ABSTRACT

La mayoría del consumo de productos de tabaco comienza durante la adolescencia y los productos de tabaco han evolucionado para incluir varios productos como los cigarrillos electrónicos. Los cigarrillos electrónicos resultan muy atractivos entre los jóvenes principalmente por la disponibilidad de sabores, la exposición a la comercialización de productos de tabaco, la curiosidad y las percepciones erróneas sobre su seguridad u poca repercusión en la salud en comparación con el daño causado por el uso de cigarrillos convencionales. Se ha encontrado asociaciones entre el uso de cigarrillos electrónicos y el uso posterior de cigarrillos convencionales en adolescentes y también se describe una fuerte asociación entre estos dispositivos y la utilización subsecuente de alcohol, marihuana, y otras drogas. La regulación de los sistemas electrónicos de administración de nicotina varía significativamente entre países. La Organización Mundial de la Salud (OMS) recomienda a los gobiernos crear reglamentaciones para evitar que los no fumadores empiecen a consumir estos productos, impedir que el hábito de fumar vuelva a normalizarse en la sociedad y proteger a las generaciones futuras. Most tobacco product use begins during adolescence and tobacco products have evolved to include various products such as e-cigarettes.E-cigarettes are very attractive among youth mainly because of the availability of flavors, exposure to tobacco product marketing, curiosity, and misperceptions about their safety or low health impact compared to the harm caused by conventional cigarette use. Associations have been found between e-cigarette use and subsequent use of conventional cigarettes in adolescents, and a strong association between these devices and subsequent use of alcohol, marijuana, and other drugs is also described.Regulation of electronic nicotine delivery systems varies significantly among countries. The World Health Organization (WHO) recommends that governments create regulations to prevent non-smokers from starting to use these products, to prevent smoking from becoming normalized again in society, and to protect future generations. ABSTRACT

Los tumores de ovario en la edad pediátrica son raros, representan 1-5 % de los tumores infantiles, con una incidencia anual de 2,6 casos por cada 100.000 pacientes. La mayoría son benignos y se tratan de quistes funcionales, sin embargo, entre 10-20 % son malignos y generalmente se presentan en adolescentes; estos últimos, se dividen en 3 grupos: tumores epiteliales, germinales, y estromales o de células sexuales. Método: Estudio retrospectivo de tipo transversal, observacional, no experimental. Se analizaron los pacientes con diagnóstico de tumor de ovario, ingresados en el Servicio de Cirugía Pediátrica del Hospital de Niños "Dr. José Manuel de los Ríos", entre el 1 de enero de 2017 y 01 de julio de 2022. Resultados: 18 pacientes incluidos en el estudio, con edad media de 8,23 años (DE 4,77); los síntomas más frecuentes presentados al momento del ingreso fueron: aumento de volumen abdominal (52,94 %, 9 pacientes), y dolor abdominal (35,29 %, 6 pacientes), entre otros. Reporte patológico: 2 pacientes con quistes de ovario funcional (11,76 %) y 16 pacientes con tumor neoplásico (88,23 %), de los cuales 8 fueron germinales (53,33 %), 5 tumores epiteliales (33,33 %) y 2 pacientes con linfoma (13,33 %). Conclusión: Los tumores de ovario en general tienen una edad promedio de presentación de 8 años y los tumores neoplásicos se presentaron principalmente en adolescentes, siendo el tipo histológico más frecuente el tumor germinal y dentro de este grupo el teratoma quístico maduro. Ovarian tumors in pediatric age are rare, representing 1-5 % of childhood tumors, with an annual incidence of 2.6 cases per 100,000 patients. Most of them are benign and functional cysts; however, between 10-20 % are malignant and generally occur in teenagers; the latter are divided into 3 groups: epithelial, germinal, and stromal or sex cell tumors. Methods: Retrospective, cross-sectional, observational, non-experimental study. Patients with a diagnosis of ovarian tumor, admitted to the Pediatric Surgery Service of the Children's Hospital "Dr. José Manuel de los Ríos", between January 01, 2017 and July 01, 2022, were analyzed. Results: 18 patients included in the study, with mean age 8.23 years (SD 4.77); the most frequent symptoms presented at admission were: increased abdominal volume (52.94 %, 9 patients), and abdominal pain (35.29 %, 6 patients), among others. Pathological report: 2 patients with functional ovarian cysts (11.76 %) and 16 patients with neoplastic tumor (88.23 %), of which 8 were germinal (53.33 %), 5 epithelial tumors (33.33 %) and 2 patients with lymphoma (13.33 %). Conclusion: Ovarian tumors in general have an average age of presentation of 8 years and neoplastic tumors occurred mainly in teenagers, the most common histological type being the germ cell tumor and within this group the mature cystic teratoma. ABSTRACT

OBJECTIVE: The aim of the study was to describe how physical exercise affects metabolic control, insulin requirements and carbohydrate intake in children who use hybrid closed-loop systems. METHODS: Cross-sectional study design. The sample included 21 children and adolescents diagnosed with type 1 diabetes. During the study, participants were monitored for a period of 7 days to gather comprehensive data on these factors. RESULTS: Nine participants (42.9%) had switched to exercise mode to raise the target glucose temporarily to 150 mg/dL. The HbA1c values ranged from 5.5% to 7.9% (median, 6.5%; IQR, 0.75). The percentage of time within the target range of 70-180 mg/dL was similar; however, there was an increased duration of hyperglycaemia and more autocorrections on exercise days. The time spent in severe hyperglycaemia (>250 mg/dL) increased by 2.7% in exercise compared to non-exercise days (P = .02). It is worth noting that hypoglycaemic episodes did not increase during the exercise days compared with non-exercise days. CONCLUSION: The hybrid closed-loop system was effective and safe in children and adolescents with type 1 diabetes during the performance of competitive sports in real life.

ABSTRACT

INTRODUCTION AND OBJECTIVES: Selective serotonin reuptake inhibitors (SSRIs) are medications used in child and adolescent psychiatry mainly for the treatment of depression, anxiety and obsessive compulsive disorder. In general, these medications are safe and well tolerated. However, they can cause adverse effects such as activation syndrome, which if not identified can negatively affect adherence and response to treatment. Activation syndrome has received little attention and can be difficult to recognise due to the lack of a clear definition and objective diagnostic measures, and also because it can be confused with a worsening of the psychiatric disorder or mania triggered by the antidepressants. For all the above, it is important that professionals who prescribe antidepressants in the paediatric population are able to identify and manage activation syndrome when it occurs. Our aim was to carry out a narrative review of activation syndrome in children and adolescents treated with SSRIs in terms of definition, prevalence, pathophysiology, associated factors, relationship with suicide risk, management strategies and recommendations for reducing the risk of suicidal behaviours when using antidepressants in this population. METHODS: We performed a non-systematic narrative review of activation syndrome in children and adolescents which involved finding information in PubMed, Ovid, EBSCO, ProQuest and Embase. Review articles, prospective and retrospective investigations, systematic reviews, meta-analyses and other articles related to activation syndrome in children and adolescents were selected. The search was limited to studies published in English and Spanish that involved children and adolescents and no limits were applied to the publication date or study design. RESULTS: A total of 62 articles were included, 61 of them in English. The results were grouped into the following topics: definition; prevalence; pathophysiology; associated factors; relationship with suicide risk; management strategies; and recommendations for reducing the risk of suicidal behaviours when using antidepressants in this population. Activation syndrome refers to a set of symptoms consisting of impulsiveness, restlessness, increased activity, insomnia, irritability, disinhibition and agitation. This syndrome is poorly characterised in terms of its definition, prevalence, risk factors and pathophysiology, a situation that limits its recognition and evaluation. There are many factors that predispose the development of the syndrome such as age, differences in brain development in the paediatric population, the characteristics of the patient or the antidepressant, disorders of neurological development, and the doses and plasma levels of the medications. It has been thought that activation syndrome may be related to suicidal tendencies. However, the evidence in support of this link is inconsistent and further studies are therefore necessary. CONCLUSIONS: Activation syndrome with SSRI is a particularly important adverse effect in children and adolescents and, when it occurs, can cause lack of adherence to or discontinuation of treatment. Strict vigilance is therefore recommended during the use of these medications.

ABSTRACT

Background: The alarming prevalence of teen mothers' exposure to perinatal intimate partner violence (IPV, 8.3-67%) and attachment disorders (ADs) among their children is a global concern, especially in sub-Saharan Africa with high teenage pregnancy rates. This study, therefore, aimed to examine the link between teen IPV and AD in their offspring. We sought also to explore the mediating roles of postpartum depression symptoms, maternal sensitivity, parenting stress, and perceived social support in the relationship between perinatal IPV and children's ADs.Method: This cross-sectional study selected a random sample of 309 teen mothers from Nyanza district. This sample size was determined using Yamane's formula, with random sampling. Various instruments were used for data collection, including questionnaires on intimate partner violence, social support, maternal sensitivity, postpartum depression symptoms and parenting stress and early trauma-related disorders. The data was analysed using SPSS, with mediation analyses performed using the PROCESS macro (version 4.1).Results: IPV was found to be significantly associated with attachment disorders. Simple mediation models showed that parenting stress completely mediated these relationships, while postpartum depression, perceived social support, and maternal sensitivity partially mediated the relationship between IPV and children's ADs. In parallel mediation model, the combined roles of all mediators fully mediated the associations between IPV and ADs.Conclusion: These findings offer valuable insights in designing or strengthening the appropriate interventions to prevent and mitigate the perinatal intimate partner violence and its detrimental impact on children's attachment disorders. Combating intimate partner violence in post-conflict situations is challenging in teen mothers, however, our results suggest that efforts to address maternal mental health and parenting practices may protect children from attachment disorders.


Parenting stress was identified as a significant mediator, fully mediating the relationship between perinatal IPV and children's attachment disorders.Postpartum depression, perceived social support, and maternal sensitivity partially mediated the link between perinatal IPV and children's attachment disorders.The study underscores the necessity for multifaceted support programmes for adolescent mothers to address IPV, alleviate parenting stress, and enhance maternal mental health and social support, promoting better attachment outcomes for their children.

ABSTRACT

Background: Strong familial bonds are crucial to building resilience among youth exposed to traumatic events in socially adverse environments. Exploring parental experiences in the aftermath of adolescents' traumatic exposure in these settings would help tailor early interventions.Objective: We qualitatively explored experiences and perceived needs among parents of teenagers aged 11-16 years who were exposed in the last three months to a potentially traumatic event in Beirut, Lebanon.Method: We purposively sampled 28 parents of 24 adolescents meeting the inclusion criteria. Semi-structured interviews were conducted, and thematic analysis was applied combined with a grounded theory approach.Results: The most frequent traumatic event was direct exposure to the violent clashes that happened in Beirut on 14 October 2021. Parents identified that the recent event exacerbated pre-existing mental health difficulties caused by cumulative stress. They were reminded of their own war experiences and tended to reject the 'sick role' associated with trauma. A majority of participants viewed resilience as a fixed trait characteristic of the Lebanese and avoided communication with their children about traumatic memories, while a significant minority criticised resilience as a myth that added pressure on them and had more open communication about trauma. Parenting styles oscillated between controlling behaviours, warmth, and avoidance, which impacted the family dynamic. Despite adversity, most parents tried to cope through social connectedness, humour, and living day by day.Conclusions: Our findings hold implications for contextual adaptations of early posttraumatic interventions aimed at strengthening family support, such as addressing parental mental health; increasing awareness among first-line responders on parents' potential representations of trauma and resilience; addressing the issue of controlling parenting; and including a component in psychoeducation on traumatic stress that validates the impact of daily stressors on mental health while avoiding direct labelling. Further research is needed to validate the impact of these domains.


Parents of adolescents recently exposed to trauma in Beirut endure cumulative stress and recall war memories.Some parents see resilience as innate, and others criticise it as a myth.Early interventions should target parental mental health and conceptions on trauma and resilience.

Background: International research has established that children and adolescents are at high risk for being exposed to violence. A systematic review published in 2023 recommended six child and adolescent self-report violence against children (VAC) measures, based on their psychometric properties, in a systematic COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) review. However, the degree of overlap and heterogeneity was not part of the analysis.Objective: To compare the six measures with respect to item overlap and differences concerning specification of exposure dimensions.Method: A content analysis of the original 174 items resulted in a reduction to 38 unique items. These items were organized visually in a co-occurrence circle using an adapted version of Fried's R code. Furthermore, a pairwise comparison of event lists was performed using the Jaccard index.Results: There was a modest overlap among the six measures. Only one item was present in all six measures, only two items were present in four measures, and 78% of the items were present in just one or two measures. The overall overlap between the six measures was 25%.Conclusions: The lack of overlap among measures reflects a heterogeneity of definitions and purposes. It also impedes progress in research, as comparisons between various studies are difficult to make in a valid and reliable way. The lack of consensus also delays efficient political initiatives, because solid, consensual knowledge about the prevalence of VAC does not exist.


Violence against children (VAC) is a high political and professional priority. A recent systematic review recommended six self-report measures based on their psychometric qualities.The VAC items were very diverse. A content analysis reduced the original 174 items to 38 unique items. A Jaccard index showed an overlap of 25%.The lack of consensus in definitions and applied measures impedes progress in research and delays important political, prophylactic initiatives.

ABSTRACT

Background: The western region of China has a dense population, relatively underdeveloped economy, and a significant number of left-behind children. Currently, the prevalence of adolescent psychological abuse, neglect, and the factors associated with these issues in the region remain unclear. This study aims to evaluate the current status of adolescent psychological abuse and neglect and its associated factors in this region.Methods: Data were collected from 50 schools in western China through cluster sampling to target adolescents aged 12 to 18. A comprehensive survey form was used to collect socio-demographic characteristics of adolescents. The Child Psychological Abuse and Neglect Scale was employed to assess the current psychological abuse and neglect of adolescents. Independent samples t-tests were used for inter-group comparisons. A Directed Acyclic Graph was constructed for controlling confounding variables. Subsequently, binary logistic regression analysis was performed, and a nomogram risk factors model was developed using R Studio.Results: This study included 12,743 teenagers, with an average age of 15.53(±1.39) years. Among them, 4,965 individuals, accounting for 39.0%, reported experiences of psychological abuse, while 4,167 individuals, accounting for 32.7%, reported experiences of neglect. The rates of psychological abuse and neglect in adolescents are influenced by gender, grade, left-behind experience, parental marital status, and living on campus (P < .05).Conclusion: Adolescents in western China exhibit higher rates of psychological abuse and neglect compared to those in the eastern and northern regions of China. Gender, grade, left-behind experience, and family factors significantly influence the psychological abuse and neglect of adolescents.


This study is the first large-scale, multi-centre, cross-sectional analysis of psychological abuse and neglect among youth in Western China, a region with relatively underdeveloped economic and social conditions.This article fills in the gap in the region's research on psychological abuse and neglect by addressing the issues of small sample size, limited coverage, and a lack of variables included.This provides a crucial theoretical foundation for enhancing the mental well-being of youth in this region and preventing psychological and mental illnesses among youth.

OBJECTIVE: The goal of our study is to determine the level of Internet addiction (IA) in adolescents by utilizing the IA scale. METHODS: We employed two tools: the IA test (IAT) and the beck depression inventory (BDI), complemented by a sociodemographic information form, to assess IA and depression levels. RESULTS: A total of 201 participants were included. A positive correlation was found between daily Internet usage time and IAT scores (r = 0.388, p < 0.001) and between BDI scores and IAT scores (r = 0.161, p = 0.013). Females had a lower mean IAT score (63.56 ± 28.08) (p < 0.001). The BDI scores varied significantly across the groups (p = 0.004). The mean BDI scores were higher in the severe addiction group (13.53 ± 7.15) compared to the moderate (11.04 ± 6.62), mild (10.11 ± 5.38), and normal usage groups (9.28 ± 5.54). A significant difference was found in gender distribution across the groups (p = 0.001). The presence of suicidal ideation differed significantly across the groups (p = 0.002). The presence of depression showed a significant difference (p = 0.038). CONCLUSIONS: Our study reveals a significant correlation between increased Internet usage and heightened levels of IA and depression among adolescents, with notable gender differences in IA severity.


OBJETIVO: Determinar el nivel de adicción a internet en adolescentes utilizando una escala de adicción a internet. MÉTODO: Nuestro estudio involucró a 201 estudiantes con adicción a internet. Empleamos dos herramientas, la IAT (internet addiction test) y el BDI (beck depression inventory), que se complementaron con un formulario de información sociodemográfica, para evaluar los niveles de adicción a internet y de depresión. RESULTADOS: Se encontró una correlación positiva entre el tiempo diario de uso de internet y las puntuaciones del IAT (r = 0.388; p < 0.001), así como entre las puntuaciones del BDI y del IAT (r = 0.161; p = 0.013). Las mujeres tuvieron una puntuación media más baja en el IAT (p < 0.001). Las puntuaciones del BDI variaron significativamente entre los grupos (p = 0.004). Las puntuaciones medias del BDI fueron más altas en el grupo de adicción grave en comparación con los grupos de adicción moderada y de uso normal. Se encontró una diferencia significativa en la distribución por sexo entre los grupos (p = 0.001). La presencia de ideación suicida difirió significativamente entre los grupos (p = 0.002). La presencia de depresión mostró una diferencia significativa (p = 0.038). CONCLUSIONES: Nuestro estudio revela una correlación significativa entre mayor uso de internet y niveles elevados de adicción y depresión en adolescentes, con diferencias de sexo notables en la gravedad de la adicción.

ABSTRACT

Background: The implementation trial BESTFORCAN aims to evaluate the dissemination of Trauma-Focused Behavioural Therapy (TF-CBT) for children and adolescents in Germany with posttraumatic stress symptoms (PTSS) after child abuse and neglect (CAN) with a focus on supervision.Objective: This update to the study protocol outlines changes made due to practical reasons in the course of the ongoing trial while maintaining methodological quality.Method: The amendments to the original study protocol comprise (1) a more refined operationalisation of the primary outcome sufficiently adherent TF-CBT therapy (SATT), (2) changes in the study sites and (3) additional inclusion of one post-gradual psychotherapy training institute.Discussion: The adaptions to the original study protocol ensured high methodological quality through the transparent presentation of protocol modification: ensuring the recruitment of participating psychotherapists in training by including a further post-gradual training institute as well as an adaption of the measurement of SATT with high external validity. The objectives, diagnostic set, and secondary outcomes remained unimpaired by the amendment. Therefore, we expect the trial to provide evidence for the effect of model-specific trauma-focused supervision on the implementation outcomes of TF-CBT as compared to supervision as usual.Trial registration: German Clinical Trials Register identifier: DRKS00020516..


Update to the study protocol of the trial BESTFORCAN that investigates the implementation of trauma-focused behavioural therapy for children and adolescents with posttraumatic stress symptoms following abuse with a focus on the role of supervision.Adaptions have been made regarding the specification of the definition of sufficiently adherent intervention, relocation of the data-handling centre and the recruitment of one additional psychotherapy institute.The adaptions have no impact on the objectives, diagnostic set, secondary outcomes, or processes of data handling.

ABSTRACT

OBJECTIVE: To assess the perceptions and beliefs of adolescents about cannabis consumption, studying the incentive and disincentive factors for consumption. METHOD: A systematic review of the literature was carried out following the PRISMA guidelines. MedLine, Embase, APA PsycInfo, Cochrane and Web of Science were searched using controlled vocabulary and free terms. We included qualitative studies published between 2000 and 2024 that assessed the perceptions of adolescents aged 10 to 19 years on the use, effects, risks of using cannabis or cannabis with other legal drugs. The quality of the studies was assessed using the CASP tool. RESULTS: Of the 3665 articles identified in the initial search, 22 complied with elegibility criteria. Data extraction yielded a series of three lines of argument linked to cannabis use: reasons for use, reasons for not using and perceived effects. The most common discourse among adolescents was related to the feeling of social and emotional well-being, fewer adverse effects than drugs, and the influence of peers on consumption. CONCLUSIONS: The review preformed reports on the reasons that adolescents allege for cannabis use and reasons for abstention with a view of the preponderance of benefits over harms. It is considered that this information may be necessary for the development of prevention programmes with the dissemination of information on the effects of consumption.

ABSTRACT

Background: Many youth with posttraumatic stress symptoms (PTSS) do not receive evidence-based care. Internet- and Mobile-Based Interventions (IMIs) comprising evidence-based trauma-focused components can address this gap, but research is scarce. Thus, we investigated the feasibility of a trauma-focused IMI for youth with PTSS.Methods: In a one-arm non-randomized prospective proof-of-concept study, 32 youths aged 15-21 years with clinically relevant PTSS (CATS ≥ 21) received access to a trauma-focused IMI with therapist guidance, comprising nine sessions on an eHealth platform accessible via web-browser. We used a feasibility framework assessing recruitment capability, sample characteristics, data collection, satisfaction, acceptability, study management abilities, safety aspects, and efficacy of the IMI in PTSS severity and related outcomes. Self-rated assessments took place pre-, mid-, post-intervention and at 3-month follow-up and clinician-rated assessments at baseline and post-intervention.Results: The sample mainly consisted of young adult females with interpersonal trauma and high PTSS levels (CATS, M = 31.63, SD = 7.64). The IMI sessions were found useful and comprehensible, whereas feasibility of trauma processing was perceived as difficult. Around one-third of participants (31%) completed the IMI's eight core sessions. The study completer analysis showed a significant reduction with large effects in self-rated PTSS at post-treatment [t(21) = 4.27; p < .001; d = 0.88] and follow-up [t(18) = 3.83; p = .001; d = 0.84], and clinician-rated PTSD severity at post-treatment [t(21) = 4.52; p < .001; d = 0.93]. The intention-to-treat analysis indicated significant reductions for PTSS at post-treatment and follow-up with large effect sizes (d = -0.97- -1.02). All participants experienced at least one negative effect, with the most common being the resurfacing of unpleasant memories (n = 17/22, 77%).Conclusion: The study reached highly burdened young adults. The IMI was accepted in terms of usefulness and comprehensibility but many youths did not complete all sessions. Exploration of strategies to improve adherence in trauma-focused IMIs for youth is warranted, alongside the evaluation of the IMI's efficacy in a subsequent randomized controlled trial.


Youth often lack access to evidence-based care after trauma. This study assessed the feasibility of a trauma-focused internet- and mobile-based intervention with therapist guidance.The intervention was accepted by youths, and the preliminary evaluation of participant responses suggests its efficacy.Future studies should examine strategies to improve adherence and the IMI's efficacy in a RCT.

ABSTRACT

Background: Losing a family member during childhood is a potentially traumatic event and increases the risk of mental health difficulties. Adolescents have the right to express their views in research of relevance to them, but few studies have involved bereaved adolescents as collaborators (i.e. Patient and Public Involvement (PPI)). Furthermore, to ensure meaningful and non-tokenistic involvement, bereaved adolescents' levels of participation and experiences of taking part in research need to be evaluated.Objectives: The aim was to describe and evaluate a PPI process working with bereaved adolescents to develop a self-management mobile app for adolescents in grief.Methods: The PPI process consisted of four workshops during which the app's logo, colours, name, content, and layout were discussed with six parentally bereaved adolescents aged 13-18 years. The adolescents were recruited through a non-profit organisation providing support for adolescents in grief. The PPI process was documented and evaluated using participant observations and an online survey completed by the adolescents, covering the themes of social context, participation, and influence.Results: The adolescents perceived the social context as comfortable and inclusive, where their knowledge was valued. Their participation was characterised by ownership and motivated by a desire to help others with similar experiences. The adolescents' ability to participate in PPI activities was assisted by the researchers' flexibility, although challenging assignments may have made participation harder. Throughout PPI activities, adolescents contributed with relevant input and reported feeling influential. The study reached the intended levels of participation and appeared to adequately fulfil the adolescents' right to participation.Conclusions: Engaging adolescents who have undergone a potentially traumatic event, such as the loss of a family member, in research can enhance the overall relevance of the study. Moreover, it can entail a meaningful and positive experience for the participating adolescents, while also fulfilling their fundamental right to participation.


A collaborative process with parentally bereaved adolescents to develop a psychosocial self-management mobile app for adolescents in grief was described and evaluated.Adolescents made significant contributions, took ownership, and experienced having influence over the decisions made, which increased the relevance of the intervention.The collaborative process reached the intended level of participation and created a positive and meaningful experience for the adolescents.

ABSTRACT

PURPOSE: To determine whether there are differences in care experience of adolescent cancer patients and their main caregiver, treated in Adolescent Cancer Units (ACUs), compared to those treated in Non-Adolescent Cancer Units (NACUs), including the COVID-19 pandemic period. METHOD: Measurement of Reported Experience in adolescent oncology patients (12-19 years old) and caregivers through ad hoc surveys. The responses of the study group of patients and caregivers treated in Adolescent Units were compared with the group not treated in them. RESULTS: It is noted that many respondents consider that they have not been affected by the COVID-19 pandemic. Significant differences were seen in communication, with better perception by the group of patients treated in ACU (87.1%) and caregivers of ACU (97.3%) compared to patients and caregivers of NACU (53.3% and 68.2% respectively). Regarding information received about the side effects, a better perception was observed among patients treated in the ACU than in the NACU (p=0.247). In the transmission of information and the possibility of fertility preservation, a significant difference was observed in favour of ACU in patients and direct caregivers (p=0.010 and p=0.018). CONCLUSIONS: ACU represents an improvement in the quality perceived by patients and main caregiver on key points in the comprehensive care of the adolescent with cancer such as information, participation in the process and decision making, approach to side effects, psychological care and help in returning to normal life. However, fertility and strategies for talking about the cancer experience, were identified as areas for future improvement.

La diabetes mellitus tipo 2 de inicio temprano ha aumentado su prevalencia en los últimos años. Los principales factores de riesgo de padecerla en la adolescencia son: la obesidad, la predisposición genética y los antecedentes familiares. Se presenta el caso de una paciente de sexo femenino, de 15 años, que acudió a los servicios de salud con diagnóstico previo de prediabetes y sobrepeso. Al examen físico se observó sobrepeso y distribución de grasa central. Los exámenes de laboratorio indicaron hiperglucemia y hormonas tiroideas dentro de rangos de normalidad. Se diagnosticó diabetes mellitus tipo 2 y se propuso tratamiento con dieta, ejercicios físicos y metformina. Se reevaluó a los 3 meses y se constató que la paciente no cumplía el tratamiento a cabalidad. Este reporte de caso tiene como objetivo describir las características clínicas y de laboratorio de una paciente adolescente con diagnóstico de diabetes mellitus tipo 2 porque el diagnóstico precoz y el tratamiento intensivo es fundamental en el manejo de esta patología en este grupo etáreo.


Early-onset type 2 diabetes mellitus has increased in prevalence in recent years. The main risk factors for suffering from it in adolescents are: obesity, genetic predisposition and family history. The case of a 15-year-old female patient who attended health services with a previous diagnosis of prediabetes and overweight is presented. The physical examination revealed overweight and central fat distribution. Laboratory tests indicated hyperglycemia and thyroid hormones within normal ranges. Type 2 diabetes mellitus was diagnosed and treatment with diet, physical exercise and metformin was proposed. She was reassessed after 3 months and it was found that the patient was not fully complying with the treatment. This case report aims to describe the clinical and laboratory characteristics of an adolescent patient diagnosed with type 2 diabetes mellitus because early diagnosis and intensive treatment is essential in the management of this pathology in this age group.

Fundamento: la hipertensión arterial es una enfermedad de origen multifactorial relacionada con factores genéticos y ambientales, que ha experimentado un incremento notable en su prevalencia durante los últimos años. Es causa silente de morbilidad y daño de órganos diana en adolescentes. La detección precoz de este padecimiento es importante como marcador de riesgo de hipertensión en la adultez. Objetivo: determinar la prevalencia y caracterizar la hipertensión arterial en adolescentes de 15-19 años de edad. Métodos: se realizó un estudio de corte transversal en el periodo de septiembre a diciembre de 2022. El universo lo constituyeron los 1561 adolescente de 15-19 años de edad, la muestra 1444 (92,50 %). El muestreo fue probabilístico sistemático. Las variables analizadas fueron: edad, sexo, color de la piel, prehipertensión e hipertensión arterial (sistólica, diastólica y sistodiastólica) y severidad de la hipertensión. Se analizaron los estadígrafos: frecuencia, porcentaje y prevalencia. Resultados: el 8,21 % fueron diagnosticados con prehipertensión arterial y el 39 (7,44 %, prevalencia aparente) con hipertensión. El 7,69 % de los hipertensos tenían elevada la tensión arterial diastólica, el 35,90 % la sistólica y el 56,41 % ambas. El 89,74 % de los hipertensos se encontraban en el estadio 1 y el 10,26 % en estadio 2. Conclusiones: la prevalencia de la hipertensión arterial en los adolescentes incluidos en la investigación fue superior a la reportada en Cuba. El mayor porcentaje de pacientes con prehipertensión e hipertensión arterial se evidenció en mayores de 19 años de edad, varones y de color de piel negra.


Foundation: high blood pressure is a disease of multifactorial origin related to genetic and environmental factors, which has experienced a notable increase in its prevalence in recent years. It is a silent cause of morbidity and target organ damage in adolescents. Early detection of this condition is important as a risk marker for hypertension in adulthood. Objective: determine the prevalence and characterize arterial hypertension in adolescents 15-19 years of age. Methods: a cross-sectional study was carried out in the period from September to December 2022. The universe consisted of 1,561 adolescents aged 15-19 years, the sample being 1,444 (92.50 %). The sampling was systematic probabilistic. The variables analyzed were: age, sex, skin color, prehypertension and arterial hypertension (systolic, diastolic and systodiastolic) and severity of hypertension. The statisticians were analyzed: frequency, percentage and prevalence. Results: 8.21 % were diagnosed with prehypertension and 39 (7.44 %, apparent prevalence) with hypertension. 7.69 % of hypertensive patients had elevated diastolic blood pressure, 35.90 % had elevated systolic blood pressure, and 56.41 % both. 89.74 % of hypertensive patients were in stage 1 and 10.26 % in stage 2. Conclusions: the prevalence of arterial hypertension in the adolescents included in the research was higher than that reported in Cuba. The highest percentage of patients with prehypertension and high blood pressure was seen in people over 19 years of age, men, and black skin color.

Desde 2007, el Servicio de Epidemiología e Infectología, ha implementado un programa de transición que busca optimizar la atención de los adolescentes con infección por el HIV durante el paso de la atención pediátrica a la de adultos. Objetivo: Describir las características clínicas, epidemiológicas, virológicas y psicosociales de los adolescentes con infección HIV atendidos en el Programa y analizar el proceso de transición. Materiales y Métodos: Estudio de cohorte retrospectivo. Se incluyeron a los adolescentes, atendidos en el Programa de Transición entre enero de 2019 y diciembre de 2023, en el Hospital Garrahan, con al menos un resultado de CV y CD4+ en ese período. Se obtuvo la información de la historia clínica electrónica y se analizaron variables clínicas, epidemiológicas, virológicas, terapéuticas y psicosociales. Resultados: Se incluyeron 124 pacientes. La vía de transmisión fue vertical en el 92,74% y el estadio clínico e inmunológico era avanzado. En el momento de la transición 77,4% se encontraban con supresión virológica y con recuperación inmunológica. El 55,6% ya realizó la transición a un centro de adultos, 31,4% continúan en el programa, 11,3% se perdieron en el seguimiento y 1,7% fallecieron. Se recopilaron los datos de 31 pacientes transferidos. La mediana de seguimiento fue de 2 años; 25 pacientes (80,6%) continúan en seguimiento. Conclusiones: A pesar de la pandemia de COVID-19, el programa logró la retención de los adolescentes con infección por HIV y una transferencia sostenida en el tiempo. Además de un programa de transición estructurado para garantizar una atención continua y de calidad, es necesario continuar evaluando la evolución postransición (AU)


Since 2007, the Epidemiology and Infectious Diseases Department has implemented a transition program to optimize the care of adolescents with HIV infection during their transition from pediatric to adult care. Objective: To describe the clinical, epidemiological, virological, and psychosocial characteristics of adolescents with HIV infection treated in the program and to analyze the transition process. Materials and Methods: A retrospective cohort study was conducted. Adolescents followed in the Transition Program at Garrahan Hospital between January 2019 and December 2023, with at least one viral load and CD4+ result during that period, were included. Information was obtained from electronic medical records, and clinical, epidemiological, virological, therapeutic, and psychosocial variables were analyzed. Results: A total of 124 patients were included. The route of transmission was vertical in 92.74%, and the clinical and immunologic stage was advanced. At the time of transition, 77.4% were virologically suppressed and had achieved immunologic recovery. Of the patients, 55.6% had already transitioned to an adult center, 31.4% were still in the program, 11.3% were lost to follow-up, and 1.7% died. Data were collected from 31 transferred patients, with a median follow-up of 2 years; 25 patients (80.6%) remain in follow-up. Conclusions: Despite the COVID-19 pandemic, the program successfully retained HIVinfected adolescents and ensured sustained transition over time. In addition to a structured transition program to ensure continuous and quality care, it is necessary to continue evaluating post-transition outcomes (AU)

El desarrollo de nuevos abordajes terapéuticos ha generado un aumento en la esperanza de vida de los pacientes con enfermedades neuromusculares (ENM). Se trata de un grupo de enfermedades heterogéneas desde la clínica y los posibles tratamientos. La transición en los pacientes con ENM, implica un gran desafío por presentar niveles intelectuales dentro de rangos promedio, compromisos motor, respiratorio y cardiológico progresivos que resulta en aumento de la dependencia física conforme aumenta la necesidad de autonomía emocional del adolescente. La descripción de transiciones exitosas en ENM incluye intervenciones psicosociales individuales o grupales con un enfoque multidimensional e interdisciplinario, que contemple la participación de la familia para reducir la ansiedad y la preocupación sobre sus hijos. En el Hospital Garrahan los pacientes con ENM son atendidos dentro del Programa de Atención, Docencia e Investigación de Pacientes con Enfermedad Neuromuscular desde 2008. En este trabajo nos proponemos describir la experiencia en transición pre y post pandemia, de los adolescentes con ENM en seguimiento en el Hospital de Pediatría Garrahan (AU)


The development of new care and therapeutic approaches has generated an increase in the life expectancy of patients with neuromuscular diseases (NMD), a group of heterogeneous diseases from a clinical point of view. The transition in patients with MND involves a great challenge due to progressive motor, respiratory and cardiological compromises that result in an increase in physical dependence as the adolescent needs emotional autonomy. The description of successful transitions in patients with MND includes individual and psychosocial interventions with a multidimensional and interdisciplinary approach with family participation. Since 2008, we developed a Care, Teaching and Research Program for Patients with NMD Disease at the Garrahan Hospital. The objective of this work is to describe the pre- and post-pandemic transition experience of adolescents with NMD follow-up in our hospital (AU)

ABSTRACT

Gender is an important determinant of health. Its relationship with inequality and violence allows us to consider being a woman as a risk factor for health. Girls and teenager girls are not exempt from this circumstance, which conditions their lives from before birth and can determine their health status throughout life. It can vary according to social contexts, as various factors intersect with gender, adding risk and vulnerability to being a woman. Gender-based violence is often identified as a problem for adult women; however, the experience of discriminatory gender-based violence is constructed throughout women's lives, producing serious individual and social consequences from childhood. Accepting this violence as a «private or domestic matter¼ often prevents seeing the true dimension of the problem, its consequences, and the need to address it as a global issue.

Abstract Introduction: Psychological suffering among adolescents and strategies for the promotion of mental health exists and is necessary. Aim: To characterize the emotional demands of school adolescents and describe knowledge about protective factors of mental health. Methods: Sociopoetic research with 30 adolescents. Results/Analysis topics: "Questions of interest about emotions and mental health"; "Dealing with stress and anxiety"; "Reflections on conflict resolution and problems faced"; and "Knowledge about protective factors of mental health". Conclusions: Recreational and artistic activities are powerful for the promotion of adolescent mental health, and can be implemented through the school-health service partnership.


Resumo Introdução: O sofrimento psicológico entre adolescentes e estratégias para a promoção da saúde mental existem e são necessários. Objetivo: Caracterizar as demandas emocionais de adolescentes escolares e descrever o conhecimento sobre fatores protetores da saúde mental. Métodos: Pesquisa sociopoética com 30 adolescentes. Resultados: Temas de análise: "Questões de interesse sobre emoções e saúde mental"; "Lidar com o stresse e a ansiedade"; "Reflexões sobre resolução de conflitos e problemas enfrentados"; e "Conhecimento sobre fatores de proteção da saúde mental". Conclusões: As atividades recreativas e artísticas são potentes para a promoção da saúde mental dos adolescentes e podem ser implementadas através da parceria escola/serviço de saúde.
