ABSTRACT

Background: The COVID-19 pandemic has created additional challenges for family health. Worry, fear, and anxiety associated with this disease can affect the perception of family health. The study's objective was to analyze the factors associated with health vulnerability of family members in the Peruvian population after pandemic. Methods: Observational, cross-sectional, and analytical study. Sampling was nonprobabilistic. The sample consisted of 519 residents who met the following inclusion criteria: Peruvian resident, of both genders, over 18 years of age, who lives with their family, and who agrees to participate in the study. For data collection, the "SALUFAM" and "PRE-COVID-19" scales were used, which measure the health vulnerability of family members and concern about the contagion of COVID-19, respectively. Data collection was done between January and March, 2023. Results: Living in the Coast region increases by 3.299 times (95% CI = 1.55-9.28; p = 0.003) the probability of lower family health vulnerability compared to residents from the Jungle region. In the same way, having a low concern about the contagion of COVID-19 increases 2.77 times (95% CI = 1.02-7.53; p = 0.044) the probability of less vulnerability to family health, unlike participants who are highly concerned about the contagion of COVID-19. Conclusions: It should be necessary to design prevention and family health promotion strategies according to the geographical region; it is also essential to provide education on the risks and the importance of prevention measures for COVID-19, regardless of their initial level of concern.

ABSTRACT

An increase in incidence and amplitude of extreme weather events (EWEs) linked to climate change, has resulted in greater human exposure and vulnerability to weather-related health effects. Increases in the occurrence of EWEs, including storms, flooding, extreme heat and wildfires, will impact health globally, with poor and vulnerable populations disproportionately affected. Vulnerability to EWEs, and the ability to adapt to these weather shocks, are influenced by existing physical, social and political limitations of a given region. As such, developing context-specific health vulnerability indices to inform planning and decision-making for policy makers and citizens alike, should be prioritized. The existence and development of health vulnerability indices in South Africa are limited, therefore, this study provides a foundation from which future indices can build. Mixed methods approaches including evidence and data analysis/synthesis and focus groups are used to understand the interconnections between extreme weather events and human health, including citizens' understanding of emergent vulnerabilities linked to these events. The methods employed in this study include: •A rapid evidence review (RER) including data extraction identifying health impacts and indicators.•Development of a draft health vulnerability index (HVI) framework.•Focus groups and individual interviews testing the draft HVI for citizen input and framework refinement.

ABSTRACT

El pueblo gitano presenta desigualdades sociales que generan peores resultados en salud que el conjunto de la población española. Se describe el proceso de creación del Servicio de Mediación de Salud Pública con la Comunidad Gitana, pionero en la Consejería de Sanidad de la Comunidad de Madrid, mostrando sus competencias dirigidas a disminuir dichas desigualdades. Los resultados de su trabajo, enmarcado en la praxis de la mediación intercultural y la intervención comunitaria, validan la necesidad de contar con profesionales de la mediación intercultural en salud pública. The Roma people have social inequalities that lead to worse health outcomes than the Spanish population as a whole. We report the process to create the Public Health Mediation Service with the Roma Community, pioneer in the Community of Madrid Health Department. We also reveal its competences aimed at reducing said inequalities. The results of its work, framed in the practice of intercultural mediation and community intervention, validate the need for intercultural mediation professionals in the field of public health. Resumo Objetivo Analisar as evidências de validade de conteúdo e processo de resposta de um banco de itens sobre vulnerabilidade em saúde. Métodos Estudo psicométrico, subsidiado pelo polo teórico de Pasquali. Foram selecionados 62 especialistas com características sociodemográficas e acadêmicas distintas. Desses, 15 responderam ao questionário de validade de conteúdo. Após os ajustes, o banco passou para fase de validade de processo de resposta e foi avaliado por 12 usuários dos serviços de saúde. Utilizou-se coeficiente de validade de conteúdo para avaliação dos itens e do banco, cuja confiabilidade foi verificada pelo teste binomial, considerando percentual de 80% e p-valor >0,05. Resultados O banco apresentou boa evidência de validade de conteúdo pelos especialistas, cujos coeficientes foram 0,87 para o elemento sujeito, 0,86 para o elemento social e 0,865 para o geral. O público-alvo realizou sugestões na fase de validade de processo de resposta para melhor compreensão dos itens (coeficiente total: 0,89), sendo considerado, claro, preciso e compreensível. Conclusão A versão final do banco de itens possui 535 itens, sendo 238 para o sujeito e 297 para o social. Apresenta indícios de evidência de validade de conteúdo e processo de resposta e está apto para testagem de validade de estrutura interna.

Entre ellos, 15 respondieron al cuestionario de validez de contenido. Después de las adaptaciones, el banco pasó a la fase de validez del proceso de respuesta y fue evaluado por 12 usuarios de los servicios de salud. Se utilizó el coeficiente de validez de contenido para evaluar los ítems y el banco, cuya fiabilidad ser verificó mediante la prueba binominal, que consideró un porcentaje de 80 y p-valor >0,05. Resultados El banco presentó buena evidencia de validez de contenido por los especialistas, cuyos coeficientes fueron 0,87 en el elemento sujeto, 0,86 en el elemento social y 0,865 en el general. El público destinatario realizó sugerencias en la fase de validez del proceso de respuesta para una mejor comprensión de los ítems (coeficiente total: 0,89) y fue considerado claro, preciso y comprensible. Conclusión La versión final del banco de ítems contiene 535 ítems, de los cuales 238 son para el sujeto y 297 para el social. Presenta indicios de evidencia de validez de contenido y proceso de respuesta y está apto para una prueba de validez de estructura interna.


Abstract Objective To analyze the evidence of content validity and response process of a health vulnerability item bank. Methods Psychometric study with support of Pasquali's theoretical pole in which 62 specialists with different sociodemographic and academic characteristics were selected and 15 of them responded to the content validity questionnaire. After adjustments, the bank moved to the response process validity phase and was evaluated by 12 health service users. A content validity coefficient was used to evaluate the items and the bank, which had the reliability assessed by the binomial test, considering a percentage of 80% and p-value >0.05. Results The bank presented good evidence of content validity by experts. The coefficients were 0.87 for the subject element, 0.86 for the social element and 0.865 for the general element. The target audience made suggestions in the response process validity phase to better understand the items (total coefficient: 0.89), which were considered clear, accurate and understandable. Conclusion The final version of the item bank has 535 items, 238 for the subject and 297 for the social element. It presents evidence of validity of content and response process and is suitable for testing the validity of internal structure.

Resumo O projeto da transposição do rio São Francisco é parte do conjunto de políticas de acesso à água no semiárido brasileiro e tem incidido sobre os processos de desterritorialização e vulnerabilização na região. O objetivo deste estudo foi analisar os processos de vulnerabilização em comunidades camponesas, decorrentes das obras da transposição do rio São Francisco. Trata-se de um estudo qualitativo, com aplicação de entrevistas semiestruturadas e realização de oficina com camponeses atingidos pela transposição no município de Sertânia (PE). O enfoque teórico-metodológico foi a matriz da reprodução social. Foram identificados diversos processos de vulnerabilização material e simbólica, além de agravos à saúde, como a destruição de reservatórios de água, poços artesianos, cacimbas e terras férteis, a desterritorialização, a incidência de doenças respiratórias agudas, decorrente das escavações para construção dos canais, e processos relativos à saúde mental, relacionando-se à reprodução social e ao modo de vida. A leitura sistêmica ancorada na autoconsciência e na conduta dos processos de vulnerabilização identificou uma ruptura do modo de vida tradicional e efeitos à saúde física e mental dos habitantes da região. Os efeitos negativos desse processo retroagem sobre as vulnerabilidades históricas nos territórios afetados, produzindo novas fragilidades nessas comunidades.


Abstract The project for the transposition of the São Francisco River is part of the set of policies for access to water in the Brazilian semi-arid region and has focused on the processes of deterritorialization and vulnerability in the region. The objective of this study was to analyze the processes of vulnerability in peasant communities, resulting from the transposition of the São Francisco River. This is a qualitative study, applying semi-structured interviews and carrying out a workshop with peasants affected by the transposition in the municipality of Sertânia (PE). The theoretical-methodological approach was the social reproduction matrix. Several processes of material and symbolic vulnerability, in addition to health problems, were identified, such as the destruction of water reservoirs, artesian wells, waterholes and fertile land, deterritorialization, the incidence of acute respiratory diseases, resulting from excavations for the construction of canals, and processes related to mental health, relating to social reproduction and to the way of life. The systemic reading anchored in self-awareness and in the conduct of vulnerability processes identified a break in the traditional way of life and effects on physical and mental health of the region inhabitants. The negative effects of this process retroact on historical vulnerabilities in the affected territories, producing new fragilities in these communities.

Resumo A saúde é um complexo de situações de condições de vida alinhadas a contextos e lugares no tempo. O processo de reparação a partir do entendimento ampliado da saúde é um conjunto de direitos, ações e medidas protagonizadas por diferentes sujeitos nas diversas dimensões da vida. Este artigo tem como objetivo analisar as percepções e ações de reparação direcionadas a camponeses que sofreram perdas materiais e simbólicas no decurso das obras da transposição do Rio São Francisco, em Sertânia, Pernambuco. Trata-se de um estudo de caso com abordagem qualitativa. Os sujeitos desta pesquisa foram camponeses maiores de 18 anos, sem especificação de sexo, residentes em três comunidades nas proximidades do canal Eixo Leste da transposição. Foram realizadas entrevistas semiestruturadas nas comunidades e uma oficina participativa. Observou-se que há uma insuficiência no reconhecimento dos direitos de reparação pelos habitantes e, consequentemente, uma não efetivação das ações desenvolvidas nesse aspecto. As ações de reparação focaram compensações monetárias para a perda de bens materiais, porém sendo muitas vezes subdimensionadas ou até mesmo ausentes. É imprescindível que a reparação seja pensada na perspectiva da complexidade do caso, considerando todas as dimensões da saúde e da vida.


Abstract Health is a complex of situations of living conditions aligned to contexts and places at a given time. The redress process based on the expanded understanding of health is a set of rights, actions, and measures carried out by different subjects in the different dimensions of life. This article aims to analyze the perceptions and redress actions directed to peasants who suffered material and symbolic losses during the transposition of the São Francisco River, in Sertânia, Pernambuco. This is a case study with a qualitative approach. The subjects of this research were peasants over 18 years of age, of without specifying gender, living in three peasant communities near the Eixo Leste of the transposition. Semi-structured interviews and a participatory workshop were carried out in the communities. An insufficiency in the recognition of reparation rights by the inhabitants and, consequently, a non-effectiveness of the actions developed to that end were observed. Remedial actions focused on monetary compensation from the loss of material goods, but were often undersized or even absent. It is essential that the repair be thought of from the perspective of the complexity of the case, considering all dimensions of health and life.

RESUMO Este artigo trata das adolescências, no plural, como construção histórico-social de um processo dinâmico, e do adolescente perante seus modos de vida e expressão em um contexto histórico-social e singular. Entende-se aqui que ações meramente prescritivas são insuficientes para responder às complexidades dos processos de adoecimento mental na sociedade contemporânea. Objetivou-se compreender o contexto das práticas de cuidado em saúde ao adolescente em vulnerabilidade, diante do uso de drogas, na dimensão das tensões dos campos de práticas do cuidado em saúde. Estudo descritivo exploratório de abordagem qualitativa, que realizou entrevistas com profissionais da área da saúde (Unidade Básica de Saúde e Centro de Atenção Psicossocial Infantojuvenil). Destacam-se as tensões no campo de práticas entre o modelo assistencial biomédico e de guerra às drogas versus o psicossocial/comunitário; redução de estigma; acesso; redução de danos e nos atendimentos individuais, bem como a importância de refletir e fomentar o processo de mudança da mentalidade diante do adoecimento mental, visando atualizar as formas de cuidar. Esse processo tem sido realizado conforme os princípios da Reforma Psiquiátrica Brasileira, com avanços e retrocessos que repercutem na atualização das práticas de cuidado em saúde mais alinhadas às propostas de ações psicossociais e redutoras de vulnerabilidade.


ABSTRACT This article deals with adolescences, in the plural, as a socio-historical construction of a dynamic process and with the experience of adolescents with their lifestyle and self-expression in a unique socio-historical context. It assumes that purely prescriptive measures are insufficient to address the complexity of mental illness processes in contemporary society. This study aims to understand the context of health care practices for vulnerable youth, particularly those involved in substance use, through the lens of tensions in health practice domains. This is an exploratory, descriptive study using a qualitative approach, with interviews conducted with health professionals (in Basic Health Unit and Psychosocial Care Center for Children and Adolescents). The study highlights the tensions within health practice between the biomedical model and the war on drugs and a community-based psychosocial approach; it underlines the need for reducing stigmatization, improving access, harm reduction, and individualized care. It also emphasizes the importance of considering and promoting a shift in mentality towards mental illness in order to develop care strategies. This process was carried out in line with the principles of the Brazilian Mental Health Reform, with advances and setbacks that affect the updating of health practices, which are more oriented towards proposals for psychosocial and vulnerability-reducing actions.

ABSTRACT

Abstract Objective: to analyze the prevalence of tuberculosis, coronavirus, chronic conditions and vulnerabilities among migrants and refugees in Brazil. Method: this is a cross-sectional study of the electronic survey type conducted with international migrants during the COVID-19 pandemic. Descriptive statistics was applied for the analysis, with calculation of position and dispersion measures. Regarding the categorical variables, relative and absolute frequencies were estimated. Results: the study participants were 553 migrants and refugees, verifying 3.07%, 7.2% and 27.3% prevalence of tuberculosis, COVID-19 and chronic conditions, respectively. Among the vulnerabilities, 32% reported unemployment, 37.6% moved to Brazil as a result of the social situation in their countries and 33.6% were living as refugees or sheltered people. Conclusion: tuberculosis, chronic diseases and COVID-19 presented higher prevalence values in migrants and refugees than in the general population. As this is a population group that still has significant difficulty accessing health services and social protection systems, based on diverse evidence, the study will subsidize public policies, Nursing care and the incorporation of new routines in the service.


Resumo Objetivo: analisar a prevalência de tuberculose, coronavírus, condições crônicas e vulnerabilidades entre migrantes e refugiados no Brasil. Método: trata-se de estudo transversal, do tipo inquérito eletrônico, realizado com migrantes internacionais durante a pandemia de COVID-19. Para a análise, aplicou-se estatística descritiva, com cálculo de medidas de posição e de dispersão. Quanto às variáveis categóricas, estimaram-se as frequências relativas e absolutas. Resultados: participaram do estudo 553 migrantes e refugiados, verificando-se prevalência de 3,07% de tuberculose, 7,2% de COVID-19 e 27,3% de condições crônicas. Entre as vulnerabilidades, 32% referiram desemprego, 37,6% mudaram para o Brasil em decorrência da situação social do seu país e 33,6% residiam em asilo e ou abrigo. Conclusão: a tuberculose, as doenças crônicas e a COVID-19 apresentaram maior prevalência em migrantes e refugiados que na população em geral. Por tratar-se de uma população ainda com grande dificuldade de acesso aos serviços de saúde e aos sistemas de proteção social, o estudo subsidiará, com base em evidências, as políticas públicas, o atendimento do enfermeiro e a incorporação de novas rotinas no serviço.

Resultados: participaron del estudio 553 inmigrantes y refugiados, la prevalencia de tuberculosis era del 3,07%, de COVID-19 del 7,2% y de condiciones crónicas del 27,3%. Entre las vulnerabilidades, el 32% reportó desempleo, el 37,6% emigró a Brasil por la situación social de su país y el 33,6% vivía en un asilo o albergue. Conclusión: la tuberculosis, las enfermedades crónicas y el COVID-19 fueron más prevalentes en inmigrantes y refugiados que en la población general. Por tratarse de una población que aún tiene grandes dificultades para acceder a los servicios de salud y sistemas de protección social, el estudio contribuirá, con base en la evidencia, a las políticas públicas, la atención de enfermería y la incorporación de nuevas rutinas en el servicio.

ABSTRACT

O estudo objetivou analisar a percepção de risco para à infecção pelo Treponema pallidum em trabalhadores da segurança pública do sexo feminino, na capital do Estado de Goiás, Goiânia. Trata-se de pesquisa qualitativa, descritiva com 47 mulheres por meio da aplicação de instrumento semiestruturado. Para análise dos acervos das narrativas utilizou-se o método de análise de conteúdo. Verificou-se a emergência de 03 (três) categorias temáticas: (Des) Conhecimento geral sobre a sífilis; Tabus em relação ao uso de preservativo em relacionamento monogâmico; e Novas ferramentas e educação em saúde. Os principais achados revelam a baixa percepção de conhecimento sobre sífilis, a sensação de invulnerabilidade e a baixa adesão ao uso de preservativos entre as mulheres na força policial. Esses resultados reforçam a necessidade de programas eficazes de intervenção para prevenir essa infecção, que tem proporções epidêmicas e projeções de aumento, entre as trabalhadoras da segurança pública.


The study aimed to analyze the perception of risk for infection by Treponema pallidum in female public security workers, in the capital of the State of Goiás, Goiânia. This is a qualitative, descriptive research with 47 women through the application of a semi-structured instrument. To analyze the collections of narratives, the content analysis method was used. There was the emergence of 03 (three) thematic categories: (Mis) General knowledge about syphilis; Taboos regarding the use of condoms in a monogamous relationship; and New tools and health education. The main findings reveal the low perception of knowledge about syphilis, the feeling of invulnerability and the low adherence to the use of condoms among women in the police force. These results reinforce the need for effective intervention programs to prevent this infection, which has epidemic proportions and is projected to increase, among public security workers.

La vulnerabilidad es un concepto ampliamente utilizado en la literatura de las ciencias sociales, e incorporado al ámbito sanitario con el desarrollo de la bioética y el modelo de determinantes sociales de la salud. Ha sido descrito por expertos como un concepto complejo y de gran utilidad para abordar poblaciones susceptibles de enfermar. Esto obliga a enfermería a identificar estos grupos y responder a sus necesidades de cuidado. Objetivo. Revisar la literatura para analizar las intersecciones en la relación conceptual entre vulnerabilidad y cuidado, desde una perspectiva de enfermería. Metodología. Se realizó una revisión integradora de la literatura basada en la sistematización propuesta por Whittemore y Knafl. Se consideró la literatura de los últimos cinco años disponible en las bases de datos Wos®, Scopus® y PubMed®. En la estrategia de búsqueda se utilizaron las palabras claves "vulnerability" and "care". Resultados. Se seleccionaron 18 artículos, coincidente con países que han desarrollado modelos sanitarios con enfoque social, como Canadá y Brasil. En el análisis se identificaron y describieron cinco temas en que se interceptan vulnerabilidad y cuidado; paradigma ontológico, epidemiológico, social y bioético, y la asociación entre vulnerabilidad y necesidades de cuidado. Conclusiones. La literatura da cuenta de la estrecha relación entre vulnerabilidad y cuidado, dado que la naturaleza de los cuidados profesionales de enfermería es la respuesta organizada socialmente a la vulnerabilidad humana.


Vulnerability is a concept widely used in the social sciences literature, and incorporated into the health field with the development of bioethics, and the model of social determinants of health. It has been described by experts as a complex concept, and very useful for addressing populations susceptible to illness. This ethically obliges nursing to identify these groups and respond to their care needs. Goal. To review the literature to analyze the intersections in the conceptual relationship between vulnerability and care, from a nursing perspective. Methodology. An integrative review of the literature was carried out based on the systematization proposed by Whittemore and Knafl. Literature from the last five years available in the Wos®, Scopus® and PubMed® databases was considered. The keywords "vulnerability" and "care" were used in the search strategy. Results. 18 articles were selected, coinciding with countries with health models with a social focus, such as Canada and Brazil. In the analysis, five themes were identified and described in which vulnerability and care intersect; ontological, epidemiological, social and bioethical paradigm, and the association between vulnerability and care needs. conclusions. The literature reports the close relationship between vulnerability and care, given that the nature of professional nursing care is the socially organized response to human vulnerability.

Isto obriga eticamente a enfermagem a identificar estes grupos e responder a suas necessidades de cuidado. Objetivo. ABSTRACT

Human activities have compelled massive environmental degradation, which causes climate vulnerability and that has emerged as a significant health issue. The present study assesses the role of social progress with greenhouse gases to determine the health vulnerability in 77 developing countries from 2011 to 2020. The empirical results are estimated by using the panel ARDL econometric approach. The study found that greenhouse gas emission proposes a U-shaped relationship to determine health vulnerability. In this study, social progress is used as the moderator variable, which shifts the turning point of the U-shaped curve. For this purpose, the interaction term of social progress with greenhouse gases shifts the turning point to the left side of the U-shaped curve and further flattens it. Furthermore, this study explores that urbanization, export openness, and government education expenditure negatively impact health vulnerability while industrialization increases health vulnerability. The study recommends that government should pay special attention to declining greenhouse gases and rising social progress to improve health vulnerability.Graphical abstact.

ABSTRACT

BACKGROUND: Leptospirosis, caused by the Leptospira bacteria, is an acute infectious disease that is mainly transmitted by exposure to contaminated soil or water, thereby presenting a wide range of subsequent clinical conditions. This study aimed to assess the distribution of cases and deaths from leptospirosis and its association with social vulnerability in the state of Rio Grande do Sul, Brazil, between 2010 and 2019. METHODS: The lethality rates and incidence of leptospirosis and their association with gender, age, education, and skin color were analyzed using chi-square tests. The spatial relationship between the environmental determinants, social vulnerability, and the incidence rate of leptospirosis in the different municipalities of Rio Grande do Sul was analyzed through spatial regression analysis. RESULTS: During the study period, a total of 4,760 cases of leptospirosis, along with 238 deaths, were confirmed. The mean incidence rate was 4.06 cases/100,000 inhabitants, while the mean fatality rate was 5%. Although the entire population was susceptible, white-colored individuals, males, people of the working-age group, along with less-educated individuals, were more affected by the disease. Lethality was higher in people with dark skin, and the prime risk factor associated with death was the direct contact of the patients with rodents, sewage, and garbage. The social vulnerability was positively associated with the incidence of leptospirosis in the Rio Grande do Sul, especially in municipalities located in the center of the state. CONCLUSIONS: It is evident that the incidence of the disease is significantly related to the vulnerability of the population. The use of the health vulnerability index showed great relevance in the evaluation of leptospirosis cases and can be used further as a tool to help municipalities identify disease-prone areas for intervention and resource allocation.

ABSTRACT

Health risk resulting from non-optimal temperature exposure, referred to as "systematic risk", has been a sustainable-development challenge in the context of global warming. Previous studies have recognized interactions between and among system components while assessing the vulnerability to climate change, but have left open the question of indicator directional interactions. The question is important, not least because indicator directional association analysis provides guidance to address climate risks by revealing the key nodes and pathways. The purpose of this work was to assess health vulnerability to short-term summer heat exposure based on a directional interaction network. Bayesian network model and network analysis were used to conduct a directional interaction network. Using indicator directional associations as weights, a weighted technique for the order of preference by similarity to ideal solution method was then proposed to assess heat-related health vulnerability. Finally, hotspots and coping strategies were explored based on the directional interaction network and health vulnerability assessments. The results showed that (1) indicator directional interactions were revealed in the health vulnerability framework, and the interactions differed between northern and southern China; (2) there was a dramatic spatial imbalance of health vulnerability in China, with the Beijing-Tianjin-Hebei Region and the Yangtze River Basin identified as hotspots; (3) particulate matter and ozone were recognized as priority indicators in the most vulnerable cities of northern China, while summer heat exposure level and variation were priority indicators in southern China; and (4) adaptive capacity could alter the extent of risk; thus, mitigation and adaptation should be implemented in an integrated way. Our study has important implications for strengthening the theoretical basis for the vulnerability assessment framework by providing indicator directional associations and for guiding policy design in dealing with heat-related health vulnerability in China.

Introdujo: Nos adolescentes, as vulnerabilidades decorrentes da infecto pelo HIV atrelam-se as singularidades biopsicossociais da fase, tornando-os um grupo prioritário para as estratégias de saúde. Objetivo: Analisar o estado da arte acerca das situacóes de vulnerabilidade de adolescentes que (con)vivem com HIV. Método: Revisao integrativa da literatura realizada em oito bibliotecas/bases de dados para responder a questao norteadora. Foram seguidas as recomendacóes padronizadas para revisao, os achados foram categorizados e discutidos de acordo com referencial da vulnerabilidade. Resultado: Foram identificadas 7.517 publicacóes, das quais 11 foram incluídas. Evidenciaram-se situacóes diversas de vulnerabilidade individuais, sociais e programáticas experienciadas por jovens com HIV, a saber: omissao do diagnóstico, estigma, discriminacao, baixa adesao a terapia antirretroviral, sofrimento emocional, entre outras. Discussao: Adolescentes que vivem com HIV sao suscetíveis a situacóes que os expóem a riscos reais e/ou potenciais. Nesse sentido, é imperioso qualificar os servicos e as acóes de saúde, em uma lógica de oferta universal e integral, livre de julgamentos baseados em crencas pessoais. Conclusao: Adolescentes que (con)vivem com HIV estao inseridos em contextos de vulnerabilidade dinámicos, subjetivos e complexos, cerceados por aspectos individuais, sociais e programáticos que influenciam negativamente o exercício de sua adolescencia, de sua saúde e de suas relacóes.


Introduction: In adolescents, the vulnerabilities resulting from HIV infection are linked to the biopsychosocial singularities of the phase, making them a priority group for health strategies. Objective: To analyze the state of the art regarding situations of vulnerability of adolescents who (co)live with HIV. Method: Integrative literature review conducted in eight libraries/databases to answer the guiding question. The standardized recommendations for review were followed, the findings were categorized and discussed according to the vulnerability framework. Result: 7,517 publications were identified, of which 11 were included. Different situations of individual, social and programmatic vulnerability experienced by young people with HIV were evidenced, namely: omission of diagnosis, stigma, discrimination, low adherence to antiretroviral therapy, emotional distress, among others. Discussion: Adolescents living with HIV are susceptible to situations that expose them to real and/or potential risks. In this sense, it is imperative to qualify health services and actions, in a logic of universal and integral offer, free of judgments based on personal beliefs. Conclusion: Adolescents who (co)live with HIV are inserted in dynamic, subjective, and complex contexts of vulnerability, constrained by individual, social and programmatic aspects that negatively influence their adolescence, their health, and their relationships.


Introducción: En los adolescentes, las vulnerabilidades derivadas de la infección por el VIH están ligadas a las singularidades biopsicosociales de la etapa, convirtiéndolos en un grupo prioritario para las estrategias de salud. Objetivo: Analizar el estado del arte sobre las situaciones de vulnerabilidad de los adolescentes que (co)viven con el VIH. Método: Revisión integrativa de la literatura realizada en ocho bibliotecas/bases de datos para responder a la pregunta guía. Se siguieron las recomendaciones estandarizadas para la revisión, los hallazgos se categorizaron y discutieron de acuerdo con el marco de vulnerabilidad. Resultado: se identificaron 7.517 publicaciones, de las cuales se incluyeron 11. Se evidenciaron diferentes situaciones de vulnerabilidad individual, social y programática que viven los jóvenes con VIH, a saber: omisión del diagnóstico, estigma, discriminación, baja adherencia a la terapia antirretroviral, angustia emocional, entre otras. Discusión: Los adolescentes que viven con VIH son susceptibles a situaciones que los exponen a riesgos reales y/o potenciales. En ese sentido, es imperativo calificar los servicios y acciones de salud, en una lógica de oferta universal e integral, libre de juicios basados en creencias personales. Conclusión: Los adolescentes que (co)viven con el VIH se insertan en contextos dinámicos, subjetivos y complejos de vulnerabilidad, constreñidos por aspectos individuales, sociales y programáticos que influyen negativamente en su adolescencia, su salud y sus relaciones.

O material foi analisado em acordo com o aporte teórico proposto por Hinds, Chaves e Cypress, que indica quatro camadas de relações contextuais inter-relacionadas que melhor compreendem o fenômeno: imediata, específica, geral e metacontextual. Introduction: In recent years the Acquired Immunodeficiency Virus (HIV) has become a major challenge for society due to the magnitude and extent of the damage caused to populations, especially adolescents. Objective: To analyze the contextual aspects of nursing care for adolescents in situations of vulnerability to HIV. Method: This is a narrative literature review with a search carried out in the databases Elsevier SciVerse Scopus, Scientific Electronic Library Online, Latin American and Caribbean Literature in Health Sciences and Google Scholar, in the period of December 20, 2020 to January 20, 2021. The material was analyzed in accordance with the theoretical contribution proposed by Hinds, Chaves and Cypress, which indicates four layers of interrelated contextual relationships that better understand the phenomenon: immediate, specific, general and metacontextual. Results: Eight studies were selected, categorized into sub-themes according to each context: adolescents in a situation of vulnerability to HIV/Aids (immediate context); the insertion of adolescents vulnerable to HIV/Aids in Primary Health Care (specific context); nursing care for adolescents in situations of vulnerability to HIV/Aids in Primary Health Care (general context); and public policies aimed at adolescents in situations of vulnerability to HIV/Aids (metacontext). Conclusion: It can be concluded that understanding the phenomena allows for greater reflection on the reality studied in order to improve nursing care in the face of the problems encountered. (AU)

ABSTRACT

Resumo O estudo teve como objetivo a revisão de trabalhos científicos publicados sobre a hanseníase como problema de saúde pública no Brasil e o marcador de vulnerabilidade para os portadores da enfermidade. Foi realizada uma scoping review com o descritor "hanseníase AND vulnerabilidade" na base de dados da Biblioteca Virtual em Saúde (BVS). Foram selecionados 29 artigos publicados entre janeiro de 2016 e dezembro de 2020 que atenderam aos critérios de inclusão. Os resultados apontaram para trabalhos científicos publicados em revistas indexadas e com diferentes fatores de impacto, com destaque para a maioria das publicações selecionadas constarem em periódicos com métricas de impacto pouco significativas aos olhos da comunidade internacional, ainda que elas tenham impacto nacional. A maioria dos estudos teve abrangência municipal, com destaque para cidades do Norte e do Nordeste brasileiro. Os trabalhos trataram a vulnerabilidade na hanseníase utilizando distintas tipologias.


Abstract The objective of this work was to review published scientific works on leprosy as a public health problem in Brazil, and the marker of vulnerability for people with the disease. A scoping review was carried out with the descriptor 'leprosy AND vulnerability', on the Virtual Health Library (BVS) database. A total of 29 articles published between January 2016 and December 2020 that met the inclusion criteria were selected. The results pointed to scientific papers published in indexed journals and with different impact factors, with emphasis on the fact that most of the selected publications appear in journals with impact metrics that are not very significant in the eyes of the international community, even though they have national impact. The majority of the studies had municipal coverage, with emphasis on cities in the North and Northeast of Brazil. The works addressed vulnerability in leprosy using different typologies.

OBJECTIVE: We investigated people's preferences in COVID-19 vaccine allocation priority, comparing different social categories based on age and occupation. Vaccine allocation preferences were related to perceived health vulnerability and economic backlash (economic negative consequences) endured by the different social groups during the pandemic. In-group favoritism in vaccine allocation preferences was analyzed. DESIGN: Data were collected through an online survey in Italy (n = 506) before the start of the vaccination campaign. MAIN OUTCOME MEASURES: Vaccine allocation preferences, health vulnerability, and economic backlash due to COVID-19, measured through ranking tasks. RESULTS: The healthcare workers category was placed at the top of the ranking in vaccine allocation priority by 65% of the respondents. Vaccine allocation priority was related to perceived health vulnerability and not economic difficulties. Limited self-preference effects emerged. People who did not consider healthcare workers a priority (1/5 of the sample) had a lower education level, were more worried about COVID-19 infection risk, and did not trust vaccines. CONCLUSIONS: A consensus emerged on who should be vaccinated first. Governments and policymakers should be aware of these preferences when designing and communicating vaccine allocation plans to predict and foster the public's acceptance of the COVID-19 vaccination programs created by experts.

ABSTRACT

OBJECTIVES: This article argues that community healthcare clinics managed by dedicated medical professionals who are familiar with the special needs of the local community may serve as effective alternatives to centralized hospitals and medical facilities, which may be disconnected from these local communities. BACKGROUND: The literature indicates that socioeconomic factors that affect an individual's ability to seek medical help when needed can cause vulnerability to public health emergencies. These factors include belonging to lower income populations, being African American, being dependent due to age (below 18 or above 65) or disability, being an immigrant, English-language ability, access to transportation means, and the strength of an individual's social network. METHOD: This study aims at developing a multifaceted methodology to identify optimal locations for deployment of temporary healthcare clinics to address health disparity issues among socially vulnerable populations, especially during pandemics and public health crises. This case study looks at the Health Enterprise Zone (HEZ) in Baltimore and ranks Census tracts based on their vulnerability, using two novel health vulnerability indices and considering their locations. RESULTS: Using the proposed methodology, the optimal tracts within the HEZ are identified as potential locations for deploying temporary healthcare clinics. CONCLUSION: The analysis of vulnerabilities to public health emergencies based on socioeconomic factors can assist in identifying potential locations for setting up temporary healthcare clinics with the goal of assisting socially vulnerable populations during outbreaks and pandemics.

ABSTRACT

The objective of this study was to systematically investigate and review studies on the concept of vulnerability associated with the health of the older population. Articles were selected, filtered, and analyzed following the steps recommended by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Screening and data extraction were performed independently by 2 reviewers using templates developed by the authors. Data extracted included specific details about population, interest, and context. Studies were eligible for inclusion if they: 1) were cross-sectional or prospective, 2) involved community-dwellers aged ≥ 60 years, 3) were published in the last 10 years, and 4) had as a concept vulnerability associated with the health of the older population. A total of 833 studies were identified and screened, 26 of which were included. Most included studies addressed vulnerability in older adults as an individual aspect, whether biological or psychological. The remaining studies reported vulnerability as affecting socio-environmental, health care system, and multifactorial aspects. Therefore, the concept of "vulnerability in older people" was not properly defined in the biomedical scientific community. When we return to the guiding question of this review, we can conclude that the conditions of vulnerability of older people are being treated broadly and diversely, producing different methodological strategies. The systematic review was conducted in the United States National Library of Medicine (PubMed), Latin American and Caribbean Health Sciences Literature (LILACS), and Scientific Electronic Library Online (SciELO) databases between August and December 2020 and updated in September 2022, with registration number CRD42022361649


O objetivo da pesquisa é investigar e revisar sistematicamente estudos sobre o conceito de vulnerabilidade associado à saúde da população idosa. Os trabalhos foram selecionados, filtrados e analisados seguindo as etapas recomendadas pela The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guideline. A triagem e a extração de dados foram realizadas por dois revisores independentes usando modelos desenvolvidos pelos autores. A extração de dados incluiu detalhes específicos da população; interesse; contexto. Consideraram-se artigos que: 1) fossem estudos transversais ou prospectivos, 2) envolvessem idosos da comunidade (≥ 60 anos), 3) fossem dos últimos dez anos; tivessem como conceito a vulnerabilidade associada à saúde da população idosa. O total de 833 estudos foi identificado e triado, e 26 deles foram incluídos. A maioria dos estudos incluídos verificou a vulnerabilidade do idoso no aspecto individual, seja biológico, seja psicológico. Em contrapartida, o restante dos estudos acredita que a vulnerabilidade atinja o aspecto socioambiental, sistema de saúde e multifatorial. Sendo assim, conceito de "vulnerabilidade do idoso" não está devidamente definido na comunidade científica biomédica. Neste caso, retornando à questão norteadora desta revisão, conclui-se que as condições de vulnerabilidade do idoso estão sendo tratadas de maneira ampla e diversa, produzindo diferentes estratégias metodológicas. ABSTRACT

Estudo com objetivo de descrever as condições de vulnerabilidades de pessoas idosas ao Vírus da Imunodeficiência Humana (HIV). Trata-se de um estudo qualitativo e que utilizou como método a história oral temática, realizado no Serviço de Atendimento Especializado, com seis idosos com 65 anos ou mais. A coleta de dados se deu por uma entrevista em profundidade com questões sobre vulnerabilidades individuais, sociais e programáticas e os dados foram explorados por meio da análise de conteúdo temática. Entre as condições encontradas estão a ausência de preservativo nas relações sexuais; baixo grau de conhecimento e de procura por consultas e exames de rotina; presença do preconceito e discriminação; ruptura no convívio familiar; descoberta tardia do HIV e baixa capacitação da equipe de saúde. Enfrentar essas condições de vulnerabilidade pressupõe um debate sobre as políticas e normativas do cuidado às pessoas com HIV e a reconfiguração do modelo de cuidado focado na prevenção. (AU)


The aim of this study was to describe the conditions of vulnerability to HIV among older people. Using the thematic oral history method, we conducted a qualitative study in the SAE with six people aged 65 and over. The data were collected using in-depth interviews covering issues related to individual, social and programmatic vulnerabilities. The data were analyzed using content analysis. Vulnerability conditions included not using a condom, low level of knowledge of appointments and tests and health-seeking behavior; prejudice and discrimination; family break-ups; late diagnosis of HIV; and limited capacity of health teams. To tackle these vulnerability conditions, it is necessary to promote a debate about policies and regulations geared towards the care of people with HIV and reshape the prevention-based care model. Estudio con el objetivo de describir las condiciones de vulnerabilidades de personas ancianas al VIH. Un estudio cualitativo, usando como método la historia oral temática, realizado en el SAE, con seis ancianos de 65 años o más, y la colecta de datos fue realizada mediante una entrevista en profundidad con preguntas sobre vulnerabilidades individuales, sociales y programáticas. Se realizó el análisis de contenido temático. Entre las condiciones encontradas están la ausencia de preservativo en las relaciones sexuales, bajo grado de conocimiento y de búsqueda de consultas y exámenes de rutina, presencia de prejuicios y discriminación, ruptura en la convivencia familiar, descubrimiento tardío del VIH y la baja capacitación del equipo de salud. El enfrentamiento de esas condiciones de vulnerabilidad presupone un debate con relación a las políticas y normativas del cuidado con las personas con VIH y la reconfiguración del modelo de cuidado enfocado en la prevención.
