ABSTRACT

To evaluate the effect of adhesive coats application on the enamel microtensile bond strength (µTBS) of universal adhesives, morphological etching pattern and their chemical interaction with hydroxyapatite (HA). Two universal adhesives were investigated: Scotchbond Universal (SBU, 3 M) and Prime&Bond Universal (PBU, Dentsply). The adhesives were applied in self-etching mode on bovine enamel (n = 8) in one (1L), two (2L) or three coats (3L) and light-cured as per manufacturers' instructions. As controls adhesives were applied to etched enamel (H3PO4-37% phosphoric acid). Bonded specimens were cut into sticks that were stored in deionized water for 24 h or 6 months prior to µTBS testing. Two-way ANOVA and Tukey's test were used for statistical analysis of bond strength with α = 5%. For morphological SEM analysis, enamel surfaces were treated as aforementioned and immediately rinsed with acetone. The intensity of monomer-calcium salt formation from each treatment was measured via infrared spectroscopy (ATR-FTIR). All treatments presented no significant reduction on µTBS after aging (p > 0.05). However, SBU attained highest µTBS when applied in 3L. PBU showed higher µTBS when applied to H3PO4 etched enamel than 1L or 2L. Etching pattern was enhanced by 3L application, particularly with PBU. Chemical interaction was notably higher for SBU than PBU, with no relevant differences with more layers or prior H3PO4-etching. The application of three adhesive coats of universal adhesives in self-etch mode using may enhance the bonding performance and etching pattern to enamel, surpassing the H3PO4-etched enamel bond for SBU. The chemical interaction with calcium from enamel is not affected by number of coats or prior phosphoric acid etching.

ABSTRACT

This systematic review evaluated the effect of different hydrofluoric acid (HF) etching regimens and a self-etch ceramic primer (SECP) on the flexural strength (FS) and fatigue failure load (FFL) of glass-ceramic materials.The identification of relevant studies was conducted by two authors in five databases: PubMED, Scopus, Web Of Science, LILACS and Virtual Health Library (BVS) until July 2022 with no year limit. The analysis was conducted in RevMan 5.4.1 Software (Cochrane Collaboration) using Random effect model at 5 %. The risk of bias of the included studies were assessed. From the 5349 articles identified, 34 were included for quantitative analysis. Meta-analysis showed that for predominantly glassy ceramics, etching with HF 5 % had no significant impact on FS, however, HF acid etching with concentrations greater than 5 % negatively impacted FS. For lithium disilicate glass-ceramics (LDGC) HF acid etching, negatively influenced FS, while increasing the FFL. HF etching negatively affected FS of hybrid ceramics. The self-etch ceramic primer and HF acid etching showed a similar impact on FFL and FS. This meta-analysis indicates that the impact of SECP and HF acid etching on the mechanical behavior of glass ceramics is material-dependent.

ABSTRACT

Objective: This study aimed to evaluate the bond strength of a universal adhesive to dentin (µTBS) using different time periods of airborne particle abrasion (APA) and two types of acid etching. Methods: Seventy-two human third molars were divided into 9 groups (n=8) according to dentin pretreatment: APA duration (0, 5, or 10s) and acid etching (no acid - NA, 37% phosphoric acid - PhoA, or 1% phytic acid - PhyA). APA was performed at a 0.5 cm distance and air pressure of 60 psi using 50 µm aluminum oxide particles. Afterwards, two coats of Single Bond Universal adhesive (3M) were applied to the dentin surface. Composite blocks were built using the incremental technique, sectioned into 1×1 mm slices and subjected to microtensile bond strength (µTBS) testing. Fracture patterns and surface topography of each dentinal pretreatment were evaluated using a Scanning electron microscope (SEM). Bond strength data were analyzed using two-way ANOVA and Bonferroni post-hoc tests. Results: The group that received pretreatment with 5s APA and PhoA presented higher µTBS values among all groups, which was statistically different when compared with the PhoA, 10APA+PhoA, and 5APA+PhyA groups. PhyA did not significantly influence the bond strength of the air-abraded groups. Finally, adhesive failure was considered the predominant failure in all groups. Conclusion: Dentin pretreated by airborne particle abrasion using aluminum oxide demonstrated an increase in bond strength when abraded for 5 seconds and conditioned with phosphoric acid in a universal adhesive system.

ABSTRACT

OBJECTIVE: Familial isolated hypoparathyroidism is a rare genetic disorder due to no or low production of the parathyroid hormone, disturbing calcium and phosphate regulation. The resulting hypocalcemia may lead to dental abnormalities, such as enamel hypoplasia. The aim of this paper was to describe the full-mouth rehabilitation of a 15-year-old girl with chronic hypocalcemia due to a rare congenital hypoparathyroidism. CLINICAL CONSIDERATIONS: In this patient, in the young adult dentition, conservative care was preferred. Onlays or stainless-steel crowns were performed on the posterior teeth, and direct or indirect (overlays and veneerlays) were performed on the maxillary premolars, canines, and incisors, using a digital wax-up. The mandibular incisors were bleached. The treatment clearly improved the patient's oral quality of life, with fewer sensitivities, better chewing, and aesthetic satisfaction. The difficulties were the regular monitoring and the limited compliance of the patient. CONCLUSION: Despite no clinical feedback in the literature, generalized hypomineralized/hypoplastic teeth due to hypoparathyroidism in a young patient can be treated as amelogenesis imperfecta (generalized enamel defects) with a conservative approach for medium-term satisfactory results. HIGHLIGHTS: This study provides new insights into the management of enamel hypoplasia caused by familial isolated hypoparathyroidism, helping to improve patient outcomes in similar cases.

ABSTRACT

OBJECTIVES: To evaluate the influence of dentin moisture on the clinical behavior of a universal adhesive on posterior teeth after 36 months of follow-up. METHODS: Forty-five patients participated in this study. Following a split-mouth design, three operators placed 90 Class I/Class II restorations over moist dentin (MD) or dry (DD) (n = 45) with resin composite (Filtek Bulk Fill) and a universal adhesive used in the etch-and-rinse mode (Single Bond Universal). Each restoration was evaluated according to the FDI and USPHS criteria (postoperative sensitivity, fracture and retention, marginal staining, marginal adaptation, and recurrence of caries) at baseline and after 6-, 12-, and 36 months. For statistical analysis, Kruskal Wallis analysis of variance rank (α = 0.05) and Kaplan-Meier survival analysis were used. RESULTS: No significant difference between groups was observed in each FDI criterion after 36 months of clinical evaluation (p > 0.05). The retention rates (confidence interval 95 %) were 97.37 % (86.5 - 99.5) for both MD and DD without significant difference between them (p > 0.05). Eight restorations (MD = 4; DD = 4) showed minimal marginal staining defects (p > 0.05). Two restorations were lost (MD = 1; DD = 1). Fifteen restorations (MD = 8; DD = 7) presented minor marginal discrepancies according to the FDI criteria (p > 0.05). CONCLUSION: The clinical performance of the universal adhesive when applied in etch-and-rinse mode was not influenced by dentin moisture in posterior bulk-fill composite restorations. CLINICAL SIGNIFICANCE: The level of dentin moisture appears not to influence the clinical efficacy of a universal adhesive when applied using the etch-and-rinse technique in posterior composite resin restorations.

ABSTRACT

Objectives: To compare three different orthodontic adhesives (Transbond XT Light Cure Adhesive, Heliosit Orthodontic, Fuji Ortho LC) bonded to two types of orthodontic brackets: ceramic brackets (Fascination Roth 0.22) and metallic brackets (Topic Roth 0.22, Dentaurum). Materials and methods: The study was performed on 18 human teeth (6 for each adhesive). The prepared teeth were divided into three groups according to the examination time. Subsequently, they were observed after 1, 2 and 3 weeks following bonding. After the experimental procedure, the teeth samples were cut in half along the longitudinal axis in the vestibulo-oral direction, fixed with conductive carbon cement, placed in a high-vacuum evaporator and then coated with carbon. One half of each sample was observed under a Field-emission gun scanning electron microscope (FEG-SEM Hitachi SU 8030, Japan), while on the second half of the samples qualitative (X-ray line-scans) and semi-quantitative point X-ray energy dispersive analyses (EDX) were performed with Thermo Noran (USA) NSS System 7, equipped with Ultra Dry detector (30 mm2 window). Results: Transbond XT had an ideal bond with the enamel and the bracket base, with rare presence of microgaps and cracks in the enamel. Heliosit Orthodontic demonstrated a better bond relationship with the bracket base than the enamel, whereas in the latter the presence of microgaps in the bond was observed. The microphotographs of Fuji Ortho LC demonstrated many cracks inside the adhesive, and some of them continued to move forward into the enamel surface. Therefore, an impression of a very solid bond relationship with the enamel exists, with cracks being present in the enamel surface and never at the enamel-adhesive interface. Microgaps also appeared at the bracket-adhesive interface. Conclusion: Transbond XT is a highly filled composite resin and is an ideal orthodontic adhesive in each aspect examined, with an ideal enamel-adhesive and bracket-adhesive interface. Heliosit Orthodontic provides better bracket-adhesive interface compared to the enamel. Fuji Ortho LC as a solid resin-modified GIC provides a better enamel-adhesive interface, compared to the bracket base.

ABSTRACT

Objectives: Durable bonding to zirconia is a challenging issue in dentistry. This study aimed to assess the effect of bioglass coating of zirconia on the microshear bond strength of resin cement to zirconia and to study the effect of thermocycling on this bond. Materials and Methods: This in-vitro experimental study was conducted on 60 yttria-stabilized tetragonal zirconia blocks in six groups (N=10) based on surface pretreatment and thermocycling. Surface pretreatments included no treatment control, alumina particle abrasion, and bioglass-coating of zirconia. Resin bonding was performed with Panavia F2.0 cements. Then, half of the specimens underwent a 24-hour incubation in 37°C water, while the other half were subjected to thermocycling (12000 cycles, 5-55°C, 60s for each batch) following the same incubation period. Subsequently, the microshear bond strength of the specimens was measured. Additionally, one block from each group was subjected to scanning electron microscopy and X-ray diffraction. The data were analyzed using Kruskal-Wallis and Mann-Whitney U tests. Results: There was a significant difference between the bond strength values of different groups (P<0.001). Alumina particle abrasion and bioglass coating equally increased the bond strength compared to the untreated control group (P<0.001). Thermocycling caused significant decreases in bond strength in all the groups (P<0.001); however, the bond strength value of the thermocycled bioglass-coated group was significantly higher than that reported for the thermocycled alumina particle abraded group (P=0.015). Conclusion: Despite the decrease in the bond strength values after thermocycling, the long-term efficacy of the bioglass coating of zirconia was promising.

ABSTRACT

This paper evaluated the influence of different protocols of silver fluoride (SF) pretreatment of artificial carious lesions on the adhesive interface of composite resin restorations and remineralization of deciduous dentin compared to silver diamine fluoride (SDF). Sixty-four deciduous molar teeth were randomly divided into 8 groups (n = 8) according to the restoration time (immediately - IM; 30 days after SDF/SF treatment - 30 D) and treatment before restoration (SDF 38 %; SDF 38 % + potassium iodide - KI; SF 38 %; SF 38 % +KI). After SDF/SF application, teeth in the IM group were restored with self-etch universal adhesive system/composite resin. Samples in the 30D groups were stored in artificial saliva (37 °C) for 30 days before receiving the same restoring protocol. Beams were obtained from all groups and subjected to bond strength tests (µTBS), ultrastructural qualitative analysis (FEG) and mineral analysis (SEM/EDX; Micro-Raman spectroscopy). The µTBS data were subjected to three-factor ANOVA and multiple comparisons (Holm-Sidak method). Bond strength values (MPa) for IM groups were 16.9 ± 2.7 (SDF); 17.6 ± 3.5 (SDF + KI); 16.8 ± 5.5 (SF); 18.4 ± 4.1 (SF + KI); and 14.9 ± 4.2 (SDF); 16.0 ± 5.4 (SDF + KI); 14.1 ± 3.6(SF); 16.4 ± 5.4 (SF + KI) for 30D groups. Bond strength wasn't influenced by the moment of restoration (IM or 30D); the use of KI didn't alter adhesion characteristics; SDF/SF solutions resulted in similar adhesive strength; calcium and phosphate expressions were identified at the interfaces on IM and 30D moments. However, 30D presented qualitative increase in these ions, compatible with remineralization. It was concluded that the adhesion of composite resin restorations in artificial caries lesions of deciduous teeth treated with SDF (38 %) and SF (38 %) had similar effects in vitro; the use of KI or the moment when restorations were accomplished did not influence the adhesion and all tested protocols promoted remineralization of carious dentin.

OBJECTIVES: To evaluate the clinical performance of posterior restorations over wet and dry dentin with an etch-and-rinse adhesive after 36 months of clinical service. METHODS: Forty-five participants were recruited, each one had at least two posterior teeth that needed restoration. Ninety restorations were placed on Class I or Class II cavities. For the restoration protocol, a simplified etch-and-rinse adhesive (Adper Single Bond 2) was applied over wet (WD) or dry dentin (DD) and later restored with a bulk-fill composite (Filtek Bulk Fill) under rubber dam isolation. Each restoration was evaluated using the World Dental Federation (FDI) criteria after 6, 12, and 36 months of clinical service, regarding the following principal restoration characteristics: postoperative sensitivity, marginal discoloration, marginal adaptation, fracture of material and retention, and recurrence of caries. Kruskal Wallis analysis of variance rank (α = 0.05) and Kaplan-Meier survival analysis were used for statistical analysis. RESULTS: After 36 months of clinical evaluation, no significant difference between groups was observed in each FDI criterion (p > 0.05). Twenty restorations (WD=10, DD=10) showed minor marginal staining, and twenty-two restorations (WD=11, DD=11) presented small marginal adaptation defects (p > 0.05). Four restorations were lost (WD = 2, DD = 2) and the fracture rates (95% confidence interval) were 94.9% for each one, without significant difference between wet and dry dentin (p > 0.05). SIGNIFICANCE: The degree of dentin moisture does not seem to affect the clinical performance of a simplified etch-and-rinse adhesive in posterior restorations when the adhesive is applied vigorously over the dentine surface.

ABSTRACT

PURPOSE: This study aimed to investigate the bonding effectiveness of two HEMA/BPA-free universal adhesives (UAs) to flat dentin, to characterize their adhesive-dentin interfacial ultrastructure, and to measure their water sorption (Wsp), water solubility (Wsl), and hydrophobicity. MATERIALS AND METHODS: The immediate and aged (50,000 thermocycles) microtensile bond strength (µTBS) to flat dentin of the HEMA/BPA-free UAs Healbond Max (HbMax; Elsodent) and Healbond MP (HbMP; Elsodent) as well as the reference adhesives OptiBond FL (Opti-FL; Kerr), Clearfil SE Bond 2 (C-SE2; Kuraray Noritake), and Scotchbond Universal (SBU; 3M Oral Care) was measured. The adhesive-dentin interfaces of HbMax and HbMP were characterized by TEM. Wsp and Wsl of all adhesive resins and of the primer/adhesive resin mixtures of HbMax, Opti-FL, and C-SE2 were measured. Hydrophobicity was determined by measuring the contact angle of water dropped on adhesive-treated dentin. RESULTS: In terms of µTBS, HbMax and HbMP performed statistically similarly to Opti-FL and C-SE2, but outperformed SBU. Aging only significantly reduced the µTBS of SBU when applied in E&R bonding mode. TEM revealed typical E&R and SE hybrid-layer ultrastructures at dentin, while electron-lucent globules of unknown origin, differing in size and shape, were observed within the adhesive resin of HbMP and even more frequently in that of HbMax. Higher Wsp was measured for the primer/adhesive resin mixtures than for the adhesive resins. Opti-FL was more hydrophobic than all other adhesives tested. CONCLUSION: The HEMA/BPA-free UAs bonded durably to flat dentin with bond strengths comparable to those of the gold-standard E&R/SE adhesives and superior to that of the HEMA/BPA-containing 1-step UA.

ABSTRACT

The process of bonding to dentin is complex and dynamic, greatly impacting the longevity of dental restorations. The tooth/dental material interface is degraded by bacterial acids, matrix metalloproteinases (MMPs), and hydrolysis. As a result, bonded dental restorations face reduced longevity due to adhesive interfacial breakdown, leading to leakage, tooth pain, recurrent caries, and costly restoration replacements. To address this issue, we synthesized and characterized a multifunctional magnetic platform, CHX@SiQuac@Fe3O4@m-SiO2, to provide several beneficial functions. The platform comprises Fe3O4 microparticles and chlorhexidine (CHX) encapsulated within mesoporous silica, which was silanized by an antibacterial quaternary ammonium silane (SiQuac). This platform simultaneously targets bacterial inhibition, stability of the hybrid layer, and enhanced filler infiltration by magnetic motion. Comprehensive experiments include X-ray diffraction, FT-IR, VSM, EDS, N2 adsorption-desorption (BET), transmission electron microscopy, scanning electron microscopy, thermogravimetric analysis, and UV-vis spectroscopy. Then, CHX@SiQuac@Fe3O4@m-SiO2 was incorporated into an experimental adhesive resin for dental bonding restorations, followed by immediate and long-term antibacterial assessment, cytotoxicity evaluation, and mechanical and bonding performance. The results confirmed the multifunctional nature of CHX@SiQuac@Fe3O4@m-SiO2. This work outlined a roadmap for (1) designing and tuning an adhesive formulation containing the new platform CHX@SiQuac@Fe3O4@m-SiO2; (2) assessing microtensile bond strength to dentin using a clinically relevant model of simulated hydrostatic pulpal pressure; and (3) investigating the antibacterial outcome performance of the particles when embedded into the formulated adhesives over time. The results showed that at 4 wt % of CHX@SiQuac@Fe3O4@m-SiO2-doped adhesive under the guided magnetic field, the bond strength increased by 28%. CHX@SiQuac@Fe3O4@m-SiO2 enhanced dentin adhesion in the magnetic guide bonding process without altering adhesive properties or causing cytotoxicity. This finding presents a promising method for strengthening the tooth/dental material interface's stability and extending the bonded restorations' lifespan.

ABSTRACT

OBJECTIVE: To investigate the effect of ceramic thickness and dental substrate (enamel vs. dentin/enamel) on the survival rate and failure load of non-retentive occlusal veneers. MATERIALS AND METHODS: Human maxillary molars (n = 60) were divided into five test-groups (n = 12). The groups (named DE-1.5, DE-1.0, DE-0.5, E-1.0, E-0.5) differed in their dental substrate (E = enamel, DE = dentin/enamel) and restoration thickness (standard: 1.5 mm, thin: 1.0 mm, ultrathin: 0.5 mm). All teeth were prepared for non-retentive monolithic lithium-disilicate occlusal veneers (IPS e.max Press, Ivoclar). Restorations were adhesively cemented (Syntac Classic/Variolink II, Ivoclar) and exposed to thermomechanical fatigue (1.2 million cycles, 1.6 Hz, 49 N/ 5-55°C). Single load to failure was performed using a universal testing-machine. A linear-regression model was applied, pairwise comparisons used the Student-Newman-Keuls method (p < 0.05). RESULTS: Three dentin-based occlusal veneers (one DE-1.0, two DE-0.5) revealed cracks after fatigue exposure, which corresponds to an overall-survival rate of 95%. Load to failure resulted in the following ranking: 2142 N(DE-0.5) > 2105 N(E-1.0) > 2075 N(E-0.5) > 1440 N(DE-1.5) > 1430 N(DE-1.0). Thin (E-1.0) and ultrathin enamel-based occlusal veneers (E-0.5) revealed high failure loads and surpassed the standard thickness dentin-based veneers (DE-1.5) significantly (p = 0.044, p = 0.022). CONCLUSION: All tested monolithic lithium disilicate occlusal veneers obtained failure loads above physiological chewing forces. Thin and ultrathin enamel-based occlusal veneers outperformed the standard thick dentin-based occlusal veneers. CLINICAL SIGNIFICANCE: Minimally invasive enamel-based occlusal veneer restorations with non-retentive preparation design may serve as a conservative treatment option.

ABSTRACT

OBJECTIVE: Demonstrate the ability of 0.5 mm thickness lithium disilicate laminates associated with resin cement to mask different substrate saturations. METHODS: 60 specimens (n = 5) were prepared with CAD/CAM lithium disilicate glass-ceramic IPS e.max CAD, 0.5 mm thick, in three degrees of translucency (HT, MT and LT); cemented by Variolink Esthetic LC Light+ (L) and Neutral (N) on composite resin substrates A1 and A4 shades. Color measurements were performed by a spectrophotometer (CM-3700d -Konica Minolta). The L*, a*, b* parameters of the tri-layer structure (laminate/ resin cement/ substrate) were used to calculate color difference ( ∆ E 00 ) by CIEDE2000 formula and Translucency Parameter ( TP 00 ). This study adopted ∆ E 00 = 0.8, as perceptibility threshold (PT), and ∆ E 00 = 1.8, as acceptability threshold (AT). Statistical analyses were performed by one-way ANOVA and Tukey tests (a = 0.05). RESULTS: Color difference between resin cement shades showed statistical differences (p < 0.0001) and ∆ E 00 > 1.8 for all groups. Color difference between substrates saturations showed a significant statistical difference (p < 0.0001) and ∆ E 00 values above AT for all groups. The TP 00 demonstrated significant statistical differences (p < 0.0001). The LTL combination, the opaquest set, showed the smallest ∆ E 00 values between substrate shades differences, however, above AT. CONCLUSION: The translucency of the lithium disilicate, the substrate saturation and the resin cement opacity influenced final color of restoration. The ceramic translucency impact on final color was reduced by opaque resin cement. Furthermore, the LT lithium disilicate and the Light+ resin cement reduced the translucency of the restoration, contributing to mask saturated substrate. CLINICAL RELEVANCE: This study shows the influence of conservative lithium disilicate laminates with different degrees of translucency associated with resin cement on final color of saturated substrate restorations. A minimally invasive esthetic protocol for masking substrates is demonstrated by associating low translucency ceramic laminates and opaque resin cement.

ABSTRACT

Molar-incisor hypomineralisation (MIH) is a qualitative defect of the enamel structure. Indirect restorations may represent the most suitable therapeutic solutions for patients presenting MIH with tooth restorative procedures. This systematic review aims to determine the feasibility of indirect restorations. MATERIALS AND METHODS: A systematic review has been performed and is reported following the PRISMA guidelines. It was performed on three databases (PubMed, Science Direct, and Google Scholar). Ten articles were included. RESULTS: Only two articles reported the use of CAD/CAM technologies, whereas the other eight preferred conventional registration and handmade stratification for ceramics. All indirect bonded restorations made of composite resins or ceramics had significant success rates. A temporary material was placed in most of the articles. There was no clear consensus for tissue conditioning before bonding. Depending on the authors and the articles, the follow-up period extended from 2 months to 6 years. CONCLUSIONS: The survival rate and the non-invasive procedures of indirect restorations are two main arguments that can help dental practitioners in daily practice. Development of CAD/ CAM technologies adds new perspectives in the registration, the design and production. However, more clinical trials are needed to confirm the conclusions.

ABSTRACT

OBJECTIVE: Obtaining strong resin bonds to computer-aided design and computer-aided manufacturing (CAD-CAM) resin-based composites with dispersed fillers (CCRBCs) poses a challenge. Therefore, this study aimed to assess the effect of three universal adhesives and a two-component silane coupling agent on the shear bond strength to three (CCRBCs). MATERIALS AND METHODS: Eight hundred and sixty-four specimens of Brilliant Crios, Lava Ultimate, and Tetric CAD were polished or grit blasted, bonded with Adhese Universal DC, One Coat 7 Universal (OC7), and Scotchbond Universal Plus Adhesive (SBU) with or without a silane primer. Shear bond strength was measured after 24 h and 10,000 thermocycles. Linear regressions were performed (α = 0.05). RESULTS: After thermocycling, bond strengths were similar for the universal adhesives on polished Brilliant Crios and Lava Ultimate (p ≥ 0.408). Grit blasted Tetric CAD showed no significant differences (p ≥ 0.096). The silane primer had minimal impact on grit blasted Tetric CAD (p ≥ 0.384). The silane primer increased the bond strength of OC7 to Brilliant Crios (p = 0.001) but decreased the adhesion of SBU to Brilliant Crios and Lava Ultimate (p ≤ 0.018). CONCLUSIONS: Bond strength of universal adhesives varied with CCRBC type. The two-component silane coupling agent showed mixed effects on adhesive performance. CLINICAL SIGNIFICANCE: Selecting universal adhesives from the same CCRBC product line does not always guarantee superior bond strength. The efficacy of silane coupling agents differs based on the bonding substrate.

ABSTRACT

Objective: This systematic review aimed to evaluate the adhesive bond strength of restorative materials to caries-affected dentin (CAD) treated with antimicrobial photodynamic therapy (aPDT) in comparison with conventional chemical disinfectants. Methods: Three databases, including the Web of Science, Scopus, and PubMed, were searched to address the focused question: "What is the effect of aPDT compared to conventional chemical disinfection techniques on the adhesive bond strength of restorative materials to CAD?." Search keywords included "dentin*" "adhes bond*" "caries-affected dentin" "photodynamic "photochemotherapy" "photosensitizing agent" "phototherapy" "photoradiation" "laser" "light activated" "photoactivated." A fixed-effects model was used in each meta-analysis and the inverse variance was used to calculate the standard mean difference (SMD). For evaluating the statistical heterogeneity, the Cochrane's Q test and the I2 statistics were used. The risk of bias was evaluated based on the Cochrane Collaboration's tool. Results: Fourteen studies were included in the qualitative as well as quantitative analysis. The results of the meta-analyses exhibited an SMD of 2.38% [95% confidence interval (CI): 2.03-2.73; p < 0.00001], indicating a statistically significant difference in the shear bond strength scores between the tested group (samples treated with aPDT) and the control group (i.e., favoring the sound dentin and/or conventional chemical disinfectants). Contrarily, an SMD of -1.46% (95% CI: -2.04 to -0.88; p < 0.00001) and -0.37% (95% CI: -0.70 to -0.03; p = 0.03) was observed, indicating a statistically significant difference in the microtensile bond strength (µTBS), as well as microleakage scores between the tested group (favoring the samples treated with aPDT) and the control group (i.e., sound dentin and/or conventional chemical disinfectants). Conclusions: Adhesive bond strength of restorative materials to CAD treated with conventional chemical disinfectants showed superior outcomes compared to photodynamic therapy (aPDT).

Abstract The aim of this study was to evaluate the influence of adding quaternary ammonium methacrylates (QAMs) to experimental adhesives by assessing the degree of conversion (DC), cytotoxicity against keratinocytes and fibroblasts, and antibacterial activity against biofilm formation. Two QAMs were added to an experimental adhesive: dimethylaminododecyl methacrylate bromododecane (DMADDM) or dimethylaminododecyl methacrylate bromohexadecane (DMAHDM) at three concentrations each: 1, 2.5, and 5 wt.%. Experimental adhesive without QAMs (control group) and commercially available Transbond XT Primer (3M Unitek, Monrovia, California, USA) were used for comparisons. The adhesives were tested for DC, cytotoxicity against keratinocytes and fibroblasts, and antibacterial activity against biofilm formation. DC, cytotoxicity against fibroblasts, and antibacterial activity were analyzed using one-way ANOVA and Tukey's multiple comparisons. Cytotoxicity against keratinocytes was evaluated using the Kruskal Wallis and Dunn's post-hoc (α = 5%) tests. Transbond showed lower DC as compared to 5% DMAHDM, 1% DMADDM, and 5% DMADDM (p < 0.05). However, all groups presented proper DC when compared to commercial adhesives in the literature. In the evaluation of cytotoxicity against keratinocytes, Transbond induced higher viability than 2.5 wt.% groups (p < 0.05). Against fibroblasts, Transbond induced higher viability as compared to 5 wt.% groups (p < 0.05). DMAHDM at 5 wt.% reduced biofilm formation when compared to all the other groups (p < 0.05). Despite their cytotoxic effect against keratinocytes, gingival fibroblasts showed higher viability. DMAHDM at 5 wt.% decreased Streptococcus mutans viability. The incorporation of DMAHDM at 5 wt.% may be a strategy for reducing the development of white spot lesions.

Por conta das irregularidades da superfície vestibular, dificuldade de colagem em algumas regiões, como também mais facilidade na confecção de arco passivo para cirurgia ortognática de benefício antecipado, a técnica da colagem indireta vem ganhando espaço. Assim, o objetivo do trabalho é relatar um caso clínico de um paciente com 27 anos de idade sem comorbidade sistêmica e diagnosticado com ameloblastoma na mandíbula, na qual foi realizada uma colagem indireta para possibilitar a confecção do arco cirúrgico de forma passiva e, com isso, a cirurgia imediata. Por meio da técnica de colagem indireta, foi feita a colagem dos bráquetes no modelo de gesso e, após a colagem, foi confeccionado o arco passivo superior e inferior. Com os guias de colagem indireta e os arcos prontos, realizou-se a colagem na boca do paciente, os arcos cirúrgicos foram instalados e o mesmo foi encaminhado para realização da cirurgia de remoção do ameloblastoma. Erros no posicionamento dos bráquetes irão interferir no resultado final, aumentando, assim, o tempo de trabalho. A técnica da colagem indireta tem se demonstrado um avanço para a Ortodontia, otimizando o tempo de atendimento clínico como também o tempo cirúrgico nos casos de benefício antecipado. (AU)


Due to the irregularities of the vestibular surface, difficulty in gluing in some regions, as well as ease in creating a passive arch for orthognathic surgery with anticipated benefits, the indirect bonding technique has been gaining ground. Thus, the objective of the work is to report a clinical case of a 27-year-old patient without systemic comorbidity and diagnosed with ameloblastoma in the mandible, to whom indirect bonding was performed to enable the creation of the surgical arch in a passive way and, with this, the immediate surgery. Using the indirect bonding technique, the brackets were bonded to the plaster model and, after bonding, the upper and lower passive arch was created. With the indirect bonding guides and the archwires ready, the bonding was done in the patient's mouth, the surgical archwires were installed, and the patient was sent to undergo surgery to remove the ameloblastoma. Errors in the positioning of the brackets will interfere with the final result, thus increasing working time. The indirect bonding technique has proven to be an advance for Orthodontics, optimizing clinical care time as well as surgical time in cases of anticipated benefit (AU)

PURPOSE: This study aimed to analyze the effects of core materials, remaining tooth structures, and interfacial bonding on stress distribution in endodontically treated teeth using finite element analysis (FEA). MATERIALS AND METHODS: Three-dimensional FEA was conducted using a reverse engineering technique based on maxillary premolars scanned by micro-computed tomography. Six models were generated with or without ferrules and with one of the following three abutment systems: metal core, resin core, or resin core with fiber posts. In each model, bonding and debonding were assumed in the dentin and surrounding structures: bonded and debonded models. The maximum principal stress values were recorded, and stress distribution of the entire restored teeth and dentin was generated. Furthermore, the distribution of the displacement vector of the debonded models was generated. RESULTS: In comparing the bonded and debonded models, the debonded models showed larger values for tensile stresses than those in bonded models for all abutment models. The models without ferrules rotated around the center of the abutment, whereas those with ferrules did not show remarkable displacement in the analysis. CONCLUSION: FEA assuming fracture of adhesive interface proved to be an effective method to clarify the significance of ferrules. It prevents stress concentration in dentin by reducing the rotation of the abutment, even when the adhesive fails.

ABSTRACT

Biofilm accumulation, the appearance of white spot lesions and the development of secondary caries are the main complications in orthodontic patients. A promising approach to fight this situation is the development of adhesive cements with improved antibacterial properties. The aim of the present study was to evaluate the possibility of improving the antibacterial properties of glass ionomer cements by incorporating different types of antimicrobial compounds without altering their physical and mechanical properties. Different concentrations of silver carbonate (SC) and an inorganic glass with encapsulated silver were added to the glass ionomer cement, as well as chitosan, to achieve synergistic antibacterial activity. Variations in the antibacterial capacity were evaluated using the agar diffusion test; the potential alteration of the physical and mechanical properties of the material was analyzed by the shear bond strength test. SEM characterization and colorimetric evaluation were also conducted. Samples of SC up to 1% and inorganic glass with encapsulated silver up to 5% showed significant improvement in their antibacterial ability without compromising shear strength. The highest antimicrobial activity was observed for Lactobacillus acidophilus, with inhibition zones of 3.8 and 3.1 mm for SC and inorganic glass, respectively. The characterization of the samples did not detect any major structural changes between the different samples. The only group that underwent a noticeable color change was the group with SC. The results show that the incorporation of silver carbonate and inorganic glass with encapsulated silver provided the glass ionomer cement with an antibacterial capacity without compromising the bond strength and without modifying the structure of the material.
