ABSTRACT

OBJECTIVE: This study aimed to determine if the major public awareness campaign for cochlear implants 'International Cochlear Implant Day' influenced national and international public interest as measured by internet search activity. METHODS: Weekly search volume data in the United States, Canada, Australia, Germany, United Kingdom, Brazil, India, Japan, and a 'Worldwide' group for the search topic 'cochlear implant' was collected from Google Trends over a 5-year period (2017-2021). The 'Campaign' window was defined as 1 week before, the week of, and 2 weeks after International Cochlear Implant Day (February 25th). 'Non-Campaign' weeks were considered any data outside the 'Campaign' window. RESULTS: Of the studied regions, the United States, United Kingdom, Australia, India, and 'Global' demonstrated a significant increase in internet search activity between 2017 and 2021. Although some individual years showed significant increases during the 'Campaign' period for Canada, Germany, Brazil, and Japan, none showed statistically significant increases over the 5-year period studied. CONCLUSION: Public awareness campaigns are recognized crucial elements to delivering effective healthcare, but their success varies worldwide. While data from Google Trends suggests that cochlear implant awareness campaigns can translate into increased internet searches, greater efforts can be made in select countries to improve public interest.

ABSTRACT

Objective: Evaluate auditory results and communicative benefits in adult patients with single-sided deafness (SSD) treated with cochlear implantation.Study Design: Observational descriptive case series.Setting: Hospital Universitario Fundación Santa Fe de Bogotá.Patients: Ten patients with profound post-lingual SSD were treated with cochlear implantation between January 2011 and March 2016. Two patients were excluded because of incomplete follow-up.Interventions: Medical records included demographic information, pre and post-operative audiometric results, and binaural and monoaural speech discrimination tests in three sound-in-noise configurationswith the cochlear implant turned on and off, respectively. Subjective improvement was evaluated using the Abbreviated Profile of Hearing Aid Benefit (APHAB) and Speech, Spatial, and Qualities of Hearing Scale (SSQ-B).Main Outcome Measures: Pre and post-operative audiometric and speech discrimination results, post-operative binaural and monoaural speech discrimination in noise results, and APHAB and SSQ-B questionnaire results.Results: Significant improvement in speech discrimination in a noisy environment was found when the noise and signal were presented from the front, and when the signal was presented to the implanted ear and the noise to the healthy ear. SSQ-B questionnaire showed improvement in all subscales, while APHAB showed improvement in all subscales except sound aversion.Conclusion: Patients with SSD showed improvement regarding communication skills and sound discrimination in a noisy environment.

Resumen Introducción: El concepto de calidad de vida se ha tornado de suma importancia y actualmente toda intervención sanitaria es valorada por el impacto que causa en esta misma. La hipoacusia bilateral profunda produce importantes consecuencias, retrasando la adquisición del lenguaje, disminuye significativamente el aprendizaje y produce dificultades en cómo se desarrolla en su entorno. El implante coclear es el tratamiento de elección en esta patología y es de suma importancia conocer el impacto real que tiene en la calidad de vida de aquellos sometidos a esta intervención. Objetivo: Conocer la calidad de vida global de los pacientes implantados en el Hospital Barros Luco Trudeau entre 2015 y 2016, usando el instrumento de medición de calidad de vida KINDL. Material y método: Estudio analítico prospectivo de cohorte, para medición de calidad de vida se utilizó el instrumento KINDL, se aplicó instrumento a menores implantados y padres a través de entrevista directa en periodos preimplante, a 6 meses y al año de implantación, datos y pruebas estadísticas se realizaron con software SPSS 13. Resultados: Se estudiaron 16 pacientes en edad pediátrica implantados en Hospital Barros Luco Trudeau. Se evidenciaron cambios en la calidad de vida global y en las subescalas de bienestar emocional, autoestima, educación, relación con amigos y familia, además de percepción de enfermedad. Conclusiones: El implante coclear en nuestra serie demostró una modificación positiva en la calidad de vida global tanto de los pacientes como de su entorno familiar.


Abstract Introduction: Quality of life has become very important, and nowadays all health interventions are valued for the impact they can have on it. Profound bilateral hearing loss produces major consequences such as delaying the acquisition of language, which results in a significant decrease in learning and creates difficulties in how the individual develops in their environment. The cochlear implant is the treatment of choice in this pathology, and it is imperative to know the real impact it has on the quality of life of those subjected to this intervention. Aim: To know the overall quality of life of the patients implanted in the Barros Luco Trudeau Hospital between 2015 and 2016, using the KINDL quality of life measurement instrument. Material and method: Prospective analytical cohort study, to measure the quality of life, the KINDL instrument was used. The instrument was applied to implanted minors and their parents through a direct interview in pre-implantation periods at six months and after one year of implantation. The data and statistics tests were made with SPSS 13 software. Results: 16 pediatric patients implanted in the Barros Luco Trudeau Hospital were studied. There were changes in the global quality of life and the subscales of emotional well-being, such as the patient's self-esteem, education, relationships with friends and family, as well as the perception of illness. Conclusions: The cochlear implant in our series demonstrated a positive change in the global quality of life for both patients and their family environments.

Abstract Introduction Considering the variability of results found in the clinical population using a cochlear implant, researchers in the area have been interested in the inclusion of quality of life measures to subjectively assess the benefits of the implantation. Objective To assess the quality of life of adult users of cochlear implant. Methods A cross-sectional and clinical study in a group of 26 adults of both genders, with mean duration of cochlear implant use of 6.6 years. The Nijmegen Cochlear Implantation Questionnaire and the generic World Health Organization Quality of Life questionnaire were sent electronically. Results The best assessed domain in the quality of life assessment for the cochlear implantation questionnaire was the social domain, whereas for the quality of life questionnaire it was the psychological domain. The variables, gender, time of cochlear implant use and auditory modality did not influence the results of both questionnaires. Only the variable level of education was correlated with the environment domain of the quality of life questionnaire. The variable telephone speech comprehension was associated with a better perception of quality of life for all the domains of the specific questionnaire and for the self-assessment of quality of life in general. Conclusion From the users' perspective, both questionnaires showed that cochlear implant brought benefits to different aspects related to quality of life.


Resumo Introdução Diante da variabilidade de resultados clínicos encontrada na população usuária de implante coclear, pesquisadores da área têm se interessado pela inclusão de medidas de qualidade de vida para avaliar os benefícios do implante coclear de maneira subjetiva. Objetivo Avaliar a qualidade de vida de adultos usuários de implante coclear. Método Estudo transversal e clínico em um grupo de 26 adultos, de ambos os gêneros, com tempo de uso médio do implante coclear de 6,6 anos. Foram usados o questionário específico Nijmegen de Implantes Cocleares e o questionário genérico World Health Organization Quality of Life, enviados via mídia eletrônica. Resultados O domínio mais bem pontuado na avaliação da qualidade de vida para o questionário Nijmegen foi o social e para o questionário World Health Organization Quality of Life foi o psicológico. As variáveis gênero, tempo de uso do implante coclear e modalidade auditiva não influenciaram os resultados dos questionários. Apenas a variável nível de instrução correlacionou-se com o domínio meio ambiente do questionário sobre qualidade de vida. A variável compreensão de fala ao telefone associou-se a uma melhor percepção da qualidade de vida para todos os domínios do questionário específico e para a autoavaliação da qualidade de vida em geral. Conclusão Na perspectiva dos usuários, o implante coclear trouxe benefícios para os diversos aspectos relacionados à qualidade de vida em ambos os questionários.

ABSTRACT

OBJECTIVE: To understand the family's experience of a child who uses a cochlear implant (CI). Specifically, to identify the difficulties, changes, and feelings entailed by deafness and the use of the CI; the coping strategies; and to understand the role of the family for the child with a CI. METHOD: Qualitative research, using Symbolic Interactionism and Straussian Grounded Theory as the theoretical and methodological frameworks, respectively. Data collection instrument: semi-structured interview. A total of 9 families (32 individuals) participated in the study. The children's ages ranged from 6 to 11 years old (mean = 8.9 years old). RESULTS: Their experience is described in the following categories: Having to fight for results, Coping with difficult situations, Recognizing that you are not alone, Learning to overcome, and Having one's life restored by the implant. CONCLUSION: Cochlear implantation changes the direction of the child and the family's life by restoring the child's opportunity to hear and to obtain good results in her personal, social, and academic development. Even after implantation, the child continues to experience difficulties and requires the family's mobilization in order to be successful. The family is the principal actor in the process of the child's rehabilitation.

ABSTRACT

Central auditory pathway maturation in children depends on auditory sensory stimulation. The objective of the present study was to monitor the cortical maturation of children with cochlear implants using electrophysiological and auditory skills measurements. The study was longitudinal and consisted of 30 subjects, 15 (8 girls and 7 boys) of whom had a cochlear implant, with a mean age at activation time of 36.4 months (minimum, 17 months; maximum, 66 months), and 15 of whom were normal-hearing children who were matched based on gender and chronological age. The auditory and speech skills of the children with cochlear implants were evaluated using GASP, IT-MAIS and MUSS measures. Both groups underwent electrophysiological evaluation using long-latency auditory evoked potentials. Each child was evaluated at three and nine months after cochlear implant activation, with the same time interval adopted for the hearing children. The results showed improvements in auditory and speech skills as measured by IT-MAIS and MUSS. Similarly, the long-latency auditory evoked potential evaluation revealed a decrease in P1 component latency; however, the latency remained significantly longer than that of the hearing children, even after nine months of cochlear implant use. It was observed that a shorter P1 latency corresponded to more evident development of auditory skills. Regarding auditory behavior, it was observed that children who could master the auditory skill of discrimination showed better results in other evaluations, both behavioral and electrophysiological, than those who had mastered only the speech-detection skill. Therefore, cochlear implant auditory stimulation facilitated auditory pathway maturation, which decreased the latency of the P1 component and advanced the development of auditory and speech skills. The analysis of the long-latency auditory evoked potentials revealed that the P1 component was an important biomarker of auditory development during the rehabilitation process.

ABSTRACT

Considerando a reabilitação da fala de crianças com deficiência auditiva e usuárias de implante coclear e evidências da possibilidade de transferência de controle de estímulos da leitura para a nomeação, este estudo teve como objetivo avaliar os efeitos de um programa de ensino de leitura de palavras simples sobre a qualidade da fala de duas crianças em testes de leitura de palavras e nomeação de figuras. O procedimento consistiu no ensino de relações condicionais baseadas em seleção de palavras impressas e de composição após a palavra ditada. A nomeação de figuras foi avaliada em um delineamento de linhas de base múltipla. Os resultados mostram melhora na qualidade da fala, se comparados à linha de base, tanto frente a palavras quanto frente a figuras. Futuros estudos, com maior número de participantes e outros delineamentos devem verificar benefícios de programas de ensino ao processo de reabilitação dessa população.


Considering the speech rehabilitation in children with hearing impairment and cochlear implant and the evidence of possible stimuli control transference of reading to naming, the objective of the present study was to evaluate the effects of a teaching program for reading simple words on the speech intelligibility of two children in tests of reading words and naming pictures. The procedure consisted of teaching conditional relations based on a selection of printed words and word composition after word dictation. Picture naming was assessed in a multibaseline design. Data showed improvement in the quality of responses when compared to baseline, both in front of words and pictures. Future studies with higher number of participants and other experimental designs may verify the benefits of teaching programs to the rehabilitation process of this population.

PURPOSE: To evaluate the quality of life of children with cochlear implants from the perspective of their parents. METHODS: A clinical and cross-sectional study was conducted with 15 parents of children using cochlear implants of both genders aged between 2 and 12 years old. Parents of these children answered the questionnaire "Children with Cochlear Implants: Parental Perspective" (CCIPP). Data related to auditory category and time of cochlear implants use were collected from medical records of the children. The percentages of responses on the CCIPP domains were tabulated and descriptively and inferentially analyzed. RESULTS: The cochlear implants had a positive effect on the quality of life of children in the self-reliance (58.9%) and social relationships (56.7%) domains. No correlation was observed between the time of cochlear implants activation (months) and any of the CCIPP domains. However, children with 24 months or less of cochlear implant use presented higher percentages on the communication domain than those with more than 24 months of cochlear implants use. A negative correlation was observed between the auditory category and the effects of the implant domain. CONCLUSION: From the perspective of parents, the use of cochlear implants improves the quality of life of their children; the shorter the time of cochlear implants use, the higher the improvement in quality of life; and the more developed the auditory skills, the lower the percentage of quality of life improvement with the cochlear implants.

OBJETIVO: Avaliar a qualidade de vida de crianças usuárias de implante coclear sob a perspectiva de seus pais. MÉTODOS: Estudo clínico e transversal com 15 pais de crianças usuárias de implante coclear, de ambos os gêneros e com idades entre 2 e 12 anos. Os pais responderam ao questionário "Crianças com Implante Coclear: Perspectiva dos Pais" (CCIPP). Foram levantados, nos prontuários das crianças, os dados referentes à categoria de audição e tempo de uso do dispositivo. Os percentuais dos domínios do CCIPP foram tabulados e analisados de forma descritiva e inferencial. RESULTADOS: Os domínios do questionário de qualidade de vida que apresentaram maior média foram autoconfiança (58,9%) e relações sociais (56,7%). Não foi verificada correlação entre o tempo (meses) de uso do implante coclear e os domínios do questionário. No entanto, as crianças com tempo de uso inferior ou igual a 24 meses apresentaram maior percentual no domínio comunicação do que aquelas com tempo de uso superior a 24 meses. Houve correlação negativa entre a categoria de audição e o domínio efeitos do implante. CONCLUSÃO: Na perspectiva dos pais, o uso do implante coclear melhora a qualidade de vida de seus filhos. Quanto menor o tempo de uso do implante coclear, maior o percentual da qualidade de vida; e, quanto mais desenvolvida a habilidade auditiva, menor o percentual de melhora da qualidade de vida. .

UNLABELLED: The use of cochlear implant (CI) in children enables the development of listening and communication skills, allowing the child's progress in school and to be able to obtain, maintain and carry out an occupation. However, the progress after the CI has different results in some children, because many children are able to interact and participate in society, while others develop limited ability to communicate verbally. The need for a better understanding of CI outcomes, besides hearing and language benefits, has spurred the inclusion of quality of life measurements (QOL) to assess the impact of this technology. OBJECTIVE: Identify the key aspects of quality of life assessed in children with cochlear implant. METHOD: Through a systematic literature review, we considered publications from the period of 2000 to 2011. CONCLUSION: We concluded that QOL measurements in children include several concepts and methodologies. When referring to children using CI, results showed the challenges in broadly conceptualizing which quality of life domains are important to the child and how these areas can evolve during development, considering the wide variety of instruments and aspects evaluated.

ABSTRACT

O uso do implante coclear (IC) em crianças possibilita o desenvolvimento das habilidades auditivas e comunicativas, permitindo o progresso da criança na escola e, futuramente, ser capaz de obter, manter e executar uma ocupação. Entretanto, existe uma variabilidade quanto ao progresso após o IC, pois muitas crianças são capazes de interagir e participar na sociedade, enquanto outras passam por limitações na habilidade de se comunicar verbalmente. A necessidade de uma melhor compreensão sobre os resultados do IC, além dos resultados em audição e linguagem, tem estimulado a inserção de medidas de qualidade de vida (QV) para avaliar o impacto dessa tecnologia. OBJETIVO: Identificar quais os principais aspectos de qualidade de vida avaliados nas crianças usuárias de implante coclear. MÉTODO: Por meio de uma revisão sistemática da literatura, considerando publicações do período de 2000 a 2011. CONCLUSÃO: Conclui-se que medir a QV em crianças inclui vários conceitos e metodologias. Quando nos referimos às crianças usuárias de IC, os resultados mostraram os desafios em conceituar, de maneira compreensiva, quais domínios de qualidade de vida são importantes para a criança e como estes domínios podem evoluir durante o seu desenvolvimento, visto a grande variedade de instrumentos e aspectos avaliados nos estudos.

INTRODUCTION: Studies of neuroplasticity have shown that the brain's neural networks change in the absence of sensory input such as hearing or vision. However, little is known about what happens when both sensory modalities are lost (deaf-blindness). Hence, this study of cortical reorganization in visually-impaired child cochlear implant (CI) candidates. OBJECTIVE: Assess cross-modal plasticity, specifically cortical reorganization for tactile representation in visually-impaired child CI candidates, through study of topography of somatosensory evoked potentials (SEP). METHODS: From April through September 2005, SEP from median and tibial nerve electrical stimulation were studied in 12 visually-impaired child CI candidates aged 3-15 years and 23 healthy controls. Following placement of 19 recording electrodes using the International 10-20 System , SEP were recorded and then processed. Topographic maps were obtained for SEP N20 (median nerve) and SEP P40 (tibial nerve), permitting assessment of cortical reorganization by comparing visually-impaired, deaf children's maps with those of healthy children by means of visual inspection and statistical comparison using a permutation test. RESULTS: SEP N20 topography was significantly more extensive in visually-impaired child CI candidates than in healthy children. An asymmetrical pattern occurred from the expansion of hand tactile activation into the temporal and occipital regions in the left hemisphere on right median nerve stimulation. This did not occur for SEP P40 on tibial nerve stimulation (right and left). Magnitude of expanded SEP N20 response was related to severity of visual impairment and longer duration of dual sensory loss. CONCLUSIONS: Changes in SEP N20 topography are evidence of cross-modal plasticity in visually-impaired child CI candidates, appearing to result from a complex interaction between severity of visual impairment and duration of multisensory deprivation.

ABSTRACT

Objetivo: investigar as dificuldades relatadas por idosos no processo de adaptação do aparelho deamplificação sonora individual (AASI), considerando as variáveis: idade, gênero, tipo e grau de perdaauditiva, tecnologia e modelo, bem como a participação da família neste processo. Métodos:participaram do estudo 20 idosos usuários de AASI (sendo 10 do gênero feminino e 10 gênero masculino)e suas famílias. Foi realizado um levantamento através de dois tipos de questionário, dos quais oprimeiro destinava-se aos idosos e o segundo as suas famílias. Ambos os questionários representamuma adaptação do questionário de handicap 15. Resultados: os resultados encontrados indicaram queas maiores dificuldades relatadas estão relacionadas ao uso do telefone, ao assistir televisão, aoescutar rádio e ao próprio manuseio do AASI. Em relação as variáveis investigadas, foi possível observarque as maiores dificuldades foram encontradas no gênero feminino; tipo de perda sensorioneural; grausevero; tecnologia analógica e programável; modelos miniretroauricular e intracanal. Conclusão: osresultados mostraram que o processo de adaptação do AASI sofre influência das variáveis investigadas,destacando dificuldade maior nas atividades de usar o telefone e ouvir rádio e televisão, bem comomanusear o próprio AASI. A família dos idosos estudados os auxilia em tal processo.


Purpose: to investigate the difficulties reported by aged people in the AASI adaptation process, whileconsidering the variables: age, gender, type and degree of auditory loss, technology and model, as wellas the participation of the family in this process. Methods: 20 aged AASI users took part in the study(being 10 of feminine gender and 10 of masculine gender) and their families. A survey was carried usingtwo types of questionnaires, of which the first one intended for the aged people and the second one totheir families. Both questionnaires represent an adaptation of the handicap15 questionnaire. Results:the results found indicated that the major reported difficulties are related to the use of the telephone,when watching TV, listening radio and the own handling of AASI. In relation to the investigated variables,it was possible to note that the major difficulties were found in the feminine gender; type of sensorioneuralloss; severe degree; analogical and programmable technology; models to miniretroauricular and intrachannel.Conclusion: The results showed that the AASI adaptation process suffers influence from theinvestigated variables. The family of the aged people studied assists them in such process.

Los objetivos propuestos fueron: presentar el implante coclear, describir algunas de las características de los pacientes implantados e informar ampliamente sobre el accionar profesional, tanto de O.R.L. como de Fonoaudiólogos y Psicólogos, ya sea en el período pre-quirúrgico como así también en el post-quirúrgico. Se elaboraron entrevistas, las que fueron realizadas personalmente a cada profesional. En ellas se recabaron datos sobre dichos pacientes y sobre la labor profesional aplicada a ellos. Se realizaron preguntas acerca de la edad, de las etiologías, de la labor fonoaudiológica pre y post-quirúrgica, etc. La información obtenida fue volcada en planillas para su posterior análisis y construcción de gráficos y cuadros. Existen coincidencias con el marco teórico desarrollado en cuanto a etiopatogenia, marca predominante de implante coclear, conformación del equipo interdisciplinario, criterios de selección, test subjetivos post-implante, accionar psicológico pre-implante, pautas que demuestran soporte familiar y test psicológicos. ABSTRACT

Cochlear implants have been designed to partially restore hearing to those people who are totally deaf. Multi-channel cochlear implants offer the opportunity to evoke acoustic perceptions like loudness and pitch, elicited by a controllable pattern of electric stimulation by means of electrodes placed in different places along the cochlear length. In this study, two psychophysical experiments were conducted with 4 patients, 1 prelingually and 3 postlingually-deafened, implanted with the multi-channel cochlear prosthesis Nucleus 22. Experiments were carried out to study the effect of varying the width of the electric biphasic pulsatile stimuli on the discriminative abilities of the pitch perception. The tests involved place pitch ranking and pulse rate discrimination. Place pitch ranking was studied by determining the just noticeable difference in pitch pairs (jnd-pp), defined as the pair of nearest electrodes which elicit different pitch perception. Pulse rate discrimination was studied by determining the just noticeable difference in pulse rate (jnd-pr) defined as the minimal difference in stimulus repetition rate over a given electrode, which elicits different pitch perceptions. Both experiments were conducted using pulses of 400, 200, 100 and 50 microseconds/phase. The results indicated that in spite of the differences in pathologies and personal histories, both jnd-pp and jnd-pr decrease by diminishing the pulse width. Speech perceptual data, measured for various pulse widths, validates the usefulness of decreased pulse width which yields favorable results in the psychophysical tests.
