The first-line option is methylprednisolone plus intravenous immunoglobulin (IVIG) or plasmapheresis, the second-line includes rituximab and/or cyclophosphamide, while third-line treatment options are bortezomib and tocilizumab. Most seizures in AIE are symptomatic, and antiseizure medications may be weaned after the acute stage. In anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis, the panelists have agreed that oral immunosuppressant agents should not be used. Patients should be evaluated at the acute and postacute stages using functional and cognitive scales, such as the Mini-Mental State Examination (MMSE), the Montreal Cognitive Assessment (MoCA), the Modified Rankin Scale (mRS), and the Clinical Assessment Scale in Autoimmune Encephalitis (CASE). Conclusion The present study provides tangible evidence for the effective management of AIE patients within the Brazilian healthcare system.


Resumo Antecedentes Encefalites autoimunes (EAIs) são um grupo de doenças inflamatórias caracterizadas pela presença de anticorpos contra antígenos neuronais e gliais, que ocasionam sintomas psiquiátricos subagudos, queixas de memória e distúrbios anormais do movimento. A maioria dos pacientes é jovem, e o atraso no tratamento está associado a pior prognóstico. Objetivo Com o apoio da Academia Brasileira de Neurologia (ABN) e da Sociedade Brasileira de Neurologia Infantil (SBNI), desenvolvemos um consenso sobre o diagnóstico e o tratamento da EAIs no Brasil utilizando a metodologia Delphi. Métodos Um total de 25 especialistas, incluindo neurologistas e neurologistas infantis, foram convidados a participar. Resultados Os especialistas concordaram que os pacientes com critérios de possíveis EAIs devem ser submetidos ao rastreio de anticorpos antineuronais no soro e no líquido cefalorraquidiano (LCR) por meio das técnicas de ensaio baseado em tecidos (tissue-based assay, TBA, em inglês) e ensaio baseado em células (cell-based assay, CBA, em inglês). As crianças também devem ser submetidas ao rastreio de de anticorpo contra a glicoproteína da mielina de oligodendrócitos (anti-myelin oligodendrocyte glycoprotein, anti-MOG, em inglês). O tratamento deve ser iniciado dentro das primeiras 4 semanas dos sintomas, sendo as opções de primeira linha metilprednisolona combinada com imunoglobulina intravenosa (IGIV) ou plasmaférese. O tratamento de segunda linha inclui rituximabe e ciclofosfamida. Bortezomib e tocilizumab são opções de tratamento de terceira linha. A maioria das crises epilépticas nas EAIs são sintomáticas, e os fármacos anticrise podem ser desmamadas após a fase aguda. Em relação à encefalite antirreceptor de N-metil-D-aspartato (anti-N-methyl-D-aspartate receptor, anti-NMDAR, em inglês), os especialistas concordaram que agentes imunossupressores orais não devem ser usados. Os pacientes devem ser avaliados na fase aguda e pós-aguda mediante escalas funcionais e cognitivas, como Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Modified Rankin Scale (mRS), e Clinical Assessment Scale in Autoimmune Encephalitis (CASE). Conclusão Esta pesquisa oferece evidências tangíveis do manejo efetivo de pacientes com EAIs no sistema de saúde Brasileiro.

ABSTRACT Objective: This article aims to reflect on the Delphi technique, a systematic process that uses the collective opinion of a group of experts to reach consensus on specific issues. Focusing on the field of nursing, we examine in a reflective manner the different modalities of this technique and its stages. Method: This is a theoretical reflection on the use of the Delphi technique, listing its different forms and conducting a critical evaluation of the method throughout its different stages. Results: Different variants of the technique are observed in the available literature, namely Traditional Delphi, Modified Delphi, e-Delphi, and the RAND/UCLA method, accompanied by epistemological and methodological changes in the traditional understanding of the Delphi technique. Despite the variability of formats, the technique should include three principles: the panel of experts, the anonymity of the responses, and the interactive process of controlled feedback. Throughout the article, a reflection is made on its stages, justification of the study area, expert panel, characteristics of the rounds, and termination criteria. Conclusion: We suggest our evaluation criteria so that authors, researchers, and reviewers can analyze the use of the Delphi technique in nursing research. It is also recommended that clear guidelines be created for presenting studies using the Delphi technique in the health area, as there are other established guidelines for other types of methods.


RESUMO Objetivo: refletir sobre a técnica de Delphi, um processo sistemático que utiliza a opinião coletiva de um grupo de especialistas para alcançar consenso sobre determinados assuntos. Focalizando especificamente na área da enfermagem, nossa intenção é examinar, de forma reflexiva, as diferentes modalidades dessa técnica e suas etapas. Método: reflexão teórica sobre a utilização da técnica Delphi, elencando suas diferentes formas e realizando uma avaliação crítica do método ao longo das suas diferentes etapas. Resultados: observa-se na literatura disponível diferentes variantes da técnica, designadamente o Delphi Tradicional, o Delphi modificado, o e-Delphi e o método RAND/UCLA, acompanhados por mudanças epistemológicas e metodológicas no entendimento tradicional da técnica de Delphi. Apesar da variabilidade de formatos, a técnica deve incluir três princípios: o painel de peritos, o anonimato das respostas e o processo interativo de retroalimentação controlada. Ao longo do artigo, é realizada uma reflexão sobre as suas etapas, justificação da área em estudo, painel de peritos, características das rondas e critérios de término. Conclusão: Sugerimos nossos critérios de avaliação para que autores, pesquisadores e revisores possam analisar a utilização da técnica de Delphi na pesquisa em enfermagem. Recomendada-se ainda a criação de diretrizes claras para a apresentação de estudos com a utilização do Delphi na área da saúde, assim como existem outras orientações estabelecidas para outro tipo de métodos.

Resultados: en la literatura disponible se observan diferentes variantes de la técnica, a saber: el Delphi Tradicional, el Delphi modificado, el e-Delphi y el método RAND/UCLA, acompañados por mudanzas epistemológicas y metodológicas en el entendimiento tradicional de la técnica de Delphi. A lo largo del artículo, es realizada una reflexión sobre sus etapas, justificación del área en estudio, panel de peritos, características de las rondas y criterios de término. ABSTRACT

RESUMEN La formación de recursos humanos en salud es una preocupación recurrente. El Campus Virtual de Salud Pública (CVSP) es la plataforma educativa de la Organización Panamericana de Salud (OPS) que busca mejorar la accesibilidad a la educación en salud pública, con énfasis en la formación en liderazgo educativo orientado a planificar, implementar y evaluar políticas y acciones educativas. El objetivo de este trabajo es presentar los resultados de una consulta a personas expertas sobre el curso virtual "Liderazgo para la Gestión Educativa en Organizaciones de Salud", como parte de una estrategia de fortalecimiento de las capacidades de liderazgo para la gestión educativa en las Américas propuesta por el CVPS/OPS. Se realizó un estudio cualitativo de investigación-acción, con una metodología Delphi de dos iteraciones de consultas, una virtual y una presencial, a expertos en gestión educativa en organizaciones de salud. Los resultados muestran la relevancia que adquieren ocho dimensiones de análisis: el perfil de los destinatarios, las competencias, el enfoque y los contenidos, las actividades, la evaluación del y para el aprendizaje, la adaptabilidad, la implementación y el seguimiento durante y poscurso. La participación de actores de diferentes ámbitos geopolíticos en el diseño e implementación de una propuesta educativa regional promueve la transformación del contexto de aplicación y mejora su potencial adopción. Se destaca el lugar estratégico del curso como catalizador en la conformación y la consolidación de una red integrada de organizaciones para fortalecer el liderazgo educativo en las Américas, y la conveniencia de la estrategia metodológica empleada para mejorar la calidad de la formación de recursos humanos en salud.


ABSTRACT The training of human resources for health (HRH) is a recurring concern. The Virtual Campus for Public Health (VCPH) - the educational platform of the Pan American Health Organization (PAHO) - seeks to improve access to public health education, emphasizing leadership training for teams that plan, implement, and evaluate policies and educational initiatives. The objective of this work is to present the results of a consultation with experts on the virtual course "Leadership for Educational Management in Health Organizations", as part of a PAHO/VCPH strategy to strengthen leadership capacities for educational management in the Americas. A qualitative action-research study was carried out using the Delphi method with two iterations of consultations (one virtual and one in person) with experts in educational management in health organizations. The results show the importance of eight dimensions of analysis: recipient profiles, competencies, approach and contents, activities, evaluation of and for learning, adaptability, implementation, and monitoring during and after the course. The participation of actors from different geopolitical spheres in the design and implementation of a regional educational initiative fosters adaptations in the context of its implementation and improves the likelihood it will be adopted. This course can play a strategic role as a catalyst in the formation and consolidation of an integrated network of organizations that strengthen educational leadership in the Americas. This study also highlights the value of the methodological strategy used to improve the quality of HRH training.


RESUMO A formação de recursos humanos em saúde é uma preocupação recorrente. O Campus Virtual de Saúde Pública (CVSP) é a plataforma educacional da Organização Pan-Americana da Saúde (OPAS), que busca melhorar a acessibilidade à educação em saúde pública, com ênfase na formação de lideranças educacionais voltadas para o planejamento, a implementação e a avaliação de políticas e ações educacionais. O objetivo deste documento é apresentar os resultados de uma consulta com especialistas sobre o curso virtual "Liderazgo para la Gestión Educativa en Organizaciones de Salud" [Liderança para a Gestão Educacional em Organizações de Saúde], como parte de uma estratégia para fortalecer as capacidades de liderança para gestão educacional na Região das Américas proposta pelo CVPS/OPAS. Foi realizado um estudo qualitativo de pesquisa-ação utilizando o método Delphi com duas rodadas de consultas, uma virtual e outra presencial, a especialistas em gestão educacional em organizações de saúde. Os resultados mostram a relevância de oito dimensões de análise: perfil do público-alvo; competências; abordagem e conteúdo; atividades; avaliação de e para as aprendizagens; adaptabilidade; implementação; e acompanhamento durante e após o curso. A participação de atores de diferentes esferas geopolíticas no delineamento e na implementação de uma proposta educacional regional promove a transformação do contexto de aplicação e aumenta seu potencial de adoção. Destaca-se o posicionamento estratégico do curso como catalisador na criação e consolidação de uma rede integrada de organizações para fortalecer a liderança educacional na Região das Américas e a adequação da estratégia metodológica empregada para melhorar a qualidade da formação de recursos humanos em saúde.

Resumo Objetivo Desenvolver e realizar a validação de conteúdo de um instrumento de autoavaliação da qualidade do cuidado em Instituições de Longa Permanência para Idosos (ILPI), denominado QualificaILPI. Método Estudo metodológico realizado entre março e dezembro de 2021. O instrumento foi desenvolvido com base em modelo multidimensional de qualidade, legislação brasileira e pesquisa bibliográfica e contém padrões de qualidade para autoavaliação das ILPI nas dimensões: ambiente, lar, cuidado, envolvimento familiar e da comunidade, equipe de trabalho e gestão. Cada padrão é descrito e seguido por uma escala, com parâmetros para classificar o nível de qualidade da ILPI em incipiente, intermediário, consolidado. A Técnica Delphi modificada foi empregada para validação por um comitê de 10 especialistas quanto a pertinência do padrão para avaliação da qualidade da ILPI, da adequação dos objetivos e da escala de avaliação, clareza, podendo fazer comentários. O padrão foi mantido quando houve 75% de concordância entre os especialistas. O instrumento foi também avaliado pelo público-alvo, constituído por coordenadores de 10 ILPI, selecionadas por conveniência. Resultados No primeiro ciclo de avaliação, foram excluídos três padrões e dois novos foram criados. No segundo, alterou-se a dimensão de um padrão e dois padrões foram unidos. Ao final, permaneceram 29 padrões divididos em seis dimensões. O público-alvo, gestores de ILPI, sugeriu alterações na redação de alguns padrões. Houve consenso de 80% ou superior em todos os padrões. Conclusão O QualificaILPI poderá contribuir para o monitoramento das ILPI favorecendo a melhoria do cuidado ofertado aos residentes.


Abstract Objective To develop and validate the content of a self-assessment instrument for the quality of care in Long-Term Care Facilities for Older Adults (Instituições de Longa Permanência para Idosos - ILPIs), named QualificaILPI. Method A methodological study conducted between March and December 2021. The instrument was developed based on a multidimensional quality model, Brazilian legislation, and literature research. It contains quality standards for self-assessment of ILPIs in the dimensions of environment, home, care, family and community involvement, work team, and management. Each standard is described and followed by a scale with parameters to classify the level of ILPI quality as incipient, intermediate, or consolidated. The modified Delphi Technique was employed for validation by a committee of 10 experts regarding the relevance of the standard for ILPI quality assessment, the appropriateness of objectives, the evaluation scale, and clarity, allowing for comments. The standard was retained when there was 75% agreement among the experts. The instrument was also evaluated by the target audience, consisting of coordinators from 10 ILPIs selected for convenience. Results In the first assessment cycle, three standards were excluded, and two new ones were created. In the second cycle, the dimension of one standard was changed, and two standards were combined. In the end, 29 standards remained, divided into six dimensions. The target audience, ILPI managers, suggested changes in the wording of some standards. There was a consensus of 80% or higher for all standards. Conclusion QualificaILPI has the potential to contribute to monitoring ILPIs, promoting the improvement of care offered to residents.

ABSTRACT

Objective: To generate data on the costs associated with the diagnosis and treatment of obstructive ypertrophic cardiomyopathy (HCM) from the perspective of the private health system in Brazil. Methods: A modified Delphi panel including seven different specialists (three clinical cardiologists with experience in obstructive HCM, two hemodynamicists with experience in septal ablation and two cardiac surgeons with expertise in myectomy), from two Brazilian states (São Paulo and Pernambuco), was conducted between August and November 2022. Two rounds of questions about the use of healthcare resources according to the functional class (NYHA I-IV) and a panel in a virtual platform were conducted to obtain the final consensus. Micro-costing defined costs and unit values were determined based on official price lists. Results: The total diagnosis cost per patient was estimated at BRL 11,486.81. The obstructive HCM management costs analysis showed average annual costs per patient of BRL 17,026.74, BRL 19,401.46, BRL 73,310.07, and BRL 94,885.75 for the functional classes NYHA I, NYHA II, NYHA III, and NYHA IV, respectively. The average costs per patient related to procedures in a year were BRL 12,698.53, BRL 13,462.30, BRL 58,841.67, and BRL 75,595.90 for the functional classes NYHA I, II, III, and IV, respectively. Conclusions: The annual costs of HCM management increased according to the functional class, highlighting the need for safe and effective strategies to improve patient's NYHA functional class while promoting a decrease in the need for invasive therapies.


Objetivo: Gerar dados acerca dos custos associados ao diagnóstico e tratamento da cardiomiopatia hipertrófica (CMH) obstrutiva, sob a perspectiva do sistema de saúde privado no Brasil. Métodos: Um painel Delphi modificado incluindo sete especialistas (três cardiologistas clínicos com experiência em CMH obstrutiva, dois hemodinamicistas com experiência em ablação de septo e dois cirurgiões cardíacos com experiência em miectomia) de dois estados brasileiros (São Paulo e Pernambuco) foi conduzido entre agosto e novembro de 2022. Foram realizadas duas rodadas de perguntas acerca da utilização de recursos de acordo com a classe funcional (NYHA I-IV) e uma reunião virtual para obtenção do consenso final. Os custos foram definidos por meio de microcusteio, e os valores unitários foram definidos com base em listas de preço oficiais. Resultados: O custo total do diagnóstico por paciente foi estimado em R$ 11.486,81. A análise de custos de manejo da CMH obstrutiva mostrou custos médios anuais por paciente de R$ 17.026,74, R$ 19.401,46, R$ 73.310,07 e R$ 94.885,75 para as classes funcionais NYHA I, NYHA II, NYHA III e NYHA IV, respectivamente. Os custos médios por paciente relacionados a procedimentos em um ano foram de R$ 12.698,53, R$ 13.462,30, R$ 58.841,67 e R$ 75.595,90 para as classes NYHA I, II, III e IV, respectivamente. Conclusões: Os custos anuais com o manejo da CMH aumentam de acordo com a classe funcional, destacando a necessidade de estratégias seguras e eficazes capazes de melhorar a classe funcional NYHA do paciente, ao mesmo tempo que promove diminuição da necessidade de terapias invasivas.

ABSTRACT

BACKGROUND: Telehealth has emerged as an alternative model for treatment delivery and has become an important component of health service delivery. However, there is inconsistency in the use of terminologies and a lack of research priorities in telehealth in musculoskeletal pain. The purpose of this international, multidisciplinary expert panel assembled in a modified three-round e-Delphi survey is to achieve a consensus on research priorities and for the standard terminology for musculoskeletal pain telehealth practice. METHODS: In this international modified e-Delphi survey, we invited an expert panel consisting of researchers, clinicians, consumer representatives, industry partners, healthcare managers, and policymakers to participate in a three-round e-Delphi. Expert panels were identified through the Expertscape website, PubMed database, social media, and a snowball approach. In Round 1, potential research priorities and terminologies were presented to panel members. Panel members rated the agreement of each research priority on a 5-point Likert scale and an 11-point numerical scale, and each terminology on a 5-point Likert scale for the "telehealth in musculoskeletal pain " field over rounds. At least 80% of the panel members were required to agree to be deemed a consensus. We analyzed the data descriptively and assessed the stability of the results using the Wilcoxon matched-pairs signed rank test. RESULTS: We performed an international e-Delphi survey from February to August 2022. Of 694 invited people, 160 panel members participated in the first round, 133 in the second round (83% retention), and 134 in the third round (84% retention). Most of the panel members were researchers 76 (47%), clinicians 57 (36%), and consumer representatives 9 (6%) of both genders especially from Brazil 31 (19%), India 22 (14%), and Australia 19 (12%) in the first round. The panel identified fourteen telehealth research priorities spanned topics including the development of strategies using information and communication technology, telehealth implementation services, the effectiveness and cost-effectiveness of telehealth interventions, equity of telehealth interventions, qualitative research and eHealth literacy in musculoskeletal pain conditions from an initial list of 20 research priorities. The consensus was reached for "digital health" and "telehealth" as standard terminologies from an initial list of 37 terminologies. CONCLUSION: An international, multidisciplinary expert consensus recommends that future research should consider the 14 research priorities for telehealth musculoskeletal pain reached. Additionally, the terms digital health and telehealth as the most appropriate terminologies to be used in musculoskeletal telehealth research. ABSTRACT

Health disparities within rural communities, notably those affecting migrant and refugee populations, are well-documented. Refugees often grapple with high disease burdens and mortality rates due to limited access to primary healthcare and their vulnerable socio-economic and political situations. This issue is particularly acute in the rural areas around Medellin, Colombia, where the refugee influx exacerbates the existing public health challenges. Studies highlight a substantial gap between community needs and public health policies, resulting in inadequate healthcare access. Our study, utilizing the Delphi technique, aimed to identify common barriers and strategies to enhance rural healthcare for refugees. Through consensus-building with community leaders, we identified six primary barriers to healthcare access and five barriers to healthcare quality. Community leaders endorsed five strategies to address the access barriers and eight strategies to improve healthcare quality. This research provides valuable insights for optimizing resource allocation and designing effective support programs for these vulnerable populations.

ABSTRACT

This publication is an update of the "Consensus on the therapeutic management of atopic dermatitis - Brazilian Society of Dermatology" published in 2019, considering the novel, targeted-oriented systemic therapies for atopic dermatitis. The initial recommendations of the current consensus for systemic treatment of patients with atopic dermatitis were based on a recent review of scientific published data and a consensus was reached after voting. The Brazilian Society of Dermatology invited 31 experts from all regions of Brazil and 2 international experts on atopic dermatitis who fully contributed to the process. The methods included an e-Delphi study to avoid bias, a literature search and a final consensus meeting. The authors added novel approved drugs in Brazil and the indication for phototherapy and systemic therapy for AD. The therapeutical response to systemic treatment is hereby reported in a suitable form for clinical practice and is also part of this updated manuscript.

ABSTRACT

BACKGROUND: The Delphi technique is a consensus method aiming to obtain statistical estimations from a qualitative approach, through an iterative process that leads to consensus within experts. The main characteristics of the technique include iteration, anonymity, feedback, and consensus reaching. When high-quality, quantitative evidence on a particular topic is insufficient, the Delphi technique can be used for making decisions in clinical scenarios. However, the quality of studies on breast cancer conducted with this technique, has not been assessed. OBJECTIVE: We aim to evaluate the quality of studies on breast cancer which used the Delphi technique as their method. METHODS: A quality assessment tool (Quali-D) was created through consensus among experts on the Delphi technique. Then, the tool was applied to studies on breast cancer which used the Delphi technique as their method. RESULTS: Studies conducted through the Delphi technique mainly assessed for quality indicators and expressed needs in patients with breast cancer. High-quality characteristics were reported in 63.89% of the studies. 98.61% used the Delphi technique due to lack of a more adequate method to solve their research question. 98.61% summarized and presented results in a clear way. In 91.67% of the studies, at least two rounds were conducted. 86.11% described the methods for expert selection in a complete manner. Only 54.17% of the studies reported an anonymous process and 4.17% of the studies disclosed conflicts of interest thoroughly. CONCLUSIONS: A variety of topics were assessed through the Delphi technique in cases where no other technique would have been more appropriate for assessing these issues. Significant limitations are present in terms of anonymity and full disclosure of conflicts of interest. We found that the quality of studies conducted with the Delphi technique regarding breast cancer is overall good. However, the limitations of each study must be considered when applying their results to clinical practice.

ABSTRACT

Background: Although observational studies have already shown promising results of flourishing, a broader concept of health based on positive psychology, there is still a gap in the literature regarding studies that combine different topics of flourishing in a single intervention. Objectives: To develop a comprehensive and integrate intervention based on positive psychology gathering different topics of flourishing to improve mental health outcomes in individuals with depressive symptoms. Methods: The following steps were performed: (1) a comprehensive literature review; (2) the designing of a 12-session group intervention based on the values, virtues, and topics of flourishing; (3) assessment of the rationale, coherence, and feasibility by a panel of healthcare professionals answering semi-structured questions, and (4) application of an e-Delphi technique including mental health experts to reach a consensus of at least 80% for each item of the protocol. Results: A total of 25 experts participated in the study, 8 in the panel with semi-structured questions and 17 in the e-Delphi technique. A three-round e-Delphi technique was required to reach a consensus for all items. In the first round, a consensus was reached for 86.2% of the items. The remaining items (13.8%) were either excluded or reformulated. In the second round, a consensus was not obtained on one item, which was reformulated and approved in the third round. Qualitative analyses of the open questions were performed and suggestions for the protocol were considered. The final version of the intervention was composed of 12 weekly group sessions with 90-min each. The topics included in the intervention were physical and mental health, virtues and character strengths, love, gratitude, kindness, volunteering, happiness, social support, family, friends and community, forgiveness, compassion, resilience, spirituality, purpose and meaning of life, imagining the "best possible future," and flourishing. Conclusion: The flourishing intervention was successfully developed using an e-Delphi technique. The intervention is ready to be tested in an experimental study to verify its feasibility and effectiveness.

Introducción: la hospitalización de una paciente afecta directamente el estado de salud de sus familiares, por lo tanto, se requiere de una intervención psicoeducativa que reduzca el efecto ocasionado en las familias para favorecer el afrontamiento y la adaptación. Objetivo: determinar los componentes con mayor consenso entre profesionales expertos para validar una intervención psicoeducativa que mejore el afrontamiento y la adaptación en familiares de pacientes hospitalizados en la Unidad de Cuidados Intensivos (UCI). Método: estudio cuantitativo y descriptivo realizado con la técnica Delphi, por medio de una única ronda entre profesionales de la salud con experiencia mayor a 2 años en atención a pacientes críticos o investigación en esta área. Resultados: se obtuvieron 14 ítems distribuidos de la siguiente manera: matriz de componentes (3 ítems) y protocolo de la intervención (11 ítems). El 7,14 % de los ítems logró un consenso total, el 85,7 % aceptable y solo el 7,14 % de los ítems no alcanzó un consenso. Conclusiones: los componentes, actividades y acciones propuestas en la intervención psicoeducativa a través de la matriz de componentes como el protocolo de intervención, según el consenso de expertos, es pertinente, congruente, factible y genera impacto, lo cual permite mejorar el afrontamiento y adaptación de familiares de pacientes hospitalizados en UCI. La intervención requiere ser validada por medio de una prueba piloto y un estudio cuasi-experimental que permita medir el impacto de estos resultados.


Introduction: A patient's hospitalization directly affects the health status of their relatives, therefore, a psychoeducational intervention is required to reduce the effect on families, favoring coping and adaptation. Objective: Determine the components with the highest consensus among experts to operationalize psychoeducational intervention that improves coping and adaptation in relatives of hospitalized patients in Intensive Care Units (ICU). Methodology: Primary quantitative and descriptive study, carried out using the Delphi technique, through a single round among health professionals with more than 2 years of experience in critical patient care or research in this area. Results: 14 items were obtained, distributed as follows: component matrix (3 items) and intervention protocol (11 items). 7.14 % of the items reached a total consensus, 85.7 % acceptable and only 7.14 % of the items did not reach a consensus. Conclusions: The components, activities and actions proposed in the psychoeducational intervention through the matrix of components, such as the intervention protocol, according to the consensus of experts proves to be pertinent, congruent, feasible, and generates impact. This allows to improve coping and adaptation of hospitalized patients in ICU's relatives. The intervention needs to be validated by means of a pilot test and a quasi-experimental study that allow measuring the impact of these results.


Introdução: A hospitalização de um paciente afeta diretamente o estado de saúde de seus familiares, portanto, é necessária uma intervenção psicoeducacional que reduza o efeito causado nas famílias, favorecendo o enfrentamento e a adaptação. Objetivo: Determinar os componentes de maior consenso entre os especialistas para operacionalizar a intervenção psicoeducacional que melhora o enfrentamento e a adaptação em familiares de pacientes internados em UTI. Metodologia: Estudo primário, quantitativo e descritivo, realizado pela técnica Delphi, por meio de rodada única, entre profissionais de saúde com mais de 2 anos de familia na assistência ao paciente crítico ou em pesquisa na área. Resultados: foram obtidos 14 itens, assim distribuídos: matriz de componentes (3 itens) e protocolo de intervenção (11 itens). 7,14 % dos itens alcançaram consenso total, 85,7 % aceitáveis e apenas 7,14 % dos itens não alcançaram consenso. Conclusões: Os componentes, atividades e ações propostas na intervenção psicoeducacional através da matriz de componentes como o protocolo de intervenção, segundo o consenso dos especialistas é pertinente, congruente, factível e gera impacto, o que permite melhorar o enfrentamento e adaptação dos familiares de pacientes internados em UTI. A intervenção familia ser validada por meio de um teste piloto e um estudo quase-experimental que permita mensurar o impacto desses resultados.

ABSTRACT Objective: To develop and establish content validation of a nutritional assessment tool for pediatric cancer patients using the Delphi method. Methods: A literature review was performed and the nutritional assessment in pediatrics and cancer construct was discussed with experts. Forty-six nutrition experts from Brazil with experience in oncology participated in the panel. Rounds were held to discuss and judge the items that should be included in this tool. With the aid of an algorithm, it was possible to simultaneously consider the adequacy, relevance and measurement of the items. The consensus was reached with an agreement equal to or greater than 80% among the experts. Results: From the 7 domains suggested by the literature, the first round generated 81 items, which were assessed for adequacy, relevance and the possibility of being measured in the routine of nutritional assessment, by specialists in the following two rounds. The percentage of specialists who responded to each round was high (above 90%) and the dropout rate between the stages was relatively low. After applying the algorithm, the ANPEDCancer tool had 36 items considered appropriate by specialists from the five different geographical regions of Brazil. Conclusions: The Delphi method allowed the construction and content validation of the nutritional assessment tool for children and adolescents with cancer, providing the first stage for use in the Brazilian hospital setting.


RESUMO Objetivo: Desenvolver e estabelecer a construção, a validação de conteúdo e a elaboração de um plano de ação para cada diagnóstico nutricional de um instrumento de avaliação nutricional para pacientes pediátricos com câncer por meio do método Delphi. Métodos: Foi realizada uma revisão da literatura e discutido com especialistas o constructo avaliação nutricional em pediatria em câncer. Participaram do painel 46 especialistas nutricionistas do Brasil com experiência em oncologia. Rodadas ocorreram, para discutir e julgar os itens que deveriam constar do instrumento. Com o auxílio de um algoritmo, foi possível ponderar simultaneamente a adequação, relevância e mensuração dos itens. O consenso foi atingido com concordância igual ou superior a 80% entre os especialistas. Resultados: Com base nos sete domínios sugeridos pela literatura científica, a primeira rodada gerou 81 itens, que foram avaliados quanto à adequação, relevância e possibilidade de ser medido na rotina de avaliação nutricional pelos especialistas nas duas rodadas seguintes. O percentual de especialistas que responderam a cada rodada foi alto (acima de 90%), e a desistência entre as etapas, relativamente baixa. Após aplicação do algoritmo, o instrumento ANPEDCancer contou com 36 itens considerados apropriados pelos especialistas das cinco distintas regiões geográficas do Brasil. Conclusões: O método Delphi permitiu a construção e validação de conteúdo do instrumento de avaliação nutricional para crianças e adolescentes com câncer, fornecendo uma primeira versão a ser aplicada e avaliada no cenário hospitalar brasileiro.

ABSTRACT Purpose mouth breathing (MB) has detrimental effects on children's growth. Diagnosis of MB is possible through a multidisciplinary approach including Speech-Language Pathologist's (SLP) assessment; however, SLPs currently have little to no defined selection criteria to determine the awake and habitual breathing pattern. This study aims at identifying relevant criteria for the assessment of the habitual and awake breathing pattern of preschool children, and developing a grid that would help SLPs diagnose MB in their clinical practice. Methods A three-rounded online international Delphi process was conducted to achieve a consensus on the relevant items and their interpretation. Agreement was established through a Content Validity Ratio calculation. Based on the agreed items, we developed a grid through a scoring function. Results Observing the child at rest (i.e., time spent with an open/closed mouth and position of the tongue/lips) was considered the most important criterion. The experts also considered that observing the breathing pattern while chewing (open/closed mouth) and after swallowing (i.e., air intake and open/ closed mouth just after swallowing) should provide relevant but secondary information in decision-making. We were able to establish a clinical grid based on those criteria. Conclusion The Delphi procedure provided content-valid criteria and conditions of observation for the myofunctional SLP assessment of the awake and habitual breathing pattern in preschoolers. A clinical validation of the developed prototype grid should be conducted in preschool children to explore its effectiveness in the diagnosis of MB.

ABSTRACT

Objective: To develop recommendations to strengthen nutritional care in the primary health care (PHC) setting in Brazil based on expert consensus. Method: A descriptive and exploratory study was performed using the Delphi technique. Of 172 invited PHC experts in nutrition, 116 accepted the invitation and 72 participated in the first Delphi round. The second round had 56 participants from the five Brazilian regions, including managers, healthcare professionals, representatives from civil society organizations, and researchers. The recommendations were developed based on thematic analysis of the responses to open-ended questions about barriers, strategies, actions, and initiatives from five components of the National Food and Nutrition Policy (food and nutrition surveillance, coordination of nutritional care, health promotion, intersectoral focus, and management of food and nutrition actions). In the second round, experts classified each proposed item regarding its pertinence and relevance for the purpose of recommendation. Results: A consensus was reached on 35 recommendations distributed in six categories: structure; PHC nutritional agenda; organization of the healthcare network and PHC; nutritional work processes in PHC; planning, monitoring and evaluation of nutritional actions in PHC; and information systems. These categories were further classified into two blocks: "PHC structuring actions to strengthen nutrition" and "nutrition structuring actions to strengthen PHC." Conclusions: The recommendations were developed by experienced participants who are familiar with both nutritional and PHC pathways from diverse perspectives, and will be useful for the management and proposition of public policies to strengthen nutritional care in the PHC setting in Brazil.


Objetivo: Elaborar recomendaciones para el fortalecimiento de la alimentación y la nutrición en la atención primaria de salud (APS) en Brasil a partir del consenso de especialistas. Métodos: Se realizó un estudio descriptivo y exploratorio con la técnica de Delfos. En total, 172 especialistas en alimentación y nutrición del sistema de APS recibieron una invitación para participar, 116 la aceptaron y 72 intervinieron en la primera ronda. En la segunda ronda hubo 56 participantes de las cinco macrorregiones brasileñas, entre gestores, profesionales de salud, representantes de la sociedad civil e investigadores. Las recomendaciones se formularon mediante un análisis temático de las respuestas a preguntas abiertas sobre obstáculos, estrategias, medidas e iniciativas referentes a cinco aspectos técnicos de la política nacional de alimentación y nutrición (vigilancia alimentaria y nutricional, atención y coordinación del cuidado nutricional, promoción de la salud, intersectorialidad y gestión de las medidas relativas a alimentación y nutrición). En la segunda ronda, los especialistas clasificaron cada punto propuesto según su pertinencia y relevancia para efecto de las recomendaciones. Resultados: Los especialistas formularon 35 recomendaciones, organizadas en seis categorías: estructura; agenda de alimentación y nutrición en la APS; organización en red y organización de la APS; procesos de trabajo relativos a la alimentación y nutrición en la APS; planificación, monitoreo y evaluación de las medidas relativas a la alimentación y nutrición en la APS; y sistema de información. Las categorías se dividieron en dos grupos: medidas estructurantes de la APS para el fortalecimiento de la alimentación y la nutrición y medidas estructurantes de alimentación y nutrición para el fortalecimiento de la APS. Conclusiones: Las recomendaciones provinieron de personas expertas que siguen la trayectoria tanto de la alimentación y la nutrición como de la APS desde diferentes puntos de vista, útiles para la gestión y la propuesta de políticas públicas destinadas a fortalecer la alimentación y la nutrición en la APS de Brasil.

Resumo Este trabalho visou elaborar e validar uma lista de verificação dos processos produtivos de dietas pediátricas para uso em lactários hospitalares. Para a elaboração do instrumento realizou-se pesquisa bibliográfica. A validação do conteúdo foi realizada com 5 especialistas, pela Técnica Delphi - utilizando a escala Likert de 5 pontos. A validação de aparência foi realizada por 3 nutricionistas, no mesmo ambiente, dia e horário. O instrumento incluiu cabeçalho, guia de instruções de preenchimento e 225 itens de avaliação que obtiveram o conteúdo validado. Os valores mínimos e máximos para o índice de validade de conteúdo (IVC) foi, respectivamente, 0,88 e 0,96. O coeficiente de correlação intraclasse (CCI) e Alfa de Cronbach (α) foi >0,80 em mais de 90% dos itens avaliados. Os valores mínimos e máximos para o índice de validade de aparência (IVA) foi, respectivamente, 0,73 e 0,93. O teste de Kruskal Wallis não apresentou diferença significativa durante as avaliações (p valor>0,05) para os blocos da versão rotina e versão gestão. As versões do instrumento apresentaram em mais de 80% dos blocos α e CCI>0,80. Portanto, o instrumento apresentou conteúdo e aparência validados tendo reprodutibilidade e confiabilidade, ou seja, viabilidade de utilização na prática do nutricionista.


Abstract This work aimed to draft and validate a checklist of the production processes of pediatric diets for use in hospital lactaries. A bibliographic search was carried out in order to draft the instrument. Content validation was performed with 5 experts, using the Delphi technique and the 5-point Likert scale. Appearance validation was performed by 3 nutritionists, in the same environment, day, and time. The instrument included a header, a guide to completion instructions, and 225 assessment items that obtained validated content. The minimum and maximum values for the content validity index (CVI) were 0.88 and 0.96, respectively. The intraclass correlation coefficient (ICC) and Cronbach's alpha (α) was >0.80 in more than 90% of the items evaluated. The minimum and maximum values for the Appearance Validity Index (AVI) were, respectively, 0.73 and 0.93. The Kruskal Wallis test showed no significant difference during the evaluations (p-value>0.05) for the blocks of the routine version and the management version. The instrument versions presented in more than 80% of the α and ICC blocks > 0.80. Therefore, the instrument presented validated content and appearance, presenting reproducibility and reliability in terms of the feasibility of use in a nutritionist's practice.

BACKGROUND: Hospital pharmacists cannot provide extensive clinical pharmacy service to every inpatient because the demand for these services can exceed pharmacists' available work time. A way to solve this issue is hospital pharmacists to prioritize their clinical tasks. Tool prioritization could determine which patients would benefit from clinical pharmacists' input. AIM: Establishing consensus on which criteria are relevant for prioritizing patients for clinical pharmacy services. METHOD: The Delphi method was performed with criteria identified from a previously published Scoping Review. The panel of experts included hospital pharmacists, who evaluated the clinical significance of criteria in a three-round Delphi panel from July to December 2020. They determined the need for sub-criteria and evaluated their clinical significance. The experts also analyzed the criteria/sub-criteria as to their applicability in clinical practice. Consensus criteria were defined as 70% or more participants scoring the criteria as critical and 15% or fewer scoring the criteria as unimportant. RESULTS: A total of 19 criteria and 230 sub-criteria were included for evaluation by panel experts based on scoping review. Twenty-nine, 22, and 17 experts participated per round, respectively. After completing the three rounds, experts suggested the inclusion of one criterion, the exclusion of one criterion, and the inclusion of 29 sub-criteria. The final list consisted of 18 criteria and 177 sub-criteria, divided into 28 groups. CONCLUSION: The result was comprehensive and coherent, potentially contributing to developing an instrument for prioritizing hospitalized patients for clinical pharmacy services.

OBJETIVO: El objetivo de este estudio es la elaboración de un protocolo para el diagnóstico y el abordaje de la enfermedad de Kawasaki, basado en la evidencia científica disponible. METODOLOGÍA: Realización de una revisión sistemática para localizar, sintetizar y analizar la evidencia científica disponible. Las bases de datos interrogadas fueron: MEDLINE (mediante OVID), EMBASE, Web of Science (WOS), Epistemonikos database, Cochrane Library y páginas web específicas de guías de práctica clínica, con los términos "'mucocutaneous lymph node síndrome", "Kawasaki disease" y "Kawasaki syndrome". Así mismo, se realizó un cuestionario telemático a pacientes y familiares. Con la información localizada y las principales cuestiones reflejadas por los pacientes, se elaboraron una serie de recomendaciones. 168 recomendaciones se sometieron a votación mediante metodología Delphi por un panel de expertos multidisciplinar, implicados en la atención a los pacientes de esta patología. RESULTADOS: Se obtuvieron un total de 165 recomendaciones para el diagnóstico, tratamiento y seguimiento, con consenso del panel de expertos. Dichas recomendaciones quedaron recogidas en un protocolo asistencial, y gráficamente en 2 algoritmos para facilitar al personal sanitario la comprensión y aplicación de las recomendaciones seleccionadas. CONCLUSIONES: La enfermedad de Kawasaki sigue siendo una enfermedad poco conocida, pero controlable con un diagnóstico, tratamiento temprano y seguimiento adecuado. Para lo cual se hace imprescindible la unificación de criterios en el manejo de estos pacientes. Los protocolos clínicos son una valiosa herramienta que aporta información a los profesionales y ayuda en la toma de decisiones.

The databases searched were: MEDLINE (via OVID), EMBASE, Web of Science (WOS), Epistemonikos database, Cochrane Library and specific web pages of clinical practice guidelines, with the terms "'mucocutaneous lymph node syndrome", "Kawasaki disease" and "Kawasaki syndrome". A telematic questionnaire was also administered to patients and relatives. A series of recommendations were drawn up based on the information collected and the main issues raised by patients. 168 recommendations were voted on using Delphi methodology by a panel of multidisciplinary experts involved in the care of patients with this pathology. RESULTS: A total of 165 recommendations for diagnosis, treatment and follow-up were obtained, with the consensus of the panel of experts. These recommendations were set out in a care protocol, and graphically in 2 algorithms to make it easier for healthcare personnel to understand and apply the selected recommendations. CONCLUSIONS: Kawasaki disease remains a poorly understood disease, but controllable with diagnosis, early treatment and appropriate followup. For this reason, it is essential to unify criteria in the management of these patients. Clinical protocols are a valuable tool that provides information to professionals and helps in decision-making.

ABSTRACT Background: Parkinson's disease (PD) is a chronic disease that presents a multitude of symptoms, with symptoms of both motor and nonmotor nature. The Delphi method is widely used to create consensuses among experts in a field of knowledge. Objective: In order to reach a consensus on the values that should be assigned to the different motor and nonmotor manifestations of Parkinson's disease, a linear evaluation index (LEI) was created. Subsequently, the metric properties of this index were studied. Methods: 120 consecutive patients with a Parkinson's diagnosis were chosen in accordance with the UKPDSBB criteria. The Delphi method was used to reach a consensus among experts regarding the values of each of the manifestations included. Subsequently, the following attributes were analyzed: quality and acceptability of the data; reliability, in terms of internal consistency, reliability index, Cronbach's alpha and standard error of measurement; and validity, in terms of convergent validity and validity for known groups. Results: Twenty-five experts participated. The importance factor did not differ between the first round and the second round (chi-square test). We analyzed the responses that assigned percentage values to the 10 dimensions of the LEI. Both in the first and in the second round, the values of the scattering coefficient Vr were always close to 0. The homogeneity index was 0.36; the corrected-item total correlation values ranged from 0.02 to 0.7; Cronbach's α was 0.69; and the SEM was 4.23 (55.1%). Conclusions: The LEI was obtained through rigorous recommended methodology. The results showed adequate metric properties.


RESUMO Antecedentes: La enfermedad de Parkinson (EP) es una enfermedad crónica que presenta una multitud de síntomas, tanto de naturaleza motora cuanto no motora. El método Delphi se utiliza ampliamente para crear un consenso entre expertos de un campo del conocimiento. Objetivos: Con el fin de llegar a un consenso sobre los valores que deben asignarse a las diferentes manifestaciones motoras y no motoras de la enfermedad de Parkinson, se creó el "Índice de Evaluación Lineal" (linear evaluation index - LEI). Posteriormente, se estudiaron las propiedades métricas de este índice. Métodos: Se eligieron 120 pacientes consecutivos con diagnóstico de Parkinson según los criterios del UKPDSBB. Se utilizó el método Delfos para llegar a un consenso entre los expertos sobre los valores de cada una de las manifestaciones incluidas. Posteriormente, se analizaron los siguientes atributos: Calidad y aceptabilidad de los datos. Fiabilidad: consistencia interna, índice de fiabilidad, alfa de Cronbach y error estándar de medida. Finalmente, Validez: validez convergente y validez para grupos conocidos. Resultados: Participaron 25 expertos, el factor de importancia entre la primera y la segunda rondas (prueba chi-cuadrado), no fue diferente. Analizamos las respuestas que asignaron valores porcentuales a las 10 dimensiones del LEI; tanto en la primera como en la segunda rondas, los valores del coeficiente de dispersión Vr siempre estuvieron cerca de 0. El índice de homogeneidad fue de 0,36; los valores corregidos de correlación ítem-total variaron de 0,02 a 0,7; alpha de Cronbach fue de 0,69. El SEM fue 4,23 (55,1%). Conclusiones: El LEI se ha obtenido siguiendo una rigurosa metodología recomendada. Los resultados han mostrado propiedades métricas adecuadas.
