ABSTRACT

With priority given to various-sized samples of market-available Hilsa (Tenualosa ilisha), human health consequences of trace metals along with total essential acids, including the fatty acid (FA) and amino acid (AA) profile were measured and compared to different size groups (G I, G II, and G III) using chemometric approaches. Essential amino acids were lower than nonessential amino acids. The G III contained the highest (97.55%) saturated and unsaturated fatty acids. The highest concentrated metal was found in G1 among the groups and the order of metal (mg/kg) was Zn (205.01) > Mn (37.37) > Fe (69.39) > Cu (1.47) > Cr (1.31) > Ni (0.42) > Pb (0.017) > Cd (0.005). Even though the adult group showed no health hazards for Hilsa consumption, non-carcinogenic risks have been identified for G1 fish consumption by children. Continued monitoring is recommended to overcome the health consequences caused by fish consumption.

ABSTRACT

BACKGROUND: Bad breath (halitosis) is a common problem affecting psycho-social wellbeing of young people. We aimed to explore the extent of self-perceived halitosis and associated factors among university students in Dhaka, Bangladesh. METHODS: A cross-sectional study was conducted among university students from November 2021 to April 2022. Six private and two public universities were approached. A total of 318 participants were conveniently selected for the study. A self-administered questionnaire was used for data collection. Students unwilling to participate were excluded. Multivariable logistic regression analysis was used to examine factors associated with halitosis. Statistical analysis was conducted using Stata Version 17. RESULTS: A total of 55.97% of students had self-perceived halitosis, with females (74.53%) having a significantly higher proportion than males (36.94%) (p < 0.001). A significantly higher proportion of halitosis was found among participants who were overweight ( 61%), had obesity (60.77%), smoked cigarette (46.79%), consumed alcohol (71.43%), lacked exercise (66.29%), were on unhealthy diet (57.35%), consumed coffee/tea (61.35%), breathed through mouth (64.60%), brushed tooth infrequently (85.71%), changed toothbrush after 6 months (77.42%), did not use toothpaste (94.74%), did not use/ sometimes used fluoride toothpaste (75.76%), lacked dental floss use (60.85%), did not use toothpick (62.87%), did chew or sometimes chewed sugar-free chewing gum (75.82%), did not clear / cleaned tongue sometimes (76.14%), did use mouth freshener regularly or occasionally (64.97%), did not use or used mouthwash sometimes (58.87%) were also associated with higher self-perceived halitosis (p < 0.05 for all). Students with gum bleeding, swollen gums, dry mouth, dental caries, food accumulation, and tooth sensitivity had a significantly (p < 0.05 for all) higher proportion of self-perceived halitosis (76.85%, 81.82%, 72.50%, 67.78%, 64.13% and 67.40%, respectively) compared to those without this problem. Being female (OR = 5.04; 95% CI: 2.01-12.62; p < 0.001), alcohol consumers (OR 7.35; 95% CI: 1.77-30.50; p = 0.006); not using sugar free chewing gum (OR = 0.25; 95% CI: 0.10-0.58; p = 0.001), lack of tongue cleaning (OR 4.62; 95% CI: 2.16-9.84; p < 0.001), and gum bleeding (OR = 7.43; 95% CI: 3.00-18.35; p < 0.001) were independently associated with halitosis on multivariable regression. CONCLUSION: This study reveals a high proportion of self-perceived halitosis and relevant factors. There should be more public education on the causes of halitosis and potential management approaches.

ABSTRACT

Human welfare and biodiversity are at risk due to the deterioration of water and sediment quality. Particularly, in last few decades, global water and sediment quality degraded due to the rapid industrialization and urbanization. This study aimed to determine the concentration of nine heavy metals and metalloid (Pb, Cr, Cd, Hg, As, Mn, Ni, Cu, and Zn) and assess the ecological risks using different pollution indices (e.g., heavy metal pollution index [HPI], Nemerow pollution index [NI], geo-accumulation index [Igeo], contamination factor [CF], degree of contamination [CD] and pollution load index [PLI], ecological risk index [ERI]) in water and sediment of the Shitalakshya River, an industrially affected urban river of Bangladesh. For the first time, 20 water and sediment samples were collected across a wider geographical area of the Shitalakshya River during both monsoon and dry seasons and analyzed using the atomic absorption spectrometer. Average concentrations of heavy metals and metalloid in water were within the Bangladesh standard except for Cr (51.69 ppb) and Mn (228.20 ppb) during monsoon season, portraying potential ecological and human health risks. Besides, average concentration of Mn (549.75 and 370.93 ppb), Ni (549.75 and 370.93 ppb), and Cu (45.34 and 36.09 ppb) in sediment during both seasons were above international standard, implying risk to aquatic sediment biota. The average HPI values indicated moderate to high contamination, whereas the NI values implied polluted water in monsoon season with severe pollution in port area of the river. Similarly, Igeo, CF, CD, and PLI elucidated different levels of contamination in the sediment, particularly during dry season. The ERI values also referred moderate ecological risk in the sediment during dry season. Overall, our findings highlight the alarming level of heavy metal pollution in the Shitalakshya River, necessitating immediate action to protect the aquatic environment, sediment biota, and human health. PRACTITIONER POINTS: This study determined the concentration of heavy metals and metalloid in water and sediment of the Shitalakshya River, Bangladesh. The study revealed that the average concentration of Cr and Mn in water exceeded national standard, whereas Mn, Ni, and Cu in sediment exceeded international limit. Potential ecological risk of heavy metals was also assessed using different pollution indices. Calculated pollution indices indicated different degree of pollution, implying critical ecological condition due to heavy metal pollution in aquatic environment and sediment biota.

ABSTRACT

The shifting of tannery industries from Hazaribagh to Savar adjacent to Dhaleshwari River might have inevitable adverse impacts, especially the heavy metal contamination of both terrestrial and aquatic ecosystems. The study was carried out to investigate the concentrations of four heavy metals in the soil, water, and plant samples collected from around Dhaleshwari River adjacent to the Bangladesh Small & Cottage Industries Corporation Tannery Industrial Estate, Dhaka. This study revealed that average concentrations of cadmium and chromium in soil exceeded the maximum permissible limit of World Health Organization (1996) and average concentrations of lead, cadmium, and chromium in water exceeded the maximum permissible limit of World Health Organization (2011) and Environmental Conservation Rules (1997). The average concentrations of lead, cadmium, and chromium in the roots and shoots of both Eichhornia crassipes and Cynodon dactylon exceeded the maximum permissible limit of Food and Agriculture Organization/ World Health Organization (2016). Ecological risk assessment using indices model for soil pollution indicated that soil is mostly contaminated with cadmium and chromium which can pose strong ecological risk Health risk assessment using indices model for water pollution revealed the high degree of contamination and unacceptable non-carcinogenic risk and carcinogenic risk for adults as well as children through ingestion of water. Average bioconcentration factor and bioaccumulation factor were higher in Eichhornia crassipes than Cynodon dactylon for lead, cadmium, and chromium. Average translocation factor was also higher in Eichhornia crassipes for all the metals except cadmium. It is suggested remedial and mitigation measures be instituted to control environmental degradation of the newly established Tannery Industrial Estate.

ABSTRACT

The purpose of this research work is to evaluate the degree of eight heavy metals (Fe, Mn, Cu, Zn, Cd, Pb, Cr, and Ni) contamination and health risks of three regularly consumed vegetables (papaya, bottle gourd, and esculent) near one of Bangladesh's busiest roadways, the Dhaka-Mymensingh highway. The heavy metal concentrations in 45 vegetable samples were analyzed using an atomic absorption spectrometer (AAS). These samples were collected from five different sampling sites based on various land use patterns adjacent to the highway. The average concentrations (mg/kg) of Cu, Fe, Mn, Zn, Cr, and Ni were found to be 9.86, 246.8, 16.9, 28.0, 9.02, and 2.02, respectively, for papaya; 14.9, 281.2, 387.6, 49.0, 10.1, and 2.92, respectively, for bottle gourd; and 17.6, 183.4, 107.2, 80.7, 7.98, and 2.34, respectively, for esculent. The mean concentrations of Cr, Zn, and Mn in papaya, bottle gourd, and esculent were higher than the acceptable limit. Correlation analysis revealed a significant positive correlation between Fe-Cu, Zn-Fe, Cu-Fe, and Fe-Zn in papaya; Cu-Zn, Fe-Cr, Zn-Ni, and Cr-Fe in bottle gourd; and Mn-Cr, Mn-Ni, Mn-Fe, and Cr-Ni in esculent, thereby indicating their common anthropogenic sources like agricultural activities, waste from the commercial area, filling station, and vehicular emissions. Health risk assessment through target hazard quotient (THQ) revealed the highest THQ of 9.52 for Cr in bottle gourd, which poses a high non-carcinogenic health risk to the localities upon the intake of these contaminated vegetables. Target cancer risk (TCR) was found to be highest for Cr in papaya (0.013) and bottle gourd (0.014). TCR trends were found for Cr in the following order: bottle gourd > esculent > papaya. This study contributed the greatest concern for both carcinogenic and non-carcinogenic health impacts through ingesting contaminated vegetables.

ABSTRACT

In this study, activity concentrations of natural radionuclides in 28 raw milk samples collected from different dairy farms in Dhaka city of Bangladesh were measured using a high-purity germanium (HPGe) detector for the first time. The activity concentration of 226Ra, 232Th, and 40K in the investigated fresh milk samples ranged from BDL (Below detection level) to 26 ± 1.6 Bq/kg, BDL to 11.7 ± 3.3 Bq/kg and 101 ± 17 to 384 ± 32 Bq/kg, respectively. No artificial radionuclides were found in the investigated samples. Present results show inline within the range of available data in the literature. Annual committed effective doses were estimated following the consumption characteristics of raw milk by city population, values are found within the limiting range recommended by international organizations due to consumption of foodstuffs. Additionally, real-time gamma-ray dose rate in the farms/sampling locations was found in the range of 0.12 ± 0.01-0.20 ± 0.01 µSv/h by using a digital gamma survey meter (Gamma Scout) and the calculated maximum annual effective dose due to outdoor absorbed dose was found to be 0.25 mSv/y, which shows lower than the United Nations Scientific Committee on the Effects of Atomic Radiation (UNSCEAR) recommended limit of 2.4 mSv/y. This study indicates that the concentration of radionuclides in the farm fresh milk of Dhaka city does not pose any unwanted risk to public health, and it is safe to consume by both children and adults with the current intake level.

ABSTRACT

This cross-sectional study examined if social capital affects women's health attributions. The study used the Internet Social Capital Scale (ISCS) and Multidimensional Health Locus of Control (MHLC) Scale to measure Social Capital and Health Locus of Control. A predefined 38-item questionnaire was used to survey 485 purposively selected women. A bidirectional reciprocal structural equation model was used to measure the covariance between Social Capital and Health Locus of Control. We hypothesized that women with strong social capital, particularly those rich in bridging ties, would exhibit a greater sense of agency and empowerment over their health, attributing their health outcomes less to internal factors like fate and more to external influences like powerful others and broader social support. However, we found that when women have higher social capital, their external health locus of control increases. Bridging and bonding social capital lower women's internal health control, but bridging social capital leads to higher attributes to powerful others. Likewise, we expected women with more social capital would exhibit a lower perception of uncontrollability over their health, but is not the case. The findings underscore the necessity for women to have more social capital.

ABSTRACT

Background: Dengue is a major public health concern in Bangladesh. This study aimed to assess the perceptions and practices of community members in Dhaka regarding community engagement and social participation for dengue prevention. Methods: A cross-sectional online survey was conducted in Dhaka City from May 2022 to December 2022. The respondents were randomly selected. The association between community participation and prevention practices was tested using the χ 2 test. Results: The findings of this study indicate that the majority of participants (92%) believed that community effort would be relied upon in the event of a dengue outbreak. Environmental cleaning campaigns were the preferred approach, and religious leaders viewed them positively. This study also revealed significant variations in knowledge levels, with those involved in community efforts and mass gatherings demonstrating greater knowledge. This study sheds light on the demographic factors that influence dengue knowledge and provides valuable insights into the development of targeted public health interventions. Conclusion: The study revealed negative perceptions and limited participation in dengue prevention among participants, with the majority demonstrating a poor understanding of preventive measures. While some showed positive attitudes towards community engagement, significant disparities existed in participation, highlighting the need for targeted educational campaigns and enhanced community mobilization efforts. Moreover, the importance of multisectoral collaboration is emphasized, underscoring the need for coordinated efforts among health departments, NGOs, religious institutions, and community leaders to effectively combat dengue transmission. Recommendations include ongoing educational initiatives, targeted interventions to promote community involvement, and fostering collaboration across sectors to strengthen dengue prevention efforts and to safeguard public health.

The presence of antibiotic-resistant pathogens in food is a serious public health concern nowadays. This study focuses on the isolation and characterization of potentially pathogenic Escherichia coli and antimicrobial-resistant pathogens in chicken curry and potato smash samples collected from the canteens and cafeteria of Dhaka University in Bangladesh. Isolates were identified by their cultural, morphological, and biochemical tests (motility indole urease test, Kliger's iron agar test, catalase test, oxidase test, methyl red and Voges-Proskauer tests). The antibiotic susceptibility test was done by the disk diffusion method. The range of total bacterial count in the potato smash and chicken curry samples was from 1.4 × 104 to 1.6 × 108 CFU/g and from 2.4 × 103 to 2.6 × 106 CFU/g, respectively. Escherichia coli, Salmonella, Vibrio, Klebsiella, Citrobacter, Enterobacter, Proteus, Clostridium, Staphylococcus, Streptococcus, Micrococcus, Bacillus, and Sarcina strains were isolated in both samples. Isolates were highly resistant to ampicillin (90.90%) followed by colistin (52.27%), azithromycin (27.27%), and tetracycline 25%. Proteus species had the highest rate of multiple antibiotic resistance (MAR; 62.5%), followed by Citrobacter species (50%). The isolated E. coli strains were further analyzed through PCR assay to detect virulent genes (EPEC: eaeA 229 bp, bfpA 450 bp, ETEC elt 322 bp, EHEC hylA 534 bp, and EIEC ial 320 bp). One E. coli isolate had the eaeA target gene under EPEC pathotypes. Escherichia coli, as a fecal indicator, may indicate fecal contamination or poor and unhygienic food handling. The findings recommend further investigations to identify potential mechanisms of contamination and preventive measures to improve the food safety level in the canteens and restaurants.

ABSTRACT

Background: Coronavirus disease 2019 (COVID-19) is a global public health concern. Evidence shows that depression and anxiety are common among patients with COVID-19 after recovery. About one-third of the total COVID-19 cases in Bangladesh have been reported in Dhaka city. Therefore, the study aimed to evaluate the prevalence of depression and anxiety among COVID-19 survivors in Dhaka city as well as to identify the factors associated with these mental health conditions. Methods: A cross-sectional study was carried out among a total of 384 COVID-19 survivors aged 18 years or older. Data collection was done through face-to-face and telephone interviews using a semi-structured questionnaire. Patient Health Questionnaire (PHQ-9) and Generalized Anxiety Disorder (GAD-7) scales were used to assess depression and anxiety, respectively. Binary logistic regression analysis was performed to identify the predictors of depression and anxiety among patients recovered from COVID-19. Results: The overall prevalence of depression and anxiety was 26.0% and 23.2%, respectively among COVID-19 survivors. The respondents who were ≥60 years were 2.62 and 3.02 times more likely to report depressive and anxiety symptoms, respectively than those aged 18 to 39 years. Hospitalised patients recovered from COVID-19 had a 2.18 times higher chance of developing anxiety than their non-hospitalised counterparts. COVID-19 recovered patients with comorbidities were at 3.35 and 2.97 times higher risk of depression and anxiety, respectively compared to those without comorbidities. Similarly, the respondents who had already passed a period of 15 days to 3 months after recovery showed 3.06 and 1.85 times higher odds of depression and anxiety, respectively than those who had already passed a period of above 3 to 6 months after recovery. Conclusion: The study reported a high prevalence of depression and anxiety among COVID-19 survivors living in Dhaka city. The findings suggest the need for appropriate interventions to reduce mental health complications in COVID-19 survivors.

Bangladesh is currently experiencing significant infrastructural development in road networking system through the construction or reconstruction of multiple roads and highways. Consequently, there is a rise in traffic intensity on roads and highways, along with a significant contamination of adjacent agricultural soils with heavy metals. The purpose of this study was to evaluate the ecological risk, health risk and the abundance of seven heavy metals (Cu, Mn, Pb, Cd, Cr, As, and Ni) in three distance gradients (0, 300, and 500 m) of agricultural soil along the Dhaka-Chattogram highway. The concentration of heavy metals was measured with an Atomic Absorption Spectrophotometer (AAS) on a total of 36 soil samples that were taken from 12 different sampling sites. Based on the findings, Cd had a high contamination factor for all distance gradients, whereas Cr had a moderate contamination factor in 67% of the study areas. According to the Pollution Load Index (PLI), Cd, Cr, and Pb were the predominant pollutants. Principal component analysis (PCA) result shows these metals mainly came from anthropogenic sources. The considerable positive correlations between Cu-Pb, Cu-Cd, Pb-Cd, and Cr-Ni all pointed to shared anthropogenic origins. As per Potential Ecological Risk Assessment (PERI) analysis, Pb, Cd, Cr, and Ni each contribute significantly and pose a moderate threat. The Target Hazard Quotient (THQ) values for all pathways of exposure to Pb and Cr in soils were more than 1, which would pose a significant risk to human health in the following order: THQadult female > THQadult male > THQchildren. This study will help to evaluate the human health risk and develop a better understanding of the heavy metal abundance scenario in the agricultural fields adjacent to this highway.

ABSTRACT

Terrestrial gamma radiation is one of the major outdoor radiation exposures to the general public that varies substantially based on the type and geological properties of the soil. The objective of this study is to evaluate the naturally occurring radioactive materials (NORMs) distribution and assess the hazard parameters in the riverbank soil within various industrial zones in the densely populated Dhaka and Chattogram cities of Bangladesh. The mean activities of 226Ra (37 ± 3), 232Th (58 ± 4), and 40K (1129 ± 18) Bqkg-1 in the assessed soil samples were found to be slightly higher than the world average values 32, 35, and 420 Bqkg-1, respectively. The mean radium equivalent activity (207.49 Bqkg-1) and the external and internal hazard indices were within the recommended limits of 370 Bqkg-1 and <1, respectively. The mean absorbed dose rate (99.47 nGyhr-1), annual effective dose (0.12 mSva-1), ELCR (4.27 × 10-4), and gamma level index (1.58) exceeded the world average values 59 nGyhr-1, 0.07 mSva-1, 2.9 × 10-4, and 1 respectively. However, the studied areas are safe from a radiological viewpoint with no radiation health hazard to the people. The results of this study can be utilized to produce factual baseline data for future studies.

ABSTRACT

Naturally occurring radioactive materials (NORMs: 232Th, 226Ra, 40K) can reach our respiratory system by breathing of road dust which can cause severe health risks. Targeting the pioneering consideration of health risks from the NORMs in road dust, this work reveals the radioactivity abundances of NORMs in road dust from a megacity (Dhaka) of a developing country (Bangladesh). Bulk chemical compositions of U, Th, and K obtained from neutron activation analysis were converted to the equivalent radioactivities. Radioactivity concentrations of 226Ra, 232Th, and 40K in road dust ranged from 60-106, 110-159, and 488-709 Bq kg-1 with an average of 84.4 ± 13.1, 126 ± 11, and 549 ± 48 Bq kg-1, respectively. Estimated 226Ra, 232Th, and 40K radioactivities were, respectively, 1.7-3.0-, 3.7-5.3-, and 1.2-1.8-folds greater than the affiliated world average values. Mechanistic pathway of NORMs' enrichment and fractionation relative to the major origin (pedosphere) were evaluated concerning the water logging, relative solubility-controlled leaching and translocation, climatic conditions, and aerodynamic fractionations (dry and wet atmospheric depositions). Computation of customary radiological risk indices invokes health risks. Noticing the ingress of NOMR-holding dust into the human respiratory system along with the associated ionizing radiations, the computed radiological indices represent only the least probable health hazards. Nevertheless, in real situations, α-particles from the radioactive decay products of 232Th and 238U can create acute radiation damages of respiratory system. Policymakers should emphasize on limiting the dust particle evolution, and public awareness is required to alleviate the health risks.

Here, we present the full-genome sequence of a pandrug-resistant V. cholerae strain KBR06 isolated from a cholera patient in Bangladesh that exhibited intermediate resistance to only two antibiotics out of 39 among 14 antibiotic categories.

Background: Dhaka City, the capital of Bangladesh, has experienced rapid and unplanned urbanization over the past few decades. This process has brought significant challenges to public health as the urban environment has become a breeding ground for various health risks. Understanding the associations between unplanned urbanization, the urban environment, and public health in Dhaka City is crucial for developing effective interventions and policies. Objectives: This review paper aims to uncover the associations between unplanned urbanization and health risks in Dhaka City, with a specific focus on the urban environment and its impact on public health. The objectives of this study are to examine the health challenges faced by the city's population, explore the specific urban environmental factors contributing to health risks, analyze the socioeconomic determinants of health in unplanned urban areas, evaluate existing policies and governance structures, identify research and data gaps, and provide recommendations for future interventions. Methods: A comprehensive literature review was conducted to gather relevant studies, articles, reports, and policy documents related to unplanned urbanization, the urban environment, and public health in Dhaka City. Various databases and online resources were searched, and the selected literature was critically analyzed to extract key findings and insights. Results: The findings reveal that unplanned urbanization in Dhaka City has led to a range of public health risks, including air pollution, inadequate water and sanitation, poor waste management, overcrowding, slums, and substandard housing conditions. These environmental factors are strongly associated with respiratory diseases, waterborne illnesses, and other adverse health outcomes. Socioeconomic determinants such as poverty, income inequality, and limited access to healthcare further exacerbate the health risks faced by the urban population. Conclusion: Unplanned urbanization in Dhaka City has significant implications for public health. Addressing the associations between unplanned urbanization, the urban environment, and public health requires comprehensive policies and interventions. Improved urban planning, enhanced infrastructure, and better policy governance are essential for mitigating health risks. Furthermore, addressing socioeconomic disparities and ensuring equitable access to healthcare services are crucial components of effective interventions.

ABSTRACT

The incidence of nontuberculous mycobacterial (NTM) infections after operations is increasing in Bangladesh but data regarding clinical presentation, diagnosis, treatment, and prognosis after treatment are lacking. In this case series, three patients having persistent serous discharge from incision wound after operation were studied. Discharge from wounds were collected, wet film microscopy was performed for pus cells and fungus, Gram stain, Ziehl-Neelsen (ZN) stain, culture in routine culture media and Lowenstein-Jensen (LJ) media, Xene-Xpert for mycobacterium tuberculosis (MTB), polymerase chain reaction (PCR) for NTM were done. NTM-positive patients were treated initially for 6 weeks with four drugs regimen (clarithromycin 500 mg 12 hourly, ciprofloxacin 500 mg 12 hourly, linezolid 400 mg 12 hourly, and amikacin 500 mg 12 hourly), followed by 5 months with three drugs regimen (clarithromycin 500 mg 12 hourly, ciprofloxacin 500 mg 12 hourly, and linezolid 400 mg 12 hourly) as a maintenance dose. Cessation of discharge occurred within 3-4 weeks after starting treatment, and the wounds were healed.

ABSTRACT

The current study seeks to explore the vulnerabilities typically encountered by female migrants in the informal sector in Dhaka city. It used a qualitative research approach, purposively selecting four areas from Dhaka city. Twenty-five semi-structured in-depth interviews during eight months of participant observation were conducted to accomplish the study objective. The authors used the capability approach theory to elucidate the phenomenon of vulnerability experienced by female migrants in the informal sector. The thematic data analyses were performed using the Granheim approach and NVivo 12 software. The findings of this study suggest that migrant women who work on the street face a number of significant challenges. These challenges include sexual assault and harassment, social stigma and cultural barriers, financial obstacles, and extortion and bribery. The authors recommend that governments provide access to state credits, social security, health insurance, and other forms of social protection for informal sector workers. The finding revealed that informal workers are often excluded from these essential benefits, making it difficult for such workers to expand their businesses or have a safety net in case of illness, unemployment, or other shocks.

ABSTRACT

Dhaka, which is the capital and largest metropolis of Bangladesh, has seen an increase in the number of documented cases of respiratory disorders. Every day in Dhaka city, a remarkable number of patients are being diagnosed with poor respiratory conditions. The majority of these patients have no other severe disease history and mostly need to be exposed to outdoor air to meet their occupational requirements, indicating that the ailment may be associated with polluted air. As this is the most pressing issue that must be addressed in order to safeguard public health, we have made an effort to focus on the current situation surrounding the sources of air pollution in the city. Since this is a viewpoint article, we gathered data from various published articles, national dailies, and international reports generated by WHO, CDC, BBC, or other environmental news/report portals to highlight the public health issue related to respiratory health. Poor respiratory health is one of the main consequences of Dhaka's contaminated air, as determined by our analysis.

Objectives: In 2022, Bangladesh had the highest dengue-related fatality (281). This study evaluated clinical profiles to detect early changes to predict dengue fever severity. Methods: This prospective observational study was performed in four government hospitals from June to November 2022 in Dhaka. Febrile patients admitted within 4th day of illness were recruited if they had a confirmed dengue viral infection either by by positive dengue nonstructural protein antigen or anti-dengue immunoglobulin (Ig)M antibody. Results: We divided 308 patients with confirmed dengue into two groups: 232 (74.3%) in nonsevere dengue and 76 (24.7%) in severe dengue. Men were 205 (66.6%), and the most affected age group was 21-30 years (47.7%). Patients with severe dengue reported a higher number of nausea 80.3%, coughs 57.9%, abdominal pain 56.6%, persistent vomitting 53.9%, dyspnea 35.5%, diarrhea 28.9%, and skin rash at 27.6%. In addition, the disease's febrile phase (≤4 days) showed thrombocytopenia (odds ratio [OR] 6.409, 95% CI 2.855-14.386, p <0.001), hemoconcentration (OR 3.428, 95% CI 1.030-11.405, p 0.045), and hypotension (OR 5.896, 95% CI 1.203-28.897, p 0.029) were associated severe disease. Conclusions: Hypotension, thrombocytopenia, and hemoconcentration during the febrile phase might indicate progression towards severe disease.

We analyze hourly PM2.5 (particles with an aerodynamic diameter of ≤ 2.5 µm) concentrations measured at the U.S. Embassy in Dhaka over the 2016 - 2021 time period and find that concentrations are seasonally dependent with the highest occurring in winter and the lowest in monsoon seasons. Mean winter PM2.5 concentrations reached ~165-175 µg/m3 while monsoon concentrations remained ~30-35 µg/m3. Annual mean PM2.5 concentration reached ~5-6 times greater than the Bangladesh annual PM2.5 standard of 15 µg/m3. The number of days exceeding the daily PM2.5 standard of 65 µg/m3 in a year approached nearly 50%. Daily-mean PM2.5 concentrations remained elevated (>65 µg/m3) for more than 80 consecutive days. Night-time concentrations were greater than daytime concentrations. The comparison of results obtained from the Community Multiscale Air Quality (CMAQ) model simulations over the Northern Hemisphere using 108-km horizontal grids with observed data suggests that the model can reproduce the seasonal variation of observed data but underpredicts observed PM2.5 in winter months with a normalized mean bias of 13-32%. In the model, organic aerosol is the largest component of PM2.5, of which secondary organic aerosol plays a dominant role. Transboundary pollution has a large impact on the PM2.5 concentration in Dhaka, with an annual mean contribution of ~40 µg/m3.
