ABSTRACT

OBJECTIVE: This study was designed to determine the efficacy of minocycline hydrochloride ointment (MHO) combined with Vitapex paste on middle-aged and elderly patients with combined periodontal-endodontic lesions (CPELs) and its effect on the inflammatory factors. METHODS: The data of 88 elderly patients with CPELs treated in the First Affiliated Hospital of Gannan Medical University from March 2020 to March 2022 were analyzed retrospectively. Among them, the patients treated with MHO and iodoform zinc oxide clove oil paste were assigned into the control group (n = 42) and the rest of the patients treated by MHO and Vitapex paste were assigned to the study group (n = 46). The inflammatory factors, periodontal indexes and efficacy were determined and compared between the two groups. The MOS 36-Item Short-Form Health Survey (SF-36) was adopted to evaluate the quality of life (QoL) of patients before and after treatment. Additionally, the adverse reactions of the two groups during treatment were analyzed and compared. The prognosis of the two groups of patients was analysed, and factors impacting their prognosis was analysed through the Logistic regression analysis. RESULTS: Before treatment, the levels of interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and interleukin-1ß (IL-1ß) were not significantly different between the two groups (all P > 0.05), while after treatment, these levels in both groups decreased significantly, with notably lower levels in the study group than those in the control group (all P < 0.05). Before treatment, the gingival index (GI), plaque index (PLI), probing depth (PD) and bleeding index (BD) of the two groups were similar (all P > 0.05); however, after treatment, the levels of GI, PD, PLI and BI of both groups decreased significantly (all P < 0.05), with more notable decreases in the study group than those in the control group (all P < 0.05). The study group showed a significantly higher overall response rate than the control group (P < 0.05). Before treatment, the SF-36 scores of the two groups were not significantly different (P > 0.05), while after treatment, both groups had significantly increased SF-36 scores, and the score in study group was significantly higher than the control group (P < 0.05). In addition, the incidence of adverse reactions was not notably different between the two groups (P > 0.05). According to univariate analysis, age, dental lesion grade, course of disease, and persistent dull pain were the risk factors affecting the prognosis of patients. According to multivariate analysis, dental lesion grade was the independent risk factor affecting the prognosis. CONCLUSION: MHO combined with Vitapex paste is effective for middle-aged and elderly patients with CPELs, which can effectively inhibit the patients' inflammatory reaction and improve their periodontal condition and QoL, with a low adverse reaction rate, so it is worthy of clinical promotion.

ABSTRACT

To compare the efficiency of conventional needle irrigation, passive ultrasonic irrigation, sonic irrigation and XP-endo Finisher in removing Vitapex paste from root canals. The root canals of human single-rooted teeth were prepared and obturated with Vitapex paste. After 2 weeks, the 48 teeth were randomly allocated into four groups: conventional needle irrigation (CNI) group, passive ultrasonic irrigation (PUI) group, sonic irrigation (SI) group and XP-endo Finisher (XP) group. The specimens of four groups were scanned using a micro-computed tomography after the Vitapex pastes was removed. The results showed that more residual paste was left in the CNI group than in the other three groups (p < 0.05). There was no significant difference among the PUI, SI and XP group (p > 0.05). And, the majority of the residual Vitapex was found in the apical third, with a small amount in the middle third after using any of the three agitated irrigation techniques.

BACKGROUND: Pulpotomy has been successfully performed in immature and mature permanent teeth with irreversible pulpitis but rarely in primary teeth. AIM: To evaluate the outcomes of iRoot BP Plus pulpotomy and Vitapex pulpectomy in primary molars with irreversible pulpitis. DESIGN: We selected 130 primary molars of 99 patients, aged 3-7 years, diagnosed with irreversible pulpitis with coronal pulp tissue and treated with iRoot BP Plus pulpotomy or Vitapex pulpectomy (median follow-up period: 18 months). They were divided into the pulpotomy (n = 88) and pulpectomy (n = 42) groups according to treatment procedure. The pulpotomy group was further divided into asymptomatic (n = 46) and symptomatic (n = 42) subgroups according to preoperative symptoms. The chi-squared test and Cox regression were performed to analyze the outcomes. RESULTS: Clinical and radiographic success rates were significantly higher in the pulpotomy group (98.9% and 95.5%) than in the pulpectomy group (88.1% and 54.8%) and did not differ significantly between asymptomatic and symptomatic pulpotomy subgroups. CONCLUSION: Irreversible pulpitis of primary molars with coronal pulp tissue can be successfully treated with iRoot BP Plus pulpotomy. Early intraradicular resorption of materials is the main adverse outcome of Vitapex pulpectomy.

BACKGROUND/PURPOSE: ProRoot MTA, Vitapex, and Metapex are widely used for pulp treatment of primary tooth. The aim of this study was to compare the pulpal responses to ProRoot MTA, Vitapex, and Metapex in a canine model of pulpotomy. MATERIALS AND METHODS: Pulpotomy procedure was performed to 34 teeth (21 incisors and 13 premolars) and ProRoot MTA, Vitapex or Metapex was applicated to artificially exposed pulp tissues. After 13 weeks, the teeth were extracted and processed with hematoxylin-eosin staining for histologic evaluation. All specimens were evaluated in several categorys related to calcific barrier, inflammatory responses and the area of calcific barrier formation was measured. RESULTS: Most of the specimens in the ProRoot MTA group developed a calcific barrier at the pulp amputation site and showed a low level of inflammatory response. However, in comparison to ProRoot MTA group, a small amount of calcific barrier formed in Vitapex and Metapex groups. CONCLUSION: This in vivo study found that Vitapex and Metapex induced similar pulpal responses but showed poor outcomes compared with using ProRoot MTA. Vitapex and Metapex are therefore not good substitutes for ProRoot MTA in direct pulp capping and pulpotomy.

ABSTRACT

BACKGROUND: Vitapex has been a popular obturating material for several decades. In recent times, lesion sterilization and tissue repair (LSTR) has shown promising results. This technique uses a mixture of 3 antibiotics for sterilization of the root canals and healing of periradicular tissues. OBJECTIVE: The objective of this systematic review was to compile all the literature comparing Vitapex and LSTR for pulpally involved primary teeth and evaluate the success rate in terms of clinical and radiographic outcomes. MATERIALS AND METHODS: This review is based on PRISMA guidelines. The electronic search on MEDLINE via PubMed database and Google scholar, cross-referencing and hand search of journals was carried out for articles from January 1, 2000 to December 31, 2019. Articles only in the English language were selected. Out of the 17 articles, 3 unique articles were identified for the review, of which 2 were randomized controlled trials and 1 was a retrospective study. RESULTS: All the three articles were assessed for their quality and all had a low risk of bias. It was found that LSTR and Vitapex had a high clinical success after 12 months. The radiographic success after 6 months was high but decreased after 12 months in both the groups. CONCLUSION: There is no difference in the success rate of LSTR as compared to Vitapex for the treatment of pulpally involved primary teeth.

ABSTRACT

OBJECTIVE: To evaluate the clinical and radiographic success rates of 3Mix and Vitapex in the treatment of necrosed primary teeth. MATERIALS AND METHODS: Seventy teeth of healthy children aged 4-9 years with necrotic primary molar (nonvital) were treated with 3Mix (lesion sterilization tissue repair therapy) and Vitapex before restoration with stainless steel crowns. The participants were followed up clinically and radiographically for 3 months and 6 months, respectively. The outcome was compared using Fisher exact test with a significance level of p < 0.05. RESULTS: Clinical success rate of 3Mix group at 3 months and 6 month follow-up was 100% and 97.14% and that of Vitapex group was 100% and 100%, respectively. The radiographic success rate of 3Mix group at 3 months and 6 months was 74.29% and 77.14% and of Vitapex group was 97.14% and 97.14%, respectively. Considering the clinical success, no statistical difference was found between the two groups at the end of 6 month (p = 1.000). But radiographically statistical significant difference was found between the two groups (p = 0.028). CONCLUSION: Non-instrumentation endodontic treatment using 3Mix has shown a good clinical success of 97.14% but the radiographic success rate at 6 months was lower (77.14%). So it can be suggested that the use of 3Mix antibiotic treatment cannot replace conventional root canal treatment over the long term. ABSTRACT

INTRODUCTION: Calcium Hydroxide (CH) is one of the most commonly used intracanal medicaments which can be used with various vehicles. AIM: The aim of this in vitro study was to evaluate the effect of three CH based intracanal medicaments on the apical sealing ability of AH Plus - guttapercha obturation. MATERIALS AND METHODS: Crowns of 100 extracted single rooted human teeth were sectioned at the Cemento-Enamel Junction (CEJ) to a standardized length. The root canals were instrumented upto ISO size 40 using step back technique and the specimens were randomly divided into two control and four experimental groups. The control groups were not medicated. Specimens in positive control group (Group I) were obturated with guttapercha without placing sealer and in negative control group (Group II) were obturated with guttapercha and AH Plus sealer. Among the experimental groups, specimens of Group III were not medicated while groups IV, V and VI were medicated with CH-saline, CH-2% Chlorhexidine (CHX) and Vitapex respectively for a period of 14 days. The medicaments were removed from the specimens and the teeth were obturated with AH Plus sealer and guttapercha using lateral compaction technique. The specimens were immersed in India ink dye, demineralized and diphanized. The extent of dye penetration was assessed using a 10X stereomicroscope. Data obtained was statistically analyzed by one-way ANOVA (p<0.05) followed by Post-hoc Tukey test. RESULTS: Amongst the three CH medicaments, CH-2% CHX when used as an intracanal medicament showed a significantly higher microleakage as compared to the other groups with p<0.001. The microleakage values between the remaining groups were not statistically significant. CONCLUSION: Under the conditions of this study it was concluded that all groups with or without intracanal medicament showed apical leakage. The vehicle used to carry CH may significantly influence the apical sealing ability of guttapercha - AH Plus obturated canals.

OBJECTIVES: The aim of this study was to evaluate the clinical and radiographic success of zinc oxide (ZnO)-ozonated oil, modified 3Mix antibiotic paste, and vitapex in the treatment of primary molars requiring pulpectomy. METHODS: Sixty-four primary molars of forty-three healthy children aged between 4 and 8 years with primary molars requiring root canal procedure were treated with ZnO-ozonated oil, modified 3Mix-MP antibiotic paste, and vitapex. Clinical follow up was done at 1, 6,12 months and 18 months while radiographical follow-up was done at 6,12 and 18 months, respectively. RESULTS: The results showed that the clinical success rates of ZnO-ozonated oil, modified 3Mix-MP paste and vitapex were 95.5%,89.5% and 100% respectively and radiographical success rates were 94.4%,80.95% and 100% respectively after 18 months period of observation. CONCLUSION: The overall success rates of ZnO-ozonated oil, vitapex and modified 3Mix antibiotic paste were comparable.

ABSTRACT

Objective:To investigate the effect of Vitapex paste in the root canal for deciduous teeth with chronic apical periodontitis.Methods:200 children with chronic apical periodontitis were selected and divided into two groups.The observation group (108 cases)was given Vitapex paste.The control group (92 cases) was given Zinc oxide iodoform paste.The effect of Vitapex paste in root canal for deciduous teeth with chronic apical periodontitis was evaluated by filling quality after 1 month'treatment,the VAS scores before and after treatment,long-term curative effect were compared between two groups.Results:After 1 month'treatment,the suitable filling rate of observation group was higher than that of the control group(P<0.05).There was no statistical significance in the VAS scores between two groups (P>0.05).After 1 week'and 1 month'treatment,the pain feeling were alleviated in two groups and significantly lighter in the observation group(P<0.05).After 6 months' follow-up,the success rate was 96.3% in the observation group and 87.0% in the control group,the success rate of observation group was higher than that of the control group (P<0.05).Conclusion:The filling process of Vitapex paste was simple and easy,which had a good therapeutic effect on chronic apical periodontitis of deciduous teeth and could relieve the toothache of patients.

BACKGROUND: Apical periodontitis (AP) is one of the most prevalent diseases of the teeth. Treatment of AP is based on the removal of the cause, i.e., bacteria from the root canals. Achievement of adequate bacterial eradication in one appointment treatment remains a controversy. AIM: This prospective study was conducted with the objective to compare the periapical healing of teeth with AP treated in (a) single visit versus (b) two visits, either with or without Vitapex as an intracanal medicament. SUBJECTS AND METHODS: Patients were selected randomly from the Department of Conservative Dentistry and Endodontics. Forty-three patients (81 teeth) met the inclusion criteria, i.e., AP (both symptomatic and asymptomatic) visible radiographically size ≥2 mm × 2 mm, not suffering from any immune-compromising disease, age between 16 and 65 years and tooth not accessed previously. Patients were randomly divided into three groups, i.e., single-visit group (Group 1), multi-visit group without any intracanal medicament (Group 2), and multi-visit group with Vitapex as interim intracanal medicament (Group 3). Comparison was done radiographically using periapical index (PAI). The primary outcome measure was the change in periapical radiolucency after 1 year assessed by PAI scores. The Mann-Whitney U-test was used to evaluate differences between groups at baseline (immediate postoperative) and at the 12-month follow-up evaluation. Change in PAI score for each group from baseline to 12-month follow-up evaluation was tested with the Wilcoxon signed rank test. The secondary outcome measures, proportion of teeth in each group that could be considered improved (decreased PAI score) or healed (PAI <2), were assessed with the Chi-square test. RESULTS: No statistically significant difference in periapical healing was found between three groups. CONCLUSION: After 1-year evaluation, no difference in periapical healing was found between single-visit treatment and multi-visit treatment groups with the given sample size.

ABSTRACT

AIM: To evaluate the outcome of root canal treatment in primary teeth using three root canal filling materials - RC Fill, Vitapex and Pulpdent root canal sealer. METHODOLOGY: The study was a single-centre, double-blind, randomized controlled trial carried out on 129 primary mandibular molars with necrotic pulps or irreversible pulpitis in 4- to 9-year-old children. Participants were selected based on specific inclusion and exclusion criteria and were randomly allocated into 3 groups: Group I - RC Fill [zinc oxide-eugenol (ZOE) with iodoform]; Group II - Vitapex (calcium hydroxide with iodoform); and Group III - Pulpdent root canal sealer (ZOE). The outcome measures were evaluated both clinically and radiographically at 6, 12 and 30 months according to modified American Association of Endodontists (AAE) criteria. The radiographic outcomes were assessed by two blinded and calibrated evaluators. Pearson's chi-square analysis was performed for both intention-to-treat (ITT) and per-protocol population. RESULT: The success rates of RC Fill, Vitapex and Pulpdent were 94%, 90% and 97%, respectively, at 30 months and the differences were not significant (P = 0.137). CONCLUSION: All three materials were found to be equally effective root filling materials for primary molars with necrotic pulps and irreversible pulpitis at 30 months. However, long-term follow-up until the eruption of the permanent successor teeth is needed for more definitive assessments.

ABSTRACT

Objective To observe the clinical curative effect of the joint application of Vitapex paste and AH plus,which cured the combined periodontic -endodontic lesion .Methods 72 patients of 113 teeth with the com-bined periodontic-endodontic lesion were chosen ,which were randomly divided into the two groups ,with all complete periodontal initial therapy and routine root canal preparation and dressing at the same time .The observation group included 36 cases of 58 teeth,when the first time to see a doctor ,injected Vitapex paste to the periapical tissue and periodontal tissue sparse area for 1 -2weeks.The sceond time,the tap root canal was closely filled withc AH plus paste and gutta-percha.The control group concludes 36 cases of 55 teeth,with the traditional method,the tap root canal was filled with AH plus paste and gutta -percha directly .Then observation of postoperative emergency response within 1 week,of periodontal clinical indicators (including the PD,BOP,PLI) in 1 month,3 months,6 months after periodontal initial therapy ,and of the long-term curative effect after 1 year.Results 1 week after root canal filling , there were 48 cases no reaction , 9 cases mild reaction and 1 case severe reaction in the observation group , while 47cases no reaction ,7 cases mild reaction and 1 case severe reaction in the control group .There was no statistical difference of evaluation of emergency response between the two groups (χ2 =0.002,P>0.05).Compared with the situations before treatment ,PD,BOP and PLI were all reduced significantly in 1,3 and 6 months after periodontal ther-apy (P0.05),while 1 month (t=2.581,5.984,1.870,all P<0.05),3 months (t=4.806,7.640,1.034,all P<0.05) and 6 months (t=4.612,8.043,1.167,all P<0.05) later,there was statistical difference of decreased value between the two groups .The 1 year follow up showed that significant efficiency in the observation group was 46.55%,and total efficiency was 91.38%,while significant efficiency 27.27% and total effi-ciency 70.91%in the control group ,and there were statistical differences of significant efficiency and total efficiency between the two groups (χ2 =16.761,21.061,all P<0.05).Conclusion With the joint application of Vitapex paste and AH plus ,and two method curing the combined periodontic -endodontic lesion ,there was no evident differ-ence compared with the traditional short -term emergency response .By long-term observation ,found that this method could effectively promote the healing of periapical tissue and periodontal tissue ,and that curative effect is remarkable compared with the traditional method .

Objective To compare clinical effect of MTA and Vitapex for young permanent teeth with periapical incomplete apical closuret . Method Teeth 90 were chosen from 80 patients at 7-25 years old young permanent teeth with periapical and randomly divided into MTA group and Vitapex group 45 in each group.Two groups were treated with MTA and Vitapex paste treatment respectively.6-month and 12-month after treatment followed up, observed clinical efficacy of two groups.Results The six-month recall, X-ray ( MTA group 93.33%, Vitapex group 88.89%) and clinical parameters (MTA group 95.56%, Vitapex group 86.67%) success rate of two group was no significant difference.When the 12-month followed up, X-ray examination of MTA group's success rate was 93.33%, significantly higher than 68.89%Vitapex group (P<0.05), clinical indicators of success rate of 95.56%MTA group,significantly higher than the 71.11%Vitapex group (P<0.05).Pain during root canal therapy (EIP) of the occurrence, MTA group than Vitapex group (P<0.05).Conclusion The effect of short-term treatment of MTA in treatment of young permanent teeth with periapical incomplete apical closure is definite,it is an ideal induce apical molding material.

INTRODUCTION: Calcium hydroxide (Ca[OH]2) microcapsules were synthesized to allow controlled release of Ca(OH)2. The aim of this study was to evaluate the influence of Ca(OH)2 microcapsules on osteoprotegerin (OPG) activity, receptor activator of nuclear factor kappa B ligand (RANKL) activity, and the OPG/RANKL ratio compared with pure Ca(OH)2 powder and Vitapex (Neo Dental Chemical Products Co Ltd, Tokyo, Japan). METHODS: One formula of Ca(OH)2 microcapsules was evaluated, and pure Ca(OH)2 powder was used as a control. A commonly used Ca(OH)2 medication containing an oily vehicle (Vitapex) was also evaluated, and the in vitro release profile of Vitapex was studied. The human osteosarcoma cell line MG63 was used to evaluate the influence of Ca(OH)2 microcapsules, pure Ca(OH)2 powder, and Vitapex on OPG and RANKL activity. The relative messenger RNA (mRNA) expression of OPG and RANKL was determined by real-time polymerase chain reaction. The protein expression of OPG and RANKL in supernatants was measured using enzyme-linked immunosorbent assay. RESULTS: Vitapex prolonged the release of Ca(OH)2 compared with pure Ca(OH)2 powder, and the release rate of Vitapex was faster than that of the microcapsules. The OPG/RANKL ratio in the microcapsules group was up-regulated at both the mRNA and protein levels compared with the negative control group and the pure Ca(OH)2 powder group. The ratio in the Vitapex group was lower than the microcapsule group both at the mRNA and protein levels. CONCLUSIONS: Ca(OH)2 microcapsules increased the expression of OPG although they did not increase the expression of RANKL compared with pure Ca(OH)2 powder and Vitapex. This increase in expression led to an increase in the OPG/RANKL ratio and eventual inhibition of osteoclast activity.

Objective To analysis 2 years follow-up clinical effects in endodontic lesion combined periodontal in treatment with Vitapex .Methods 112 patients with periodontal endodontic lesion combined periodontal were randomly divided into control group (56 cases ,75 teeth) and study group(56 cases ,77 teeth) .Treatment of Zinc oxide glycerin paste combined with gutta percha filling root canal was used in control group ,while Vitapex combined with gutta percha filling root canal was used in study group .After 2 years ,the clinical data of the two groups were analysed .Results The total effective rate of anterior and posterior in study group were significant higher than in control group(P<0 .05) .The total effective rate of of I type ,II type and III type lesions in study group were higher than in control group(P<0 .05) .Conclusion Treatment of Vitapex combined with gutta percha filling root canal in endodontic lesion combined periodontal has better clinical effectiveness to promote periapical and periodontal tissue healing than control group .

ABSTRACT

Objective To investigate the clinical effect of vitapex paste in the treatment of deciduous teeth with periapical inflammation.Methods 112 children with chronic apicitis of deciduous teeth were divided into two groups randomly and averagely.The research group used vitapex paste,while the control group used zinc oxide eugenol paste.The clinical effects of the 2 groups were compared.Results Compared to the control group,the response to root canal filling of the research group was much weaker ( 10.7% to 25.0% ),which had statistical difference ( x2 =3.02,P =0.0 1 1 ).There wasalso significant difference betweenthe success rateofthetwogroups after 1 year followed up( x2 =5.16,P =0.004).Conclusion sing vitapex paste for root canal filling had a higher success rate and a weaker respose to root canal filling.

ABSTRACT

Objective To evaluate the clinical effect of Vitapex paste in the treatment of chronic periapical periodontitis.Methods Seventy patients who suffer from chronic periapical periodontitis were selected randomly.Their root canals were filled with Vitapex paste for almost one year.Results 68 teeth of the 70 patients received succesful results,and the success rate reached 97%.Conclusion The application of Vitapex paste in the treatment of chronic periapical periodontitis not only helps the healing of the inflammation,but also leads to a good closure of apical foramen.

ABSTRACT

Objective:To evaluate the effect of Vitapex in root canal therapy (RCT) for infected permanent teeth.Methods:159 permanent teeth with chronic periapical periodontitis were treated by RCT, and the canals were filled with Vitapex (in 54 teeth),ZOE (in 52 teeth) and ZOE plus iodoform (in 53 teeth) respectively.The effects were evaluated by clinical and radiological observation.Results:The ratio (%) of apical tissue reaction in 3 days following RCT in the group of Vitapex,ZOE and ZOE plus iodoform was 14.8,59.6 and 50.9 (P0.05) respectively.Conclusion:Vitapex is effective in the treatment of infected root canals.
