ABSTRACT

Introducción y objetivos:El levosimendán ambulatorio repetitivo es una opción como puente al trasplante cardiaco (TxC), aunque la evidencia sobre su eficacia y su seguridad es escasa. El objetivo del registro LEVO-T es describir a los pacientes en lista de TxC que reciben levosimendán, sus pautas y los eventos clínicos durante el seguimiento, en comparación con los que no lo reciben. Métodos: Se revisó en retrospectiva a los pacientes en lista de espera para TxC electivo de 14 centros españoles desde 2015 hasta 2020. Resultados: Se incluyó a 1.015 pacientes consecutivos; los 238 (23,4%) que recibieron levosimendán mostraron más ingresos por insuficiencia cardiaca (IC) el año anterior y peor perfil clínico. Las dosis fijas por necesidades clínicas fueron la pauta más frecuente. Dos pacientes (0,8%) presentaron arritmias ventriculares no mortales. No hubo diferencias en hospitalizaciones por IC entre los que comenzaron levosimendán en los primeros 30 días después de inclusión y los que no (el 33,6 frente al 34,5%; p=0,848). De estos últimos, 102 (32,9%) pasaron a levosimendán después de un ingreso por IC, y la tasa de ingresos por IC/mes varió de 0,57 antes del levosimendán a 0,21 después. El análisis mediante emparejamiento por puntuación de propensión no mostró diferencias entre los pacientes con y sin levosimendán en la supervivencia a 1 año tras la inclusión en lista (HR=1,03; IC95%, 0,36-2,97; p=0,958) ni en la supervivencia tras el TxC (HR=0,97; IC95%, 0,60-1,56; p=0,958). Conclusiones: El levosimendán ambulatorio repetitivo como puente al trasplante cardiaco es un tratamiento frecuente y seguro que podría reducir ingresos por IC. (AU)

No differences in HF hospitalizations were found between patients who started levosimendan in the first 30 days after listing and those who did not (33.6% vs 34.5%; P=.848). Among those who did not, 102 patients (32.9%) crossed over to levosimendan after an HF admission. These patients had a rate of 0.57 HF admissions per month before starting levosimendan and 0.21 afterwards. Propensity score matching analysis showed no differences in survival at 1 year after listing between patients receiving levosimendan and those who did not (HR, 1.03; 95%CI, 0.36-2.97; P=.958) or in survival after HT (HR, 0.97; 95%CI, 0.60-1.56; P=.958). Conclusions: Repetitive levosimendan in an ambulatory setting as a bridge to heart transplantation is commonly used, is safe, and may reduce HF hospitalizations. (AU)

Introducción y objetivos: Existe controversia acerca de los resultados del trasplante cardiaco en pacientes con miocardiopatía hipertrófica (MCH) o restrictiva (MCR). Métodos: Análisis retrospectivo de receptores adultos de un primer trasplante cardiaco entre 1984 y 2021 incluidos en un registro nacional. La mortalidad al primer y quinto año postrasplante en receptores con MCH y MCR se comparó con la de receptores con miocardiopatía dilatada (MCD). Resultados: Se incluyó a 3.703 pacientes (3.112 MCD; 331 MCH y 260 MCR) con seguimiento mediano de 5,0 años (3,1-5,0). En comparación con la MCD, el riesgo ajustado de mortalidad a 1 año fue: MCH: hazard ratio (HR)=1,38; intervalo de confianza del 95% (IC95%), 1,07-1,78; p=0,01, MCR: HR=1,48; IC95%, 1,14-1,93; p=0,003. El riesgo ajustado a 5 años fue: MCH: HR=1,17; IC95%, 0,93-1,47; p=0,18; MCR: HR=1,52; IC95%, 1,22-1,89; p<0,001. En los últimos 20 años, la MCR mejoró significativamente la supervivencia a 1 año (R2 ajustada=0,95) y a 5 años (R2=0,88); la MCH mejoró la supervivencia a 5 años (R2=0,59) y a 1 año permaneció estable (R2=0,16). Conclusiones: Se asoció la MCR y la MCH a peor pronóstico precoz postrasplante que la MCD. La diferencia desfavorable se mantuvo para la supervivencia a 5 años solo para la MCR. Se observa una tendencia temporal a mejor pronóstico precoz y tardío para la MCR, y solo para el tardío en la MCH. (AU)

Conclusions: Both RCM and HCM were linked to a less favorable early posttransplant prognosis compared with DCM. However, at the 5-year mark, this unfavorable difference was evident only for RCM. Notably, a substantial temporal enhancement in both early and late mortality was observed for RCM, while for HCM, this improvement was mainly evident in late mortality. (AU)

ABSTRACT

Introducción: El trasplante cardiopulmonar (TCP) ha presentado una disminución progresiva en el número de procedimientos. En nuestro país existe poca información al respecto, siendo el objetivo de este estudio analizar la experiencia de un hospital de referencia. Métodos: Estudio observacional unicéntrico de una cohorte histórica en el periodo entre 1990 y 2021. Las asociaciones entre categorías se evaluaron mediante la prueba de X2 o la f de Fisher. La supervivencia se analizó a través del método de Kaplan-Meier. Las diferencias se evaluaron con el estudio de log-rank y el análisis multivariante con el método de Cox. Resultados: Se observó una reducción del número de procedimientos realizados en el último decenio (2000-2009: 19 [44,2%]; 2010-2021: 15 [34,8%]). La mortalidad posoperatoria precoz fue de 23,3%, reduciéndose a 13,3% a partir del 2010. La intrahospitalaria fue de 41%, disminuyendo a 33% en 2010. Los factores asociados a la mortalidad fueron cirugía torácica previa, corticoterapia, circulación extracorpórea (CEC) mayor a 200 min, tiempo de isquemia mayor a 300 min y dehiscencia traqueal (p < 0,005). La supervivencia global a uno, cinco y 10 años fue de 58, 44,7 y 36,1%, respectivamente. Los factores asociados a menores tasas de supervivencia fueron cirugía torácica previa, donante masculino, CEC mayor 200 min, tiempo de isquemia mayor a 300 min, dehiscencia traqueal y diferencia de pesos (p < 0,005). Conclusiones: Existe una disminución en el número de procedimientos, siendo más evidente en la última década, pero evidenciando una mejora tanto de la mortalidad posoperatoria y supervivencia.(AU)

The main goal of this study is to analyze the experience of a national reference hospital. Methods: We performed a retrospective study of a historical cohort of heart–lung transplanted patients in a single center, during a 30 years period (from 1990 to 2021). The associations between variables were evaluated using the χ2 test or Fisher's exact test. Survival was analyzed using the Kaplan–Meier method. Results: A decrease in the number of procedures performed in the last decade was observed [2000–2009: 19 procedures (44.2%); 2010–2021: 15 procedures (34.8%)]. Early postoperative mortality was 23.3%, falling to 13.3% from 2010. In-hospital mortality was 41%, falling to 33% from 2010. Main factors related to higher mortality: previous thoracic surgery, corticosteroid therapy, extracorporeal circulation (ECLS) greater than 200 min, ischemia time greater than 300 min, and tracheal dehiscence (p < 0.005). Overall survival at one, five, and ten years was 58%, 44.7%, and 36.1%, respectively. Factors associated with lower survival rates: previous thoracic surgery, male donor, extracorporeal circulation greater than 200 min, ischemia time greater than 300 min, tracheal dehiscence and weight difference (p < 0.005). Conclusions: There has been a progressive decrease in the number of heart–lung transplantations, being more evident in the last decade, but showing an improvement in both mortality and survival.(AU)

Introducción y objetivos: El Registro español de trasplante cardiaco actualiza sus datos anualmente. En este artículo se presentan los datos correspondientes al año 2022.Métodos: Se describen las principales características clínicas, del tratamiento recibido y de los resultados en términos de supervivencia de los procedimientos realizados en 2022, así como las tendencias de estos desde el año 2013.Resultados: En 2022 se han realizado 311 trasplantes cardiacos (un 3,0% más que el año anterior). No se han observado cambios relevantes en las características demográficas y clínicas en 2022 respecto a los años inmediatamente anteriores, lo que confirma las tendencias ya descritas en la última década a una disminución de los procedimientos urgentes y el uso de asistencia circulatoria, sobre todo de dispositivos de asistencia ventricular. En el último decenio, las supervivencias son del 81,4 y el 73,4% a 1 año y a los 3 años, con una mejoría numérica que no ha alcanzado significación estadística.Conclusiones: En la última década se observa una estabilización en las características de los procedimientos de trasplante cardiaco y de sus resultados. ABSTRACT

La hemocromatosis es una enfermedad caracterizada por el excesivo depósito de hierro en múltiples órganos, entre ellos hígado, páncreas, piel y corazón. La infiltración de este último es un importante factor en morbilidad y mortalidad. Presentamos un caso de un paciente pediátrico con insuficiencia cardíaca terminal que ameritó trasplante cardíaco, que resultó sin complicaciones. Posterior a la cirugía, mostró mejoría bioquímica y clínica, lo que influyó positivamente en su calidad de vida y prolongó su supervivencia.


Hemochromatosis is a disease characterized by excess iron stores in multiple organs, including the liver, pancreas, skin, and heart. The infiltration of the heart is an important factor in morbidity and mortality. Here we describe the case of a pediatric patient with end-stage heart failure who required a heart transplantation, with no complications. After the surgery, she showed biochemical and clinical improvement, with a positive impact on her quality of life and a prolonged survival.

La persona trasplantada de corazón y la familia experimentan diferentes tipos de transición, y un acompañante facilitador de su formación para una transición saludable es fundamental.La Enfermería de Rehabilitación (ER), debido a sus habilidades, puede ayudar a las personas a recuperar la estabilidad y el bienestar, sin embargo, existe poca difusión de su fundamento en lasteorías de enfermería. Objetivo: Analizar el aporte de la Teoría de las Transiciones en la práctica dela enfermería rehabilitadora orientada al empoderamiento de la persona trasplantada al corazón ya la familia. Metodología: Estudio crítico-reflexivo guiado por la Teoría de las Transiciones de AfafMeleis y por la legislación reguladora para la práctica del enfermero rehabilitador. Resultado: Sepercibe que la teoría de las Transiciones sustenta la intervención de Enfermería de Rehabilitaciónen las áreas de conocimiento y aprendizaje de habilidades de la persona trasplantada cardíaca y sufamilia, conduciendo a su empoderamiento, toma de decisiones y acción, haciendo que la personase sienta empoderada para superar los retos diarios derivados de su proceso de transición. Conclusión: Es fundamental demostrar la conceptualización en la práctica de enfermería, siendo la transición un foco significativo para la Enfermería de Rehabilitación.(AU)

On July 21, 1988, a successful heart transplant was carried out for the first time in Mexico; 34 years later, several hospitals in the country have performed this procedure. We present information and comments on the results obtained within this period at Mexican Social Security Institute La Raza National Medical Center "Dr. Gaudencio González Garza" General Hospital, where 234 transplants have been performed (one out of every three carried out throughout the country), with a short- and mid-term survival comparable to that reported in the rest of the world. This hospital is the only center that performs simultaneous heart-kidney transplantation and elective heart re-transplantation, with favorable results.


El 21 de julio de 1988, en México se realizó con éxito el primer trasplante de corazón; 34 años después, diversos centros hospitalarios del país efectúan este procedimiento. Aquí se presenta información y comentarios de los resultados obtenidos en este periodo en el Hospital General "Dr. Gaudencio González Garza", Centro Médico Nacional La Raza, Instituto Mexicano del Seguro Social, donde se han llevado a cabo 234 trasplantes (uno de cada tres efectuados en el país), con una supervivencia a corto y mediano plazo equiparable a la reportada en el resto del mundo. Ese hospital es el único centro que realiza trasplante simultá de corazón-riñón y retrasplante cardiaco electivo, con resultados favorables.

Exponemos la experiencia del Instituto Nacional de Cardiología de una serie de casos de pacientes sometidos a trasplante cardiaco entre mayo de 2016 y junio 2022. Se realizaron 14 trasplantes, 13 fueron del sexo masculino. La edad osciló entre 19 y 62 años. Las etiologías fueron cardiopatías de tipo idiopática en 57% y valvular en 21%. El 50% se trasplantó en INTERMACS 4 (Interagency Registry for Mechanically Assisted Circulatory Support), 21% INTERMACS 3 y solo 28% en INTERMACS 2. Tres pacientes se trasplantaron con asistencia circulatoria tipo membrana circulación extracorpórea. Las complicaciones más frecuentes fueron las infecciosas. La mortalidad hospitalaria fue 35,7%. Hubo un fallecido en el seguimiento tras 5 años de trasplante.


We present the experience of the National Institute of Cardiology of a series of cases of patients undergoing heart transplantation between May 2016 and June 2022. Fourteen transplants were performed, 13 of the patients were male. The age ranged between 19 and 62 years. The etiologies were idiopathic heart disease in 57% and valvular heart disease in 21%. Fifty percent was transplanted in INTERMACS 4 (Interagency Registry for Mechanically Assisted Circulatory Support), 21% in INTERMACS 3 and only 28% in INTERMACS 2. Three patients were transplanted with membrane type extracorporeal circulation circulatory support. The most frequent complications were infectious. Hospital mortality was 35.7%. There was one patient who died during follow-up after 5 years of transplantation.

Resumen El 21 de julio de 1988, en México se realizó con éxito el primer trasplante de corazón; 34 años después, diversos centros hospitalarios del país efectúan este procedimiento. Aquí se presenta información y comentarios de los resultados obtenidos en este periodo en el Hospital General "Dr. Gaudencio González Garza", Centro Médico Nacional La Raza, Instituto Mexicano del Seguro Social, donde se han llevado a cabo 234 trasplantes (uno de cada tres efectuados en el país), con una supervivencia a corto y mediano plazo equiparable a la reportada en el resto del mundo. Ese hospital es el único centro que realiza trasplante simultáneo de corazón-riñón y retrasplante cardiaco electivo, con resultados favorables.


Abstract On July 21, 1988, a successful heart transplant was carried out for the first time in Mexico; 34 years later, several hospitals in the country have performed this procedure. We present information and comments on the results obtained within this period at Mexican Social Security Institute La Raza National Medical Center "Dr. Gaudencio González Garza" General Hospital, where 234 transplants have been performed (one out of every three carried out throughout the country), with a short- and mid-term survival comparable to that reported in the rest of the world. This hospital is the only center that performs simultaneous heart-kidney transplantation and elective heart re-transplantation, with favorable results.

Objetivo:Descrever as atividades implementadas pelo enfermeiro no processo de captação de coração e pulmão em um centro transplantador brasileiro e sua implicação no aumento do número de transplantes realizados. Métodos: Estudo descritivo e observacional, com foco no organograma instituído sobre as atividades do enfermeiro para avaliação e o aceite do doador. Resultados:Com a formação da unidade, exclusivamente dedicada ao processo captação/transplante, houve entre os meses de agosto de 2013 e dezembro de 2021, um aumento de 138,18% na realização de transplantes cardíacos adultos, 76,54% nos transplantes pediátricos e/ou cardiopatias congênitas e 63,22% nos procedimentos de pulmão,quando comparado ao mesmo período dos anos anteriores.Conclusão: O estudo descreve pela primeira vez a importância da atuação do enfermeiro na implementação de um programa de transplantes de sucesso e como suas ações contribuíram para o aumento do número de procedimentos realizados e podem servir de modelo para outros centros.(AU)


Objective: To describe the activities performed by nurses in the heart and lung harvesting process in a Brazilian transplant center and their implication in the increase in the number of transplants performed. Methods: Descriptive and observational study, focusing on the organizational chart established on the activities of nurses for the evaluation and acceptance of the donor. Results: With the formation of the unit, exclusively dedicated to the capture/transplantation process, between August 2013 and December 2021, there was an increase of 138.18% in adult heart transplants, 76.54% in pediatric transplants and/ or congenital heart disease and 63.22% in lung procedures, when compared to the same period in previous years. Conclusion: The study describes for the first time the importance of nurses' performance in the implementation of a successful transplant program and how their actions contributed to the increase in the number of procedures performed and can serve as a model for other centers.(AU)

Objetivo- Descrever o perfil alimentar de pacientes pós transplante cardíaco (TC) tardio de um centro transplantador brasileiro. Método- Estudo observacional transversal com delineamento descritivo. O estudo ocorreu em uma instituição de ensino público de Minas Gerais. A coleta de dados ocorreu no período de 2017 a 2019. Resultados-. A amostra do estudo foi composta por 62 indivíduos transplantados entre os anos de 2006 a 2016. Aproximadamente 63% era do sexo masculino. A média de idade foi de 46,53 anos. O consumo de carboidratos e gorduras foi adequado em 46,77% e 59,68% da amostra, respectivamente. Já o consumo de proteínas foi acima do recomendado em 77,42% e o consumo de fibras abaixo do recomendado em 79,03%. Conclusão- Houve a predominância do sexo masculino. Os indivíduos apresentaram um consumo adequado somente de carboidratos e gorduras. Não houve na literatura estudos descrevendo a atuação do enfermeiro no processo de nutrição neste cenário(AU)


Objective- To describe the dietary profile of patients after late heart transplantation (HT) from a Brazilian transplant center. Method- Cross-sectional observational study with descriptive design. The study took place in a public education institution in Minas Gerais. Data collection took place from 2017 to 2019. Results-. The study sample consisted of 62 individuals transplanted between 2006 and 2016. Approximately 63% were male. The mean age was 46.53 years. The consumption of carbohydrates and fats was adequate in 46.77% and 59.68% of the sample, respectively. The consumption of proteins was above the recommended in 77.42% and the consumption of fibers below the recommended in 79.03%. Conclusion- There was a predominance of males. The individuals presented an adequate consumption of only carbohydrates and fats. There were no studies in the literature describing the role of nurses in the nutrition process in this scenario(AU)

Método- Estudio observacional transversal con diseño descriptivo. El estudio se llevó a cabo en una institución de educación pública en Minas Gerais. La recolección de datos se llevó a cabo de 2017 a 2019. Resultados-. La muestra del estudio estuvo compuesta por 62 individuos trasplantados entre 2006 y 2016. Aproximadamente el 63% eran hombres. La edad media fue de 46,53 años. El consumo de carbohidratos y grasas fue adecuado en el 46,77% y 59,68% de la muestra, respectivamente. El consumo de proteína estuvo por encima del nivel recomendado en un 77,42% y el consumo de fibra por debajo del nivel recomendado en un 79,03%. Conclusión- Hubo predominio del sexo masculino. Los individuos presentaron un consumo adecuado de solo carbohidratos y grasas. No hubo estudios en la literatura que describieran el papel del enfermero en el proceso de nutrición en este escenario.(AU)

Resumen Introducción: En México se realizan trasplantes de corazón (TC) desde 1988. Objetivo: Revisar la productividad mexicana en TC entre 2006 y 2019 y compararla con la de otros países americanos y de la península ibérica. Métodos: Se recabó la información mexicana de las listas de espera (LE) de TC y de los TC realizados anualmente en el periodo, que se expresaron como tasas por millón de pobladores (pmp); la información de 2019 se comparó con la reportada en América y la península ibérica. Resultados: En el periodo estudiado, los TC en México pasaron de 0.12 pmp en 2006 a 0.25 pmp en 2019 y representaron entre 1 y 2 % de todos los trasplantes de órganos sólidos. Entre 13 países, en 2019 México ocupó el 12° lugar en cuanto a la tasa de TC pmp y el 11° lugar en cuanto a la tasa del número de pacientes registrados por primera vez en la LE para un corazón (0.42 pmp). Entre 2016 y 2019, solo un centro mexicano autorizado alcanzó un volumen superior a 10 TC/año. Conclusiones: Debido a las bajas cifras en los principales indicadores relacionados con el TC, en México urge replantear las políticas de salud en insuficiencia cardiaca y TC.


Abstract Introduction: In Mexico, heart transplants (HTs) have been performed since 1988. Objective: To review Mexican productivity in terms of HT between 2006 and 2019 and compare it with that of American and Iberian Peninsula countries. Methods: Mexican information was collected from HT waiting lists (WL) and from the HTs carried out annually in the period, and was expressed as rates per million population (pmp); 2019 information was compared with that reported at the Pan American and Iberian levels. Results: In the studied period, the rate of HTs in Mexico went from 0.12 pmp in 2006 to 0.25 pmp in 2019, with HTs accounting for between 1 and 2% of all solid organ transplants. Among 13 countries, in 2019 Mexico ranked 12th in the HT rate pmp and 11th in the rate of patients registered for the first time in the WL for a heart (0.42 pmp). Between 2016 and 2019, only one authorized Mexican center reached a volume higher than 10 HT/year. Conclusions: Given the low figures in the main indicators related to HT in Mexico, it is urgent to rethink health policies in heart failure and HT.

La respiración de Cheyne-Stokes es una variante cíclica o tipo de apnea central del sueño poco frecuente en la población pediátrica. Se describe a una paciente de 12 años con insuficiencia cardíaca grave relacionada con miocardiopatía dilatada que demostró trastornos del sueño con características de respiración de Cheyne-Stokes, que se resolvieron completamente después del trasplante cardíaco.


Cheyne-Stokes respiration is a cyclic variant or type of central sleep apnea rare in pediatric population. We describe a 12-year-old female patient with severe heart failure related to dilated cardiomyopathy who demonstrated sleep disorders with Cheyne-Stokes breathing features, which completely resolved following cardiac transplantation.

RESUMO Objetivo Mensurar a não adesão medicamentosa nos pacientes pós-transplante cardíaco mediante o uso da Escala Basel para Avaliação de Aderência a Medicamentos Imunossupressores e Escala Analógica Visual; comparar os resultados das biópsias realizadas, com comorbidades prevalentes e sobrevida. Método Coorte histórica de abordagem quantitativa. A população foi composta de pacientes transplantados no período de 2009 a 2016. Resultados Participação de 60 pacientes. A mensuração da não adesão por meio do instrumento Escala Basel para Avaliação de Aderência a Medicamentos Imunossupressores foi de 46,7% e adesão de 53,3% dos pacientes. O grupo com maior dificuldade de não adesão foi aquele com relato de atraso de até 2 horas do prescrito (25%), porém, sem interrupção nas medicações. O diagnóstico inicial foi Doença de Chagas (33,3%). As comorbidades estudadas foram hipertensão arterial sistêmica, diabetes mellitus (DM), dislipidemias e insuficiência renal crônica. Conclusão A avaliação por meio da Escala Basel para Avaliação de Aderência a Medicamentos Imunossupressores verificou não adesão medicamentosa de 46,7% dos pacientes transplantados cardíacos. A Escala Analógica Visual pelo autorrelato do paciente e avaliação do enfermeiro apresentaram valores elevados (93,3% vs 83,3%). A Escala Basel para Avaliação de Aderência a Medicamentos Imunossupressores tende a se aproximar das dificuldades informadas pelos pacientes, quanto há alteração de doses, atrasos ou antecipações de horário e dose.


RESUMEN Objetivo Medir la falta de adherencia a la medicación en pacientes después de un trasplante de corazón utilizando la Basel Assessment of Adherence to Immunosuppressive Medications Scale (BAASIS) y la Escala Visual Analógica (EVA); comparar los resultados de las biopsias realizadas con las comorbilidades prevalentes y la supervivencia. Método Cohorte histórica con un enfoque cuantitativo. La población consistió en pacientes trasplantados de 2009 a 2016. Resultados Participación de 60 pacientes. La medición utilizando la BAASIS fue del 46,7% para no adherencia y del 53,3% para la adherencia de los pacientes. El grupo con la mayor dificultad en la no adherencia fue de aquellos con un retraso de hasta 2 horas en la ingesta de medicamentos desde el tiempo prescrito (25%), aunque sin interrupción en los medicamentos. El diagnóstico inicial fue enfermedad de Chagas (33,3%). Las comorbilidades estudiadas fueron hipertensión arterial sistémica, diabetes mellitus, dislipidemia e insuficiencia renal crónica. Conclusión La evaluación con BAASIS mostró la no adherencia a la medicación en el 46,7% de los pacientes con trasplante de corazón. La EVA según el autoinforme de los pacientes y la evaluación de los enfermeros mostró valores altos (93,3% frente a 83,3%). La BAASIS tiende a abordar las dificultades reportadas por los pacientes, cuando hay un cambio en las dosis, retrasos o anticipaciones de tiempo y dosis.


ABSTRACT Objective To measure medication non-adherence in patients after heart transplantation using the Basel Assessment of Adherence to Immunosuppressive Medications Scale (BAASIS) and the Visual Analogue Scale (VAS); to compare the results of biopsies performed with the prevalent comorbidities and survival. Method Quantitative historical cohort. The population consisted of patients undergoing transplantation between 2009 and 2016. Results Participation of 60 patients. The measurement using the BAASIS was 46.7% of non-adherence and 53.3% of patient adherence. The group with greater difficulty in non-adherence reported up to 2 hours delay of medication intake in relation to the prescribed time (25%), although there was no interruption in medications. The initial diagnosis was Chagas disease (33.3%). The studied comorbidities were systemic arterial hypertension (SAH), diabetes mellitus (DM), dyslipidemia (DLP) and chronic renal failure (CRF). Conclusion Assessment using the BAASIS showed medication non-adherence in 46.7% of heart transplant patients. The VAS according to patients' self-report and nurse's assessment showed high values (93.3% vs 83.3%). The BAASIS tends to address the difficulties reported by patients, when there is a change in doses, delays or anticipations of time and dose.

ABSTRACT

Los programas de rehabilitación cardíaca (RC) disponen de una sólida evidencia científica en cuanto a beneficios en casos de pacientes adultos en situación de pre y post trasplante. Sin embargo, hasta el momento actual, no existen estudios de rehabilitación pretrasplante en edad infantil y se desconocen sus efectos. El objetivo de nuestro artículo es presentar los resultados de un programa de rehabilitación cardíaca en el caso de una paciente en lista de espera de trasplante.


Cardiac rehabilitation (CR) programs have solid scientific evidence regarding their benefits in adult patients in pre- and post-transplant situations. However, up to the present time, there are no studies of pretransplant rehabilitation in children and its effects are unknown. The aim of our article is to present the results of a cardiac rehabilitation program in the case of a patient on the transplant waiting list.

RESUMEN Introducción: La biopsia endomiocárdica es un procedimiento de mucha utilidad para valorar el rechazo agudo en pacientes trasplantados, sin embargo, puede causar complicaciones. Objetivo: El propósito de este estudio es demostrar la eficacia y seguridad del ecocardiograma como guía para la biopsia endomiocárdica. Material y métodos: Se realizó un análisis retrospectivo de pacientes trasplantados sometidos a biopsia endomiocárdica entre noviembre de 2004 y noviembre de 2016. Se analizaron datos demográficos y complicaciones mediante estadística descriptiva. Resultados: Un total de 200 pacientes se sometieron a 2665 biopsias endomiocárdicas, con un promedio de 13 procedimientos por paciente. Se produjeron complicaciones en 5 pacientes: 2 neumotórax (0,1%), 3 alteraciones transitorias en el electrocardiograma (0,1%). No se logró acceso vascular en 25 pacientes (0,9%). Ningún paciente falleció durante el procedimiento o requirió cirugía de urgencia para tratar las complicaciones. Conclusión: La biopsia endomiocárdica guiada por ecocardiografía es un procedimiento seguro y con baja tasa de complicaciones.


ABSTRACT Background: Endomyocardial biopsy is a very useful procedure to assess acute rejection in transplant patientis, but it can cause several complications. Objective: The purpose of this study is to demonstrate the efficacy and safety of echocardiography as a guide for endomyo-cardial biopsy sampling. Methods: A retrospective analysis of transplant patientis undergoing endomyocardial biopsies was carried out between No-vember 2004 and November 2016. Demographic data and complications were analyzed with descriptive statistics. Resultis: A total of 200 patientis underwent 2,665 endomyocardial biopsies, with an average of 13 procedures per patient. Complications occurred in 5 patientis: 2 pneumothorax (0.1%) and 3 transient alterations in the electrocardiogram (0.1%), and vascular access could not be achieved in 25 patientis (0.9%). No patient died during the procedure or required emergency surgery to treat the complications. Conclusion: Endomyocardial biopsy guided by echocardiography is a safe procedure with a low rate of complications.
