ABSTRACT

El estudio de los tumores del sistema nervioso central (SNC) resulta ser un tema de gran consideración y su conocimiento reviste una alta importancia en la práctica médica. Las clasificaciones de las neoplasias del SNC comenzaron a mediados del siglo XIX hasta que en 1979 la Organización Mundial de la Salud (OMS) publicó la primera edición de una sistemática útil con el objetivo de establecer un lenguaje común para todas las especialidades médicas. Al día de hoy, 5 ediciones actualizaron la taxonomía neoplásica. La quinta edición del año 2021 consolida el cambio de paradigma dado por los avances moleculares, si bien todavía la transición se encuentra en proceso entre la caracterización morfológica y la biológica molecular. En este artículo, se analizan las nuevas modificaciones incorporadas en las diferentes familias tumorales más frecuentes en pediatría haciendo hincapié en aquella información de utilidad para el médico pediatra en su práctica diaria y la consulta multidisciplinaria.

ABSTRACT

Adequately reporting of preeclampsia is a challenge. The aim of this study was to compare the diagnosis of preeclampsia based on clinical and laboratory findings and ICD-10 codes (International Classification of Diseases) at admission and discharge in a referral center, over a one-year period. Among 2,839 women admitted for childbirth, 208 presented confirmed preeclampsia, based on chart audits. Among these, 77.4% at admission presented ICD-10 codes assigned as hypertension, and 47.1% at discharge. In 14.4% of confirmed preeclampsia, ICD-10 codes for hypertension were not used. Adequate reporting of preeclampsia is key for clinical surveillance, counselling and guiding public health interventions.

ABSTRACT

Introducción. El Estudio Nacional de la Discapacidad y Dependencia (ENDIDE) y el III Estudio Nacional de la Discapacidad (ENDISC) son investigaciones de relevancia en Chile que utilizan la estadística para comprender la situación de las personas con discapacidad en el país. Estos estudios recopilan datos cuantitativos y cualitativos sobre la discapacidad, permitiendo obtener información actualizada sobre su prevalencia, tipos de discapacidad, acceso a servicios y barreras enfrentadas. La utilización de la estadística en estos estudios es crucial para identificar necesidades, diseñar políticas inclusivas y evaluar el impacto de las intervenciones en la calidad de vida de las personas con discapacidad en Chile. Es relevante comprender las metodologías y pertinencia de estos datos para el desarrollo de la kinesiología, según indica la Convención sobre los Derechos de las Personas con discapacidad.


Background. The National Study on Disability and Dependency (ENDIDE) and the III National Study on Disability (ENDISC) are relevant investigations in Chile that use statistics to understand the situation of people with disabilities in the country. These studies collect quantitative and qualitative data on disability, allowing to obtain updated information on its prevalence, types of disability, access to services and barriers faced. The use of statistics in these studies is crucial to identify needs, design inclusive policies and assess the impact of explosions on the quality of life of people with disabilities in Chile. It is important to understand the methodologies and relevance of these data for the development of kinesiology, as indicated by the Convention on the Rights of Persons with Disabilities.

Objetivo: descrever o desempenho de um software na seleção automática da causa básica de morte no Peru, entre 2016 e 2019. Métodos: estudo descritivo do desempenho de um software na seleção automatizada da causa básica de morte ao longo dos anos (teste qui-quadrado de tendência) e da correlação entre a forma de declaração de óbito e o desempenho do software (coeficientes de correlação e determinação). Resultados: foram analisadas 446.217 declarações de óbito; a proporção de declarações com causa básica de morte aumentou de 69,6%, em 2016, para 78,8%, em 2019 (p-valor < 0,001); observou-se correlação linear direta entre as declarações eletrônicas de óbito e o desempenho do software (coeficiente de correlação = 0,95; R2 = 0,89). Conclusão: o software apresentou bom desempenho na seleção automática da causa básica de morte, com aumento significativo entre 2016 e 2019.


Objective: to describe software performance in the automatic selection of the underlying cause of death in Peru, between 2016 and 2019. Objetivo: describir el desempeño de un software en la selección automática de la causa básica de muerte en Perú, entre 2016 y 2019. Métodos: estudio descriptivo de la tendencia del desempeño de un software para seleccionar la causa básica de muerte a través de los años (chi cuadrado de tendencia) y la correlación entre los certificados de defunción electrónicos y el desempeño del software (coeficientes de correlación y determinación). Resultados: se analizaron 446.217 certificados; la proporción de certificados con causa básica de muerte aumentó de 69,6% en 2016 a 78,8% en 2019 (p-valor < 0,001); se observó una correlación lineal directa entre certificados electrónicos y el desempeño del software (coeficiente de correlación = 0,95; R2 = 0,89). Conclusión: el software presentó un buen desempeño en la selección de la causa básica de muerte y aumentó significativamente entre 2016 y 2019.

ABSTRACT

1) Empirical psychiatric research has failed to validate or verify the causes of depressive disorder. 2) Historically, psychiatry has been looking for many causes and is currently favoring a model of multi-causality, acting at various levels of interaction with diffuse limits. 3) Purely scientific psychiatry is based on the concept that a person is an "entity in itself" that presently suffers from a disorder caused by modifications in the impulses of the neurons in its brain. 4) The question that remains is whether depression is a real entity independent of human activities, a pragmatic entity employed for reasons of utility, or an entity constructed by the prevailing social forces in Western civilization. 5) The rationale for depression can be answered if we understand a human being as a being-in-the-world that a] is projected into the future to enact its existence, b] is placed in a condition that determines him and hinders his self-determination and c] lives among others that impel him to submit to the rules valid for "common people.

Background: In 2020, a unique social experience was provided by the pandemic of coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2. Interventions to tackle the pandemic may affect the burden of other respiratory diseases. Objective: This study aims to assess the impact of the COVID-19 mitigation strategies on hospitalizations for asthma in children aged between 1 and 14 years, adults aged between 20 and 59 years, and elderly older than 60 years. Methods: Data from hospital admissions for asthma were obtained from the Department of Informatics of Brazilian Public Health System database in the period between January 2016 and December 2020 and analyzed by age groups. To evaluate the effect of containment measures on the incidence of asthma and respiratory system diseases (total), the absolute reduction and relative reduction were calculated by analyzing the subsets from 2016 to 2019 versus 2020. Results: There was a significant reduction in the average incidence of hospitalizations in 2020, with numbers ranging from -59% (incidence rate ratio, 0.41 [0.37-0.45]) for age 1 to 14 years (prepandemic 1,393.2/100,000 vs pandemic 574.9/100.000), -37% (incidence rate ratio, 0.63 [0.49-0.80]) for age 20 to 59 years (prepandemic 160.2/100,000 vs pandemic 101.1/100,000), and -60% (incidence rate ratio, 0.40 [0.33-0.47]) for older than 60 years (prepandemic 460.6/100,000 vs pandemic 185.3/100,000). Conclusions: Ashtma hospitalizations decreased in 2020, especially in the pediatric group and the older group during the COVID-19 pandemic, which may be associated with the reduction in the incidence of many respiratory viral infections.

Objective: Acute type A aortic dissection (ATAAD) is a surgical emergency with significant morbidity and mortality, as well as significant center-level variation in outcomes. Our study aims to leverage a nationally representative database to assess contemporary in-hospital outcomes in surgical repair of ATAAD, as well as the association of age and sex with outcomes. Methods: The National Inpatient Sample was queried to identify hospital discharge records of patients aged ≥18 years who underwent urgent surgical repair of ATAAD between 2017 and 2018. Patients with a diagnosis of thoracic aortic dissection, who underwent surgical intervention of the ascending aorta, were identified. Patient demographics were assessed, and predictors of in-hospital mortality were identified. Results: We identified 7805 weighted cases of surgically repaired ATAAD nationally, with an overall mortality of 15.3%. Mean age was 60.0 ± 13.6 years. There was a male predominance, although female subjects made up a larger proportion of older age groups-female subjects up 18.4% of patients younger than 40 years with ATAAD but 53.6% of patients older than 80 years. In multivariable analysis controlling for sex, race, comorbidities, and malperfusion, age was a significant predictor of mortality. Patients aged 71 to 80 years had a 5.3-fold increased risk of mortality compared with patients ≤40 years old (P < .001), and patients aged >80 years had a 6.8-fold increased risk of mortality (P < .001). Sex was not significantly associated with mortality. Conclusions: Surgical repair of ATAAD continues to carry high risk of morbidity and mortality, with outcomes impacted significantly by patient age, regardless of patient comorbidity burden.

BACKGROUND: There is a lack of population-based studies of anaphylaxis from low- and middle-income countries. This hampers public health planning and investments and may influence availability of adrenaline auto-injectors. OBJECTIVE: We conducted the first national population-based study of anaphylaxis hospitalization in Brazil. METHODS: Descriptive study using routinely reported data to the Brazilian Hospital Information System for the years 2011-2019. Information available is coded based on the International Classification of Diseases (ICD)-10 and covers main cause of hospitalization (primary cause) and any conditions contributing to it (secondary cause). RESULTS: Over 9 years, we identified 5716 admissions due to anaphylaxis for all causes. The average hospitalization rate related to anaphylaxis was 0.71/100,000 population per year, with a 2.4% (95% CI 1.9%, 2.9%) increase per annum over the study period. Admissions were more frequent among females (52.8%), except for cases due to insect sting. Most admissions occurred in adulthood, from 30 to 59 years (36.3%) but 13.8% in preschool children (0-4 years). There were more young children admitted for food-related anaphylaxis, and more adults admitted for drug/iatrogenic-related anaphylaxis. There were 334 cases (5.8% of admissions) of fatal anaphylaxis over the study period, with increased case fatality rate over time. CONCLUSIONS AND CLINICAL RELEVANCE: This is the first study of anaphylaxis hospital admissions using nation-wide data from a low- or middle-income country. Hospital admissions and fatalities from anaphylaxis in Brazil appear to be increasing.

ABSTRACT

The highest proportion of deaths among patients with cardiovascular diseases is due to ischemic heart disease (IHD), which is the second most common cause of death in Peru. This study aims to measure and identify changes in the temporal trend in mortality from ischemic heart disease in the Peruvian population. An ecological study was carried out with data from individual death records from the Ministry of Health between 2005 and 2017. A death was considered attributable to IHD if it was registered with the codes I20 to I25 of the ICD-10. Crude and adjusted mortality rates for IHD were calculated for the general population by age and according to sex. A joinpoint regression analysis was performed to assess trends in IHD mortality. There were 61,524 deaths due to IHD (55.69% men) from 2005 to 2017. According to the ICD-10, acute myocardial infarction (I21) accounted for the highest proportion of deaths (88.16%), followed by chronic IHD (I25), with 6.53%. In general, a decrease in adjusted IHD mortality rates was found in the general population over time (45.34 in 2005; 22.18 in 2017). By sex, men possessed a 1.5-fold higher rate than women. The highest mortality rates from IHD were found in the natural coastal region (68.55%) and in urban areas (86.43%). A joinpoint regression analysis showed a reduction in the mortality trend over time due to IHD for both the general population and the population when grouped by sex. In conclusion, there was a continuous decrease in mortality rates due to IHD in the Peruvian population between 2005 and 2017. Strategies focused on mitigating the impact of this disease are required and should emphasize the subgroups most likely to die from this cause.

BACKGROUND: Generalized pustular psoriasis (GPP) is a rare and severe phenotype of psoriasis characterized by sudden outbreak of widespread coalescent sterile pustules associated with a spectrum of systemic symptoms. OBJECTIVE: We aimed to describe the epidemiology and treatment of GPP in Brazil from the public health care system perspective. METHODS: This was a retrospective public claims database study, using outpatient and inpatient databases, with information from January 2018 to August 2020, based on records of health resource utilization by patients with GPP. Outpatient treatment regimens and fatal inpatient outcomes were described. RESULTS: In total, 1458 outpatients of all ages were identified, of whom 53% were women. We estimated the GPP prevalence in Brazil to be between 0.7 and 0.9 per 100,000. Acitretin was the most commonly dispensed drug. Of all the outpatients, 769 outpatients could be tracked in the inpatient database, and 151 had hospital admissions during the study period. Of them, 5.3% had a fatal outcome during hospitalization. A primary skin condition or an infection was the most frequent hospitalization cause. LIMITATION: The International Classification of Diseases codes for GPP and psoriasis have not been previously validated in this context. CONCLUSION: GPP is a rare disease in Brazil and affects individuals of all ages and both sexes. Hospitalizations and disease-related deaths highlight the need for its prompt diagnosis, close medical follow-up, and effective treatment.

Background: Emerging pandemic viruses may have multiple deleterious effects on maternal health. This study examines the effects of a pandemic influenza virus on cause-specific maternal mortality time series, using Argentinian vital statistics. Methods: We conducted a population-based natural experiment from national vital records of maternal deaths between 1980 and 2017. Joinpoint regression models were used to model time series of the maternal mortality ratio (MMR). The sensitivity of the registry to detect the effects of the pandemic H1N1 2009 influenza virus on cause-specific MMR was analysed using a panel of parallel interrupted time series (ITS). Findings: Over this 38-year study, the MMR decreased by 58·6% (69·5 to 28·8 deaths/100,000 live births), transitioning from direct obstetric causes (67·0 to 21·1/100,000 live births; 68·4% decrease) to indirect causes (2·6 to 7·7/100,000 live births; 196·2% increase). The regression analysis showed an average reduction of -2·2%/year (95% CI: -2·9 to -1·4) with 2 join points in the total trend (1998 and 2009). Parallel ITS analyses revealed the pandemic H1N1 virus had an increasing effect on mortality from the respiratory system- and sepsis-related complications (level change 4·7 and 1·6/100,000 live births respectively), reversing after the outbreak. No effect was found on MMR from hypertensive disorders, haemorrhage, abortive outcomes, other direct obstetric causes, and indirect non-respiratory comorbidities. Interpretation: The Argentinian maternal death registry appears sensitive to detect different effects of emerging infectious epidemics on maternal health. In a population-based natural experiment, pandemic H1N1 virus impacted maternal mortality almost exclusively from the respiratory system- and sepsis-related complications. ABSTRACT Objective To describe the clinical characteristics and treatment of children with sepsis, severe sepsis, and septic shock at a pediatric emergency department of a public hospital. Methods A retrospective, observational study. The medical records of patients included in the hospital Pediatric Sepsis Protocol and patients with discharge ICD-10 A41.9 (sepsis, unspecified), R57 (shock) and A39 (meningococcal meningitis) were evaluated. Results A total of 399 patients were included. The prevalence of sepsis, severe sepsis, and septic shock at the emergency room were 0.41%, 0.14% and 0.014%, respectively. The median age was 21.5 months for sepsis, 12 months for severe sepsis, and 20.5 months for septic shock. Sepsis, severe sepsis, and septic shock were more often associated with respiratory diseases. The Respiratory Syncytial Virus was the most common agent. The median time to antibiotic and fluid administration was 3 hours in patients with sepsis and severe sepsis. In patients with septic shock, the median times to administer antibiotics, fluid and vasoactive drugs were 2 hours, 2.5 hours and 6 hours, respectively. The median length of hospital stay for patients with sepsis, severe sepsis and septic shock were 3 days, 4 days and 1 day, respectively. The overall mortality was 2%. Conclusion Sepsis had a low prevalence. Early diagnosis and recognition are a challenge for the emergency care pediatrician, the first place of admission.

OBJECTIVE: Autoimmune diseases generate an impact on the morbidity and mortality of patients and are a burden for the health system through hospital admissions and readmissions. The prevalence of readmission of patients with these diseases has not yet been described as a group, but rather as sub-phenotype. The objective of this study is to determine the prevalence of hospital readmissions in a Colombian population with autoimmunity and the factors related to readmission. METHODS: All patients with autoimmune diseases who were evaluated by the rheumatology service and hospitalized between August 2018 and December 2019 at the Fundación Hospital Infantil Universitario De San José de Bogotá were described. A bivariate analysis was done, and three multivariate logistic regression models were built with the dependent variable being readmission. RESULTS: Of the total 199 admissions, 131 patients were evaluated and 32% were readmitted. The most frequent sub-phenotype in both groups (readmission and no readmission) was SLE (51% and 59%). The most frequent cause of hospitalization and readmission was disease activity (68.7% and 64.3%). History of hypertension was associated with readmission (adjusted OR: 2.98-95% CI: 1.15-7.72). In a second model adjusted for confounding variables, no factor was associated. In a third model analyzing the history of kidney disease and previous use of immunosuppressants (adjusted for confounding variables), the previous use of immunosuppressants was related to readmission (OR: 2.78-95% CI 1.12-6.89). CONCLUSION: Up to a third of patients with autoimmunity were readmitted and arterial hypertension was an associated factor. This suggested a greater systemic compromise and accumulated damage in patients who have these two conditions that may favor readmission. A history of immunosuppressant use may play a role in readmission, possibly by increasing the risk of developing infections.

ABSTRACT

Since 1948, the World Health Organization has been publishing the International Statistical Classification of Diseases and Related Health Problems (ICD), a coded system of causes of disease and death with an in-depth revision every 10-15 years. In its latest revision, the ICD-11 uses nomenclature characterizing old age as "initial and final geriatric periods," implying the medicalization of this stage of life, which has created confusion and sparked controversy. This article discusses the new nomenclature proposed, given the current knowledge about old age and the aging process and its most accepted definition. The ICD not only classifies diseases but periods of life and "health-related problems," and old age per se is not a health-related problem for many people at this stage of life. It is therefore essential to change or provide a more nuanced definition of the term "old age" in the ICD-11, so it is not perceived as a symptom, sign, or anomalous clinical outcome, and to introduce nomenclature that more accurately reflects pathological aging. Two terms that are enjoying growing experimental and bibliographic support are "fragility" and "loss of intrinsic capacity," which offer greater precision when defining the condition of a person who is not experiencing healthy aging.


Desde 1948, a Organização Mundial da Saúde publica um sistema codificado de causas de doença e morte sob o nome de Classificação Estatística Internacional de Doenças e Problemas Relacionados à Saúde (CID), com revisões profundas a cada 10-15 anos. Sua última versão, a CID-11, utiliza determinada terminologia para caracterizar a velhice como "períodos geriátricos iniciais e finais", o que implica uma medicalização dessa fase da vida que tem gerado confusão e polêmica. Este artigo discute a nova terminologia proposta à luz dos conhecimentos atuais sobre a velhice e o processo de envelhecimento, e sua definição mais aceita. A CID não classifica apenas as doenças, mas também períodos da vida e "problemas de saúde", sendo que a velhice por si só não representa um problema de saúde para muitos que se encontram nesta fase da vida. A partir dessa perspectiva, é imprescindível alterar ou qualificar o verbete "velhice" da CID-11 para que não seja percebido como sintoma, sinal ou resultado clínico anormal, e em seu lugar introduzir termos que melhor reflitam o estado de envelhecimento patológico. Entre os termos que têm crescente suporte experimental e bibliográfico estão "fragilidade" e "perda de capacidade intrínseca", que proporcionam uma precisão muito maior na definição da condição de quem não goza de um envelhecimento saudável.

Resumo: Apesar da Lista Brasileira de Causas de Morte Evitáveis (LBE), na sua versão para crianças menores de 5 anos (LBE < 5), não contemplar óbitos fetais, alguns estudos a utilizaram sem ou com poucas adaptações. Apresentamos uma proposta de adaptação da LBE para óbitos fetais (LBE-OF) e os resultados de sua aplicação aos óbitos fetais no Estado do Rio de Janeiro, Brasil, em 2018, comparados aos da LBE < 5. Revisamos as categorias da 10ª revisão da Classificação Internacional de Doenças (CID-10) presentes na LBE < 5 e as realocamos nos grupos de evitabilidade, segundo momento do óbito em relação ao parto e as causas condizentes com óbitos fetais. Condições que não esclarecem os determinantes do óbito foram alocadas como causas mal definidas. Óbitos fetais no Estado do Rio de Janeiro - selecionados das bases do Sistema de Informações sobre Mortalidade (SIM) - foram classificados segundo LBE-OF e a LBE < 5. Ao classificar os 2.585 óbitos fetais do Estado do Rio de Janeiro ocorridos em 2018, observou-se que, segundo a LBE < 5, predominariam causas "reduzíveis por adequada atenção ao parto" (42,9%), já pela LBE-OF, aquelas "reduzíveis por adequada atenção à mulher na gestação" foram as mais frequentes (43,6%). As causas mal definidas ocuparam a segunda posição segundo a LBE-OF (35,4%) e a terceira posição segundo LBE < 5. Cerca de 30% dos óbitos fetais mudaram de grupos e subgrupos de evitabilidade, mostrando maior coerência com o perfil de atenção obstétrica. Embora identificando um maior percentual de causas mal definidas, a LBE-OF coaduna-se mais com a fisiopatologia dos óbitos fetais. Sua inserção no SIM avançaria no sentido de monitorar e qualificar a investigação de causas de morte fetal.


Abstract: Although the Brazilian List of Avoidable Causes of Deaths (LBE in Portuguese), in its version for children under 5 years of age (LBE < 5), does not include stillbirths, some studies have used the list with or without adaptations. We present a proposal for adaptation of the LBE for stillbirths (LBE-OF in Portuguese) and the results of its application to stillbirths in the State of Rio de Janeiro, Brazil, in 2018, compared to the results with LBE < 5. We reviewed the categories from the 10th revision of the International Classification of Diseases (ICD-10) in the LBE < 5 and reassigned them in the avoidability groups, according to time of death in relation to delivery and the causes consistent with stillbirths. Conditions that did not elucidate the determinants of death were allocated as ill-defined causes. Stillbirths in the State of Rio de Janeiro, selected from the databases of the Mortality Information System (SIM in Portuguese), were classified according to LBE-OF and LBE < 5. When classifying the 2,585 stillbirths that occurred in the State of Rio de Janeiro in 2018, we found that according to LBE < 5, there were predominantly causes "reducible by adequate care in labor and delivery" (42.9%), while according to LBE-OF, the most frequent causes were "reducible by adequate care for during pregnancy" (43.6%). Ill-defined causes ranked second according to the LBE-OF (35.4%) and third according to LBE < 5. Some 30% of stillbirths changed groups and subgroups of avoidability, showing greater consistency with the profile of obstetric care. Although identifying a higher percentage of ill-defined causes, the LBE-OF is more consistent with the pathophysiology of fetal deaths. The inclusion of stillbirths in the SIM would be a positive step in monitoring and upgrading the investigation of causes of fetal death.


Resumo: A pesar de que la Lista Brasileña de Causas de Muerte Evitables (LBE), en su versión para niños menores de 5 años (LBE < 5), no contemple los óbitos fetales, algunos estudios la utilizaron sin o con pocas adaptaciones. Presentamos una propuesta de adaptación de la LBE para óbitos fetales (LBE-OF) y los resultados de su aplicación a óbitos fetales en el Estado de Rio de Janeiro, Brasil, en 2018, comparados con los de la LBE < 5. Revisamos las categorías de la 10ª revisión de la Clasificación Internacional de Enferemedades (CIE-10) presentes en la LBE < 5, y las asignamos en los grupos de evitabilidad, según el momento del óbito, en relación con el parto y las causas acordes con óbitos fetales. Las condiciones que no aclararon los determinantes del óbito fueron asignadas como causas mal definidas. Los óbitos fetales del Estado de Río de Janeiro -seleccionados de las bases del Sistema de Información sobre Mortalidad (SIM)- se clasificaron según el LBE-OF y la LBE < 5. Al clasificar los 2.585 óbitos fetales del Estado de Rio de Janeiro ocurridos en 2018, se observó que, según la LBE < 5 años, predominarían causas "reducibles por una adecuada atención al parto" (42,9%), mientras que por la LBE-OF, aquellas "reducibles por una adecuada atención a la mujer en la gestación" fueron las más frecuentes (43,6%). Resumo: O aborto permitido por lei no Brasil é um direito pouco garantido devido a múltiplas barreiras de acesso. A dificuldade em obter-se registros confiáveis e de fácil acesso, que subsidiem ações de monitoramento e avaliação pertinentes a essa temática, é outra questão preocupante relacionada à organização do sistema de saúde. Assim, esta pesquisa teve como objetivo analisar os registros de atendimentos a mulheres que realizaram aborto legal no Município de Porto Alegre, Rio Grande do Sul, no período de 2013 a 2018 e identificar sua prevalência no Sistema de Informações Hospitalares (SIH). Para isso, os prontuários dos casos de aborto previstos em lei, realizados nos quatro serviços de referência foram avaliados in loco. Os dados obtidos foram cruzados com o banco do SIH do Sistema Único de Saúde (SUS). A razão de prevalência foi estimada por meio do modelo de regressão Poisson, com variância robusta. Foram identificados, nos prontuários, 236 casos de aborto legal, dos quais 95 (40,3%) estavam registrados no SIH. Os casos cujos prontuários tinham o CID O04 (aborto por razões médicas) identificado em seus registros internos tiveram uma prevalência de 3,02 (IC95%: 1,83-4,98) vezes de constarem no SIH do que aqueles que não identificaram. Foi observado que o número de registros de aborto legal no SIH diferia dependendo do hospital; no entanto, verificou-se que, independentemente do hospital, o fato do registro interno descrever o CID O04 aumenta a prevalência de registros no SIH. Conclui-se que a falta de padronização e o sub-registro dificultam a obtenção de informações fidedignas na base de dados nacionais do SUS, aumentando a invisibilidade do aborto legal.


Abstract: Abortion as allowed by law in Brazil is a right that is rarely enforced, due to the multiple barriers to access. Another troublesome issue related to the health system's organization is the difficulty in obtaining reliable and easily accessible records to back monitoring and evaluation of legally authorized abortion. The current study thus aimed to analyze the patient records of women that had undergone legal abortion in the city of Porto Alegre, Rio Grande do Sul State, from 2013 to 2018 and to identify the procedure's prevalence in the Hospital Information System (SIH). The study analyzed on site the patient files for legally permitted abortions performed in the four referral services in the city. The authors cross-analyzed the data with the database of the SIH under the Brazilian Unified National Health System (SUS). Prevalence ratio was estimated with a Poisson regression model with robust variance. Examination of the patient files identified 236 cases of legal abortion, of which 95 (40.3%) were recorded in the SIH. The cases in which the patient files contained ICD O04 (medical abortion) identified in their internal records had 3.02 times higher prevalence (95%CI: 1.83-4.98) of being recorded in the SIH than those without the ICD code. The number of records of legal abortions in the SIH differed according to the hospital. However, independently of the hospital, the fact that the internal record listed ICD O04 increased the prevalence of records in the SIH. In conclusion, the lack of standardization and under-recording hindered the collection of trustworthy information in the national database of the SUS, further increasing the invisibility of legal abortion in Brazil.


Resumen: El aborto está permitido por ley en Brasil, aunque es un derecho poco garantizado, debido a múltiples barreras de acceso. La dificultad de conseguir registros confiables y de fácil acceso, que apoyen acciones de supervisión y evaluación pertinentes para esta temática, es otra cuestión preocupante relacionada con la organización del sistema de salud. De este modo, el objetivo de esta investigación fue analizar los registros de atención a mujeres, que abortaron legalmente en el Municipio de Porto Alegre, Rio Grande do Sul, durante el período de 2013 a 2018 e identificar su prevalencia en el Sistema de Informaciones Hospitalarias (SIH). Para tal fin, se evaluaron in loco los historiales de los casos de aborto previstos en ley, efectuados en cuatro servicios de referencia. Los datos obtenidos se cruzaron con el banco de datos del SIH del Sistema Único de Salud (SUS). La razón de prevalencia se estimó mediante el modelo de regresión Poisson con variancia robusta. Se identificaron en los registros médicos, 236 casos de aborto legal, de los cuales 95 (40,3%) estaban registrados en el SIH. Los casos cuyos historiales tenían el CIE O04 (aborto por razones médicas), identificado en sus registros internos, contaron con una prevalencia de 3,02 veces (IC95%: 1,83-4,98) respecto a constar en el SIH, en relación con aquellos que no se identificaron. Se observó que el número de registros de aborto legal en el SIH difería dependiendo del hospital; no obstante, se verificó que, independientemente del hospital, el hecho de que el registro interno describa el CIE O04 aumenta la prevalencia de registros en el SIH. Se concluye que la falta de estandarización y el subregistro dificultan la obtención de información fidedigna en la base de datos nacionales del SUS, aumentando la invisibilidad del aborto legal.

ABSTRACT

The International Classification of Diseases (ICD) provides a common language for use worldwide as a diagnostic and classification tool for epidemiology, clinical purposes and health management. Since its first edition, the ICD has maintained a framework distributing conditions according to topography, with the result that some complex conditions, such as allergies and hypersensitivity disorders (A/H) including anaphylaxis, have been poorly represented. The change in hierarchy in ICD-11 permitted the construction of the pioneer section addressed to A/H, which may result in more accurate mortality and morbidity statistics, including more accurate accounting for mortality due to anaphylaxis, strengthen classification, terminology and definitions. The ICD-11 was presented and adopted by the 72nd World Health Assembly in May 2019, and the implementation is ongoing worldwide. We here present the outcomes from an online survey undertaken to reach out the allergy community worldwide in order to peer review the terminology, classification and definitions of A/H introduced into ICD-11 and to support their global implementation. Data are presented here for 406 respondents from 74 countries. All of the subsections of the new A/H section of the ICD-11 had been considered with good accuracy by the majority of respondents. We believe that, in addition to help during the implementation phase, all the comments provided will help to improve the A/H classification and to increase awareness by different disciplines of what actions are needed to ensure more accurate epidemiological data and better clinical management of A/H patients.
