ABSTRACT

Introduction: Acute appendicitis secondary to parasitic infections is uncommon, being detected in less than 1% of cases. Balantidium coli is a parasite found in pigs and primates with zoonotic potential. To date, only three cases of acute appendicitis induced by this parasite have been documented globally. Case: A 7-year-old female patient, who consumed pork daily, presented to the emergency department with a one-day history of abdominal pain in the lower quadrants, described as colic-like, alongside abdominal distension. Initial abdominal radiography led to a diagnosis of intestinal obstruction. Conservative management without therapeutic response necessitated referral to a higher complexity center. Upon admission, an abdominal computed tomography scan diagnosed acute appendicitis and secondary ileus. During surgical intervention, an appendiceal phlegmon formed by loops of the small intestine was mechanically released, revealing a perforated appendix with extensive fecal peritoneal contamination. Pathological analysis identified an inflammatory infiltrate and the presence of Balantidium coli trophozoites. Medical treatment included Piperacillin-Tazobactam and Metronidazole. The patient was discharged after 10 days of medical treatment. Discussion: Acute appendicitis caused by Balantidium coli is a rare occurrence. It is crucial to identify parasites in pathological samples due to their impact on postoperative management. The close contact between humans and pigs, especially in developing countries, suggests that the prevalence of parasitic infection and colonization by Balantidium coli may be higher than currently recognized. Regarding the identification of this patient's specific exposure, the regular consumption of pork suggests the hypothesis that improper processing is linked to the acquisition of the parasitic infection.

Introducción: Las parasitosis intestinales y la anemia son un problema de salud pública mundial. Estos parásitos tienen tropismo hacia el intestino delgado, afectan la absorción de micronutrientes durante la eritropoyesis, produciendo la aparición de un síndrome anémico por un recuento bajo de glóbulos rojos y déficit de hemoglobina. Objetivo: Establecer la asociación de la infección por parásitos intestinales y síndrome anémico en niños en edad escolar. Materiales y métodos: Búsq ueda sistemática de literatura publicada entre 2010-2021 sobre asociación entre infección por parásitos intestinales y síndrome anémico en escolares. Resultados: Se identificó 1151 publicaciones, al aplicar los criterios de inclusión y exclusión, se redujeron a 33, encontrándose 9 agentes asociados a anemia, siendo A. lumbricoides (27,27%), A. duodenalis y T. trichiura los helmintos más prevalentes, y G. duodenalis (6,06%) el protozoario más común. El 39,39% de los estudios incluyó ambos agentes. África (21), Asia (6), Sudamérica (5) y Centroamérica (1) tienen la mayoría de publicaciones. Se observa asociación significativa entre infección parasitaria y la anemia IC=95%. Conclusión: La evidencia demuestra alta prevalencia de anemias carenciales de tipo ferropénica y megaloblástica, con asociación significativa entre un mayor porcentaje de infecciones por helmintos y síndrome anémico, en comparación con infecciones por protozoos.


Introduction: Intestinal parasitic infections and anemia are a global public health problem. These parasites have a tropism for the small intestine, which affects the micronutrients absorption during erythropoiesis and causes an anemic syndrome due to a low red blood cell count and hemoglobin deficiency. Objective: To establish the association of intestinal parasite infection and anemic syndrome in schoolchildren. Materials and methods: Systematic search of literature published between 2010 and 2021 about the association between intestinal parasitic infections and anemic syndrome in schoolchildren. Results: 1151 publications were identified, which were reduced to 33 when the inclusion and exclusion criteria were applied. There were 9 parasites, and the helminths commonly associated with anemia were A. lumbricoides (27.27%), A. duodenalis y T. trichiura, whereas G. duodenalis (6.06%) was the most frequent protozoan. The regions with most publications were Africa (21), Asia (6), South America (5), and Central America (1). There was a significant association between parasitic infection and anemia (CI=95%). Conclusion: High prevalence of deficiency anemia, such as iron deficiency and megaloblastic anemia, was observed. Also, there was a significant association between a higher percentage of helminth infections and anemic syndrome compared to infections caused by protozoans.


Introdução: Parasitas intestinais e anemia constituem um problema global de saúde pública. Esses parasitas têm tropismo para o intestino delgado, afetam a absorção de micronutrientes durante a eritropoiese, produzindo o aparecimento de uma síndrome anêmica devido à baixa contagem de glóbulos vermelhos e à deficiência de hemoglobina. Objetivo: Estabelecer a associação entre infecção por parasitas intestinais e síndrome anêmica em crianças em idade escolar. Materiais e métodos: Pesquisa sistemática da literatura publicada entre 2010-2021 sobre a associação entre infecção por parasitas intestinais e síndrome anêmica em escolares. Resultados: foram identificadas 1.151 publicações, ao aplicar os critérios de inclusão e exclusão, foram reduzidos para 33, encontrando 9 agentes associados à anemia, sendo A. lumbricoides (27,27%), A. duodenalis e T. trichiura os helmintos mais prevalentes e G. duodenalis (6,06%) o protozoário mais comum. 39,39% dos estudos incluíram ambos os agentes. África (21), Ásia (6), América do Sul (5) e América Central (1) têm o maior número de publicações. Observa-se associação significativa entre infecção parasitária e anemia IC=95%. Conclusão: As evidências mostram alta prevalência de anemias ferroprivas e megaloblásticas, com associação significativa entre maior percentual de infecções helmínticas e síndrome anêmica, em comparação com infecções por protozoários.

Introducción. El intestino primitivo rota durante la vida embrionaria. Cuando ocurre de forma inadecuada aparece la malrotación intestinal que puede llevar a la obstrucción o al vólvulo del intestino medio. La incidencia disminuye con el aumento de la edad. La malrotación intestinal es una de las principales causas de complicaciones del tracto gastrointestinal en la edad pediátrica. Métodos. Estudio retrospectivo, observacional, transversal y analítico, de la experiencia durante 10 años en pacientes menores de 15 años con diagnóstico de malrotación intestinal, tratados en el Hospital Infantil de San Vicente Fundación, en Medellín, Colombia. Se buscó la asociación entre variables demográficas, clínicas e imagenológicas con los desenlaces. Resultados. Se obtuvieron 58 pacientes con malrotación intestinal, 65 % menores de 1 año. En 29,3 % de los pacientes se hizo el diagnóstico con la presentación clínica; los síntomas predominantes fueron distensión abdominal y emesis. En el 24,1 % se confirmó el diagnóstico con imágenes. Las variables con una diferencia estadísticamente significativa a favor de encontrar una malrotación complicada fueron choque séptico (OR=11,7), síndrome de respuesta inflamatoria sistémica (OR=8,4) y deshidratación (OR=5,18). Conclusiones. La malrotación intestinal tiene complicaciones como perforación, peritonitis, vólvulo e intestino corto. El vólvulo se acompaña de shock y sepsis, con mortalidad hasta del 50 %. Las imágenes diagnósticas son una ayuda, pero no se puede basar la conducta médica en estas porque ninguna imagen garantiza el diagnóstico definitivo. Los signos de alarma son poco específicos. En menores de un año con emesis, distensión y dolor abdominal se debe sospechar malrotación intestinal.


Introduction. The primitive intestine rotates during embryonic life. When it occurs inappropriately, intestinal malrotation appears, which can lead to obstruction or midgut volvulus. The incidence decreases when age increases. Intestinal malrotation is one of the main causes of complications of the gastrointestinal tract in pediatric age. Method. Retrospective, observational, cross-sectional and analytical study of the experience over 10 years in patients under 15 years of age with a diagnosis of intestinal malrotation, treated at Hospital Infantil of San Vicente Fundación, in Medellín, Colombia. The association between demographic, clinical and imaging variables with the outcomes was sought. Results. There were 58 patients with intestinal malrotation, 65% under one year of age. In 29.3% of patients, intestinal malrotation was diagnosed clinically. The predominant symptoms were abdominal distension and emesis. In 24.1% the diagnosis was confirmed with imaging. The variables with a statistically significant difference in favor of finding a complicated malrotation were septic shock (OR=11.7), systemic inflammatory response syndrome (OR-8.4), and dehydration (OR=5.18). Conclusions. Malrotation has complications such as perforation, peritonitis, volvulus, and short bowel. Volvulus is accompanied by shock and sepsis, with mortality of up to 50%. Diagnostic images are helpful, but medical conduct cannot be based on them because no image guarantees a definitive diagnosis. The warning signs are not very specific. In children under one year of age with emesis, distension and abdominal pain, intestinal malrotation should be suspected.

ABSTRACT Objective: To describe the first known case of human Bertiellosis in Paraná (Brazil). Case description: A 6-year-old male residing in the Brazilian state of Paraná was suffering from intermittent nonspecific abdominal pain and abdominal distension, associated with expulsion of live tapeworms in his feces for 7 months. He had a history of interaction with monkeys on an island. His first feces analysis was inconclusive, with an initial hypothesis of an atypical Taenia. However, after additional research, the parasitologist identified pregnant proglottids of Bertiella sp. The patient was initially treated with an unknown dosage of albendazole and nitazoxanide, as it was believed he had been infected with Taenia sp. Since the symptoms persisted, praziquantel 10 mg/kg was prescribed without further proglottids elimination. Comments: Human Bertiellosis is a rare zoonosis, with less than one hundred cases reported. However, it is a cause of chronic abdominal pain and should be kept as a differential diagnosis, especially in cases reporting recurrent tapeworm expulsion in feces and refractory treatment with albendazole.


RESUMO Objetivo: Descrever o primeiro caso conhecido de bertielose humana no Paraná, Brasil. Descrição do caso: Criança de seis anos do sexo masculino, residente no Paraná, Brasil, apresentava dor abdominal inespecífica intermitente e distensão abdominal, associadas à expulsão de helmintos vivos em suas fezes havia sete meses. Tinha um histórico de interação com macacos em uma ilha. Sua primeira análise de fezes foi inconclusiva, com hipótese inicial de uma Taenia atípica. No entanto, após pesquisas adicionais, o parasitologista identificou proglótides gravídicas de Bertiella sp. O paciente foi inicialmente tratado com uma dosagem desconhecida de albendazol e nitazoxanida, pois se acreditava que havia sido infectado por Taenia sp. Diante da persistência dos sintomas, foi prescrito praziquantel 10 mg/kg, sem mais eliminação de proglótides. Comentários: A bertielose humana é uma zoonose rara, com menos de cem casos relatados. No entanto, é uma causa de dor abdominal crônica e deve ser mantida como diagnóstico diferencial, principalmente nos casos que relatam expulsão recorrente de helmintos nas fezes e que são refratários ao tratamento com albendazol.

Intestinal parasites continue to be a public health problem in low- and middle-income countries. Broad use of anthelmintics during deworming programs is still necessary in many regions. However, description of the usage of these medications in general medical practice has been limited. The objective of this study was to determine the use of anthelmintic drugs and their indications in a group of Colombian patients. This was a descriptive study from a drug-dispensing database, identifying patients with prescriptions for anthelmintic drugs. A total of 381 cases were randomly selected, and their medical records were reviewed, analyzing sociodemographic, clinical, and pharmacological variables (indication of use). The lack of diagnosis registration or clinical manifestations of parasites was determined as a prescription without indication. In total, 50.9% (n = 194) of patients were female, and 67.4% of all patients were under 18 yr of age. The diagnosis of helminthiases was clearly stated in 114 (29.9%) patients, and only 4.2% (n = 16) of these had microbiological confirmation. The most commonly used anthelmintic drug was albendazole (70.4% of all prescriptions). The use of anthelmintics was not indicated in 266 cases (69.8%). Nutritional supplements or vitamin prescriptions were associated with using anthelmintics without indication (odds ratio: 2.25; 95% confidence interval: 1.26-4.03). A high proportion of patients lacked symptoms or diagnoses in their clinical records that supported the use of anthelmintic drugs.

RESUMEN El objetivo de este estudio fue examinar la prevalencia de parásitos intestinales de niños y animales domésticos en dos barrios periurbanos de la ciudad de Corrientes, Argentina. También evaluamos las características de los humanos, factores socio-ambientales y prácticas de higiene asociadas con la presencia de parásitos. Las muestras fecales fueron examinadas usando técnicas de concentración por sedimentación y flotación. Se detectaron huevos de Enterobius vermicularis en niños utilizando el método de Graham. El análisis de los datos fue univariado y bivariado. En total, 58 viviendas fueron evaluadas, de las cuales se obtuvieron 146 muestras de heces de niños y 101 muestras de animales. Se registró al menos una especie de parásito en 54 viviendas (93,1%). Encontramos parásitos en el 52,7% de los niños, principalmente los protozoos Blastocystis spp. (35,6%) y Giardia spp. (21,2%). Se detectaron parásitos en 67,32% de los animales, principalmente ancilostomídeos (60,7%). En conclusión, es evidente que el ambiente doméstico presenta condiciones favorables para la transmisión de estos parásitos.


ABSTRACT This study aimed to examine the prevalence of intestinal parasites in children and domestic animals from two peri-urban neighborhoods in Corrientes, Argentina. We also evaluated the characteristics of humans, socio-environmental features, and hygiene practices associated with the presence of parasites. Fecal samples were examined using techniques of concentration by sedimentation and flotation. The Graham method was used to diagnose Enterobius vermicularis eggs in children. We carried out the univariate and bivariate analysis of the data. We analyzed 58 dwellings, from which we obtained 146 stool samples from children and 101 from animals. We found at least one parasite species in 54 dwellings (93.1%). We found that 52.7% of children had parasites, mainly Blastocystis spp. (35.6%) and Giardia spp. (21.2%). We found that 67.32% of the animals had parasites, the most prevalent species being hookworms (60.7%). In conclusion, it is evident that the domestic environment can favor the transmission of these parasites.

Introducción: La infección por Fasciola hepatica es una enfermedad zoonótica de distribución mundial, desatendida y subdiagnosticada. Objetivo: Determinar la frecuencia de Fasciola hepatica en una población preescolar en Tartar Chico, distrito de Baños del Inca, en la región Cajamarca. Métodos: Estudio transversal en 48 niños de una institución educativa inicial. Los padres entregaron 3 muestras de heces para el estudio parasitológico seriado y completaron una encuesta epidemiológica. La identificación de F. hepatica y otros parásitos se realizó con las pruebas de sedimentación rápida de Lumbreras, examen directo y Kato-Katz. Para describir usamos frecuencias y porcentajes, para el análisis bivariado aplicamos Chi-cuadrado o prueba exacta de Fisher. Resultados: La frecuencia de Fasciola hepatica fue 4,17%. Además, estimamos una proporción de 8,33% para Ascaris lumbricoides, 4,17% de Diphyllobothrium pacificum y 2,08% de uncinarias; así como parásitos contaminantes Entamoeba coli, Blastocystis hominis. Conclusión: Encontramos una frecuencia de 4,17% de fascioliasis entre preescolares de una comunidad altoandina del Perú.


Introduction: Fasciola hepatica infection is a globally distributed, neglected and underdiagnosed zoonotic disease. Objectives: To determine the frequency of Fasciola hepatica infection among a preschool population in Tartar Chico, Baños del Inca, Cajamarca. Methods: Cross-sectional study in 48 children of an initial educational institution. Parents delivered 3 stool samples for the serial parasitological study and completed an epidemiological survey. The identification of F. hepatica and other parasites was carried out with the Lumbreras rapid sedimentation tests, direct examination, and Kato-Katz. For descriptive analysis, frequency and percentages were used, for the bivariate analysis, Chi-square or Fisher's exact test was used. Results: The frequency of F. hepatica was 4,17%. In addition, a proportion of 8,33% of Ascaris lumbricoides, 4,17% of Diphyllobothrium pacificum and 2,08% of hookworms; as well as contaminating parasites Entamoeba coli, Blastocystis hominis. Conclusions: A frequency of 4,17% of fascioliasis was found among preschoolers from a high Andean community in Peru.

ABSTRACT

ABSTRACT Objectives: to assess Popular Health Education practices on intestinal parasites, carried out by telephone contact with men living in urban communities in Rio de Janeiro, Brazil, during the COVID-19 syndemic. Method: a quasi-experimental, quantitative and descriptive study, carried out with men aged 20 to 59 years. Pre-test was applied, and participants were divided into two groups: control and experimental. Popular Education in Health practices were carried out with an experimental group, and post-test was applied for both. Results: health education practices were significant in the experimental group, with a reduction in incorrect answers. There was an increase in incorrect answers in the control group's post-test. Conclusions: the Brazilian National Policy for Popular Education in Health contributed to qualify men's health literacy on intestinal parasites. Practice by telephone contact proved to be a powerful strategy for nursing and public policies to access this group and promote health in Primary Health Care.


RESUMEN Objetivos: evaluar las prácticas de Educación Popular en Salud sobre parásitos intestinales, realizadas por contacto telefónico con hombres residentes en comunidades urbanas de Río de Janeiro, Brasil, durante la sindemia de la COVID-19. Métodos: estudio cuasiexperimental, cuantitativo y descriptivo, realizado con hombres de 20 a 59 años. Se aplicó la preprueba y se dividió a los participantes en dos grupos: control y experimental. Se realizaron prácticas de Educación Popular en Salud con un grupo experimental y a ambos se les aplicó la posprueba. Resultados: las prácticas de educación en salud fueron significativas en el grupo experimental, con reducción de respuestas incorrectas. Hubo un aumento de respuestas incorrectas en la posprueba del grupo control. Conclusiones: la Política Nacional de Educación Popular en Salud contribuyó a cualificar la alfabetización en salud de los hombres sobre parásitos intestinales. La práctica por contacto telefónico demostró ser una poderosa estrategia de enfermería y de políticas públicas para acceder a ese grupo y promover la salud en la Atención Primaria de Salud.


RESUMO Objetivos: avaliar práticas de Educação Popular em Saúde sobre parasitoses intestinais, realizadas por contato telefônico com homens residentes em comunidades urbanas do Rio de Janeiro, Brasil, durante a sindemia de COVID-19. Métodos: estudo quase-experimental, quantitativo e descritivo, realizado com homens de 20 a 59 anos. Foi aplicado o pré-teste, e os participantes, distribuídos em dois grupos: controle e experimental. Foram realizadas práticas de Educação Popular em Saúde com grupo experimental e aplicado o pós-teste para ambos. Resultados: práticas de educação em saúde foram significativas no grupo experimental, com redução das respostas incorretas. Houve aumento de respostas incorretas no pós-teste do grupo controle. Conclusões: a Política Nacional de Educação Popular em Saúde contribuiu para qualificar a literacia em saúde de homens sobre parasitoses intestinais. A prática por contato telefônico mostrou-se uma potente estratégia para a enfermagem e as políticas públicas acessarem este grupo e promoverem saúde na Atenção Primária à Saúde.

ABSTRACT

Enteric duplication cysts are rare congenital malformations with a low incidence and there are only a few reports in the literature. Their clinical presentation varies according to the location and the type of duplication. Their overall prognosis is good if early surgical intervention is provided. We report a 2-month-old boy who presented with a case of a giant gastroduodenal duplication cyst with a juxta-pancreatic communication and was successfully treated surgically. It is imperative to be aware of this rare congenital malformation that can present clinically with a wide range of non-specific symptoms that can cause significant morbidity and mortality if the treatment is delayed.

RESUMEN Introducción: las enfermedades inflamatorias intestinales constituyen un grupo heterogéneo de patologías inmunológicas crónicas, de curso no predecible, que incluye a la enfermedad de Crohn, la colitis ulcerosa y la colitis indeterminada, donde la susceptibilidad a la enfermedad, fenotipo y respuesta al tratamiento está determinada por la compleja interacción entre factores genéticos y ambientales. Objetivo: determinar los aspectos clínicos, epidemiológicos, endoscópicos e histológicos de la colitis ulcerosa en la edad pediátrica, en Pinar del Río en el período de enero de 2018 a diciembre 2019. Métodos: se realizó un estudio observacional, descriptivo y transversal en un universo y muestra constituido por 41 niños diagnosticados con la enfermedad en este periodo de tiempo. Para el análisis de los resultados se utilizaron los métodos de la estadística descriptiva como las frecuencias absolutas y relativas. Resultados: predominó el sexo masculino con una edad de entre uno y cinco años, como factores de riesgo predominaron los antecedentes de infecciones anteriores, la dieta inadecuada y el tiempo de lactancia materna menor de cuatro meses. Las manifestaciones clínicas más frecuente fueron: diarreas, rectorragia y el dolor abdominal. Conclusiones: se presentó la enfermedad con mayor frecuencia en menores de cinco años, de raza blanca y procedencia urbana. Las manifestaciones digestivas que predominaron fueron las diarreas con sangre, la rectorragia y las extradigestivas la pérdida de peso. La presencia de eritema, perdida del patrón vascular y las presencia de ulceras fueron los hallazgos endoscópicos más frecuentes encontrados. En el 85,5 % de los casos se presenta la colitis ulcerosa.


ABSTRACT Introduction: inflammatory bowel diseases constitute a heterogeneous group of chronic immunologic pathologies, of unpredictable course, including Crohn's disease, ulcerative colitis and indeterminate colitis, where disease susceptibility, phenotype and response to treatment is determined by the complex interaction between genetic and environmental factors. Objective: to determine the clinical, epidemiological, endoscopic and histological aspects of ulcerative colitis in the pediatric age in Pinar del Río in the period from January 2018 to December 2019. Methods: an observational, descriptive and transversal study was carried out in a universe and sample constituted by 41 children diagnosed with the disease in this period of time. For the analysis of the results, descriptive statistical methods such as absolute and relative frequencies were used. Results: the predominant sex was male with an age between 1 and 5 years, and the predominant risk factors were a history of previous infections, inadequate diet and breastfeeding for less than 4 months. The most frequent clinical manifestations were: diarrhea, rectorrhagia and abdominal pain. Conclusions: the disease occurred more frequently in children under 5 years of age, of white race and urban origin. The predominant digestive manifestations were bloody diarrhea, rectorrhagia and extra-digestive manifestations were weight loss. The presence of erythema, loss of vascular pattern and the presence of ulcers were the most frequent endoscopic findings. Ulcerative colitis was present in 85.5 % of the cases.

Blastocystis spp. es el protista intracelular que en los últimos años ha infectado a más de mil millones de personas a nivel mundial. Sin embargo, el aumento en la prevalencia en México y su potencial patógeno son inciertos, por lo que este microorganismo aún se encuentra bajo investigación. Principalmente a nivel pediátrico, la blastocistosis es estudiada con mayor atención, debido a que destaca sobre otros agentes en diversos estudios realizados mundialmente y en México. El objetivo de este trabajo fue mediante revisión bibliográfica, evidenciar la frecuencia y su transición parasitaria como el agente más prevalente en la actualidad, pese a que no se asocia a sintomatología clínica.


Blastocystis spp. is the intracellular protist that in recent years has infected more than a billion people worldwide. However, the increase in prevalence in Mexico and its pathogenic potential are uncertain, so this microorganism is still under investigation. Mainly at the pediatric level, blastocystosis is studied with greater attention, due to the fact that it stands out over other agents in various studies carried out worldwide and in Mexico. The objective of this work was to show the frequency and its parasitic transition as the most prevalent agent today through a bibliographic review, despite the fact that it is not associated with clinical symptoms.

ABSTRACT

OBJECTIVE: Intestinal parasitic infections (IPIs) are a public health challenge in developing countries such as Colombia, causing anaemia and delayed growth and development in children. We aimed to estimate the geographical and prevalence trend of IPIs in the last 30 years in school and preschool children in Colombia. METHODS: We conducted a systematic review and meta-analysis. We identified potential manuscripts through PubMed, EMBASE, Web of Science, LILACS, Scielo and Google Scholar on the IPIs prevalence in school and preschool children in Colombia. Articles included in the qualitative analysis were published between 1990 and 2020 in English or Spanish and met the inclusion criteria. Subsequently, a random-effects meta-analysis, a meta-regression and a trend analysis were performed. RESULTS: We identified 2292 articles; 109 were included in the qualitative review, and 79 articles were included in the meta-analysis. The estimated IPI prevalence was 55% (95% CI: 48-63). By age group, the prevalence in preschool children was 37% (95% CI: 26-49) and 66% (95% CI: 52-78) in schoolchildren. The prevalence by region was heterogeneous, with the Amazon being the highest (69%) and the Santanderes the lowest (28%). In the last 20 years, the prevalence of helminthiasis has decreased (from 64.66% in 1990-1995 to 22.09% in 2016-2020). CONCLUSION: The prevalence of IPIs is high (>30%) in three of the seven regions in Colombia. Biannual administration of mass deworming in schoolchildren is recommended in the Amazon region. Public policies aiming to control IPIs should be reinforced. Further prevalence studies should include Cesar, Guaviare, Vichada and Vaupés, where the epidemiology of IPIs is unknown. ABSTRACT Introduction: Tufting enteropathy is a rare cause of congenital diarrhea in neonates. It is characterized by the abnormal distribution of epithelial adhesion molecules, which causes enterocytes to shed into the lumen, forming the characteristic tufts. Case summary: A 15-day-old female neonate was taken by her parents to the emergency department of a tertiary care hospital due to diarrheal stools she had been experiencing since birth. The patient presented with dehydration, abnormal weight loss, metabolic acidosis, and acute kidney failure. She received treatment with alizapride, loperamide, zinc sulfate, and probiotics, but after 75 days of treatment she was still symptomatic. An upper tract endoscopy and colonoscopy were performed, finding flattening of the villi and lymphoid cells in the lamina propria. However, the symptoms persisted, and she died at the age of ten months. A post-mortem exome sequencing reported tufting enteropathy. Conclusions. When congenital diarrhea is present, tufting enteropathy should be considered. An early molecular study would allow to evaluate the possibility of performing an intestinal transplant or modifying the treatment to meet the patient's palliative care needs.


RESUMEN Introducción. La enteropatía en penacho es una causa rara de diarrea congénita en neonatos; esta se caracteriza por una alteración de la adhesión epitelial que ocasiona desprendimiento de enterocitos hacia el lumen y, en consecuencia, forma los característicos penachos. Se describe el caso de una paciente con esta patología. Presentación del caso. Neonata de 15 días de vida, quien fue llevada por sus padres al servicio de urgencias de un hospital de tercer nivel debido a que desde su nacimiento tuvo deposiciones diarreicas y a causa de esto presentó deshidratación, pérdida de peso, acidosis metabólica e insuficiencia renal aguda. La paciente recibió manejo con alizaprida, loperamida, sulfato de zinc y probióticos, pero a los 75 días de tratamiento continuaba sintomática. Se le practicó una endoscopia de vías digestivas y una colonoscopia que mostraron aplanamiento de las vellosidades e infiltrado de células linfoides en la lámina propia. Los síntomas continuaron y la menor falleció a los 10 meses de nacida. El resultado del exoma post mortem reportó enteropatía en penacho. Conclusiones. Ante la presencia de diarrea congénita, se debe sospechar de una enteropatía en penacho y considerar el estudio molecular temprano, pues este permite evaluar la posibilidad de realizar un trasplante intestinal o modificar el tratamiento según las necesidades de cuidado paliativo del paciente.

A cross-sectional survey to assess the prevalence of Enterobius vermicularis infection and its associated factors among the child population of infant, preschool and school age in the urban, rural and indigenous population of Iguazú city, in subtropical Argentina was presented. Additionally, the status of enterobiasis at country level was reviewed and analysed. Enterobius vermicularis presence was assessed employing an oviscopic serial sampling technique. Statistical analysis of socio-demographic determinants was performed by generalized linear mixed models at individual, household and community levels. Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines were used to gather national information about E. vermicularis prevalence spanning the decade 2010­2020. A total of 916 children from 470 families participated. Overall prevalence was 29.8%, with 25.3, 30.7 and 34.2% detected for children inhabiting urban, rural and indigenous villages, respectively. The multi-level analysis showed that the presence of E. vermicularis was mostly determined by individual (e.g. age, playing habits, previous pinworm infection) and household-level factors (e.g. family size, overcrowding conditions). Interestingly, WASH variables, such as waste disposal, analysed at community level were also important. Data were analysed to provide eco-epidemiological features of enterobiasis in a heterogeneous subtropical child population in the same territory but with different socio-sanitary realities. The importance of promoting multi-level actions against the determinants identified, to control this public health problem integratively was evidenced. The scoping review of national data updated the state of knowledge of this parasitosis, identifying risk determinants and gaps in knowledge at country level.

Background: Diarrhea induced by infectious factors may lead to significant health problems in dogs. Canine parvovirus(CPV), canine coronavirus (CCV), canine distemper virus (CDV), Giardia spp., Escherichia coli (E. coli), and Salmonella spp. are the important infectious agents that may induce diarrhea in dogs. The present study aimed to investigatethe effect of CPV, CCV, CDV, Giardia spp., E. coli, and Salmonella spp. infections on the change in serum calprotectin(Calp) concentration.Materials, Methods & Results: A total of 30 dogs were enrolled in the study. The study dogs were divided into 3 groups.Healthy animals as confirmed by clinical examination and animals negative for the specified pathogens were placed inGroup 1. Animals infected by one or more agents, including CPV, CCV, CDV, and Giardia spp., but negative for E. coli orSalmonella spp. were placed in Group 2. Finally, animals positive for E. coli or Salmonella spp. and infected or not infectedby one or more agents, including CPV, CCV, CDV, and Giardia spp., were placed in Group 3. Stool samples and rectaland conjunctival swab samples were collected to investigate the etiologic agents that induced diarrhea. Introducción: La enfermedad inflamatoria intestinal es un padecimiento crónico de causa multifactorial, en los últimos años se ha incrementado el número de casos diagnosticados, por lo que su enfoque clínico-endoscópico e histopatológico es de vital importancia para proveer atención integral a los pacientes. Objetivo: Caracterizar clínica-endoscópica e histopatológicamente la enfermedad inflamatoria intestinal. Métodos: Se realizó un estudio observacional transversal, el universo estuvo constituido por 78 pacientes diagnosticados con enfermedad inflamatoria intestinal y la muestra quedó conformada por 54 pacientes. La información fue recogida en formulario a propósito de la investigación. Resultados: Predominaron los pacientes con edad de 50 años o más. Primó el sexo femenino y el color de la piel blanca. El síntoma cardinal más frecuente fue la diarrea y el tiempo de evolución que primó fue de cuatro a seis meses. La localización más frecuente de las lesiones fue el colon izquierdo y primó la colitis ulcerativa como principal variante. Predominó la mucosa de aspecto granular y las lesiones continuas en los rasgos endoscópicos, así como la distorsión de criptas y la inflamación de la lámina propia dentro de la histología. Sobresalió como principal complicación la fístula y de estos casos la mayoría requirió tratamiento quirúrgico. Conclusiones: Mayor afectación a pacientes de mediana edad, sexo femenino y de raza blanca. Principal síntoma asociado es la diarrea. Variante con mayor superioridad es la colitis ulcerativa. Adecuada correlación clínico-endoscópica e histopatológica. Las principales complicaciones encontradas fueron los trayectos fistulosos y la estenosis intestinal que requirieron en su mayoría tratamiento quirúrgico.


Introduction: Inflammatory bowel disease is a chronic disease of multifactorial etiology that in recent years has increased the number of diagnosed cases, so the clinical-endoscopic and histopathological approach is of vital importance to provide comprehensive care to patients. Objective: To characterize the inflammatory bowel disease clinical-endoscopic and histopathologically. Methods: A cross-sectional observational study was carried out. The universe was composed of 78 patients diagnosed with inflammatory bowel disease and the sample was made up of 54 patients. The data obtained from biopsy ballots were collected in a form. Results: Patients aged between 50 and 69 years old predominated. Female sex and white skin color prevailed. The cardinal symptom that predominated was diarrhea followed by enterorrhagia and the time of evolution that prevailed was 4 to 6 months. The most frequent location of the lesions was the left colon and ulcerative colitis predominated as the main variant. Continuous lesions and granular appearance of the mucosa predominated in endoscopic features, while in histological features crypt distortion and inflammatory infiltrate of the same layer prevailed. Among the complications, the fistula stood out and most of these patients required surgical treatment. Conclusions: Greater affectation in middle ages, female and white skin color patients. The main associated symptom is diarrhea. The most common variant was ulcerative colitis. Adequate clinical-endoscopic and histopathological correlation was evidenced. The main complication was the fistulous tracts and intestinal stenosis, that mostly require surgical treatment.

ABSTRACT Intestinal parasitic infections (IPIs) are neglected diseases caused by helminths and protozoa, with the relationships between parasite, host and environment having the potential to produce high morbidity and incapacity to work and mortality in vulnerable areas. This study assessed the prevalence of IPIs concerning socio-environmental conditions and analyzed the knowledge, attitudes and practices related to these diseases among men living in the slums of Rio de Janeiro city, Rio de Janeiro State, Brazil. A cross-sectional study was conducted in an agglomeration of urban slums between 2018 and 2019, with men aged between 20 and 59 years. A socioeconomic status questionnaire and an IPIs knowledge, attitudes and practices questionnaire (KAPQ) were applied. Coproparasitological diagnoses (n=454) were performed using four methods and samples of water for household consumption (n=392) were subjected to microbiological and physicochemical analysis. A total of 624 participants were enrolled. About 40% of the households had "water unsuitable for consumption". Only one Major Area, MA 3 was not statistically significant for IPIs (AOR=0.75; 95% CI: 0.30-1.88; p=0.55). The overall prevalence of IPIs was 23.8%. Endolimax nana (n=65, 14.3%) and hookworm (n=8, 1.7%) were the most frequently identified parasites. The analysis of the frequency of responses to the KAPQ has shown that men reported to seeking medical care if they were suspicious of IPIs, and around 35% would self-medicate. The results have shown the need to adopt integrated health education practices targeting male residents in urban slums to qualify the care with water for human consumption and promote self-care about IPIs. The household can be considered strategic for Primary Health Care activities for men.

INTRODUCTION: Intestinal apicomplexa protozoa are a recognized cause of gastroenteritis. They are endemic in Honduras and their epidemiology varies in different population groups. OBJECTIVE: To identify risk factors for cyclosporiasis, cryptosporidiosis, and cystoisosporiasis. MATERIALS AND METHODS: We conducted a case-control study in a hospital-based population. We performed the diagnosis using the modified Ziehl-Neelsen staining technique and collected the information from laboratory records and clinical charts. RESULTS: Cyclosporiasis was associated with diarrhea (OR=2.28; 95%CI: 1.10-4.89), weight loss (OR=12.7; 95%CI: 2.49-122.00), watery stools (OR=2.42; 95%CI: 1.26-4.65), and infection with another protozoan (OR=3.13; 95%CI: 1.66-5.95). Cryptosporidiosis was associated with HIV infection (OR=15.43; 95%CI: 3.34-71.22), diarrhea (OR=3.52; 95%CI: 1.40-9.40), lymphopenia (OR=6.16; 95%CI: 1.99-18.98), and green color stools (OR=3.00; 95%CI: 1.23-7.30). Cystoisosporiasis was associated with HIV infection (OR=11.20; 95%CI: 3.53-35.44), diarrhea (OR=7.30; 95%CI: 1.89-28.52), leukopenia (OR=4.28; 95%CI: 1.33-13.75), green color stools (OR=11.59; 95%CI: 1.16-558.60), and Charcot-Leyden crystals (OR=11.59; 95%CI: 1.16-558.60). CONCLUSIONS: In this hospital-based population from Honduras, HIV infection was a risk factor for cryptosporidiosis and cystoisosporiasis, but not for cyclosporiasis.


Introducción. Los protozoos Apicomplexa intestinales son causa reconocida de gastroenteritis. Estas parasitosis son endémicas en Honduras y su epidemiologia varía según los grupos poblacionales. Objetivo. Identificar los factores de riesgo para ciclosporiasis, criptosporidiosis y cistoisosporiasis. Materiales y métodos. Se hizo un estudio de casos y controles en población hospitalaria. El diagnóstico se hizo utilizando la coloración modificada de Ziehl-Neelsen. La información se obtuvo del registro de laboratorio y las historias clínicas. Resultados. La ciclosporiasis se asoció con diarrea (OR=2,28; IC95% 1,10-4.89), pérdida de peso (OR=12,7; IC95% 2,49-122), heces líquidas (OR=2,42; IC95% 1,26-4,65), infección con otros protozoos (OR=3,13; IC95% 1,66-5,95). La criptosporidiosis se asoció con el HIV (OR=15,43; IC95% 3,34-71,22), la diarrea (OR=3,52; IC95% 1,40-9,40), la linfopenia (OR=6,16; IC95% 1,99-18,98), las heces de color verde (OR=3,00; IC95% 1,23-7,30).
