ABSTRACT

A fine and fair depiction of basal cell carcinoma is documented with great fidelity in "Caricature" by Bartolomeo Passerotti (1529-1592). The cancer is pearly white and contains an elevated centre with a sharp and somewhat depressed outline due to ulceration of the lesion. The painting is of essential didactic worth for practicing medical doctors. In addition, the masterpiece contains images of the brown elevated lesions that could turn out to be verrucous melanocytic nevi or pigmented seborrheic keratoses, but it cannot be excluded that these nodules are also basal cell carcinomas covered by a brownish crust. It is standard that microscopic verification is required for all these tumours. However, a pearly white irregular tumour is the most characteristic macroscopic presentation of basal cell carcinoma.

ABSTRACT

The history of zinc chloride therapy before and including Frederic E. Mohs' (1910-2002) early contributions to dermatologic oncology is presented. In 1932, Mohs devised a method of cutaneous surgery that employed zinc-chloride paste to devitalize basal or squamous cell carcinoma. Because zinc chloride coagulates malignant tissue without destroying cellular architecture, he described the surgery and its preservative effect as the fixed-tissue method. This method involved the serial removal of devitalized malignant tissue and the freezing and histologic examination of each layer; the process was continued until a cancer-free plane was reached. In 1953, a modified version of the micrographic system, the fresh-tissue method, was introduced. For improved efficiency, Mohs retained the systematic mapping of a neoplasm but eliminated the chemical escharotic. Although the fresh-tissue method has become the preferred treatment mode, zinc-chloride paste therapy remains in use today. Although the history of Mohs micrographic surgery is well documented, its background has not received sufficient attention. This paper surveys the chemical synthesis of zinc chloride, the formulation of zinc-chloride paste as a cutaneous escharotic, and the extent to which Mohs' method compares with those of 19th and early 20th century predecessors.

ABSTRACT

BACKGROUND: The development of knowledge concerning the role of sun exposure in causing skin cancer has been a gradual one. OBJECTIVE: This article reviews the article by Urbach who used manikin coated with an ultraviolet dosimeter to see exactly where on the head and neck the exposure was greatest. CONCLUSION: Urbach showed that the areas of greatest sun exposure on his manikins corresponded with the location of 95% of squamous cell carcinoma and 66% of basal cell carcinoma. He also clearly showed the importance of scattered sky and reflected radiation.

ABSTRACT

Se reseña previamente sobre la epidemiología histogenésis patogenia, conducta biológica, clínica, así como sus características histopatológicas, nueva forma de clasificación de acuerdo al patrón de crecimiento histopatológico del Epitelioma Basocelular. 142 pacientes (con informe anátomo patológico de Epitelioma Basocelular) que acudieron al Hospital Militar Central Militar Central, fueron estudiados; de los cuales 82 fueron varones, y 60 mujeres. La mayoría de los pacientes estuvieron entre la 5ta. y 8ava. década de la vida con rango de edad de 16 a 89 años. Se realizó la lectura de las laminas con diagnóstico de Epitelioma Basocelular; se les clasificó de acuerdo a su variedad histológica. Se obtuvo información de las solicitudes de examen histopatológico y de historias clínicas para cumplir con el total de los objetivos planteados. Se encontró que el Epitelioma Basocelular es la neoplasia maligna más frecuente del total de neoplasias malignas del organismo. La presunción diagnóstica clínica estuvo correcta en 76.8 por ciento de los casos, por los servicios tratantes: Cirugía Plástica, Dermatología y Cirugía General. Los subtipos histopatológicos con más frecuente ocurrencia, resultaron ser las variedades con patrón de crecimiento circunscripto: con el sólido en primer lugar. Las variedades con patrón difuso presentaron a los tipos superficial y morfeiforme como más frecuentes

ABSTRACT

This historical study has examined the persistent controversy about the propensity of epithelioma adenoides cysticum (EAC) to transform into multiple rodent ulcers, by reviewing cases reported through the years that seemed to support the idea. After focusing on the biological behaviour and natural history of the tumours and comparing them with our present knowledge of the behaviour of EAC, we believe that these cases were incorrectly diagnosed. Rather than EAC, they were probably examples of the naevoid basal cell carcinoma syndrome. The latter syndrome was actually established by the recognition that patients who have multiple rodent ulcers that appear early in life and behave aggressively usually have a constellation of associated developmental defects and that the tumours sould be classified as genetically determined basal cell carcinomas and not as EAC.
