ABSTRACT

INTRODUCTION: Some epidemiological data suggest that there may be an inverse relationship between cholesterol levels and the risk of thyroid cancer in the overall population. The present study was aimed to evaluate the lipid profile specifically in subjects with Bethesda category IV thyroid nodules, and compare whether there were any differences between those with benign and malignant nodules. METHODS: Single-centre, retrospective study on 204 subjects treated by partial or total thyroidectomy for excision of a Bethesda category IV thyroid nodule, who had undergone a blood lipid profile test in the 12 months prior to surgery. In addition to lipid measures, other demographic, clinical, biochemical and ultrasound data were collected. RESULTS: Seventy-five subjects (36.8%) were diagnosed with thyroid carcinoma in the definitive histopathological examination. Patients with thyroid cancer had lower levels of total cholesterol, LDL-cholesterol and non-HDL-cholesterol than subjects with benign thyroid diseases. There were no differences in HDL-cholesterol, triglycerides or total cholesterol/HDL-cholesterol ratio. There were no differences either between groups in other clinical, biochemical and ultrasound variables, including the use of lipid-lowering drugs. In multivariate analysis, only LDL-cholesterol was independently associated with malignancy. Subjects with follicular carcinoma showed the lowest cholesterol levels, while those with papillary carcinoma had intermediate values between the group with follicular carcinoma and the group with benign thyroid diseases. CONCLUSIONS: In subjects with cytologically indeterminate Bethesda category IV thyroid nodules, levels of total cholesterol, non-HDL-cholesterol and, particularly, LDL-cholesterol are lower among those with malignant nodules.

ABSTRACT

Uterine mullerian adenosarcoma (MA) is a rare biphasic tumour that accounts for less than 0.5% of uterine neoplasms. The age range of presentation is wide, with the median age in the 5th decade of life. It usually has a good prognosis; however, it worsens when it presents with sarcomatous overgrowth, heterologous elements or infiltrates the myometrium. We report the case of a 63-year-old woman presenting with abnormal vaginal bleeding and a sensation of solid material coming out of the cervical canal who was diagnosed with mullerian adenosarcoma with sarcomatous overgrowth (MASO) and presence of heterologous elements after performing a mass biopsy and subsequent hysterectomy. We reviewed the literature, focusing especially on the differential diagnoses to be evaluated, as well as the differences in prognosis and treatment according to whether or not they present histologic features of poor prognosis.

ABSTRACT

Case report is a narrative description of the problem of one or several patients. The CARE checklist (CAse REport) is the consensus document for reporting clinical case reports and through adaptations to the different CARE disciplines is used to define standards for authors in scientific journals; however, the specificity of the nursing process makes it difficult to adjust nursing case reports to CARE. The aim was to analyze the publications of clinical cases with a nursing perspective in scientific journals, as well as the quality standards and evaluation systems used. Few journals reviewed agreed to publish nursing case reports or stated standards for authors to adjust to CARE. Preliminary results indicated average or poor adherence to CARE, with the most reported elements being: Keywords, patient information and introduction. Adherence was lower for the elements: Timeline, therapeutic intervention, follow-up and outcomes, and patient perspective. The characteristics of the nursing process implies a low adherence to CARE, so it is necessary to unify criteria to guide researchers, authors, reviewers and editors of scientific journals, as well as to improve the rigor and quality of the reports. Currently, there are no specific guidelines for reporting clinical case reports with a nursing perspective available. These normative gaps could be solved by developing a CARE extension adapted to the methodological characteristics of the nursing process.

ABSTRACT

El recontorneo estético es una cirugía plástica periodontal que tiene como objetivo restablecer una sonrisa más armoniosa de acuerdo al aspecto facial del paciente, el principal motivo para realizar este procedimiento es cuando existe un diagnóstico de una erupción pasiva alterada o sonrisa gingival. La sonrisa gingival es una de las principales preocupaciones de acuerdo a la estética cuando existe una exposición gingival de más de 3 mm en lo cual es justificable realizar el recontorneo estético. Paciente femenino de 28 años de edad, con diagnóstico de erupción pasiva alterada 1B con un seguimiento de 4 meses. El tratamiento consistió en realizar el recontorneo estético para corregir la sonrisa gingival. El recontorneo estético es un procedimiento quirúrgico que se realiza con el objetivo de mejorar la estética del paciente cuando existe una exposición gingival excesiva y para facilitar la armonía de acuerdo a su aspecto facial.


Esthetic recontouring is a periodontal plastic surgery that aims to restore a more harmonious smile according to the patient's facial appearance, since the main reason to perform this procedure is when there is a diagnosis of an altered passive eruption or gingival smile. The gingival smile is one of the main concerns according to aesthetics when there is a gingival exposure of more than 3 mm in which it is justifiable to perform the aesthetic recontouring. 28-year-old female patient with a diagnosis of altered passive eruption 1B with a 4-month follow-up. Treatment consisted of esthetic recontouring to correct the gingival smile. Esthetic recontouring is a surgical procedure performed with the aim of improving the patient's esthetics when there is excessive gingival exposure and to facilitate harmony according to facial appearance.

Introducción: La hiponatremia se encuentra asociada aproximadamente con el 10% de los pacientes con traumatismo craneoencefálico, su baja frecuencia y la intuición clínica de esta relación la hace importante en el presente informe de caso. Objetivo: Describir la asociación de la hiponatremia con el traumatismo craneoen-cefálico con la finalidad de mejorar la comprensión de esta relación en el personal de salud y promover la implementación de estrategias de diagnóstico y manejo más efectivas basadas en predicciones bioquímicas y anatómicas actualizadas. Presentación del caso: Paciente masculino, adulto joven, con antecedentes de dos traumatismos craneoencefálicos, con hiponatremia sérica severa y síntomas persistentes de mareo y cefalea, tratado con diuréticos de asa y cloruro de sodio vía oral, con mejoría posterior al vigésimo día de hospitalización. Discusión: Ciertas alteraciones neurológicas con manifestación clínica evidente, se presentan por bajos niveles de sodio sérico, o por causas anatómicas y fisioló-gicas diferentes. Se ha demostrado una relación causal con mecanismo fisiopato-lógico no bien descrito sobre la hiponatremia y el traumatismo craneoencefálico. Conclusión: Es crucial tener una vigilancia meticulosa en pacientes con hipona-tremia severa, aunque presenten pocos síntomas clínicos (considerando antece-dentes como el traumatismo craneoencefálico en la anamnesis inicial y de segui-miento) como en este caso. Además, se destaca la necesidad de investigar las vías metabólicas que podrían verse afectadas por traumatismos craneoencefálicos, y que podrían tener un impacto directo en los niveles de sodio en sangre.


Introduction: Hyponatremia is found to be associated with approximately 10% of patients with traumatic brain injury. Despite its low frequency, the clinical intuition regarding this relationship underscores its significance in this case report. Objective: Describe the association between hyponatremia and traumatic brain in-jury, aiming to enhance healthcare professionals' understanding of this correlation and to advocate for the implementation of more effective diagnostic and manage-ment strategies based on updated biochemical and anatomical predictions. Case Presentation: A young adult male patient with a history of two traumatic brain injuries, severe serum hyponatremia, and persistent symptoms of dizziness and headache, treated with loop diuretics and oral sodium chloride, exhibited improvement after twenty days of hospitalization. Discussion: Certain neurological alterations with evident clinical manifestation are characterized by low levels of serum sodium, possibly stemming from distinct ana-tomical and physiological causes. A causal relationship with a poorly described pathophysiological mechanism between hyponatremia and traumatic brain injury has been suggested. Conclusion: Meticulous monitoring is imperative for patients with severe hypona-tremia, even when clinical symptoms are minimal, as observed in this case. Fur-thermore, emphasis is placed on the need to investigate metabolic pathways that may be affected by traumatic brain injuries, potentially exerting a direct impact on blood sodium levels.

ABSTRACT

This report describes how postoperative delirium in an elderly man during COVID-19 pandemic led to a serious event involving a central venous catheter. Delirium is a common cause of perioperative morbidity and mortality, and is characterised by an alteration in consciousness and perception and a reduced ability to focus, sustain or shift attention. The event was analysed by a multidisciplinary committee which developed a risk stratification delirium protocol in order to prevent similar events in the future.

ABSTRACT

Objetivo: desarrollar una aplicación digital para realizar el informe de turno de la supervisión con los indicadores de interés del Departamento de Enfermería en un hospital de alta complejidad de la seguridad social en Costa Rica. Metodología: investigación descriptiva estructurada en etapas para el diseño, desarrollo, e implementación de una aplicación digital denominada Supervisores enfermería WEB. Se desarrolló en un periodo de 2 años, en 4 etapas (Planificación, Desarrollo, Validación e Implementación), con la participación de supervisores de enfermería de los diferentes servicios y personal del Centro de Gestión de Informática del hospital. Resultados: La etapa de planificación del sistema contempló 2 fases. La primera, la exploración de la necesidad con el grupo de supervisión y jefaturas de enfermería y la segunda la solicitud de apoyo al Centro de Gestión Informática (CGI). Se revisaron los indicadores o información del reporte escrito y establecieron los indicadores para estructurar el sistema digital. La etapa de desarrollo contempló las fases: estructura del sistema (indicadores), y seguimiento del diseño. La etapa de validación incluyó las fases de prueba de campo y en ambiente de producción. La etapa de Implementación incluyó la socialización del sistema a todo el grupo y la utilización del sistema por parte de todo el equipo para realizar el informe de turno. Conclusión: el Sistema digital para el reporte de la supervisión de enfermería registra los indicadores de interés y responde a las necesidades de la gestión clínica y administrativa del Departamento de Enfermería en un Hospital de alta complejidad


Objective: to develop a digital application to make the supervision shift report with the in-dicators of interest of the Nursing Department in a high complexity hospital of the social security in Costa Rica. Methodology: descriptive research structured in stages for the design, development and implementation of a digital application called Nursing Supervisors WEB. It was developed over a period of 2 years, in 4 stages (Planning, Development, Validation, Implementation), with the participation of nursing supervisors from different services and personnel from the hospital's Informatics Management Center. Results: The system planning stage included 2 phases. The first was the exploration of the need with the group of supervi-sors and nursing managers and the second was the request for support from the Informatics Management Center (CGI). The indicators or information from the written report were re-viewed and the indicators were established to structure the digital system. The development stage included the following phases: system structure (indicators), and design follow-up. The validation stage included the phases of field testing and production environment. The imple-mentation stage included the socialization of the system to the whole group and the use of the system by the whole team to make the shift report. Conclusion: the Digital System for Reporting Nursing Supervision records the indicators of interest and responds to the needs of clin-ical and administrative management of the Nursing Department in a high complexity Hospital


Objetivo: desenvolver um aplicativo digital para produzir o relatório de turno de supervisão com os indicadores de interesse do Departamento de Enfermagem em um hospital de alta com-plexidade do sistema de seguridade social da Costa Rica. Metodologia: investigação descritiva estruturada em etapas para a conceção, desenvolvimento e implementação de uma aplicação dig-ital designada por Nursing Supervisors WEB. Foi desenvolvida ao longo de 2 anos, em 4 fases (Planeamento, Desenvolvimento, Validação, Implementação), com a participação de enfermei-ros supervisores dos diferentes serviços e pessoal do Centro de Gestão Informática do hospital. Resultados: A fase de planeamento do sistema incluiu 2 fases. A primeira foi a exploração da necessidade com o grupo de supervisores e gestores de enfermagem e a segunda foi o pedido de apoio ao Centro de Gestão de Informação (CGI). Foram revistos os indicadores ou informações do relatório escrito e estabelecidos os indicadores para a estruturação do sistema digital. A etapa de desenvolvimento incluiu as fases: estruturação do sistema (indicadores) e acompanhamento da conceção. A fase de validação incluiu as fases de teste no terreno e de teste em ambiente de produção. A fase de implementação incluiu a socialização do sistema a todo o grupo e a uti-lização do sistema por toda a equipa para produzir o relatório de turno. Conclusões: o sistema digital de relatório de supervisão de enfermagem registra os indicadores de interesse e responde às necessidades da gestão clínica e administrativa do Departamento de Enfermagem em um hospital de alta complexidade.

Introdução: Tumor glômico é uma neoplasia mesenquimal rara que compreende menos de 2% dos tumores de partes moles. Trata-se de um tumor com comportamento clínico benigno, de maior incidência no sexo feminino, entre a segunda e quarta década de vida. Objetivo: Apresentar um caso clínico de tumor glômico. Material e Método: Estudo tipo relato de caso. Relato: Homem de 35 anos de idade com nódulo em antebraço direito há 5 anos. Tratava-se de um nódulo, não ulcerado, eritemato-acastanhado, medindo cerca de 3 milímetros de diâmetro, doloroso a palpação e sensível ao contato com líquidos em baixas temperaturas. Após biópsia incisional da lesão, o exame foi compatível com o aspecto histopatológico de Tumor Glômico, com a presença de vasos sanguíneos dilatados e circundados por pequenas células glômicas com núcleos ovalados, citoplasma eosinofílico e bordas regulares, o que permitiu o correto diagnóstico e o planejamento terapêutico. Até a conclusão do trabalho, o paciente não havia realizado a remoção cirúrgica do tumor, porém não apresentou limitações ou queixas importantes. Conclusão: O estudo do caso do tumor glômico permitirá maior elucidação de novos casos, tendo em vista a raridade do tumor, o quadro clínico pouco específico e escassez de literatura disponível


Introduction: Glomus tumor is a rare mesenchymal neoplasm that comprises less than 2% of soft tissue tumors. It is a tumor with a benign clinical behavior, with a higher incidence in females, between the second and fourth decade of life. Objective: To present a clinical case of glomus tumor. Material and Method: Case report study. Report: 35-year-old man with a nodule on his right forearm for 5 years. It was a non-ulcerated, erythematous-brown nodule, measuring approximately 3 millimeters in diameter, painful on palpation and sensitive to contact with liquids at low temperatures. After incisional biopsy of the lesion, the examination was compatible with the histopathological appearance of Glomus Tumor, with the presence of dilated blood vessels surrounded by small glomus cells with oval nuclei, eosinophilic cytoplasm and regular borders, which allowed the correct diagnosis and planning therapeutic. Until the conclusion of the work, the patient had not undergone surgical removal of the tumor, but did not present any limitations or major complaints. Conclusion: The study of the glomus tumor case will allow greater elucidation of new cases, given the rarity of the tumor, the non-specific clinical picture and the scarcity of available literature


Introducción: El tumor glómico es una neoplasia mesenquimatosa rara que comprende menos del 2% de los tumores de tejidos blandos. Es un tumor de comportamiento clínico benigno, con mayor incidencia en el sexo femenino, entre la segunda y cuarta década de la vida. Objetivo: Presentar un caso clínico de tumor glómico. Material y Método: Estudio de caso clínico. Informe: Hombre de 35 años con nódulo en antebrazo derecho desde hace 5 años. Se trata de un nódulo de color marrón eritematoso, no ulcerado, de aproximadamente 3 milímetros de diámetro, doloroso a la palpación y sensible al contacto con líquidos a bajas temperaturas. Luego de la biopsia incisional de la lesión, el examen fue compatible con el aspecto histopatológico de Tumor Glómico, con presencia de vasos sanguíneos dilatados rodeados de pequeñas células glómicas con núcleos ovalados, citoplasma eosinofílico y bordes regulares, lo que permitió el correcto diagnóstico y planificación terapéutica. Hasta la conclusión del trabajo, el paciente no había sido sometido a extirpación quirúrgica del tumor, pero no presentaba limitaciones ni quejas importantes. Conclusión: El estudio del caso de tumor glómico permitirá un mayor esclarecimiento de nuevos casos, dada la rareza del tumor, el cuadro clínico inespecífico y la escasez de literatura disponible

ABSTRACT

Apresentamos e discutimos a construção de um instrumento de compliance para tratamento de dados pessoais e dados pessoais sensíveis do Núcleo Telessaúde UFSC, com base na Lei Geral de Proteção de Dados Pessoais. Trata-se da elaboração de diretrizes para um Relatório de Impacto à Proteção de Dados Pessoais visando à preservação da dimensão humana do dado e à preservação de direitos. A pesquisa foi qualitativa e exploratória, tendo o relato de experiência como metodologia. O levantamento bibliográfico e a análise documental permitiram a investigação, realizada em 2022, de processos, etapas e fluxos do tratamento dos dados. A análise dos dados foi qualitativa, por comparação dos resultados com a legislação vigente e com a adequação ao princípio da autodeterminação informativa. Os resultados demonstraram que as propostas para o relatório contribuíram para um tratamento de dados mais adequado ao ordenamento jurídico e, consequentemente, mais humanizado


We present and discuss the construction of a compliance instrument for the processing of personal data and sensitive personal data of the Telessaúde UFSC Center, based on the General Data Protection Law. It is a model for the elaboration of guidelines for the construction of an Impact Report on the Protection of Personal Data that collaborates to the preservation of the human dimension of data and the preserva-tion of rights. The research was qualitative and exploratory, adopting experience report as methodology. Bibliographical research and documental analysis enabled the investigation of the processes, stages and flows of data processing, carried out in 2022. Data analysis was carried out qualitatively, comparing the results with current legislation and adequacy to the principle of informative self-determination. Results showed that the proposed guidelines for the report contributed to a data processing more appropriate to the legal system and, consequently, more humanized


Presentamos y discutimos la construcción de un instrumento de cumplimiento para el tratamiento de datos personales y datos personales sensibles del Núcleo Telessaúde UFSC, con base en la Ley General de Pro-tección de Datos. Es la preparación de directrices para un Informe de Impacto de Protección de Datos Per-sonales para preservación de la dimensión humana de los datos y preservar los derechos. La investigación cualitativa y exploratoria adoptó un relato de experiencia como metodología. El levantamiento bibliográfico y el análisis documental permitieron la indagación de procesos, etapas y flujos de procesamiento de datos, realizados en 2022. El análisis de datos fue cualitativa, mediante la comparación de los resultados con la legislación vigente y la adecuación al principio de autodeterminación informativa. Los resultados mostra-ron que las directrices propuestas para el informe contribuyó a un tratamiento de datos más adecuado al ordenamiento jurídico y, en consecuencia, más humano

ABSTRACT

La escritura de artículos académicos es una competencia necesaria para la difusión del conocimiento científico y para el desarrollo profesional de quienes trabajan en diversas disciplinas. Sin embargo, a pesar de su importancia, esta habilidad compleja no suele ser enseñada en forma sistemática, lo que puede operar como una barrera para que los investigadores comuniquen los resultados de sus trabajos. En esta primera entrega, sintetizamos los principales consejos que han brindado expertos en la temática, añadiendo algunos de nuestra experiencia personal que consideramos útiles para facilitar el proceso de la escritura académica y el desarrollo de esta competencia en un contexto colaborativo. En una segunda entrega profundizaremos respecto de la problemática de la escritura de las diferentes secciones de un artículo científico y se ofrecerán consejos para optimizarla y volverla lo más eficaz posible. Academic writing is essential for scientific knowledge dissemination and the professional development of those working in various disciplines. Yet, however important this complex skill is, it is not usually taught systematically, a fact that can act as a barrier for researchers to communicate the results of their work. In this first part, we synthesize the main tips provided by experts in the field, adding some of our personal experiences that they consider relevant to facilitate the process of academic writing and develop this skill in a collaborative context. In a second article, we will go deeper into the problem of writing the different sections of a scientific article and offer advice on ways to optimize it and make it as effective as possible. (AU)

Uno de los aspectos metodológicos esenciales de una investigación es la delimitación de los sujetos a estudiar. Este artìculo aborda la tendencia actual a una excesiva e injustificada declaración de los criterios de inclusión y de exclusión en todo tipo de investigación. Se defiende la idea de que una adecuada utilización de las categorías universo y muestra hace innecesaria la declaración de criterios de inclusión y de exclusión en el diseño metodológico de la mayoría de los proyectos de trabajo de terminación de la especialidad que presentan nuestros residentes.


One of the essential methodological aspects of an investigation is the delimitation of the subjects to be studied. This article addresses the current trend towards an excessive and unjustified statement of inclusion and exclusion criteria in all types of research. The idea is defended that an adequate use of the universe and sample categories makes it unnecessary to declare inclusion and exclusion criteria in the methodological design of most of the specialty completion work projects presented by our residents.

Contar con una guía que establezca un formato único para la elaboración del informe correspondiente a las tesis de las especialidades de las ciencias médicas ha sido, hasta el momento, una necesidad de los tribunales y residentes. Por ello, en la Universidad de Ciencias Médicas de Santiago de Cuba se propuso confeccionar una guía a tal efecto, en la cual se mantienen los aspectos formales y científicos establecidos previamente, pero se añaden elementos en el formato en cuanto a la escritura y la extensión general y por secciones. En este artículo se expone dicha guía, que se constituye en un instrumento de utilidad tanto para los especialistas de primer grado, al facilitar y perfeccionar la confección del trabajo de tesis, como para los tribunales estatales que tienen a su cargo la evaluación de estos informes.


Having a guide that establishes a single format for preparing the report corresponding to theses in medical science specialties has been, until now, a need for courts and residents. For this reason, at the University of Medical Sciences from Santiago de Cuba it was proposed to prepare a guide for this purpose, in which the formal and scientific aspects previously established are maintained, but elements are added in the format in terms of writing and general extension and by parts. This article presents this guide, which constitutes a useful instrument both for first-degree specialists, by facilitating and perfecting the preparation of the thesis work, and for the state courts that are in charge of evaluating these reports.

ABSTRACT

El síndrome de Klippel-Trenaunay-Weber es una malformación vascular congénita poco frecuente. Está caracterizada por una triada de manifestaciones clínicas que comprende una malformación vascular venular, linfática y venosa, junto con hipertrofia esquelética; aumento de tejidos blandos de uno o más miembros; y la presencia de fistulas arteriovenosas. Se presenta el caso de una niña de 5 años de edad, a quien se le confirmó por hallazgos clínicos e imagenológicos un síndrome de Klippel-Trenaunay-Weber. Esta es una enfermedad rara, poco frecuente, con un patrón de herencia genética no bien definido, que necesita de un manejo multidisciplinario. El tratamiento de elección es el clínico sintomático, enfocado a evitar la discapacidad, mejorar la capacidad funcional, calidad de vida y prevenir complicaciones. Se presenta el caso por lo poco frecuente del padecimiento para mostrar su seguimiento y además con fines docentes.


Klippel-Trenaunay-Weber syndrome is a rare congenital vascular malformation. Its characterized by a triad of clinical manifestations that includes venular, lymphatic, and venous vascular malformation, together with skeletal hypertrophy, soft tissue enlargement of one or more limbs, and presence of arteriovenous fistulas. We present the case of a 5-year-old girl who was confirmed by clinical and imaging findings to have Klippel-Trenaunay-Weber syndrome. This is a rare, infrequent disease with a not well-defined genetic inheritance pattern that requires multidisciplinary management. The treatment of choice is the symptomatic clinic, focused on avoiding disability, improving functional capacity, quality of life and preventing complications. The case is presented due to the infrequent nature of this condition to show its follow-up and also for teaching purpose

El síndrome de Hamman se caracteriza por la presencia de aire libre en el espacio intersticial del mediastino, asociado a enfisema subcutáneo. Se presenta el caso de un paciente masculino de 19 años de edad, remitido al Servicio de Cirugía General del Hospital General Docente Leopoldito Martínez, por presentar disnea de moderada intensidad, acompañada de aumento súbito de volumen del cuello y región anterior del tórax, posterior a un acceso de tos durante una crisis aguda de asma bronquial. Al examen físico se identificó enfisema subcutáneo en cuello, tórax, pared anterior del abdomen y miembros superiores. Se observó signo de Hamman positivo. Se indicó radiografía y tomografía axial computadorizada simple de tórax, que evidenció el enfisema subcutáneo y el neumomediastino. Se concluyó como un síndrome de Hamman. El tratamiento fue conservador y la evolución satisfactoria. Este síndrome es una entidad poco frecuente y autolimitada, que se presenta generalmente en pacientes jóvenes masculinos, aparentemente sanos. El diagnóstico se define por los estudios de imaginología y el tratamiento conservador es el recomendado. Se presenta este caso con el objetivo de exponer la presentación, diagnóstico y tratamiento de este síndrome.


Hamman's syndrome is characterized by the presence of free air in the interstitial space of the mediastinum, associated with subcutaneous emphysema. The case of a 19-years-old male patient, referred to the General Surgery Service of the Leopoldito Martínez General Teaching Hospital, presenting with moderate intensity dyspnea, accompanied by a sudden increase in volume of the neck and anterior region of the thorax, after a coughing fit during an acute attack of bronchial asthma is presented. The physical examination identified subcutaneous emphysema in the neck, thorax, anterior wall of the abdomen and upper limbs. A positive Hamman sign was observed. Chest X-ray and simple computed tomography were indicated, showing subcutaneous emphysema and pneumomediastinum. It was concluded as a Hamman syndrome. The treatment was conservative and the evolution was satisfactory. Hamman's syndrome is a rare and self-limiting entity that generally occurs in young, apparently healthy male patients. The diagnosis is defined by imaging studies and conservative treatment is recommended. This case is presented with the aim of exposing the presentation, diagnosis and treatment of this syndrome.

Introdução: O tumor neuroendócrino pancreático gastrointestinal foi descoberto em 1907 por Siegfried Oberdorfer, que mais tarde, em 1929, descreveu seu potencial maligno, relativamente raro. Os tumores neuroendócrinos gastrointestinais e do ducto pancreaticobiliar são tumores heterogêneos com diferentes comportamentos biológicos e clínicos, dependendo da origem do tumor primário, tipos de células neuroendócrinas e características patológicas. Podem aparecer na maioria dos órgãos, ter diferentes etiologias e diferentes características clínicas, morfológicas e genômicas. Objetivo: Relatar o caso de um paciente com neoplasia neuroendócrina aderida em mesentério e intestino grosso. Material e Método: Estudo tipo relato de caso. Resultados: Paciente do sexo masculino, 80 anos, com lesão neoplásica neuroendócrina em mesentério e íleo terminal grau 1, não funcionante. Hipótese diagnóstica: suboclusão intestinal por massa tumoral de aproximadamente 4 cm. Tratamento cirúrgico realizado em 2020, guiado por exame histológico demonstrando a presença de neoplasia neuroendórina midgut pT3pN1M0 em íleo terminal (margem circunferencial, IPN1, IAL1, AVS0, micrometástase para 1 LN em 4 avaliados), IHQ: tumor neuroendócrino pancreático gastrointestinal grau 1 (ki67: 1%), não funcionante e sem evidências de doença a distância. Paciente apresentou uma boa evolução pós cirúrgica, entretanto com insuficiência renal aguda, a qual teve melhora posteriormente. Durante o acompanhamento ambulatorial, paciente apresentou perda ponderal considerável, sugestiva de recidiva. Conclusão: Condição relativamente rara, o tumor NNE pode ser inicialmente assintomático ou acompanhado por dores abdominais em cólica, assim, devem ser buscados diagnósticos diferenciais para dor abdominal. Laparotomia exploradora, cirurgia de remoção da massa e análise histológica com estadiamento para definir o tratamento oncológico de escolha fazem parte da conduta médica


Introduction: The gastrointestinal neuroendocrine pancreatic tumor was discovered in 1907 by Siegfried Oberdorfer, who later, in 1929, described its malignant potential, relatively rare. Gastrointestinal and pancreaticobiliary neuroendocrine tumors are heterogeneous tumors with different biological and clinical behaviors, depending on the origin of the primary tumor, types of neuroendocrine cells and pathological characteristics. They may appear in most organs, have different etiologies and different clinical, morphological and genomic characteristics. Objective: To report the case of a patient with neuroendocrine neoplasia adhered to the mesentery and large intestine. Material and Method: Study type case report. Results: Male patient, 80 years old, with neuroendocrine neoplastic lesion in mesentery and grade 1 terminal ileum, not functioning. Diagnostic hypothesis: intestinal subocclusion by tumor mass of approximately 4cm. Surgical treatment performed in 2020, guided by histological examination demonstrating the presence of neuroendocrine midgut neoplasm pT3pN1M0 in terminal ileum (circumferential margin, IPN1, IAL1, AVS0, micrometastasis for 1 LN in 4 evaluated), IHC: gastrointestinal neuroendocrine pancreatic tumor grade 1 (ki67: 1%) no evidence of distant disease. The patient presented a good post-surgical evolution, however with acute renal failure, which improved later. During outpatient follow-up, the patient presented considerable weight loss, suggestive of recurrence. Conclusion: Relatively rare condition, the NNE tumor may be initially asymptomatic or accompanied by abdominal pain in colic, thus, differential diagnoses for abdominal pain should be sought. Exploratory laparotomy, mass removal surgery and histological analysis with staging to define the oncological treatment of choice are part of the medical approach

Tratamiento quirúrgico realizado en 2020, guiado por examen histológico demostrando la presencia de neoplasia neuroendocrina de intestino medio pT3pN1M0 en íleon terminal (margen circunferencial, IPN1, IAL1, AVS0, micrometástasis a 1 LN en 4 evaluados), IHC: tumor neuroendocrino pancreático gastrointestinal grado 1 (ki67: 1 %), no funcionante y sin evidencia de enfermedad a distancia. El paciente tuvo una buena evolución postoperatoria, sin embargo con insuficiencia renal aguda, que mejoró posteriormente. Durante el seguimiento ambulatorio, la paciente presentó una importante pérdida de peso, sugestiva de recidiva. Conclusión: Condición relativamente rara, el tumor NNE puede inicialmente ser asintomático o acompañarse de dolor abdominal tipo cólico, por lo que se deben buscar diagnósticos diferenciales para el dolor abdominal. La laparotomía exploradora, la cirugía para extirpar la masa y el análisis histológico con estadificación para definir el tratamiento oncológico de elección son parte del manejo medico

El informe operatorio es un documento oficial que forma parte indispensable de la historia clínica. A pesar de que se le atribuyen funciones médicas (asistencial, docente e investigativa), legal y administrativas, con frecuencia no cumple las especificidades requeridas en la redacción. El permanente y continuo mejoramiento del desempeño profesional de los cirujanos demanda el rescate de las especificidades para la confección de tan importante documento. Es considerada una habilidad profesional específica de los médicos de las especialidades quirúrgicas, al ser parte de la práctica quirúrgica habitual e incluye aspectos cuantitativos (formales y sustanciales) y cualitativos (contenido informativo), de obligatorio cumplimiento por especialistas y residentes en formación. Constituye una responsabilidad individual que debe ser controlada por directivos y docentes de cada servicio quirúrgico. Este trabajo expresa la posición de la autora sobre el necesario rescate de la calidad de la confección del informe operatorio como parte indispensable de la seguridad del enfermo quirúrgico(AU)


The operation note is an official document that is an indispensable part of the clinical history. Despite the fact that medical (care, teaching and investigative), legal and administrative functions are attributed to it, it frequently does not meet the specificities required in the wording. The permanent and continuous improvement of the professional performance of surgeons demands the rescue of the specificities for the preparation of such an important document. It is considered a specific professional skill for physicians in surgical specialties, as it is part of routine surgical practice and includes quantitative (formal and substantial) and qualitative (informational content) aspects, of mandatory compliance by specialists and residents in training. It constitutes an individual responsibility that must be controlled by directors and teachers of each surgical service. This work expresses the author's position on the necessary rescue of the quality of the preparation of the operation note as an essential part of the safety of the surgical patient(AU)

ABSTRACT

El linfoma primario del sistema nervioso central es una forma de enfermedad extraganglionar originada en el cerebro, la leptomeninges, la médula espinal o los ojos. Los tumores espinales son neoplasias de baja prevalencia y pueden causar una morbimortalidad neurológica considerable. El linfoma aislado que surge dentro del conducto dural es la forma menos común de linfoma primario del sistema nervioso central: representa aproximadamente el 1% de los casos y se observa más a menudo en el contexto de diseminación secundaria que como el sitio primario de origen. Los síntomas son inespecíficos y dependen del nivel espinal involucrado. La presentación es insidiosa e incluye dorsalgia, debilidad y dificultad progresiva para la deambulación. La resonancia magnética es la modalidad de elección para búsqueda de lesiones dentro del conducto espinal/raquídeo, en pacientes que presentan síntomas neurológicos. El tratamiento quirúrgico no resulta útil, y el objetivo principal de la cirugía es conocer el diagnóstico histológico. (AU)


A primary central nervous system lymphoma is a form of extranodal disease originating in the brain, leptomeninges, spinal cord, or eyes. Spinal tumors are low-prevalence neoplasms and can cause considerable neurological morbidity and mortality. An isolated lymphoma emerging within the dural canal is the rarest form of primary central nervous system lymphoma: it accounts for approximately 1% of cases occurring more often in the context of secondary dissemination than as the primary site of origin. Symptoms are nonspecific and depend on the spinal level involved. The presentation is insidious and includes dorsalgia, weakness, and progressive difficulty in ambulatory function. MRI is the modality of choice to search for lesions within the spinal/rachial canal in patients presenting with neurological symptoms. Surgical treatment is not helpful, and the main objective of surgery is to know the histological diagnosis. (AU)
