ABSTRACT

Background: Hereditary haemorrhagic telangiectasia (HHT) is characterized by the presence of telangiectases and larger arteriovenous malformations in different organs. Mucocutaneous telangiectases can bleed and become an aesthetic concern, impairing quality of life (QoL). However, the best treatment approach has not been defined yet. Objective: To evaluate the efficacy and safety of dual wavelength sequential 595/1064nm laser (DWSL) compared to 1064nm laser (Nd:YAG) alone. Secondarily, to evaluate QoL impairment in HHT patients, and its improvement with laser therapy. Methods A comparative randomized split-body double-blinded prospective study (DWSL vs Nd:YAG). Demographic, clinical and treatment characteristics were recorded. The severity and degree of improvement were evaluated by three blinded examiners who scored pre-treatment and post-treatment pictures on a 5-point scale. Patients fulfilled Skindex-29 and FACE-Q® tests and assessed procedure-associated pain and patient satisfaction. Results: 111 treatment areas (55 treated with DWSL and 56 with Nd:YAG) from 26 patients were analyzed. The median number of laser sessions was 2 (interquartile range [IQR] 2–4; mean 2.90 vs 2.88, respectively). The median improvement score, irrespective of location, was significantly higher for Nd:YAG compared to DWSL: 3 (IQR 2–3; mean 2.61) vs 2 (IQR 2–3; mean 2.32), p=0.031. Both FACE-Q index and Skindex-29 test results improved significantly (p<0.001), and 92.4% patients reported a high degree of satisfaction (≥8). No severe adverse events were reported. Conclusions DWSL and Nd:YAG laser are convenient, safe and effective treatment options for mucocutaneous telangiectases in HHT patients. However, Nd:YAG delivered better results with better tolerability. QoL was significantly improved by both treatments. (AU)

ABSTRACT

Irradiation of dentin with the Nd:YAG laser shows promise for the quality of adhesion, as long as it is applied under specific parameters and in defined steps. The objective of this study is to comparatively evaluate the impact of irradiation with Nd:YAG laser on the formation and stability of the hybrid layer in human dentin, pretreated with acid and primer of conventional adhesive system, "in vitro". Seventy healthy molars had their roots and coronal portions horizontally sectioned, until complete exposure of occlusal dentin. The samples were divided into: G1 (control n=20); G2 (experimental n=20), with 10 teeth from each group destined for microshearing (MC) and 10 teeth for scanning electron microscopy (SEM). In the 30 remaining teeth analyzed: texture by Cone Beam Computed Tomography (AT=15); and Fourier Transform Infrared Spectroscopy (FTIR=15) in 3 moments: dentin control; after treatment with acid + primer; and after laser irradiation. Statistically, we opted for 1-way ANOVA, Pearson's Chi Square, T test and Tukey's multiple comparisons with a significance of 5%. FTIR showed a reduction in phosphate and carbonate levels in the primer moments and after laser irradiation, but there was no denaturation of the exposed collagen. The texture analysis evidenced a trend towards the formation of a more expressive tissue after the use of the laser. The CM test showed mean values of 36.5 and 35.2MPa for G1 and G2 respectively (p>0.05) and therefore, immediately, the laser showed no deleterious effects on the adhesive process. In addition, greater penetrability of the adhesive was observed in the SEM evaluation after laser application. New studies with longitudinal evaluation are necessary to verify if the laser effect could have been protected from hydrolytic degradation, since there is a reduction in the local water content during the formation of the hybrid layer (AU).

ABSTRACT

BACKGROUND: Melasma is an acquired and treatment-resistant aging facial skin condition prevalent among the older Asian females. The Q-switched toning laser is the most widely used for the treatment of moderate to severe melasma in Asia. Recently, the picosecond laser has been introduced for various pigmentary disorders such as melasma. We evaluated the efficacy and safety of dual toning combined with 1064 nm Nd:YAG picosecond and quasi-long-pulsed laser in the treatment of severe melasma. SUBJECTS AND METHODS: Twenty Vietnamese females, mean age 41.9 ± 6.4 years, Fitzpatrick skin type IV, with clinical diagnosis of severe dermal and mixed-type melasma were treated by at least eight sessions picosecond Nd:YAG 1064 nm reducing Melasma Area and Severity Index (MASI) to less than 15%. Then they were treated with 3 sessions of dual toning combined picosecond Nd:YAG 1064 nm laser having an energy and spot size of 0.6-0.8 J/cm2 and 8 mm with micropulsed mode Nd:YAG 1064 nm laser (350 µs) having an energy and spot size of 2.6 J/cm2 and 15 mm with ICD off. Mild and even erythema was the endpoint (40-42°C using the infrared thermometer). Treatments were given every 4 weeks. Improvement was rated by MASI at the baseline (T1), after eight sessions of picotoning (T2), and three sessions of dual toning (T3). RESULTS: The mean MASI score of 20 patients at baseline (T1) was 16.24 ± 4.88 (min 6.0; max 28.8); after at least eight picotoning sessions, the mean MASI was 15.12 ± 4.69, representing a 7.44 ± 4.41% reduction in MASI score. The MASI score of the dual toning laser continued to decrease after three dual toning sessions from 15.12 ± 4.6 to 9.77 ± 3.86 (MASI score reduced by a mean of 5.35 ± 2.64), achieving 35.15 ± 13.51% reduction from T2 and 40.17 ± 12.14% reduction from baseline MASI (T1). There were no unexpected side effects in any patients. CONCLUSION: The dual toning method using the 1064 nm Nd:YAG picosecond and microsecond laser was safe and effective and well tolerated by all patients without downtime. However, larger number of studies should be conducted with more objective measurement techniques to confirm the results of this preliminary study.

ABSTRACT

BACKGROUND: Port Wine Stain (PWS) is only partially and superficially treated with the Pulsed dye laser (PDL) because of its limited depth of penetration. The 1064-nm long pulsed Nd:YAG laser has greater depth of penetration and is used to treat deeper vessels. The dual sequential wavelength laser (DSWL) which combines PDL/Nd:YAG (595/1064 nm) can be more effective for the treatment of deeper, nodular portwine stains due to its synergistic effect. The purpose of this study is to evaluate the efficacy and safety of DSWL in the treatment of portwine stains after five treatment sessions. MATERIALS AND METHODS: A total of 11 patients with PWS lesions on the head and neck, who were treated with DSWL for at least five sittings at monthly interval were included in this study. The assessment of the therapeutic response and grading of improvement was done with a scale of 0 to 4 by comparing the photographs taken before treatment and after 5 sittings of laser treatment. RESULTS: Out of the 11 patients, 5 patients (45.45%) had more than 70% improvement. Six out of eleven patients (54.54%) had 40 to 70% improvement at the end of five sittings with no adverse effects including purpura. CONCLUSION: Dual Sequential Wavelength Laser is a good modality of treatment for portwine stains. It has enhanced penetration and effective clearance of thicker, nodular, recalcitrant lesions of PWS. DSWL is safe and efficacious and it can be recommended as a therapeutic modality for portwine stains.

ABSTRACT

The case concerns a 20 year-old woman with a sudden visual loss in her left eye. In the fundus examination, a focal pre-retinal haemorrhage was observed in the superior temporal vascular branch with accompanying vitreous haemorrhage. There was a favourable spontaneous outcome. Eighteen months later, she presented with a new visual loss in the same eye, showing a sub-hyaloid pre-macular haemorrhage that was satisfactorily drained by hyaloidotomy using a Nd-YAG laser. The patient had also presented with an upper digestive tract haemorrhage and menorrhagia. A haemostasis study was performed that showed a coagulation factor VII deficiency. This is a very uncommon disorder that has not been previously described in association with the appearance of pre-retinal haemorrhages.

ABSTRACT

ABSTRACT Purpose: The purpose of this study was to evaluate the effects of the application of antioxidant on the shear bond strengths (SBS) of orthodontic brackets bonded to human enamel after extra-coronal bleaching with hydrogen peroxide (HP). Materials and methods: Fifty freshly extracted premolars were randomly divided into three groups. One group (Group 1) was bleached with 40% HP (n=20), another group (Group 2) was bleached with 40% HP and activated with an Nd: YAG laser (n=20), and a control group (Group 3) received no treatment (n=10). After the bleaching procedure, Groups 1 and 2 were further divided into two subgroups (Group 1a, Group 1b, Group 2a, and Group 2b). Specimens in Groups 1a and 2a (n=10) received no antioxidant application. A 10% sodium ascorbate solution was applied to specimens in Groups 1b and 2b (n=10). The brackets were then bonded with light-cure adhesive paste to the enamel surfaces of all experimental groups. Specimens were subjected to SBS tests for debonding. Results: The SBS of brackets bonded in Groups 1 and 2 were significantly lower than those of brackets bonded to the control group (p<0.001); however, no statistically significant differences in SBS were noted in Groups 1 and 2 (p>0.001). The antioxidant-treated SBS of brackets in Groups 1b and 2b were significantly higher than those of Groups 1a and 2a (p<0.001). Conclusions: The application of 40% HP had a negative effect on the bond strength. The use of antioxidant for 15 min resulted in the reversal of failed bond strength.

ABSTRACT

BACKGROUND: Treatment of nail psoriasis remains a challenging and often disappointing situation. OBJECTIVE: To compare the efficacy, adverse reactions and tolerability of treatment of nail psoriasis with PDL vs. Nd:YAG, in association with betametasona calcipotriol gel. METHODS: An open, prospective intrapatient left-to-right study was designed. The right hand of each patient received treatment with PDL and the left hand with Nd:YAG. Betamethasone calcipotriol gel was applied once a day during the first week after each laser session. A total of four sessions were administered. RESULTS: The clinical efficacy was evaluated according to the NAPSI score. All patients showed improvement in nail bed and nail matrix psoriasis. The global NAPSI mean declined in 15.46 (p<0.000). There was neither statistical difference between the reduction in nail bed and matrix NAPSI nor in the treatment with PDL vs. Nd:YAG. The administration of Nd:YAG was more painful. No serious adverse effects were documented. LIMITATIONS: No random assignment and the small number of patients. CONCLUSIONS: PDL and Nd:YAG have proven to be an effective treatment for nail psoriasis with no serious adverse effect. No statistically significant difference was found between the two treatments.

ABSTRACT

CLINICAL CASE: An 82-year-old man was admitted to the emergency department complaining of a sudden painless visual loss in his left eye (OS). He was diagnosed with branch retinal artery occlusion (BRAO) with a visible embolus. In 2012, he had a central artery occlusion (CRAO) in his right eye (OD). An embolysis with Nd:YAG laser was attempted, the retinal arterial blood flow was restored completely and the visual field was improved, with no secondary complications. CONCLUSIONS: Nd:YAG laser embolysis is a treatment to be considered in patients with BRAO with a visible embolus. The risks and benefits of the procedure should be evaluated, comparing it with possible permanent loss of visual acuity and other vascular complications caused by BRAO.

ABSTRACT

Objective: The aim of this study was to evaluate the adhesion of a self-etching system to cariesaffected dentin (CAD) and sound dentin (SD) after different caries removal techniques by using microtensile bond strength test (μTBS). Materials and Methods: Twenty-four extracted human molars with coronal carious lesions were used. The samples were randomly divided into 5 groups, according to caries removal method: G1 - negative control (no removal); G2 abrasive disc; G3 – slow speed steel bur; G4 - Nd:YAG laser (energy density 21.2 J/cm2 ); G5 - Er:YAG laser (energy density 80.24J/cm2 ). SD and CAD/ infected dentin substrates were tested. Self-etch system (Clearfil SE Bond) was applied and resin crowns were built up. Results: One-way ANOVA showed statistically significant difference between experimental groups (p < 0.001), with significantly lower results for infected dentin (G1), when compared to all other experimental groups. A statistically significant higher bond strength value was observed for SD irradiated with Er:YAG laser when compared to CAD. For bur and Nd:YAG laser specimens, SD and CAD showed the same bonding performance. Conclusion: infected dentin is not an adequate substrate for bonding; Nd:YAG laser and steel burs showed the same bonding performance for both SD and CAD; while both dentins, when irradiated by Er:YAG laser, did not adequately interact with self-etching system resulting in poor adhesion

ABSTRACT

The aim of the present study was to compare the effects of Nd:YAG laser application in root canals on bond strengths of fiber posts.Material and Methods: Thirty single-rooted bovines were randomly divided into three groups (n = 10); root canal instrumentation was performed, and pretreatment was conducted as follows: C group: conventional treatment (without laser irradiation); ALC group: Nd:YAG laser was applied after adhesive; and LAC group: Nd:YAG laser was applied before adhesive. The fiber posts Rebilda 15 DC (Voco) were cemented with an adhesive system and resin cement,in accordance with the manufacturer’s instructions. Six slices with 1.0mm height was obtained for ofeach root and bond strength was measured by pushout test using a universal testing machine (0.5 mm/min). Data were analyzed using Kruskal-Wallis and Dunn’s tests (p < 0.05). Results: Push-out bond strengths to root canal dentin were affected by the type of treatment and root third. The use of Nd: YAGlaser after the application of adhesive system had ahigher bonding performance compared with the useof laser before the application. However, both of them were similar to the control group. Also, cervical and medium third presented higher bond strength values than the apical third. Conclusion: It must be conclude, in the conditions in which the studywas conduct, that the use of laser irradiation didnot improve the bonding performance of resinous cement inside the root canal...

ABSTRACT

Posterior capsule opacification (PCO) is the most common complication after cataract surgery, with an incidence of 30%. It tends to be considered a normal event in the natural history of cataract surgery. Better understanding of its pathophysiology and advancement of intraocular lens material and design along with the improvement of phacoemulsification technique have contributed to decrease the incidence of PCO. Although treatment by Nd: YAG laser posterior capsulotomy is quick and non-invasive, the opening of the posterior capsule may be associated with numerous complications. Prevention remains the best measure for controlling this pathology.

ABSTRACT

PURPOSE: To compare Nd:YAG laser rates following implantation of AcrySof(®) SN60WF (Alcon), Akreos(®) AO-MI60 (Baush & Lomb), and Hoya(®) YA-60BB (Hoya) intraocular lenses. METHODS: This retrospective study was conducted at three French centers with each implanting at least two of the three implants. Included patients had undergone uncomplicated cataract surgery with at least 3 years of follow-up. Records of patients implanted with one of the three IOL's were drawn randomly from the surgical logs. Postoperative data were obtained from the medical records of either the surgeon or the referring physician. Time elapsing until Nd:YAG laser was analysed using Kaplan-Meier survival curves. RESULTS: Three hundred eyes were implanted (AcrySof(®) 126, Akreos(®) 89, and Hoya(®) 85). AcrySof(®) recipients were the youngest (AcrySof(®) 72.1, Akreos(®) 76.4, and Hoya(®) 75.2 years of age: P=0.0007). The sex ratio was 4:6 male:female. Follow-up was longest for Hoya eyes (AcrySof(®) 29.4, Akreos(®) 24.6 and Hoya(®) 34.6 months; P=0.0002). Eyes implanted with AcrySof(®) had 1.74 times less chance of Nd:YAG laser treatment than Hoya eyes (P=0.0327) and 3.50 times less than Akreos(®) eyes (P<0.0001). The results remained unchanged when the analysis was restricted to events in the first 24 months (Risk Ratios: Hoya(®)=2.64: P=0.02; and Akreos(®)=4.22: P=0.0001). Adjustment on unbalanced confounding variables did not alter the results. CONCLUSIONS: Eyes with AcrySof(®) implants required significantly fewer Nd:YAG laser capsulotomies than those with Hoya(®) and Akreos(®) implants and were therefore less subject to Nd:YAG laser treatment complications, thus ensuring better vision at the lowest cost.

ABSTRACT

The purpose of this study was to evaluate the influence of CPP-ACP (MI Paste) and CPP-ACP associated to NaF (MI Paste Plus) on microtensile bond strength of dentin softened by a soft drink, irradiated or not with laser Nd:YAG. Sixty human molars had their crowns sectioned from mesial to distal face in order to obtain two specimens. The enamel was flattened until the exposure of 3 mm dentin area and then were embedded in acrylic resin. The samples were randomly divided into four groups (n=30): CG (control group); EC (erosive challenge); EC.M (erosive challenge + MI Paste); EC.MP (erosive challenge + MI Paste Plus). The erosive challenge consisted by a single immersion of the specimens in Sprite Zero, for 30 min. The EC.M and EC.MP were submitted to remineralizing treatment with the application of a 0,5 mm layer, for 3 min. The groups were subdivided into two subgroups: WITH (WL) or WITHOUT (W) Nd:YAG laser application. Laser application was made by the energy density of 174J/cm2, 140 mJ/pulse, 10 Hz, 1.4W, and the sequence was: adhesive system + Nd:YAG laser + photopolimerization. All the samples were restored by the adhesive system Clearfil SE Bond (Kuraray) and the composite Z350XT (3M-ESPE). After 24h storage in deionized water, the samples were cut in sticks, with 1mm2 cross-section area, then was accomplished the microtensile test. The microtensile data were analyzed with two-way ANOVA. Significant statistically differences were found to the treatment factor (p=0.00) and Tukey test showed that the EC group presented lower values to microtensile bond strength. The averages of each group, in ascending order, were: EC.WL (29.22) < EC.W (33.50) < EC.M.W (33.89) < EC.MP.W (34.96) < CG.W (36.40) < EC.MP.WL (36.92) < CG.WL (37.46) < EC.M.WL (37.68). It was concluded that: the erosive challenge decreased the bond strength compared to control group; the CPP-ACP and CPP-ACP associated to NaF increased the bond strength4 compared to the groups which had not ...
