ABSTRACT

BACKGROUND: Few ≥ 10-year follow-up studies of temporomandibular joint (TMJ) discectomy without replacement in patients with disc displacement (DD) analyse the relationship between the surgery and osteoarthritis (OA) exist. OBJECTIVES: To radiologically evaluate bony joint changes and OA development 10 and 30 years after TMJ discectomy as well as 30-year clinical outcome. METHODS: Twenty-two discectomy patients at the University of Oslo, Norway, with records confirming initial TMJ diagnosis and attendance of 10-year radiological follow-up were evaluated and eligible for 30-year follow-up. Primary variables: discectomy and CT-/CBCT-diagnosed OA at follow-ups. Secondary variables: perioperative TMJ diagnoses and remodelling at follow-up. Unoperated TMJs (Unop-TMJs) in unilaterally operated patients were controls. Statistical association and correlation analyses were performed for the 10-year follow-up (significance level p < 0.05). RESULTS: Twenty-two patients attended the 10-year follow-up (mean follow-up 11 years) with 27 operated TMJs (Op-TMJs) and 17 Unop-TMJs. OA perioperatively was associated with DD without reduction (p = 0.001) and additional disc abnormalities (p = 0.016). Although statistically non-significant, the number of TMJs with OA had increased at 10-year follow-up (p = 0.114, Op-TMJs: 14 to 20 joints; Unop-TMJs: 2 to 5 joints). Remodelling was correlated with discectomy (p = 0.003) and to OA (p = 0.006). Nine patients attended the 30-year follow-up (mean follow-up 32 years, 11 Op-TMJs). All TMJs with OA at 30-year follow-up had OA at 10-year follow-up. Mean maximal interincisal opening was 39 mm. No DC-TMD-diagnosed arthralgia was found. CONCLUSION: Osteoarthritis developed similarly between Op- and Unop-TMJs. Only remodelling, not OA, was correlated to the surgery. The clinical results were still favourable at final follow-up.

ABSTRACT

Objectives  This study was undertaken to investigate the prevalence of temporomandibular disorders (TMDs) and the impact of various contributing factors among undergraduate healthcare students in the region of Taif, Saudi Arabia. Methods A total of 100 undergraduate students were recruited from both the College of Medicine and the College of Dentistry at Taif University, Taif, Saudi Arabia. Data were collected on demographic parameters and the Helkimo's index (anamnestic {Ai} and clinical dysfunction {Di} component) using an anonymous self-administered questionnaire, as well as clinical examinations.  Results A significantly high prevalence (97%) of TMDs was observed among the sampled students, with most of them (44.0%) experiencing severe symptoms that might negatively impact their quality of life. On clinical examinations, most of the students (75.0%) showed signs of mild clinical dysfunction, which might indicate an early stage of TMD. Moreover, factors that include older age, majoring in "dentistry" studies, being allergic, having oral habits, poor mental health, and previous COVID-19 infections were found to be significantly associated with TMDs. Conclusion The findings indicate a relatively high TMD prevalence among the sampled undergraduate healthcare students, especially those studying "dentistry". Curriculum modifications, coupled with more awareness and education, are recommended to achieve early diagnosis and help in reducing the incidence of TMD among this population.

ABSTRACT

OBJECTIVES: The main aim of this study was to evaluate bioelectrical activity the immediate effect of the post-isometric relaxation technique (PIR) and compression technique in patients with myalgia of the masticatory muscles. MATERIALS AND METHODS: 97 people were divided into 3 groups: the first group underwent PIR, the second group underwent trigger point compression technique, and the third group consisted of individuals without myalgia of the masticatory muscles, serving as the control group and not experiencing any therapeutic intervention. RESULTS: After the therapy, a decrease in the bioelectrical activity of the masticatory muscles and an improvement in jaw mobility were observed. These results approached those of the control group, indicating a return of the masticatory muscles to their physiological activity. CONCLUSIONS: Both PIR therapy and trigger point compression appear to have similar therapeutic properties. Further comparative studies between these therapies are suggested to aid in treating TMDs.

ABSTRACT

PURPOSE: This network meta-analysis (NMA) of randomized controlled trials (RCTs) aimed to identify effective initial conservative treatment strategies for patients with temporomandibular joint disorders (TMD). STUDY SELECTION: RCTs comparing treatment options for TMD published between January 2000 and July 2021 were retrieved from the databases of PubMed and Embase via a comprehensive electronic search. Patients diagnosed with myalgia (muscle pain) or arthralgia (joint pain) according to pain-related Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) and the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) were eligible for inclusion. Twelve treatment options and a placebo were included in the mutual comparisons. The risk of bias was assessed using Risk of Bias 2.0. Forest plots of direct comparisons between individual studies were created using MetaInsight. NMA was performed using R statistical software (netmeta). RESULTS: Twenty-four RCTs involving 1336 patients assessing pain and 12 RCTs involving 614 patients assessing maximal mouth opening were identified. Low-level laser therapy (standard mean difference [SMD]: -2.12, 95% confidence interval [CI]: -3.18, -1.06), self-exercise (SMD: -1.51, 95% CI: -2.82, -0.2), and stabilization splints (SMD: -1.16, 95% CI: -2.02, -0.29) were effective in improving pain; however, the certainty of evidence was very low. Self-exercise (SMD: 0.71, 95% CI: -0.58, 2.01), stabilization splints (SMD: 0.65, 95% CI: -0.09, 1.39), and low-level laser therapy (SMD: 0.63, 95% CI: -0.34, 1.6) were effective in improving maximal mouth opening; however, the certainty of evidence was very low. CONCLUSIONS: Stabilization splints, self-exercise, and low-level laser therapy may be effective in the initial treatment of TMD.

ABSTRACT

Sleep bruxism is both masticatory muscle activities characterized by repeated or prolonged tooth contact as well as bracing or thrusting of the jaw. This meta-analysis evaluates the differences between sexes and which therapy is most effective in treating bruxism. A literature search was performed on PubMed, Lilacs, Web of Science and Scopus, and articles published from 2000 to 2022 were considered according to the keywords entered. The term "Bruxism" has been combined with "Children" using the boolean connector AND. At the end of the research, 1462 studies were identified from the search conducted on the three engines. Only four were chosen to draw up the present systematic study. The Forrest plot found that photo biomodulation therapy has a higher efficacy (Odds Ratio (OR) 0.10; 95% Confidence Interval (CI) from 0.03 to 0.43), followed by treatment with hydroxyzine (OR 0.19; 95% CI from 0.03 to 1.04). The average between girls and boys with bruxism is 18.5 for boys and 19.5 for girls. This meta-analysis showed that treatment by photobiomodulation has more significant effects on bruxism, followed by treatment with hydroxyisalazine. However, this meta-analysis has limitations due to the diversity of treatment evaluation methods.

ABSTRACT

This study presents a layered transition metal dichalcogenide/black germanium (b-Ge) heterojunction photodetector that exhibits superior performance across a broad spectrum of wavelengths spanning from visible (vis) to shortwave infrared (SWIR). The photodetector includes a thin layer of b-Ge, which is created by wet etching of germanium (Ge) wafer to form submicrometer pyramidal structures. On top of this b-Ge layer, the WS2 thin film is deposited using pulsed laser deposition. In comparison to conventional germanium, b-Ge absorbs about 25% more light between 850 and 1750 nm wavelengths. The WS2/b-Ge photodetector has a peak photoresponsivity of 0.65 A/W, which is more than twice the photoresponsivity of the WS2/Ge photodetector at 1540 nm. Additionally, it shows better responsivity and response speed compared with other similar state-of-the-art photodetectors. Such an improvement in the performance of the device is credited to the light-trapping effect enabled by the germanium pyramids. Theoretical simulations employing the finite-difference time-domain technique help validate the concept. This novel photodetector holds promise for efficient detection of light across the vis to SWIR spectrum.

ABSTRACT

The factors differentiating temporomandibular disorders (TMD) pain from tooth pain is of interest to dentists. Prior to receiving therapy, participants answered the six-question TMD Pain Screener questionnaire. Applying validated Diagnostic Guidelines for TMD (DC/TMD), an orofacial pain specialist and endodontic resident with board certification performed endodontic and TMD examinations. In our study, sensitivity was higher for TMD screener regarding identification of all types of TMDs (0.94 (0.80-0.97), TMD pain not referring to teeth (0.94 (0.76-0.98), TMD pain referring to teeth only (0.94 (0.64-1.00)). TMD Pain Screener questionnaire can be applied in identification of TMDS in patient seeking endodontic therapy.

ABSTRACT

INTRODUCTION AND AIMS: Gastroesophageal reflux disease (GERD) and temporomandibular joint disorder (TMD) are relatively common conditions with a potential causal relationship. This study aims to investigate the possible causal relationship between GERD and TMD through bidirectional Mendelian randomization analysis. METHODS: Using data from large GWAS databases, we conducted bidirectional Mendelian randomization analyses to investigate the potential causal link between GERD and TMD. Instrumental variables were selected from the IEU platform, comprising 129,080 GERD cases and 473,524 controls from the UK Biobank. TMD data from the FinnGen project included 6,314 cases and 222,498 controls. RESULTS: The forward MR analysis suggested that GERD may increase the risk of TMD (OR = 1.47, 95% CI: 1.20-1.81, P = 2e-4). The Weighted Median method also yielded significant results (OR = 1.53, 95% CI: 1.14-2.04, P = 4.1e-3). However, the reverse MR analysis did not reveal a significant association between TMD and GERD (OR = 1.02, 95% CI: 0.98-1.05, P = .33). CONCLUSION: This study, employing MR analysis, provides initial evidence supporting a potential causal relationship between GERD and TMD. The findings contribute to a better understanding of the relationship between these two conditions and offer insights for future clinical investigations. CLINICAL RELEVANCE: The findings of this study hold potential clinical significance in guiding early management strategies for GERD, reducing the incidence of TMD, and optimizing healthcare resource allocation, thereby improving patient quality of life. Further clinical studies are warranted to validate these findings and explore underlying mechanisms.

ABSTRACT

The article presents a case of a 13-year-old adolescent male patient who started orthodontic treatment at the age of 12. Before treatment, he was diagnosed with narrow maxilla, proclination of upper incisors, deep overbite, distal occlusion bilaterally with significant sagittal overjet in frontal area, skeletal Class II, and hypodivergent growth pattern. During treatment, the patient is in his pubertal growth spurt. About 2 months after intermaxillary Class II elastics (1/4 heavy, 6.5 Oz) were applied, he complained of pain during mastication, wide opening of the mouth, and sometimes during protrusive and lateral movements in the right TMJ. The TMJ X-ray examination did not reveal abnormal morphological changes. Occlusion was evaluated by an electromyographic device, Teethan. The result was typical for Class II malocclusion. During the bilateral palpation of the zones of TMJ and opening of the mouth and chewing, the patient reported pain on the right side. There was no clicking in the joint. The elastic wear was stopped, and soon afterwards, the pain disappeared. These complaints point to a possible relationship between orthodontic treatment and TMJ pain. However, the disappearance of complaints after the removal of the Class II elastics points that the temporomandibular joint disorder (TMD) symptoms are reversible and resolved.

ABSTRACT

OBJECTIVE: This scoping review aims to clarify the concept of vertical dimension of occlusion (VDO) in prosthetic dentistry, addressing prevalent myths, and controversies regarding its clinical management and impact on the stomatognathic system. METHODS: This paper critically examines common beliefs about VDO alteration and its effects on temporomandibular disorders (TMD) through an extensive literature review and an international survey. The survey included 862 general dentists and prosthodontists and explored their approaches to altering VDO and their perceptions of the clinical implications. The literature review provided a thorough analysis of existing research on VDO modification techniques, the adaptability of the stomatognathic system, and the relationship between VDO changes and TMD. RESULTS: The survey revealed insights into dentists' perspectives on VDO, showing that while many practitioners test a new VDO due to concerns about patient adaptation, a significant majority observed that patients typically adjust well to a new VDO regardless of the technique used. The survey highlighted differing beliefs about the need for a gradual process and trial phase for VDO alteration, as well as the stability of modified VDO. Literature review suggests the stability and adaptability of VDO changes due to muscle adaptability and indicates no direct causal or curative link between VDO changes and TMD. Traditional methods for determining VDO in complete dentures may not be suitable for worn dentition, underscoring the need for tailored approaches. CONCLUSION: This paper suggests that the traditional belief in the need for a prolonged trial phase for VDO alterations may be reconsidered in light of evidence supporting the adaptability of the stomatognathic system. Moreover, it emphasizes the importance of distinguishing between methods suitable for complete dentures and those for managing cases of worn dentition. The purported link between VDO alterations and TMD is also found to lack scientific evidence, highlighting the need for individualized patient care based on current evidence and clinical judgment.

ABSTRACT

OBJECTIVES: Temporomandibular disorders (TMD) affect 10% of the population in France, significantly impacting patient's health and quality of life. The multifactorial etiology of TMD complicates its treatment. Consequently, adopting a multidisciplinary approach that encourages collaboration among healthcare professionals is recommended. Nevertheless, this approach does not seem to occur on a regular basis. The aim of this study was to assess the dentist's treatment support of TMD in the context of the inter-professional collaboration between dental practitioners and physiotherapists. MATERIEL AND METHODS: A cross-sectional observational study was conducted based on an online questionnaire from January 15th to April 30th, 2023. The data are collected through Professional Broad, Regional Unions of Health Professional and Healthcare Professional Communities and Territories. RESULTS: Of the 420 responses analyzed, the main first-line treatment provided by the dentist was the correction of dental occlusion (84%). The rate of referral to a physiotherapist was 57% and was 75% for second-placed treatment. The main reason for the lack of referral was a lack of awareness of physiotherapy role in the management of TDM. We observed that 70% of dental health practitioners were interested about inter-professional collaboration and the development of postgraduate training. CONCLUSION: Given the difficulty of managing TDM, efforts should be made to improve inter professional management. CLINICAL RELEVANCE: The implementation of appropriate teaching in initial training seems to be essential to allow dentists to open the range of treatment for TMD with increased knowledge of physiotherapy techniques for an adapted prescription to the patient.

ABSTRACT

Targeting heterologous multi-transmembrane domain (TMD) proteins to plant chloroplasts requires sequences in addition to the chloroplast transit peptide (cTP). The N-terminal domain (N-region), located C-terminal to the cTP in chloroplast inner envelope membrane proteins, is an essential region for import. However, it was unclear if the N-region functions solely as a spacer sequence to facilitate cTP access or if it plays an active role in the import process. This study addresses the N-region's role by using combinations of cTPs and N-regions from Arabidopsis chloroplast inner envelope membrane proteins to direct the cyanobacterial protein SbtA to the chloroplast. We find that the sequence context of the N-region affects the chloroplast import efficiency of SbtA, with particular sequences mis-targeting the protein to different cellular sub-compartments. Additionally, specific cTP and N-region pairs exhibit varying targeting efficiencies for different heterologous proteins. Substituting individual N-region motifs did not significantly alter the chloroplast targeting efficiency of a particular cTP and N-region pair. We conclude that the N-region exhibits contextual functioning and potentially functional redundancy in motifs.

BACKGROUND: The study aimed to retrospectively assess the efficacy of hyaluronic acid (HA) in managing temporomandibular disorders (TMD) using the diagnostic criteria for temporomandibular disorders (DC/TMD). There has been an ongoing debate regarding the effectiveness of HA as a treatment option for TMD, which necessitated a thorough evaluation. METHODS: The review adhered to PRISMA guidelines conducted across eight different databases, including PubMed, Embase, Web of Science, Cochrane Library, Scopus, ScienceDirect, PsycINFO and CINAHL. The selection criteria included studies that evaluated the efficacy of HA in TMD patients, utilised DC/TMD, and were published in English. Data extraction and quality assessment were performed independently by two reviewers. ROB-2 tool was employed to assess methodological quality of the assessed studies. RESULTS: A total of 10 studies met the inclusion criteria. They demonstrated that HA was effective in improving various symptoms of TMD, such as pain, mouth opening and joint sounds over control group. But on the other end, platelet-rich plasma (PRP) was found to be better than HA intervention in alleviation of TMD symptoms. However, the degree of improvement varied across the studies. Some studies reported adverse effects, but these were typically minor and transient. Risk of bias assessment was low in all the included studies. CONCLUSION: The findings suggest that HA can be an effective treatment for TMD when evaluated with DC/TMD. However, the variation in effectiveness across studies indicates the need for individualised treatment planning and careful monitoring of adverse effects. Further research is needed to refine the treatment protocols and understand the long-term effectiveness and safety of HA in TMD management.

ABSTRACT

OSAS and TMDs represent multifactorial nosologic entities, whose central, functional and psycho-social aspects are gaining growing attention within the scientific community. In our previous commentary, we wanted to point out that structural aspects should not be forgotten in a clinical and research context. The inherent complexity of the matter could make it difficult to quantify the exact contribution of every single factor. The multifaceted nature of OSAS and TMDs pathophysiology could sustain several phenotypes in both conditions, and the anatomic parameters may assume different weights according to each phenotype, possibly justifying literature discrepancies. Thus, a patient with a co-existing OSAS and TMD (umbrella terms per se, each of them including different pathophysiological and clinical characteristics) represents an even greater challenge to researchers and practitioners. The scientific and therapeutic community should keep on looking for evidence to offer the best possible answers to such daring questions, in the most collaborative and fruitful way.

Establishing reliable electrical contacts to atomically thin materials is a prerequisite for both fundamental studies and applications yet remains a challenge. In particular, the development of contact techniques for air-sensitive monolayers has lagged behind, despite their unique properties and significant potential for applications. Here, we present a robust method to create contacts to device layers encapsulated within hexagonal boron nitride (hBN). This method uses plasma etching and metal deposition to create 'vias' in the hBN with graphene forming an atomically thin etch-stop. The resulting partially fluorinated graphene (PFG) protects the underlying device layer from air-induced degradation and damage during metal deposition. PFG is resistive in-plane but maintains high out-of-plane conductivity. The work function of the PFG/metal contact is tunable through the degree of fluorination, offering opportunities for contact engineering. Using the in situ via technique, we achieve ambipolar contact to air-sensitive monolayer 2H-molybdenum ditelluride (MoTe2) with more than 1 order of magnitude improvement in on-current density compared to previous literature. The complete encapsulation provides high reproducibility and long-term stability. The technique can be extended to other air-sensitive materials as well as air-stable materials, offering highly competitive device performance.

ABSTRACT

Objectives: This study aimed to find if a relationship exists between antero-posterior malocclusions and the level of musculoskeletal disorders in adults, including body posture and static foot analysis. Methods: In all, 420 participants were recruited through convenience sampling (Kraków University students and patients of a local dentist's practice). Following general medical interviews, dental examinations and consideration of inclusion and exclusion criteria, 90 healthy volunteers (ages 19-35) were enrolled and assigned to three groups (n = 30) based on occlusion type (Angle's molar Class I, II or III). The research procedure involved occlusion and temporomandibular disorder assessment conducted by a dental specialist. Comprehensive morphological measurements of body asymmetry were performed using the Videography 2D package and FreeSTEP software, which calculated the parameters determined from anterior, posterior and lateral projection photos. Foot loading distribution was analyzed using the FreeMED baropodometric platform. Results: Significant differences were demonstrated in the positioning of the head, cervical and lumbar spine in the sagittal plane among individuals with the analyzed occlusal classes (p < 0.05). Individuals with Angle's Class II exhibited significantly greater forward head positions and greater depths of cervical and lumbar lordosis compared with individuals with Class III or Class I. Those with overbites had higher forefoot loading. The Class III individuals exhibited greater L-R displacement, indicating a larger angle of displacement of the centers of the right and left feet relative to the lower edge of the measurement platform, suggesting pelvic rotation. Conclusions: An inclination for concurrent occurrences of malocclusions and posture deviations in the sagittal plane was observed. An interdisciplinary approach involving dentistry and physiotherapy specialists which utilizes tools for comprehensive posture assessment is crucial for diagnosing and treating such conditions.

OBJECTIVE: Anxiety tends to increase muscle tension and painful symptoms; however, its symptoms can be associated with temporomandibular dysfunction (TMD). Therefore, it is crucial for professionals to identify the presence of both comorbidities and determine whether they are related. This study aims to systematically evaluate the literature and answer the question: "Is there an association between anxiety and TMD in adults?" METHODS: The study followed the PRISMA protocol and was registered in PROSPERO. The custom search was performed in PubMed, Embase, Scopus, and Lilacs databases, as inclusion criteria observational studies that evaluated the association between anxiety and TMD in adults. RESULTS: A total of 1640 articles were found. After removing duplicates, 988 were evaluated by title and abstract. Of these, 28 were selected for reading in full, of which 13 were included in the present review. CONCLUSION: The association of anxiety with TMD is positive, except for the minority indicating a low relationship between the disorders, which is explained by neuroscience.

BACKGROUND: This systematic review examines and evaluates the relationship between salivary cortisol levels and temporomandibular disorder (TMD) in young adult patients. METHOD: Six databases-PubMed, Scopus, Web of Science, Google Scholar, ProQuest, and Cochrane Library-were utilized to screen eligible studies. A systematic search was performed based on PECO questions and eligibility criteria. The research question for this review was "Do salivary cortisol levels correlate with TMD in individuals aged 18-40?" The risk of bias for quality assessment was determined by the Cochrane tool. PRISMA guidelines were followed while performing this review. RESULT: A total of fourteen studies were included in this review. Of these, eleven were observational studies (four cross-sectional and seven case-control), and three were randomized control trials. Eleven of the included studies presented a low to moderate risk in the qualitative synthesis. The total sample size of the included studies was 751 participants. The included studies suggest higher salivary cortisol levels in TMD patients than in healthy individuals. CONCLUSIONS: The findings of this review indicate higher salivary cortisol levels in adult patients with TMD than in healthy controls. Thus, supportive psychological treatment and clinical modalities should be provided to patients with TMD. Moreover, higher-quality studies with low heterogeneity are required to support this finding.
